In Kenya, as the 2030 SDG deadline approaches, the gap in maternal, neonatal, and child health services remains significant. Addressing these challenges is critical to improving Maternal, Neonatal, and Child Health (MNCH) outcomes. Objective: This study explores how integration of digital health innovations into the MNCH chain of service delivery affects the quality of MNCH care within the selected PHC settings in Kajiado, Kisii and Migori Counties in Kenya. Methodology: This Quasi-experimental study was conducted 1-year post-intervention targeting a total of 482 pregnant women from intervention and control sites in Kisii, Kajiado and Migori Counties, Kenya. Data was analysed using Chi-Square test comparing frequencies between intervention and control groups when both variables are categorical. Results: Pre-intervention data revealed an increase in first ANC coverage within first trimester, from 167 to 278 post-intervention (p Linda mama social health insurance registrations increased from 1008 to 1135. At the intervention sites, 938 pregnant women got screened by midwives using portable mobile Obstetric Point-of-Care Ultrasound (OPOCUS) technology compared to the 27 cases that accessed ultrasound services in the noncontiguous control sites. The pilot sites midwives earned themselves an incentive income totaling Ksh 400,000 while the Community Health Promoters (CHPs) who created demand for OPOCUS earned an incentive income totaling Ksh 327,195 from their IGAs that were project supported. There was a significant increase in mobile health application usage and e-resources access for health information in the intervention group (p services and improved adherence to referrals. Conclusion: The success of digital health interventions in improving health-seeking behaviour, knowledge, and service uptake highlights the potential of such innovations to strengthen health systems and achieve universal health coverage. A large number of controlled studiesand meta-analyses have shown that laparoscopic colorectal surgery is associated with the same benefits than other minimally invasive procedures, including lesser pain, earlier recovery of bowel transit and shorter hospital stay. On the other hand, despite initial concerns about oncological safety, well-designed prospective randomized multicentre trials have demonstrated that oncological outcomes of laparoscopy and open surgery are similar. Although the use of laparoscopy in colorectal surgery has increased in recent years, the percentages of patients treated with surgery using minimally invasive techniques are still reduced and there are also substantial differences among centres. It has been argued that the limiting factor for the use of laparoscopic procedures is the number of surgeons with adequate skills to perform a laparoscopic colectomy rather than the tumour of patients' characteristics. In this regard, future efforts to increase the use of laparoscopic techniques in colorectal surgery will necessarily require more efforts in teaching surgeons. We here present a review of recent controversies of the use of laparoscopy in colorectal surgery, such as in rectal cancer operations, the possibility of reproducing complete mesocolon excision, and the benefits of intracorporeal anastomosis after right hemicolectomy. We also describe the results of latest innovations such as single incision laparoscopic surgery, robotic surgery and natural orifice transluminal endoscopic surgery for colon and rectal diseases.展开更多

These number...Up to a quarter of polyps and adenomas are missed during colonoscopy due to poor visualization behind folds and the inner curves of flexures, and the presence of flat lesions that are difficult to detect. These numbers may however be conservative because they mainly come from back-to-back studies performed with standard colonoscopes, which are unable to visualize the entire mucosal surface. In the past several years, new endoscopic techniques have been introduced to improve the detection of polyps and adenomas. The introduction of high definition colonoscopes and visual image enhancement technologies have been suggested to lead to better recognition of flat and small lesions, but the absolute increase in diagnostic yield seems limited. Cap assisted colonoscopy and water-exchange colonoscopy are methods to facilitate cecal intubation and increase patients comfort, but show only a marginal or no benefit on polyp and adenoma detection. Retroflexion is routinely used in the rectum for the inspection of the dentate line, but withdrawal in retroflexion in the colon is in general not recommended due to