Lifelong training,retraining,reskilling,upskilling and knowledge gaps in emergency medicine:a cross-sectional survey study

BACKGROUND:Identifying and managing medical emergencies presents challenges in healthcare,where familiarity with established algorithms is essential for high-quality care.This study assessed healthcare professionals’ understanding of the latest resuscitation guidelines and explored their views on lifelong training models.METHODS:This cross-sectional study used two multiple-choice questionnaires with 50 questions developed by academic emergency and critical care consultants based on the 2021 Consensus on Science with Treatment Recommendations (CoSTRs) by the International Liaison Committee on Resuscitation(ILCOR).Healthcare staff involved in emergency coverage completed assessments on emergency management,self-evaluated their knowledge,and shared perspectives on continuous workplace education.RESULTS:Of the 1,427 distributed questionnaires,1,034 (72.5%) were completed.Knowledge gaps were more pronounced for pediatric algorithms from the European Resuscitation Council (ERC) and American Heart Association (AHA) compared to adult protocols (P<0.001).In multivariate logistic regression,being a physician,holding a Master of Science (MSc) degree,and younger age were independently associated with passing scores≥70%(all P<0.001).Most participants (97.3%) favored brief,employer-funded teamwork refresher sessions every4–6 months over the current four-year training model (0.6%)(P<0.001).CONCLUSION:This study highlights healthcare life support providers’ insufficient expertise in current resuscitation guidelines.The importance of short-format retraining,upskilling,and reskilling programs with post-training assessments is evident,as most respondents expressed a strong learning motivation to participate if employer-funded. 展开更多

It has been postulated that the persistent short intravaginal ejaculation latency time (IELT) of men with lifelong premature ejaculation (LPE) is related to 5-hydroxytryptamine (HT)2c receptor functioning. The aim of this study was to investigate the relationship of Cys23Ser 5-HT2c receptor gene polymorphism and the duration of IELT in men with LPE. Therefore, a prospective study was conducted in 64 Dutch Caucasian men with LPE. Baseline IELT during coitus was assessed by stopwatch over a 1-month period. All men were genotyped for Cys23Ser 5-HT2c receptor gene polymorphism. Allele frequencies and genotypes of Cys and Ser variants of 5-HT2c receptor gene polymorphism were determined. Association between Cys/Cys and Ser/Ser genotypes and the natural logarithm of the IELT in men with LPE were.investigated. As a result, the geometric mean, median and natural mean IELT were 25.2, 27.0, 33.9s, respectively. Of all men, 20.0%, 10.8%, 23.1% and 41.5% ejaculated within 10, 10-20, 20-30 and 30-60s after vaginal penetration. Of the 64 men, the Cys/Cys and Ser/Ser genotype frequency for the Cys23Ser polymorphism of the 5-HT2c receptor gene was 81% and 19%, respectively. The geometric mean IELT of the wildtypes (Cys/Cys) is significantly lower (22.6s; 95% CI 18.3-27.8s) than in male homozygous mutants (Ser/Ser) (40.4s; 95% CI 20.3-80.4s) (P = 0.03). It is concluded that Cys23Ser 5-HT2c receptor gene polymorphism is associated with the IELT in men with LPE. Men with Cys/Cys genotype have shorter IELTs than men with Ser/Ser genotypes. 展开更多

Recent research has highlighted structural and functional abnormalities in the cerebral cortex of patients with premature ejaculation(PE).These anomalies could play a pivotal role in the physiological mechanisms underlying PE.This study leveraged functional magnetic resonance imaging(fMRI),a noninvasive technique,to explore these neural mechanisms.We conducted resting-state fMRI scans on 36 PE patients and 22 healthy controls(HC),and collected data on Premature Ejaculation Diagnostic Tool(PEDT)scores and intravaginal ejaculation latency time(IELT).Employing a surface-based regional homogeneity(ReHo)approach,we analyzed local neural synchronous spontaneous activity,diverging from previous studies that utilized a volume-based ReHo method.Areas with significant ReHo differences between PE and HC groups underwent surface-based functional connectivity(FC)analysis.Significant discrepancies in ReHo and FC across the cortical surface were observed in the PE cohort.Notably,PE patients exhibited decreased ReHo in the left triangular inferior frontal gyrus and enhanced ReHo in the right middle frontal gyrus.The latter showed heightened connectivity with the left lingual gyrus and the right orbital superior frontal gyrus.Furthermore,a correlation between ReHo and FC values with PEDT scores and IELT was found in the PE group.Our findings,derived from surface-based fMRI data,underscore specific brain regions linked to the neurobiological underpinnings of PE. 展开更多

