Summary:Nurses'work-related fatigue has been recognized as a threat to nurse health and patient safety.The aim of this study was to assess the prevalence of fatigue among first-line nurses combating with COVID-19 ...Summary:Nurses'work-related fatigue has been recognized as a threat to nurse health and patient safety.The aim of this study was to assess the prevalence of fatigue among first-line nurses combating with COVID-19 in Wuhan,China,and to analyze its influencing factors on fatigue.A multi-center,descriptive,cross-sectional design with a convenience sample was used.The statistical population consisted of the first-line nurses in 7 tertiary general hospitals from March 3,2020 to March 10,2020 in Wuhan of China.A total of 2667 samples from 2768 contacted participants completed the investgation,with a response rate of 96.35%.Social-demographic questionnaire,work-related questionnaire,Fatigue Scale-14,Generalized Anxiety Disorder-7,Patient Health Questionnaire-9,and Chinese Perceived Stress Scale were used to conduct online survey.The descriptive statistic of nurses'social-demographic characteristics was conducted,and the related variables of work,anxiety,depression,perceived stress and fatigue were analyzed by Z-tests,nonparametric test and Pearson's correlation analysis.The significant factors which resulted in nurses'fatigue were further analyzed by multiple linear regression analysis.The median score for the first-line nurses'fatigue in Wuhan was 4(2,8).The median score of physical and mental fatigue of them was 3(1,6)and 1(0,3)respectively.According to the scoring criteria,35.06%nurses(w=935)of all participants were in the fatigue status,their median score of fatigue was 10(8,11),and the median score of physical and mental fatigue of them was 7(5,8)and 3(2,4)respectively.Multiple linear regression analysis revealed the participants in the risk groups of anxiety,depression and perceived stress had higher scores on physical and mental fatigue and the statistically significant positive correlation was observed between the variables and nurses'fatigue,the frequency of exercise and nurses'fatigue had a statistically significant negative correlation,and average daily working hours had a significantly positive correlation with nurses'fatigue,and the frequency of weekly night shift had a low positive correlation with nurses'fatigue(P<0.01).There was a moderate level of fatigue among the first-line nurses fighting against COVID-19 pandemic in Wuhan,China.Government and health authorities need to formulate and take effective intervention strategies according to the relevant risk factors,and undertake preventive measures aimed at reducing health hazards due to increased work-related fatigue among first-line nurses,and to enhance their health status and provide a safe occupational environment worldwide.Promoting both medical and nursing safety while combating with the pandemic currently is warranted.展开更多

Objectives:Nurses working in critical care units may encounter substantial work-related strain,and elevated levels of stress can lead to burnout,ultimately impacting both the quality of nursing care and their overall ...Objectives:Nurses working in critical care units may encounter substantial work-related strain,and elevated levels of stress can lead to burnout,ultimately impacting both the quality of nursing care and their overall working experience.This study aimed to assess job burnout and determine the factors that contribute to it among critical care nurses in the Eastern Province of Saudi Arabia.Methods:This study employed a descriptive,exploratory,cross-sectional research design.A total of 220 nurses employed in the critical care units of 5 private hospitals in the eastern region were selected for participation in this study using a convenience sample method.An electronic survey was distributed to critical care nurses in the Eastern Province who satisfied the specified inclusion criteria.The job burnout questionnaire utilized in this study was derived from previously conducted research,which has been established as a reliable and valid survey instrument.The process of data analysis was conducted utilizing the SPSS program.The scientific research conducted on human subjects adhered rigorously to all ethical considerations.Results:The highest percentage of nurses at the critical care units reported experiencing moderate levels of job burnout.Nurses exhibiting greater levels of professional experience demonstrated a statistically significant reduction in overall burnout scores when compared to their less experienced counterparts.Furthermore,within the realm of sociodemographic factors,it was found that the only significant independent predictor for job burnout was the level of experience among critical care nurses.Conclusions:The prevalence of burnout among nurses in critical care settings was found to be significant,with most participants reporting moderate levels of burnout which can yield significant ramifications for nurses and healthcare professionals.Consequently,healthcare organizations must accord primacy to the welfare of their staff and adopt proactive strategies to mitigate job burnout.展开更多

