Background There have been numerous intervention studies focusing on the development of preterm infants,but there has been limited investigation into the home environment as a determinant of developmental outcomes in ...Background There have been numerous intervention studies focusing on the development of preterm infants,but there has been limited investigation into the home environment as a determinant of developmental outcomes in preterm infants.The aspects and extent to which the home environment affects the early(18 months corrected age)neuropsychological development of preterm infants are still unclear.Aims This study aimed to analyse the effect of the home environment on the neuropsychiatric development of preterm infants at 18 months corrected age after discharge from the neonatal intensive care unit(NICU).It also sought to provide a basis for promoting neuropsychiatric development among preterm infants by improving the home environment.Methods In this retrospective cross-sectional study,275 preterm infants born between January 2019 and January 2022 were followed up for systematic management after discharge from the NICU at Shanghai Children's Hospital.The Home Nurture Environment Questionnaire was used to assess the home environment of the infants and analyse its impact on the developmental quotient(evaluated by the Gesell Developmental Scale)and the rate of developmental delays at 18 months corrected age.Results A total of 41.454%of the infants were extremely preterm.The developmental quotient scores at 18 months corrected age were in the middle of the scale.The language domain had the highest rate of developmental delay(46.182%),followed by the adaptive domain(37.091%).Multiple logistic regression analyses showed that compared with infants in supportive home environments,infants with moderate/unsupportive home environments had significantly elevated risks of development delay:2.162-fold for global(odds ratio(OR)2.162,95% confidence interval(Cl)1.274 to 3.665,p=0.004),2.193-fold for fine motor(OR 2.193,95%CI 1.161 to 4.140,p=0.016),2.249-fold for language(0R 2.249,95%CI 1.336 to 3.786,p=0.002)and 2.042-fold for personal-social(OR 2.042,95%CI 1.149 to 3.628,p=0.015).Conclusions A supportive home environment is a crucial protective factor for the neuropsychological development of preterm infants.It is associated with higher developmental quotient scores and protects against neuropsychiatric delays.Incorporating evaluation and continuous improvement of the home environment into the management framework for preterm infants to promote optimal neurodevelopment is essential.展开更多

Objective:To clarify the concept of in-home respite care in dementia care and identify changes in the service content over time to help providers and users better understand this sustainable service.Method:A literatur...Objective:To clarify the concept of in-home respite care in dementia care and identify changes in the service content over time to help providers and users better understand this sustainable service.Method:A literature search was conducted through Chinese databases China National Knowledge Infrastructure(CNKI),Wanfang,VIP,and SinoMed,as well as English databases PubMed,the Cochrane Library,Web of Sciences,and Embase.Articles published from January 1980 to December 2024 were identified.Rogers’conceptual analysis of evolution was used for this concept analysis,including six steps:identifying the concept and its context,selecting appropriate databases,determining relevant literature,identifying the concept’s attributes,antecedents,and consequences,choosing a concept exemplar if appropriate,and defininghypotheses and implications for further concept development.Results:Thirty-one articles were included.This conceptual analysis revealed the evolution of in-home respite care service content over time and summarized three key attributes.The antecedents included factors related to people with dementia,family caregivers,and the social environment(aging society,government support).The consequences of in-home respite services include delayed institutional placement and reduced security risk events for people with dementia.For family caregivers,consequences include reduced caregiving stress,improved quality of life,and perceived benefitsfrom rest periods.Conclusion:In-home respite care can be interpreted as family-centered home care that provides temporary relief from family caregivers’responsibilities in caring for people with dementia to reduce caregiver burden.The trend of service specialization and attention on dementia families’needs in service provision are future research focus.展开更多

