Background This study compared knee osteoarthritis(OA)outcomes specific to pain,physical function,and quality of life in later life based on strength training(ST)participation over a lifetime.Methods Participants from...Background This study compared knee osteoarthritis(OA)outcomes specific to pain,physical function,and quality of life in later life based on strength training(ST)participation over a lifetime.Methods Participants from the Osteoarthritis Initiative(n=3192)were grouped by ST engagement during ages 12–18 years,19–34 years,35–49 years,and 50+years.Participants were categorized as:No ST(no ST at any point;61.7±9.0 years(mean±SD)),Some ST(engaged in ST during 1–3 life stages;58.9±8.7 years),and Lifelong ST(consistently engaged in ST across all life stages;55.6±8.1 years).Measures were collected at baseline and Year 4:Western Ontario and McMaster Universities Osteoarthritis Index Scores(WOMAC;pain,daily activities),Knee Injury and Osteoarthritis Outcome Score(KOOS;sports,recreation),Physical Activity Score for the Elderly(PASE),Short Form-12 Physical Component Score(SF-12 PCS),mobility disability,chair rise time,and walking speed(20 m and 400 m).Results At Year 4,the Lifelong ST group reported better WOMAC activity scores in the right knee along with better WOMAC pain,KOOS sports/recreation,and PASE scores compared to other groups(p<0.05).The Lifelong ST group had the lowest incidence of mobility disability of all groups(0.8%vs.2.3%–4.1%;p=0.015)and maintained the fastest walking speeds in Year 4.Conclusion For those with knee OA,ST throughout life may help preserve function and mobility,allowing for greater physical activity engagement while keeping pain levels relatively lower.展开更多

Objective To compare the influences of acupuncture combined with rehabilitative training and simple rehabilitative training on neural functional recovery of stroke patients during recovery phase.Methods Sixty-seven st...Objective To compare the influences of acupuncture combined with rehabilitative training and simple rehabilitative training on neural functional recovery of stroke patients during recovery phase.Methods Sixty-seven stroke patients during recovery phase were randomly divided into an acupuncture combined with rehabilitative training group(group A) and a simple rehabilitative training group(group B) through random number table method,with 34 cases and 33 cases respectively;33 cases and 31 cases were respectively completed actually other than cases dropped out during the observation.Rehabilitation methods such as motion control and functional exercise training were primarily used for rehabilitative training,once daily for 45 min at a time,and 6 times a week;Baihui(百会 GV 20) and Fengchi(风池 GB 20) on the head and relevant acupoints on limbs were selected for acupuncture,once every other day,and 3 times a week.Both groups were treated for 8 weeks,National Institnte of Health Stroke Scale(NIHSS) was used to assess neurologic impairment,Fugl-Meyer scale was used to assess motor function,Barthel was used to assess activity of daily living,and comprehensive assessment was finally made to patients.Results NIH score,Fugl-Meyer scale scores of upper and lower limbs,Barthel score,and comprehensive assessment of group A after treatment were respectively:12.78±2.99,37.55±2.09 and25.67±3.93,63.76±4.65,11.33±1.98,superior to 16.06±4.57,28.71±4.07 and20.41±2.98,53.8214.35,15.18±1.72 of group B.Indices of both groups were significantly improved compared with those before treatment(P〈0.01,P〈0.05),but improvement of indices of group A was more obvious that that of group B after treatment subject to comparison(all P〈0.05).Conclusions Acupuncture combined with rehabilitative training has definite therapeutic effect on recovery of various functions such as neural functional recovery of stroke patients during recovery phase,obviously superior to simple rehabilitative training.展开更多
Stroke patients experience varying degrees of upper limb functional impairment.Although bilateral arm training can help stroke patients recover movement after stroke,little is known about the way in which the brain an...Stroke patients experience varying degrees of upper limb functional impairment.Although bilateral arm training can help stroke patients recover movement after stroke,little is known about the way in which the brain and muscles work together during this type of training.To address this,we conducted a cross-sectional study at The Seventh Affiliated Hospital,Sun Yat-sen University in China,where we observed the connection between brain and muscle activity during bilateral upper limb training in 21 stroke patients and 17 healthy controls.We used functional near-infrared spectroscopy and surface electromyography to measure changes in cerebral cortex oxygenation and upper limb muscle contraction signals,respectively.The results showed that,compared with the healthy control group,stroke patients had reduced functional connectivity and more irregular muscle activity in the affected flexor muscle during bilateral upper limb training.Moreover,we found a significant correlation between the surface electromyographic signal characteristics of upper limb muscles and cerebral oxygenation indicators of multiple brain regions in stroke patients.These findings indicate that bilateral upper limb training is an effective rehabilitation method that improves upper limb motor function in stroke patients by promoting brain functional connectivity and improving muscle activity patterns.展开更多