However, studies have rev...Objective: Physical assessment is an indispensable and pivotal skill that nurses must aptly monitor, evaluate, and deliver timely care, particularly in the context of critically ill patients. However, studies have revealed instances where nurses demonstrate inaccurate practices. This study is aimed to measure the physical assessment skills of critical care nurses and the differences between levels of nursing education. Methods: A descriptive cross-sectional study was conducted involving a simple random sample of 101 nurses employed in critical care units. Results: Of the 101 participants, 56.4% were identified as female and 43.6% were male. In the sample, 71.0% held a diploma in nursing, while 29.0% held a Bachelor of Science in Nursing (BSN). Vital signs, orientation, and skin assessments were performed correctly in ≥90% of cases, whereas the majority (≥60%) did not correctly assess neck veins, Homan’s signs, breath sounds, heart sounds, and bowel sounds. Only ≤10% of participants accurately executed these assessments. Significant associations were found between sex and pain (P=0.034), as well as between the type of education and closure of the health assessment (P=0.041). Further, female nurses scored 10.0 ± 2.3, and male nurses 8.5 ± 2.5, diploma holders scored 9.4 ± 1.8, and BSN holders scored 9.3 ± 2.7 at the global rating. Conclusions: The presence of an existing gap in the accurate assessment of patients poses a potential risk to patient care. It is essential to investigate the factors contributing to this gap and to develop strategies to address it, thereby enhancing the quality of physical assessment and ultimately improving patient care.展开更多
Objective:To investigate the current situation of nurses’lack of nursing care in southern Xinjiang and analyze its influencing factors.Methods:A convenient sampling method was used to select 1476 nurses from two tert...Objective:To investigate the current situation of nurses’lack of nursing care in southern Xinjiang and analyze its influencing factors.Methods:A convenient sampling method was used to select 1476 nurses from two tertiary hospitals in Kashi from May 2022 to December 2022 as the subjects of a cross-sectional survey.The nurses in southern Xinjiang were investigated by general information questionnaire and nursing deficiency scale-nurse version.Results:A total of 1476 valid questionnaires were collected in this study.The score of nurses’lack of care in southern Xinjiang was(56.05±9.36),which was above the middle level.Among them,the scores of timely responses to call bells,view patient medical records,fully understand their own patient’s condition,and provide emotional support for patients and/or family members were relatively low.Univariate analysis showed that there were significant differences in the scores of nurses’age,nurses’level,nursing career satisfaction and nursing job satisfaction(p<0.05).Multivariate analysis showed that the occupational satisfaction and nursing job satisfaction of nursing staff were the main factors affecting the lack of nursing work(p<0.05).Conclusion:The lack of nursing care of nurses in southern Xinjiang is at the upper middle level.The younger the age,the lower the satisfaction of nursing profession and the lower the satisfaction of nursing position,the more serious the lack of nursing care.In order to solve this problem,nursing managers should actively respond to the influencing factors of nursing absence and actively seek effective management measures to reduce the incidence of nursing absence and ensure the safety of patients.展开更多

BACKGROUND In recent years,emerging clinical research has prioritized assessment of combined therapeutic efficacy and safety parameters when programmed death 1 or its ligand(PD-1/L1)inhibitors are incorporated into fi...BACKGROUND In recent years,emerging clinical research has prioritized assessment of combined therapeutic efficacy and safety parameters when programmed death 1 or its ligand(PD-1/L1)inhibitors are incorporated into first-line standard-of-care(SOC)therapy for metastatic colorectal cancer(mCRC).However,data obtained from these trials demonstrated conflicting evidence concerning survival benefits and clinical outcomes.AIM To evaluate the therapeutic impact and safety parameters of combining PD-1/L1 inhibitors with SOC protocols as first-line treatment for mCRC.METHODS Four biomedical databases(PubMed,Embase,Cochrane Library,Web of Science)were systematically interrogated to identify eligible studies published up to October 12,2024.The analysis focused on evaluating the primary outcome of overall survival(OS)in the mCRC population with secondary outcomes of progression-free survival(PFS),overall response rate(ORR),and incidence rate of grade≥3 adverse events.Additionally,we performed exploratory analyses in the microsatellite stable/mismatch repair-proficient(MSS/pMMR)subpopulation,based on a subset of the included studies.Subgroup analyses according to PD-1/L1 inhibitor use were conducted in both the overall population and the MSS/pMMR subgroup.RESULTS This pooled analysis incorporated six randomized controlled trials involving 675 patients with mCRC receiving first-line therapy.The combination of PD-1/L1 inhibitors with SOC regimens demonstrated a significant PFS advantage over SOC monotherapy in intention-to-treat populations[hazard ratio(HR)=0.8,95%confidence interval(CI):0.65-0.98,P=0.033].Nevertheless,the MSS/pMMR subgroup showed no PFS benefit(HR=0.83,95%CI:0.67-1.03,P=0.091),and no cohort exhibited OS improvement(intention-to-treat:HR=0.84,95%CI:0.66-1.05,P=0.124;MSS/pMMR:HR=0.79,95%CI:0.60-1.03,P=0.083).Comparable outcomes were observed for ORR(risk ratio=1.03,95%CI:0.90-1.17,P=0.711)and incidence rate of grade≥3 adverse events(risk ratio=1.12,95%CI:0.93-1.36,P=0.245)between treatment arms.CONCLUSION The findings indicated that integrating PD-1/L1 blocking agents with SOC regimens for mCRC as first-line treatment failed to demonstrate significant improvements in ORR.Existing clinical data remain inadequate to establish OS advantages,particularly in patients with MSS/pMMR,despite exhibiting manageable toxicity profiles.Subsequent confirmation through rigorously designed phase III clinical trials remains essential to verify these therapeutic outcomes.展开更多
