Visceral adiposity index and cardiorespiratory fitness:Unmasking risk of impaired fasting glucose among adolescents

Type 2 diabetes mellitus(T2DM)is increasing among adolescents,but paediatric risk predictors are relatively underdeveloped.This study aimed to establish the associations of visceral adiposity index(VAI)and cardiorespiratory fitness(CRF)with fasting plasma glucose in 418 Nigerian adolescents aged 11 years to 19 years.Using a cross-sectional design,participants were stratified by VAI and CRF tertiles to examine variations in impaired fasting glucose(IFG)risk.The findings of this study revealed significant gender differences:In the case of boys,high VAI and low CRF is associated with IFG,while no association was present in girls.CRF,measured by the 20-meter shuttle run,was a stronger predictor of IFG than VAI,suggesting that physical fitness is a protective factor against glucose dysreg-ulation.These findings point to VAI and CRF as useful,non-invasive predictors of risk for T2DM in youth,supporting school-based fitness programs that promote CRF and attenuate visceral adiposity,particularly in males.Future work must validate these predictors across various ethnic populations and identify other risk factors that can augment plans for early interventions aimed at the prevention of adolescent T2DM. 展开更多

Objective We aimed to investigate the patterns of fasting blood glucose(FBG)trajectories and analyze the relationship between various occupational hazard factors and FBG trajectories in male steelworkers.Methods The study cohort included 3,728 workers who met the selection criteria for the Tanggang Occupational Cohort(TGOC)between 2017 and 2022.A group-based trajectory model was used to identify the FBG trajectories.Environmental risk scores(ERS)were constructed using regression coefficients from the occupational hazard model as weights.Univariate and multivariate logistic regression analyses were performed to explore the effects of occupational hazard factors using the ERS on FBG trajectories.Results FBG trajectories were categorized into three groups.An association was observed between high temperature,noise exposure,and FBG trajectory(P<0.05).Using the first quartile group of ERS1 as a reference,the fourth quartile group of ERS1 had an increased risk of medium and high FBG by 1.90and 2.21 times,respectively(odds ratio[OR]=1.90,95%confidence interval[CI]:1.17–3.10;OR=2.21,95%CI:1.09–4.45).Conclusion An association was observed between occupational hazards based on ERS and FBG trajectories.The risk of FBG trajectory levels increase with an increase in ERS. 展开更多

Objective To investigate the association between long-term glycemic control and cerebral infarction risk in patients with diabetes through a large-scale cohort study.Methods This prospective,community-based cohort study included 12,054 patients with diabetes.From 2006 to 2012,38,272 fasting blood glucose(FBG)measurements were obtained from these participants.FBG trajectory patterns were generated using latent mixture modelling.Cox proportional hazards models were applied to assess the subsequent risk of cerebral infarction associated with different FBG trajectory patterns.Results At baseline,the mean age of the participants was 55.2 years.Four distinct FBG trajectories were identified based on FBG concentrations and their changes over the 6-year follow-up period.After a median follow-up of 6.9 years,786 cerebral infarction events were recorded.Different trajectory patterns were associated with significantly varied outcome risks(Log-Rank P<0.001).Compared with the low-stability group,Hazard Ratio(HR)adjusted for potential confounders were 1.37 for the moderate-increasing group,1.23 for the elevated-decreasing group,and 2.08 for the elevated-stable group.Conclusion Sustained high FBG levels were found to play a critical role in the development of ischemic stroke among patients with diabetes.Controlling FBG levels may reduce the risk of cerebral infarction. 展开更多

