Resting-state brain network remodeling after different nerve reconstruction surgeries:a functional magnetic resonance imaging study in brachial plexus injury rats

Distinct brain remodeling has been found after different nerve reconstruction strategies,including motor representation of the affected limb.However,differences among reconstruction strategies at the brain network level have not been elucidated.This study aimed to explore intranetwork changes related to altered peripheral neural pathways after different nerve reconstruction surgeries,including nerve repair,endto-end nerve transfer,and end-to-side nerve transfer.Sprague–Dawley rats underwent complete left brachial plexus transection and were divided into four equal groups of eight:no nerve repair,grafted nerve repair,phrenic nerve end-to-end transfer,and end-to-side transfer with a graft sutured to the anterior upper trunk.Resting-state brain functional magnetic resonance imaging was obtained 7 months after surgery.The independent component analysis algorithm was utilized to identify group-level network components of interest and extract resting-state functional connectivity values of each voxel within the component.Alterations in intra-network resting-state functional connectivity were compared among the groups.Target muscle reinnervation was assessed by behavioral observation(elbow flexion)and electromyography.The results showed that alterations in the sensorimotor and interoception networks were mostly related to changes in the peripheral neural pathway.Nerve repair was related to enhanced connectivity within the sensorimotor network,while end-to-side nerve transfer might be more beneficial for restoring control over the affected limb by the original motor representation.The thalamic-cortical pathway was enhanced within the interoception network after nerve repair and end-to-end nerve transfer.Brain areas related to cognition and emotion were enhanced after end-to-side nerve transfer.Our study revealed important brain networks related to different nerve reconstructions.These networks may be potential targets for enhancing motor recovery. 展开更多

BACKGROUND Successful aging(SA)refers to the ability to maintain high levels of physical,cognitive,psychological,and social engagement in old age,with high cognitive function being the key to achieving SA.AIM To explore the potential characteristics of the brain network and functional connectivity(FC)of SA.METHODS Twenty-six SA individuals and 47 usual aging individuals were recruited from community-dwelling elderly,which were taken the magnetic resonance imaging scan and the global cognitive function assessment by Mini Mental State Examination(MMSE).The resting state-functional magnetic resonance imaging data were preprocessed by DPABISurf,and the brain functional network was conducted by DPABINet.The support vector machine model was constructed with altered functional connectivities to evaluate the identification value of SA.RESULTS The results found that the 6 inter-network FCs of 5 brain networks were significantly altered and related to MMSE performance.The FC of the right orbital part of the middle frontal gyrus and right angular gyrus was mostly increased and positively related to MMSE score,and the FC of the right supramarginal gyrus and right temporal pole:Middle temporal gyrus was the only one decreased and negatively related to MMSE score.All 17 significantly altered FCs of SA were taken into the support vector machine model,and the area under the curve was 0.895.CONCLUSION The identification of key brain networks and FC of SA could help us better understand the brain mechanism and further explore neuroimaging biomarkers of SA. 展开更多

Studies have shown that functional network connection models can be used to study brain net- work changes in patients with schizophrenia. In this study, we inferred that these models could also be used to explore functional network connectivity changes in stroke patients. We used independent component analysis to find the motor areas of stroke patients, which is a novel way to determine these areas. In this study, we collected functional magnetic resonance imaging datasets from healthy controls and right-handed stroke patients following their first ever stroke. Using independent component analysis, six spatially independent components highly correlat- ed to the experimental paradigm were extracted. Then, the functional network connectivity of both patients and controls was established to observe the differences between them. The results showed that there were 11 connections in the model in the stroke patients, while there were only four connections in the healthy controls. Further analysis found that some damaged connections may be compensated for by new indirect connections or circuits produced after stroke. These connections may have a direct correlation with the degree of stroke rehabilitation. Our findings suggest that functional network connectivity in stroke patients is more complex than that in hea- lthy controls, and that there is a compensation loop in the functional network following stroke. This implies that functional network reorganization plays a very important role in the process of rehabilitation after stroke. 展开更多