Purpose:This study aimed to determine the nurses’profile characteristics that are associated with moral distress among nurses in selected government hospitals in Shangdong,China.Methods:This is a descriptive-correlat...Purpose:This study aimed to determine the nurses’profile characteristics that are associated with moral distress among nurses in selected government hospitals in Shangdong,China.Methods:This is a descriptive-correlational study.A total of 185 clinical registered nurses who met the eligibility criteria from different departments in two tertiary governmental hospitals in Jinan city were purposively selected to participate in this study.The instruments included the general demographic characteristics and Chinese version of Moral Distress Scale-Revised(MDS-R).Correlation technique,specifically Spearman’s rho,was utilized to determine the significant correlation between the selected nurse’s characteristics and moral distress.Ethical considerations were given by the Far Eastern University ethics review committee(FEU-ERC Code:2020-2021-079).Results:The number of patients nurses handled per shift(rs=0.650,p=0.032)and the type of patients that nurses handled(rs=0.718,p=0.020)are the nurses’profile factors that are significantly correlated with nurses’moral distress(p-value<0.05).As there is an increase in the number of patients handled every shift and the patients are becoming unstable,requiring complex care,the frequency and intensity of moral distress increase as well.Conclusion:The nurses’moral distress was registered at a low level in this study.Characteristics related to patients,such as the number and type assigned to nurses,are correlated to moral distress.The main source of the high frequency and intensity of moral distress among nurses is“futile care”and“false hope”.Educational learning program is recommended to manage and alleviate the moral distress of nurses.展开更多

Method: A cross-sectional survey was conducted among 937 nurses in Yunnan Province using the Beliefs and Attitudes Scale on Nurses’ Prescriptive Authority. The scale assessed four dimensions: perceived need, self-efficacy, perceived benefits, and perceived barriers. Multiple linear regression analysis was used to identify factors influencing the overall score and each dimension. Results: The total score of the Beliefs and Attitudes Scale was 89.17 ± 17.69, indicating a moderate level of awareness and positive attitude among nurses. The highest-scoring dimension was perceived benefits (34.94 ± 8.04), while the lowest was perceived barriers (15.23 ± 3.5). Age was identified as a significant factor influencing the overall score and self-efficacy dimension (P < 0.05). Years of practice influenced the perceived benefits dimension (P = 0.051), while gender, age, and professional title were key factors affecting the perceived barriers dimension (P < 0.05). Male nurses and senior nurses demonstrated more caution toward potential risks associated with prescriptive authority. Conclusion: Nurses in Yunnan Province exhibit moderate levels of belief in and attitudes toward prescriptive authority, with age being the most significant influencing factor. Tailored training programs, policy promotion, and practical guidance are recommended to enhance nurses’ understanding and support for prescriptive authority, thereby improving nursing practices and addressing regional healthcare challenges.展开更多

Objective:The combination of science and art in nursing is essential for providing effective care.Since art is inherent and a part of human personality traits,it is believed that nurses’personality traits are importa...Objective:The combination of science and art in nursing is essential for providing effective care.Since art is inherent and a part of human personality traits,it is believed that nurses’personality traits are important to achieve this cohesive combination in nursing.Accordingly,this study was conducted to determine the relationship between nurses’personality traits and the esthetics of nursing care.Methods:A cross-sectional descriptive design was employed.Study participants that consisted of 95 nurses and 285 patients from health centers in Iran were selected by convenience sampling method.Measures included the five-factor personality questionnaires(NEO-FFI)scale and Esthetics of Nursing Care Scale(ENCS).Results:The findings indicated a significant relationship between neuroticism(r=−0.149,P=0.028)and extraversion traits(r=0.136,P=0.045)of nurses in esthetics nursing care.In this study,no significant relationship was found between the personality traits and esthetics of nursing care using nurses’demographic information.Conclusions:The esthetics of nursing care depends on nurse personality traits.Since the art of nursing complements the expected care,it is suggested that nursing managers pay attention to the personality traits of nurses in planning to provide effective care.展开更多