Impairments on body function,activities of daily living(ADL)and cognition are common after stroke.Eccentric resistance training(ERT)may be implemented to improve them.The primary objectives were to evaluate whether ER...Impairments on body function,activities of daily living(ADL)and cognition are common after stroke.Eccentric resistance training(ERT)may be implemented to improve them.The primary objectives were to evaluate whether ERT improves body function,ADL and cognition after stroke.The secondary objectives were to evaluate whether ERT improves strength,gait,quality of life,and self-perceived health.Seven electronic databases were searched:Cochrane Central Register of Controlled Trials,Cochrane Stroke Group Trials Register,PubMed,Epistemonikos,Embase,SPORTDiscus,and WHO International Clinical Trials Registry Platform.Last search was run in December 2023,including studies since 2012.Selection criteria were studies with stroke participants of both sexes,aged 18 or more,with an intervention based on ERT.Accepted languages were English,Spanish or French.First search was done in pairs.Authors removed duplicate studies and those which did not meet inclusion criteria through title and abstracts.Finally,all authors,independently,screened the final search results and extracted data.Of 68 records identified,15 were eligible and 6 were finally included(with moderate risk of bias),analyzing 159 participants.Body function(4 interventions,n=84),gait(4 interventions,n=115)and strength(3 interventions,n?78)showed significantly better results when ERT was performed.Meta-analysis could not be done because of the few studies and their heterogeneity.This review provided low-moderate quality evidence suggesting that ERT might be effective at improving body function,strength,and gait after stroke.Besides,no harm was documented,and it was well-accepted.展开更多

However, the underlying mechanism remains un- clear. In this study, 18 healthy subjects and five patients after subacute stroke were included. The five patients were scanned using functional MRI prior to training, 3 weeks after training and at a 12-week follow-up, and then compared with healthy subjects. The FugI-Meyer Assessment and Wolf Motor Function Test scores of the hemiplegic upper limbs of stroke patients were significantly increased 3 weeks after training and at the 12-week follow-up. Functional MRI results showed that contralateral primary sensorimotor cortex was activated after Kinect-based virtual reality training in the stroke patients compared with the healthy subjects. Contralateral primary sensorimotor cortex, the bilateral supplementary motor area and the ipsilateral cerebellum were also activated during hand-clenching in all 18 healthy subjects. Our findings indicate that Kinect-based virtual reality training could promote the recovery of upper limb motor function in subacute stroke patients, and brain reorganization by Kinect-based virtual reality training may be linked to the contralateral sen- sorimotor cortex.展开更多

The purpose of this study was to examine the effects of high-intensity interval training(HIIT) on the acute regulation of the mRNA expression of several myokines, including the prototypical myokine interleukin-6(IL-6), and recently identified myokines fibronectin type III domain-containing protein 5(FNDC5)(irisin) and meteorin-like protein(METRNL).Methods: Both before and after a 20-day period of twice-daily high-volume HIIT, 9 healthy males(20.5 ± 1.5 years performed a standardized bout of high-intensity interval exercise(HIIE; 5 × 4 min at ~80% pretraining peak power output) with skeletal muscle biopsy samples(vastus lateralis) obtained at rest, immediately following exercise, and at 3 h recovery.Results: Before training, a single bout of HIIE increased IL-6(p < 0.05) and METRNL(p < 0.05) mRNA expression measured at 3 h recovery when compared to rest. Following 20 days of HIIT, IL-6 and FNDC5 mRNA were increased at 3 h recovery from the standardized HIIE bout when compared to rest(both p < 0.05). Resting METRNL and FNDC5 mRNA expression were higher following training(p < 0.05), and there was an overall increase in FNDC5 mRNA post-training(main effect of training, p < 0.05).Conclusion: In human skeletal muscle(1) an acute bout of HIIE can induce upregulation of skeletal muscle IL-6 mRNA both before and after a period of intensified HIIT;(2) Resting and overall FNDC5 mRNA expression is increased by 20 days of HIIT; and(3) METRNL mRNA expression is responsive to both acute HIIE and short-term intense HIIT. Future studies are needed to confirm these findings at the protein and secretion level in humans.展开更多

This is true even for people with various co...Aerobic capacity, which is expressed as peak oxygen consumption (VO2peak), is well-known to be an independent predictor of all-cause mortality and cardiovascular prognosis. This is true even for people with various coronary risk factors and cardiovascular diseases. Although exercise training is the best method to improve VO2peak, the guidelines of most academic societies recommend 150 or 75 min of moderate- or vigorous- intensity physical activities, respectively, every week to gain health benefits. For general health and primary and secondary cardiovascular prevention, high-intensity interval training (HIIT) has been recognized as an efficient exercise protocol with short exercise sessions. Given the availability of the numerous HIIT protocols, which can be classified into aerobic HIIT and anaerobic HIIT [usually called sprint interval training (SIT)], professionals in health-related fields, including primary physicians and cardiologists, may find it confusing when trying to select an appropriate protocol for their patients. This review describes the classifications of aerobic HIIT and SIT, and their differences in terms of effects, target subjects, adaptability, working mechanisms, and safety. Understanding the HIIT protocols and adopting the correct type for each subject would lead to better improvements in VO2peak with higher adherence and less risk.展开更多