the risk of perforation. In contrast, colonoscopy with the Third-Eye Retroscope<sup>®</sup> may result in considerable lower miss rates compared to standard colonoscopy, but this technique is not practical in case of polypectomy and is more time consuming. The recently introduced Full Spectrum Endoscopy™ colonoscopes maintains the technical capabilities of standard colonoscopes and provides a much wider view of 330 degrees compared to the 170 degrees with standard colonoscopes. Remarkable lower adenoma miss rates with this new technique were recently demonstrated in the first randomized study. Nonetheless, more studies are required to determine the exact additional diagnostic yield in clinical practice. Optimizing the efficacy of colorectal cancer screening and surveillance requires high definition colonoscopes with improved virtual chromoendoscopy technology that visualize the whole colon mucosa while maintaining optimal washing, suction and therapeutic capabilities, and keeping the procedural time as low and patient discomfort as optimal as possible.展开更多

文摘Background:Head and neck cancers(HNC)account for a significant global health burden,with increasing incidence rates and complex treatment requirements.Traditional diagnostic and therapeutic approaches,while effective,often result in substantial morbidity and limitations in personalized care.This review provides a comprehensive overview of the latest innovations in diagnostics and therapeutic strategies for HNC from 2015 to 2024.Methods:A review of literature focused on pe-reviewed journals,clinical trial databases,and oncology conference proceedings.Key areas include molecular diagnostics,imaging technologies,minimally invasive surgeries,and innovative therapeutic strategies.Results:Technologies like liquid biopsy next-generation sequencing(NGS)have greatly improved diagnostic accuracy and personalization in HNC care.These advancements have improved survival rates and enhanced patients’quality of life.Personalized therapeutic approaches,including immune checkpoint inhibitors,precision radiation therapy,and surgery,have led to enhanced treatment efficacy while reducing side effects.The integration of AI and machine learning into diagnostics and treatment planning shows promise in optimizing clinical decision-making and predicting treatment outcomes.Conclusion:The current innovations in diagnostics and therapeutics are reshaping the management of head and neck cancer,offering more tailored and effective approaches to care.Overall,the continuous integration of these innovations in clinical practice is reshaping HNC treatment and improving patient outcomes and survival rates.Future research should focus on further refining these technologies,addressing challenges related to accessibility,and exploring their long-term clinical benefits in diverse patient populations.
文摘This editorial critically evaluated the recent study by AlMousa et al,which examined the impact of the Arabic version of the American Academy of Or-thopedic Surgeons Foot and Ankle Outcomes Questionnaire(AAOS-FAOQ)on postoperative quality of life and recovery in Arabic-speaking patients with traumatic foot and ankle injuries.In the context of systemic linguistic exclusion in orthopedic research—where English-language journals dominated most publi-cations and non-English-speaking populations faced dual barriers of trial under-representation and semantic distortions(e.g.,mistranslations of terms like"joint instability"in Arabic)—AlMousa et al's work highlighted the transformative po-tential of culturally adapted methodologies.Their rigorous four-stage adaptation framework validated the Arabic AAOS-FAOQ as a reliable tool,enhancing ecological validity and reducing bias in patient-reported outcomes.However,limitations such as regional specificity(Gulf-centric sampling)and short follow-up periods(4 months)underscored broader challenges in non-English research:Redundant studies,prolonged hospital stays for limited English proficiency patients,and underrepresentation of certain ethnic groups in trials.To dismantle linguistic hegemony,we proposed semantic reconstruction(e.g.,integrating culturally specific indicators like"prayer posture"),dialect-aware neural tran-slation,and World Health Organization led terminology standardization.In line with these proposed solutions,AlMousa et al’s study exemplified how language-sensitive adaptations could bridge equity gaps,while future efforts would need to balance cultural specificity with cross-study comparability through AI-driven multilingual databases and policy mandates for cultural adaptation roadmaps.
