Objectives: The aim of our work was to assess the knowledge, attitudes and practices of mothers of children aged 0 - 24 months on exclusive breastfeeding in the Central African Republic. Methods: This was a cross-sectional, descriptive and analytical study conducted from September 15 to October 15, 2024 among mothers of infants aged 0 to 24 months. Sociodemographic, obstetric and breastfeeding-related data were collected through individual interviews conducted during sensitizations on good feeding practices organized by the Tina Touadera Foundation. The chi2 test was used to test for relationships between variables, and the p significance level was set at 0.05. Results: The average age of the mothers surveyed was 27.67 years. 65.69% (n = 247) of mothers lived in urban areas and 55.85% (n = 210) were Muslim. 56.38% (n = 212) were living common-law and 34.04% (n = 128) were poor. Secondary-school mothers (44.42%, n = 167) and housewives (53.72%, n = 202) were in the majority. Exclusive breastfeeding (EBF) was correctly defined by 79.26% (n = 298) of mothers and actually practised in 24.20% (n = 91) of cases. The main source of information was a health professional in 75.36% (n = 304) of cases. Among the 285 mothers who practised mixed breastfeeding, lack of time (33.33%) was the main reason. They acknowledged having given water (100%), corn porridge (75.09%) and/or artificial milk (24.91%) before the first 6 months of life. The average time for introducing water was 2.2 months, and for porridge/formula 2.79 months. More than half the mothers (55.05%) said they did not know their infants’ weaning age. Factors positively influencing the use of EBF were age under 29, residence in an urban area, primiparity, having been informed about AME by a health professional, and being a housewife or shopkeeper (p Conclusion: Mothers’ level of knowledge was heterogeneous but insufficient overall. An effective system of information and education from pregnancy to the first six months of life is needed to promote breastfeeding.展开更多

This study, set against the backdrop of the shipping industry, combines top-down and bottom-up schema design strategies to achieve precise and flexible knowledge representation. The research adopts a semi-structured approach, innovatively constructing an adaptive schema generation mechanism based on reinforcement learning, which models the knowledge graph construction process as a Markov decision process. This method begins with general concepts, defining foundational industry concepts, and then delves into abstracting core concepts specific to the maritime domain through an adaptive pattern generation mechanism that dynamically adjusts the knowledge structure. Specifically, the study designs a four-layer knowledge construction framework, including the data layer, modeling layer, technology layer, and application layer. It draws on a mutual indexing strategy, integrating large language models and traditional information extraction techniques. By leveraging self-attention mechanisms and graph attention networks, it efficiently extracts semantic relationships. The introduction of logic-form-driven solvers and symbolic decomposition techniques for reasoning significantly enhances the model’s ability to understand complex semantic relationships. Additionally, the use of open information extraction and knowledge alignment techniques further improves the efficiency and accuracy of information retrieval. Experimental results demonstrate that the proposed method not only achieves significant performance improvements in knowledge graph retrieval within the shipping domain but also holds important theoretical innovation and practical application value.展开更多

To achieve this, we surveyed a convenience sample of 73 health professionals including 23 radiologists working in the hospitals surveyed to assess knowledge and level of implementation of radiation protection principles. Also, physical parameters were taken for the calculation of entry doses in paediatric and adult radiology units for comparison with the International Commission on Radiological Protection (ICRP) diagnostic reference levels. After analysis of the data, the following was found: although radiologists have sufficient knowledge of radiation protection standards, technical constraints do not allow them to observe the dose limitation principle recommended by the ICRP. This is why several radiology departments, including those of the HPGRB, the MWANZI clinic and CIRIRI hospital, have proved to be very irradiating for children. However, radiologists and non-radiologists alike do not contribute positively to the optimisation of radiation protection in the diagnostic use of X-rays. Therefore, support in the implementation of radiation protection principles and regular monitoring of the units as well as replacement of non-standard equipment is necessary to promote patient and environmental safety by optimising radiation protection.展开更多