Objective To observe the effect of electroacupuncture at ear and body acupoints for the instant fasting blood glucose of model machins with type 2 diabetes mellitus(T2DM).Methods For 10 machins with type2 diabetes,the electroacupuncture was applied at "Zusanli"(足三里 ST 36),"Sanyinjiao"(三阴交 SP 6) and three "Jiangtangdian"(降糖点) in the ear continuously for 5 days,with the frequency of 2/15 Hz(The 2 Hz and the 15 Hz were transferred every second) and current of2 mA.The treatment was given once every day for 30 min.The fasting blood glucose was measured before acupuncture,after acupuncture for15 min and 30 min in the 5 days.Results On the first day,after 30-minute acupuncture the fasting blood glucose was reduced obviously compared with that before treatment(P〈0.01).On the third and the fifth days,after acupuncture for 15 min and 30 min the fasting blood glucose was apparently lower than that before acupuncture(all P〈0.05).On the fourth day,after acupuncture for 30 min the fasting blood glucose was decreased markedly than that before acupuncture(P〈0.05).After 30-minute acupuncture on the fifth day,the fasting blood glucose was lower obviously than that before treatment on the first day(P〈0.05).Conclusion ①The electroacupuncture at ear and body acupoints can reduce the instant fasting blood glucose of T2 DM machins apparently.② There is time effect on instant fasting blood glucose of T2 DM machins treated by electroacupuncture.The efficacy of 30-minute stimulation is superior to that of 15-minute acupuncture.After acupuncture continuously for 4 days,the effect is better than that with acupuncture for3 days successively. 展开更多

Recent studies have shown that acute blood glucose elevation in patients with ST-segment elevation myocardial infarction(STEMI)suggests a poor prognosis.To investigate the effect of fasting blood glucose(FBG)on the risk of heart failure(HF)and left ventricular systolic dysfunction(LVSD)in non-diabetic patients undergoing primary percutaneous coronary intervention(PCI)for acute STEMI,we retrospectively recruited consecutive non-diabetic patients who underwent primary PCI for STEMI in our hospital from February 2003 to March 2015.The patients were divided into two groups according to the FBG level.A total of 623 patients were recruited with an age of 61.3±12.9 years,of whom 514(82.5%)were male.The HF risk(odds ratio 3.401,95%confidence interval(CI)2.144–5.395,P<0.001)was significantly increased in patients with elevated FBG than those with normal FBG.Elevated FBG was also independently related to LVSD(β1.513,95%CI 1.282–1.785,P<0.001)in a multiple logistics regression analysis.In conclusion,elevated FBG was independently associated with 30-day HF and LVSD risk in non-diabetic patients undergoing primary PCI for STEMI. 展开更多

Background:Type 2 diabetes(T2D)is a polygenic metabolic disease,character-ized by high fasting blood glucose(FBG).The ability of cranberry(CRN)fruit to regulate glycemia in T2D patients is well known.Here,a cohort of 13 lines of the genetically diverse Collaborative Cross(CC)mouse model was assessed for the effect of non-dialyzable material(NDM)of cranberry extract in lowering fasting blood glucose.Methods:Eight-week-old mice were maintained on either a standard chow diet(con-trol group)or a high-fat diet(HFD)for 12 weeks,followed by injections of intraperi-toneal(IP)NDM(50 mg/kg)per mouse,three times a week for the next 6 weeks.Absolute FBG(mg/dl)was measured bi-weekly and percentage changes in FBG(%FBG)between weeks 0 and 12 were calculated.Results:Statistical analysis showed a significant decrease in FBG between weeks 0 and 12 in male and female mice maintained on CHD.However,a non-significant in-crease in FBG values was observed in male and female mice maintained on HFD dur-ing the same period.Following administration of NDM during the following 6 weeks,the results show a variation in significant levels of FBG lowering between lines,male and female mice and under the different diets.Conclusion:The results suggest that the efficacy of NDM treatment in lowering FGB depends on host genetic background(pharmacogenetics),sex of the mouse(phar-macosex),and diet(pharmacodiet).All these results support the need for follow-up research to better understand and implement a personalized medicine approach/uti-lization of NDM for reducing FBG. 展开更多

