Occupational therapy with or without combined acupuncture on upper limb pain and hand functions in children with spastic hemiplegic cerebral palsy:A three-arm randomized,placebo-controlled trial 被引量:3

Objective:This study is conducted to investigate the effects of occupational therapy(OT) with or without combined acupuncture for upper limb pain and hand functions among children with spastic hemiplegic cerebral palsy.Methods:A total of 90 patients who met the inclusion criteria were enrolled in a three-arm randomized,placebo-controlled trial,and were allocated to mono-occupational therapy(OT) group(patients=30),OT combined with real-acupuncture(ORA) group(patients=30),and OT combined with placeboacupuncture(OPA) group(patients=30),respectively.In addition to receiving the conventional OT program as the basic treatment for total 8 weeks,patients in ORA and OPA group also received corresponding acupuncture treatment 5 days per week for the first two weeks,3 days per week for the next two weeks,2 days per week for another two weeks,and 1 day per week for the last two weeks.Non-communicating children’s pain checklist-revised scale(NCCPC-R) was used for assessing the upper limb pain among patients.Squeeze dynamometry,modified Ashworth scale(MAS),manual muscle testing(MMT),JebsenTaylor hand function test(JTHFT) and box and block test(BBT) were used for assessing various aspects of upper limb and hand functions among patients.Meanwhile,adverse effects were monitored and recorded.Results:After 8-week treatment,NCCPC-R global scores witnessed a dramatic decline in ORA group(P<0.05),but not in either OPA(P> 0.05) or OT group(P> 0.05).Dramatic increases were identified in both squeeze dynamometry scores and MMT scores in all three groups(all P <0.05).Furthermore,the increases of ORA group were more significant than those of another two groups(both P <0.05).There were no significant changes in MAS global scores in each group(all P> 0.05).The BBT global scores of all three groups significantly increased(all P <0.05) after treatment.Among them,the increase of the ORA group was more than that of the other two groups(P <0.05).The 8-week treatment also had a significant impact on several sub-tests of JTFHT in each group.No significant adverse event was reported.Conclusion:ORA is a potential and promising alternative therapy for mitigating upper limb pain as well as improving both upper limb and hand functions among children with spastic hemiplegic cerebral palsy. 展开更多

Although some patients have successful peripheral nerve regeneration,a poor recovery of hand function often occurs after peripheral nerve injury.It is believed that the capability of brain plasticity is crucial for the recovery of hand function.The supplementary motor area may play a key role in brain remodeling after peripheral nerve injury.In this study,we explored the activation mode of the supplementary motor area during a motor imagery task.We investigated the plasticity of the central nervous system after brachial plexus injury,using the motor imagery task.Results from functional magnetic resonance imaging showed that after brachial plexus injury,the motor imagery task for the affected limbs of the patients triggered no obvious activation of bilateral supplementary motor areas.This result indicates that it is difficult to excite the supplementary motor areas of brachial plexus injury patients during a motor imagery task,thereby impacting brain remodeling.Deactivation of the supplementary motor area is likely to be a serious problem for brachial plexus injury patients in terms of preparing,initiating and executing certain movements,which may be partly responsible for the unsatisfactory clinical recovery of hand function. 展开更多

Background: Carpal Tunnel Syndrome (CTS) is the compression of the median nerve in carpal tunnel of wrist which leads to the typical symptoms of numbness, paresthesia and sometimes pain in the patient’s hand. The symptoms are usually sensed in the first three radial fingers and the lateral side of the ring finger because these areas are innervated by the median nerve. Objective: To assess hand function among patient with Carpal Tunnel Syndrome (CTS). Methods: This was a descriptive study in which 41 subjects (27 females and 14 males) were included from Ghurki trust teaching hospital, Lahore and Shalamar hospital, Lahore (from August 2018 to October 2018) according to inclusion criteria. Samples were collected according to WHO (World Health Organization) calculator. Hand function was measured with Boston Carpal Tunnel Questionnaire (BCTQ). Results: Majority of the patients with CTS fell into mild functional severity and mild symptom severity. The mean score for functional severity was 2.34 and for symptom severity it was 2.97. Conclusion: Activities of daily living such as grasping, writing, gripping and carrying objects were affected by the symptoms of pain, numbness, tingling and weakness in the patients suffering from CTS. 展开更多