The search was conducted using the key words "high-intensity intermittent training" or "high-intensity interval exercise" or "interval running" or "sprint interval training" and "endurance runners" or "long distance runners". A systematic approach was used to evaluate the 783 articles identified for initial review. Studies were included if they investigated HIIT in recreational endurance runners. The methodological quality of the studies was evaluated using the Physiotherapy Evidence Database(PEDro) scale(for intervention studies) and the modified Downs and Black Quality Index(for cross-sectional studies).Results: Twenty-three studies met the inclusionary criteria for review. The results are presented in 2 parts: cross-sectional(n = 15) and intervention studies(n = 8). In the 15 cross-sectional studies selected, endurance runners performed at least 1 HIIT protocol, and the acute impact on physiological, neuromuscular, metabolic and/or biomechanical variables was assessed. Intervention studies lasted a minimum of 4 weeks, with 10 weeks being the longest intervention period, and included 2 to 4 HIIT sessions per week. Most of these studies combined HIIT sessions with continuous run(CR) sessions; 2 studies' subjects performed HIIT exclusively.Conclusion: HIIT-based running plans(2 to 3 HIIT sessions per week, combining HIIT and CR runs) show athletic performance improvements in endurance runners by improving maximal oxygen uptake and running economy along with muscular and metabolic adaptations. To maximize the adaptations to training, both HIIT and CR must be part of training programs for endurance runners.展开更多
BACKGROUND The recovery of limb function after ankle fracture surgery is a gradual process.The main purpose of implementing early functional exercise,joint mobility,muscle contraction function,passive ankle flexion an...BACKGROUND The recovery of limb function after ankle fracture surgery is a gradual process.The main purpose of implementing early functional exercise,joint mobility,muscle contraction function,passive ankle flexion and extension exercises,or physical factor therapy techniques is to achieve the rapid recovery of normal physiological limb function.However,currently the most effective rehabilitation training method is staged limb functional exercise,which promotes rapid recovery of limb function while preventing adverse consequences caused by overwork or insufficient training.Staged limb functional exercise divides the rehabilitation process into multiple stages,each of which has specific training objectives and contents.This method helps patients gradually restore limb function.Nevertheless,some patients still exhibit poor limb function after standardized exercise.Therefore,a functional evaluation should be performed to analyze the impact of staged functional training after ankle fracture surgery.AIM To perform a functional evaluation and determine the influencing factors of staged functional training in patients with ankle fracture.METHODS A retrospective study enrolled 150 patients who underwent surgical treatment for ankle fracture from May 2020 to May 2022 at our hospital.Univariate and multivariate linear regression analyses were performed on general data,functional exercise compliance scale for orthopedic patients,Social Support Rating Scale(SSRS),American Orthopedic Foot and Ankle Score(AOFAS)Ankle-Hindfoot Score,and pain factors[serum bradykinin(BK),prostaglandin E2(PGE2),5-hydroxytryptamine(5-HT)].RESULTS Based on the AOFAS Ankle-Hindfoot Scale,the cases were divided into the excellent function(n=111)and ordinary function(n=39)groups.Univariate analysis revealed that monthly family income,education level,diabetes mellitus,functional exercise compliance scale of orthopedic patients score,SSRS,BK,PGE2,and 5-HT significantly influenced limb function after ankle fracture(P<0.05);Multiple linear regression analysis showed that the functional exercise compliance scale score,SSRS,BK,PGE2,and 5-HT were independent risk factors affecting functional performance after staged functional exercise(P<0.05).CONCLUSION Exercise compliance,SSRS,and pain level are the independent risk factors affecting functional performance after staged functional training following ankle surgery.Clinical nursing care after ankle surgery should include analgesic and health education measures to ensure optimal recovery of limb function.展开更多

How to facilitate this plasticity h...Following central nervous system injury, axonal sprouts form distal to the injury site and extend into the denervated area, reconstructing neural circuits through neural plasticity. How to facilitate this plasticity has become the key to the success of central nervous system repair. It remains controversial whether fine motor skill training contributes to the recovery of neurological function after spinal cord injury. Therefore, we established a rat model of unilateral corticospinal tract injury using a pyramidal tract cutting method. Horizontal ladder crawling and food ball grasping training procedures were conducted 2 weeks before injury and 3 days after injury. The neurological function of rat forelimbs was assessed at 1, 2, 3, 4, and 6 weeks after injury. Axon growth was observed with biotinylated dextran amine anterograde tracing in the healthy corticospinal tract of the denervated area at different time periods. Our results demonstrate that compared with untrained rats, functional recovery was better in the forelimbs and forepaws of trained rats. The number of axons and the expression of growth associated protein 43 were increased at the injury site 3 weeks after corticospinal tract injury. These findings confirm that fine motor skill training promotes central nervous system plasticity in spinal cord injury rats.展开更多

