The last five years have been an exciting time in the study of esophageal motor disorders due to the recent advances in esophageal function testing. New technologies have emerged, such as intraluminal impedance, while conventional techniques, such as manometry, have enjoyed many improvements due to advances in transducer technology, computerization and graphic data presentation. While these techniques provide more detailed information regarding esophageal function, our understanding of whether they can improve our ability to diagnose and treat patients more effectively is evolving. These techniques are also excellent research tools and they have added substantially to our understanding of esophageal motor function in dysphagia. This review describes the potential benefits that these new technologies may have over conventional techniques for the evaluation of dysphagia. 展开更多

BACKGROUND The quality of life in patients who develop low anterior resection syndrome(LARS)after surgery for mid-low rectal cancer is seriously impaired.The underlying pathophysiological mechanism of LARS has not been fully investigated.AIM To assess anorectal function of mid-low rectal cancer patients developing LARS perioperatively.METHODS Patients diagnosed with mid-low rectal cancer were included.The LARS score was used to evaluate defecation symptoms 3 and 6 mo after anterior resection or a stoma reversal procedure.Anorectal functions were assessed by threedimensional high resolution anorectal manometry preoperatively and 3-6 mo after surgery.RESULTS The study population consisted of 24 patients.The total LARS score was decreased at 6 mo compared with 3 mo after surgery(P<0.05),but 58.3%(14/24)lasted as major LARS at 6 mo after surgery.The length of the high-pressure zone of the anal sphincter was significantly shorter,the mean resting pressure and maximal squeeze pressure of the anus were significantly lower than those before surgery in allpatients (P < 0.05), especially in the neoadjuvant therapy group after surgery (n = 18). The focalpressure defects of the anal canal were detected in 70.8% of patients, and those patients had higherLARS scores at 3 mo postoperatively than those without focal pressure defects (P < 0.05). Spasticperistaltic contractions from the new rectum to anus were detected in 45.8% of patients, whichwere associated with a higher LARS score at 3 mo postoperatively (P < 0.05).CONCLUSIONThe LARS score decreases over time after surgery in the majority of patients with mid-low rectalcancer. Anorectal dysfunctions, especially focal pressure defects of the anal canal and spasticperistaltic contractions from the new rectum to anus postoperatively, might be the majorpathophysiological mechanisms of LARS. 展开更多

Distal esophageal spasm (DES) is a rare major motility disorder in the Chicago dassification of esophageal motility disorders (CC).DES is diagnosed by finding of≥20%premature contractions,with normal lower esophageal sphincter (LES)relaxation on high-resolution manometry (HRM) in the latest version of CCv3.0.This feature differentiates it from achalasia type 3,which has an elevated LES relaxation pressure.Like other spastic esophageal disorders,DES has been linked to conditions such as gastroesophageal reflux disease,psychiatric conditions,and narcotic use.In addition to HRM,ancillary tests such as endoscopy and barium esophagram can provide supplemental information to differentiate DES from other conditions.Functional lumen imaging probe (FLIP),a new cutting-edge diagnostic tool,is able to recognize abnormal LES dysfunction that can be missed by HRM and can further guide LES targeted treatment when esophagogastric junction outflow obstruction is diagnosed on FLIP.Medical treatment in DES mostly targets symptomatic relief and often fails.Botulinum toxin injection during endoscopy may provide a temporary therapy that wears off over time.Myotomy through peroral endoscopic myotomy or via surgical Heller myotomy can provide long term relief in cases with persistent symptoms. 展开更多

Many patients are successfully treated with lifestyle modifications andproton pump inhibitor therapy, but a subset of patients require more aggressiveintervention for control of their symptoms. Surgical treatment with fundoplicationis a viable option for patients with GERD, as it attempts to improve the integrityof the lower esophageal sphincter (LES). While surgery can be as effective asmedical treatment, it can also be associated with side effects such as dysphagia,bloating, and abdominal pain. Therefore, a thorough pre-operative assessment iscrucial to select appropriate surgical candidates. Newer technologies arebecoming increasingly available to help clinicians identify patients with true LESdysfunction, such as pH-impedance studies and high-resolution manometry(HRM). Pre-operative evaluation should be aimed at confirming the diagnosis ofGERD, ruling out any major motility disorders, and selecting appropriate surgicalcandidates. HRM and pH testing are key tests to consider for patients with GERDlike symptoms, and the addition of provocative measures such as straight legraises and multiple rapid swallows to HRM protocol can assess the presence ofunderlying hiatal hernias and to test a patient’s peristaltic reserve prior tosurgery. 展开更多