BACKGROUND: It has shown that abnormality of peripheral nerve conduction velocity during onset of diabetes mellitus is not related to age and sex, but to symptoms, illness course and level of fasting blood glucose. OBJECTIVE: To measure correlation of abnormality of peripheral nerve conduction velocity with various illness courses, symptoms and levels of fasting blood glucose of patients with type 2 diabetes mellitus. DESIGN: Case analysis. SETTING: Department of Neurology, Central People's Hospital of Huizhou. PARTICIPANTS: A total of 128 patients who were diagnosed as type 2 diabetes mellitus were selected from Central People's Hospital of Huizhou from September 2001 to October 2005. There were 75 males and 53 females aged 32-83 years and the illness course ranged from 1 month to 20 years. METHODS: All 128 patients with type 2 diabetes mellitus received neuro-electrophysiological study and their clinical data were retrospectively analyzed to measure peripheral nerve conduction velocity and fasting blood glucose so as to investigate the correlation of peripheral nerve conduction velocity with clinical symptoms, illness course and levels of fasting blood glucose. MAIN OUTCOME MEASURES: Correlation of peripheral nerve conduction velocity with clinical symptoms, illness course and levels of fasting blood glucose. RESULTS: All 128 patients with type 2 diabetes mellitus were involved in the final analysis. ① Among 128 patients, 114 patients had abnormality of peripheral nerve conduction velocity; 110 patients had clinical symptoms, including 102 patients having abnormality of peripheral nerve conduction velocity; 18 patients did not have clinical symptoms, including 12 patients having abnormality of peripheral nerve conduction velocity. There were significant differences between them (χ 2=8.275, P =0.04). ② Among 128 patients, illness course of 75 patients was equal to or less than 5 years, including 27 patients having abnormality of peripheral nerve conduction velocity; illness course of 53 patients was more than 5 years, including 35 patients having abnormality of peripheral nerve conduction velocity. There were significant differences between them (χ 2=11.469, P =0.003). ③ Among 128 patients, levels of fasting blood glucose of 75 patients was equal to or lower than 11 mmol/L, including 41 patients having abnormality of peripheral nerve conduction velocity; levels of fasting blood glucose of 53 patients was higher than 11 mmol/L, including 38 patients having abnormality of peripheral nerve conduction velocity. There were significant differences between them (χ 2=4.023, P =0.134). CONCLUSION: ① Abnormality of peripheral nerve conduction velocity of patients with type 2 diabetes mellitus is related to illness courses and clinical symptoms. The longer the illness course is, the severer the abnormality of peripheral nerve conduction velocity is. Abnormality of peripheral nerve conduction velocity always occurs on patients who have clinical symptoms. ② Abnormality of peripheral nerve conduction velocity is not related to levels of fasting blood glucose. 展开更多

Objective Recaticimab(SHR-1209)significantly reduces low-density lipoprotein cholesterol levels.However,its effect on glucose metabolism remains unclear.This study aimed to evaluate its effect on glycemic parameters in a Chinese population.Methods Recaticimab versus placebo was administered in a 5:1 ratio to 110 hyperlipidemia patients who were followed up for 24 weeks.Glycated hemoglobin(HbA1c)levels were measured at baseline every 12 weeks.Fasting plasma glucose(FPG)levels were measured at baseline at week 1,3,5,8,12,16,20,and 24.Repeated-measures mixed-effects models were used to determine the longitudinal association between reacticimab and FPG and HbA1c levels.Results Among the 81 participants with normal glucose metabolism,HbA1c levels significantly decreased(F=4.568,P=0.036).In the 29 participants with abnormal glucose metabolism,a significant time effect was observed for FPG levels(F=2.492,P=0.016).For participants with normal and abnormal glucose metabolism,no significant group×time interaction effects on FPG or HbA1c levels were identified.Conclusion Recaticimab showed no adverse glycemic effects in participants with normal or abnormal glucose metabolism,indicating its safety in patients with or without diabetes. 展开更多