Method: 27 elite male taekwondo players (mean ± standard deviation: age 24.4 ± 4.9 years, body mass 75.7 ± 15.1 kg, height 181.4 ± 7.8 cm) were allocated to two experimental (n = 15) and control (n = 12) groups. The eight-week FT was divided into four distinct phases for the subjects of the experimental group. Variables of pain and function were assessed by Visual Analogue Scale (VAS) and Oswestry Disability Index (ODI), respectively. The movement time of Ap Douliou Chagi kick (MTAK) and speed of Ap Douliou Chagi kick (SAK) was used for the variable of performance. Results: Considering the results of pretest before the implementation of the scores protocol, after FT intervention, VAS (p = 0.004), ODI (p = 0.005), MTAK on the right leg (p = 0.029) and the left leg (p = 0.013) decreased and SAK on both legs (p = 0.000) increased significantly in comparison with the control group. Conclusion: It is assumed that FT can reduce the pain intensity and improve the function and performance in taekwondo players.展开更多
文摘Background This study compared knee osteoarthritis(OA)outcomes specific to pain,physical function,and quality of life in later life based on strength training(ST)participation over a lifetime.Methods Participants from the Osteoarthritis Initiative(n=3192)were grouped by ST engagement during ages 12–18 years,19–34 years,35–49 years,and 50+years.Participants were categorized as:No ST(no ST at any point;61.7±9.0 years(mean±SD)),Some ST(engaged in ST during 1–3 life stages;58.9±8.7 years),and Lifelong ST(consistently engaged in ST across all life stages;55.6±8.1 years).Measures were collected at baseline and Year 4:Western Ontario and McMaster Universities Osteoarthritis Index Scores(WOMAC;pain,daily activities),Knee Injury and Osteoarthritis Outcome Score(KOOS;sports,recreation),Physical Activity Score for the Elderly(PASE),Short Form-12 Physical Component Score(SF-12 PCS),mobility disability,chair rise time,and walking speed(20 m and 400 m).Results At Year 4,the Lifelong ST group reported better WOMAC activity scores in the right knee along with better WOMAC pain,KOOS sports/recreation,and PASE scores compared to other groups(p<0.05).The Lifelong ST group had the lowest incidence of mobility disability of all groups(0.8%vs.2.3%–4.1%;p=0.015)and maintained the fastest walking speeds in Year 4.Conclusion For those with knee OA,ST throughout life may help preserve function and mobility,allowing for greater physical activity engagement while keeping pain levels relatively lower.

文摘[Objectives]To investigate the clinical efficacy of core stability training combined with conventional rehabilitation in the functional recovery of patients suffering from chronic low back pain.[Methods]A randomized controlled trial design was employed in this study.Ninety patients with chronic low back pain were recruited and randomly assigned to either a control group(n=45),which received conventional rehabilitation,or an experimental group(n=45),which received conventional rehabilitation combined with core stability training.Both groups underwent treatment for 6 weeks.Assessments were conducted using the visual analogue scale(VAS),Oswestry disability index(ODI),and finger-to-floor test prior to treatment,6 weeks following treatment,and during the follow-up period,respectively.[Results]Prior to treatment,no statistically significant differences were observed between the two patient groups in terms of general information and various baseline measurements(P>0.05).Following 6 weeks of treatment and throughout the follow-up period,both groups demonstrated significant improvements in VAS scores,ODI scores,and lumbar anteflexion range of motion compared to baseline measurements(P<0.05).Notably,the magnitude of improvement in the experimental group exceeded that of the control group,with this inter-group difference reaching statistical significance(P<0.05).No serious adverse reactions were reported during the treatment process.[Conclusions]Core stability training combined with conventional rehabilitation can significantly enhance the alleviation of pain and functional impairments in patients suffering from chronic low back pain.This approach holds valuable implications for the optimization of rehabilitation treatment protocols.
文摘[Objectives]To synthesize evidence on HIIT versus moderate-intensity continuous training(MICT)or routine rehabilitation in stroke survivors.[Methods]We systematically searched 8 databases(PubMed,EMBASE,CENTRAL,Web of Science,SPORTSDiscus,PsycINFO,SCOPUS,CINAHL)up to May 2025.Seventeen randomized controlled trials(RCTs;total n=1142)met inclusion criteria:adults with stroke,device-based HIIT(≥70%HRR/VO 2peak),and outcomes assessing VO_(2)peak,6-min walk distance(6MWD),or Berg Balance Scale(BBS).Methodological quality was evaluated using the PEDro scale.Pooled effect sizes(Hedges'g)were calculated via random-effects models,with heterogeneity quantified by I^(2).[Results]HIIT significantly improved peak oxygen uptake(VO_(2)peak)versus controls(g=0.59,95%CI:0.44-0.75,p<0.001;I^(2)=16.29%).Low heterogeneity and symmetrical funnel plots supported robustness.HIIT also enhanced walking endurance(6MWD:g=0.32,95%CI:0.16-0.48,p<0.01;I^(2)=30%).In contrast,no significant benefit was observed for balance function(BBS:g=0.07,95%CI:-0.13-0.26,p=0.50;I^(2)=0%).[Conclusions]HIIT is a safe and highly effective intervention for enhancing aerobic capacity and walking function post-stroke.Its benefits are maximized at higher intensities and longer durations but do not extend to balance improvement.Integrating HIIT into stroke rehabilitation protocols is strongly recommended to promote functional independence.
