BACKGROUND Pelvic fractures are often associated with significant morbidity,including injuries to the urinary tract.Understanding the incidence and risk factors for urinary tract injury in these patients is crucial fo...BACKGROUND Pelvic fractures are often associated with significant morbidity,including injuries to the urinary tract.Understanding the incidence and risk factors for urinary tract injury in these patients is crucial for prompt diagnosis and management.This meta-analysis aims to synthesize existing evidence to determine the overall incidence and identify specific risk factors associated with urinary tract injuries in patients with pelvic fractures.AIM To determine the incidence and risk factors for urinary tract injuries in patients with pelvic fractures.METHODS A systematic search of PubMed,EMBASE,Scopus,and the Cochrane Library was conducted without date restrictions.Studies examining the incidence and risk factors of urinary tract injuries in patients with pelvic fractures were included.Data extracted included demographics,injury mechanism,pelvic fracture type,urinary tract injury incidence,mortality,and discharge disposition.Review Manager 5.4 was used for data analysis.RESULTS Ten studies comprising 22700 patients were included.The pooled incidence of urinary tract injury associated with pelvic fracture was 6.88%(95%CI:6.20%-7.55%).Vehicle,motorcycle,and pedestrian accidents were identified as risk factors for urinary tract injury,with relative risks(RR)of 1.08(95%CI:1.06-1.11),1.89(95%CI:1.78-2.00),and 1.53(95%CI:1.20-1.95),respectively.Pubic fracture and pelvic ring disruption were significantly associated with urinary tract injury[odds ratio(OR)1.94,95%CI:1.09-3.44 and OR 5.53,95%CI:4.67-6.54,respectively)].Patients without urinary tract injury were more likely to be discharged home(RR 0.79,95%CI:0.67-0.92).Mortality was higher in patients with urinary tract injury(OR 1.92,95%CI:1.77-2.09).CONCLUSION Urinary tract injury occurs in nearly 7%of patients with pelvic fractures.Motorcycle accidents,pubic fractures,and pelvic ring disruptions are significant risk factors.Urinary tract injury following pelvic fracture is associated with increased mortality.展开更多

BACKGROUND Postoperative delirium(POD)is a concerning complication of organ transplantation.With organ transplantation offering hope to patients with end-stage organ disease,understanding the incidence and risk factor...BACKGROUND Postoperative delirium(POD)is a concerning complication of organ transplantation.With organ transplantation offering hope to patients with end-stage organ disease,understanding the incidence and risk factors of POD is crucial,as it can significantly affect patients’prognosis and healthcare costs.AIM To systematically evaluate the incidence and risk factors of POD following organ transplantation to facilitate clinical prevention and optimize patient management and prognosis.METHODS Multiple databases such as PubMed and their reference lists were comprehensively searched using a combination of keywords related to organ transplantation and POD.Relevant observational studies on patients who had undergone solid organ transplantation and randomized controlled trials containing relevant analyses were included.Duplicated,data-deficient,non-English,and non-original data studies were excluded.Data were extracted independently by two researchers and then cross-checked.The Newcastle-Ottawa scale was used to evaluate the quality of the included studies.RevMan 5.3 was employed for data analysis.The pooled incidence of POD was calculated according to the data type,and the fixed or random effect model was employed to analyze risk factors based on heterogeneity.Subsequently,sensitivity analysis and publication bias assessments were performed.RESULTS A total of 39 relevant literatures were included.The overall incidence of POD in the organ transplant group was 20%[95%confidence interval(CI):18%-22%];liver transplant group,22%(95%CI:17%-26%);lung transplant group,34%(95%CI:23%-45%);and kidney transplant group,6%(95%CI:2%-10%).Primary graft dysfunction increased the POD risk,with a pooled odds ratio(OR)(95%CI)of 1.78(1.09-2.91).A history of hepatic encephalopathy increased the POD risk,with a pooled OR(95%CI)of 3.19(2.30-4.43).The higher the Acute Physiology and Chronic Health Evaluation II score,the greater the POD risk,with a pooled OR(95%CI)of 1.52(1.09-2.12).A history of alcohol abuse increased the POD risk,with a pooled OR(95%CI)of 2.84(1.74-4.65).Thus,the higher the model for end-stage liver disease score,the greater the POD risk,with a pooled OR(95%CI)of 2.49(1.14-5.43).POD was more likely to develop in patients with preoperative infections,with a pooled OR(95%CI)of 2.78(1.56-4.97).The use of diuretics increased the POD risk,with a pooled OR(95%CI)of 2.36(1.38-4.04).CONCLUSION In this study,the overall incidence of POD in patients who underwent organ transplantation is 20%.The incidence varies among different types of organ transplantation,and multiple factors can increase the POD risk.展开更多
BACKGROUND Post-hepatectomy portal vein thrombosis(PH-PVT)is a life-threatening complication;however,the available literature on this topic is limited.AIM To examine the incidence,risk factors,and outcomes associated ...BACKGROUND Post-hepatectomy portal vein thrombosis(PH-PVT)is a life-threatening complication;however,the available literature on this topic is limited.AIM To examine the incidence,risk factors,and outcomes associated with PH-PVT.METHODS Medical records of patients who underwent hepatic resection for various diseases between February 2014 and December 2023 at Beijing Tsinghua Changgung Hospital affiliated with Tsinghua University(Beijing,China)were retrospectively reviewed.The patients were divided into a PH-PVT group and a non-PH-PVT group.Univariate and multivariate logistic regression analyses were performed to identify the risk factors for PH-PVT.RESULTS A total of 1064 patients were included in the study cohort,and the incidence and mortality rates of PH-PVT were 3.9%and 35.7%,respectively.The median time from hepatectomy to the diagnosis of PH-PVT was 6 days.Multivariate analysis revealed that hepatectomy combined with pancreaticoduodenectomy(HPD)[odds ratio(OR)=7.627(1.390-41.842),P=0.019],portal vein reconstruction[OR=6.119(2.636-14.203),P<0.001]and a postoperative portal vein angle<100°[OR=2.457(1.131-5.348),P=0.023]were independent risk factors for PH-PVT.Age≥60 years[OR=8.688(1.774-42.539),P=0.008]and portal vein reconstruction[OR=6.182(1.246-30.687),P=0.026]were independent risk factors for mortality in PH-PVT patients.CONCLUSION Portal vein reconstruction,a postoperative portal vein angle<100°and HPD were independent risk factors for PHPVT.Age≥60 years and portal vein reconstruction were independent risk factors for mortality in PH-PVT patients.展开更多

