Hybrid Post-Processing Procedure for Displacement-Based Plane Elements

In the analysis of high-rise building, traditional displacement-based plane elements are often used to get the in-plane internal forces of the shear walls by stress integration. Limited by the singular problem produced by wall holes and the loss of precision induced by using differential method to derive strains, the displacement-based elements cannot always present accuracy enough for design. In this paper, the hybrid post-processing procedure based on the Hellinger-Reissner variational principle is used for improving the stress precision of two quadrilateral plane elements. In order to find the best stress field, three different forms are assumed for the displacement-based plane elements and with drilling DOF. Numerical results show that by using the proposed method, the accuracy of stress solutions of these two displacement-based plane elements can be improved. 展开更多

Dezert-Smarandache(DSm) theory, a new information fusion theory, is widely applied in image processing, multiple targets tracking identification, and other areas for its excellent processing ability of imperfect information. However, earlier research on DSm theory mainly focused on one sort of questions. An evidence fusion procedure is proposed based on the hybrid DSm model to compensate for a lack of research on the entire information procedure of DSm theory. This paper analyzes the evidence fusion procedure, as well as correlative node input and output information. Key steps and detailed procedures of evidence fusion are also discussed. Finally, an experiment illustrates the efficiency of the proposed evidence fusion procedure. 展开更多

Form 2008 to 2009, four patients with complex thoracic aortic disease, including aortic aneurysms and dissections, were successfully treated in our department with a new treatment approach:hybrid procedure.Combined open surgery and endovascular repair were performed in these patients without deep hypothermia or circulatory arrest.Compared to those who underwent traditional open surgery in the same period, time of mechanical ventilation and ICU stay was decreased in these four patients.All of them were discharged soon after operation without postoperative complications or death.The result suggests that this new approach could be an option for thoracic aortic disease, but long-term and large-population studies are still required to demonstrate the safety and validity. 展开更多

DE is applied to generating the elite individuals of population,while TS is used for finding the optimal value by making perturbation in selected elite individuals. A lower bounding technique is developed to evaluate the quality of proposed algorithm. Experimental results verify the effectiveness and feasibility of proposed algorithm. 展开更多

Objective The lack of clarity regarding the application performance of a hybrid operating room(HOR)and the uncertainty of surgical scheduling often lead to its inefficient application.This study aimed to review the clinical application of our neurosurgical HOR and propose a scale to score cases clearly.Methods We reviewed the operating procedures and duration of stay in 1865 HOR cases.The actual procedures of each case were summarized into 5 application types,and numerical assignment was used to distinguish the dependence of each type on our HOR:surgical procedures combined with interventional procedures(4 points,the highest dependence),surgical procedures combined with imaging procedures(3 points),interventional procedures(2 points),imaging procedures(1 point),and surgical procedures(0 points,the lowest dependence).Results A novel scale that could score 1865 cases into those 5 grades was developed.The percentages by grade were as follows:4 points,4.24%;3 points,4.88%;2 points,20.75%;1 point,69.38%;and 0 points,0.75%.The cumulative usage time was 4241.9 h,the duration of which was as follows:4 points,16.17%;3 points,15.50%;2 points,31.32%;1 point,35.62%;and 0 points,1.39%.Conclusions The HOR serves as a multifunctional room to treat neurosurgical diseases.The scale helps to quickly prioritize cases that rely more on HOR,providing guidelines for surgical scheduling.Although our HOR is unsuitable for emergency cases,it clearly shows the application performance of our HOR to provide a reference for promoting its efficient application. 展开更多

Ⅱ型杂交治疗急性A型主动脉夹层的疗效分析

目的比较Ⅱ型杂交技术和改良孙氏手术治疗急性A型主动脉夹层(ATAAD)患者的早中期疗效,旨在评估Ⅱ型杂交技术治疗ATAAD的有效性。方法回顾性分析河北医科大学第四医院2020年2月至2023年12月行全主动脉弓替换手术的患者,本次研究共入选患... 目的比较Ⅱ型杂交技术和改良孙氏手术治疗急性A型主动脉夹层(ATAAD)患者的早中期疗效,旨在评估Ⅱ型杂交技术治疗ATAAD的有效性。方法回顾性分析河北医科大学第四医院2020年2月至2023年12月行全主动脉弓替换手术的患者,本次研究共入选患者90例,其中45例接受了改良全主动脉弓置换及象鼻支架置入术(改良组),45例接受了一期Ⅱ型杂交修复术(杂交组),所有患者手术均完成顺利。结果改良组年龄60岁以上13例,杂交组11例(P=0.634),术前基础病高血压、糖尿病、冠心病两组比较差异无统计学意义(P>0.05),两组患者术前肾功能不全、心包填塞差异无统计学意义(P>0.05)。杂交组和改良组转机时间、主动脉阻断时间、低流量时间差异均有统计学意义(P<0.001)。患者术后拔管时间、ICU停留时间、术后住院时间、二次开胸止血例数,两组比较差异无统计学意义(P>0.05)。术后并发症仅在多器官功能障碍综合征上杂交组优于改良组(P<0.05)。结论Ⅱ型杂交治疗ATAAD能够缩短体外循环时间、主动脉阻断时间及低流量时间,术后并发症优于改良孙氏手术,但远期疗效仍需大样本进一步深入研究。 展开更多

