The three-dimensional finite element method is used to solve the problem of the quarter-elliptical comer crack of the bolt-hole in mechanical joints being subjected to remote tension. The square-root stress singularity around the corner crack front is simulated using the collapsed 20-node quarter point singular elements. The contact interaction between the bolt and the hole boundary is considered in the finite element analysis. The stress intensity factors (SIFs) along the crack front are evaluated by using the displacement correlation technique. The effects of the amount of clearance between the hole and the bolt on the SIFs are investigated. The numerical results indicate that the SIF for mode I decrease with the decreases in clearance, and in the cases of clearance being present, the corner crack is in a mix-mode, even if mode I loading is dominant. 展开更多

Despite significant improvements over the past several decades in diagnosis,treatment and prevention of periprosthetic joint infection(PJI),it still remains a major challenge following total joint arthroplasty.Given the devastating nature and accelerated incidence of PJI,prevention is the most important strategy to deal with this challenging problem and should start from identifying risk factors.Understanding and well-organized optimization of these risk factors in individuals before elective arthroplasty are essential to the ultimate success in reducing the incidence of PJI.Even though some risk factors such as demographic characteristics are seldom changeable,they allow more accurate expectation regarding individual risks of PJI and thus,make proper counseling for shared preoperative decision-making possible.Others that increase the risk of PJI,but are potentially modifiable should be optimized prior to elective arthroplasty.Although remarkable advances have been achieved in past decades,many questions regarding standardized practice to prevent this catastrophic complication remain unanswered.The current study provide a comprehensive knowledge regarding risk factors based on general principles to control surgical siteinfection by the review of current literature and also share own practice at our institution to provide practical and better understandings. 展开更多

Vertex centrality of complex networks based on joint nonnegative matrix factorization and graph embedding

Finding crucial vertices is a key problem for improving the reliability and ensuring the effective operation of networks,solved by approaches based on multiple attribute decision that suffer from ignoring the correlation among each attribute or the heterogeneity between attribute and structure. To overcome these problems, a novel vertex centrality approach, called VCJG, is proposed based on joint nonnegative matrix factorization and graph embedding. The potential attributes with linearly independent and the structure information are captured automatically in light of nonnegative matrix factorization for factorizing the weighted adjacent matrix and the structure matrix, which is generated by graph embedding. And the smoothness strategy is applied to eliminate the heterogeneity between attributes and structure by joint nonnegative matrix factorization. Then VCJG integrates the above steps to formulate an overall objective function, and obtain the ultimately potential attributes fused the structure information of network through optimizing the objective function. Finally, the attributes are combined with neighborhood rules to evaluate vertex's importance. Through comparative analyses with experiments on nine real-world networks, we demonstrate that the proposed approach outperforms nine state-of-the-art algorithms for identification of vital vertices with respect to correlation, monotonicity and accuracy of top-10 vertices ranking. 展开更多

Objective To explore the relative factors affecting the functional restoration of knee joint after the fracture of patella, and to anticipate the effect of middle-term and short-time rehabilitation therapy. Method 23 patients with limited flexion function of knee joint after the fracture of patella, accepted the routine rehabilitation treatment in out -patient service, and some of the patients adopted local heat therapy at the same time. The 13 factors have been selected for regression analysis step by step under the help of SPSS statistical software. Result The flexible degree of the knee joint before rehabilitation (X1), the non-smooth posterior edge of patella (X2), and the immobilization time of the joint (X3) are relative factors for the flexible degree of knee joint 1 month after rehabilitation (Y1) and the flexible degree of knee joint 3 months after rehabilitation (Y2), and Y1 can also be affected by whether surgery therapy or not (X4), (partial regression coefficient P< 0.05). The differences between theoretical mean and practical mean of Y1 and Y2 are 5.6% and 4.2% respectively in the 23 cases. Conclusion The flexible degree of the knee joint before rehabilitation, whether the posterior edge of patella is smooth or not, and the immobilization time of the joint are relative factors affecting the middle-term and short-term flexible function of knee joint after the fracture of patella, and short-term restoration can also be affected by whether surgery therapy or not. The effects of middle-term and short-term rehabilitation can be predicted by regression equation primarily. 展开更多