Methods Fasting 7:30 and 8:00 a.m. on one day, two and three days before acupuncture, diabetes. Acupuncture was applied at Yishu (胰俞 Extra), Feishu (肺俞BL 13), Pisha which was (脾俞 BL 20), Shenshu (肾俞 BL 23), Sanyinjiao (三阴交 SP 6), Taxixi (太溪 KI 3) and auxiliary points, followed by needle remaining for 30 minutes, for continuous 3 days. Blood glucose values measured immediately after acupuncture were compared with those obtained before acupuncture. Results Before acupuncture application, no significant differences were found between the values measured at 7:30 and 8:00 a.m. (P〉0.05) in fasting blood glucose values. Fasting blood glucose values measured at 8:00 were lower than those at 7:30 on the three days during acupuncture treatment (P〈O.05), and they were also lower than those measured at 8:00 before acupuncture (P〈0.05). BACKGROUND Retinal microcirculation alterations are early indicators of diabetic microvascular complications.Optical coherence tomography angiography(OCTA)is a noninvasive method to assess these changes.This study analyzes changes in retinal microcirculation in prediabetic patients during short-term increases in blood glucose using OCTA.AIM To investigate the changes in retinal microcirculation in prediabetic patients experiencing short-term increases in blood glucose levels using OCTA.METHODS Fifty volunteers were divided into three groups:Group 1[impaired fasting glucose(IFG)or impaired glucose tolerance(IGT)],Group 2(both IFG and IGT),and a control group.Retinal microcirculation parameters,including vessel density(VD),perfusion density(PD),and foveal avascular zone(FAZ)metrics,were measured using OCTA.Correlations between these parameters and blood glucose levels were analyzed in both the fasting and postprandial states.RESULTS One hour after glucose intake,the central VD(P=0.023),central PD(P=0.026),and parafoveal PD(P<0.001)were significantly greater in the control group than in the fasting group.In Group 1,parafoveal PD(P<0.001)and FAZ circularity(P=0.023)also increased one hour after glucose intake.However,no significant changes were observed in the retinal microcirculation parameters of Group 2 before or after glucose intake(P>0.05).Compared with the control group,Group 1 had a larger FAZ area(P=0.032)and perimeter(P=0.018),whereas Group 2 had no significant differences in retinal microcirculation parameters compared with the control group(P>0.05).Compared with Group 1,Group 2 had greater central VD(P=0.013)and PD(P=0.008)and a smaller FAZ area(P=0.012)and perimeter(P=0.010).One hour after glucose intake,Group 1 had a larger FAZ area(P=0.044)and perimeter(P=0.038)than did the control group,whereas Group 2 showed no significant differences in retinal microcirculation parameters compared with the control group(P>0.05).Group 2 had greater central VD(P=0.042)and PD(P=0.022)and a smaller FAZ area(P=0.015)and perimeter(P=0.016)than Group 1.At fasting,central PD was significantly positively correlated with blood glucose levels(P=0.044),whereas no significant correlations were found between blood glucose levels and OCTA parameters one hour after glucose intake.CONCLUSION A short-term increase in blood glucose has a more pronounced effect on retinal microcirculation in prediabetic patients with either IFG or IGT. 展开更多