目的总结应用"一站式"复合技术(Hybrid)治疗8例冠心病的临床疗效。方法 8例冠心病患者用hy-brid技术进行心肌再血管化,所有患者术前肺功能检查结果基本正常,其中合并高血压6例、糖尿病4例、陈旧性心肌梗死5例、肾功能不全2例... 目的总结应用"一站式"复合技术(Hybrid)治疗8例冠心病的临床疗效。方法 8例冠心病患者用hy-brid技术进行心肌再血管化,所有患者术前肺功能检查结果基本正常,其中合并高血压6例、糖尿病4例、陈旧性心肌梗死5例、肾功能不全2例、心功能不全3例。术前冠状动脉造影提示左前降支存在75%~100%狭窄,同时伴有右冠状动脉或左回旋支70%~98%狭窄。心脏彩超提示左室射血分数0.41~0.62(0.55±0.16)。结果全组无死亡、出血、脑卒中、心律失常、心肌梗死、伤口感染等并发症。心脏不停跳下冠状动脉旁路移植术手术时间1.6~2.7(1.7±0.9)h;气管插管时间5~13(7.8±2.9)h;胸腔24 h引流量为65~410(213.4±189.3)mL;ICU滞留时间1~2(1.4±0.5)d;术后住院5~9(5.7±1.9)d。随访0.7~2.7年,无远期死亡和心肌梗死,无再次手术。结论"一站式"复合技术(Hybrid)治疗多支冠状动脉病变是一项安全、有效的手术方式,其远期疗效仍有待于更多的临床应用和临床试验加以验证。 展开更多

中青年急性Stanford A型主动脉夹层患者杂交手术与孙氏手术短期预后比较研究

背景急性Stanford A型主动脉夹层(ASAAD)发病越来越趋于年轻化,ASAAD不同的发病特点有其相应的手术治疗选择。目前,孙氏手术与杂交手术是应用较广的手术方式,但对于中青年患者两种手术方式的短期预后研究较少。目的对比分析60岁以下中青... 背景急性Stanford A型主动脉夹层(ASAAD)发病越来越趋于年轻化,ASAAD不同的发病特点有其相应的手术治疗选择。目前,孙氏手术与杂交手术是应用较广的手术方式,但对于中青年患者两种手术方式的短期预后研究较少。目的对比分析60岁以下中青年ASAAD患者行杂交手术与孙氏手术的短期预后情况。方法回顾性观察2015年1月—2021年6月在河北医科大学第四医院心脏血管外科接受手术治疗的266例年龄<60岁的ASAAD患者,其中行孙氏手术184例(孙氏手术组),杂交手术82例(杂交手术组),应用倾向性评分进行匹配,卡钳值设定为0.03,共获得了78对匹配患者。采用Kaplan-Meier生存曲线分析两组患者院内全因死亡率。结果倾向性匹配评分匹配后两组均78例,其中男58例、女20例;两组指标除肌钙蛋白I(cTnI)外,其他指标比较,差异均无统计学意义(P>0.05)。倾向性评分匹配后:术中杂交手术组进行冠状动脉/股动脉搭桥术手术(28.2%与11.5%,P=0.009)较孙氏手术组多,最低鼻温[28.0(27.0,28.8)℃与25.5(24.6,28.6)℃,P<0.001]和最低肛温[28.5(27.5,29.5)℃与26.3(26.0,28.6)℃,P<0.001]较孙氏手术组高,乳酸最高值[5.2(1.9,9.9)mmol/L与9.9(4.3,15.1)mmol/L,P<0.001]较孙氏手术组低,红细胞[10.0(6.0,12.0)×10^(9)/L与13.0(8.0,16.5)×10^(9)/L,P=0.004]、冷沉淀[(12.8±2.8)u与(16.4±4.3)u,P<0.001]、血浆[600.0(443.7,800.0)mL与800.0(587.5,1412.5)mL,P=0.011]的输注量较孙氏手术组少。倾向匹配评分后杂交手术组低蛋白血症(33.3%与9.0%,P<0.001)发生率高于孙氏手术组。Kaplan-Meier生存曲线分析显示,杂交手术组患者30 d院内死亡率低于孙氏手术组(7.7%与25.6%,P=0.030)。结论中青年ASAAD患者行杂交手术后30 d院内死亡率降低,但中风、截瘫与低蛋白血症等并发症较孙氏手术多,需个性化的合理选择两种手术方式,尽可能地规避风险,加强术后并发症的管理。 展开更多