文摘[Objectives]To investigate the clinical effects of implementing structured phased rehabilitation training,in addition to conventional rehabilitation,on shoulder joint function and pain alleviation in patients with rotator cuff injuries managed conservatively.[Methods]Eighty patients diagnosed with rotator cuff injury were selected and randomly assigned to either the control group or the experimental group,each comprising 40 individuals.The control group received conventional rehabilitation treatment,whereas the experimental group underwent phased rehabilitation training in addition to the conventional treatment for 6 weeks.Assessments were conducted prior to treatment,6 weeks following treatment,and 8 weeks after the completion of treatment(follow-up period).The visual analogue scale(VAS)was employed to evaluate pain intensity,the Constant-Murley score was utilized to assess shoulder joint function,and the shoulder joint range of motion was measured.[Results]Prior to treatment,no statistically significant differences were observed between the two patient groups across all measured indicators(P>0.05).Following 6 weeks of treatment and throughout the follow-up period,both groups exhibited significant reductions in VAS scores compared to baseline measurements,alongside improvements in Constant-Murley scores and shoulder joint range of motion(P<0.05).Furthermore,the magnitude of improvement in the experimental group was significantly greater than that in the control group(P<0.05).[Conclusions]Phased rehabilitation training can enhance shoulder joint function and alleviate pain in patients with rotator cuff injuries beyond the effects of conventional rehabilitation treatment,demonstrating notable clinical application value.
文摘High-intensity interval training(HIIT),a highly efficient and distinctive exercise format,has sparked growing academic interest in sports performance training.This article synthesizes theoretical and applied evidence to analyze mechanisms of HIIT in neuromuscular activation,hormonal responses,muscle fiber adaptation,and metabolic pathway effects.It focuses on its effectiveness in enhancing explosive power,maximum strength,and strength endurance,while also examining the integration of HIIT with traditional resistance training,periodized programming,and personalized prescription.Scientific implementation of HIIT can effectively diversify or even replace conventional strength training,not only offering positive directional changes for strength development but also introducing innovative approaches to sports performance training practices.

文摘The recovery of motor function in spastic hemiplegia following stroke is closely associated with cortical reorganisation.This study aimed to investigate the effects of warm acupuncture combined with rehabilitation training on cortical activity and upper limb function using functional near-infrared spectroscopy.Ninety patients were randomly assigned to a warm acupuncture group,a rehabilitation training group,or a combined intervention group.After four weeks of intervention,clinical scores and changes in oxyhaemoglobin concentration in the affected motor cortex were assessed.Results demonstrated that the combined intervention group showed significantly greater improvement in Fugl-Meyer scores,reduced muscle tone,and enhanced activation in the affected primary motor cortex compared to the single intervention groups.Furthermore,functional improvement was significantly correlated with cortical activation changes.These findings indicate a synergistic effect between warm acupuncture and rehabilitation training,potentially mediated through promoting functional reorganisation of the affected cerebral cortex.This provides neuroimaging evidence supporting integrated Chinese and Western medicine rehabilitation protocols.
文摘Objective:To investigate the positive eff ect of mindfulness training and exercise therapy on functional recovery and quality of life in elderly patients with fractures of the lower back.Methods:Collection of medical records of patients with lumbar vertebral fractures in Suzhou Municipal Hospital(North District),60 patients underwent percutaneous vertebroplasty(PVP/PKP),they were divided into control and experimental groups according to a random number table,30 people per group;In addition to traditional care method,Also intervened with behavioural awareness training and exercise therapy,By comparing the Oswestry dysfunction index score(ODI),visual simulation score(VAS),ability of daily living score(ADL),quality of life score(SF-36),To evaluate the eff ectiveness of the intervention.Results:Before the intervention,the ODI,VAS,ADL,and SF-36 scores were basically the same,with no significant diff erence(P>0.05).After treatment,the ODI and VAS scores of the experimental group and the control group were signifi cantly lower,and the experimental group was significantly lower than the control group(P<0.05);the ADL scores of the experimental group were signifi cantly higher than those of the control group(P<0.05);their SF-36 quality of life scores in all dimensions were signifi cantly higher(P<0.05).Conclusion:Through the rehabilitation treatment method combining mindfulness behavior training and exercise intervention,it can eff ectively promote the functional recovery of osteoporotic lumbar fractures in the elderly,thus reducing their pain and improving their quality of life.
文摘Objective To compare the influences of acupuncture combined with rehabilitative training and simple rehabilitative training on neural functional recovery of stroke patients during recovery phase.Methods Sixty-seven stroke patients during recovery phase were randomly divided into an acupuncture combined with rehabilitative training group(group A) and a simple rehabilitative training group(group B) through random number table method,with 34 cases and 33 cases respectively;33 cases and 31 cases were respectively completed actually other than cases dropped out during the observation.Rehabilitation methods such as motion control and functional exercise training were primarily used for rehabilitative training,once daily for 45 min at a time,and 6 times a week;Baihui(百会 GV 20) and Fengchi(风池 GB 20) on the head and relevant acupoints on limbs were selected for acupuncture,once every other day,and 3 times a week.Both groups were treated for 8 weeks,National Institnte of Health Stroke Scale(NIHSS) was used to assess neurologic impairment,Fugl-Meyer scale was used to assess motor function,Barthel was used to assess activity of daily living,and comprehensive assessment was finally made to patients.Results NIH score,Fugl-Meyer scale scores of upper and lower limbs,Barthel score,and comprehensive assessment of group A after treatment were respectively:12.78±2.99,37.55±2.09 and25.67±3.93,63.76±4.65,11.33±1.98,superior to 16.06±4.57,28.71±4.07 and20.41±2.98,53.8214.35,15.18±1.72 of group B.Indices of both groups were significantly improved compared with those before treatment(P〈0.01,P〈0.05),but improvement of indices of group A was more obvious that that of group B after treatment subject to comparison(all P〈0.05).Conclusions Acupuncture combined with rehabilitative training has definite therapeutic effect on recovery of various functions such as neural functional recovery of stroke patients during recovery phase,obviously superior to simple rehabilitative training.