BACKGROUND Data regarding the most suitable diagnostic method for the diagnosis of glucose impairment in asymptomatic children and adolescents are inconclusive.Furthermore,limited data are available on the reproducibility of the oral glucose tolerance test(OGTT)in children and adolescents who are obese(OB).AIM To investigate the usefulness of the OGTT as a screening method for glucose dysregulation in children and adolescents.METHODS Eighty-one children and adolescents,41 females,either overweight(OW),OB or normal weight(NW)but with a strong positive family history of type 2 diabetes mellitus(T2DM),were enrolled in the present observational study from the Outpatient Clinic of Paediatric Endocrinology of the University Hospital of Patras in Greece.One or two 3-h OGTTs were performed and glucose,insulin and Cpeptide concentrations were measured at several time points(t=0 min,t=15 min,t=30 min,t=60 min,t=90 min,t=120 min,t=180 min).RESULTS Good repetitiveness was observed in the OGTT response with regard to T2DM,while low repetitiveness was noted in the OGTT response with regard to impaired glucose tolerance(IGT)and no repetitiveness with regard to impaired fasting glucose(IFG).In addition,no concordance was observed between IFG and IGT.During the 1st and 2nd OGTTs,no significant difference was found in the glucose concentrations between NW,OW and OB patients,whereas insulin and C-peptide concentrations were higher in OW and OB compared to NW patients at several time points during the OGTTs.Also,OW and OB patients showed a worsening insulin and C-peptide response during the 2nd OGTT as compared to the 1st OGTT.CONCLUSION In mild or moderate disorders of glucose metabolism,such as IFG and IGT,a diagnosis may not be reached using only one OGTT,and a second test or additional investigations may be needed.When glucose metabolism is profoundly impaired,as in T2DM,one OGTT is probably more reliable and adequate for establishing the diagnosis.Excessive weight and/or a positive family history of T2DM possibly affect the insulin and C-peptide response in the OGTT from a young age. 展开更多

BACKGROUND The dynamic characteristics of glucose metabolism and its risk factors in patients living with human immunodeficiency virus(PLWH)who accepted primary treatment with the efavirenz(EFV)plus lamivudine(3TC)plus tenofovir(TDF)(EFV+3TC+TDF)regimen are unclear and warrant investigation.AIM To study the long-term dynamic characteristics of glucose metabolism and its contributing factors in male PLWH who accepted primary treatment with the EFV+3TC+TDF regimen for 156 wk.METHODS This study was designed using a follow-up design.Sixty-one male treatmentnaive PLWH,including 50 cases with normal glucose tolerance and 11 cases with prediabetes,were treated with the EFV+3TC+TDF regimen for 156 wk.The glucose metabolism dynamic characteristics,the main risk factors and the differences among the three CD4+count groups were analyzed.RESULTS In treatment-naive male PLWH,regardless of whether glucose metabolism disorder was present at baseline,who accepted treatment with the EFV+3TC+TDF regimen for 156 wk,a continuous increase in the fasting plasma glucose(FPG)level,the rate of impaired fasting glucose(IFG)and the glycosylated hemoglobin(HbA1c)level were found.These changes were not due to insulin resistance but rather to significantly reduced isletβcell function,according to the homeostasis model assessment ofβcell function(HOMA-β).Moreover,the lower the baseline CD4+T-cell count was,the higher the FPG level and the lower the HOMA-βvalue.Furthermore,the main risk factors for the FPG levels were the CD3+CD8+cell count and viral load(VL),and the factors contributing to the HOMA-βvalues were the alanine aminotransferase level,VL and CD3+CD8+cell count.CONCLUSION These findings provide guidance to clinicians who are monitoring FPG levels closely and are concerned about IFG and decreased isletβcell function during antiretroviral therapy with the EFV+3TC+TDF regimen for long-term application. 展开更多

Objective:To analyze the effect of blood glucose control mursing in intensive care umit (ICU)patients.Methods:The clinical data of 70 severe patients in ICU of ou hospital fom January 2019 to May 2020 were retrospectively analyzed.The climical data of 34 patients with routine intervention were divided into the control group,and the clinical data of 36 patients with routine intervention and blood glucose control mursing were divided into the observation group,all were intervened for 14 days.The blood glucose levels and prognosis of the two groups were compared before intervention and at the end of 14 days of intervention,the time required o achieve the standard blood glucose level of the two groups was recorded Results:After 14 days of intervention,the fasting blood glucose level of the observation group was lower than the control group,the difference was satisically significant(P<0.05);the time of blood glucose reaching the standard in the observation group was shorter than that in the control group,the difference was statistically significant(P<0.05);on the 14th day of intervention,the sequential organ failure asessment score(SOFA)score of the two groups was lower than before intervention,the SOFA score of the observation group was lower than control group,the difference was statistically significant(P<0.05).Conclusion:The effect of blood glucose control mursing in ICU is better,which effectively controlled the blood glucose level of patients and improved the prognosis of patients. 展开更多