Background:Diabetic foot,a severe complication of diabetes,is characterized by chronic refractory wounds.Sanhuang Oil,a topical herbal formula,demonstrates significant therapeutic effects including antibacterial,anti-inflammatory,and immunomodulatory activities.However,its active constituents and mechanisms of action against diabetic foot remain to be elucidated.Methods:In this study,the chemical constituents of Sanhuang Oil were identified using UPLC-QE-Orbitrap-MS.Subsequently,the mechanism by which Sanhuang Oil promotes diabetic foot ulcer healing was predicted by integrating network pharmacology and molecular docking.Additionally,diabetic mouse model was established in ICR mice using a combination of a high-fat diet(HFD)and streptozotocin(STZ)chemical induction.A full-thickness skin defect was created on the dorsum of the mice.Wound healing and the healing rate were observed following Sanhuang Oil intervention.The mechanism underlying Sanhuang Oil’s promotion of diabetic ulcer healing was further investigated using transcriptomics and histopathological examination(H&E staining).Results:A total of 97 active ingredients were identified from Sanhuang Oil.Network pharmacology analysis predicted 543 common targets,and Kyoto Encyclopedia of Genes and Genomes(KEGG)pathway enrichment analysis identified 203 relevant pathways.Molecular docking further confirmed high binding affinity(binding energy≤−5.0 kcal/mol)between specific active components in Sanhuang Oil(e.g.,coptisine,phellodendrine,baicalein)and key targets associated with diabetic foot ulcers(e.g.,EGFR,AKT1,STAT3).In vivo experimental results demonstrated that the wound healing rate was significantly higher in Sanhuang Oil-treated groups compared to the model group(P<0.001).HE staining revealed that the high-dose Sanhuang Oil group exhibited more pronounced epithelial tissue coverage over the wound,reduced inflammatory cell infiltration,and increased collagen deposition and fibroblast proliferation.transcriptomic analysis identified Pdk4,Ttn,Csrp3,Actn2,Myoz2,Tnnc2,Myod1,Myog,Myot,and Myf6 as key regulatory proteins involved in promoting wound healing.Conclusion:Sanhuang Oil promotes wound healing in diabetic ulcer mice,potentially by mitigating inflammation and regulating key targets such as Pdk4 to enhance fibroblast function.These findings provide novel insights into the multi-target,multi-pathway mechanism of Sanhuang Oil for treating diabetic foot ulcers. 展开更多

The detailed process and mechanism of colonic motility are still unclear, and colonic motility disorders are associated with numerous clinical diseases. Colonic manometry is considered to the most direct means of evaluating colonic peristalsis. Colonic manometry has been studied for more than 30 years;however, the long duration of the examination, high risk of catheterization, huge amount of real-time data, strict catheter sterilization, and high cost of disposable equipment restrict its wide application in clinical practice. Recently, highresolution colonic manometry (HRCM) has rapidly developed into a major technique for obtaining more effective information involved in the physiology and/or pathophysiology of colonic contractile activity in colonic dysmotility patients. This review focuses on colonic motility, manometry, operation, and motor patterns, and the clinical application of HRCM. Furthermore, the limitations, future directions, and potential usefulness of HRCM in the evaluation of clinical treatment effects are also discussed. 展开更多