Objective:This study aims to analyze research trends,co-authorship networks,and hotspots over the past decade regarding research on exercise therapy at home for stroke patients.Methods:Articles on exercise therapy at ...Objective:This study aims to analyze research trends,co-authorship networks,and hotspots over the past decade regarding research on exercise therapy at home for stroke patients.Methods:Articles on exercise therapy at home for stroke patients were routinely searched for in the SCOPUS database from 2013 to 2023.To evaluate and predict the most popular topics and trends in this field,the data collected from the reports are processed using the VOSviewer program.Results:The final analysis covered 1943 articles.The number of publications has steadily increased over the past decade.The United States has made the most significant contributions in this field.The University of Toronto(Canada)and Cramer,S.C.were the most productive institutions and researchers.The journal Disability and Rehabilitation has the highest number of publications(Citescore 4.4;SJR 0.76).The research area in this field is predominantly dominated by medicine.The frequently occurring keywords include“stroke,”“rehabilitation,”and“telerehabilitation.”Innovations,such as telerehabilitation and virtual reality(VR),are emerging as key trends,enhancing patient engagement and accessibility in home-based therapy.Conclusions:Using bibliometric analysis and network visualization,this study summarizes the latest research on home-based exercise therapy for stroke patients,highlighting the impact of innovative technologies,such as telerehabilitation and VR.This analysis identifies research gaps,trends,and popular subjects,providing a comprehensive framework for future studies on key topics,collaborative initiatives,and developmental patterns.展开更多
Objective:To explore the perspectives of high-risk pregnant women regarding the home care program for high-risk pregnancies.Methods:This qualitative study was conducted in a university hospital and 5 comprehensive hea...Objective:To explore the perspectives of high-risk pregnant women regarding the home care program for high-risk pregnancies.Methods:This qualitative study was conducted in a university hospital and 5 comprehensive health centers in Ahvaz,Iran,from February 2023 to July 2023.The sampling method used was purposive sampling considering the maximum possible diversity,which continued until data saturation.11 in-depth and semistructured interviews were conducted with high-risk pregnant women.Data were simultaneously analysed using Graneheim and Lundman content analysis by MAXQDA version 20 software.Results:Data analysis extracted 3 themes,12 categories,and 26 subcategories.The themes and categories were“high-risk pregnancy and related issues(pursuing treatment and paying attention to the high-risk situation by pregnant women,psychological aspect of high-risk pregnancy on pregnant women and caregivers,emotional and physical aspects of high-risk pregnancy,the consequences of high-risk pregnancy on the people around them and empowerment of high-risk pregnant women)”,“support dimension(family and community support for high-risk pregnant women)”,and“framework of program(acculturalization,means and equipment,security,manpower and the necessary arrangements for the home care)”.Conclusions:High-risk pregnancy affects various aspects of the lives of high-risk pregnant women and their families.Creating the necessary conditions for providing care at home plays a vital role in supporting high-risk pregnant women and removing related obstacles to receiving prenatal care in person.It will be especially beneficial for high-risk pregnant women in low-income areas.展开更多

Home gardens serve as a crucial adaptation strategy for tribal communities in Keonjhar district,Odisha,enhancing food security,income generation,and climate resilience.This study examines the role of home gardens in s...Home gardens serve as a crucial adaptation strategy for tribal communities in Keonjhar district,Odisha,enhancing food security,income generation,and climate resilience.This study examines the role of home gardens in sustaining tribal livelihoods while addressing challenges such as climate variability,soil degradation,and market constraints.Adopting a mixed-methods approach,it integrates the Sustainable Livelihoods Framework(SLF)and socio-ecological resilience theory.Data from 120 tribal households across four villages in Banspal block were collected through structured interviews and focus group discussions.A key innovation lies in integrating indigenous knowledge systems with the Problem Facing Index(PFI)methodology to generate quantified,community-driven insights on climate adaptation barriers.Findings reveal that home gardens contribute around ₹10,000 per season to household income,reducing market dependency by over 70% during crises such as the COVID-19 lockdown.Over 75% of households face high to medium challenges due to climate variability(PFI=252),soil degradation(PFI=251),and wildlife intrusions(PFI=250).Other notable constraints include pests(75%)and poor market access(61.7%).Although 82.5% reported no issue with agricultural inputs,water scarcity remains a seasonal concern for 23%.Despite constraints,home gardens were found to enhance all five SLF capitals:natural,human,social,financial,and physical.The study recommends contextsensitive interventions,including seed distribution,organic inputs,weather-based advisories,and strengthened institutional support.It highlights the potential for scaling home garden models across other marginalized agroecological regions in India and Asia,aligning them with national rural development and climate adaptation programs.展开更多