文摘As a response to the complexity of modern health care,nursing education is undergoing significant transformation.This paper aims to explore changing dynamics in nursing education,highlighting trends,challenges,and the potential of Design-Based Research(DBR)as a methodological approach for curricular enhancement.This study employed a qualitative synthesis methodology was utilised,employing a systematic literature analysis of peer-reviewed articles published from 2019 to 2023.Relevant studies were searched in primary databases such as PubMed,Scopus,and Google Scholar,using specific keywords pertaining to nursing education,innovative pedagogical approaches,and DBR.The review revealed significant trends in nursing education including empathy and resilience,cultural competence,technology,interprofessional education,as well as competency-based approaches.Major challenges included teacher shortages,limited clinical placements,technological adaptability,and student mental health issues.DBR became an approach to address these challenges and offers an iterative model for curriculum development and educational innovation.DBR is a systematic and collaborative way to improve nursing education.Despite the barriers to implementation,DBR can lead to a more applicable curriculum based on real-world healthcare needs and create a nimbler educational process to prepare nursing graduates for working in complex healthcare environments.

Technological innovation offers a transformative pathway for climate change adaptation and peacebuilding, with emphasis on a holistic approach to managing resource conflicts and environmental challenges. This paper explores the synergies between emerging technologies and strategic framework to mitigate climate-induced tensions and foster resilience. It focuses on the application of renewable energy systems to reduce dependence on contested resources, blockchain technology to ensure transparency in climate finance, equitable resource allocation and Artificial Intelligence (AI) to enhance early warning systems for climate-related disaster and conflicts. Additionally, technologies such as precision agriculture and remote sensing empower communities to optimize resource use, adapt to shifting environmental conditions, and reduce competition over scares resources. These innovations with inclusive governance and local capacity-building are very primordial. Ultimately, the convergence of technology, policy, and local participation offers a scalable and replicable model for addressing the dual challenges of environmental degradation and instability, thereby paving the way for a more sustainable and peaceful future.

文摘The micro-public opinion environment,characterized by information fragmentation,instantaneous dissemination,emotional contagion,and diverse values,has profoundly transformed the information acquisition patterns and cognitive formation processes of college students.It also poses new challenges and opportunities for innovation in ideological and political education(hereinafter referred to as“IPE”)for college students.Starting from the contemporary value of IPE for college students,this paper analyzes the logical connection between the micro-public opinion environment and IPE,thoroughly examines the core challenges faced by IPE in this context,and proposes specific innovative paths from four perspectives:content,methods,subjects,and mechanisms.The aim is to provide insights for enhancing the pertinence and effectiveness of IPE for college students in the micro-public opinion environment.

文摘Nine decades ago in 1933,Evarts A.Graham performed the first successful pneumonectomy in a patient with primary pulmonary squamous cell carcinoma.The patient survived for another 30 years,which drew the curtain on the surgical treatment of lung cancer.Surgical resection continues to be the cornerstone of multidisciplinary treatment for patients with early-stage non-small cell lung cancer and a proportion of those with locally advanced disease.Moreover,recent years have seen developments in automatic control,biomechanics,robotics,image transmission,artificial intelligence,three-dimensional reconstruction and printing,biological pharmacy,and molecular biology.Therefore,there is now an increasing focus on how to integrate these technologies into lung cancer surgery to improve quality of life,resect the tumor accurately,expand the population that is suitable for surgical management,predict disease recurrence with better accuracy,and ultimately achieve long-term survival.This article systematically reviews the innovative achievements that may be detrimental to current clinical practice and in future clinical trials,and simultaneously provides a brief overview of the polyvagal perspective in this field.