High-resolution transmission electron microscopy(HRTEM)promises rapid atomic-scale dynamic structure imaging.Yet,the precision limitations of aberration parameters and the challenge of eliminating aberrations in Cs-corrected transmission electron microscopy constrain resolution.A machine learning algorithm is developed to determine the aberration parameters with higher precision from small,lattice-periodic crystal images.The proposed algorithm is then validated with simulated HRTEM images of graphene and applied to the experimental images of a molybdenum disulfide(MoS_(2))monolayer with 25 variables(14 aberrations)resolved in wide ranges.Using these measured parameters,the phases of the exit-wave functions are reconstructed for each image in a focal series of MoS_(2)monolayers.The images were acquired due to the unexpected movement of the specimen holder.Four-dimensional data extraction reveals time-varying atomic structures and ripple.In particular,the atomic evolution of the sulfur-vacancy point and line defects,as well as the edge structure near the amorphous,is visualized as the resolution has been improved from about 1.75?to 0.9 A.This method can help salvage important transmission electron microscope images and is beneficial for the images obtained from electron microscopes with average stability. 展开更多

BACKGROUND Little is known about the clinical significance of upper esophageal sphincter(UES)motility disorders and their association with the treatment response of typeⅡachalasia.None of the three versions of the Chicago Classification of Esophageal Motility Disorders has defined UES abnormality metrics or their function.UES abnormalities exist in some patients and indicate a clinically significant problem in patients with achalasia.AIM To demonstrate the manometric differentiation on high-resolution esophageal manometry between subjects with abnormal UES and normal UES,and the association between UES type and the treatment response of typeⅡachalasia.METHODS In total,498 consecutive patients referred for high-resolution esophageal manometry were analyzed retrospectively.The patients were divided into two groups,those with normal and abnormal UES function.UES parameters were analyzed after determining lower esophageal sphincter(LES)function.Patients with typeⅡachalasia underwent pneumatic dilation for treatment.Using mixed model analyses,correlations between abnormal UES and treatment response were calculated among subjects with typeⅡachalasia.RESULTS Of the 498 consecutive patients,246(49.40%)were found to have UES abnormalities.Impaired relaxation alone was the most common UES abnormality(52.85%,n=130).The incidence rate of typeⅡachalasia was significantly higher in subjects with abnormal UES than those with normal UES(9.77%vs 2.58%,P=0.01).After pneumatic dilation,LES resting pressure,LES integrated relaxation pressure,and UES residual pressure were significantly decreased(41.91±9.20 vs 26.18±13.08,38.94±10.28 vs 16.71±5.65,and 11.18±7.93 vs 5.35±4.77,respectively,P<0.05).According to the Eckardt score,subjects with typeⅡachalasia and abnormal UES presented a significantly poorer treatment response than those with normal UES(83.33%vs 0.00%,P<0.05).CONCLUSION Impaired relaxation alone is the most common UES abnormality.The incidence of typeⅡachalasia is associated with abnormal UES.TypeⅡachalasia with abnormal UES has a poorer treatment response,which is a potentially prognostic indicator of treatment for this disease. 展开更多

Semi-Solid and Solid Bolus Swallows in High-Resolution Oesophageal Manometry for the Detection of Motility Disorders