AIM To evaluate the changes in the 8^(th) edition American Joint Committee on Cancer(AJCC) for defining stage?ⅠB and?ⅡA pancreatic cancer and identify their prognostic factors.METHODS Pancreatic cancer patients were selected from the Surveillance Epidemiology and End Results database(1973-2013). The enrolled patients were divided into?ⅠB and?ⅡA groups based on tumor size according to the 8^(th) edition AJCC criteria. Clinical characteristics, including age, gender, race, tumor size, primary site, and grade were summarized. Univariate and multivariate analyses were performed to explore the prognostic factors of the?ⅠB and?ⅡA stages of pancreatic cancer under new criteria.RESULTS A total of 1349 pancreatic cancer patients were included. More patients had stage?ⅠB rather than stage?ⅡA. Stage?ⅠB tumors(54.85%) were mainly located in the head of the pancreas, while stage?ⅡA tumors were more often located in the tail and head of the pancreas(35.21% and 31.75%, respectively). The survival time of stage?ⅠB and?ⅡA patients had no significant difference. Univariate and multivariate analyses indicated that the prognostic factors of survival for stage?ⅠB and?ⅡA patients were different. for stage?ⅠB patients, age and primary site were the independent prognostic factors; for stage?ⅡA patients, age and grade were the independent prognostic factors. The risk of death was lower among patients aged ≤ 65 years than those aged > 65 years.CONCLUSION The prognostic factors for stage?ⅠB and?ⅡA patients are different, but age is the independent prognostic factor for all patients. The survival time of stage?ⅠB and?ⅡA patients has no significant difference. 展开更多

In order to improve the bending load-carrying capacity (BLCC) of undermatched butt joint under three-point bending load, the influence of joint geometric parameters on stress concentration factors (SCF) at the weld bottom center and the weld toe of uudermatched butt joint with single V-groove are studied respectively based on the finite element method in this paper. Results show that the reinforcement height and the cover pass width play decisive role in the BLCC for undermatched butt joint. BLCC of undermatched butt joint can be improved by choosing the appropriate joint geometric parameters. 展开更多

Analysis of the stress intensity factor of welded joints under prior corrosion and complex stress fields

Taking cruciform welded joints under different corrosion degrees, together with 45° inclined angle and full penetration load-carrying fillet, as the research object, the stress intensity factor of a quarter-circular comer crack of welded joints is calculated based on FRANC3D with ABAQUS. Effects of different corrosion pit sizes, crack aspect ratios, and crack depths on stress intensity factor are analyzed. The results show that pit depth plays a major role in stress intensity factor, while the effect of pit radius is relatively small. The cracking modes of the surface and the deepest point are mode I, and mixed modes I and II, respectively. Effects of pit depths, crack aspect ratios, and crack depths on the stress intensity factor at the surface point are greater than at the deepest point. 展开更多

为研究不锈钢圆管T型相贯节点的疲劳性能,通过应力集中系数试验、疲劳加载试验及有限元分析,探究其疲劳破坏规律。应力集中系数试验结果表明:焊缝处应变从鞍点到冠点呈递减趋势,且距离焊缝越近应变值越大;不锈钢节点的应变集中系数调整... 为研究不锈钢圆管T型相贯节点的疲劳性能,通过应力集中系数试验、疲劳加载试验及有限元分析,探究其疲劳破坏规律。应力集中系数试验结果表明:焊缝处应变从鞍点到冠点呈递减趋势,且距离焊缝越近应变值越大;不锈钢节点的应变集中系数调整参数ω与碳素钢节点存在差异,主管为1.25,支管为1.17。疲劳加载试验显示:裂纹萌生于主管鞍点附近,与最大热点应力位置一致,疲劳寿命为149 655次循环;节点刚度退化分为快速退化、缓慢退化和裂纹发展3个阶段。有限元分析表明,参数β和γ对应力集中系数影响显著,模拟结果与试验结果吻合良好。 展开更多

