According to the measurement principle of the traditional interferometer,a narrowband signal model is established and used,however,for wideband signals or multiple signals,this model is invalid.For the problems of dir...According to the measurement principle of the traditional interferometer,a narrowband signal model is established and used,however,for wideband signals or multiple signals,this model is invalid.For the problems of direction finding with interferometer for wideband signals and multiple signals scene,a frequency domain phase interferometer is proposed and the concrete implementation scheme is given.The proposed method computes the phase difference in frequency domain,and finds multi-target results with judging the spectrum amplitude changing,and uses the frequency phase difference to compute the arrival angle.Theoretical analysis and simulation results show that the proposed method effectively solves the problem of the angle estimation with phase interferometer for wideband signals,and has good performance in multiple signals scene with nonoverlapping spectrum or partially overlapping.In addition,the wider the signal bandwidth,the better direction finding performance of this algorithm.展开更多

Background The diagnostic delay of cardiac amyloidosis(CA)is known to be substantially long.A prolonged time from symptoms onset to diagnosis negatively impacts quality of life and life expectancy of the affected pati...Background The diagnostic delay of cardiac amyloidosis(CA)is known to be substantially long.A prolonged time from symptoms onset to diagnosis negatively impacts quality of life and life expectancy of the affected patients.We aim to describe the role of the incidental finding of amyloid deposits in prostatic tissue as an early marker of CA.Methods A systematic cardiological evaluation,comprising ECG,echocardiogram and 99mTc-DPD scintigraphy,was offered to a cohort of 19 patients with incidental prostatic amyloidosis(PA)findings,propectively detected between 2014-2023,to assess cardiac involvement.Results The median age of the patients was 80.2 years(IQR:74.9-82.6 years).Histopathological study revealed amyloid deposits within the walls of small vessels(predominantly small arteries)in 18 patients and mainly in the stroma in the remaining case.All of them were immunohistochemically positive for transthyretin(ATTR)except one patient,with known myeloma,which was unconclusive fo ATTR.Clonal dyscrasia was excluded in the rest of the patients.Thirteen patients(68.4%)underwent all cardiological tests,4 patients(21.1%)underwent only ECG and echocardiographic evaluation and two patients(10.5%)refused to undergo any cardiological study.Among 13 individuals undergoing the complete evaluation,six patients were eventually diagnosed with CA(46.15%).All of them were asymptomatic from a cardiovascular point of view at the time of the prostate biopsy.Conclusion The finding of PA should prompt a complete cardiovascular examination,given the significant percentage of patients eventually diagnosed with early-stage CA.Multidisciplinary collaboration among different medical specialists must be encouraged,given the potential clinical impact of CA early diagnosis.展开更多

To describe and analyze the current status of benefit-finding research in patients with chronic diseases and to provide a reference for clinical practice and future research in this field.Using Arksey and O’Malley’s...To describe and analyze the current status of benefit-finding research in patients with chronic diseases and to provide a reference for clinical practice and future research in this field.Using Arksey and O’Malley’s scoping review methodology as a framework,we searched PubMed,Web of Science,Science Direct,and CINAHL(4 English databases);and China National Knowledge Infrastructure,the VIP Database for Chinese Technical Periodicals,the Wan Fang Database and China Biomedical Database(4 Chinese databases)for literature on benefit finding in patients with chronic diseases,with a search time frame of construction to April 20,2024,and the literature was screened,extracted,and summarized.A total of 44 papers were included,comprising 18 in Chinese and 26 in English.Among them,a total of 19 assessment tools were used,and the influential factors investigated were categorized into socio-demographic characteristics,disease-related characteristics,psychological and behavioral characteristics,coping styles,and social support.Benefit finding is found to be a positive psychological experience and important for patients with chronic diseases,and the influencing factors are complex and diverse,with a wide range of assessment tools.However,domestic tools lack autonomy and specificity.Clinical caregivers and staff should develop relevant assessment tools and formulate a personalized research plan based on the characteristics of patients with chronic diseases in China to promote the adoption of healthy lifestyles by patients with chronic diseases,improve their self-management ability,and provide a practical basis for improving the quality of life of patients with chronic diseases.展开更多