This study aims to determine the prevalence of delirium in critically ill patients diagnosed with SAP, and to identify risk factors associated with delirium in this patient population.METHODS: This was a retrospective, multicenter study, which enrolled adult patients diagnosed with SAP who admitted intensive care unit(ICU) for at least 24 h. Patient assessment was conducted using the Richmond Agitation-Sedation Scale(RASS) and the Confusion Assessment Method for the ICU(CAM-ICU). The cumulative incidence of delirium was determined. Demographic, clinical data, and length of ICU stay were compared between patients with and without delirium. A logistic regression model was employed to identify potential risk factors for delirium. RESuLTS: A total of 1,814 patients were included from seven hospitals in Anhui province, China. Delirium was observed in 25.2% of patients. Logistic regression analysis identified APACHE II scores(odds ratio [OR]=3.37, 95% confidence interval [CI]: 1.09–10.43, P=0.04), physical restraint(OR=11.11, 95%CI: 4.35–28.39, P<0.05), invasive mechanical ventilation(IMV)(OR=2.44, 95%CI: 1.41–4.25, P=0.002), and ICU length of stay ≥ 7 days(OR=3.14, 95%CI: 2.27–4.36, P<0.05) as independent risk factors of delirium.cONcLuSION: The present study revealed a substantial incidence of delirium in critically ill patients with SAP, associated with factors including APACHE II score, IMV, physical restraint, and prolonged ICU stays.展开更多
Objective:Esophageal cancer has made a great contribution to the cancer burden in Jiangsu Province,East China.This study was aimed at reporting esophageal cancer incidence trend in 2009-2019 and its prediction to 2030...Objective:Esophageal cancer has made a great contribution to the cancer burden in Jiangsu Province,East China.This study was aimed at reporting esophageal cancer incidence trend in 2009-2019 and its prediction to 2030.Methods:The burden of esophageal cancer in Jiangsu in 2019 was estimated using 54 cancer registries’data selected from Jiangsu Cancer Registry.Incident cases of 16 cancer registries were applied for the temporal trend from 2009 to 2019.The burden of esophageal cancer by 2030 was projected using the Bayesian age-period-cohort(BAPC)model.Results:About 24,886 new cases of esophageal cancer(17,233 males and 7,653 females)occurred in Jiangsu in 2019.Rural regions of Jiangsu had the highest incidence rate.The age-standardized incidence rate(ASIR,per 100,000 population)of esophageal cancer in Jiangsu decreased from 27.72 per 100,000 in 2009 to 14.18 per 100,000 in 2019.The BAPC model showed that the ASIR would decline from 13.01 per 100,000 in 2020 to 4.88 per 100,000 in 2030.Conclusions:According to the data,esophageal cancer incidence rates were predicted to decline until 2030,yet the disease burden is still significant in Jiangsu.The existing approaches to prevention and control are effective and need to be maintained.展开更多
Background As the population in China rapidly ages,the prevalence of mild cognitive impairment(MCI)is increasing considerably.However,the causes of MCI vary.The continued lack of understanding of the various subtypes ...Background As the population in China rapidly ages,the prevalence of mild cognitive impairment(MCI)is increasing considerably.However,the causes of MCI vary.The continued lack of understanding of the various subtypes of MCI impedes the implementation of effective measures to reduce the risk of advancing to more severe cognitive diseases.Aims To estimate the prevalence and incidence rates of two MCI subtypes—amnestic MCI(aMCI)and vascular cognitive impairment without dementia(VCIND)—and to determine modifiable factors for them among older individuals in a multiregional Chinese cohort.Method This 1-year longitudinal study surveyed a random sample of participants aged≥60 years from a large,community-dwelling cohort in China.Baseline lifestyle data were self-reported,while vascular and comorbid conditions were obtained from medical records and physical examinations.In total,3514 and 2051 individuals completed the baseline and 1-year follow-up assessments,respectively.Logistic and linear regression analyses were used to identify the modifiable factors for MCI subtypes and predictors of cognitive decline,respectively.Results Among our participants,aMCI and VCIND demonstrated prevalence of 14.83%and 2.71%,respectively,and annual incidence(per 1000 person-years)of 69.6 and 10.6,respectively.The risk factor for aMCI was age,whereas its protective factors were high education level,tea consumption and physical activity.Moreover,VCIND risk factors were age,hypertension and depression.The presence of endocrine disease,cerebral trauma or hypertension was associated with a faster decline in cognition over 1 year.Conclusions MCI is a serious health problem in China that will only worsen as the population ages if no widespread interventions are implemented.Preventive strategies that promote brain activity and support healthy lifestyle choices are required.We identified modifiable factors for MCI in older individuals.The easy-to-adopt solutions such as tea consumption and physical activity can aid in preventing MCI.展开更多
Background The Global Cancer Observatory(GLOBOCAN)2022 and the Global Burden of Disease(GBD)2021 are two primary databases for assessing the global cancer burden.This study aimed to compare the incidence and mortality...Background The Global Cancer Observatory(GLOBOCAN)2022 and the Global Burden of Disease(GBD)2021 are two primary databases for assessing the global cancer burden.This study aimed to compare the incidence and mortality rates of tracheal,bronchus,and lung(TBL)cancer reported in these databases and to analyze the observed discrepancies.Methods Age-standardized rates(ASRs),including age-standardized incidence rates and age-standardized mortality rates for TBL cancer,were obtained from GLOBOCAN 2022 and GBD 2021 for the most recent available year.Differences in ASRs at the national level between the two databases were quantified using pairwise differences,calculated as the absolute difference in ASRs divided by the mean of the ASRs from both sources.Correlations between macroeconomic factors and pairwise differences in ASRs were evaluated,and country features correlated with high pairwise differences were assessed.The data sources and methods used in the two databases were also compared.Results Strong correlations were identified between ASRs reported by GLOBOCAN 2022 and GBD 2021;however,significant differences were observed between estimates from the two data sources.African countries commonly exhibited larger pairwise differences in ASRs,whereas European countries demonstrated smaller pairwise differences in ASRs.Additionally,some populous developing countries,including China,South Africa,Brazil and India,showed smaller differences in ASRs.Countries lacking vital registration systems or high-quality population-based cancer registries displayed larger differences in ASRs.Furthermore,differences in ASRs were negatively correlated with macroeconomic factors.The data sources and estimation methods used by the two databases were inconsistent.Conclusions Discrepancies in TBL cancer incidence and mortality were observed between GLOBOCAN 2022 and GBD 2021.While differences in sources and methods partially explain these discrepancies,a country's cancer surveillance maturity and economic status also correlate with the accuracy of the estimates.Estimating the cancer burden in less wealthy countries remains a substantial challenge,necessitating long-term assistance and investment.展开更多
Despite numerous research investigations to understand the influences of various structural parameters,to the authors'knowledge,no research has been the effect of different angles of incidence on stab response and...Despite numerous research investigations to understand the influences of various structural parameters,to the authors'knowledge,no research has been the effect of different angles of incidence on stab response and performance of different types of protective textiles.Three distinct structures of 3D woven textiles and 2D plain weave fabric made with similar high-performance fiber and areal density were designed and manufactured to be tested.Two samples,one composed of a single and the other of 4-panel layers,from each fabric type structure,were prepared,and tested against stabbing at[0○],[22.5○],and[45○]angle of incidence.A new stabbing experimental setup that entertained testing of the specimens at various angles of incidence was engineered and utilized.The stabbing bench is also equipped with magnetic sensors and a UK Home Office Scientific Development Branch(HOSDB)/P1/B sharpness engineered knives to measure the impact velocity and exerted impact energy respectively.A silicon compound was utilized to imprint the Back Face Signature(BFS)on the backing material after every specimen test.Each silicon print was then scanned,digitized,and precisely measured to evaluate the stab response and performance of the specimen based on different performance variables,including Depth of Trauma(DOT),Depth of Penetration(DOP),and Length of Penetration(LOP).Besides,the post-impact surface failure modes of the fabrics were also measured using Image software and analyzed at the microscale level.The results show stab angle of incidence greatly influences the stab response and performance of protective textiles.The outcome of the study could provide not only valuable insights into understanding the stab response and capabilities of protective textiles under different angle of incidence,but also provide valuable information for protective textile manufacturer,armor developer and stab testing and standardizing organizations to consider the angle of incidence while developing,testing,optimizing,and using protective textiles in various applications.展开更多
Background:The National Cancer Center(NCC)of China regularly reports the nationwide statistics on cancer incidence and mortality in China.The International Agency for Research on Cancer(IARC)calculates and publishes t...Background:The National Cancer Center(NCC)of China regularly reports the nationwide statistics on cancer incidence and mortality in China.The International Agency for Research on Cancer(IARC)calculates and publishes the cancer burden of countries around the world every two years.To ensure consistency between the actual surveillance data in China and the data published by IARC,NCC has received approval from the National Health Commission and IARC to simultaneously release the cancer burden data for China in GLOBOCAN 2022.Methods:There were a total of 700 registries reporting high-quality data on cancer incidence and mortality across China in 2018,of which 106 registries with continuous monitoring from 2010 to 2018 were used to establish an age-period-cohort model to simulate the trend of cancer incidence and mortality and to estimate the incidence and mortality in China in 2022.In addition,we analyzed the temporal trends of age-standardized cancer incidence and mortality from 2000 to 2018 using data from 22 continuous cancer registries.Results:It was estimated about 4,824,700 new cancer cases and 2,574,200 new cancer deaths occurred in China in 2022.Cancers of the lung,colon-rectum,thyroid,liver and stomach were the top five cancer types,accounting for 57.42%of new cancer cases.Cancers of the lung,liver,stomach,colon-rectum and esophagus were the five leading causes of cancer deaths,accounting for 67.50%of total cancer deaths.The crude rate and age-standardized incidence rate(ASIR)were 341.75 per 100,000 and 201.61 per 100,000,respectively.The crude mortality rate was 182.34 per 100,000 and the age-standardized mortality rate(ASMR)was 96.47 per 100,000.The ASIR of all cancers combined increased by approximately 1.4%per year during 2000–2018,while the ASMR decreased by approximately 1.3%per year.We observed decreasing trends in ASIR and ASMR for cancers of the esophagus,stomach,and liver,whereas the ASIR increased significantly for cancers of the thyroid,prostate,and cervix.Conclusions:Cancer remains a major public health concern in China,with a cancer profile that reflects the coexistence of developed and developing regions.Sustained implementation of prevention and control measures has resulted in significant reductions in the incidence and mortality rates of certain historically high incidence cancers,such as esophageal,stomach and liver cancers.Adherence to the guidelines of the Healthy China Action Plan and the Cancer Prevention and Control Action Plan,along with continued efforts in comprehensive risk factor control,cancer screening,early diagnosis and treatment,and standardization of diagnostic and therapeutic protocols,are key strategies to effectively mitigate the increasing cancer burden by 2030.展开更多
Objective Brain metastases significantly impact the clinical course of patients with hepatocellular carcinoma(HCC).This study aimed to examine the age-related incidence,demographics,and survival of patients with HCC a...Objective Brain metastases significantly impact the clinical course of patients with hepatocellular carcinoma(HCC).This study aimed to examine the age-related incidence,demographics,and survival of patients with HCC and brain metastases.Methods Data of HCC patients from 2010 to 2015 in the Surveillance,Epidemiology,and End Results(SEER)Registry were screened for the presence of brain metastases.They were stratified by age and ethnicity.Multivariable logistic and Cox regression analyses were used to identify factors associated with brain metastases and those with overall survival(OS)and liver cancer-specific survival(CSS),respectively.Results A total of 141 HCC patients presenting with brain metastases were identified,accounting for 0.35% of all HCC patients and 2.37% of patients with metastatic disease.Among all HCC patients,the incidence rate was the highest among patients aged 30-49 years old(0.47%).Ethnicity was not associated with the presence of brain metastases at the time of HCC diagnosis.However,African-American patients presented with a significantly lower disease-specific survival[median time:1 month;interquartile range(IQR):0-3.0 months].Initial lung or bone metastasis was independently associated with an increased risk of the presence of brain metastases[odds ratio(OR):12.62,95% confidence interval(CI):8.40-18.97]but was not associated with a worse OS or CSS among those with brain metastases.Conclusion This study identified the age-related incidence and risk factors of brain metastases in HCC patients.These results may contribute to the consideration of brain screening among patients with initial metastatic HCC with lung or bone metastases,and influence the counseling of this patient population regarding their prognosis.展开更多

