Background:Home accessibility modifi cations are crucial for promoting independent living and quality of life among persons with disabilities.While developed countries have established comprehensive policy frameworks,...Background:Home accessibility modifi cations are crucial for promoting independent living and quality of life among persons with disabilities.While developed countries have established comprehensive policy frameworks,developing nations like China face unique challenges in program design and implementation.Objective:This study conducts a systematic comparative analysis of home accessibility modification policies across China,Japan,Germany,and Sweden,identifying key policy dimensions and proposing evidence-based recommendations for strengthening China’s policy framework.Methods:We employed a multi-dimensional analytical framework examining legislative foundations,eligibility criteria,funding mechanisms,and service delivery models.Data were collected from primary legislation,governmental regulations,official statistics,and peer-reviewed literature.Results:Significant cross-national variations exist in policy approaches.Japan and Germany utilize social insurance models with standardized assessments,Sweden adopts a universal rights-based approach,while China employs a targeted assistance model focused on economically disadvantaged households.China completed 1.28 million household renovations during its 14th Five-Year Plan,demonstrating strong implementation capacity;future policy refi nement could draw on international experience to strengthen assessment standardization,broaden eff ective coverage,and improve the sustainability of fi nancing.Conclusions:China can benefi t from international experience in developing standardized assessment protocols,diversifying funding mechanisms,and establishing professional service delivery systems,while acknowledging contextual constraints unique to developing country settings.展开更多
Cyber-criminals target smart connected devices for spyware distribution and security breaches,but existing Internet of Things(IoT)security standards are insufficient.Major IoT industry players prioritize market share ...Cyber-criminals target smart connected devices for spyware distribution and security breaches,but existing Internet of Things(IoT)security standards are insufficient.Major IoT industry players prioritize market share over security,leading to insecure smart products.Traditional host-based protection solutions are less effective due to limited resources.Overcoming these challenges and enhancing the security of IoT Devices requires a security design at the network level that uses lightweight cryptographic parameters.In order to handle control,administration,and security concerns in traditional networking,the Gateway Node offers a contemporary networking architecture.By managing all network-level computations and complexity,the Gateway Node relieves IoT devices of these responsibilities.In this study,we introduce a novel privacy-preserving security architecture for gateway-node smart homes.Subsequently,we develop Smart Homes,An Efficient,Anonymous,and Robust Authentication Scheme(EARAS)based on the foundational principles of this security architecture.Furthermore,we formally examine the security characteristics of our suggested protocol that makes use of methodology such as ProVerif,supplemented by an informal analysis of security.Lastly,we conduct performance evaluations and comparative analyses to assess the efficacy of our scheme.Performance analysis shows that EARAS achieves up to 30%to 54%more efficient than most protocols and lower computation cost compared to Banerjee et al.’s scheme,and significantly reduces communication overhead compared to other recent protocols,while ensuring comprehensive security.Our objective is to provide robust security measures for smart homes while addressing resource constraints and preserving user privacy.展开更多

The contemporary smart cities,smart homes,smart buildings,and smart health care systems are the results of the explosive growth of Internet of Things(IoT)devices and deep learning.Yet the centralized training paradigm...The contemporary smart cities,smart homes,smart buildings,and smart health care systems are the results of the explosive growth of Internet of Things(IoT)devices and deep learning.Yet the centralized training paradigms have fundamental issues in data privacy,regulatory compliance,and ownership silo alongside the scaled limitations of the real-life application.The concept of Federated Deep Learning(FDL)is a privacy-by-design method that will enable the distributed training of machine learning models among distributed clients without sharing raw data and is suitable in heterogeneous urban settings.It is an overview of the privacy-preserving developments in FDL as of 2018-2025 with a narrow scope on its usage in smart cities(traffic prediction,environmental monitoring,energy grids),smart homes/buildings/IoT(non-intrusive load monitoring,HVAC optimization,anomaly detection)and the healthcare application(medical imaging,Electronic Health Records(EHR)analysis,remote monitoring).It gives coherent taxonomy,domain pipelines,comparative analyses of privacy mechanisms(differential privacy,secure aggregation,Homomorphic Encryption(HE),Trusted Execution Environments(TEEs),blockchain enhanced and hybrids),system structures,security/robustness defense,deployment/Machine Learning Operation(MLOps)issues,and the longstanding challenges(non-IID heterogeneity,communication efficiency,fairness,and sustainability).Some of the contributions made are structured comparisons of privacy threats,practical design advice on urban areas,recognition of open problems,and a research roadmap into the future up to 2035.The paper brings out the transformational worth of FDL in building credible,scalable,and sustainable intelligent urban ecosystems and the need to do further interdisciplinary research in standardization,real-world testbeds,and ethical governance.展开更多
