Assessing intersectional gender analysis in Nepal’s health management information system:a case study on tuberculosis for inclusive health systems

Background Tuberculosis(TB)remains a major public health problem in Nepal,high in settings marked by prevalent gender and social inequities.Various social stratifiers intersect,either privileging or oppressing individuals based on their characteristics and contexts,thereby increasing risks,vulnerabilities and marganilisation associated with TB.This study aimed to assess the inclusiveness of gender and other social stratifiers in key health related national policies and the Health Management Information System(HMIS)of National Tuberculosis Programme(NTP)by conducting an intersectional analysis of TB cases recorded via HMIS.Methods A desk review of key policies and the NTP’s HMIS was conducted.Retrospective intersectional analysis utilized two secondary data sources:annual NTP report(2017-2021)and records of 628 TB cases via HMIS 6.5 from two TB centres(2017/18-2018/19).Chi-square test and multi-variate analysis was used to assess the association between social stratifers and types of TB,registration category and treatment outcome.Results Gender,social inclusion and concept of intersectionality are incorporated into various health policies and strategies but lack effective implementation.NTP has initiated the collection of age,sex,ethnicity and location data since 2014/15 through the HMIS.However,only age and sex disaggregated data are routinely reported,leaving recorded social stratifiers of TB patients static without analysis and dissemination.Furthermore,findings from the intersectional analysis using TB secondary data,showed that male more than 25 years exhibited higher odds[adjusted odds ratio(a OR)=4.95,95%confidence interval(CI):1.60-19.06,P=0.01]of successful outcome compared to male TB patients less than 25 years.Similarly,sex was significantly associated with types of TB(P<0.05)whereas both age(P<0.05)and sex(P<0.05)were significantly associated with patient registration category(old/new cases).Conclusions The results highlight inadequacy in the availability of social stratifiers in the routine HMIS.This limitation hampers the NTP’s ability to conduct intersectional analyses,crucial for unveiling the roles of other social determinants of TB.Such limitation underscores the need for more disaggregated data in routine NTP to better inform policies and plans contributing to the development of a more responsive and equitable TB programme and effectively addressing disparities. 展开更多

The hardness of nanotwinned diamond(nt-diamond)is reported to be more than twice that of the natural diamond,thanks to the fine spaces between twin boundaries(TBs),which block dislocation propagation during deformation.In this work,we explore the effects of additional TBs in nt-diamond using molecular dynamics(MD)calculations and introduce a novel intersectional nanotwinned diamond(int-diamond)template for future laboratory synthesis.The hardness of this int-diamond is predicted by first analyzing individual dislocation slip modes in twinned grains and then calculating the bulk properties based on the Sachs model.Here we show that the hardness of the int-diamond is much higher than that of nt-diamond.The hardening mechanism of intdiamond is attributed to the increased critical resolved shear stress due to the presence of intersectional TBs in nt-diamond;this result is further verified by MD simulations.This work provides a new strategy for designing new super-hard materials in experiments. 展开更多

The cultivated peanut(Arachis hypogaea L.)is a significant edible oilseed crop.Although substantial progress has been made in breeding new peanut varieties through conventional methods,the narrow gene base of the cultivated peanut(stemming from six evolutionary bottlenecks)poses growing challenges for developing breakthrough varieties via intraspecific hybridization alone.Consequently,extensive efforts have been directed towards the utilization of incompatible wild Arachis species.In this study,we present the first genetic diversity assessment of 30 progenies derived from intersectional crosses between the high-oleic peanut cultivar Huayu 665 and two wild species,A.paraguariensis and A.appresipilla,alongside 32 cultivated materials and 29 wild accessions,using transposon element markers.The hybrids demonstrated greater genetic diversity than the cultivars,expanding the gene base for peanut breeding.Hybrid progenies with enhanced insect pest resistance and elevated levels of both oleic acid and oil/protein content were tentatively obtained.This work lays the groundwork for the future development of high-yielding,high-quality peanut varieties through the utilization of crop wild relatives. 展开更多