Objective:To determine the causes of COVID-19 deaths at home.Methods:This descriptive cross-sectional study was conducted on confirmed COVID-19 deaths,which were confirmed to occur at home using the“Electronic Death ...Objective:To determine the causes of COVID-19 deaths at home.Methods:This descriptive cross-sectional study was conducted on confirmed COVID-19 deaths,which were confirmed to occur at home using the“Electronic Death Registration System(EDRS)”database,from February 20,2020,to September 23,2021,in the Fars province,south of Iran.A semi-structured,face-to-face survey consisting of 29 items was filled by the family caregivers,and the information was confirmed based on medical records.Results:One hundred and ninety-three deaths were included,with 60.1%male and 39.9%female.More than 75%of cases had at least one comorbid condition.Death was unexpected for 65.8%of the families and 45.1%of the deceased cases had been medically managed at home during the disease.According to the family caregivers’opinion,the most frequent causes of death at home in their COVID-19 patients were early discharge/release from the hospital(28.5%),physician's reassurance to continue home quarantine/supportive care(14.5%),hopeless/oldest-old patient(11.9%),and family/relative recommendation to not to hospitalize their patient due to fears,myths,etc.(8.8%).Conclusions:In family caregivers’belief,deaths at home might be related to healthcare system preparedness,physicians’knowledge,and public education and empowerment.展开更多

BACKGROUND Cancer patients with an implanted venous access port(IVAP)often manage their care at home during chemotherapy intervals,including maintaining the device,monitoring complications,and following medication ins...BACKGROUND Cancer patients with an implanted venous access port(IVAP)often manage their care at home during chemotherapy intervals,including maintaining the device,monitoring complications,and following medication instructions.Home care ensures continued support after discharge.However,due to factors such as age,gender,culture,psychological status,and family support,the quality of home care varies significantly.Understanding these factors can help provide targeted guidance to improve the care of cancer patients.AIM To explore IVAP chemotherapy on home care quality and its association with mental health and family support for cancer patients.METHODS This investigative study was based on a medical records system.It investigated the relationship between psychological status,family support,and home care quality in 180 patients with cancer undergoing IVAP chemotherapy.Psychological status was assessed using the State Anxiety Inventory(S-AI);family support was assessed using the Perceived Social Support Scale(PSSS),and home care quality was evaluated using the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire(EORTC QLQ-C30).Pearson’s correlation and Structural Equation Modeling were used to analyze the interplay between these factors.RESULTS The average S-AI score was 47.52±14.47,PSSS was 52.48±12.64,and EORTC QLQ-C30 was 70.09±17.32.A substantial inverse relationship was observed between the EORTC QLQ-C30 and S-AI scores(r=-0.712).A significant positive correlation was found between the EORTC QLQ-C30 and the PSSS,with a correlation coefficient of(r=0.744).The multiple linear regression analysis indicated that family social support,psychological status,and average monthly family income were the main factors influencing the variation in the quality of home care,explaining 71.9%of the variation.The Structural Equation Modeling results indicated that psychological status acted as a partial mediator in the association between family social support and home care quality of life,explaining 32.78%of the mediation effect.CONCLUSION Psychological status and family social support positively impacted cancer patients’home care quality,with psychology partially mediating this effect.展开更多

文摘Objective:This study aims to analyze research trends,co-authorship networks,and hotspots over the past decade regarding research on exercise therapy at home for stroke patients.Methods:Articles on exercise therapy at home for stroke patients were routinely searched for in the SCOPUS database from 2013 to 2023.To evaluate and predict the most popular topics and trends in this field,the data collected from the reports are processed using the VOSviewer program.Results:The final analysis covered 1943 articles.The number of publications has steadily increased over the past decade.The United States has made the most significant contributions in this field.The University of Toronto(Canada)and Cramer,S.C.were the most productive institutions and researchers.The journal Disability and Rehabilitation has the highest number of publications(Citescore 4.4;SJR 0.76).The research area in this field is predominantly dominated by medicine.The frequently occurring keywords include“stroke,”“rehabilitation,”and“telerehabilitation.”Innovations,such as telerehabilitation and virtual reality(VR),are emerging as key trends,enhancing patient engagement and accessibility in home-based therapy.Conclusions:Using bibliometric analysis and network visualization,this study summarizes the latest research on home-based exercise therapy for stroke patients,highlighting the impact of innovative technologies,such as telerehabilitation and VR.This analysis identifies research gaps,trends,and popular subjects,providing a comprehensive framework for future studies on key topics,collaborative initiatives,and developmental patterns.