Metformin on Blood Glucose Control in Diabetic Patients

Objective: to explore the practical value of metformin in the treatment of diabetic patients, and further analyze the influence of metformin on blood glucose control. Methods: a total of 120 diabetic patients were randomly selected and divided into 2 groups. Collection time: June 2019, deadline: June 2021. In this study, 60 patients were included in the control group (receiving conventional treatment) and the observation group (receiving metformin treatment). The treatment effect, improvement of blood glucose and lipid and creatinine were compared between the two groups. Results: the total effective rate in the observation group was 96.67%, compared with 83.33% in the control group (P<0.05). Before the treatment, two groups after treatment in patients with fasting glucose project, 2 h postprandial blood glucose, glycosylated hemoglobin, project (the corresponding indicators are as follows: FBG, 2 HPG, HbA1C), cholesterol and triglycerides, high-density lipoprotein cholesterol (the corresponding indicators are as follows: TG, TC, HDL) level no bigger difference (P > 0.05);After treatment, the levels of FBG, 2hPG, HbA1C, TG and TC in 2 groups were lower than before treatment, and the observation group was obviously lower than the control group, and the HDL level was higher than the control group after treatment (P<0.05). There was no significant difference in serum creatinine and urine creatinine between the two groups before treatment (P > 0.05). The serum creatinine and urine creatinine levels in the observation group were lower than those in the control group after treatment, but there was no significant difference (P > 0.05). Conclusion: metformin has a significant effect on the control of fasting blood glucose and other indicators and the improvement of blood lipid indicators in diabetic patients, and has no effect on renal function. The comprehensive effect is good, and this program is worthy of clinical promotion. 展开更多

Objective This study aimed to investigate the impact of glycemic control and diabetes duration on subsequent myocardial infarction(MI)in patients with both coronary heart disease(CHD)and type 2 diabetes(T2D).Methods We conducted a retrospective cohort study of 33,238 patients with both CHD and T2D in Shenzhen,China.Patients were categorized into 6 groups based on baseline fasting plasma glucose(FPG)levels and diabetes duration(from the date of diabetes diagnosis to the baseline date)to examine their combined effects on subsequent MI.Cox proportional hazards regression models were used,with further stratification by age,sex,and comorbidities to assess potential interactions.Results Over a median follow-up of 2.4 years,2,110 patients experienced MI.Compared to those with optimal glycemic control(FPG<6.1 mmol/L)and shorter diabetes duration(<10 years),the fullyadjusted hazard ratio(HR)(95%Confidence Interval[95%CI])for those with a diabetes duration of≥10 years and FPG>8.0 mmol/L was 1.93(95%CI:1.59,2.36).The combined effects of FPG and diabetes duration on MI were largely similar across different age,sex,and comorbidity groups,although the excess risk of MI associated with long-term diabetes appeared to be more pronounced among those with atrial fibrillation.Conclusion Our study indicates that glycemic control and diabetes duration significant influence the subsequent occurrence of MI in patients with both CHD and T2D.Tailored management strategies emphasizing strict glycemic control may be particularly beneficial for patients with longer diabetes duration and atrial fibrillation. 展开更多