BACKGROUND Several conditions may present with acute neurological symptoms,thus mimicking the presentation of stroke.Although the underlying disorder can be diagnosed after careful medical,neurological,and radiologica...BACKGROUND Several conditions may present with acute neurological symptoms,thus mimicking the presentation of stroke.Although the underlying disorder can be diagnosed after careful medical,neurological,and radiological examinations,a few conditions,such as Wernicke encephalopathy(WE),may present a particular diagnostic difficulty.WE is a neurological disorder caused by deficiency of thiamine(B1 vitamin),most often resulting from alcoholism,malnutrition,hyperemesis gravidarum or bariatric surgery.The diagnosis of WE in a certain historical,clinical setting is easily suggested,but in a few cases presenting with acute neurological deficits,it can be particularly challenging.CASE SUMMARY We present the case of a 63-year-old man who was brought to the emergency department after developing weakness of the left extremities,dizziness and a confusional state,which had lasted for approximately 30 minutes.The patient had a similar episode of a confusional state approximately two months earlier;at that time,a transient ischemic attack was suspected and he was started on aspirin.The initial clinical evaluation and imaging findings were unremarkable for stroke,but the patient’s symptoms,history of chronic alcohol abuse and abnormal liver function tests prompted the consideration of WE.Magnetic resonance imaging findings in subthalamic areas and electroencephalogram data of diffuse delta activity supported this diagnosis.CONCLUSION Through this case report,we aim to underscore the importance of considering WE as a differential diagnosis in patients presenting with symptoms suggestive of stroke,especially when the presentation is atypical or when risk factors for thiamine deficiency are present.Since intravenous thiamine significantly improves outcomes,delayed recognition and treatment in some cases might be deleterious.展开更多
AIM:To analyze the ocular findings of patients who received lifelong leptin therapy due to congenital leptin deficiency(CLD),an extremely rare condition.METHODS:A prospective,cross-sectional comparative study was perf...AIM:To analyze the ocular findings of patients who received lifelong leptin therapy due to congenital leptin deficiency(CLD),an extremely rare condition.METHODS:A prospective,cross-sectional comparative study was performed on six patients with CLD and 13 healthy age-and sex-matched controls.The central corneal thickness(CCT),anterior chamber depth(ACD),axial length(AL),keratometry(K1,K2),optical coherence tomography(OCT),and OCT angiography parameters were compared between the leptin and control groups at the baseline visit.The change in these measurements in leptin patients over a two-year period was analyzed.RESULTS:CLD patients had lower mean AL,ACD,and CCT(P≤0.012 for all).Mean K1,K2(P≤0.047 for both),choroidal thickness(P≤0.001),and central ganglion cell layer(GCL)thickness(P=0.029)were higher in the leptin group.Perifoveal superficial capillary plexus(SCP)density was decreased in all quadrants except the temporal region(P<0.05),and parafoveal deep capillary plexus(DCP)density was decreased in the superior hemisphere,temporal quadrant(P≤0.036 for both)and nasal quadrant(P=0.048)in the leptin group.During the two-year follow-up,no changes in anterior and posterior segment measurements were observed in the leptin patients,except for subfoveal choroidal thickness(P<0.001).CONCLUSION:CLD patients exhibit structural alterations in both the anterior and posterior segments of the eye,including notable changes in retinal and choroidal vasculature.However,there is limited evidence concerning the influence of leptin therapy on the eye.展开更多
BACKGROUND Papillary fibroelastomas are rare,benign cardiac tumors typically found on the heart valves.This case series presents three patients with aortic valve fibroe-lastoma incidentally detected during endoscopic ...BACKGROUND Papillary fibroelastomas are rare,benign cardiac tumors typically found on the heart valves.This case series presents three patients with aortic valve fibroe-lastoma incidentally detected during endoscopic ultrasound(EUS)for pancreatic and biliary pathologies.These cases highly get the potential of EUS in identifying subtle cardiac lesions during gastrointestinal evaluation,raise important que-stions about the true prevalence of these lesions,and highlight the need for es-tablished guidelines for their management and surveillance.The detection of fib-roelastomas during routine EUS procedures emphasizes the importance of tho-rough