Background/Aims: High-resolution oesophageal manometry utilises water swallows to evaluate oesophageal function. However, small volumes of water are not representative of normal eating and as a result often produce normal manometry studies in patients with dysphagia. This study sets out to establish optimal diagnostic thresholds for semi-solid solid swallows and evaluate their ability to uncover motility abnormalities in patients with motility disorders. Method: Manometry was performed using ten 5-mL single water swallows followed by two semi-solid and two solid swallows in the upright position. Normative values for the adjunctive tests were obtained from patient controls while patients with major motility disorders were used to establish the optimal diagnostic thresholds. Diagnostic thresholds identified were prospectively tested in patients with normal water swallows but oesophagus related symptoms and in those with minor and major motility disorders. Results: Normal values for semi-solid and solid were determined in patient controls (n = 100). Development of diagnostic thresholds included 120 patients with major motility disorders. Optimal diagnostic thresholds identified for oesophagogastric junction dysfunction in semi-solid and solid swallows (IRP > 15.5 mmHg). Hypercontractilty and spasm used existing thresholds (>8000 mmHg-s-cm and < 4.5 s, respectively) but modified frequency of ≥50% of adjunctive swallows. Diagnostic thresholds were applied to symptomatic patients with normal water swallows (n = 70) identifying 12/70 (17%) to have abnormal adjunctive swallows. One of 30 patients (3%) with ineffective motility had abnormal adjunctive swallow and 12 patients with oesophageal spasm, oesophagogastric junction obstruction, and hypercontractility had abnormal adjunctive swallows that moved them up the motility disorder hierarchy. Introduction: Children with surgically repaired esophageal atresia (EA) show esophageal dysmotility. Due to the performance of high-resolution manometry (HRM), three motility alteration patterns have been described, which allowed to know the segmental alterations. Objective: To describe the esophageal motility patterns found through HRM in teenagers with EA and to relate these with the associated esophageal pathology and its severity. Materials and Method: Ten teenagers were included with no history of esophageal blockage or dilations in the last six months, who were orally fed and asymptomatic. Through performance of HRM, we found surgical and endoscopic history, as well as of esophageal biopsies and pH monitoring. Results: We found the following patterns: aperistalsis, pressurization and distal contraction. 70% showed distal contraction, and 100% of esophageal endoscopies and biopsies were normal. 57% of the esophageal pH monitoring analyzed was pathologic. In the pressurization and aperistalsis groups, we observed severe esophagitis and requirement of Nissen antireflux procedure in 100% of the cases. Esophageal pH monitoring analyzed was 100% pathologic. Conclusion: We described the esophageal segmental alterations in teenagers with atresia by means of HRM. The distal contraction group showed better development, without severe esophagitis or requirement of antireflux procedure. Therefore, by performing an HRM in teenagers with EA, we could predict the future esophageal behavior, according to the peristaltic pattern, since there are significant differences among the groups in study. 展开更多

The objective of image-based virtual try-on is to seamlessly integrate clothing onto a target image, generating a realistic representation of the character in the specified attire. However, existing virtual try-on methods frequently encounter challenges, including misalignment between the body and clothing, noticeable artifacts, and the loss of intricate garment details. To overcome these challenges, we introduce a two-stage high-resolution virtual try-on framework that integrates an attention mechanism, comprising a garment warping stage and an image generation stage. During the garment warping stage, we incorporate a channel attention mechanism to effectively retain the critical features of the garment, addressing challenges such as the loss of patterns, colors, and other essential details commonly observed in virtual try-on images produced by existing methods. During the image generation stage, with the aim of maximizing the utilization of the information proffered by the input image, the input features undergo double sampling within the normalization procedure, thereby enhancing the detail fidelity and clothing alignment efficacy of the output image. Experimental evaluations conducted on high-resolution datasets validate the effectiveness of the proposed method. Results demonstrate significant improvements in preserving garment details, reducing artifacts, and achieving superior alignment between the clothing and body compared to baseline methods, establishing its advantage in generating realistic and high-quality virtual try-on images. 展开更多

[Objective]The paper aimed to effectively reduce the occurrence of bacterial resistance associated with breeding practices and to mitigate food safety risks by controlling the illegal use of veterinary drugs in self-formulated feed at the source.[Method]A screening database comprising 274 illegally added chemical drugs in self-formulated feed was established utilizing ultra-performance liquid chromatography coupled with quadrupole/electrostatic field orbitrap high-resolution mass spectrometry(HPLC-Q-Exactive Focus/MS).Subsequently,253 batches of self-formulated feed samples from various farms in Hebei Province were screened and quantitatively analyzed.[Result]The screening results indicated the presence of 8 pharmaceutical components across 10 batches of self-formulated feed samples,with a detection rate of 3.2%and concentrations ranging from 0.06 to 28851.8μg/g.[Conclusion]The application of high-resolution mass spectrometry is feasible and highly significant for the risk monitoring of illegally added drugs in self-formulated feed. 展开更多

