Hepatic portal venous gas(HPVG)is a rare yet clinically significant radiological manifestation characterized by diverse etiologies,unclear pathophysiology,and nonspecific clinical presentations.Its appearance frequently heralds a severe underlying condition,with a reported mortality rate as high as 75%,earning it the moniker“harbinger of death”.In recent years,with advancements in diagnosis and treatment of HPVG,its mortality rate has been reduced to below 40%.Nevertheless,the infrequency of HPVG,coupled with its complex etiologies and rapid progression,often leaves clinicians failing to provide timely intervention due to a lack of awareness,resulting in the death of patient.Currently,there is no consensus on the standardized diagnosis and treatment strategies for HPVG.The purpose of this review is to synthesize the existing literature on HPVG and to discuss the advancements in understanding its clinical manifestation,etiologies,pathogenesis,diagnostic methods,and treatment strategies,as well as to emphasize the necessity for further research to enhance our comprehension of its pathophysiology and to refine diagnostic and therapeutic approaches. 展开更多

Owing to the complex and often asymptomatic presentations,the diagnosis of biliopancreatic diseases,including pancreatic and biliary malignancies,remains challenging.Recent technological advancements have remarkably improved the diagnostic accuracy and patient outcomes in these diseases.This review explores key advancements in diagnostic modalities,including biomarkers,imaging techniques,and artificial intelligence(AI)-based technologies.Biomarkers,such as cancer antigen 19-9,KRAS mutations,and inflammatory markers,provide crucial insights into disease progression and treatment responses.Advanced imaging modalities include enhanced computed tomography(CT),positron emission tomography-CT,magnetic resonance cholangiopancreatography,and endoscopic ultrasound.AI integration in imaging and pathology has enhanced diagnostic precision through deep learning algorithms that analyze medical images,automate routine diagnostic tasks,and provide predictive analytics for personalized treatment strategies.The applications of these technologies are diverse,ranging from early cancer detection to therapeutic guidance and real-time imaging.Biomarker-based liquid biopsies and AI-assisted imaging tools are essential for non-invasive diagnostics and individualized patient management.Furthermore,AI-driven models are transforming disease stratification,thus enhancing risk assessment and decision-making.Future studies should explore standardizing biomarker validation,improving AI-driven diagnostics,and expanding the accessibility of advanced imaging technologies in resource-limited settings.The continued development of non-invasive diagnostic techniques and precision medicine approaches is crucial for optimizing the detection and management of biliopancreatic diseases.Collaborative efforts between clinicians,researchers,and industry stakeholders will be pivotal in applying these advancements in clinical practice. 展开更多

Recent years have witnessed transformative changes brought about by artificial intelligence(AI)techniques with billions of parameters for the realization of high accuracy,proposing high demand for the advanced and AI chip to solve these AI tasks efficiently and powerfully.Rapid progress has been made in the field of advanced chips recently,such as the development of photonic computing,the advancement of the quantum processors,the boost of the biomimetic chips,and so on.Designs tactics of the advanced chips can be conducted with elaborated consideration of materials,algorithms,models,architectures,and so on.Though a few reviews present the development of the chips from their unique aspects,reviews in the view of the latest design for advanced and AI chips are few.Here,the newest development is systematically reviewed in the field of advanced chips.First,background and mechanisms are summarized,and subsequently most important considerations for co-design of the software and hardware are illustrated.Next,strategies are summed up to obtain advanced and AI chips with high excellent performance by taking the important information processing steps into consideration,after which the design thought for the advanced chips in the future is proposed.Finally,some perspectives are put forward. 展开更多

Risk factors for paternal perinatal depression in Chinese advanced maternal age couples:A regression mixture model

BACKGROUND Paternal perinatal depression(PPD)is closely associated with maternal mental health challenges,marital strain,and adverse child developmental outcomes.Despite its significant impact,PPD remains under-recognized in family-centered clinical practice.Concurrently,against the backdrop of rising rates of delayed marriage and China’s Maternity Incentive Policy,the proportion of women giving birth at an advanced maternal age is increasing.Nevertheless,research specifically examining PPD among spouses of older mothers remains critically scarce,both in China and globally.AIM To investigate PPD and its influencing factors in Chinese advanced maternal age families.METHODS This cross-sectional study included 358 participants;it was conducted among fathers of pregnant women of advanced maternal age at five hospitals in the Pearl River Delta region of China from September 2023 to June 2024.Data were collected via a general information questionnaire,the Social Support Rating Scale,and the Edinburgh Postnatal Depression Scale.Latent profile analysis and regression mixture models(RMMs)were adopted to analyze the latent PPD types and factors that influenced PPD.RESULTS The incidence of PPD was 16.48%,and three profiles were identified:Low-symptomatic(175 cases,48.89%),monophasic(140 cases,39.10%),and high-symptomatic(43 cases,12.01%).The RMM analysis revealed that first pregnancy,low income(<¥3000/month),part-time work,and a history of abnormal pregnancy were positively associated with the high-symptomatic type(P<0.05).Conversely,high subjective support and support utilization were negatively associated with the high-symptomatic type compared with the low-symptomatic type(P<0.05).Good couple relationships,high objective and subjective support,and high support utilization were negatively associated with monophasic disorder(P<0.05).CONCLUSION PPD incidence is high among Chinese fathers with advanced maternal age partners,and the characteristics of depression are varied.Healthcare practitioners should prioritize individuals with low levels of social support. 展开更多