Objective:To clarify the concept of in-home respite care in dementia care and identify changes in the service content over time to help providers and users better understand this sustainable service.Method:A literatur...Objective:To clarify the concept of in-home respite care in dementia care and identify changes in the service content over time to help providers and users better understand this sustainable service.Method:A literature search was conducted through Chinese databases China National Knowledge Infrastructure(CNKI),Wanfang,VIP,and SinoMed,as well as English databases PubMed,the Cochrane Library,Web of Sciences,and Embase.Articles published from January 1980 to December 2024 were identified.Rogers’conceptual analysis of evolution was used for this concept analysis,including six steps:identifying the concept and its context,selecting appropriate databases,determining relevant literature,identifying the concept’s attributes,antecedents,and consequences,choosing a concept exemplar if appropriate,and defininghypotheses and implications for further concept development.Results:Thirty-one articles were included.This conceptual analysis revealed the evolution of in-home respite care service content over time and summarized three key attributes.The antecedents included factors related to people with dementia,family caregivers,and the social environment(aging society,government support).The consequences of in-home respite services include delayed institutional placement and reduced security risk events for people with dementia.For family caregivers,consequences include reduced caregiving stress,improved quality of life,and perceived benefitsfrom rest periods.Conclusion:In-home respite care can be interpreted as family-centered home care that provides temporary relief from family caregivers’responsibilities in caring for people with dementia to reduce caregiver burden.The trend of service specialization and attention on dementia families’needs in service provision are future research focus.展开更多
Background There have been numerous intervention studies focusing on the development of preterm infants,but there has been limited investigation into the home environment as a determinant of developmental outcomes in ...Background There have been numerous intervention studies focusing on the development of preterm infants,but there has been limited investigation into the home environment as a determinant of developmental outcomes in preterm infants.The aspects and extent to which the home environment affects the early(18 months corrected age)neuropsychological development of preterm infants are still unclear.Aims This study aimed to analyse the effect of the home environment on the neuropsychiatric development of preterm infants at 18 months corrected age after discharge from the neonatal intensive care unit(NICU).It also sought to provide a basis for promoting neuropsychiatric development among preterm infants by improving the home environment.Methods In this retrospective cross-sectional study,275 preterm infants born between January 2019 and January 2022 were followed up for systematic management after discharge from the NICU at Shanghai Children's Hospital.The Home Nurture Environment Questionnaire was used to assess the home environment of the infants and analyse its impact on the developmental quotient(evaluated by the Gesell Developmental Scale)and the rate of developmental delays at 18 months corrected age.Results A total of 41.454%of the infants were extremely preterm.The developmental quotient scores at 18 months corrected age were in the middle of the scale.The language domain had the highest rate of developmental delay(46.182%),followed by the adaptive domain(37.091%).Multiple logistic regression analyses showed that compared with infants in supportive home environments,infants with moderate/unsupportive home environments had significantly elevated risks of development delay:2.162-fold for global(odds ratio(OR)2.162,95% confidence interval(Cl)1.274 to 3.665,p=0.004),2.193-fold for fine motor(OR 2.193,95%CI 1.161 to 4.140,p=0.016),2.249-fold for language(0R 2.249,95%CI 1.336 to 3.786,p=0.002)and 2.042-fold for personal-social(OR 2.042,95%CI 1.149 to 3.628,p=0.015).Conclusions A supportive home environment is a crucial protective factor for the neuropsychological development of preterm infants.It is associated with higher developmental quotient scores and protects against neuropsychiatric delays.Incorporating evaluation and continuous improvement of the home environment into the management framework for preterm infants to promote optimal neurodevelopment is essential.展开更多

