In a double-blind randomized controlled trial, 112 patients with hemiplegia after stroke were randomly divided into two groups: experimental and control. In experimental group, the patients received LF r TMS and VR training treatment, and those in control group received sham r TMS and VR training treatment. Participants in both groups received therapy of 6 days per week for 4 weeks. The primary endpoint including the upper limb motor function test of Fugl-meyer assessment(U-FMA) and wolf motor function test(WMFT), and the secondary endpoint including modified Barthel index(MBI) and 36-item Short Form Health Survey Questionnaire(SF-36) were assessed before and 4 weeks after treatment. Totally, 108 subjects completed the study(55 in experimental group and 53 in control group respectively). After 4-week treatment, the U-FMA scores [mean difference of 13.2, 95% confidence interval(CI) 3.6 to 22.7, P〈0.01], WMFT scores(mean difference of 2.9, 95% CI 2.7 to 12.3, P〈0.01), and MBI scores(mean difference 16.1, 95% CI 3.8 to 9.4, P〈0.05) were significantly increased in the experimental group as compared with the control group. The results suggested the combined use of LF r TMS with VR training could effectively improve the upper limb function, the living activity, and the quality of life in patients with hemiplegia following subacute stroke, which may provide a better rehabilitation treatment for subacute stroke.展开更多

AIM: To investigate the feasibility of a 6-wk progressive strength-training programme commenced shortly after hip fracture surgery in community-dwelling patients.METHODS: This prospective, single-blinded cohort study ...AIM: To investigate the feasibility of a 6-wk progressive strength-training programme commenced shortly after hip fracture surgery in community-dwelling patients.METHODS: This prospective, single-blinded cohort study evaluated 31 community-dwelling patients from four outpatient geriatric health centres aged 60 years or older, who started a 6-wk programme at a mean of 17.5 ± 5.7 d after hip fracture surgery. The intervention consisted primarily of progressive fractured kneeextension and bilateral leg press strength training(twice weekly), with relative loads commencing at 15 and increasing to 10 repetitions maximum(RM), with three sets in each session. The main measurements included progression in weight loads, hip fracture-related pain during training, maximal isometric knee-extension strength, new mobility score, the timed up and go test, the 6-min walk test and the 10-meter fast speed walk test, assessed before and after the programme.RESULTS: Weight loads in kilograms in the fractured limb knee-extension strength training increased from 3.3 ± 1.5 to 5.7 ± 1.7 and from 6.8 ± 2.4 to 7.7 ± 2.6, respectively, in the first and last 2 wk(P < 0.001). Correspondingly, the weight loads increased from 50.3 ± 1.9 to 90.8 ± 40 kg and from 108.9 ± 47.7 to 121.9 ± 54 kg in the bilateral leg press exercise(P < 0.001). Hip fracture-related pain was reduced, and large improvements were observed in the functional outcome measurements, e.g., the 6-min walk test improved from 200.6 ± 79.5 to 322.8 ± 68.5 m(P < 0.001). The fractured limb knee-extension strength deficit was reduced from 40% to 17%, compared with the non-fractured limb. Ten patients reported knee pain as a minor restricting factor during the last 10 RM knee-extension strength-training sessions, but with no significant influences on performance. CONCLUSION: Progressive strength training, initiated shortly after hip fracture surgery, seems feasible and does not increase hip fracture-related pain. Progressive strength training resulted in improvement, although a strength deficit of 17% persisted in the fractured limb compared with the non-fractured limb.展开更多

The basic idea is to...Based on the analysis of the high-order compatibility optimization method proposed by predecessors, a new training image optimization method based on data event repetition probability is proposed. The basic idea is to extract the data event contained in the condition data and calculate the number of repetitions of the extracted data events and their repetition probability in the training image to obtain two statistical indicators, unmatched ratio and repeated probability variance of data events. The two statistical indicators are used to characterize the diversity and stability of the sedimentary model in the training image and evaluate the matching of the geological volume spatial structure contained in data of the well block to be modeled. The unmatched ratio reflects the completeness of geological model in training image, which is the first choice index. The repeated probability variance reflects the stationarity index of geological model of each training image, and is an auxiliary index. Then, we can integrate the above two indexes to achieve the optimization of training image. Multiple sets of theoretical model tests show that the training image with small variance and low no-matching ratio is the optimal training image. The method is used to optimize the training image of turbidite channel in Plutonio oilfield in Angola. The geological model established by this method is in good agreement with the seismic attributes and can better reproduce the morphological characteristics of the channels and distribution pattern of sands.展开更多