AIM:To investigate the efficacy of the anal fistula plug(AFP) compared to the mucosa advancement flap(MAF),considered the best procedure for patients with a complex anal fistula.METHODS:The literature search included PubMed,EMBASE,Cochrane Library and OVID original studies on the topic of AFP compared to MAF for complex fistula-in-ano that had a deadline for publication by April 2011.Randomized controlled trials,controlled clinical trials and prospective cohort studies were included in the review.After information collection,a meta-analysis was performed using data on overall success rates as well as incidence of incontinence and morbidity.The quality of postoperative life was also included with the clinical results.RESULTS:Six studies involving 408 patients(AFP = 167,MAF = 241) were included in the meta-analysis.The differences in the overall success rates and incidence of fistula recurrence were not statistically significant between the AFP and MAF [risk difference(RD) =-0.12,95%CI:-0.39-0.14;RD = 0.13;95%CI:-0.18-0.43,respectively].However,for the AFP,the risk of postoperative impaired continence was lower(RD =-0.08,95%CI:-0.15--0.02) as was the incidence of other complications(RD =-0.06,95%CI:-0.11-0.00).The postoperative quality of life,for patients treated using the AFP was superior to that of the MAF patients.Patients treated with the AFP had less persistent pain of a shorter duration and the healing time of the fistula and hospital stay were also reduced.CONCLUSION:The AFP is an effective procedure for patients with a complex anal fistula;it has the same success rate but a lower risk of complications than the MAF and may also be associated with an improved postoperative quality of life.Additional evidence is needed to confirm these findings. 展开更多

Select group of patients with concurrent esophagealand gastric stricturing secondary to corrosive intake requires colonic or free jejunal transfer. These technically demanding reconstructions are associated with significant complications and have up to 18% ischemic conduit necrosis. Following corrosive intake, up to 30% of such patients have stricturing at the pyloro-duodenal canal area only and rest of the stomach is available for rather less complex and better perfused gastrointestinal reconstruction. Here we describe an alternative technique where we utilize stomach following distal gastric resection along with Roux-en-Y reconstruction instead of colonic or jejunal interposition. This neo-conduit is potentially superior in terms of perfusion, lower risk of gastro-esophageal anastomotic leakage and technical ease as opposed to colonic and jejunal counterparts. We have utilized the said technique in three patients with acceptable postoperative outcome. In addition this technique offers a feasible reconstruction plan in patients where colon is not available for reconstruction due to concomitant pathology. Utility of this technique may also merit consideration for gastroesophageal junction tumors. 展开更多

Efficacy of invisible advancement correction for mandibular retraction in adolescents based on Pancherz analysis 被引量:3

BACKGROUND Mandibular retraction is the main etiological mechanism of class II malocclusion in China and the subsequent distal molar relationship can cause functional discomfort in mastication,breathing and the temporomandibular joint.The use of mandibular advancement(MA)devices has recently emerged as an adolescent mandibular retraction treatment;however,current studies regarding the effect thereof are relatively few,and there is lack of sufficient clinical support.AIM To investigate the clinical effect of invisalign MA on the treatment of mandibular retraction in adolescents.METHODS This study included 30 adolescent patients who underwent treatment with the MA appliances from December 2017 to June 2021.The lateral cephalometric data before and after treatment were collected and imported into Dolphin Imaging software.The changes were measured by linear measurement superimposed with lateral cephalometric trajectory based on the Pancherz technology.RESULTS There was no significant difference in the length and position of maxilla before and after the treatment.The position of the mandible moved 3.13 mm,the length increased 4.14 mm,the mandibular ramus length increased 4.09 mm,the body length increased 4.25 mm,and the position of the condyle moved 1.03 mm forward after treatment.Additionally,changes in the incisor sagittal position and labial inclination were observed.The position of the upper incisor point moved back 1.33 mm,without statistical difference,the inclination and tooth angle decreased by 3.44°and 4.06°,respectively;the position of the lower incisor point was moved 2.98 mm,and the inclination and tooth angle increased by 2.62°and 1.23°,respectively.Furthermore,changes in the incisor overjet and molar relationship were seen.Overjet decreased by 4.31 mm,of which 1.78 mm was due to dental factors,accounting for 41.3%of the effect as opposed to 58.7%due to skeletal factors.Molar relationship improved 3.87 mm,with 1.34 mm due to dental factors,and dental and skeletal factors were accounted for 34.6%and 65.4%of the effect,respectively.CONCLUSION For adolescent patients with mandible retraction,invisalign MA can effectively promote the mandible growth,and it was proven to be mainly due to skeletal effects. 展开更多