BACKGROUND Cancer patients with an implanted venous access port(IVAP)often manage their care at home during chemotherapy intervals,including maintaining the device,monitoring complications,and following medication ins...BACKGROUND Cancer patients with an implanted venous access port(IVAP)often manage their care at home during chemotherapy intervals,including maintaining the device,monitoring complications,and following medication instructions.Home care ensures continued support after discharge.However,due to factors such as age,gender,culture,psychological status,and family support,the quality of home care varies significantly.Understanding these factors can help provide targeted guidance to improve the care of cancer patients.AIM To explore IVAP chemotherapy on home care quality and its association with mental health and family support for cancer patients.METHODS This investigative study was based on a medical records system.It investigated the relationship between psychological status,family support,and home care quality in 180 patients with cancer undergoing IVAP chemotherapy.Psychological status was assessed using the State Anxiety Inventory(S-AI);family support was assessed using the Perceived Social Support Scale(PSSS),and home care quality was evaluated using the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire(EORTC QLQ-C30).Pearson’s correlation and Structural Equation Modeling were used to analyze the interplay between these factors.RESULTS The average S-AI score was 47.52±14.47,PSSS was 52.48±12.64,and EORTC QLQ-C30 was 70.09±17.32.A substantial inverse relationship was observed between the EORTC QLQ-C30 and S-AI scores(r=-0.712).A significant positive correlation was found between the EORTC QLQ-C30 and the PSSS,with a correlation coefficient of(r=0.744).The multiple linear regression analysis indicated that family social support,psychological status,and average monthly family income were the main factors influencing the variation in the quality of home care,explaining 71.9%of the variation.The Structural Equation Modeling results indicated that psychological status acted as a partial mediator in the association between family social support and home care quality of life,explaining 32.78%of the mediation effect.CONCLUSION Psychological status and family social support positively impacted cancer patients’home care quality,with psychology partially mediating this effect.展开更多

文摘Objective:To clarify the concept of in-home respite care in dementia care and identify changes in the service content over time to help providers and users better understand this sustainable service.Method:A literature search was conducted through Chinese databases China National Knowledge Infrastructure(CNKI),Wanfang,VIP,and SinoMed,as well as English databases PubMed,the Cochrane Library,Web of Sciences,and Embase.Articles published from January 1980 to December 2024 were identified.Rogers’conceptual analysis of evolution was used for this concept analysis,including six steps:identifying the concept and its context,selecting appropriate databases,determining relevant literature,identifying the concept’s attributes,antecedents,and consequences,choosing a concept exemplar if appropriate,and defininghypotheses and implications for further concept development.Results:Thirty-one articles were included.This conceptual analysis revealed the evolution of in-home respite care service content over time and summarized three key attributes.The antecedents included factors related to people with dementia,family caregivers,and the social environment(aging society,government support).The consequences of in-home respite services include delayed institutional placement and reduced security risk events for people with dementia.For family caregivers,consequences include reduced caregiving stress,improved quality of life,and perceived benefitsfrom rest periods.Conclusion:In-home respite care can be interpreted as family-centered home care that provides temporary relief from family caregivers’responsibilities in caring for people with dementia to reduce caregiver burden.The trend of service specialization and attention on dementia families’needs in service provision are future research focus.

文摘Background There have been numerous intervention studies focusing on the development of preterm infants,but there has been limited investigation into the home environment as a determinant of developmental outcomes in preterm infants.The aspects and extent to which the home environment affects the early(18 months corrected age)neuropsychological development of preterm infants are still unclear.Aims This study aimed to analyse the effect of the home environment on the neuropsychiatric development of preterm infants at 18 months corrected age after discharge from the neonatal intensive care unit(NICU).It also sought to provide a basis for promoting neuropsychiatric development among preterm infants by improving the home environment.Methods In this retrospective cross-sectional study,275 preterm infants born between January 2019 and January 2022 were followed up for systematic management after discharge from the NICU at Shanghai Children's Hospital.The Home Nurture Environment Questionnaire was used to assess the home environment of the infants and analyse its impact on the developmental quotient(evaluated by the Gesell Developmental Scale)and the rate of developmental delays at 18 months corrected age.Results A total of 41.454%of the infants were extremely preterm.The developmental quotient scores at 18 months corrected age were in the middle of the scale.The language domain had the highest rate of developmental delay(46.182%),followed by the adaptive domain(37.091%).Multiple logistic regression analyses showed that compared with infants in supportive home environments,infants with moderate/unsupportive home environments had significantly elevated risks of development delay:2.162-fold for global(odds ratio(OR)2.162,95% confidence interval(Cl)1.274 to 3.665,p=0.004),2.193-fold for fine motor(OR 2.193,95%CI 1.161 to 4.140,p=0.016),2.249-fold for language(0R 2.249,95%CI 1.336 to 3.786,p=0.002)and 2.042-fold for personal-social(OR 2.042,95%CI 1.149 to 3.628,p=0.015).Conclusions A supportive home environment is a crucial protective factor for the neuropsychological development of preterm infants.It is associated with higher developmental quotient scores and protects against neuropsychiatric delays.Incorporating evaluation and continuous improvement of the home environment into the management framework for preterm infants to promote optimal neurodevelopment is essential.