文摘Stroke patients experience varying degrees of upper limb functional impairment.Although bilateral arm training can help stroke patients recover movement after stroke,little is known about the way in which the brain and muscles work together during this type of training.To address this,we conducted a cross-sectional study at The Seventh Affiliated Hospital,Sun Yat-sen University in China,where we observed the connection between brain and muscle activity during bilateral upper limb training in 21 stroke patients and 17 healthy controls.We used functional near-infrared spectroscopy and surface electromyography to measure changes in cerebral cortex oxygenation and upper limb muscle contraction signals,respectively.The results showed that,compared with the healthy control group,stroke patients had reduced functional connectivity and more irregular muscle activity in the affected flexor muscle during bilateral upper limb training.Moreover,we found a significant correlation between the surface electromyographic signal characteristics of upper limb muscles and cerebral oxygenation indicators of multiple brain regions in stroke patients.These findings indicate that bilateral upper limb training is an effective rehabilitation method that improves upper limb motor function in stroke patients by promoting brain functional connectivity and improving muscle activity patterns.
文摘Impairments on body function,activities of daily living(ADL)and cognition are common after stroke.Eccentric resistance training(ERT)may be implemented to improve them.The primary objectives were to evaluate whether ERT improves body function,ADL and cognition after stroke.The secondary objectives were to evaluate whether ERT improves strength,gait,quality of life,and self-perceived health.Seven electronic databases were searched:Cochrane Central Register of Controlled Trials,Cochrane Stroke Group Trials Register,PubMed,Epistemonikos,Embase,SPORTDiscus,and WHO International Clinical Trials Registry Platform.Last search was run in December 2023,including studies since 2012.Selection criteria were studies with stroke participants of both sexes,aged 18 or more,with an intervention based on ERT.Accepted languages were English,Spanish or French.First search was done in pairs.Authors removed duplicate studies and those which did not meet inclusion criteria through title and abstracts.Finally,all authors,independently,screened the final search results and extracted data.Of 68 records identified,15 were eligible and 6 were finally included(with moderate risk of bias),analyzing 159 participants.Body function(4 interventions,n=84),gait(4 interventions,n=115)and strength(3 interventions,n?78)showed significantly better results when ERT was performed.Meta-analysis could not be done because of the few studies and their heterogeneity.This review provided low-moderate quality evidence suggesting that ERT might be effective at improving body function,strength,and gait after stroke.Besides,no harm was documented,and it was well-accepted.
文摘Objective:To evaluate the effect of task-oriented training combined with a lower limb rehabilitation robot on improving motor function and ankle joint function in stroke patients with hemiplegia.Methods:Sixty-three stroke patients with hemiplegia admitted to our hospital from January 2022 to June 2024 were randomly divided into observation group(32 cases)and control group(31 cases)using the envelope method.The control group received task-oriented training,while the observation group received additional lower limb rehabilitation robot training.The motor function(Fugl-Meyer Assessment of Lower Extremity,FMA-LE)and ankle joint function(Active Dorsiflexion Range of Motion,DF AROM)were compared between the two groups.Results:After treatment,the levels of FMA-LE and DF AROM in both groups increased significantly,and the improvement in each index in the observation group was better than that in the control group(P<0.05).Conclusion:The combination of task-oriented training and lower limb rehabilitation robot training can more effectively improve the overall motor function of the lower limbs and the active dorsiflexion ability of the ankle joint in stroke patients with hemiplegia.

文摘The Kinect-based virtual reality system for the Xbox 360 enables users to control and interact with the game console without the need to touch a game controller, and provides rehabilitation training for stroke patients with lower limb dysfunctions. However, the underlying mechanism remains un- clear. In this study, 18 healthy subjects and five patients after subacute stroke were included. The five patients were scanned using functional MRI prior to training, 3 weeks after training and at a 12-week follow-up, and then compared with healthy subjects. The FugI-Meyer Assessment and Wolf Motor Function Test scores of the hemiplegic upper limbs of stroke patients were significantly increased 3 weeks after training and at the 12-week follow-up. Functional MRI results showed that contralateral primary sensorimotor cortex was activated after Kinect-based virtual reality training in the stroke patients compared with the healthy subjects. Contralateral primary sensorimotor cortex, the bilateral supplementary motor area and the ipsilateral cerebellum were also activated during hand-clenching in all 18 healthy subjects. Our findings indicate that Kinect-based virtual reality training could promote the recovery of upper limb motor function in subacute stroke patients, and brain reorganization by Kinect-based virtual reality training may be linked to the contralateral sen- sorimotor cortex.