These changes are often ignored by clinicians owing to the extent of motor disability of the affected hand. Finger tapping frequency and Lind-mark hand function score showed that the motor function of unaffected hands in stroke patients was poorer than that of a healthy control hand. After 2 weeks of rehabilitation treatment, motor function of the unaffected hand of stroke patients was obviously improved. Therefore, attention should also be paid to motor function in the unaffected hand of stroke patients with hemiplegia during rehabilitation. 展开更多

There have been no studies reported on the difference in cortical activation during use of volar and dorsal hand splints.We attempted to investigate the difference in cortical activation in the somatosensory cortical area during use of volar and dorsal hand splints by functional magnetic resonance imaging(f MRI).We recruited eight healthy volunteers.f MRI was performed while subjects who were fitted with volar or dorsal hand splints performed grasp-release movements.Regions of interest were placed on the primary motor cortex(M1),primary somatosensory cortex(S1),posterior parietal cortex(PPC),and secondary somatosensory cortex(S2).Results of group analysis of f MRI data showed that the total numbers of activated voxels in all ROIs were significantly higher during use of volar hand splint(3,376) compared with that(1,416) during use of dorsal hand splint.In each ROI,use of volar hand splint induced greater activation in all ROIs(M1:1,748,S1 :1,455,PPC:23,and S2:150) compared with use of dorsal hand splint(M1:783,S1:625,PPC:0,and S2:8).The peak activated value was also higher during use of volar hand splint(t-value:17.29) compared with that during use of dorsal hand splint(t-value:13.11).Taken together,use of volar hand splint induced greater cortical activation relevant to somatosensory function than use of dorsal hand splint.This result would be important for the physiatrist and therapist to apply appropriate somatosensory input in patients with brain injury. 展开更多

Objective: To observe the effects of Xingnao Kaiqiao acupuncture(醒脑开窍针) on the motor function of upper limb and hand in the recovery period after stroke. Methods: Seventy-six cases of hemiplegia patients with ischemic stroke were divided into the treatment group and the control group(n=38 in each). Based on the Brunnstrom's stage of Xingnao Kaiqiao acupuncture combined with rehabilitation training was used in the treatment group, and the control group was given rehabilitation training. FuglMeyer Assessment of the upper extremity(FMA-UE), Action Research Arm Test(ARAT) and Simple Test for Evaluating hand Function(STEF) were adopted separately to compare scores before treatment and 8 weeks after treatment. Results: The difference was not statistically significant in the two groups of patients for comparison of FMA-UE, ARAT and STEF scores before treatment(P>0.05). The difference was statistically significant in the two groups of score comparison of FMA-UE, ARAT and STEF after treatment(P<0.05). Conclusion: The Xingnao Kaiqiao acupuncture has its unique advantages in improving recovery of motor function of upper limb and hand in recovery period after stroke. 展开更多

Embodied semantics theory asserts that the meaning of action-related words is neurally represented through networks that overlap with or are identical to networks involved in sensory-motor processing. While some studies supporting this theory have focused on Chinese characters, less attention has been paid to their semantic radicals. Indeed, there is still disagreement about whether these radicals are processed independently. The present study investigated whether radicals are processed separately and, if so, whether this processing occurs in sensory-motor regions. Materials consisted of 72 high-frequency Chinese characters, with 18 in each of four categories: hand-action verbs with and without hand-radicals, and verbs not related to hand actions, with and without hand-radicals. Twenty-eight participants underwent functional MRI scans while reading the characters. Compared to characters without hand-radicals, reading characters with hand-radicals activated the right medial frontal gyrus. Verbs involving hand-action activated the left inferior parietal lobule, possibly reflecting integration of information in the radical with the semantic meaning of the verb. The findings may be consistent with embodied semantics theory and suggest that neural representation of radicals is indispensable in processing Chinese characters. 展开更多