文摘The smart home platform integrates with Internet of Things(IoT)devices,smartphones,and cloud servers,enabling seamless and convenient services.It gathers and manages extensive user data,including personal information,device operations,and patterns of user behavior.Such data plays an essential role in criminal inves-tigations,highlighting the growing importance of specialized smart home forensics.Given the rapid advancement in smart home software and hardware technologies,many companies are introducing new devices and services that expand the market.Consequently,scalable and platform-specific forensic research is necessary to support efficient digital investigations across diverse smart home ecosystems.This study thoroughly examines the core components and structures of smart homes,proposing a generalized architecture that represents various operational environments.A three-stage smart home forensics framework is introduced:(1)analyzing application functions to infer relevant data,(2)extracting and processing data from interconnected devices,and(3)identifying data valuable for investigative purposes.The framework’s applicability is validated using testbeds from Samsung SmartThings and Xiaomi Mi Home platforms,offering practical insights for real-world forensic applications.The results demonstrate that the proposed forensic framework effectively acquires and classifies relevant digital evidence in smart home platforms,confirming its practical applicability in smart home forensic investigations.

文摘To the editor:Adverse home environments(AHE),characterised by family conflict,parental separation or dysfunctional parenting,are linked to negative mental health outcomes in children and adults.12 AHE disproportionately affect children with neurodevelopmental disorders such as attention-deficit/hyperactivity disorder(ADHD),which is characterised by inattention,hyperactivity/impulsivity and functional impairments.3 Apart from core symptoms,including inattention and hyperactivity,disruptive behaviour disorders(DBD),such as oppositional defiant disorder(ODD)and conduct disorder(CD),may be associated with AHE.Conduct problems are risk factors for ODD.And CD has become a main concern for childhood mental health.
文摘BACKGROUND Cancer patients with an implanted venous access port(IVAP)often manage their care at home during chemotherapy intervals,including maintaining the device,monitoring complications,and following medication instructions.Home care ensures continued support after discharge.However,due to factors such as age,gender,culture,psychological status,and family support,the quality of home care varies significantly.Understanding these factors can help provide targeted guidance to improve the care of cancer patients.AIM To explore IVAP chemotherapy on home care quality and its association with mental health and family support for cancer patients.METHODS This investigative study was based on a medical records system.It investigated the relationship between psychological status,family support,and home care quality in 180 patients with cancer undergoing IVAP chemotherapy.Psychological status was assessed using the State Anxiety Inventory(S-AI);family support was assessed using the Perceived Social Support Scale(PSSS),and home care quality was evaluated using the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire(EORTC QLQ-C30).Pearson’s correlation and Structural Equation Modeling were used to analyze the interplay between these factors.RESULTS The average S-AI score was 47.52±14.47,PSSS was 52.48±12.64,and EORTC QLQ-C30 was 70.09±17.32.A substantial inverse relationship was observed between the EORTC QLQ-C30 and S-AI scores(r=-0.712).A significant positive correlation was found between the EORTC QLQ-C30 and the PSSS,with a correlation coefficient of(r=0.744).The multiple linear regression analysis indicated that family social support,psychological status,and average monthly family income were the main factors influencing the variation in the quality of home care,explaining 71.9%of the variation.The Structural Equation Modeling results indicated that psychological status acted as a partial mediator in the association between family social support and home care quality of life,explaining 32.78%of the mediation effect.CONCLUSION Psychological status and family social support positively impacted cancer patients’home care quality,with psychology partially mediating this effect.