BACKGROUND Gestational diabetes mellitus(GDM)is one of the most prevalent metabolic disorders of pregnancy.Advanced glycation end-products(AGEs)are a complex and highly heterogeneous group of compounds formed from amino acids and reducing sugars.High-AGE diet exposure during pregnancy may cause adverse effects.AIM To investigate the expression levels of AGE and AGE receptor(RAGE)in the serum and placenta of pregnant women with GDM and to assess the association of their mediated oxidative stress response with perinatal outcomes.METHODS This study retrospectively analyzed the clinical data of 126 pregnant women with GDM who gave birth in the Obstetrics Department of Obstetrics and Gynecology Hospital of Fudan University from January 2023 to January 2024.A total of 85 pregnant women of similar age without GDM during the same period were selected as the control group.Fasting blood glucose,glycated hemoglobin,AGEs,soluble RAGE(sRAGE),and oxidative stress were compared in both groups.Postpartum placental tissue was collected to identify RAGE protein expression.Participants with GDM were categorized based on perinatal outcomes into normal(n=89)and adverse perinatal outcome groups(n=37),and differences in serum AGE–RAGE levels and oxidative stress were analyzed.The influencing factors of adverse perinatal outcomes were analyzed using logistic regression.RESULTS The GDM group demonstrated notably higher serum AGE(t=8.955)and malondialdehyde(MDA)levels(t=14.14)and lower sRAGE(t=16.37)and superoxide dismutase(SOD)levels(t=18.50)than the control group at 24-28 weeks of gestation and before delivery(P<0.0001).Serum AGE levels were positively correlated with MDA and negatively related to SOD at 24-28 weeks of pregnancy(SOD:r=0.393,MDA:r=0.424,P<0.0001)and before delivery(SOD:r=0.443,MDA:r=0.492,P<0.0001),whereas AGE was inversely associated with sRAGE in the GDM group(r=-0.495,P<0.0001).Serum AGE levels were significantly higher(t=9.225,P<0.0001)and the sRAGE level(r=3.563,P<0.0001)was significantly lower in participants with adverse perinatal outcomes than those with normal perinatal outcomes in the GDM group.Logistic regression analysis revealed AGE level as a risk factor(OR=1.056,P<0.0001)and sRAGE level(OR=0.949,P<0.0001)as a protective factor for adverse perinatal outcomes in GDM.CONCLUSION High serum AGE level is a risk factor for adverse perinatal outcomes in GDM,whereas high sRAGE levels are protective.AGEs and RAGE may be associated with oxidative stress in pregnant women with GDM. 展开更多

Diabetes and impaired fasting glucose(IFG)are significant global health concerns.However,the potential effects of mixed heavy metal exposure on these conditions remain underexplored.This study aims to investigate the combined effects of multiple metals on diabetes risk and explore the mediating role of BodyMass Index(BMI)in rural China.Across-sectional analysis involved 2313 adults from 12 provinces in rural China.Urinary levels of zinc(Zn),chromium(Cr),nickel(Ni),cadmium(Cd),and lead(Pb)were quantified using inductively coupled plasma mass spectrometry.Fasting blood glucose(FBG)levels were measured with an automatic biochemical analyzer.Logistic regression models and the Bayesian Kernel Machine Regression(BKMR)model were used to examine associations and interactions.Mediation analysis was performed to assess the role of BMI.The results of our study indicate that there is a significant association between urinary Zn(OR=2.38,95%CI:1.57,3.60),Cr(OR=1.24,95%CI:1.31,1.61),and Ni(OR=1.51,95%CI:1.05,2.18)and the diabetes risk.The study revealed that exposure to Ni amplified the associations between Zn,Cr and diabetes/IFG risk.Additionally,BMI was identified as a significant mediator in the relationship between metal exposure,particularly Cr/Cd,and diabetes risk.These findings reveal a complex link between multiple metals,such as Zn,Cr,and Ni,and diabetes risk,and emphasize the potential opposite mediating effects of BMI in different metal-induced diabetes mechanisms.Further investigation of these mechanisms is warranted. 展开更多