Background: The mechanisms by which acupuncture affects poststroke cognitive impairment (PSCI) remain unclear. Objective: To investigate brain functional network (BFN) changes in patients with PSCI after acupuncture therapy. Methods: Twenty-two PSCI patients who underwent acupuncture therapy in our hospital were enrolled as research subjects. Another 14 people matched for age, sex, and education level were included in the normal control (HC) group. All the subjects underwent resting-state functional magnetic resonance imaging (rs-fMRI) scans;the PSCI patients underwent one scan before acupuncture therapy and another after. The network metric difference between PSCI patients and HCs was analyzed via the independent-sample t test, whereas the paired-sample t test was employed to analyze the network metric changes in PSCI patients before vs. after treatment. Results: Small-world network attributes were observed in both groups for sparsities between 0.1 and 0.28. Compared with the HC group, the PSCI group presented significantly lower values for the global topological properties (γ, Cp, and Eloc) of the brain;significantly greater values for the nodal attributes of betweenness centrality in the CUN. Although the relationship between anesthesia and consciousness has been investigated for decades, our understanding of the underlying neural mechanisms of anesthesia and consciousness remains rudimentary, which limits the development of systems for anesthesia monitoring and consciousness evaluation. Moreover, the current practices for anesthesia monitoring are mainly based on methods that do not provide adequate information and may present obstacles to the precise application of anesthesia. Most recently, there has been a growing trend to utilize brain network analysis to reveal the mechanisms of anesthesia, with the aim of providing novel insights to promote practical application. This review summarizes recent research on brain network studies of anesthesia, and compares the underlying neural mechanisms of consciousness and anesthesia along with the neural signs and measures of the distinct aspects of neural activity. Using the theory of cortical fragmentation as a starting point, we introduce important methods and research involving connectivity and network analysis. We demonstrate that whole-brain multimodal network data can provide important supplementary clinical information. More importantly, this review posits that brain network methods, if simplified, will likely play an important role in improving the current clinical anesthesia monitoring systems. 展开更多

Extracting Multiple Nodes in a Brain Region of Interest for Brain Functional Network Estimation and Classification