Objective: The aim of the study was to evaluate the effect of platelet rich growth factors (PRGFs) in treatment of temporomandibular joint disc displacement. Materials and Methods: The study subjects included 8 females having bilateral anterior disc displacement with reduction and 1 female having bilateral anterior disc displacement without reduction with the age range between 20 - 35 years. The process of obtaining PRGFs was carried out following the Anitua Technique. Results: Clinical parameters of Interincisal distance, Lateral excursion of mandible using digital caliper in millimeters and Visual Analogue Scale (VAS 0/10) for pain intensity score were used. All of these parameters were running through the intervals of two, four, and eight weeks till the end of the follow-up period at twenty-six weeks (six months). The participated patients showed the clinical improvement in the different clinical statuses such as interincisal distance;lateral excursion of mandible and Pain Score. Conclusion: the study reported early efficacy of PRGFs after the arthrocentesis of the joint in treatment of TMJ disc displacement, and according to our results, the injection of PRGFs could be a possible alternative treatment for patients who did not respond to standard treatment. 展开更多

目的基于膝关节液中免疫因子、细胞计数分类、膝关节液涂片结果等25个指标,建立用于区分膝骨关节炎(OA)与类风湿关节炎(RA)的机器学习模型。方法分别选取100例和40例择期进行膝关节置换术的OA与RA患者。在术前抽取患者的膝关节液,检测... 目的基于膝关节液中免疫因子、细胞计数分类、膝关节液涂片结果等25个指标,建立用于区分膝骨关节炎(OA)与类风湿关节炎(RA)的机器学习模型。方法分别选取100例和40例择期进行膝关节置换术的OA与RA患者。在术前抽取患者的膝关节液,检测其中的有核细胞计数及分类,测量免疫因子包括肿瘤坏死因子α(TNF-α)、白细胞介素1β(IL-1β)、IL-6、IL-8、IL-15、基质金属蛋白酶3(MMP3)、MMP9、MMP13、类风湿因子(RF)、血清淀粉样蛋白A(SAA)、C-反应蛋白(CRP)等的表达水平,并进行涂片染色及镜检分类。通过单因素二元Logistic回归、Lasso回归和多因素二元Logistic回归筛选OA或RA的独立影响因子。基于独立影响因子分别构建逻辑回归、随机森林和支持向量机3种机器学习模型。使用受试者工作特征曲线(ROC)、校准曲线、决策曲线分析(DCA)对各模型进行评价和比较。结果共筛选出5个用于区分OA与RA的膝关节液指标,分别是IL-1β[优势比(OR)=10.512,95%可信区间(95%CI)为1.048-105.42,P=0.045)、IL-6(OR=1.007,95%CI为1.001-1.014,P=0.022)、MMP9(OR=3.202,95%CI为1.235-8.305,P=0.017)、MMP13(OR=1.002,95%CI为1-1.004,P=0.049)、RF(OR=1.091,95%CI为1.01-1.179,P=0.026)。综合ROC、校准曲线、DCA的结果,随机森林模型的准确性(0.979)、敏感度(0.98)、曲线下面积(AUC为0.996,95%CI为0.991-1)均最高,且具有良好的有效性和可行性,其区分能力优于其他2种模型。结论基于膝关节液免疫因子建立的机器学习模型在区分OA与RA方面有一定价值,能够为临床早期鉴别诊断和防治OA与RA提供参考。 展开更多