文摘Objective:To investigate the effects of robot-assisted training on upper limb function in elderly stroke patients.Methods:Seventy elderly stroke patients treated in the Rehabilitation Medicine Department of Jiangsu Provincial Organ Hospital from January 2023 to December 2023 were randomly divided into an intervention group(n=35)and a control group(n=35).In addition to conventional rehabilitation and nursing care,both groups received 40 minutes of daily upper limb training.The control group underwent entirely manual training,whereas the intervention group received a combination of 20 minutes of manual and 20 minutes of robot-assisted training.All participants completed this protocol five times weekly for four weeks,with assessments of upper limb motor function and activities of daily living(ADL)conducted pre-and post-intervention.Results:After treatment,both groups showed significant increases in Fugl-Meyer Assessment(FMA-UE)and Barthel Index(BI)scores compared to before treatment(p<0.01).The intervention group had higher FMA-UE scores than the control group(p<0.05),while there was no significant difference in BI scores between the two groups after treatment(p>0.05).Conclusion:Robot-assisted training can improve upper limb motor function and enhance ADL capabilities in elderly stroke patients.

文摘Background: Exercise promotes numerous phenotypic adaptations in skeletal muscle that contribute to improved function and metabolic capacity. An emerging body of evidence suggests that skeletal muscle also releases a myriad of factors during exercise, termed "myokines". The purpose of this study was to examine the effects of high-intensity interval training(HIIT) on the acute regulation of the mRNA expression of several myokines, including the prototypical myokine interleukin-6(IL-6), and recently identified myokines fibronectin type III domain-containing protein 5(FNDC5)(irisin) and meteorin-like protein(METRNL).Methods: Both before and after a 20-day period of twice-daily high-volume HIIT, 9 healthy males(20.5 ± 1.5 years performed a standardized bout of high-intensity interval exercise(HIIE; 5 × 4 min at ~80% pretraining peak power output) with skeletal muscle biopsy samples(vastus lateralis) obtained at rest, immediately following exercise, and at 3 h recovery.Results: Before training, a single bout of HIIE increased IL-6(p < 0.05) and METRNL(p < 0.05) mRNA expression measured at 3 h recovery when compared to rest. Following 20 days of HIIT, IL-6 and FNDC5 mRNA were increased at 3 h recovery from the standardized HIIE bout when compared to rest(both p < 0.05). Resting METRNL and FNDC5 mRNA expression were higher following training(p < 0.05), and there was an overall increase in FNDC5 mRNA post-training(main effect of training, p < 0.05).Conclusion: In human skeletal muscle(1) an acute bout of HIIE can induce upregulation of skeletal muscle IL-6 mRNA both before and after a period of intensified HIIT;(2) Resting and overall FNDC5 mRNA expression is increased by 20 days of HIIT; and(3) METRNL mRNA expression is responsive to both acute HIIE and short-term intense HIIT. Future studies are needed to confirm these findings at the protein and secretion level in humans.
文摘Aerobic capacity, which is expressed as peak oxygen consumption (VO2peak), is well-known to be an independent predictor of all-cause mortality and cardiovascular prognosis. This is true even for people with various coronary risk factors and cardiovascular diseases. Although exercise training is the best method to improve VO2peak, the guidelines of most academic societies recommend 150 or 75 min of moderate- or vigorous- intensity physical activities, respectively, every week to gain health benefits. For general health and primary and secondary cardiovascular prevention, high-intensity interval training (HIIT) has been recognized as an efficient exercise protocol with short exercise sessions. Given the availability of the numerous HIIT protocols, which can be classified into aerobic HIIT and anaerobic HIIT [usually called sprint interval training (SIT)], professionals in health-related fields, including primary physicians and cardiologists, may find it confusing when trying to select an appropriate protocol for their patients. This review describes the classifications of aerobic HIIT and SIT, and their differences in terms of effects, target subjects, adaptability, working mechanisms, and safety. Understanding the HIIT protocols and adopting the correct type for each subject would lead to better improvements in VO2peak with higher adherence and less risk.

文摘Objective: This systematic review aimed to critically analyze the literature to determine how high-intensity intermittent training(HIIT) affects recreational endurance runners in the short-and long-term.Methods: Electronic databases were searched for literature dating from January 2000 to October 2015. The search was conducted using the key words "high-intensity intermittent training" or "high-intensity interval exercise" or "interval running" or "sprint interval training" and "endurance runners" or "long distance runners". A systematic approach was used to evaluate the 783 articles identified for initial review. Studies were included if they investigated HIIT in recreational endurance runners. The methodological quality of the studies was evaluated using the Physiotherapy Evidence Database(PEDro) scale(for intervention studies) and the modified Downs and Black Quality Index(for cross-sectional studies).Results: Twenty-three studies met the inclusionary criteria for review. The results are presented in 2 parts: cross-sectional(n = 15) and intervention studies(n = 8). In the 15 cross-sectional studies selected, endurance runners performed at least 1 HIIT protocol, and the acute impact on physiological, neuromuscular, metabolic and/or biomechanical variables was assessed. Intervention studies lasted a minimum of 4 weeks, with 10 weeks being the longest intervention period, and included 2 to 4 HIIT sessions per week. Most of these studies combined HIIT sessions with continuous run(CR) sessions; 2 studies' subjects performed HIIT exclusively.Conclusion: HIIT-based running plans(2 to 3 HIIT sessions per week, combining HIIT and CR runs) show athletic performance improvements in endurance runners by improving maximal oxygen uptake and running economy along with muscular and metabolic adaptations. To maximize the adaptations to training, both HIIT and CR must be part of training programs for endurance runners.