Purpose: Brain functional networks (BFNs) has become important approach for diagnosis of some neurological or psychological disorders. Before estimating BFN, obtaining blood oxygen level dependent (BOLD) representative signals from brain regions of interest (ROIs) is important. In the past decades, the common method is generally to take a ROI as a node, averaging all the voxel time series inside it to extract a representative signal. However, one node does not represent the entire information of this ROI, and averaging method often leads to signal cancellation and information loss. Inspired by this, we propose a novel model extraction method based on an assumption that a ROI can be represented by multiple nodes. Methods: In this paper, we first extract multiple nodes (the number is user-defined) from the ROI based on two traditional methods, including principal component analysis (PCA), and K-means (Clustering according to the spatial position of voxels). Then, canonical correlation analysis (CCA) was issued to construct BFNs by maximizing the correlation between the representative signals corresponding to the nodes in any two ROIs. Finally, to further verify the effectiveness of the proposed method, the estimated BFNs are applied to identify subjects with autism spectrum disorder (ASD) and mild cognitive impairment (MCI) from health controls (HCs). Results: Experimental results on two benchmark databases demonstrate that the proposed method outperforms the baseline method in the sense of classification performance. Conclusions: We propose a novel method for obtaining nodes of ROId based on the hypothesis that a ROI can be represented by multiple nodes, that is, to extract the node signals of ROIs with K-means or PCA. 目的:现有研究中多是基于传统Meta分析研究重复经颅磁刺激、经颅直流电刺激等单一刺激方案对脑卒中患者下肢运动功能障碍的疗效,无法明确何种刺激方案为最优选择。此次研究采用网状Meta分析的方式,系统评价非侵入性脑刺激的不同方案治... 目的:现有研究中多是基于传统Meta分析研究重复经颅磁刺激、经颅直流电刺激等单一刺激方案对脑卒中患者下肢运动功能障碍的疗效,无法明确何种刺激方案为最优选择。此次研究采用网状Meta分析的方式,系统评价非侵入性脑刺激的不同方案治疗脑卒中后下肢运动功能障碍的临床疗效。方法:检索中国知网、万方、维普、中国生物医学文献数据库、Pub Med、Medline和Web of science数据库,搜集关于非侵入性脑刺激不同方案治疗脑卒中后下肢运动功能障碍的随机对照试验,检索时段为各数据库建库至2024-10-01。对纳入研究进行数据提取,利用Rev Man5.4.1软件进行传统Meta分析并评估纳入研究的质量,Stata 17.0软件进行网状Meta分析。结果:(1)纳入研究39篇,共2 920例患者,涉及常规康复训练、高频重复经颅磁刺激、低频重复经颅磁刺激、间歇性θ爆发刺激、连续性θ爆发刺激、经颅直流电刺激6种治疗方法;(2)网状Meta分析结果显示,高频重复经颅磁刺激、低频重复经颅磁刺激、间歇性θ爆发刺激、经颅直流电刺激在Fugl-Meyer下肢运动功能评定量表方面的效果均优于常规康复训练;(3)在提高Berg平衡量表评分方面,高频重复经颅磁刺激、低频重复经颅磁刺激、间歇性θ爆发刺激与常规康复训练之间差异有显著性意义(P <0.05),高频重复经颅磁刺激与间歇性θ爆发刺激之间差异有显著性意义(P <0.05);(4)在提高患者改良Barthel或Barthel指数中,高频重复经颅磁刺激、低频重复经颅磁刺激、间歇性θ爆发刺激、经颅直流电刺激优于常规康复训练;(5)累积排序图下显示,在Fugl-Meyer下肢运动功能评定量表、Berg平衡量表、改良Barthel或Barthel指数方面,高频重复经颅磁刺激的疗效最好,其次为低频重复经颅磁刺激。结论:高频重复经颅磁刺激、低频重复经颅磁刺激都可以改善脑卒中后患者的下肢运动功能以及平衡功能,并且能不同程度地提高患者的日常生活活动能力,且高频重复经颅磁刺激的疗效优于低频重复经颅磁刺激。 展开更多