BACKGROUND Microcystic adnexal carcinoma(MAC)is a rare malignant tumor of the skin that is commonly found on the face.It grows slowly and has a low mortality rate.However,for various reasons,including strong histological invasiveness,clinical inexperience and inadequate procedure design,immediate or permanent facial deformity may occur after surgical operations.CASE SUMMARY This article describes a middle-aged female artist who was diagnosed with MAC on the left upper lip.She declined the recommended treatment plan,which included two-stage reconstruction,skin grafting,or surgery that could have resulted in obvious facial dysfunction or esthetic deformity.We accurately designed a personalized procedure involving a“jigsaw puzzle advancement flap”for the patient based on the lesion location and the estimated area of skin loss.The procedure was successful;both pathological R0 resection and immediate and long-term esthetic reconstruction effects were achieved.CONCLUSION This study suggests that when treating facial MAC or other skin malignancies,a surgical team should have sufficient plastic surgery-related knowledge and skills.An optimal surgical plan for an individual is needed to achieve good facial esthetics and functional recovery and shorten the treatment course. 展开更多

Mandibular advancement devices(MADs)are one of the treatments used for Obstructive sleep apnea(OSA).MADs try to maintain the mandible in an advanced position to keep the upper airways open when sleeping.To achieve this goal,most current MADs limit the mouth opening to a few millimetres.The study of the kinematic behaviour of the patient’s jaw is essential in order to design devices that allow greater aperture ranges.For this purpose,a 3D multibody model that reproduces jaw movement has been developed in this work.To this end,the movement of the lower incisor has been determined by means of a vision system and reflective markers.In addition,the kinematics of the temporomandibular joint has been modelled.Next,the device is designed and printed using a cam-follower mechanism.This way,the cam profiles and the followers are optimally designed and positioned for each patient depending on the physiognomy of the jaw and the opening and advancing movement prescribed by the specialist. 展开更多

Neurological diseases and injuries present some of the great- est challenges in modern medicine, often causing irrevers- ible and lifelong burdens in the people whom they afflict. Conditions of stroke, traumatic brain injury, spinal cord injury, and neurodegenerative diseases have devastating con- sequences on millions of people each year, and yet there are currently no therapies or interventions that can repair the structure of neural circuits and restore neural tissue function in the brain and spinal cord. Despite the challenges of over- coming these limitations, there are many new approaches under development that hold much promise. Neural tissue engineering aims to restore and influence the function of damaged or diseased neural tissue generally through the use of stem cells and biomaterials. Many types of biomaterials may be implemented in various designs to influence the survival, differentiation, and function of developing stem cells, as well as to guide neurite extension and morphological architecture of cell cultures. Such designs may aim to reca- pitulate the cellular interactions, extracellular matrix char- acteristics, biochemical factors, and sequences of events that occur in neurodevelopment, in addition to supporting cell survival, differentiation, and integration into innate neural tissue. 展开更多

Examining the Relationship of Organizational Intelligence with Innovation Management and Career Advancement in an Organization

The present study aimed to examine the relationship of organizational intelligence with innovation management and career advancement in an organization. Considering its objective, this was a descriptive-correlational study. The statistical population included all high school managers in Zahedan at the year 2015. The sample contained 52 male managers and 51 female managers whom were selected through applying stratified random sampling method. To collect data, Organizational Intelligence Questionnaire, Innovation Management Inventory and Questionnaire of Career Advancement were used, the validity of which was assessed through applying the standpoint of a number of university professors and their reliability was obtained 0.74, 0.76, 0.84, respectively, using Cronbach’s alpha coefficient. Results indicated that considering managers’ perspectives, the status of organizational intelligence and career advancement were desirable;however, the status of innovation management was moderate. Moreover, organizational intelligence and its components were significantly and positively correlated with innovation management (r = 0.512, P < 0.01), and there was a significant and positive relationship between organizational intelligence and career advancement (r = 0.334, P < 0.01). Findings demonstrated that components of desire to change, spirit, unity and agreement, application of knowledge, performance pressure were significantly correlated to career advancement. However, components of strategic perspective and common fate were not significantly correlated with career advancement. Additionally, among the components of organizational intelligence, in the first step, application of knowledge and in the second step, desire to change were the best predicators of innovation management (which together could predict 28%). Application of knowledge was the only variable which entered the predictive model and predicted 16% of career advancement. Results of independent t-test indicated that no significant difference was found between male and female managers, considering their organizational intelligence, innovation management and career advancement. 展开更多