This psychobiographical study on Bessie Head,a bi-racial writer from the South African apartheid period,explored early experiences that defined her identity,personality development and prominence in post-colonial African literature.For the case conceptual framing,we integrate Donald W.Winnicott’s object relations theory with intersectional feminism to explore her identity by the complex interactions between her psychological experiences and socio-political contexts.Our mixed-methodological approach that combines du Plessis’(2017)structured framework and Knight’s(2019)Phenomenological-hermeneutic Life-narrative Analysis.The analysis revealed three central findings.First,Bessie’s identity development was shaped by early abandonment and racial liminality,resulting in a fragmented sense of self.Second,her emotional and psychological instability was compounded by intersecting experiences of racism,sexism,and exile.Third,her writing and relationships functioned as coping mechanisms and transitional phenomena,offering potential spaces of psychological resilience.From this psychobiographical study framed on intersectional research,the evidence suggests that marginalized identities and a commitment to social justice are revealed by exploring the relationships between individual psychology and systemic influences. 展开更多

Laser-directed energy deposition(L-DED)is an advanced additive manufacturing technology primarily adopted in metal three-dimensional printing systems.The L-DED process is characterized by various defects,thus necessitating the extensive use of in-situ monitoring to enable real-time adjustments of process parameters by detecting molten-pool features.To address the challenge of accurately extracting the molten-pool morphology from an undetached spatter,an innovative monitoring method based on the U-Net(U-shaped network)is proposed herein.A lightweight architecture accelerates the processing speed,whereas an enhanced loss function incorporating weight maps augments the segmentation precision.The model performance is evaluated by comparing its segmentation accuracy and processing speed with those of the conventional U-Net,using the mean intersection over union(MIoU)as the segmentation metric.The improved model demonstrates superior segmentation accuracy at the interface between the molten pool and spatter,with a peak MIoU of 0.9798 achieved on the test set.Furthermore,this model processes each image in an extremely short time of 17.9 ms.Using this segmentation algorithm,the error in extracting the molten-pool width from single-track experiments is within 0.1 mm.The proposed method for monitoring the molten-pool morphology is suitable for deployment in online monitoring systems,thus providing a foundation for subsequent process-parameter regulation. 展开更多

In this paper, the problem of cubature Kalman fusion filtering(CKFF) is addressed for multi-sensor systems under amplify-and-forward(AaF) relays. For the purpose of facilitating data transmission, AaF relays are utilized to regulate signal communication between sensors and filters. Here, the randomly varying channel parameters are represented by a set of stochastic variables whose occurring probabilities are permitted to exhibit bounded uncertainty. Employing the spherical-radial cubature principle, a local filter under AaF relays is initially constructed. This construction ensures and minimizes an upper bound of the filtering error covariance by designing an appropriate filter gain. Subsequently, the local filters are fused through the application of the covariance intersection fusion rule. Furthermore, the uniform boundedness of the filtering error covariance's upper bound is investigated through establishing certain sufficient conditions. The effectiveness of the proposed CKFF scheme is ultimately validated via a simulation experiment concentrating on a three-phase induction machine. 展开更多

Ultrasound-Guided Hydro Dissection: A Novel Treatment Approach for Intersection Syndrome

Activities such as dexterous sports, prolonged use of personal handheld devices, and extensive desktop keyboard usage are common contributors to wrist pain. Intersection syndrome, a form of inflammatory tenosynovitis, occurs at the intersection of the first and second dorsal compartments of the wrist. The first dorsal compartment is comprised of the tendons of abductor pollicis longus and extensor pollicis brevis, while the second dorsal compartment contains the tendons of extensor carpi radialis longus and extensor carpi radialis brevis. Intersection syndrome is diagnosed by pain localized to the dorsoradial forearm, approximately five cm proximal to the wrist joint, which worsens with resisted wrist and thumb extension. To date, the use of hydro dissection with 5% dextrose under ultrasound guidance as a treatment for Intersection syndrome has not been reported. This case report presents the first report on ultrasound-guided hydro dissection as a therapeutic approach for intersection syndrome. Methods: A case report, with informed consent, involving a 32-year-old male athlete. The patient, a hurling player, presented with chronic right wrist pain diagnosed as intersection syndrome. The condition significantly affected his work, sporting activities, and daily living activities. Previous conservative management and physiotherapy had failed to alleviate his symptoms. To confirm the diagnosis, relevant imaging was performed, supplemented by dynamic ultrasound assessment. The procedure was performed aseptically. Continuous ultrasound guidance was employed to ensure accurate needle placement. Once the needle tip position was confirmed, an initial injection of 5 mL of 0.25% chirocaine was administered. 10 mL of 5% dextrose was injected under ultrasound guidance for hydro dissection, with good visualization of the solution’s distribution. Conclusion: Ultrasound-guided hydro dissection has not previously been documented as a treatment option for intersection syndrome. In this case, it proved to be an effective pain-relieving therapy with sustained effect at three-month clinical follow-up. Further studies are required. 展开更多