文摘Summary:Nurses'work-related fatigue has been recognized as a threat to nurse health and patient safety.The aim of this study was to assess the prevalence of fatigue among first-line nurses combating with COVID-19 in Wuhan,China,and to analyze its influencing factors on fatigue.A multi-center,descriptive,cross-sectional design with a convenience sample was used.The statistical population consisted of the first-line nurses in 7 tertiary general hospitals from March 3,2020 to March 10,2020 in Wuhan of China.A total of 2667 samples from 2768 contacted participants completed the investgation,with a response rate of 96.35%.Social-demographic questionnaire,work-related questionnaire,Fatigue Scale-14,Generalized Anxiety Disorder-7,Patient Health Questionnaire-9,and Chinese Perceived Stress Scale were used to conduct online survey.The descriptive statistic of nurses'social-demographic characteristics was conducted,and the related variables of work,anxiety,depression,perceived stress and fatigue were analyzed by Z-tests,nonparametric test and Pearson's correlation analysis.The significant factors which resulted in nurses'fatigue were further analyzed by multiple linear regression analysis.The median score for the first-line nurses'fatigue in Wuhan was 4(2,8).The median score of physical and mental fatigue of them was 3(1,6)and 1(0,3)respectively.According to the scoring criteria,35.06%nurses(w=935)of all participants were in the fatigue status,their median score of fatigue was 10(8,11),and the median score of physical and mental fatigue of them was 7(5,8)and 3(2,4)respectively.Multiple linear regression analysis revealed the participants in the risk groups of anxiety,depression and perceived stress had higher scores on physical and mental fatigue and the statistically significant positive correlation was observed between the variables and nurses'fatigue,the frequency of exercise and nurses'fatigue had a statistically significant negative correlation,and average daily working hours had a significantly positive correlation with nurses'fatigue,and the frequency of weekly night shift had a low positive correlation with nurses'fatigue(P<0.01).There was a moderate level of fatigue among the first-line nurses fighting against COVID-19 pandemic in Wuhan,China.Government and health authorities need to formulate and take effective intervention strategies according to the relevant risk factors,and undertake preventive measures aimed at reducing health hazards due to increased work-related fatigue among first-line nurses,and to enhance their health status and provide a safe occupational environment worldwide.Promoting both medical and nursing safety while combating with the pandemic currently is warranted.

文摘Objective Systematically integrate nurses’experience with“Internet Nursing Service”to analysis the nurses’experiences with“Internet Nursing Service”,and to provide a theoretical reference for formulating a more rational“Internet Nursing Service”model.Methods A systematic search in PubMed,Embase,Web of Science,the Cochrane Library,CINAHL,China National Knowledge Infrastructure(CNKI),Wanfang Database,and Chinese Biomedical Literature Database was conducted to collect qualitative research on nurses’experiences with“Internet Nursing Service,”with a retrieval time limit from December 2019 to June 2024.Qualitative meta-synthesis was performed through line-by-line coding of relevant quotes,organization of codes into descriptive themes,and development of analytical themes.Results A total of 19 studies were included,one study was rated as Grade A in quality evaluation,and the remaining studies were rated as Grade B.Collectively synthesized into three integrated results:Harvest and growth,Difficulties and challenges,and Expectations and support.Harvest and growth,include 1)manifestation of self-value,2)enhancing nursing capabilities,3)optimizing nursing resources;Difficulties and challenges,include 1)lack of safety guarantee,2)role conflict;Expectations and support include,1)expectation for professional knowledge and skill training,2)expectations for service platform optimization,3)expectation for reasonable charges,4)expectation for related policy support.Conclusion“Internet Nursing Service”model benefits both nurses and patients,but still full of challenges.It aids in the decentralization of medical resources.Management departments still need to encourage nurses to actively invest in“Internet Nursing Service”while ensuring their safety and interests.
文摘Objectives:Nurses working in critical care units may encounter substantial work-related strain,and elevated levels of stress can lead to burnout,ultimately impacting both the quality of nursing care and their overall working experience.This study aimed to assess job burnout and determine the factors that contribute to it among critical care nurses in the Eastern Province of Saudi Arabia.Methods:This study employed a descriptive,exploratory,cross-sectional research design.A total of 220 nurses employed in the critical care units of 5 private hospitals in the eastern region were selected for participation in this study using a convenience sample method.An electronic survey was distributed to critical care nurses in the Eastern Province who satisfied the specified inclusion criteria.The job burnout questionnaire utilized in this study was derived from previously conducted research,which has been established as a reliable and valid survey instrument.The process of data analysis was conducted utilizing the SPSS program.The scientific research conducted on human subjects adhered rigorously to all ethical considerations.Results:The highest percentage of nurses at the critical care units reported experiencing moderate levels of job burnout.Nurses exhibiting greater levels of professional experience demonstrated a statistically significant reduction in overall burnout scores when compared to their less experienced counterparts.Furthermore,within the realm of sociodemographic factors,it was found that the only significant independent predictor for job burnout was the level of experience among critical care nurses.Conclusions:The prevalence of burnout among nurses in critical care settings was found to be significant,with most participants reporting moderate levels of burnout which can yield significant ramifications for nurses and healthcare professionals.Consequently,healthcare organizations must accord primacy to the welfare of their staff and adopt proactive strategies to mitigate job burnout.