目的探讨重复经颅磁刺激(rTMS)作用拇指大脑皮层运动代表区对急性缺血性脑卒中患者手部功能的影响。方法收集2023年7月至2024年3月于我院收治的缺血性脑卒中急性期患者78例为研究对象,依据治疗方法不同分为试验组(46例,接受高频rTMS治疗... 目的探讨重复经颅磁刺激(rTMS)作用拇指大脑皮层运动代表区对急性缺血性脑卒中患者手部功能的影响。方法收集2023年7月至2024年3月于我院收治的缺血性脑卒中急性期患者78例为研究对象,依据治疗方法不同分为试验组(46例,接受高频rTMS治疗)和对照组(32例,接受rTMS假性刺激治疗)。治疗10d后,比较2组患者的临床疗效。收集2组患者治疗前、治疗10d后的腕手关节主动活动度(AROM)、神经功能缺损程度评分、改良Barthel指数(MBI)量表、手背部肌肉运动诱发电位(MEP)振幅、简化Fugl-Meyer运动功能评分法(FMA)(手部分)。分别于治疗前、治疗10d后获取fMRI图像。采用独立成分分析(ICA)及基于种子点的功能网络分析方法提取静息态下的运动功能网络。结果试验组的总有效率分别为89.13%,明显高于对照组(68.75%)(P<0.05)。治疗10d后,与对照组比较,试验组的FMA评分、神经功能缺损评分、腕手部关节AROM各参数、MBI评分及MEP振幅的治疗前后差值均明显较高,差异有统计学意义(P<0.05)。试验组rTMS治疗后,脑卒中患者的运动网络显示患侧中央后回、患侧中央前回、健侧中央后回及患侧辅助运动区(SMA)的功能连通性显著增强(P<0.05,经Alphasim校正);对照组rTMS假性治疗后,运动网络显示健侧中央前回、健侧额中回及双侧SMA的功能连通性明显增强(P<0.05,经Alphasim校正);与对照组比较,试验组在双侧SMA、患侧额中回、健侧中央后回及双侧中央前回的功能网络连接性增强更为显著。然而,试验组在健侧额中回运动网络功能连通性的影响较弱(P<0.05,经Alphasim校正)。试验组rTMS治疗后,患侧M1与患侧SMA及健侧SMA、患侧SMA与患侧M1、患侧前辅助运动区(PMA)及健侧M1、患侧PMA与患侧SMA及健侧M1/SMA、健侧M1与患侧SMA/PMA、健侧SMA与患侧M1/PMA之间的功能连接性增强显著(P<0.05)。对照组则未观察到上述功能连接性的显著增强(P>0.05)。结论高频rTMS可有效改善急性缺血性脑卒中患者的手部功能,可能机制包括增强患侧运动网络连通性、促进受损半球运动区恢复及优化双侧大脑间功能协调。 展开更多