目的分析针灸联合新型可调三点式膝关节牵引装置对退行性膝关节骨关节炎患者疼痛、功能及炎症指标的影响。方法纳入退行性膝关节骨关节炎患者180例为研究对象,随机数字表法分为3组,针灸组60例采用针灸治疗,牵引组60例采用可调式三点膝... 目的分析针灸联合新型可调三点式膝关节牵引装置对退行性膝关节骨关节炎患者疼痛、功能及炎症指标的影响。方法纳入退行性膝关节骨关节炎患者180例为研究对象,随机数字表法分为3组,针灸组60例采用针灸治疗,牵引组60例采用可调式三点膝关节牵引治疗,联合组60例采用针灸联合可调三点式膝关节牵引治疗,对比两组治疗效果、中医症状积分、疼痛情况、关节功能、炎症因子以及膝间距和胫股角情况等。结果联合组治疗总有效率为96.67%(58/60),远高于针灸组的80.0%(48/60)、牵引组的81.67%(49/60),组间比较差异有统计学意义(P<0.05);治疗后联合组关节疼痛、关节肿胀、屈伸不利、腰膝酸软4项中医症状积分、VAS评分均低于针灸组、牵引组(P<0.05);治疗后联合组Lequesne髋膝关节功能评分(LequesneMG评分)、WOMAC关节炎指数(WOMAC评分)低于针灸组、牵引组(P<0.05);治疗后联合组白细胞介素-6(IL-6)、白细胞介素-1β(IL-1β)、高敏C反应蛋白(hs-CRP)低于针灸组、牵引组(P<0.05);治疗后联合组胫股角大于针灸组、牵引组,膝间距小于针灸组、牵引组,组间差异有统计学意义(P<0.05)。结论对退行性膝关节骨关节炎患者采用针灸联合可调三点式膝关节牵引治疗效果明确,能有效改善患者症状,减轻疼痛程度,利用纠正膝关节内外翻情况,减轻炎症因子水平,下调hs-CRP、IL-1β、IL-6表达,提高膝关节活动度,促进膝关节功能恢复。 展开更多

穿越断层破碎带隧道变刚度衬砌振动台试验研究

依托我国西部地区穿越大型活动断裂带隧道工程,基于“引”和“让”的抗震理念设计穿越断层隧道变刚度衬砌结构,实现结构诱导变形、减小地震损伤范围的目的,并开展穿越断层破碎带隧道变刚度衬砌振动台试验进行验证。在水平剪切地震动激励... 依托我国西部地区穿越大型活动断裂带隧道工程,基于“引”和“让”的抗震理念设计穿越断层隧道变刚度衬砌结构,实现结构诱导变形、减小地震损伤范围的目的,并开展穿越断层破碎带隧道变刚度衬砌振动台试验进行验证。在水平剪切地震动激励下,根据隧道结构的加速度、接触应力、应变响应特征,以及隧道结构整体、局部连接处震害形态,探究强震作用下变刚度衬砌节段的损伤特征和减震抗错性能。研究结果表明:小于0.2g地震动时隧道结构沿轴向加速度放大系数呈断层破碎带中部大、交界面附近小的特点,地震动大于0.2g时分布形式则相反;边际谱幅值放大系数小于1时,表明隧道结构能量耗散、产生损伤,0.2g地震动后断层破碎带内隧道仰拱局部出现损伤,0.8g地震动后上盘与断层交界面隧道边际谱幅值放大系数降至0.3,结构损伤最为严重;交界面附近围岩-衬砌接触应力峰值小于上、下盘,但围岩发生塑性破坏的时机更早;破碎带内隧道仰拱损伤早于上、下盘,但损伤程度更轻;断层交界面附近衬砌开裂、错断最严重,需重点加强抗震设计;断层破碎带中部及上、下盘距交界面较远处由于衬砌变刚度结构和柔性接头的减震、诱导变形功能,震后隧道结构损伤程度较轻、范围较小。文中结论可为高烈度地震区隧道工程抗震与减震设计提供一定的参考依据。 展开更多