In the ideal case of parallel incident light, the ITF of the FP resonant cavity can be expressed by the Airy function. However, in reality, it is difficult to achieve perfect parallelism with collimated beams. In this article, a theoretical model is established for non-parallel light incidence, which assumes that the non-parallel incident light is a cone-shaped beam, and the cone angle is used to quantify the non-parallelism of the beam. The transmittance function of the FP resonant cavity under non-parallel light incidence is derived. The accuracy of the model is experimentally verified. Based on this model, the effects of divergence angle, tilt angle and FP cavity parameters(reflectivity, cavity length)on the ITF are studied. The reasons for the decrease in peak value, broadening and asymmetry of the interference peak under non-parallel light incidence are explained. It is suggested that a fine balance between the interference peak and the collimation effect of the incident light should be considered in the design and application of FP resonant cavities, especially for tilted applications such as angle-scanned spectroscopy. The research results of this article have certain significance for the design and application of FP resonant cavities.展开更多
Background Major depressive disorders(MDDs)impose substantial burdens on individuals and society;however,further detailed analysis is still needed for its long-term trends.Aims This study aimed to analyse the gender-s...Background Major depressive disorders(MDDs)impose substantial burdens on individuals and society;however,further detailed analysis is still needed for its long-term trends.Aims This study aimed to analyse the gender-specific temporal trends and cohort variations of MDD incidence among Chinese residents over the past three decades.Methods Employing the age-period-cohort-interaction model and leveraging data from the Global Burden of Disease Study 2019,this research identified and analysed incidence trends of MDD among Chinese males and females aged 5-94 years from 1990 to 2019 across three dimensions,encompassing age,period and birth cohort.Results The analysis reveals age-related effects,indicating heightened MDD risk among adolescents and older adults.Specifically,individuals entering the older adulthood at the age of 65-69 significantly increased the risk of MDD by 64.9%.People aged 90-94 years witnesseda 105.4%increase in MDD risk for the overall population,with females and males in this age group experiencing a 75.1%and 103.4%increase,respectively.In terms of period effects,the risk of MDD displayed a decline from 1990 to 1994,followed by a rebound in 2008.Cohort effects demonstrated diverse generational patterns,with generationⅠand generationⅢmanifesting opposing‘age-as-level'trends.GenerationⅡand generationⅣexhibited'cumulative disadvantage'and'cumulative advantage'patterns,respectively.Age effects indicated an overall higher risk of MDD incidence in females,while cohort effects showed greater variations of MDD incidence among females.Conclusions The study underscores the substantial effects of age,period and cohort on MDD across genders in China.Priority interventions targeting vulnerable populations,including children,adolescents,older adults,females and the post-millennium birth cohort,are crucial to mitigate the impact of MDD.展开更多
Sea surface temperature(SST)is a crucial physical parameter in meteorology and oceanography.This study demonstrates that the influence of earth incidence angle(EIA)on the SST retrieved from the microwave radiation ima...Sea surface temperature(SST)is a crucial physical parameter in meteorology and oceanography.This study demonstrates that the influence of earth incidence angle(EIA)on the SST retrieved from the microwave radiation imager(MWRI)onboard FengYun-3(FY-3)meteorological satellites should not be ignored.Compared with algorithms that do not consider the influence of EIA in the regression,those that integrate the EIA into the regression can enhance the accuracy of SST retrievals.Subsequently,based on the recalibrated Level 1B data from the FY-3/MWRI,a long-term SST dataset was reprocessed by employing the algorithm that integrates the EIA into the regression.The reprocessed SST data,including FY-3B/MWRI SST during 2010-2019,FY-3C/MWRI SST during 2013-2019,and FY-3D/MWRI SST during 2018-2020,were compared with the in-situ SST and the SST dataset from the Operational Sea Surface Temperature and Ice Analysis(OSTIA).The results show that the FY-3/MWRI SST data were consistent with both the in-situ SST and the OSTIA SST dataset.Compared with the Copernicus Climate Change Service V2.0 SST,the absolute deviation of the reprocessed SST,with a quality flag of 50,was less than 1.5℃.The root mean square errors of the FY-3/MWRI orbital,daily,and monthly SSTs,with a quality flag of 50,were approximately 0.82℃,0.69℃,and 0.37℃,respectively.The primary discrepancies between the FY-3/MWRI SST and the OSTIA SST were found mainly in the regions of the western boundary current and the Antarctic Circumpolar Current.Overall,this reprocessed SST product is recommended for El Niño and La Niña events monitoring.展开更多