目的:探讨高精度经颅直流电刺激(HD-tDCS)联合反复促进疗法(RFE)在脑卒中恢复期患者中的应用效果。方法:选取2022年5月—2024年5月山西医科大学第一医院收治的123例脑卒中恢复期偏瘫患者,采用随机数字表法将其分为常规组、HD-tDCS组、... 目的:探讨高精度经颅直流电刺激(HD-tDCS)联合反复促进疗法(RFE)在脑卒中恢复期患者中的应用效果。方法:选取2022年5月—2024年5月山西医科大学第一医院收治的123例脑卒中恢复期偏瘫患者,采用随机数字表法将其分为常规组、HD-tDCS组、联合干预组,各41例。常规组实施常规康复训练,HD-tDCS组实施常规康复训练和HD-tDCS干预,联合干预组在常规组基础上实施HD-tDCS联合RFE干预。比较三组干预前后肢体偏瘫程度(Brunnstrom分期)、手功能[手臂动作调查测试表(ARAT)]、上肢运动功能[Fugl-Meyer上肢运动功能评估(FMA-UL)]、表面肌电图指标[腕伸肌与指伸肌的表面肌电均方根值(RMS)、积分肌电值(iEMG)]及日常生活能力[改良Barthel指数(MBI)]。结果:干预后,三组Brunnstrom分期比较,差异有统计学意义(P<0.05);干预后,联合干预组Ⅵ期占比高于常规组(58.54%vs 21.95%),差异有统计学意义(P<0.05)。干预后,三组ARAT评分、FMA-UL评分、腕伸肌与指伸肌的RMS、iEMG水平均升高,且联合干预组均高于HD-tDCS组、常规组,三组MBI评分均升高,联合干预组高于常规组,差异均有统计学意义(P<0.05)。结论:HD-tDCS联合RFE可改善脑卒中恢复期患者偏瘫程度,促进患者手功能、上肢运动功能恢复,增强上肢神经-肌肉兴奋程度,提升日常生活自理能力。 展开更多

目的探讨手关节操质量持续改进对类风湿关节炎(rheumatoid arthritis,RA)疗效的影响。方法选取2020年4—12月在厦门大学附属第一医院治疗的155例RA患者作为研究对象,2020年4—6月收治的75例患者纳入干预前组,未进行手关节操质量持续改... 目的探讨手关节操质量持续改进对类风湿关节炎(rheumatoid arthritis,RA)疗效的影响。方法选取2020年4—12月在厦门大学附属第一医院治疗的155例RA患者作为研究对象,2020年4—6月收治的75例患者纳入干预前组,未进行手关节操质量持续改进干预;2020年7—12月收治的80例患者纳入干预后组,进行手关节操质量持续改进干预。对比2组关节疼痛个数、关节肿胀个数及疼痛改善情况、手关节功能康复情况、患者满意度、手关节操掌握率。结果训练12周后,干预后组关节疼痛个数、关节肿胀个数分别为(4.75±0.65)个、(5.86±0.52)个,少于干预前组的(5.21±0.72)个、(6.24±0.67)个,数字评定量表评分法(numerical rating scale,NRS)、手功能障碍评分量表(signals of functional impairment,SOFI)评分分别为(1.07±0.22)分、(1.08±0.22)分,低于干预前组的(1.85±0.54)分、(2.54±0.32)分,比较差异均有统计学意义(P<0.05)。干预后组患者日常生活、加深关节操认识、护患沟通及心理健康评分分别为(20.48±2.26)分、(21.15±2.43)分、(20.59±2.21)分及(21.35±2.52)分,高于干预前组的(15.23±2.13)分、(16.32±2.35)分、(16.21±2.11)分及(17.44±2.32)分,差异均有统计学意义(P<0.05)。干预后组手关节操掌握率为81.25%,高于干预前组的61.33%,差异有统计学意义(P<0.05)。结论RA患者手关节操干预期间实施质量持续改进,能够减少关节疼痛及关节肿胀个数,降低疼痛程度,促进功能康复,提高患者满意度和关节操掌握率。 展开更多

[目的]探讨尺神经松解前置术联合黄芪桂枝五物汤对肘管综合征(CTS)气血两虚证患者手内肌功能及尺神经运动传导功能的影响。[方法]采用真实世界回顾性队列研究,收集2021年3月—2023年3月河北省沧州中西医结合医院行尺神经松解前置术的100例CTS气血两虚证患者,根据治疗方案不同分成联合组50例,手术组50例。手术组接受尺神经松解前置术+甲钴胺治疗,联合组在手术组基础上联合黄芪桂枝五物汤治疗。比较两组患者临床疗效、手内肌功能、尺神经运动传导功能、手握力、上肢功能评定表(DASH)评分。[结果]联合组的临床疗效总有效率为80.00%,高于手术组的62.00%,差异有统计学意义(P<0.05)。手内肌功能总优良率为86.00%,高于手术组的66.00%,差异有统计学意义(P<0.05)。尺神经功能优良率为78.00%,高于手术组的58.00%,差异有统计学意义(P<0.05)。治疗后,两组尺神经运动传导速度(MCV)及手握力均升高,且联合组高于手术组(P<0.05),DASH评分均降低,且联合组低于手术组(P<0.05),差异有统计学意义。两组治疗前后病情分度比较差异均无统计学意义(P>0.05),且治疗后两组均无重度患者。[结论]尺神经松解前置术联合黄芪桂枝五物汤能促进CTS气血两虚证患者手内肌功能及尺神经运动传导功能的恢复,疗效优于术后联合西药治疗。 展开更多