目的:已有研究证实,神经调控技术对帕金森病患者的临床症状有较为明显的改善作用,并且不同神经调控刺激方式之间存在疗效差异。采用网状Meta分析评价不同神经调控技术刺激方式改善帕金森病患者睡眠障碍及抑郁焦虑等非运动症状的疗效,探... 目的:已有研究证实,神经调控技术对帕金森病患者的临床症状有较为明显的改善作用,并且不同神经调控刺激方式之间存在疗效差异。采用网状Meta分析评价不同神经调控技术刺激方式改善帕金森病患者睡眠障碍及抑郁焦虑等非运动症状的疗效,探讨最佳神经调控刺激方案。方法:应用计算机检索中国生物医学文献数据库、万方数据库、维普数据库、中国知网及Web of Science、PubMed、The Cochrane Library、EMbase数据库中关于神经调控技术改善帕金森病患者睡眠障碍及抑郁焦虑的随机对照试验,对照组采用常规治疗(药物、常规康复治疗等)或假刺激,试验组在对照组的基础上加用神经调控技术。采用PEDro量表和Cochrane协作网推荐的偏倚风险评估工具对纳入的文献进行质量评价,采用RevMan 5.4和Stata 17.0对4个结局指标(睡眠障碍、焦虑症状、抑郁症状、生活质量)进行网状Meta分析。结果:①最终纳入29项随机对照试验,涉及6种神经调控刺激方式,包括经颅直流电刺激、高频重复经颅磁刺激、低频重复经颅磁刺激、丘脑底核脑深部电刺激、苍白球内侧脑深部电刺激、多靶点脑深部电刺激。②网状Meta分析结果显示:与常规治疗相比,经颅直流电刺激[SMD=-2.57,95%CI=(-4.52,-0.63),P<0.05]在改善帕金森病患者睡眠障碍方面的疗效最佳;在改善帕金森病患者抑郁症状方面,苍白球内侧脑深部电刺激[SMD=-1.00,95%CI=(-1.87,-0.14),P<0.05]的疗效最佳,其次为低频重复经颅磁刺激[SMD=-0.91,95%CI=(-1.60,-0.23),P<0.05]、丘脑底核脑深部电刺激[SMD=-0.82,95%CI=(-1.56,-0.08),P<0.05]、高频重复经颅磁刺激[SMD=-0.75,95%CI=(-0.97,-0.53),P<0.05];在改善帕金森病患者焦虑症状方面,高频重复经颅磁刺激[SMD=-0.86,95%CI=(-1.54,-0.18),P<0.05]的疗效最佳;在改善患者生活质量方面,苍白球内侧脑深部电刺激[SMD=-0.79,95%CI=(-1.55,-0.04),P<0.05]的疗效最佳,其次为高频重复经颅磁刺激[SMD=-0.63,95%CI=(-0.90,-0.36),P<0.05]、经颅直流电刺激[SMD=-0.50,95%CI=(-0.80,-0.19),P<0.05]。结论:神经调控技术在改善帕金森病患者非运动症状方面具有显著疗效,其中,经颅直流电刺激改善睡眠障碍的疗效最佳,苍白球内侧脑深部电刺激改善抑郁症状的疗效最佳,高频重复经颅磁刺激改善焦虑症状的疗效最佳,苍白球内侧脑深部电刺激改善生活质量的疗效最佳。 展开更多

功能性近红外光谱技术预测围术期老年关节置换术患者术后谵妄的作用

目的探讨功能性近红外光谱技术(fNIRS)监测下的神经影像学特征在围术期老年关节置换术患者发生术后谵妄(POD)中的作用,为临床早期预测提供依据。方法纳入105例选择全身麻醉下行关节置换术的老年患者,于术前1天行简易精神状态评价量表(MM... 目的探讨功能性近红外光谱技术(fNIRS)监测下的神经影像学特征在围术期老年关节置换术患者发生术后谵妄(POD)中的作用,为临床早期预测提供依据。方法纳入105例选择全身麻醉下行关节置换术的老年患者,于术前1天行简易精神状态评价量表(MMSE)对患者认知进行评估,手术开始前使用fNIRS监测患者执行任务态时的脑氧饱和度变化,并于术后24、48、72 h使用3分钟谵妄诊断量表(3D-CAM)评估患者POD的发生情况。进行脑网络分析并采用Logistic回归分析探究老年关节置换术患者术前执行任务态时fNIRS监测数据与POD的关系,并构建受试者工作特征曲线(ROC曲线)评估诊断效能,Hosmer-Lemeshow拟合优度检验检测模型拟合优度。结果在105例患者中,有效分析例数为100例,其中20例(20%)发生POD。脑网络分析显示POD组(0.069±0.118)功能连接相关系数r值低于非POD组(0.073±0.084),其中右侧初级躯体感觉皮质-右侧初级运动皮层(RS1-RM1)、左侧前极区-右侧布罗卡三角区(LFP-RBA44)通道连接性低是POD发生的重要影响因素(P<0.05)。基于此结果构建ROC曲线下面积分别为0.797与0.784。Hosmer-Lemeshow拟合优度检验结果显示模型拟合良好(均P>0.5)。结论fNIRS监测的脑氧饱和度数据提取出的神经影像学特征与围术期老年关节置换术患者发生POD的风险存在显著相关性,其中术前RS1-RM1、LFP-RBA44脑网络通道连接性低是POD发生的重要影响因素,依托fNIRS预测POD的发生有利于早期干预并降低围术期并发症风险,提升医疗质量,推动精准医疗实践。 展开更多