In oceanic and atmospheric science,finer resolutions have become a prevailing trend in all aspects of development.For high-resolution fluid flow simulations,the computational costs of widely used numerical models increase significantly with the resolution.Artificial intelligence methods have attracted increasing attention because of their high precision and fast computing speeds compared with traditional numerical model methods.The resolution-independent Fourier neural operator(FNO)presents a promising solution to the still challenging problem of high-resolution fluid flow simulations based on low-resolution data.Accordingly,we assess the potential of FNO for high-resolution fluid flow simulations using the vorticity equation as an example.We assess and compare the performance of FNO in multiple high-resolution tests varying the amounts of data and the evolution durations.When assessed with finer resolution data(even up to number of grid points with 1280×1280),the FNO model,trained at low resolution(number of grid points with 64×64)and with limited data,exhibits a stable overall error and good accuracy.Additionally,our work demonstrates that the FNO model takes less time than the traditional numerical method for high-resolution simulations.This suggests that FNO has the prospect of becoming a cost-effective and highly precise model for high-resolution simulations in the future.Moreover,FNO can make longer high-resolution predictions while training with less data by superimposing vorticity fields from previous time steps as input.A suitable initial learning rate can be set according to the frequency principle,and the time intervals of the dataset need to be adjusted according to the spatial resolution of the input when training the FNO model.Our findings can help optimize FNO for future fluid flow simulations. 展开更多

Understanding vegetation water availability can be important for managing vegetation and combating climate change.Changes in vegetation water availability throughout China remains poorly understood,especially at a high spatial resolution.Standardized Precipitation Evapotranspiration Index(SPEI)is an ideal water availability index for assessing the spatiotemporal characteristics of drought and investigating the vegetation-water availability relationship.However,no high-resolution and long-term SPEI datasets over China are available.To fill this gap,we developed a new model based on machine learning to obtain high-resolution(1 km)SPEI data by combining climate variables with topographical and geographical features.Here,we analyzed the long-term drought over the past century(1901–2020)and vegetation-water availability relationship in the past two decades(2000–2020).The century-long drought trend analyses indicated an overall drying trend across China with increasing drought frequency,duration,and severity during the past century.We found that drought events in 1901–1961 showed a larger increase than that in 1961–2020,with the Qinghai-Xizang Plateau showing a significant drying trend during 1901–1960 but a wetting trend during 1961–2020.There were 13.90%and 28.21%of vegetation in China showing water deficit and water surplus respectively during 2000–2020.The water deficit area significantly shrank from 2000 to 2020 across China,which is dominated by the significant decrease in water deficit areas in South China.Among temperature,precipitation,and vegetation abundance,temperature is the most important factor for the vegetation-water availability dynamics in China over the past two decades,with high temperature contributing to water deficit.Our findings are important for water and vegetation management under a warming climate. 展开更多

BACKGROUND Rectal cancer is a common malignant tumor of the digestive system,with older patients representing the predominantly affected population.Magnetic resonance imaging(MRI)has been widely applied in preoperative tumor assessment;however,the value of high-resolution MRI(HR-MRI)combined with dynamic contrast-enhanced(DCE)scanning in the preoperative diagnosis of rectal cancer in older patients remains unclear.AIM To evaluate the value of HR-MRI combined with DCE scanning in the preoperative diagnosis of rectal cancer in older patients.METHODS This retrospective study included 148 consecutive older female patients with rectal cancer who were treated at our hospital between December 2020 and December 2024.Clinical data and HR-MRI and DCE scan findings were collected.Histopathological examination after surgical resection served as the gold standard.The diagnostic accuracy of MRI for preoperative T and N staging was calculated.Consistency,sensitivity,and specificity between HR-MRI combined with DCE scanning and pathological staging were analyzed using the k test.Among the 148 patients,the overall accuracy of T staging was 84.5%.Sensitivity for T1,T2,T3,and T4 staging was 75.00%,62.50%,89.47%,and 90.48%,respectively,whereas specificity was 100.00%,94.35%,79.25%,and 96.06%,respectively.T staging based on HR-MRI combined with DCE scanning showed good agreement with pathological staging(k=0.8176,P<0.001).For N staging,sensitivity and specificity were 54.88%and 84.85%for N0,36.96%and 72.55%for N1,and 70.00%and 73.44%for N2,respectively;agreement with pathological N staging was poor(k=0.259,P<0.001).CONCLUSION HR-MRI combined with DCE scanning demonstrates high diagnostic accuracy for T staging of rectal cancer in older patients and can provide a theoretical basis for treatment planning.However,its diagnostic accuracy for N staging requires improvement. 展开更多