The association between dyslipidemia and elevated fasting glucose in type 2 diabetes is well known. In non-diabetes, whether this association still exists, and whether dyslipidemia is an independent risk factor for high fasting plasma glucose (FPG) levels are not clear. This cross-sectional study recruited 3460 non-diabetic Chinese subjects (1027 men, and 2433 women, aged 35-75 years old) who participated in a health survey. Men and women were classified into tertiles by levels of plasma lipids respectively. In women, the prevalence of impaired fasting glucose (IFG) was decreased with increased HDL-C. A stepwise increase in HDL-C was associated with decreasing FPG levels (lowest tertiles, FPG: 5.376 ± 0.018; middle tertiles, 5.324± 0.018; highest tertiles, 5.276±0.018mmol/L; P = 0.001). Reversely, FPG levels increased from lowest tertiles to highest tertiles of LDL-C, TC, and TG. we found that women in the first tertile with lower HDL-C level had a 1.75-fold increase in risk of IFG compared with non-diabetic women in the third tertile with higher HDL-C level (OR: 1.75; 95% CI: 1.20-2.56). In men, no significant association was found. We took age, BMI, waist/hip ratio, education, smoking, alcohol drinking, and physical exercise as adjusted variables. In Chinese non-diabetic women, dyslipidemia is independently associated with high levels of FPG; TG, HDL-C, and LDL-C are predictors of IFG independent of BMI and waist/hip ratio. 展开更多

Gilg have been used in folk medicine for the treatment of diabetes in Guangdong Province,China.In this study,effects of ethanol extract of Aquilaria sinensis leaves on hyperglycemia were investigated in diabetic db/db mice.After 4 weeks of administration,the 95%ethanol(EtOH) extract of Aquilaria sinensis leaves (AE),especially at high dose level(600 mg/kg),activated AMP-activated protein kinase(AMPK),resulting in reduced fasting blood glucose and glycosylated hemoglobin levels in db/db mice.In addition,the oral glucose tolerance test(OGTT test) showed that AE could remarkably improve insulin resistance.Compared to Thiazolinediones(TZDs),no weight gain was observed after AE administration,which is a severe side effect of TZDs.The data suggested that AE could act as an activator of AMPK,and might be used as an alternative to TZDs in the management of obesity-related diabetes. 展开更多

Efficacy and safety of saxagliptin in patients with type 2 diabetes mellitus:a meta-analysis of randomized controlled trials 被引量:2

As a new oral hypoglycemic agent, saxagliptin belongs to the class of dipeptidyl peptidase-4 (DPP-4) inhibitors. However, it remains inconclusive whether saxagliptin is associated with increased risk of adverse events (AE) and efficacy as add-on treatment. Therefore, we performed an up-to-date meta-analysis to compare the efficacy and safety of saxagliptin with placebo and other oral hypoglycemic agents in adult patients with type 2 diabetes mellitus (T2DM). Randomized clinical trials (RCTs) comparing saxagliptin with comparators were retrieved by selecting articles from Pubmed, Embase, Cochrane Library and Clinical Trials Registry Platform up to Oct. 2013. Weighted mean difference (WMD) was used to analyze the effect of hypoglycemic agents on HbAlc, weight and fasting plasma glucose (FPG). While the patients who achieved HbAlc〈7.0% and had AE were analyzed as relative risks (RR). A total of 18 articles from 16 RCTs and one clinic trial from the WHO International Clinical Trials Registry Platform met the included criterion. Clinically significant decrease from baseline HbAlc compared with placebo was certified for 2.5 mg/day saxagliptin (WMD = -0.45%, 95% CI, -0.48% to -0.42%) and 5 mg/d saxagliptin (WMD = -0.52%, 95% CI, -0.60% to -0.44%). Saxagliptin as add-on therapy was superior to thiazolidinediones, up-titrated glyburide, up-titrated metformin or metformin monotherapy in achieving HbA1c〈7.0%. Treatment with saxagliptin had negligible effect on weight, and it was considered weight neutral. Saxagliptin treatment did not increase the risk of hypoglycemia (RR = 1.28, 95% CI 0.72 to 2.27, P = 0.40) and serious adverse experiences (RR = 1.25, 95% CI 0.94 to 1.66, P = 0.13). No statistically significant differences were observed between saxagliptin and comparators in terms of the risk of infections. The present study showed that saxagliptin was effective in improving glycaemic control in T2DM with a low risk of hypoglycaemia and incidence of infections in either monotherapy or add-on treatment. This founding should be further certified by large-sample size and good-designed RCT. 展开更多