Mechanism of proppant transport and deposition in rough intersecting fractures after offshore fracturing

To accurately analyze proppant transport in rough intersecting fractures and elucidate the interaction mechanisms among liquid,particles,and rough walls,this study reconstructed a numerical model of fractures in inhomogeneous reservoirs with varying brittleness index(BI).Various auto-correlation Gaussian rough fracture models were created using Matlab to assess roughness through the fractal dimension method.This research innovatively combined Boolean operations to establish three-dimensional rough fracture models,incorporating(Computational Fluid Dynamics)CFD-DEM(Discrete Element Method)with a bidirectional method for cosimulation.The proppant transport in fractures was categorized into three zones based on the difference in the turbulent kinetic energy.Artificially induced fracture roughness increases fluid retention and turbulence,causing plugging effects and limiting proppant flow into branch fractures.Additionally,compared with the superior deposition and significant support effects of the spherical proppant,the low-sphericity proppant traveled farther under fracturing fluid,inducing more pronounced plugging near curved fracture intersections;the variation in fracture intersection angles primarily impacted the wall shear stress within the flow field,indicating smaller angles led to higher shear energy at the intersection.Compared with the intersection angle of 30°,the height and area deposited in the 90 branch fracture increased by 52.25%and 65.33%,respectively:notably,injecting proppant from smaller to larger particles(S:M:L)and a low velocity effectively ensured fracture conductivity near the wellbore at joint roughness coefficient(JRC)≥46 while achieving satis-factory placement in the branch fracture,making it a recommended approach. 展开更多

In the next few years traffic will happen most of the time.This was triggered by the growing rate of vehicles against the road capacity which is not balance.All the time the congestion in the city of Semarang has occured at peak hours.Congestion also occured in between Teuku Umar and Setia Budi road Jatingaleh because of a plot intersection(Kesatrian intersection,PLN intersection and Jatingaleh intersection)with the Toll Road.Jatingaleh is located in the southern city of Semarang which is a central meeting point between the upper and lower Semarang where the vehicle flows in through a combination of local current and regional traffic,and the flow of vehicles coming in and out from highway.The main cause of the problems that occurred in the area of Jatingaleh is due to the numbers of vehicles movement that occurs at the intersections.With the above issues,it is necessary to analyse the existing conditions and look into some solutions.Before carrying out an analysis a field surveys at peak hours for example morning(06:00 to 08:00 am)and for the afternoon(04:00 to 06:00 pm)should be conducted,then the number of vehicles is counted manually with“short-breakcounting”according to types of vehicles.From the analysis we found that the degree of saturation(DS)is 1.61 between Teuku Umar and Setia Budi road during the morning peak hours and 1.56 during the afternoon peak hours.This means that the capacity of the existing road is no longer able to accommodate the traffic flow.One of the solutions for the congestion that occurs at the intersection of Jatingaleh is to apply the efficiency of the intersection that is not in a plot with a Fly over,Underpass and the combination of Fly Over-Underpass.Base on the flow reduction calculation with 3 comparative modeling it shows that the Fly Over is the most technically efficient to be applied in this research. 展开更多