目的探究孤独症谱系障碍共病注意缺陷/多动障碍(the comorbidity of autism spectrum disorder and attention-deficit/hyperactivity disorder,ASD+ADHD)和单纯孤独症谱系障碍(ASD-only)的动态脑功能连接差异。方法从孤独症脑影像数据... 目的探究孤独症谱系障碍共病注意缺陷/多动障碍(the comorbidity of autism spectrum disorder and attention-deficit/hyperactivity disorder,ASD+ADHD)和单纯孤独症谱系障碍(ASD-only)的动态脑功能连接差异。方法从孤独症脑影像数据交流2库(Autism Brain Image Data Exchange II,ABIDE-2)纳入40例ASD+ADHD患者、74例ASD-only患者及161名典型发育(typically developing,TD)者的静息态功能磁共振成像数据,进行动态功能连接分析,提取动态功能连接状态的时间属性进行组间比较。结果聚类分析得到3种重复出现、高度结构化的功能连接状态:状态1为强连接状态,以高强度的功能连接为主,出现频率低(6.16%);状态2为局部连接状态,局部有高强度正功能连接,其余多为弱连接,最频繁出现(51.31%);状态3为广泛连接状态,高强度正功能连接分布相对广泛,出现频率为42.63%。三组的状态3平均停留时间(P=0.006)和状态转换次数(P<0.001)差异有统计学意义,其中ASD+ADHD组的状态3平均停留时间[40.67(23.20,107.25)窗]高于ASD-only组[29.00(13.31,53.50)窗]和TD组[26.50(10.17,50.75)窗(]P<0.05),ASD+ADHD组状态转换次数[2(1,3)次]低于ASD-only组[3(2,5)次]和TD组[3(2,4)次(]P<0.05,Bonferroni校正)。ASD+ADHD组的状态3平均停留时间与执行功能行为评级量表的任务监控维度呈正相关,但无统计学意义(r=0.355,P=0.054)。结论与ASD-only和TD组相比,ASD+ADHD组患者偏好于停留在广泛连接状态中,较少切换状态模式,表现出脑功能活动灵活性不足,这可能是ASD+ADHD潜在的神经生理学特征。 展开更多

目的:已有研究证实非侵入性脑刺激能够改善孤独症儿童局限性、重复性行为和社交障碍,然而不同刺激方案的疗效存在差异。此次研究系统评价非侵入性脑刺激对孤独症儿童核心症状的疗效,并比较不同刺激方案的疗效差异。方法:检索中国知网、... 目的:已有研究证实非侵入性脑刺激能够改善孤独症儿童局限性、重复性行为和社交障碍,然而不同刺激方案的疗效存在差异。此次研究系统评价非侵入性脑刺激对孤独症儿童核心症状的疗效,并比较不同刺激方案的疗效差异。方法:检索中国知网、维普、万方和CBM、PubMed、Embase、Cochrane Library和Web of Science数据库,搜集非侵入性脑刺激治疗孤独症儿童核心症状的随机对照试验,检索时限为各数据库建库至2025年3月,由2名研究者分别独立进行文献筛选、数据提取,使用Cochrane偏倚风险评价工具进行文献质量评价,采用Revman 5.4和Stata 17.0软件进行Meta分析。结果:最终纳入27篇文献,共涉及非侵入性脑刺激的10种刺激方案,涵盖1701例孤独症患儿。①传统Meta分析结果显示,非侵入性脑刺激在改善儿童孤独症评定量表、孤独症行为评定量表、孤独症治疗评估量表和重复刻板行为检查表评分方面效果均优于常规康复;②网状Meta分析结果显示,与常规康复相比,高频重复经颅磁刺激背外侧前额叶皮质[MD=-6.00,95%CI(-8.68,-3.33),P<0.05,SUCRA=89.5%]改善儿童孤独症评定量表评分的效果最佳,而高频重复经颅磁刺激Broca区[MD=-15.11,95%CI(-18.28,-11.95),P<0.05,SUCRA=91.1%]改善孤独症行为评定量表评分的效果最佳。结论:现有证据表明,高频重复经颅磁刺激改善孤独症儿童核心症状的疗效最佳。由于纳入研究普遍存在方法学异质性较大与样本规模有限的双重局限性,未来亟待通过大样本、方法学严谨的随机对照试验进一步验证当前结论的可靠性。 展开更多