文摘Emergency department nurses face severe occupational stress leading to anxiety,depression,and burnout,which significantly impair their well-being and patientcare quality.This narrative review examined the role of mindfulness-based stress reduction(MBSR)in addressing these challenges.Rooted in nonjudgmental present-moment awareness,MBSR enhances emotional regulation and reduces psychological distress by fostering adaptive coping strategies.Studies have demonstrated its efficacy in lowering anxiety,depressive symptoms,and emotional exhaustion,while improving workplace well-being,empathy,and job satisfaction.Mechanistically,MBSR improves interoceptive awareness and autonomic balance,as evidenced by physiological markers such as heart rate variability.However,gaps remain in long-term efficacy assessments,personalized interventions,and integration with multidisciplinary approaches.Future research should prioritize tailored biomarker-driven programs,longitudinal studies,and scalable implementation strategies in high-stress clinical settings.This review underscores MBSR’s potential as a sustainable,evidence-based tool to enhance emergency department nurses’mental health and professional performance,advocating for broader adoption and further refinement of its practical applications.

文摘Objective:First-line nurse managers are more likely to work according to gender beliefs and stereotypes,which may affect their managerial competence.This study is aimed at comparing managerial competence of male and female first-line nurse managers in public hospitals in Indonesia.Methods:This study employed a descriptive comparative approach with a cross-sectional survey with a total of 256 participants selected from 18 public hospitals.To measure managerial competence,the managerial competence scale for Indonesian first-line nurse managers was used.Data were analyzed using descriptive analyses using mean,standard deviation,and Independent t-test.Results:Managerial competence of male and female first-line nurse managers was not significantly different(P=0.555).Female nurse managers descriptively reported/received higher ranking in facilitating spiritual nursing care,managing self,staffing and professional development,utilizing informatics,and applying quality care improvement.Male nurse managers repor ted higher in leadership and financial management.Conclusions:Female and male first-line nurse managers should be treated equally for leadership and managerial development.
文摘Objective:This article employs a scoping review methodology,integrates knowledge and information about current pediatric nurses'practices and perceptions regarding family-centered care in pediatric settings.Methods:Published articles were retrieved from databases including EBSCO host,PubMed,Springer,Science Direct,Ovid,and CINAHL between 2013 and 2023.Results:The finding shows a better understanding of pediatric nurses'perceptions of family-centered care in association with their clinical settings.However,the evidence indicates that integrating family-centered care components into health care services is difficult and confusing to nurses and is often not implemented in a clinical setting.As evidenced by this review,studies have consistently reported similar results;family-centered care was a good perception and understood by pediatric nurses as a concept but inconsistently used in a daily practice setting.Conclusions:This scoping review is the first phase in promoting a strategic plan to provide educational interventions for pediatric nurses to implement family-centered care in their daily practice settings.It's necessary to recognize pediatric nurses'perceptions and practices concerning family-centered care to provide optimal healthcare services in pediatric settings.
文摘Objective: Physical assessment is an indispensable and pivotal skill that nurses must aptly monitor, evaluate, and deliver timely care, particularly in the context of critically ill patients. However, studies have revealed instances where nurses demonstrate inaccurate practices. This study is aimed to measure the physical assessment skills of critical care nurses and the differences between levels of nursing education. Methods: A descriptive cross-sectional study was conducted involving a simple random sample of 101 nurses employed in critical care units. Results: Of the 101 participants, 56.4% were identified as female and 43.6% were male. In the sample, 71.0% held a diploma in nursing, while 29.0% held a Bachelor of Science in Nursing (BSN). Vital signs, orientation, and skin assessments were performed correctly in ≥90% of cases, whereas the majority (≥60%) did not correctly assess neck veins, Homan’s signs, breath sounds, heart sounds, and bowel sounds. Only ≤10% of participants accurately executed these assessments. Significant associations were found between sex and pain (P=0.034), as well as between the type of education and closure of the health assessment (P=0.041). Further, female nurses scored 10.0 ± 2.3, and male nurses 8.5 ± 2.5, diploma holders scored 9.4 ± 1.8, and BSN holders scored 9.3 ± 2.7 at the global rating. Conclusions: The presence of an existing gap in the accurate assessment of patients poses a potential risk to patient care. It is essential to investigate the factors contributing to this gap and to develop strategies to address it, thereby enhancing the quality of physical assessment and ultimately improving patient care.