Gender Differences in Motivation and Learning Preferences of Pharmacists in Lifelong Learning

Objectives: To analyse motivation and preferences of pharmacists who participate in CE (continuing education) to develop suitable lifelong learning programmes for pharmacists. Methods: An online questionnaire, which explored the motivation and preferences of the pharmacists to lifelong learning, was sent to all members of the Royal Dutch Pharmaceutical Society (4321) in the Netherlands. The data were analysed using a non-hierarchical clustering technique. Key findings: Two clusters of pharmacists were discovered. Cluster A pharmacists (n = 474) were more motivated by credit points (63.5% vs. 47.2%), personal interest (84.1% vs. 56.3%), updating knowledge (73.8% vs. 56.8%) and topicality of CE courses (47.7% vs. 26.1%). Cluster B pharmacists (n = 199) were predominantly motivated by the aspect "duty as a care-giver" (97.0% vs. 0 % in cluster A). Pharmacists who belonged to cluster A tended to be women (60.5%), often worked part-time (29.3%) and mostly preferred lectures (71.1%). Cluster B pharmacists consisted of statistically significantly more male pharmacists (52.8%, p = 0.001), worked more full time (77.4%, p = 0.009) and mostly preferred blended learning (62.3%, p = 0.047). Conclusions: These results suggest the use of different education formats for different kinds of pharmacists to participate in CE activities. 展开更多

In recent years, the Ministry of Education has been calling for the all-round development of morality, intelligence, physique, beauty and labor, encouraging students to take more sports and build up their physique. The effect is not obvious enough. Especially in high school, schools generally pay attention to intellectual education, pay close attention to cultural courses, and pursue the promotion rate. Once upon a time, physical education, as a subject that was not valued highly, was often occupied by other subjects on the grounds that "the physical education teacher was ill" and "the physical education teacher asked for leave", while the students were helpless and unwilling to do so. When the students enter the university campus after the college entrance examination, the students' physique is relatively weak. Besides, due to the influence of traditional concepts, they do not attach importance to physical education class. As a result, the courses fail to keep up with the times and the phenomenon of failing the physical examination is common. As the last important stage of school physical education, university enables college students to enjoy happiness, strengthen their physique, improve their personality, exercise their will and establish lifelong sports awareness through various practical forms. Therefore, the purpose of this paper is to elaborate the significance of cultivating college students' lifelong sports in higher vocational colleges, and point out that college students' interest and habits in physical exercise should be cultivated and stimulated through various channels, so as to learn and master several sports skills and become lifelong hobbies, which will benefit them all their lives and form lifelong sports awareness. 展开更多

Multi-agent embodied intelligence has been integrated into warehouse systems for transporting commodities,which can be formulated as the lifelong multi-agent pickup and delivery(LMAPD)problem.However,existing research has not addressed the energy limitations associated with the LMAPD problem,rendering it inapplicable to real-world systems.In this study,we formulate the energy-limited lifelong multi-agent pickup and delivery(EL-MAPD)problem,in which each agent consumes or recharges a specific amount of energy for its actions.Furthermore,we theoretically define a realistic subclass of EL-MAPD instances known as well-formed EL-MAPD instances.This subclass ensures the existence of feasible solutions,preventing scenarios in which agents run out of energy or collide with each other.To obtain a high-quality feasible solution under the framework of concurrent planning and execution,we propose the fallback priority planning(FPP)algorithm.The FPP algorithm employs a fallback mechanism to ensure the acquisition of a feasible action in each planning episode.Experimental results demonstrate that the FPP algorithm effectively balances throughput and energy consumption.Furthermore,to enhance the warehouse system’s throughput in real-world scenarios,our findings suggest decreasing the planning time limit for each planning episode,increasing the number of agents,and implementing a recharge strategy wherein agents complete the recharging process only when their energy levels are full. 展开更多