目的:系统比较多种神经调控技术对脑卒中患者上肢运动功能恢复的疗效与安全性,并通过网状Meta分析对不同干预措施的相对优势进行排序,为临床康复干预提供循证依据。方法:系统检索中国知网、万方数据库、维普数据库、中国生物医学文献数... 目的:系统比较多种神经调控技术对脑卒中患者上肢运动功能恢复的疗效与安全性,并通过网状Meta分析对不同干预措施的相对优势进行排序,为临床康复干预提供循证依据。方法:系统检索中国知网、万方数据库、维普数据库、中国生物医学文献数据库、PubMed、EMbase、Web of Science、Cochrane Library数据库,纳入关于不同神经调控技术治疗脑卒中后上肢运动障碍的随机对照试验,检索时限为各数据库建库至2025年8月公开发表的中英文文献。对照组采用假刺激以及传统康复手段,试验组为在对照组基础上加入不同的神经调控技术。采用Cochrane随机对照试验偏倚风险评价工具对纳入文献进行质量评价。采用Stata 16.0和RevMan 5.4软件进行网状Meta分析。结果:纳入51项随机对照试验,涉及12种神经调控刺激方式。网状Meta分析结果显示:与常规治疗相比,高频重复经颅磁刺激(MD=11.50,95%CI:6.83-16.16,P<0.05)在改善脑卒中患者上肢基础运动功能恢复方面的疗效最佳;在改善脑卒中患者上肢功能的实用性与精细性方面,连续性节律爆发刺激(MD=12.10,95%CI:44.99-19.21,P<0.05;MD=9.60,95%CI:1.32-17.88,P<0.05)的疗效最佳;在改善脑卒中患者日常生活活动能力受限或伴有明显痉挛方面,阴极经颅直流电刺激(MD=15.40,95%CI:0.03-30.77,P<0.05;MD=-0.83,95%CI:-1.64至-0.03,P<0.05)的疗效最佳。结论:在以促进上肢基础运动功能恢复为目标时,高频重复经颅磁刺激疗效最好;在提升上肢功能的实用性与精细性方面,连续性节律爆发刺激疗效最好;对于提高日常生活能力伴有明显痉挛的患者,阴极经颅直流电刺激疗效最好。 展开更多