assessment and multidisciplinary approaches in managing unexpected dis-coveries.This also highlights the potential thromboembolic risks associated with fibroelastomas and challenges in determining appropriate management strategies for patients who are asymptomatic.CASE SUMMARY The patients aged 72 years,51 years,and 42 years underwent EUS for various indications when aortic valve lesions consistent with fibroelastomas were discovered.These findings were subsequently confirmed by transesophageal echocardiography in two cases,with the third patient awaiting assessment.The lesions ranged in size from 0.61 cm to 1.6 cm in diameter and exhibited characteristic sonographic features of fibroelastomas,including hyperechoic appearance and attachment to the aortic valve leaflets.These cases highlight the potential of EUS in identifying subtle cardiac lesions during gastrointestinal evaluation,raising questions about the true prevalence of fib-roelastomas and the need for establishing management guidelines.CONCLUSION This case series raises important questions regarding the prevalence of aortic valve fibroelastoma lesions in the general population.This highlights the urgent need for comprehensive evidence-based guidelines to standardize the management and long-term surveillance of affected patients.展开更多

文摘Mucopolysaccharidoses(MPS)encompass a spectrum of inherited lysosomal storage disorders caused by deficiencies in enzymes required for glycosami-noglycan(GAG)degradation.These enzymatic deficits lead to GAG accumulation within lysosomes,resulting in progressive multiorgan damage,with skeletal ab-normalities prominently affecting diagnostic imaging.Radiologists play a crucial role in identifying characteristic skeletal changes,including skull defor-mities like J-shaped sella turcica,cranial thickening,spinal abnormalities such as odontoid hypoplasia and kyphosis,and unique thoracic and pelvic malformations.This review synthesizes radiographic findings across MPS subtypes,underscoring the importance of early diagnosis and continual imaging to monitor disease progre-ssion,particularly in the context of enzyme replacement therapy(ERT).While ERT offers symptom stabilization,it provides limited reversal of established structural abnormalities.Comprehensive radiographic assessment remains in-dispensable for guiding both symptomatic management and potential surgical intervention,thereby enhancing clinical outcomes for MPS patients.
文摘To describe and analyze the current status of benefit-finding research in patients with chronic diseases and to provide a reference for clinical practice and future research in this field.Using Arksey and O’Malley’s scoping review methodology as a framework,we searched PubMed,Web of Science,Science Direct,and CINAHL(4 English databases);and China National Knowledge Infrastructure,the VIP Database for Chinese Technical Periodicals,the Wan Fang Database and China Biomedical Database(4 Chinese databases)for literature on benefit finding in patients with chronic diseases,with a search time frame of construction to April 20,2024,and the literature was screened,extracted,and summarized.A total of 44 papers were included,comprising 18 in Chinese and 26 in English.Among them,a total of 19 assessment tools were used,and the influential factors investigated were categorized into socio-demographic characteristics,disease-related characteristics,psychological and behavioral characteristics,coping styles,and social support.Benefit finding is found to be a positive psychological experience and important for patients with chronic diseases,and the influencing factors are complex and diverse,with a wide range of assessment tools.However,domestic tools lack autonomy and specificity.Clinical caregivers and staff should develop relevant assessment tools and formulate a personalized research plan based on the characteristics of patients with chronic diseases in China to promote the adoption of healthy lifestyles by patients with chronic diseases,improve their self-management ability,and provide a practical basis for improving the quality of life of patients with chronic diseases.

文摘Reconstructing the trajectories of charged particles in high-energy physics experiments is a complex task,particularly for long-lived particles.At the future Super Tau-Charm Facility(STCF),such particles are expected to appear in several key benchmark physics processes.A Common Tracking Software was used to reconstruct the trajectories of long-lived particles,revealing that the track-finding performance of the widely used combinatorial Kalman filter is limited by its seeding algorithm.This limitation can be mitigated by guiding the combinatorial Kalman filter using initial tracks provided by the Hough transform.The track-finding performance of the combined Hough transform and combinatorial Kalman filter was evaluated using the process J∕ψ→Λ(→pπ−)Λ(→pπ+)at STCF.