文摘Objective:This study aims to analyze research trends,co-authorship networks,and hotspots over the past decade regarding research on exercise therapy at home for stroke patients.Methods:Articles on exercise therapy at home for stroke patients were routinely searched for in the SCOPUS database from 2013 to 2023.To evaluate and predict the most popular topics and trends in this field,the data collected from the reports are processed using the VOSviewer program.Results:The final analysis covered 1943 articles.The number of publications has steadily increased over the past decade.The United States has made the most significant contributions in this field.The University of Toronto(Canada)and Cramer,S.C.were the most productive institutions and researchers.The journal Disability and Rehabilitation has the highest number of publications(Citescore 4.4;SJR 0.76).The research area in this field is predominantly dominated by medicine.The frequently occurring keywords include“stroke,”“rehabilitation,”and“telerehabilitation.”Innovations,such as telerehabilitation and virtual reality(VR),are emerging as key trends,enhancing patient engagement and accessibility in home-based therapy.Conclusions:Using bibliometric analysis and network visualization,this study summarizes the latest research on home-based exercise therapy for stroke patients,highlighting the impact of innovative technologies,such as telerehabilitation and VR.This analysis identifies research gaps,trends,and popular subjects,providing a comprehensive framework for future studies on key topics,collaborative initiatives,and developmental patterns.
文摘This paper focuses on the study of mechanical design and control measures in smart home systems.First of all,it elaborates on the theoretical foundation of mechatronics technology,including its multidisciplinary integration characteristics,system design principles,and constituent elements.It then reviews the research progress in this field,followed by a detailed analysis of mechatronics design in systems such as smart lighting and smart security,as well as the application of control algorithms and communication protocols in smart homes.Finally,it discusses challenges such as system compatibility and data security risks,proposing corresponding solutions to provide theoretical and practical references for the development of smart home systems.
文摘Objective:To explore the perspectives of high-risk pregnant women regarding the home care program for high-risk pregnancies.Methods:This qualitative study was conducted in a university hospital and 5 comprehensive health centers in Ahvaz,Iran,from February 2023 to July 2023.The sampling method used was purposive sampling considering the maximum possible diversity,which continued until data saturation.11 in-depth and semistructured interviews were conducted with high-risk pregnant women.Data were simultaneously analysed using Graneheim and Lundman content analysis by MAXQDA version 20 software.Results:Data analysis extracted 3 themes,12 categories,and 26 subcategories.The themes and categories were“high-risk pregnancy and related issues(pursuing treatment and paying attention to the high-risk situation by pregnant women,psychological aspect of high-risk pregnancy on pregnant women and caregivers,emotional and physical aspects of high-risk pregnancy,the consequences of high-risk pregnancy on the people around them and empowerment of high-risk pregnant women)”,“support dimension(family and community support for high-risk pregnant women)”,and“framework of program(acculturalization,means and equipment,security,manpower and the necessary arrangements for the home care)”.Conclusions:High-risk pregnancy affects various aspects of the lives of high-risk pregnant women and their families.Creating the necessary conditions for providing care at home plays a vital role in supporting high-risk pregnant women and removing related obstacles to receiving prenatal care in person.It will be especially beneficial for high-risk pregnant women in low-income areas.

文摘Objective:To explore the barriers and facilitators associated with the implementation of horticultural activities(HA)in nursing care homes,and aim to identify findings that will enhance the development of more effective implementation strategies for such activities.Methods:This qualitative descriptive study recruited a purposive sample of potential adopters related to HA in nursing care homes,and the conventional content analysis method was adopted.Results:A total of 6 older adults,3 social workers,1 nurse,and 1 physician participated in this study.The analysis revealed 4 themes barriers and 2 themes facilitators to the implementation of HA.Conclusions:Future intervention programs should be meticulously designed to address the identified influencing factors,and the insights gained can serve as a reference for identifying and mitigating barriers in the implementation of other physical activity programs.In turn,this will contribute to improved physical functioning and healthy aging.