By takin...The main goal of this research is to assess the impact of race, age at diagnosis, sex, and phenotype on the incidence and survivability of acute lymphocytic leukemia (ALL) among patients in the United States. By taking these factors into account, the study aims to explore how existing cancer registry data can aid in the early detection and effective treatment of ALL in patients. Our hypothesis was that statistically significant correlations exist between race, age at which patients were diagnosed, sex, and phenotype of the ALL patients, and their rate of incidence and survivability data were evaluated using SEER*Stat statistical software from National Cancer Institute. Analysis of the incidence data revealed that a higher prevalence of ALL was among the Caucasian population. The majority of ALL cases (59%) occurred in patients aged between 0 to 19 years at the time of diagnosis, and 56% of the affected individuals were male. The B-cell phenotype was predominantly associated with ALL cases (73%). When analyzing survivability data, it was observed that the 5-year survival rates slightly exceeded the 10-year survival rates for the respective demographics. Survivability rates of African Americans patients were the lowest compared to Caucasian, Asian, Pacific Islanders, Alaskan Native, Native Americans and others. Survivability rates progressively decreased for older patients. Moreover, this study investigated the typical treatment methods applied to ALL patients, mainly comprising chemotherapy, with occasional supplementation of radiation therapy as required. The study demonstrated the considerable efficacy of chemotherapy in enhancing patients’ chances of survival, while those who remained untreated faced a less favorable prognosis from the disease. Although a significant amount of data and information exists, this study can help doctors in the future by diagnosing patients with certain characteristics. It will further assist the health care professionals in screening potential patients and early detection of cases. This could also save the lives of elderly patients who have a higher mortality rate from this disease.展开更多
Introduction: Total knee arthroplasty (TKA) has been established as a transformative solution in the treatment of advanced degenerative diseases of the knee, such as osteoarthritis, rheumatoid arthritis, and posttraum...Introduction: Total knee arthroplasty (TKA) has been established as a transformative solution in the treatment of advanced degenerative diseases of the knee, such as osteoarthritis, rheumatoid arthritis, and posttraumatic arthritis. In this sense, TKA surgery, which seeks to replace the damaged joint with prosthetic components, has proven to be highly effective in relieving pain, improving joint function, and, ultimately, significantly increasing patients’ quality of life. The present study describes the TKA and revision knee arthroplasty (RKA) rates and, identifies the associated co morbidities in the Colombian context. Methods: A retrospective cohort study was carried out. It describes demographic and clinical characteristics between two groups of patients, TKA or RKA, and its association with mortality at 30 days, 90 days, or one year after the intervention. Results: The incidence rate of the population undergoing TKA was approximately 11.71 cases per 100,000 inhabitants. Furthermore, the incidence rate for revision knee arthroplasty (RKA) procedures in the same period was around 0.96 per 100,000 inhabitants. In both groups at 30 days postoperatively, a total mortality rate of 0.09%was recorded. When the follow-up was extended to 90 days, it increased to 0.15%;at one year postoperatively, it rose to 0.88%. Conclusion: Mortality after surgery was low in Colombia in 2019. Although RKA is a beneficial procedure, in certain circumstances, it was noted that it carries a higher risk compared to primary TKA. Our results emphasize the importance of careful evaluation of co morbidities and risk factors in patients undergoing these surgical procedures. The application of quality-of-life questionnaires should be considered in future studies on effectiveness and mortality for TKA and RKA in our country.展开更多