目的探讨Hybrid技术在治疗儿童心脏复杂畸形合并肌部室间隔缺损(m VSD)的疗效。方法回顾性分析2010年12月—2016年12月我院收治的18例(男9例,女9例)复杂心脏畸形伴m VSD患儿的临床资料。复杂心脏畸形主要包括:法洛氏四联征、主动脉弓缩... 目的探讨Hybrid技术在治疗儿童心脏复杂畸形合并肌部室间隔缺损(m VSD)的疗效。方法回顾性分析2010年12月—2016年12月我院收治的18例(男9例,女9例)复杂心脏畸形伴m VSD患儿的临床资料。复杂心脏畸形主要包括:法洛氏四联征、主动脉弓缩窄、完全性大动脉转位、右心室双出口、二尖瓣裂等。全组均在体外循环心内直视下行复杂畸形Ⅰ期解剖矫治及镶嵌封堵术。结果术后4例患者死亡,1例因术后感染,1例因复杂畸形未能修复,1例因封堵器移位致二尖瓣损伤,1例因不明原因心律失常。1例由于存在巨大膜部室间隔缺损行Ⅰ期手术时未发现肌部室缺,行非计划再次手术。余患儿均完成Ⅰ期手术矫治后顺利出院。随访(1个月~5年)无死亡。14例患儿中14例封堵器位置良好,12例无明显残余分流,13例心功能正常(射血分数0.63±0.05)。结论Hybrid治疗m VSD安全、有效,尤其适用于合并其他心内、外复杂畸形的患儿或复杂血管情况的患者。 展开更多

Critical aortic valve stenosis in newborns is the cause of a severe clinical condition with the onset of symptoms during first hours after birth.We present a clinical case of a successful surgical correction of a critical aortic stenosis using a hybrid method applied in a newborn during the first day of life.The infant was diagnosed with a hypoplastic left heart complex with an intact atrial septum(aortic and mitral valves stenosis variant),that led to the cardiogenic shock and acute pulmonary edema.The procedure included bilateral banding of the pulmonary artery branches and atrioseptostomy with stenting of the interatrial septum.The surgery was performed through a median sternotomy. 展开更多

Objective: The present study was conceived to analyze the clinical benefit of hybrid interventions with surgical common femoral artery (CFA) reconstruction coupled to superficial femoral/popliteal endovascular recanalization for severe infrainguinal multilevel occlusive disease in high-risk ASA Class 3 - 4 patients. Material and Methods: From August 2008 until May 2015, a series of 143 hybrid infrainguinal interventions in 124 ASA Class 3 - 4 patients were performed in our department for Rutherford category 2 - 6 ischemic presentations. Patient demographics, specific risk factors, technical characteristics and patency results were retrospectively examined during a mean 36.8 months of follow-up. In a majority of 94 limbs (65%), the endovascular stage of interventions focused on long (>15 cm) femoropopliteal occlusions in parallel to regular CFA surgical revascularization. Two or three runoff tibial trunks were evinced in 84% cases, while one or none permeable vessel was found in 23 (16%) limbs. Results: Inasmuch surgical approach was successful in all cases, the endovascular stage was technically profitable in 134 (93%) cases. The ABI posto-peratively improved (>1.5) in 73% of cases, while clinical presentation gained at least one Rutherford category in 89% limbs. The mean hospital stay was 6.1 days (3 - 12 days) whereas the 30-day mortality rate in this homogeneous “high-risk” group of patients was 3.2%. Global risk factors alike age (>70 years/p = 0.0005), smoking ((p = 0.0170) and female gender (p = 0.0111), together with CTOs length (>15 cm/(p = 0.0470), severe calcifications (p = 0.0001), poor tibial runoff (p = 0.0001), TASC “C” and “D” lesions (p = 0.360 and (p = 0.0394), the stent number ((n = 3) and length (>6 cm) ((p = 0.0039 and (p = 0.0003) and the initial ABI scoring ((p = 0.0051) showed statistical negative influence on primary patency. Conclusion: Hybrid infrainguinal revascularization may afford useful results in selected ASA “high risk” patients, owning low invasiveness, reproducibility and acceptable patency in return to punctual postoperative surveillance. 展开更多