文摘Objective:To investigate the current situation of nurses’lack of nursing care in southern Xinjiang and analyze its influencing factors.Methods:A convenient sampling method was used to select 1476 nurses from two tertiary hospitals in Kashi from May 2022 to December 2022 as the subjects of a cross-sectional survey.The nurses in southern Xinjiang were investigated by general information questionnaire and nursing deficiency scale-nurse version.Results:A total of 1476 valid questionnaires were collected in this study.The score of nurses’lack of care in southern Xinjiang was(56.05±9.36),which was above the middle level.Among them,the scores of timely responses to call bells,view patient medical records,fully understand their own patient’s condition,and provide emotional support for patients and/or family members were relatively low.Univariate analysis showed that there were significant differences in the scores of nurses’age,nurses’level,nursing career satisfaction and nursing job satisfaction(p<0.05).Multivariate analysis showed that the occupational satisfaction and nursing job satisfaction of nursing staff were the main factors affecting the lack of nursing work(p<0.05).Conclusion:The lack of nursing care of nurses in southern Xinjiang is at the upper middle level.The younger the age,the lower the satisfaction of nursing profession and the lower the satisfaction of nursing position,the more serious the lack of nursing care.In order to solve this problem,nursing managers should actively respond to the influencing factors of nursing absence and actively seek effective management measures to reduce the incidence of nursing absence and ensure the safety of patients.

文摘This article first reviews the basic concepts and theoretical frameworks of empathic ability.Subsequently,it analyzes the characteristics of student nurses'empathic ability,its measurement methods,and influencing factors,and explores effective strategies for enhancing it.Finally,this paper summarizes the impact of student nurses'empathic ability on nursing practice,professional development,and emotional health,and proposes directions for future research.Through the integration and analysis of existing literature,this study provides nursing educators with a solid theoretical foundation and practical guidance for enhancing student nurses'empathic ability.

文摘The global burden of Helicobacter pylori infection continues to drive the need for effective,well-tolerated,and regionally adaptable eradication regimens.Recently Han et al presented compelling results from a multicenter randomized controlled trial in China,demonstrating the non-inferiority and potential superiority of vonoprazan(VPZ)-based triple therapy over the standard 14-day bismuth quad-ruple regimen.Both the 10-day and 14-day VPZ-amoxicillin-clarithromycin regi-mens achieved eradication rates exceeding 90%per protocol,with similar or fewer adverse events,suggesting improved patient tolerability.This study reinforces the potential of VPZ to overcome the limitations of proton pump inhibitor-based regimens,particularly in populations with CYP2C19 polymorphisms and increa-sing clarithromycin resistance.These findings also lend support to shortened trea-tment durations,which could enhance adherence and reduce antimicrobial expo-sure.These data highlight an important shift in the therapeutic landscape,positio-ning VPZ as a strong candidate for first-line empirical therapies.However,broa-der implementation requires careful consideration of local resistance patterns,drug accessibility,and pharmacoeconomic implications.
文摘BACKGROUND In recent years,emerging clinical research has prioritized assessment of combined therapeutic efficacy and safety parameters when programmed death 1 or its ligand(PD-1/L1)inhibitors are incorporated into first-line standard-of-care(SOC)therapy for metastatic colorectal cancer(mCRC).However,data obtained from these trials demonstrated conflicting evidence concerning survival benefits and clinical outcomes.AIM To evaluate the therapeutic impact and safety parameters of combining PD-1/L1 inhibitors with SOC protocols as first-line treatment for mCRC.METHODS Four biomedical databases(PubMed,Embase,Cochrane Library,Web of Science)were systematically interrogated to identify eligible studies published up to October 12,2024.The analysis focused on evaluating the primary outcome of overall survival(OS)in the mCRC population with secondary outcomes of progression-free survival(PFS),overall response rate(ORR),and incidence rate of grade≥3 adverse events.Additionally,we performed exploratory analyses in the microsatellite stable/mismatch repair-proficient(MSS/pMMR)subpopulation,based on a subset of the included studies.Subgroup analyses according to PD-1/L1 inhibitor use were conducted in both the overall population and the MSS/pMMR subgroup.RESULTS This pooled analysis incorporated six randomized controlled trials involving 675 patients with mCRC receiving first-line therapy.The combination of PD-1/L1 inhibitors with SOC regimens demonstrated a significant PFS advantage over SOC monotherapy in intention-to-treat populations[hazard ratio(HR)=0.8,95%confidence interval(CI):0.65-0.98,P=0.033].Nevertheless,the MSS/pMMR subgroup showed no PFS benefit(HR=0.83,95%CI:0.67-1.03,P=0.091),and no cohort exhibited OS improvement(intention-to-treat:HR=0.84,95%CI:0.66-1.05,P=0.124;MSS/pMMR:HR=0.79,95%CI:0.60-1.03,P=0.083).Comparable outcomes were observed for ORR(risk ratio=1.03,95%CI:0.90-1.17,P=0.711)and incidence rate of grade≥3 adverse events(risk ratio=1.12,95%CI:0.93-1.36,P=0.245)between treatment arms.CONCLUSION The findings indicated that integrating PD-1/L1 blocking agents with SOC regimens for mCRC as first-line treatment failed to demonstrate significant improvements in ORR.Existing clinical data remain inadequate to establish OS advantages,particularly in patients with MSS/pMMR,despite exhibiting manageable toxicity profiles.Subsequent confirmation through rigorously designed phase III clinical trials remains essential to verify these therapeutic outcomes.