文摘BACKGROUND Pelvic fractures are often associated with significant morbidity,including injuries to the urinary tract.Understanding the incidence and risk factors for urinary tract injury in these patients is crucial for prompt diagnosis and management.This meta-analysis aims to synthesize existing evidence to determine the overall incidence and identify specific risk factors associated with urinary tract injuries in patients with pelvic fractures.AIM To determine the incidence and risk factors for urinary tract injuries in patients with pelvic fractures.METHODS A systematic search of PubMed,EMBASE,Scopus,and the Cochrane Library was conducted without date restrictions.Studies examining the incidence and risk factors of urinary tract injuries in patients with pelvic fractures were included.Data extracted included demographics,injury mechanism,pelvic fracture type,urinary tract injury incidence,mortality,and discharge disposition.Review Manager 5.4 was used for data analysis.RESULTS Ten studies comprising 22700 patients were included.The pooled incidence of urinary tract injury associated with pelvic fracture was 6.88%(95%CI:6.20%-7.55%).Vehicle,motorcycle,and pedestrian accidents were identified as risk factors for urinary tract injury,with relative risks(RR)of 1.08(95%CI:1.06-1.11),1.89(95%CI:1.78-2.00),and 1.53(95%CI:1.20-1.95),respectively.Pubic fracture and pelvic ring disruption were significantly associated with urinary tract injury[odds ratio(OR)1.94,95%CI:1.09-3.44 and OR 5.53,95%CI:4.67-6.54,respectively)].Patients without urinary tract injury were more likely to be discharged home(RR 0.79,95%CI:0.67-0.92).Mortality was higher in patients with urinary tract injury(OR 1.92,95%CI:1.77-2.09).CONCLUSION Urinary tract injury occurs in nearly 7%of patients with pelvic fractures.Motorcycle accidents,pubic fractures,and pelvic ring disruptions are significant risk factors.Urinary tract injury following pelvic fracture is associated with increased mortality.
文摘In April 2024,the International Agency for Research on Cancer(IARC)published the 2022 Global Cancer Statistics Report in CA:A Cancer Journal for Clinicians,focusing on the incidence of 36 kinds of cancers in 185 countries or regions around the world,different gender,geographical area and Human Development Index differences in different economic regions.Previously,China's National Cancer Center(NCC)collaborated with IARC to estimate China's cancer incidence data using actual and up-to-date surveillance data from China.This collaboration aimed to ensure consistency between the cancer burden data published by IARC and NCC.In February of the same year,the NCC released the latest estimated statistics regarding the cancer incidence in China for 2022 in the Journal of the National Cancer Center.This research seeks to gather and evaluate the latest reports on cancer incidence from both a global perspective and within China for the year 2022.By conducting a multi-dimensional analysis of cancer incidence patterns,gender distribution,geographic spread,and trends over time,the study highlights the distinctions in the burden of malignant tumors between China and the global scenario.Additionally,it explores the risk factors associated with high-incidence cancer types and examines the current status of prevention and control measures.The findings aim to serve as a valuable reference for developing effective cancer prevention and control strategies in China.

文摘BACKGROUND Postoperative delirium(POD)is a concerning complication of organ transplantation.With organ transplantation offering hope to patients with end-stage organ disease,understanding the incidence and risk factors of POD is crucial,as it can significantly affect patients’prognosis and healthcare costs.AIM To systematically evaluate the incidence and risk factors of POD following organ transplantation to facilitate clinical prevention and optimize patient management and prognosis.METHODS Multiple databases such as PubMed and their reference lists were comprehensively searched using a combination of keywords related to organ transplantation and POD.Relevant observational studies on patients who had undergone solid organ transplantation and randomized controlled trials containing relevant analyses were included.Duplicated,data-deficient,non-English,and non-original data studies were excluded.Data were extracted independently by two researchers and then cross-checked.The Newcastle-Ottawa scale was used to evaluate the quality of the included studies.RevMan 5.3 was employed for data analysis.The pooled incidence of POD was calculated according to the data type,and the fixed or random effect model was employed to analyze risk factors based on heterogeneity.Subsequently,sensitivity analysis and publication bias assessments were performed.RESULTS A total of 39 relevant literatures were included.The overall incidence of POD in the organ transplant group was 20%[95%confidence interval(CI):18%-22%];liver transplant group,22%(95%CI:17%-26%);lung transplant group,34%(95%CI:23%-45%);and kidney transplant group,6%(95%CI:2%-10%).Primary graft dysfunction increased the POD risk,with a pooled odds ratio(OR)(95%CI)of 1.78(1.09-2.91).A history of hepatic encephalopathy increased the POD risk,with a pooled OR(95%CI)of 3.19(2.30-4.43).The higher the Acute Physiology and Chronic Health Evaluation II score,the greater the POD risk,with a pooled OR(95%CI)of 1.52(1.09-2.12).A history of alcohol abuse increased the POD risk,with a pooled OR(95%CI)of 2.84(1.74-4.65).Thus,the higher the model for end-stage liver disease score,the greater the POD risk,with a pooled OR(95%CI)of 2.49(1.14-5.43).POD was more likely to develop in patients with preoperative infections,with a pooled OR(95%CI)of 2.78(1.56-4.97).The use of diuretics increased the POD risk,with a pooled OR(95%CI)of 2.36(1.38-4.04).CONCLUSION In this study,the overall incidence of POD in patients who underwent organ transplantation is 20%.The incidence varies among different types of organ transplantation,and multiple factors can increase the POD risk.
文摘BACKGROUND Post-hepatectomy portal vein thrombosis(PH-PVT)is a life-threatening complication;however,the available literature on this topic is limited.AIM To examine the incidence,risk factors,and outcomes associated with PH-PVT.METHODS Medical records of patients who underwent hepatic resection for various diseases between February 2014 and December 2023 at Beijing Tsinghua Changgung Hospital affiliated with Tsinghua University(Beijing,China)were retrospectively reviewed.The patients were divided into a PH-PVT group and a non-PH-PVT group.Univariate and multivariate logistic regression analyses were performed to identify the risk factors for PH-PVT.RESULTS A total of 1064 patients were included in the study cohort,and the incidence and mortality rates of PH-PVT were 3.9%and 35.7%,respectively.The median time from hepatectomy to the diagnosis of PH-PVT was 6 days.Multivariate analysis revealed that hepatectomy combined with pancreaticoduodenectomy(HPD)[odds ratio(OR)=7.627(1.390-41.842),P=0.019],portal vein reconstruction[OR=6.119(2.636-14.203),P<0.001]and a postoperative portal vein angle<100°[OR=2.457(1.131-5.348),P=0.023]were independent risk factors for PH-PVT.Age≥60 years[OR=8.688(1.774-42.539),P=0.008]and portal vein reconstruction[OR=6.182(1.246-30.687),P=0.026]were independent risk factors for mortality in PH-PVT patients.CONCLUSION Portal vein reconstruction,a postoperative portal vein angle<100°and HPD were independent risk factors for PHPVT.Age≥60 years and portal vein reconstruction were independent risk factors for mortality in PH-PVT patients.