目的 探讨基层地区强直性脊柱炎(AS)患者髋关节病变的临床特点及危险因素。方法 纳入于临泉县人民医院确诊的AS患者107例,根据是否合并髋关节受累分为髋关节受累组(47例)与非髋关节受累组(60例)。比较两组患者一般临床资料、实验室检查... 目的 探讨基层地区强直性脊柱炎(AS)患者髋关节病变的临床特点及危险因素。方法 纳入于临泉县人民医院确诊的AS患者107例,根据是否合并髋关节受累分为髋关节受累组(47例)与非髋关节受累组(60例)。比较两组患者一般临床资料、实验室检查结果与影像学检查结果。采用多因素logistic回归分析评估AS患者合并髋关节病变的影响因素。结果 与非髋关节受累组比较,髋关节受累组患者BMI、中轴关节首发患者比例更低;AS病程、病程≥10年、发病年龄≤16岁患者比例、延误治疗时间、C反应蛋白(CRP)、AS疾病活动评分(ASDAS)更高;与非髋关节受累组相比,髋关节受累组0~3级患者比例更低,4级患者比例更高(P<0.05)。多因素logistic回归分析结果显示,发病年龄≤16岁、病程≥10年、碱性磷酸酶(ALP)、骶髂关节炎放射学分级、ASDAS均为AS合并髋关节病变的危险因素;BMI是其保护因素(P<0.05)。病程越长,AS患者髋关节受累的发生率越高(P<0.05)。结论 基层地区应更加关注低BMI、发病年龄≤16岁、病程≥10年、高ALP、骶髂关节炎分级严重和高ASDAS的AS患者髋关节受累情况。 展开更多

为研究钢管混凝土KK(CFST-KK)型节点应力集中系数(SCF)计算方法,开展CFST-KK型节点模型试验和实体有限元分析,通过试验与有限元结果对比分析验证了有限元模型正确性;通过有限元参数分析揭示CFST-KK型节点SCF的敏感性以及管内混凝土的影... 为研究钢管混凝土KK(CFST-KK)型节点应力集中系数(SCF)计算方法,开展CFST-KK型节点模型试验和实体有限元分析,通过试验与有限元结果对比分析验证了有限元模型正确性;通过有限元参数分析揭示CFST-KK型节点SCF的敏感性以及管内混凝土的影响机理;探讨现有相贯管节点SCF计算方法的适用性,基于节点刚度推导得到钢管KK(CHS-KK)型节点SCF计算方法,基于变形相似原则并考虑管内混凝土与主管管壁的剥离变形作用,提出了CFST-KK型节点SCF计算方法。结果表明:有限元模型最大误差为8.0%,具有良好的精度;计算CFST-KK型节点SCF时,可只考虑轴力和面内弯矩的作用而忽略面外弯矩的作用;主管径厚比对CFST-KK型节点SCF的影响最为显著,主管内填混凝土可提升主管管壁整体径向刚度,使CFST-KK型节点SCF减小28.5%~48.9%;采用现有相贯管节点SCF计算方法计算CFST-KK型节点SCF时均存在较大误差;扩展有限元参数分析验证了文章提出的CHS-KK和CFST-KK型节点SCF计算方法具有良好的精度且计算结果偏于安全。 展开更多