文摘BACKGROUND Several conditions may present with acute neurological symptoms,thus mimicking the presentation of stroke.Although the underlying disorder can be diagnosed after careful medical,neurological,and radiological examinations,a few conditions,such as Wernicke encephalopathy(WE),may present a particular diagnostic difficulty.WE is a neurological disorder caused by deficiency of thiamine(B1 vitamin),most often resulting from alcoholism,malnutrition,hyperemesis gravidarum or bariatric surgery.The diagnosis of WE in a certain historical,clinical setting is easily suggested,but in a few cases presenting with acute neurological deficits,it can be particularly challenging.CASE SUMMARY We present the case of a 63-year-old man who was brought to the emergency department after developing weakness of the left extremities,dizziness and a confusional state,which had lasted for approximately 30 minutes.The patient had a similar episode of a confusional state approximately two months earlier;at that time,a transient ischemic attack was suspected and he was started on aspirin.The initial clinical evaluation and imaging findings were unremarkable for stroke,but the patient’s symptoms,history of chronic alcohol abuse and abnormal liver function tests prompted the consideration of WE.Magnetic resonance imaging findings in subthalamic areas and electroencephalogram data of diffuse delta activity supported this diagnosis.CONCLUSION Through this case report,we aim to underscore the importance of considering WE as a differential diagnosis in patients presenting with symptoms suggestive of stroke,especially when the presentation is atypical or when risk factors for thiamine deficiency are present.Since intravenous thiamine significantly improves outcomes,delayed recognition and treatment in some cases might be deleterious.
文摘AIM:To analyze the ocular findings of patients who received lifelong leptin therapy due to congenital leptin deficiency(CLD),an extremely rare condition.METHODS:A prospective,cross-sectional comparative study was performed on six patients with CLD and 13 healthy age-and sex-matched controls.The central corneal thickness(CCT),anterior chamber depth(ACD),axial length(AL),keratometry(K1,K2),optical coherence tomography(OCT),and OCT angiography parameters were compared between the leptin and control groups at the baseline visit.The change in these measurements in leptin patients over a two-year period was analyzed.RESULTS:CLD patients had lower mean AL,ACD,and CCT(P≤0.012 for all).Mean K1,K2(P≤0.047 for both),choroidal thickness(P≤0.001),and central ganglion cell layer(GCL)thickness(P=0.029)were higher in the leptin group.Perifoveal superficial capillary plexus(SCP)density was decreased in all quadrants except the temporal region(P<0.05),and parafoveal deep capillary plexus(DCP)density was decreased in the superior hemisphere,temporal quadrant(P≤0.036 for both)and nasal quadrant(P=0.048)in the leptin group.During the two-year follow-up,no changes in anterior and posterior segment measurements were observed in the leptin patients,except for subfoveal choroidal thickness(P<0.001).CONCLUSION:CLD patients exhibit structural alterations in both the anterior and posterior segments of the eye,including notable changes in retinal and choroidal vasculature.However,there is limited evidence concerning the influence of leptin therapy on the eye.
文摘BACKGROUND Papillary fibroelastomas are rare,benign cardiac tumors typically found on the heart valves.This case series presents three patients with aortic valve fibroe-lastoma incidentally detected during endoscopic ultrasound(EUS)for pancreatic and biliary pathologies.These cases highly get the potential of EUS in identifying subtle cardiac lesions during gastrointestinal evaluation,raise important que-stions about the true prevalence of these lesions,and highlight the need for es-tablished guidelines for their management and surveillance.The detection of fib-roelastomas during routine EUS procedures emphasizes the importance of tho-rough assessment and multidisciplinary approaches in managing unexpected dis-coveries.This also highlights the potential thromboembolic risks associated with fibroelastomas and challenges in determining appropriate management strategies for patients who are asymptomatic.CASE SUMMARY The patients aged 72 years,51 years,and 42 years underwent EUS for various indications when aortic valve lesions consistent with fibroelastomas were discovered.These findings were subsequently confirmed by transesophageal echocardiography in two cases,with the third patient awaiting assessment.The lesions ranged in size from 0.61 cm to 1.6 cm in diameter and exhibited characteristic sonographic features of fibroelastomas,including hyperechoic appearance and attachment to the aortic valve leaflets.These cases highlight the potential of EUS in identifying subtle cardiac lesions during gastrointestinal evaluation,raising questions about the true prevalence of fib-roelastomas and the need for establishing management guidelines.CONCLUSION This case series raises important questions regarding the prevalence of aortic valve fibroelastoma lesions in the general population.This highlights the urgent need for comprehensive evidence-based guidelines to standardize the management and long-term surveillance of affected patients.