文摘Objective:To explore the perspectives of high-risk pregnant women regarding the home care program for high-risk pregnancies.Methods:This qualitative study was conducted in a university hospital and 5 comprehensive health centers in Ahvaz,Iran,from February 2023 to July 2023.The sampling method used was purposive sampling considering the maximum possible diversity,which continued until data saturation.11 in-depth and semistructured interviews were conducted with high-risk pregnant women.Data were simultaneously analysed using Graneheim and Lundman content analysis by MAXQDA version 20 software.Results:Data analysis extracted 3 themes,12 categories,and 26 subcategories.The themes and categories were“high-risk pregnancy and related issues(pursuing treatment and paying attention to the high-risk situation by pregnant women,psychological aspect of high-risk pregnancy on pregnant women and caregivers,emotional and physical aspects of high-risk pregnancy,the consequences of high-risk pregnancy on the people around them and empowerment of high-risk pregnant women)”,“support dimension(family and community support for high-risk pregnant women)”,and“framework of program(acculturalization,means and equipment,security,manpower and the necessary arrangements for the home care)”.Conclusions:High-risk pregnancy affects various aspects of the lives of high-risk pregnant women and their families.Creating the necessary conditions for providing care at home plays a vital role in supporting high-risk pregnant women and removing related obstacles to receiving prenatal care in person.It will be especially beneficial for high-risk pregnant women in low-income areas.
文摘BACKGROUND Esophageal cancer(EC)is an aggressive malignancy with low survival(10%-30%).Nutritional problems are present throughout the perioperative period and are key to prognosis.Home enteral nutrition appears to improve the nutritional status of patients with EC.Few studies have addressed the experience of patients administering their own nutritional fluids and managing their own feeding tubes at home.The aim of this study was to explore the real-life experience of self-management of feeding tubes in patients at home after EC surgery in the first 3 months after discharge following surgery.AIM To explore feeding tube self-management experience of patients at home 3 months after discharge following EC surgery.METHODS Face-to-face semi-structured interviews were conducted with 18 patients using a feeding tube at home after EC surgery.Thematic analysis of the recordings identified themes related to feeding tube self-management.RESULTS Patients expressed similar feelings about their tube management experiences concerning three contextualized themes:Self-management dilemmas,distressing experiences,and self-management facilitators.CONCLUSION There are many dilemmas and problems in self-managing feeding tubes in postoperative homebound patients with EC.Clinical staff should provide guidance to promote a positive change in self-management behavior.

文摘Home gardens serve as a crucial adaptation strategy for tribal communities in Keonjhar district,Odisha,enhancing food security,income generation,and climate resilience.This study examines the role of home gardens in sustaining tribal livelihoods while addressing challenges such as climate variability,soil degradation,and market constraints.Adopting a mixed-methods approach,it integrates the Sustainable Livelihoods Framework(SLF)and socio-ecological resilience theory.Data from 120 tribal households across four villages in Banspal block were collected through structured interviews and focus group discussions.A key innovation lies in integrating indigenous knowledge systems with the Problem Facing Index(PFI)methodology to generate quantified,community-driven insights on climate adaptation barriers.Findings reveal that home gardens contribute around ₹10,000 per season to household income,reducing market dependency by over 70% during crises such as the COVID-19 lockdown.Over 75% of households face high to medium challenges due to climate variability(PFI=252),soil degradation(PFI=251),and wildlife intrusions(PFI=250).Other notable constraints include pests(75%)and poor market access(61.7%).Although 82.5% reported no issue with agricultural inputs,water scarcity remains a seasonal concern for 23%.Despite constraints,home gardens were found to enhance all five SLF capitals:natural,human,social,financial,and physical.The study recommends contextsensitive interventions,including seed distribution,organic inputs,weather-based advisories,and strengthened institutional support.It highlights the potential for scaling home garden models across other marginalized agroecological regions in India and Asia,aligning them with national rural development and climate adaptation programs.
文摘Bronchiectasis is a chronic inflammatory airway disease,and patients often suffer from recurrent airway infections leading to decreased lung function and impaired quality of life.In this study,the effects of supervised pulmonary rehabilitation training on pulmonary function,training compliance,and quality of life in patients with bronchiectasis under home rehabilitation mode are investigated.Ninety stable patients were selected,and the observation group adopted the home-supervised mode of pulmonary rehabilitation training.The results showed that the observation group’s pulmonary function indexes,quality of life,and training adherence were better than those of the control group.The differences were statistically significant(P<0.05).The supervised pulmonary rehabilitation training in home rehabilitation mode can effectively improve patients’pulmonary function and quality of life,and improve training compliance,which has good clinical application value.