文摘BACKGROUND: Severe acute pancreatitis(SAP) is commonly associated with acute organ failure, but its effects on cerebral function within intensive care unit(ICU) patients remains inadequately researched. This study aims to determine the prevalence of delirium in critically ill patients diagnosed with SAP, and to identify risk factors associated with delirium in this patient population.METHODS: This was a retrospective, multicenter study, which enrolled adult patients diagnosed with SAP who admitted intensive care unit(ICU) for at least 24 h. Patient assessment was conducted using the Richmond Agitation-Sedation Scale(RASS) and the Confusion Assessment Method for the ICU(CAM-ICU). The cumulative incidence of delirium was determined. Demographic, clinical data, and length of ICU stay were compared between patients with and without delirium. A logistic regression model was employed to identify potential risk factors for delirium. RESuLTS: A total of 1,814 patients were included from seven hospitals in Anhui province, China. Delirium was observed in 25.2% of patients. Logistic regression analysis identified APACHE II scores(odds ratio [OR]=3.37, 95% confidence interval [CI]: 1.09–10.43, P=0.04), physical restraint(OR=11.11, 95%CI: 4.35–28.39, P<0.05), invasive mechanical ventilation(IMV)(OR=2.44, 95%CI: 1.41–4.25, P=0.002), and ICU length of stay ≥ 7 days(OR=3.14, 95%CI: 2.27–4.36, P<0.05) as independent risk factors of delirium.cONcLuSION: The present study revealed a substantial incidence of delirium in critically ill patients with SAP, associated with factors including APACHE II score, IMV, physical restraint, and prolonged ICU stays.
文摘Objective:Esophageal cancer has made a great contribution to the cancer burden in Jiangsu Province,East China.This study was aimed at reporting esophageal cancer incidence trend in 2009-2019 and its prediction to 2030.Methods:The burden of esophageal cancer in Jiangsu in 2019 was estimated using 54 cancer registries’data selected from Jiangsu Cancer Registry.Incident cases of 16 cancer registries were applied for the temporal trend from 2009 to 2019.The burden of esophageal cancer by 2030 was projected using the Bayesian age-period-cohort(BAPC)model.Results:About 24,886 new cases of esophageal cancer(17,233 males and 7,653 females)occurred in Jiangsu in 2019.Rural regions of Jiangsu had the highest incidence rate.The age-standardized incidence rate(ASIR,per 100,000 population)of esophageal cancer in Jiangsu decreased from 27.72 per 100,000 in 2009 to 14.18 per 100,000 in 2019.The BAPC model showed that the ASIR would decline from 13.01 per 100,000 in 2020 to 4.88 per 100,000 in 2030.Conclusions:According to the data,esophageal cancer incidence rates were predicted to decline until 2030,yet the disease burden is still significant in Jiangsu.The existing approaches to prevention and control are effective and need to be maintained.

文摘Background As the population in China rapidly ages,the prevalence of mild cognitive impairment(MCI)is increasing considerably.However,the causes of MCI vary.The continued lack of understanding of the various subtypes of MCI impedes the implementation of effective measures to reduce the risk of advancing to more severe cognitive diseases.Aims To estimate the prevalence and incidence rates of two MCI subtypes—amnestic MCI(aMCI)and vascular cognitive impairment without dementia(VCIND)—and to determine modifiable factors for them among older individuals in a multiregional Chinese cohort.Method This 1-year longitudinal study surveyed a random sample of participants aged≥60 years from a large,community-dwelling cohort in China.Baseline lifestyle data were self-reported,while vascular and comorbid conditions were obtained from medical records and physical examinations.In total,3514 and 2051 individuals completed the baseline and 1-year follow-up assessments,respectively.Logistic and linear regression analyses were used to identify the modifiable factors for MCI subtypes and predictors of cognitive decline,respectively.Results Among our participants,aMCI and VCIND demonstrated prevalence of 14.83%and 2.71%,respectively,and annual incidence(per 1000 person-years)of 69.6 and 10.6,respectively.The risk factor for aMCI was age,whereas its protective factors were high education level,tea consumption and physical activity.Moreover,VCIND risk factors were age,hypertension and depression.The presence of endocrine disease,cerebral trauma or hypertension was associated with a faster decline in cognition over 1 year.Conclusions MCI is a serious health problem in China that will only worsen as the population ages if no widespread interventions are implemented.Preventive strategies that promote brain activity and support healthy lifestyle choices are required.We identified modifiable factors for MCI in older individuals.The easy-to-adopt solutions such as tea consumption and physical activity can aid in preventing MCI.

文摘Background The Global Cancer Observatory(GLOBOCAN)2022 and the Global Burden of Disease(GBD)2021 are two primary databases for assessing the global cancer burden.This study aimed to compare the incidence and mortality rates of tracheal,bronchus,and lung(TBL)cancer reported in these databases and to analyze the observed discrepancies.Methods Age-standardized rates(ASRs),including age-standardized incidence rates and age-standardized mortality rates for TBL cancer,were obtained from GLOBOCAN 2022 and GBD 2021 for the most recent available year.Differences in ASRs at the national level between the two databases were quantified using pairwise differences,calculated as the absolute difference in ASRs divided by the mean of the ASRs from both sources.Correlations between macroeconomic factors and pairwise differences in ASRs were evaluated,and country features correlated with high pairwise differences were assessed.The data sources and methods used in the two databases were also compared.Results Strong correlations were identified between ASRs reported by GLOBOCAN 2022 and GBD 2021;however,significant differences were observed between estimates from the two data sources.African countries commonly exhibited larger pairwise differences in ASRs,whereas European countries demonstrated smaller pairwise differences in ASRs.Additionally,some populous developing countries,including China,South Africa,Brazil and India,showed smaller differences in ASRs.Countries lacking vital registration systems or high-quality population-based cancer registries displayed larger differences in ASRs.Furthermore,differences in ASRs were negatively correlated with macroeconomic factors.The data sources and estimation methods used by the two databases were inconsistent.Conclusions Discrepancies in TBL cancer incidence and mortality were observed between GLOBOCAN 2022 and GBD 2021.While differences in sources and methods partially explain these discrepancies,a country's cancer surveillance maturity and economic status also correlate with the accuracy of the estimates.Estimating the cancer burden in less wealthy countries remains a substantial challenge,necessitating long-term assistance and investment.
文摘Despite numerous research investigations to understand the influences of various structural parameters,to the authors'knowledge,no research has been the effect of different angles of incidence on stab response and performance of different types of protective textiles.Three distinct structures of 3D woven textiles and 2D plain weave fabric made with similar high-performance fiber and areal density were designed and manufactured to be tested.Two samples,one composed of a single and the other of 4-panel layers,from each fabric type structure,were prepared,and tested against stabbing at[0○],[22.5○],and[45○]angle of incidence.A new stabbing experimental setup that entertained testing of the specimens at various angles of incidence was engineered and utilized.The stabbing bench is also equipped with magnetic sensors and a UK Home Office Scientific Development Branch(HOSDB)/P1/B sharpness engineered knives to measure the impact velocity and exerted impact energy respectively.A silicon compound was utilized to imprint the Back Face Signature(BFS)on the backing material after every specimen test.Each silicon print was then scanned,digitized,and precisely measured to evaluate the stab response and performance of the specimen based on different performance variables,including Depth of Trauma(DOT),Depth of Penetration(DOP),and Length of Penetration(LOP).Besides,the post-impact surface failure modes of the fabrics were also measured using Image software and analyzed at the microscale level.The results show stab angle of incidence greatly influences the stab response and performance of protective textiles.The outcome of the study could provide not only valuable insights into understanding the stab response and capabilities of protective textiles under different angle of incidence,but also provide valuable information for protective textile manufacturer,armor developer and stab testing and standardizing organizations to consider the angle of incidence while developing,testing,optimizing,and using protective textiles in various applications.