文摘Background: While global efforts have led to a decline in maternal and neonatal mortality, Sub-Saharan Africa continues to face disproportionately high rates, remaining far above the Sustainable Development Goal (SDG) targets. In Kenya, as the 2030 SDG deadline approaches, the gap in maternal, neonatal, and child health services remains significant. Addressing these challenges is critical to improving Maternal, Neonatal, and Child Health (MNCH) outcomes. Objective: This study explores how integration of digital health innovations into the MNCH chain of service delivery affects the quality of MNCH care within the selected PHC settings in Kajiado, Kisii and Migori Counties in Kenya. Methodology: This Quasi-experimental study was conducted 1-year post-intervention targeting a total of 482 pregnant women from intervention and control sites in Kisii, Kajiado and Migori Counties, Kenya. Data was analysed using Chi-Square test comparing frequencies between intervention and control groups when both variables are categorical. Results: Pre-intervention data revealed an increase in first ANC coverage within first trimester, from 167 to 278 post-intervention (p Linda mama social health insurance registrations increased from 1008 to 1135. At the intervention sites, 938 pregnant women got screened by midwives using portable mobile Obstetric Point-of-Care Ultrasound (OPOCUS) technology compared to the 27 cases that accessed ultrasound services in the noncontiguous control sites. The pilot sites midwives earned themselves an incentive income totaling Ksh 400,000 while the Community Health Promoters (CHPs) who created demand for OPOCUS earned an incentive income totaling Ksh 327,195 from their IGAs that were project supported. There was a significant increase in mobile health application usage and e-resources access for health information in the intervention group (p services and improved adherence to referrals. Conclusion: The success of digital health interventions in improving health-seeking behaviour, knowledge, and service uptake highlights the potential of such innovations to strengthen health systems and achieve universal health coverage. We recommend the intervention for a scale-up in other PHC settings in Kenya.

文摘The introduction of laparoscopy is an example of surgical innovation with a rapid implementation in many areas of surgery. A large number of controlled studiesand meta-analyses have shown that laparoscopic colorectal surgery is associated with the same benefits than other minimally invasive procedures, including lesser pain, earlier recovery of bowel transit and shorter hospital stay. On the other hand, despite initial concerns about oncological safety, well-designed prospective randomized multicentre trials have demonstrated that oncological outcomes of laparoscopy and open surgery are similar. Although the use of laparoscopy in colorectal surgery has increased in recent years, the percentages of patients treated with surgery using minimally invasive techniques are still reduced and there are also substantial differences among centres. It has been argued that the limiting factor for the use of laparoscopic procedures is the number of surgeons with adequate skills to perform a laparoscopic colectomy rather than the tumour of patients' characteristics. In this regard, future efforts to increase the use of laparoscopic techniques in colorectal surgery will necessarily require more efforts in teaching surgeons. We here present a review of recent controversies of the use of laparoscopy in colorectal surgery, such as in rectal cancer operations, the possibility of reproducing complete mesocolon excision, and the benefits of intracorporeal anastomosis after right hemicolectomy. We also describe the results of latest innovations such as single incision laparoscopic surgery, robotic surgery and natural orifice transluminal endoscopic surgery for colon and rectal diseases.
文摘Up to a quarter of polyps and adenomas are missed during colonoscopy due to poor visualization behind folds and the inner curves of flexures, and the presence of flat lesions that are difficult to detect. These numbers may however be conservative because they mainly come from back-to-back studies performed with standard colonoscopes, which are unable to visualize the entire mucosal surface. In the past several years, new endoscopic techniques have been introduced to improve the detection of polyps and adenomas. The introduction of high definition colonoscopes and visual image enhancement technologies have been suggested to lead to better recognition of flat and small lesions, but the absolute increase in diagnostic yield seems limited. Cap assisted colonoscopy and water-exchange colonoscopy are methods to facilitate cecal intubation and increase patients comfort, but show only a marginal or no benefit on polyp and adenoma detection. Retroflexion is routinely used in the rectum for the inspection of the dentate line, but withdrawal in retroflexion in the colon is in general not recommended due to the risk of perforation. In contrast, colonoscopy with the Third-Eye Retroscope<sup>®</sup> may result in considerable lower miss rates compared to standard colonoscopy, but this technique is not practical in case of polypectomy and is more time consuming. The recently introduced Full Spectrum Endoscopy™ colonoscopes maintains the technical capabilities of standard colonoscopes and provides a much wider view of 330 degrees compared to the 170 degrees with standard colonoscopes. Remarkable lower adenoma miss rates with this new technique were recently demonstrated in the first randomized study. Nonetheless, more studies are required to determine the exact additional diagnostic yield in clinical practice. Optimizing the efficacy of colorectal cancer screening and surveillance requires high definition colonoscopes with improved virtual chromoendoscopy technology that visualize the whole colon mucosa while maintaining optimal washing, suction and therapeutic capabilities, and keeping the procedural time as low and patient discomfort as optimal as possible.