Currently,the design of advanced compressor blades has reached the full ThreeDimensional(3D)modeling stage.When analyzing the reasons for the failure of popular corner stall prediction criteria for axial compressors to predict the corner flow state in modern compressor3D blades with end-bend and composite bend-sweep characteristics,it is believed that,in addition to the dihedral angle factor in the corner,the variation of the dihedral angle along the flow path is an important factor that has not been considered to date.In light of this,this study first uses the characteristic effects of the diffuser on the deceleration and pressure increase in airflow to design a series of physical models of varying dihedral angle diffusers that are equivalent to compressors.Based on these models,a quantization parameter is established to characterize the development speed of the intersection of boundary layers at the corner under varying dihedral angle and adverse pressure gradient conditions.After combining this with the effects of secondary flow,a Modified diffusion factor DJ(MDJ)is developed to describe the development of corner flow from the leading edge of the blade to its trailing edge under varying dihedral angle conditions.Finally,based on a compressor cascade database,an improved criterion for predicting corner stall in axial compressors using the MDJ diffusion factor is proposed.The validation results,based on extensive experimental data of compressor blades,reveal that this improved criterion can significantly enhance the accuracy of corner stall predictions in the 3D blades of modern compressors compared to currently used prediction criteria,by taking into account the effects of variations in the dihedral angle. 展开更多

The intersection is a widely used traffic line structure from the shallow tunnel to the deep roadway,and determining the subsidence hidden danger area of the roof is the key to its stability control.However,applying traditional maximum equivalent span beam(MESB)theory to determine deformation range,peak point,and angle influence poses a challenge.Considering the overall structure of the intersection roof,the maximum equivalent triangular plate(METP)theory is proposed,and its geometric parameter calculation formula and deflection calculation formula are obtained.The application of the two theories in 18 models with different intersection angles,roadway types,and surrounding rock lithology is verified by numerical analysis.The results show that:1)The METP structure of the intersection roof established by the simulation results of each model successfully determined the location of the roof’s high displacement zone;2)The area comparison method of the METP theory can be reasonably explained:①The roof subsidence of the intersection decreases with the increase of the intersection angle;②The roof subsidence at the intersection of different roadway types has a rectangular type>arch type>circular type;③The roof subsidence of the intersection with weak surrounding rock is significantly larger than that of the intersection with hard surrounding rock.According to the application results of the two theories,the four advantages of the METP theory are compared and clarified in the basic assumptions,mechanical models,main viewpoints,and mechanism analysis.The large deformation inducement of the intersection roof is then explored.The J 2 peak area of the roof drives the large deformation of the area,the peak point of which is consistent with the center of gravity position of the METP.Furthermore,the change in the range of this peak is consistent with the change law of the METP’s area.Hence,this theory clarifies the large deformation area of the intersection roof,which provides a clear guiding basis for its initial support design,mid-term monitoring,and late local reinforcement. 展开更多

Light-induced conical intersections(LICIs)present a distinctive mechanism for nonadiabatic coupling,thereby facilitating ultrafast chemical reactions,including the indirect photodissociation of diatomic molecules.In contrast to static conical intersections,LICIs are dynamically tunable,providing a pathway for precise control of molecular dissociation.In this study,we employ the time-dependent quantum wave packet method to investigate the dissociation dynamics of the OH molecule,focusing on its ground state X^(2)Πand repulsive state 1^(2)Σ~-.By varying laser field parameters(intensity,full width at half maximum(FWHM),and wavelength),we elucidate how nonadiabatic coupling governs selective dissociation channel control.Our findings reveal that the choice of initial vibrational states and the tailoring of laser conditions significantly influence dissociation pathways,providing theoretical insights into manipulating molecular dynamics via LICIs.These results provide a foundation for future experimental studies and the development of advanced molecular control techniques. 展开更多

A new two-state diabatic potential energy matrix(DPEM)for H3 has been constructed,based on the fun-damental invariant neural network(FI-NN)diabatization method pro-posed in our previous work[Phys.Chem.Chem.Phys.21,15040(2019)].In that initial effort,a two-state DPEM was constructed only with a 10 eV energy threshold.The current work aims to expand the en-ergy range and improve the accura-cy of DPEM.This is achieved by the utilization of full configuration inter-action(FCI)with aug-cc-pVnZ ba-sis sets and complete basis set(CBS)extrapolation.The original dataset is augmented with additional points with higher adiabatic energies,which give rise to a total of 10985 data points.The DPEM constructed in this work now enables accurate representation of adiabatic energies up to 18 eV.Quantum dynamic calculations based on this DPEM are nearly identical to those obtained from benchmark surfaces,which makes it the most accurate DPEM for the H3 system to date,therefore facilitating detailed exploration of reaction mechanisms at higher collision energies. 展开更多