文摘Background:Invasive meningococcal disease is a potentially life-threatening infection caused by Neisseria meningitidis.Vaccination is highly effective in preventing meningitis and reducing its associated complications. This study aims to assess the United Arab Emirates(UAE)parents meningitis knowledge and explore vaccination attitudes and practices.Methodology:An observational cross-sectional study was conducted between March and May 2024 across the UAE using convenience sampling through social media, electronic mail, and word of mouth.A 58-item questionnaire collected information regarding demographics, meningitis knowledge, attitudes and practices, and meningococcal vaccination knowledge and practices. Results:Of 443 parents included, more than a third had no knowledge about meningitis.Symptoms and severity were well-recognized overall, but clear gaps were evident regarding complications and meningococcal vaccines.Only 10.38%(46/443)identified themselves as knowledgeable/very knowledgeable regarding meningococcal vaccines.A healthcare provider recommendation was the strongest factor encouraging parents to vaccinate their child. Most vaccine hesitancy was seen regarding side effects.Views and practices regarding the two vaccines aligned closely, with more than four-fifths of participants needing more information and the major reasons for not vaccinating a child being a lack of information or lack of recommendation.Social media and governmental websites were the most common sources for learning more about meningitis, with doctors ranking third. In fact, trust in doctors varied with less than half having moderate or strong trust in their main paediatrician.Multivariate logistic regression revealed that coronavirus disease 2019 vaccination status[adjusted odds ratio(AOR):0.365,95%confidence interval(CI):0.172 to 0.774,P=0.013]and female gender(AOR:2.741,95%CI:1.184 to 6.347,P=0.019)were significant predictors of vaccine hesitancy. Conclusion:Meningococcal vaccine hesitancy is a significant concern, primarily driven by fears of side effects and lack of knowledge and physician recommendation. However, vaccine attitudes were overall positive and highly dependent on physician involvement. There is a need for targeted educational initiatives enhancing meningitis disease awareness and vaccine uptake.
文摘Introduction: Breastfeeding is the best way to provide ideal nutrition for optimal infant growth and development. An effective system of information and education from pregnancy to the first six months of life is needed to promote breastfeeding.

文摘With the continuous development of artificial intelligence and natural language processing technologies, traditional retrieval-augmented generation (RAG) techniques face numerous challenges in document answer precision and similarity measurement. Experimental results demonstrate that the proposed method not only achieves significant performance improvements in knowledge graph retrieval within the shipping domain but also holds important theoretical innovation and practical application value.
文摘BACKGROUND Glaucoma is the leading cause of irreversible blindness worldwide, with 60.5 million affected individuals,of whom 11 million are from India.Due to its asymptomatic nature, the disease largely remains underdiagnosed or diagnosed in advanced stages, where little can be done to salvage functional vision. The literature suggests that a lack of knowledge is one of the reasons for its grave consequences.Assessment of awareness is the first step in planning management.Several studies have been conducted in the Indian community, but data from healthcare providers, who play a significant role in educating the masses directly or indirectly, are limited.AIM To identify awareness, knowledge, and attitudes about glaucoma among healthcare workers in a tertiary center in India.METHODS This cross-sectional study was conducted at a tertiary care institute in Eastern India.Data were collected from 423 participants by systematic stratified sampling after Institutional Ethics Committee approval via a pretested,self-designed,semistructured,validated questionnaire.Statistical analyses were performed using the Statistical Package for Social Sciences Software v22.0.Continuous variables are expressed as the means±SD for parametric values and medians with interquartile ranges for nonparametric values. The associations between the variables were studied via multivariate linear and logistic regression.P<0.05 was considered statistically significant.RESULTS Most respondents were 20–30 years old(n=345,81.6%).The knowledge regarding glaucoma was good, and almost 56.3%of the participants gained knowledge from their medical training. The majority were aware that it has a familial predisposition and is secondary to high intraocular pressure, leading to irreversible peripheral vision loss.Only 42%knew about the life-long requirements of treatment. The resident group scored highest on knowledgeand attitude-based questions, whereas the faculty group scored highest on practice-based questions.Although 62%of the nursing staff had good attitude scores, their knowledge and practice scores were lower. The occupation group response difference was statistically significant(P<0.05)for all the knowledge-based questions.CONCLUSION Although the majority of healthcare providers are aware of glaucoma, there is a dearth of knowledge about treatment modalities.Education via seminars and media can improve their knowledge, attitudes, and practices.
文摘In the context of the“Two New”initiatives, high school mathematics instruction still grapples with three interlocking problems:knowledge fragmentation, limited cultivation of higher-order thinking, and weak alignment among teaching, learning, and assessment. To counter these challenges,we propose an Inquiry-Construction Double-Helix model that uses a domain-specific knowledge graph as its cognitive spine. The model interweaves two mutually reinforcing strands-student-driven inquiry and systematic knowledge construction-into a double-helix trajectory analogous to DNA replication. The Inquiry Strand is launched by authentic,situation-based tasks that shepherd students through the complete cycle:question→hypothesis→verification→reflection. The Construction Strand simultaneously externalizes, restructures, and internalizes core disciplinary concepts via visual, hierarchical knowledge graphs.Within the flow of a lesson, the two strands alternately dominate and scaffold each other, securing the co-development of conceptual understanding, procedural fluency, and mathematical literacy.Empirical evidence demonstrates that this model significantly enhances students’systematic knowledge integration,problem-solving transfer ability, and core mathematical competencies, offering a replicable and operable teaching paradigm and practical pathway for deepening high school mathematics classroom reform.
文摘The aim of this research was to assess the knowledge and practices of radiological versus non-radiological health professionals on the optimisation of radiation protection in paediatric and adult radiology in BUKAVU hospitals. Therefore, support in the implementation of radiation protection principles and regular monitoring of the units as well as replacement of non-standard equipment is necessary to promote patient and environmental safety by optimising radiation protection.