文摘Objective:To determine the causes of COVID-19 deaths at home.Methods:This descriptive cross-sectional study was conducted on confirmed COVID-19 deaths,which were confirmed to occur at home using the“Electronic Death Registration System(EDRS)”database,from February 20,2020,to September 23,2021,in the Fars province,south of Iran.A semi-structured,face-to-face survey consisting of 29 items was filled by the family caregivers,and the information was confirmed based on medical records.Results:One hundred and ninety-three deaths were included,with 60.1%male and 39.9%female.More than 75%of cases had at least one comorbid condition.Death was unexpected for 65.8%of the families and 45.1%of the deceased cases had been medically managed at home during the disease.According to the family caregivers’opinion,the most frequent causes of death at home in their COVID-19 patients were early discharge/release from the hospital(28.5%),physician's reassurance to continue home quarantine/supportive care(14.5%),hopeless/oldest-old patient(11.9%),and family/relative recommendation to not to hospitalize their patient due to fears,myths,etc.(8.8%).Conclusions:In family caregivers’belief,deaths at home might be related to healthcare system preparedness,physicians’knowledge,and public education and empowerment.
文摘This paper focuses on the core challenges of the smart home enterprise ecological collaboration platform,and deeply discusses the absence of a governance mechanism and the inefficiency of the supply chain.The purpose is to improve the overall efficiency by constructing an effective collaborative governance framework and optimizing the supply chain process.It is found that the implementation of multi-agent dynamic contract governance,the construction of an open data sharing middle platform,the introduction of AI-driven elastic supply chain planning,and the establishment of a distributed cloud manufacturing network are the key paths.From the research conclusion,these measures can significantly improve the transparency of cross-agent collaboration,break the data barriers,and achieve the accurate matching of supply and demand,and finally promote the ecological collaboration efficiency of the smart home industry to achieve a substantial leap.
文摘Objective:To explore the construction and application value of a home skin care guidance program for neonates.Methods:From February 2024 to February 2025,60 neonates were selected as samples and randomly grouped by drawing.Group A received the constructed home skin care program for neonates,while Group B received routine care.The mastery of skin care knowledge by parents,emotional scores of mothers,and adverse reaction rates of neonates were compared.Results:The mastery of skin care knowledge by parents in Group A was higher than that in Group B,P<0.05.The anxiety(SAS)and depression(SDS)scores of mothers in Group A were lower than those in Group B,P<0.05.The adverse reaction rate of neonates in Group A was lower than that in Group B,P<0.05.Conclusion:The construction of a home skin care program for neonates can reduce adverse reactions of mothers,improve parents’mastery of neonatal skin care knowledge,and is beneficial for reducing adverse events such as neonatal eczema,diaper dermatitis,and infection.
文摘BACKGROUND Cancer patients with an implanted venous access port(IVAP)often manage their care at home during chemotherapy intervals,including maintaining the device,monitoring complications,and following medication instructions.Home care ensures continued support after discharge.However,due to factors such as age,gender,culture,psychological status,and family support,the quality of home care varies significantly.Understanding these factors can help provide targeted guidance to improve the care of cancer patients.AIM To explore IVAP chemotherapy on home care quality and its association with mental health and family support for cancer patients.METHODS This investigative study was based on a medical records system.It investigated the relationship between psychological status,family support,and home care quality in 180 patients with cancer undergoing IVAP chemotherapy.Psychological status was assessed using the State Anxiety Inventory(S-AI);family support was assessed using the Perceived Social Support Scale(PSSS),and home care quality was evaluated using the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire(EORTC QLQ-C30).Pearson’s correlation and Structural Equation Modeling were used to analyze the interplay between these factors.RESULTS The average S-AI score was 47.52±14.47,PSSS was 52.48±12.64,and EORTC QLQ-C30 was 70.09±17.32.A substantial inverse relationship was observed between the EORTC QLQ-C30 and S-AI scores(r=-0.712).A significant positive correlation was found between the EORTC QLQ-C30 and the PSSS,with a correlation coefficient of(r=0.744).The multiple linear regression analysis indicated that family social support,psychological status,and average monthly family income were the main factors influencing the variation in the quality of home care,explaining 71.9%of the variation.The Structural Equation Modeling results indicated that psychological status acted as a partial mediator in the association between family social support and home care quality of life,explaining 32.78%of the mediation effect.CONCLUSION Psychological status and family social support positively impacted cancer patients’home care quality,with psychology partially mediating this effect.