目的探讨基于运动学习理论的手功能康复训练对经桡动脉入路行经皮冠状动脉介入(PCI)术后患者上肢及手功能恢复、疼痛缓解及手掌肿胀程度改善的效果。方法选取2022年1—10月于北京市大兴区人民医院住院的PCI术后患者400例,2022年1—5月20... 目的探讨基于运动学习理论的手功能康复训练对经桡动脉入路行经皮冠状动脉介入(PCI)术后患者上肢及手功能恢复、疼痛缓解及手掌肿胀程度改善的效果。方法选取2022年1—10月于北京市大兴区人民医院住院的PCI术后患者400例,2022年1—5月200例作为对照组,2022年6—10月200例作为观察组。对照组接受常规护理,观察组在常规护理基础上增加基于运动学习理论的手功能康复训练。通过上肢及手功能障碍问卷(DASH)、疼痛视觉模拟评分法(VAS)评分、手掌周径等指标,量化比较两组患者术后功能恢复情况、疼痛程度及手掌肿胀程度的差异。结果观察组患者干预后DASH评分显著低于对照组(P<0.05),VAS评分在术后30 min、2 h及4 h均显著低于对照组(P<0.05),术后2、4 h的手掌周径也显著短于对照组(P<0.05)。此外,观察组患者的肢体疼痛、肿胀缓解效果及手功能改善评分均显著优于对照组(P<0.05)。结论基于运动学习理论的手功能康复训练能够有效促进PCI术后患者的上肢及手功能恢复,缓解术后疼痛,减少手掌肿胀,具有较高的临床应用价值。 展开更多

Background: In Japan, due to the COVID-19 pandemic, various behavioral restrictions were implemented from March 2020, resulting in an increased number of elderly people with reduced participation in care and rehabilitation. As a result, the decline in motor and cognitive functions became more pronounced. Purpose: The study examined how activities combining passive horticultural therapy, which is always introduced, with active functional recovery training, which is used in the functional recovery training of patients with hemiplegia, influence the behavior of elder people using the facility. Methods: 36 elder people with care need 1 or 2 who regularly used the day-care facilities after the pandemic ended and who agreed to participate in the study was included. The assessment data were every three months from December 2021 to December 2023 after the COVID-19 pandemic, the MMSE and Moca-J, TMIG-Index, and GDS15. AI-based text mining was used to compare changes in visit reports before and after the introduction of the Posture Chart and Red Code, and the relationship between the language used. Results: The results of MMSE and Moca-J were both significantly higher in Group B and C than in Group A. TIMG assessment, was significantly higher in Group B and C than in Group A, and Group C was significantly higher than Group B. The GDS, an assessment of depression in old age, tended to be significantly higher in Group A than in Groups B and C. In the “Co-Occurrence Network Analysis” shown after the introduction of the latest functional recovery training equipment such as the Red Cord, its results showed a high association between the words Bathing and CARE and words such as POSTURE, Red-Code, and AI, App, which are used in functional training and rehabilitation, and their analysis. Conclusion: At day-care facilities specializing in functional recovery training and rehabilitation, a combination of horticultural therapy and the latest functional training equipment found to help maintain the physical and mental health of elderly people certified as requiring nursing care. Since this is a short-term study following behavioral restrictions, long-term research should continue to study the effects of adding functional training, handled by rehabilitation professionals, to horticultural therapy. 展开更多