文摘Background:The National Cancer Center(NCC)of China regularly reports the nationwide statistics on cancer incidence and mortality in China.The International Agency for Research on Cancer(IARC)calculates and publishes the cancer burden of countries around the world every two years.To ensure consistency between the actual surveillance data in China and the data published by IARC,NCC has received approval from the National Health Commission and IARC to simultaneously release the cancer burden data for China in GLOBOCAN 2022.Methods:There were a total of 700 registries reporting high-quality data on cancer incidence and mortality across China in 2018,of which 106 registries with continuous monitoring from 2010 to 2018 were used to establish an age-period-cohort model to simulate the trend of cancer incidence and mortality and to estimate the incidence and mortality in China in 2022.In addition,we analyzed the temporal trends of age-standardized cancer incidence and mortality from 2000 to 2018 using data from 22 continuous cancer registries.Results:It was estimated about 4,824,700 new cancer cases and 2,574,200 new cancer deaths occurred in China in 2022.Cancers of the lung,colon-rectum,thyroid,liver and stomach were the top five cancer types,accounting for 57.42%of new cancer cases.Cancers of the lung,liver,stomach,colon-rectum and esophagus were the five leading causes of cancer deaths,accounting for 67.50%of total cancer deaths.The crude rate and age-standardized incidence rate(ASIR)were 341.75 per 100,000 and 201.61 per 100,000,respectively.The crude mortality rate was 182.34 per 100,000 and the age-standardized mortality rate(ASMR)was 96.47 per 100,000.The ASIR of all cancers combined increased by approximately 1.4%per year during 2000–2018,while the ASMR decreased by approximately 1.3%per year.We observed decreasing trends in ASIR and ASMR for cancers of the esophagus,stomach,and liver,whereas the ASIR increased significantly for cancers of the thyroid,prostate,and cervix.Conclusions:Cancer remains a major public health concern in China,with a cancer profile that reflects the coexistence of developed and developing regions.Sustained implementation of prevention and control measures has resulted in significant reductions in the incidence and mortality rates of certain historically high incidence cancers,such as esophageal,stomach and liver cancers.Adherence to the guidelines of the Healthy China Action Plan and the Cancer Prevention and Control Action Plan,along with continued efforts in comprehensive risk factor control,cancer screening,early diagnosis and treatment,and standardization of diagnostic and therapeutic protocols,are key strategies to effectively mitigate the increasing cancer burden by 2030.
文摘Objective Brain metastases significantly impact the clinical course of patients with hepatocellular carcinoma(HCC).This study aimed to examine the age-related incidence,demographics,and survival of patients with HCC and brain metastases.Methods Data of HCC patients from 2010 to 2015 in the Surveillance,Epidemiology,and End Results(SEER)Registry were screened for the presence of brain metastases.They were stratified by age and ethnicity.Multivariable logistic and Cox regression analyses were used to identify factors associated with brain metastases and those with overall survival(OS)and liver cancer-specific survival(CSS),respectively.Results A total of 141 HCC patients presenting with brain metastases were identified,accounting for 0.35% of all HCC patients and 2.37% of patients with metastatic disease.Among all HCC patients,the incidence rate was the highest among patients aged 30-49 years old(0.47%).Ethnicity was not associated with the presence of brain metastases at the time of HCC diagnosis.However,African-American patients presented with a significantly lower disease-specific survival[median time:1 month;interquartile range(IQR):0-3.0 months].Initial lung or bone metastasis was independently associated with an increased risk of the presence of brain metastases[odds ratio(OR):12.62,95% confidence interval(CI):8.40-18.97]but was not associated with a worse OS or CSS among those with brain metastases.Conclusion This study identified the age-related incidence and risk factors of brain metastases in HCC patients.These results may contribute to the consideration of brain screening among patients with initial metastatic HCC with lung or bone metastases,and influence the counseling of this patient population regarding their prognosis.

文摘Aiming at the characteristics of multi-stage and(extremely)small samples of the identification problem of key effectiveness indexes of weapon equipment system-of-systems(WESoS),a Bayesian intelligent identification and inference model for system effectiveness assessment indexes based on dynamic grey incidence is proposed.The method uses multi-layer Bayesian techniques,makes full use of historical statistics and empirical information,and determines the Bayesian estima-tion of the incidence degree of indexes,which effectively solves the difficulties of small sample size of effectiveness indexes and difficulty in obtaining incidence rules between indexes.Sec-ondly,The method quantifies the incidence relationship between evaluation indexes and combat effectiveness based on Bayesian posterior grey incidence,and then identifies key system effec-tiveness evaluation indexes.Finally,the proposed method is applied to a case of screening key effectiveness indexes of a missile defensive system,and the analysis results show that the proposed method can fuse multi-moment information and extract multi-stage key indexes,and has good data extraction capability in the case of small samples.

文摘The Fabry–Perot(FP) resonant cavity is widely used in laser and spectroscopic measurements due to its unique interference transfer function(ITF). In the ideal case of parallel incident light, the ITF of the FP resonant cavity can be expressed by the Airy function. However, in reality, it is difficult to achieve perfect parallelism with collimated beams. In this article, a theoretical model is established for non-parallel light incidence, which assumes that the non-parallel incident light is a cone-shaped beam, and the cone angle is used to quantify the non-parallelism of the beam. The transmittance function of the FP resonant cavity under non-parallel light incidence is derived. The accuracy of the model is experimentally verified. Based on this model, the effects of divergence angle, tilt angle and FP cavity parameters(reflectivity, cavity length)on the ITF are studied. The reasons for the decrease in peak value, broadening and asymmetry of the interference peak under non-parallel light incidence are explained. It is suggested that a fine balance between the interference peak and the collimation effect of the incident light should be considered in the design and application of FP resonant cavities, especially for tilted applications such as angle-scanned spectroscopy. The research results of this article have certain significance for the design and application of FP resonant cavities.