文摘BACKGROUND The recovery of limb function after ankle fracture surgery is a gradual process.The main purpose of implementing early functional exercise,joint mobility,muscle contraction function,passive ankle flexion and extension exercises,or physical factor therapy techniques is to achieve the rapid recovery of normal physiological limb function.However,currently the most effective rehabilitation training method is staged limb functional exercise,which promotes rapid recovery of limb function while preventing adverse consequences caused by overwork or insufficient training.Staged limb functional exercise divides the rehabilitation process into multiple stages,each of which has specific training objectives and contents.This method helps patients gradually restore limb function.Nevertheless,some patients still exhibit poor limb function after standardized exercise.Therefore,a functional evaluation should be performed to analyze the impact of staged functional training after ankle fracture surgery.AIM To perform a functional evaluation and determine the influencing factors of staged functional training in patients with ankle fracture.METHODS A retrospective study enrolled 150 patients who underwent surgical treatment for ankle fracture from May 2020 to May 2022 at our hospital.Univariate and multivariate linear regression analyses were performed on general data,functional exercise compliance scale for orthopedic patients,Social Support Rating Scale(SSRS),American Orthopedic Foot and Ankle Score(AOFAS)Ankle-Hindfoot Score,and pain factors[serum bradykinin(BK),prostaglandin E2(PGE2),5-hydroxytryptamine(5-HT)].RESULTS Based on the AOFAS Ankle-Hindfoot Scale,the cases were divided into the excellent function(n=111)and ordinary function(n=39)groups.Univariate analysis revealed that monthly family income,education level,diabetes mellitus,functional exercise compliance scale of orthopedic patients score,SSRS,BK,PGE2,and 5-HT significantly influenced limb function after ankle fracture(P<0.05);Multiple linear regression analysis showed that the functional exercise compliance scale score,SSRS,BK,PGE2,and 5-HT were independent risk factors affecting functional performance after staged functional exercise(P<0.05).CONCLUSION Exercise compliance,SSRS,and pain level are the independent risk factors affecting functional performance after staged functional training following ankle surgery.Clinical nursing care after ankle surgery should include analgesic and health education measures to ensure optimal recovery of limb function.

文摘Following central nervous system injury, axonal sprouts form distal to the injury site and extend into the denervated area, reconstructing neural circuits through neural plasticity. How to facilitate this plasticity has become the key to the success of central nervous system repair. It remains controversial whether fine motor skill training contributes to the recovery of neurological function after spinal cord injury. Therefore, we established a rat model of unilateral corticospinal tract injury using a pyramidal tract cutting method. Horizontal ladder crawling and food ball grasping training procedures were conducted 2 weeks before injury and 3 days after injury. The neurological function of rat forelimbs was assessed at 1, 2, 3, 4, and 6 weeks after injury. Axon growth was observed with biotinylated dextran amine anterograde tracing in the healthy corticospinal tract of the denervated area at different time periods. Our results demonstrate that compared with untrained rats, functional recovery was better in the forelimbs and forepaws of trained rats. The number of axons and the expression of growth associated protein 43 were increased at the injury site 3 weeks after corticospinal tract injury. These findings confirm that fine motor skill training promotes central nervous system plasticity in spinal cord injury rats.
文摘Background: The aim of this study was to assess the effects of functional training (FT) on pain, function, and performance in elite male taekwondo players with CMLBP. Method: 27 elite male taekwondo players (mean ± standard deviation: age 24.4 ± 4.9 years, body mass 75.7 ± 15.1 kg, height 181.4 ± 7.8 cm) were allocated to two experimental (n = 15) and control (n = 12) groups. The eight-week FT was divided into four distinct phases for the subjects of the experimental group. Variables of pain and function were assessed by Visual Analogue Scale (VAS) and Oswestry Disability Index (ODI), respectively. The movement time of Ap Douliou Chagi kick (MTAK) and speed of Ap Douliou Chagi kick (SAK) was used for the variable of performance. Results: Considering the results of pretest before the implementation of the scores protocol, after FT intervention, VAS (p = 0.004), ODI (p = 0.005), MTAK on the right leg (p = 0.029) and the left leg (p = 0.013) decreased and SAK on both legs (p = 0.000) increased significantly in comparison with the control group. Conclusion: It is assumed that FT can reduce the pain intensity and improve the function and performance in taekwondo players.