High-resolution sub-meter satellite data play an increasingly crucial role in the 3D real-scene China construction initiative.Current research on 3D reconstruction using high-resolution satellite data primarily focuses on two approaches:Multi-stereo fusion and multi-view matching.While algorithms based on these two methodologies for multi-view image 3D reconstruction have reached relative maturity,no systematic comparison has been conducted specifically on satellite data to evaluate the relative merits of multi-stereo fusion versus multi-view matching methods.This paper conducts a comparative analysis of the practical accuracy of both approaches using high-resolution satellite datasets from diverse geographical regions.To ensure fairness in accuracy comparison,both methodologies employ non-local dense matching for cost optimization.Results demonstrate that the multi-stereo fusion method outperforms multi-view matching in all evaluation metrics,exhibiting approximately 1.2%higher average matching accuracy and 10.7%superior elevation precision in the experimental datasets.Therefore,for 3D modeling applications using satellite data,we recommend adopting the multi-stereo fusion approach for digital surface model(DSM)product generation. 展开更多

During drilling operations,the low resolution of seismic data often limits the accurate characterization of small-scale geological bodies near the borehole and ahead of the drill bit.This study investigates high-resolution seismic data processing technologies and methods tailored for drilling scenarios.The high-resolution processing of seismic data is divided into three stages:pre-drilling processing,post-drilling correction,and while-drilling updating.By integrating seismic data from different stages,spatial ranges,and frequencies,together with information from drilled wells and while-drilling data,and applying artificial intelligence modeling techniques,a progressive high-resolution processing technology of seismic data based on multi-source information fusion is developed,which performs simple and efficient seismic information updates during drilling.Case studies show that,with the gradual integration of multi-source information,the resolution and accuracy of seismic data are significantly improved,and thin-bed weak reflections are more clearly imaged.The updated seismic information while-drilling demonstrates high value in predicting geological bodies ahead of the drill bit.Validation using logging,mud logging,and drilling engineering data ensures the fidelity of the processing results of high-resolution seismic data.This provides clearer and more accurate stratigraphic information for drilling operations,enhancing both drilling safety and efficiency. 展开更多

A novel approach is proposed to optimize the weighted HRRP. In the approach, the separability of weighted HRRPs in different targets is measured by de- signing an objective function, and the weighted coefficients are computed by using the gradient descent method, thus enhancing the influence of stable range cells. Simulation results based on five aircraft models show that the approach can effectively optimize the weighted HRRP and improve the recognition accuracy. 展开更多

BACKGROUND Pseudoachalasia mimics primary achalasia in symptoms and diagnostic findings,as observed in gastroscopy and barium swallow studies.However,pseudoachalasia,often associated with malignancies like metastatic breast cancer,requires prompt differentiation to avoid misdiagnosis and inappropriate treatment.This report highlights a rare case of pseudoachalasia secondary to metastatic breast cancer and highlights the diagnostic value of esophageal motility changes.CASE SUMMARY A 52-year-old woman presented with a one-year history of intermittent dysphagia following breast cancer surgery.Initial examinations suggested achalasia,but the patient’s high-resolution manometry(HRM)results showed a rapid shift from ineffective esophageal motility to type Ⅱ achalasia within four months.Further investigations revealed metastatic adenocarcinoma of the cardia,originating from the breast.CONCLUSION In patients with a history of malignancy,rapidly evolving esophageal motility abnormalities should raise suspicion of pseudoachalasia.HRM plays a crucial role in differentiating between primary and secondary achalasia.Early diagnosis through advanced imaging and pathology is essential for proper management. 展开更多