文摘Background Major depressive disorders(MDDs)impose substantial burdens on individuals and society;however,further detailed analysis is still needed for its long-term trends.Aims This study aimed to analyse the gender-specific temporal trends and cohort variations of MDD incidence among Chinese residents over the past three decades.Methods Employing the age-period-cohort-interaction model and leveraging data from the Global Burden of Disease Study 2019,this research identified and analysed incidence trends of MDD among Chinese males and females aged 5-94 years from 1990 to 2019 across three dimensions,encompassing age,period and birth cohort.Results The analysis reveals age-related effects,indicating heightened MDD risk among adolescents and older adults.Specifically,individuals entering the older adulthood at the age of 65-69 significantly increased the risk of MDD by 64.9%.People aged 90-94 years witnesseda 105.4%increase in MDD risk for the overall population,with females and males in this age group experiencing a 75.1%and 103.4%increase,respectively.In terms of period effects,the risk of MDD displayed a decline from 1990 to 1994,followed by a rebound in 2008.Cohort effects demonstrated diverse generational patterns,with generationⅠand generationⅢmanifesting opposing‘age-as-level'trends.GenerationⅡand generationⅣexhibited'cumulative disadvantage'and'cumulative advantage'patterns,respectively.Age effects indicated an overall higher risk of MDD incidence in females,while cohort effects showed greater variations of MDD incidence among females.Conclusions The study underscores the substantial effects of age,period and cohort on MDD across genders in China.Priority interventions targeting vulnerable populations,including children,adolescents,older adults,females and the post-millennium birth cohort,are crucial to mitigate the impact of MDD.

文摘Sea surface temperature(SST)is a crucial physical parameter in meteorology and oceanography.This study demonstrates that the influence of earth incidence angle(EIA)on the SST retrieved from the microwave radiation imager(MWRI)onboard FengYun-3(FY-3)meteorological satellites should not be ignored.Compared with algorithms that do not consider the influence of EIA in the regression,those that integrate the EIA into the regression can enhance the accuracy of SST retrievals.Subsequently,based on the recalibrated Level 1B data from the FY-3/MWRI,a long-term SST dataset was reprocessed by employing the algorithm that integrates the EIA into the regression.The reprocessed SST data,including FY-3B/MWRI SST during 2010-2019,FY-3C/MWRI SST during 2013-2019,and FY-3D/MWRI SST during 2018-2020,were compared with the in-situ SST and the SST dataset from the Operational Sea Surface Temperature and Ice Analysis(OSTIA).The results show that the FY-3/MWRI SST data were consistent with both the in-situ SST and the OSTIA SST dataset.Compared with the Copernicus Climate Change Service V2.0 SST,the absolute deviation of the reprocessed SST,with a quality flag of 50,was less than 1.5℃.The root mean square errors of the FY-3/MWRI orbital,daily,and monthly SSTs,with a quality flag of 50,were approximately 0.82℃,0.69℃,and 0.37℃,respectively.The primary discrepancies between the FY-3/MWRI SST and the OSTIA SST were found mainly in the regions of the western boundary current and the Antarctic Circumpolar Current.Overall,this reprocessed SST product is recommended for El Niño and La Niña events monitoring.
文摘The main goal of this research is to assess the impact of race, age at diagnosis, sex, and phenotype on the incidence and survivability of acute lymphocytic leukemia (ALL) among patients in the United States. By taking these factors into account, the study aims to explore how existing cancer registry data can aid in the early detection and effective treatment of ALL in patients. Our hypothesis was that statistically significant correlations exist between race, age at which patients were diagnosed, sex, and phenotype of the ALL patients, and their rate of incidence and survivability data were evaluated using SEER*Stat statistical software from National Cancer Institute. Analysis of the incidence data revealed that a higher prevalence of ALL was among the Caucasian population. The majority of ALL cases (59%) occurred in patients aged between 0 to 19 years at the time of diagnosis, and 56% of the affected individuals were male. The B-cell phenotype was predominantly associated with ALL cases (73%). When analyzing survivability data, it was observed that the 5-year survival rates slightly exceeded the 10-year survival rates for the respective demographics. Survivability rates of African Americans patients were the lowest compared to Caucasian, Asian, Pacific Islanders, Alaskan Native, Native Americans and others. Survivability rates progressively decreased for older patients. Moreover, this study investigated the typical treatment methods applied to ALL patients, mainly comprising chemotherapy, with occasional supplementation of radiation therapy as required. The study demonstrated the considerable efficacy of chemotherapy in enhancing patients’ chances of survival, while those who remained untreated faced a less favorable prognosis from the disease. Although a significant amount of data and information exists, this study can help doctors in the future by diagnosing patients with certain characteristics. It will further assist the health care professionals in screening potential patients and early detection of cases. This could also save the lives of elderly patients who have a higher mortality rate from this disease.
文摘Introduction: Total knee arthroplasty (TKA) has been established as a transformative solution in the treatment of advanced degenerative diseases of the knee, such as osteoarthritis, rheumatoid arthritis, and posttraumatic arthritis. In this sense, TKA surgery, which seeks to replace the damaged joint with prosthetic components, has proven to be highly effective in relieving pain, improving joint function, and, ultimately, significantly increasing patients’ quality of life. The present study describes the TKA and revision knee arthroplasty (RKA) rates and, identifies the associated co morbidities in the Colombian context. Methods: A retrospective cohort study was carried out. It describes demographic and clinical characteristics between two groups of patients, TKA or RKA, and its association with mortality at 30 days, 90 days, or one year after the intervention. Results: The incidence rate of the population undergoing TKA was approximately 11.71 cases per 100,000 inhabitants. Furthermore, the incidence rate for revision knee arthroplasty (RKA) procedures in the same period was around 0.96 per 100,000 inhabitants. In both groups at 30 days postoperatively, a total mortality rate of 0.09%was recorded. When the follow-up was extended to 90 days, it increased to 0.15%;at one year postoperatively, it rose to 0.88%. Conclusion: Mortality after surgery was low in Colombia in 2019. Although RKA is a beneficial procedure, in certain circumstances, it was noted that it carries a higher risk compared to primary TKA. Our results emphasize the importance of careful evaluation of co morbidities and risk factors in patients undergoing these surgical procedures. The application of quality-of-life questionnaires should be considered in future studies on effectiveness and mortality for TKA and RKA in our country.

Key findings reveal a concerning upward trajectory in cancer incidence rates, placing Costa Rica at the forefront within Central America. While prostate cancer and breast cancer dominate, disparities emerge when scrutinizing gender-specific trends. Notably, stomach and cervical cancers show declines, potentially attributed to targeted interventions. However, colorectal and liver cancers witness mortality increases, necessitating strategic responses. Geographical disparities persist across provinces, highlighting the need for equitable healthcare access. In conclusion, this commentary underscores the urgency of addressing the burgeoning cancer burden in Costa Rica, calling for evidence-based interventions and collaborative efforts on a global scale.