文摘Since Google introduced the concept of Knowledge Graphs(KGs)in 2012,their construction technologies have evolved into a comprehensive methodological framework encompassing knowledge acquisition, extraction, representation, modeling, fusion, computation, and storage.Within this framework, knowledge extraction,as the core component, directly determines KG quality. In military domains, traditional manual curation models face efficiency constraints due to data fragmentation, complex knowledge architectures, and confidentiality protocols.Meanwhile, crowdsourced ontology construction approaches from general domains prove non-transferable,while human-crafted ontologies struggle with generalization deficiencies. To address these challenges, this study proposes an OntologyAware LLM Methodology for Military Domain Knowledge Extraction(LLM-KE).This approach leverages the deep semantic comprehension capabilities of Large Language Models(LLMs)to simulate human experts’cognitive processes in crowdsourced ontology construction, enabling automated extraction of military textual knowledge. It concurrently enhances knowledge processing efficiency and improves KG completeness.Empirical analysis demonstrates that this method effectively resolves scalability and dynamic adaptation challenges in military KG construction, establishing a novel technological pathway for advancing military intelligence development.

文摘The rapid development of information technology and accelerated digitalization have led to an explosive growth of data across various fields. As a key technology for knowledge representation and sharing, knowledge graphs play a crucial role by constructing structured networks of relationships among entities. However, data sparsity and numerous unexplored implicit relations result in the widespread incompleteness of knowledge graphs. In static knowledge graph completion, most existing methods rely on linear operations or simple interaction mechanisms for triple encoding, making it difficult to fully capture the deep semantic associations between entities and relations. Moreover, many methods focus only on the local information of individual triples, ignoring the rich semantic dependencies embedded in the neighboring nodes of entities within the graph structure, which leads to incomplete embedding representations. To address these challenges,we propose Two-Stage Mixer Embedding(TSMixerE),a static knowledge graph completion method based on entity context. In the unit semantic extraction stage,TSMixerE leveragesmulti-scale circular convolution to capture local features atmultiple granularities, enhancing the flexibility and robustness of feature interactions.A channel attention mechanism amplifies key channel responses to suppress noise and irrelevant information, thereby improving the discriminative power and semantic depth of feature representations. For contextual information fusion,a multi-layer self-attentionmechanism enables deep interactions among contextual cues, effectively integrating local details with global context.Simultaneously, type embeddings clarify the semantic identities and roles of each component, enhancing the model’s sensitivity and fusion capabilities for diverse information sources.Furthermore,TSMixerE constructs contextual unit sequences for entities, fully exploring neighborhood information within the graph structure to model complex semantic dependencies, thus improving the completeness and generalization of embedding representations.

文摘Globally, mealtimes provide key insights into cultural and social values and practices. We examine mealtime environments and eating practices in two different African settings using quantitative observational and questionnaire data.Participating families(N=80)were recruited from two rural locations in Zambia and Kenya. Results following descriptive analysis showed that meals are typically taken as a family in a sociable context, providing opportunities to nurture children’s positive behaviours. In both communities,mothers(71.25%)were most likely to be present and typically prepared meals and provided the food.We observed a few distractions being used during mealtimes, and children finished their food with little to no conflict.Plate sharing varied across the two sites and was more common in Zambia, where we also observed more traditional practices such as eating with hands(as opposed to cutlery)and sitting on mats on the ground(as opposed to seated on chairs or sofas).Overall, our findings suggest more similarities in the cultural and social values across the two African contexts despite slight differences in mealtime practices. In particular, positive mealtime behaviours(little food refusal, lack of conflict)were common across all contexts, regardless of the extent to which traditional practices(such as plate-sharing and sitting on the floor)were used.Findings align with Family Systems theory, which states that the family is understood best by conceptualizing it as a complex, dynamic, and changing collection of parts, subsystems, and family members.

文摘This paper proposes a deep learning-based 3D LiDAR perception framework designed for applications such as autonomous robots and vehicles. To address the high dependency on large-scale annotated data—an inherent limitation of deep learning models—this study introduces a hybrid perception architecture that incorporates expertdriven LiDAR processing techniques into the deep neural network.Traditional 3DLiDAR processingmethods typically remove ground planes and apply distance-or density-based clustering for object detection. In this work, such expert knowledge is encoded as feature-level inputs and fused with the deep network, therebymitigating the data dependency issue of conventional learning-based approaches.Specifically, the proposedmethod combines two expert algorithms—Patchwork++for ground segmentation and DBSCAN for clustering—with a PointPillars-based LiDAR detection network. We design four hybrid versions of the network depending on the stage and method of integrating expert features into the feature map of the deep model.Among these,Version 4 incorporates a modified neck structure in PointPillars and introduces a new Cluster 2D Pseudo-Map Branch that utilizes cluster-level pseudo-images generated from Patchwork++and DBSCAN.This version achieved a+3.88%improvement mean Average Precision(mAP)compared to the baseline PointPillars. The results demonstrate that embedding expert-based perception logic into deep neural architectures can effectively enhance performance and reduce dependency on extensive training datasets, offering a promising direction for robust 3D LiDAR object detection in real-world scenarios.