目的研究膝关节镜日间手术静脉血栓栓塞(VTE)的发生率,探讨其发生的相关危险因素。方法回顾性研究,连续纳入2022年1月至2023年7月遂宁市中心医院接受单侧膝关节镜日间手术且符合纳入及排除标准的1019例病人,其中男558例,女461例,年龄范... 目的研究膝关节镜日间手术静脉血栓栓塞(VTE)的发生率,探讨其发生的相关危险因素。方法回顾性研究,连续纳入2022年1月至2023年7月遂宁市中心医院接受单侧膝关节镜日间手术且符合纳入及排除标准的1019例病人,其中男558例,女461例,年龄范围8~74岁。术前彩超排除下肢静脉血栓,术后第1天复查下肢血管超声后出院,如有血栓则转诊血管外科。根据术后超声将病人分为VTE组和非VTE组,统计术后VTE发生率,采用单因素及二元多重logistic回归分析VTE发生的危险因素。结果1019例日间手术病人中128例12.56%(128/1019)出现VTE。其中肺栓塞1例0.10%(1/1019),近端深静脉血栓(DVT)病人4例0.39%(4/1019),远端DVT病人123例12.07%(123/1019)。术后分析结果显示,女性、年龄增大、胆固醇增高、高脂血症、脑梗死病史、下肢静脉曲张、手术时间长、手术术式是影响发生血栓的独立危险因素(OR>1)。相较于关节清理术,关节镜腘窝囊肿切除术,前交叉韧带(ACL)重建术,后交叉韧带(PCL)重建术病人发生血栓的概率明显升高,其OR值分别为45.30,3.62,4.87。结论膝关节镜日间手术后VTE的发生率为12.56%,主要为远端DVT(12.07%)。女性、年龄增大、胆固醇增高、高脂血症、脑梗死病史、下肢静脉曲张、手术时间长、手术方式是发生血栓的危险因素。相较于关节清理术,关节镜腘窝囊肿切除术,ACL重建术和PCL重建术血栓形成风险明显增高。 展开更多

目的通过回顾性病例分析儿童陈旧性孟氏骨折术后肘关节再脱位的危险因素。方法回顾性分析2009-01—2023-12手术治疗的64例儿童陈旧性孟氏骨折,将术后肘关节再脱位作为因变量,性别、体重、损伤侧别、病程、尺骨延长长度(尺骨鹰嘴近端至... 目的通过回顾性病例分析儿童陈旧性孟氏骨折术后肘关节再脱位的危险因素。方法回顾性分析2009-01—2023-12手术治疗的64例儿童陈旧性孟氏骨折,将术后肘关节再脱位作为因变量,性别、体重、损伤侧别、病程、尺骨延长长度(尺骨鹰嘴近端至尺骨远端连线的长度)、尺骨向后成角矫正度(手术前后侧位X线片上尺骨近端解剖轴与尺骨中段解剖轴夹角的差值)、环状韧带是否重建(以肱三头肌筋膜条或前臂筋膜重建环状韧带)、肱桡关节是否克氏针固定作为自变量,进行多因素Logistic回归分析术后肘关节再脱位的影响因素。结果18例(28.1%)术后肘关节再脱位,6例术后2~3周再次脱位,12例术后1~2个月再次脱位。再脱位组病程较未再脱位组长,再脱位组肱桡关节克氏针固定比例高于未再脱位组,差异有统计学意义(P<0.05);两组在性别、体重、损伤侧别、环状韧带重建比例、尺骨向后成角矫正度、尺骨延长长度差异无统计学意义(P>0.05)。多因素Logistic回归分析结果显示,病程(OR=4.366,95%CI为1.454~13.113,P=0.009)、肱桡关节克氏针固定(OR=3.984,95%CI为1.106~14.353,P=0.035)是儿童陈旧性孟氏骨折术后肘关节再脱位的危险因素。结论陈旧性孟氏骨折患儿病程延长与肱桡关节克氏针固定均会增加术后肘关节再脱位风险,因此孟氏骨折患儿应早发现、早诊断、早治疗,陈旧性孟氏骨折术中需要重建肱桡关节的稳定性,但是不能过度依赖克氏针固定,否则术后肘关节再脱位风险增加。 展开更多