文摘Purpose:This study aimed to determine the nurses’profile characteristics that are associated with moral distress among nurses in selected government hospitals in Shangdong,China.Methods:This is a descriptive-correlational study.A total of 185 clinical registered nurses who met the eligibility criteria from different departments in two tertiary governmental hospitals in Jinan city were purposively selected to participate in this study.The instruments included the general demographic characteristics and Chinese version of Moral Distress Scale-Revised(MDS-R).Correlation technique,specifically Spearman’s rho,was utilized to determine the significant correlation between the selected nurse’s characteristics and moral distress.Ethical considerations were given by the Far Eastern University ethics review committee(FEU-ERC Code:2020-2021-079).Results:The number of patients nurses handled per shift(rs=0.650,p=0.032)and the type of patients that nurses handled(rs=0.718,p=0.020)are the nurses’profile factors that are significantly correlated with nurses’moral distress(p-value<0.05).As there is an increase in the number of patients handled every shift and the patients are becoming unstable,requiring complex care,the frequency and intensity of moral distress increase as well.Conclusion:The nurses’moral distress was registered at a low level in this study.Characteristics related to patients,such as the number and type assigned to nurses,are correlated to moral distress.The main source of the high frequency and intensity of moral distress among nurses is“futile care”and“false hope”.Educational learning program is recommended to manage and alleviate the moral distress of nurses.
文摘Objective: To investigate the current beliefs and attitudes of nurses in Yunnan Province toward prescriptive authority, analyze the influencing factors, and provide evidence for future research and policy formulation to support the establishment of nurses’ prescriptive authority in China. Method: A cross-sectional survey was conducted among 937 nurses in Yunnan Province using the Beliefs and Attitudes Scale on Nurses’ Prescriptive Authority. The scale assessed four dimensions: perceived need, self-efficacy, perceived benefits, and perceived barriers. Multiple linear regression analysis was used to identify factors influencing the overall score and each dimension. Results: The total score of the Beliefs and Attitudes Scale was 89.17 ± 17.69, indicating a moderate level of awareness and positive attitude among nurses. The highest-scoring dimension was perceived benefits (34.94 ± 8.04), while the lowest was perceived barriers (15.23 ± 3.5). Age was identified as a significant factor influencing the overall score and self-efficacy dimension (P < 0.05). Years of practice influenced the perceived benefits dimension (P = 0.051), while gender, age, and professional title were key factors affecting the perceived barriers dimension (P < 0.05). Male nurses and senior nurses demonstrated more caution toward potential risks associated with prescriptive authority. Conclusion: Nurses in Yunnan Province exhibit moderate levels of belief in and attitudes toward prescriptive authority, with age being the most significant influencing factor. Tailored training programs, policy promotion, and practical guidance are recommended to enhance nurses’ understanding and support for prescriptive authority, thereby improving nursing practices and addressing regional healthcare challenges.
文摘This paper focuses on the key role and practical paths of operating room nurses in infection control,and studies the difficult problems in the prevention and control of surgical site infections.Analyze the infection risk links in the operating room and the core responsibilities of nurses,reveal the multiple role mechanisms of nurses in infection control,design practical strategies from dimensions such as process optimization,skill improvement,and monitoring feedback,and construct an infection control system involving all staff.Research shows that operating room nurses can effectively reduce the risk of infection,improve medical quality,provide core guarantees for patient safety,and promote the transformation of operating room infection control towards refined management through practical actions such as standardized operation execution,real-time risk monitoring,and team collaboration supervision.
文摘Objective:To assess knowledge and practice among nurses toward neonatal resuscitation in hospitals in Hebron.Methods:A cross-sectional study design was conducted among 151 nurses from Hebron.Data were collected using a self-administered questionnaire.Multiple logistic regression was conducted to test the associated factors with the dependent variables.Results:Only 9.3%of participants had a standardized level of knowledge,while 68.9%reported adequate skills for neonatal resuscitation.Factors associated with a standardized level of knowledge were age(p=0.001)and supportive supervision(p=0.007).Factors associated with the standardized practice level were the presence of hospital guidelines(p=0.028),previous training of nurses(p=0.011),and the presence of supportive supervision(p<0.001).Conclusions:The majority of emergency nurses in Hebron hospitals demonstrated a substandard level of knowledge of neonatal resuscitation.However,most of those nurses surprisingly reported a standardized practice level.The presence of hospital guidelines,training programs,or direct supervision for nurses are factors that can be considered to improve both knowledge and practice toward neonatal resuscitation among emergency nurses in Hebron.