目的:通过网状Meta分析整合直接与间接证据,比较不同非侵入性脑刺激技术及参数对帕金森病患者步态和平衡功能的影响,并排序最佳干预方案。方法:检索CNKI、万方、维普、CBM、PubMed、Cochrane图书馆、EMbase和Web of Science数据库,筛选... 目的:通过网状Meta分析整合直接与间接证据,比较不同非侵入性脑刺激技术及参数对帕金森病患者步态和平衡功能的影响,并排序最佳干预方案。方法:检索CNKI、万方、维普、CBM、PubMed、Cochrane图书馆、EMbase和Web of Science数据库,筛选有关非侵入性脑刺激改善帕金森病患者步态和平衡障碍的随机对照试验,检索时限截止到2025-06-16。对纳入的研究进行数据提取,采用RevMan 5.4.1软件和Stata 17.0软件进行统计学处理。结果:①纳入47篇研究,共2767例患者,试验组1399例、对照组1368例;②传统Meta分析结果显示,高频重复经颅磁刺激可以降低统一帕金森病评定量表第三部分(UPDRSⅢ)评分、冻结步态问卷(FOG-Q)评分,缩短起立-行走计时试验(TUGT)时间,增加步长,改善步速,提高Berg平衡量表评分,且均优于常规治疗(P<0.05);经颅直流电刺激可以降低冻结步态问卷评分、缩短起立-行走计时试验时间,增加步长,改善步速,提高Berg平衡量表评分,且均优于常规治疗(P<0.05);低频重复经颅磁刺激可以降低统一帕金森病评定量表第三部分评分,与常规治疗比较有显著性差异(P<0.05);3种脑刺激方法对于步频的改善均无统计学意义(P>0.05),受限于研究数量少,需进一步验证;③网状Meta分析结果显示(以下排序均基于间接比较),在降低统一帕金森评定量表第三部分评分方面:高频重复经颅磁刺激选择初级运动皮质区(M1)+背外侧前额叶皮质区(DLPFC)双侧靶点联合刺激的累积概率排序最高(95.2%),在初级运动皮质区上刺激排序次之(72.5%);在缩短起立-行走计时试验时间方面:高频重复经颅磁刺激在背外侧前额叶皮质区刺激的累积概率排序最高(85.5%),在初级运动皮质区上刺激排序次之(69.0%);在改善步速方面:高频重复经颅磁刺激在背外侧前额叶皮质区的累积概率排序最高(92.5%),在初级运动皮质区上刺激排序次之(76.7%);在提高Berg平衡量表评分方面:高频重复经颅磁刺激在初级运动皮质区上刺激的累积概率排序最高(79.9%),经颅直流电刺激在小脑上刺激排序次之(79.8%);④GRADE证据质量评价结果显示,统一帕金森评定量表第三部分、冻结步态问卷评分、起立-行走计时试验、步长证据等级为中级,步速、步频、Berg平衡量表评分为低级。结论:不同类型非侵入性脑刺激都可以改善帕金森病患者的步态和平衡功能。背外侧前额叶皮质区靶向的高频重复经颅磁刺激对步态功能的改善优于初级运动皮质(中等证据),而初级运动皮质靶向的高频重复经颅磁刺激对平衡功能的改善优于小脑靶向的经颅直流电刺激(低级证据)。 展开更多

Maintaining sustained attention during a prolonged cognitive task often comes at a cost: high levels of mental fatigue. Heuristically, mental fatigue refers to a feeling of tiredness or exhaustion, and a disengagement from the task at hand;it manifests as impaired cognitive and behavioral performance. In order to effectively reduce the undesirable yet preventable consequences of mental fatigue in many real-world workspaces, a better understanding of the underlying neural mechanisms is needed, and continuous efforts have been devoted to this topic. In comparison with conventional univariate approaches, which are widely utilized in fatigue studies, convergent evidence has shown that multivariate functional connectivity analysis may lead to richer information about mental fatigue. In fact, mental fatigue is increasingly thought to be related to the deviated reorganization of functional connectivity among brain regions in recent studies. In addition, graph theoretical analysis has shed new light on quantitatively assessing the reorganization of the brain functional networks that are modulated by mental fatigue. This review article begins with a brief introduction to neuroimaging studies on mental fatigue and the brain connectome, followed by a thorough overview of connectome studies on mental fatigue. Although only a limited number of studies have been published thus far, it is believed that the brain connectome can be a useful approach not only for the elucidation of underlying neural mechanisms in the nascent field of neuroergonomics, but also for the automatic detection and classification of mental fatigue in order to address the prevention of fatigue-related human error in the near future. 展开更多