二术苓皮汤联合五子散封包疗法治疗髋关节滑膜炎患者的效果观察

目的观察二术苓皮汤联合五子散封包疗法治疗髋关节滑膜炎患者的效果。方法选取88例髋关节滑膜炎患者,通过随机数字表法按照1∶1比例分为对照组和实验组,每组44例。对照组给予基础治疗,实验组在基础治疗上加用二术苓皮汤联合五子散封包... 目的观察二术苓皮汤联合五子散封包疗法治疗髋关节滑膜炎患者的效果。方法选取88例髋关节滑膜炎患者,通过随机数字表法按照1∶1比例分为对照组和实验组,每组44例。对照组给予基础治疗,实验组在基础治疗上加用二术苓皮汤联合五子散封包疗法治疗。比较两组临床疗效,治疗前后的中医证候积分、髋关节相关指标和炎症因子[白细胞介素-1β(IL-1β)、红细胞沉降率(ESR)、白细胞介素-6(IL-6)]水平。结果实验组总有效率(95.45%)较对照组(75.00%)明显更高(P<0.05)。治疗3周后,两组疼痛跛行、关节肿胀、关节压痛、肢体困重积分均较本组治疗前明显降低,且实验组比对照组更低(P<0.05)。治疗3周后,两组股骨颈前的间隙、髋关节腔积液量均较本组治疗前明显降低,Harris评分较本组治疗前明显升高,且实验组股骨颈前的间隙(5.33±0.77)mm、髋关节腔积液量(1.20±0.31)ml比对照组的(6.12±0.87)mm、(1.73±0.44)ml更低,Harris评分(90.68±8.03)分比对照组的(83.15±7.51)分更高(P<0.05)。治疗3周后,两组IL-1β、ESR、IL-6水平均较本组治疗前明显降低,且实验组IL-1β(5.27±0.70)ng/L、ESR(12.44±2.88)mm/h、IL-6(16.10±2.94)ng/L比对照组的(5.99±0.84)ng/L、(17.08±3.29)mm/h、(19.98±3.59)ng/L更低(P<0.05)。结论髋关节滑膜炎患者应用二术苓皮汤联合五子散封包疗法治疗效果显著,能够明显降低中医证候积分和炎症因子水平,改善髋关节相关指标。 展开更多

目的探讨关节镜下关节清理术联合胫骨高位截骨术治疗膝关节骨性关节炎(KOA)的疗效。方法选取2024年2-12月河北省衡水市第四人民医院收治的226例KOA患者为研究对象,按照随机数字表法分为对照组和治疗组,各113例。2组均先给予常规治疗,对... 目的探讨关节镜下关节清理术联合胫骨高位截骨术治疗膝关节骨性关节炎(KOA)的疗效。方法选取2024年2-12月河北省衡水市第四人民医院收治的226例KOA患者为研究对象,按照随机数字表法分为对照组和治疗组,各113例。2组均先给予常规治疗,对照组接受胫骨高位截骨术治疗,治疗组除接受关节镜下关节清理术治疗,还进行胫骨高位截骨术治疗。比较2组的治疗疗效、血清基质金属蛋白酶13(MMP-13)、白细胞介素17(IL-17)、膝关节功能(疼痛、僵硬评分)、一氧化氮(NO)含量、生活质量[采用SF-36量表对患者术前及术后12周生理机能(PF)、躯体疼痛(BP)、社会功能(SF)进行评估]情况。结果治疗前,2组血清MMP-13、IL-17水平,疼痛评分、僵硬评分、NO水平,以及PF、BP、SF评分比较,差异均无统计学意义(P>0.05)。治疗后,治疗组总有效率明显高于对照组,差异有统计学意义(χ^(2)=4.294,P<0.05)。治疗后,治疗组血清MMP-13、IL-17水平及疼痛评分、僵硬评分、NO水平均低于对照组,差异均有统计学意义(P<0.05)。治疗后,2组PF、BP、SF评分均升高,且治疗组明显高于对照组,差异均有统计学意义(P<0.05)。结论关节镜下关节清理术联合胫骨高位截骨术治疗KOA,可以有效减轻炎症反应,缓解疼痛,改善膝关节功能,提高患者生活质量。 展开更多