文摘1.The state of the world’s health Today’s global health landscape presents pressing challenges that demand urgent action.Nearly half of the global population lacks access to Universal Health Coverage(UHC),leaving millions vulnerable.Each year,100 million people are driven into extreme poverty by medical expenses.Noncommunicable diseases(NCDs),such as cancer,heart disease,and diabetes,are responsible for 41 million deaths annuallyd71%of global mortality.Communicable diseases remain a significant threat,particularly in emerging economies,with only 81%of children receiving vaccinations in 2021da 5%decline from previous years.Compounded by this,humanitarian crises in 2024 are expected to impact 300 million people,and the proportion of the population aged over 60 is projected to nearly double between 2015 and 2050.Additionally,mental illness affects over 970 million people,posing significant challenges to healthcare providers,particularly nurses.
文摘Objective:Patient falls in the healthcare settings are unwanted events that can have severe consequences for both inpatients and outpatients;however,falls are preventable and nurses play an important role in this prevention.The purpose of this study was to investigate nurse activities in fall prevention and its relevant factors.Methods:This descriptive cross-sectional study was conducted at Can Tho Central General Hospital,with the participation of 90 nurses.Data were collected using a structured questionnaire consisting of five parts,including personal characteristics,knowledge about fall prevention,self-efficacy in fall prevention,nursing practice environment,and fall prevention activities.The data were entered and analyzed using SPSS 18.0 software with descriptive statistics,independent t-test,one-way ANOVA,and Pearson’s correlation coefficients.Results:The mean score of participants’knowledge in fall prevention was 4.67(SD=2.50)out of 11 scores;the self-efficacy score was 49.43(SD=8.55)out of 66 scores;nursing practice environment score was 29.69(SD=4.00)out of 40 scores;and the fall prevention activity scores were 75.58(SD=9.96)out of 100 scores.Factors related to fall prevention activities included gender(p=0.03),place of work(p=0.02),number of fall prevention training sessions(p=0.03),self-efficacy in fall prevention(r=0.61,p<0.001),and the nursing practice environment(r=0.25,p<0.05).Conclusions:An intervention program for fall prevention can be implemented to enhance the knowledge about fall prevention among nurses and improve the nursing practice environment to enhance patient safety.
文摘Esophageal cancer is an aggressive malignancy often diagnosed at advanced stages,with esophageal squamous cell carcinoma being the predominant subtype worldwide.Standard first-line chemotherapy provides limited survival benefits,with a median overall survival of less than 1 year.Recent advancements in immunotherapy,particularly immune checkpoint inhibitors(ICIs),have trans-formed the treatment landscape,improving overall survival and progression-free survival.However,response rates remain variable,with programmed death ligand 1(PD-L1)expression being the primary predictive biomarker.The variability in PD-L1 testing methods and immune microenvironment alterations after prior treatments complicate patient selection for ICIs.Several phase 3 trials,including KEYNOTE-590 and CheckMate 648,have demonstrated the efficacy of ICIs combined with chemotherapy,particularly in patients positive for PD-L1.Despite these advances,long-term survival remains low,emphasizing the need for better biomarkers and novel therapeutic strategies.This review explored current first-line treatment options for esophageal squamous cell carcinoma,challenges in biomarker-based patient selection,and emerging therapeutic approaches.
文摘Objective:The combination of science and art in nursing is essential for providing effective care.Since art is inherent and a part of human personality traits,it is believed that nurses’personality traits are important to achieve this cohesive combination in nursing.Accordingly,this study was conducted to determine the relationship between nurses’personality traits and the esthetics of nursing care.Methods:A cross-sectional descriptive design was employed.Study participants that consisted of 95 nurses and 285 patients from health centers in Iran were selected by convenience sampling method.Measures included the five-factor personality questionnaires(NEO-FFI)scale and Esthetics of Nursing Care Scale(ENCS).Results:The findings indicated a significant relationship between neuroticism(r=−0.149,P=0.028)and extraversion traits(r=0.136,P=0.045)of nurses in esthetics nursing care.In this study,no significant relationship was found between the personality traits and esthetics of nursing care using nurses’demographic information.Conclusions:The esthetics of nursing care depends on nurse personality traits.Since the art of nursing complements the expected care,it is suggested that nursing managers pay attention to the personality traits of nurses in planning to provide effective care.