Patients with age-related hearing loss face hearing difficulties in daily life.The causes of age-related hearing loss are complex and include changes in peripheral hearing,central processing,and cognitive-related abilities.Furthermore,the factors by which aging relates to hearing loss via changes in audito ry processing ability are still unclear.In this cross-sectional study,we evaluated 27 older adults(over 60 years old) with age-related hearing loss,21 older adults(over 60years old) with normal hearing,and 30 younger subjects(18-30 years old) with normal hearing.We used the outcome of the uppe r-threshold test,including the time-compressed thres h old and the speech recognition threshold in noisy conditions,as a behavioral indicator of auditory processing ability.We also used electroencephalogra p hy to identify presbycusis-related abnormalities in the brain while the participants were in a spontaneous resting state.The timecompressed threshold and speech recognition threshold data indicated significant diffe rences among the groups.In patients with age-related hearing loss,information masking(babble noise) had a greater effect than energy masking(speech-shaped noise) on processing difficulties.In terms of resting-state electroencephalography signals,we observed enhanced fro ntal lobe(Brodmann’s area,BA11) activation in the older adults with normal hearing compared with the younger participants with normal hearing,and greater activation in the parietal(BA7) and occipital(BA19) lobes in the individuals with age-related hearing loss compared with the younger adults.Our functional connection analysis suggested that compared with younger people,the older adults with normal hearing exhibited enhanced connections among networks,including the default mode network,sensorimotor network,cingulo-opercular network,occipital network,and frontoparietal network.These results suggest that both normal aging and the development of age-related hearing loss have a negative effect on advanced audito ry processing capabilities and that hearing loss accele rates the decline in speech comprehension,especially in speech competition situations.Older adults with normal hearing may have increased compensatory attentional resource recruitment represented by the to p-down active listening mechanism,while those with age-related hearing loss exhibit decompensation of network connections involving multisensory integration. 展开更多

Cleft lip and/or palate(CLP)are the most common craniofacial malformations in humans.Speech problems often persist even after cleft repair,such that follow-up articulation training is usually required.However,the neural mechanism behind effective articulation training remains largely unknown.We used fMRI to investigate the differences in brain activation,functional connectivity,and effective connectivity across CLP patients with and without articulation training and matched normal participants.We found that training promoted task-related brain activation among the articulation-related brain networks,as well as the global attributes and nodal efficiency in the functional-connectivity-based graph of the network.Our results reveal the neural correlates of effective articulation training in CLP patients,and this could contribute to the future improvement of the post-repair articulation training program. 展开更多

AIM To increase our insight in the neuronal mechanisms underlying cognitive side-effects of antiepileptic drug(AED) treatment.METHODS The relation between functional magnetic resonance-acquired brain network measures, AED use, and cognitive function was investigated. Three groups of patients with epilepsy with a different risk profile for developing cognitive side effects were included: A "low risk" category(lamotrigine or levetiracetam, n=16), an "intermediate risk" category(carbamazepine, oxcarbazepine, phenytoin, or valproate, n=34) and a "high risk" category(topiramate, n=5). Brain connectivity was assessed using resting state functional magnetic resonance imaging and graph theoretical network analysis. The Computerized Visual Searching Task was used to measure central information processing speed, a common cognitive side effect of AED treatment. RESULTS Central information processing speed was lower in patients taking AEDs from the intermediate and high risk categories, compared with patients from the low risk category. The effect of risk category on global efficiency was significant(P < 0.05, ANCOVA), with a significantly higher global efficiency for patient from the low category compared with the high risk category(P < 0.05, post-hoc test). Risk category had no significant effect on the clustering coefficient(ANCOVA, P > 0.2). Also no significant associations between information processing speed and global efficiency or the clustering coefficient(linear regression analysis, P > 0.15) were observed. CONCLUSION Only the four patients taking topiramate show aberrant network measures, suggesting that alterations in functional brain network organization may be only subtle and measureable in patients with more severe cognitive side effects. 展开更多
