Objective: To investigate the biological basis of qi, blood and vessel in immune thrombocytopenia(ITP) patients with syndrome of qi failing to govern blood(SQFGB) based on traditional Chinese medicine.Methods: A total...Objective: To investigate the biological basis of qi, blood and vessel in immune thrombocytopenia(ITP) patients with syndrome of qi failing to govern blood(SQFGB) based on traditional Chinese medicine.Methods: A total of 52 ITP patients with SQFCB were enrolled and divided into bleeding group(38 cases) and non-bleeding group(14 cases).Bleeding group was further divided into mild qi deficiency group(25 cases) and moderate/severe qi deficiency group(13 cases) based on Chinese Medicine syndrome score.20 healthy volunteer were recruited as control group.The count of platelet(PLT) was taken as the blood related indicator.The expressions of cytokines including IL-1β, IL-17 A, TNF-α, CD40 L, and TGF-β, detected by Aim Plex Multiple Immunoassays for Flow, were taken as the qi related indicators.The expressions of VEGF-A, detected by Aim Plex Multiple Immunoassays for Flow and NO, NOS, and ET-1 detected by ELISA, were taken as the vessel related indicators.Results: As compared to the control group, the count of PLT, taken as the blood related indicator, was significantly lower in ITP group patients with SQFCB(P<0.05).The expression levels of IL-17 A and TNF-α, taken as the qi related indicators, were significantly higher, while those of CD40 L, IL-1β, and TGF-β, also taken as the qi related indicators, were significantly lower in ITP patients with SQFCB, respectively(P<0.05).The expression levels of NO and ET-1, taken as the vessel related indicators, were significantly higher, while the expression levels of NOS and VEGF-A also taken as the vessel related indicators, were significantly lower in ITP patients with SQFCB, respectively(P<0.05).The count of PLT, taken as the blood related indicator, was significantly lower in moderate/severe group than those in mild group(P<0.05).The expression levels of CD40 L and TGF-β, taken as the qi related indicators, were also significantly lower in moderate/severe group than those in mild group, respectively(P<0.05).Conclusion: The count of PLT might be the biological basis of blood.The expressions of NO, NOS, ET-1 and VEGF-A might be the biological basis of vessel.The expressions of IL-1β, IL-17 A, TNF-α, TGF-β, and CD40 L may be the biological basis of qi.The expressions of CD40 L and TGF-β could reflect the degree of qi deficiency in ITP patients based on the theory of qi and blood.展开更多
Objective To observe the clinical efficacy of pediatric Tuina(Chinese therapeutic massage)plus oral administration of Chinese medication for pediatric anorexia due to spleen failing in transportation.Methods A total o...Objective To observe the clinical efficacy of pediatric Tuina(Chinese therapeutic massage)plus oral administration of Chinese medication for pediatric anorexia due to spleen failing in transportation.Methods A total of 94 children with anorexia due to spleen failing in transportation were randomized into an observation group and a control group by the random number table method at a ratio of 1:1,with 47 cases in each group.The control group was treated with modified Bu Huan Jin Zheng Qi San,and the observation group was treated with additional Tuina manipulation of Yun Shui Ru Tu based on the treatment in the control group.After 4 weeks of treatment,the clinical efficacy of the two groups was observed,and the traditional Chinese medicine(TCM)symptom score,serum leptin,neuropeptide Y,and serum zinc and calcium levels were compared.Results After 4 weeks of treatment,the total effective rate of the observation group was 87.2%,and that of the control group was 74.5%.The difference between the two groups was statistically significant( P<0.05).After treatment,the TCM symptom scores in both groups decreased significantly(P<0.05),and the scores in the observation group were lower than those in the control group(P<0.05).After treatment,the serum leptin level in both groups decreased,and the level in the observation group was lower than that in the control group(P<0.05).The serum neuropeptide Y,zinc,and calcium levels in both groups increased after treatment,and the levels in the observation group were higher than those in the control group(P<0.05).Conclusion Tuina manipulation of Yun Shui Ru Tu plus oral administration of Chinese medication is significantly effective in treating pediatric anorexia due to spleen failing in transportation,which can improve the appetite of children and improve the symptoms of anorexia,and the curative effect is better than that of oral administration of Chinese medication alone.展开更多
BACKGROUND The pathophysiology behind gastroesophageal reflux disease and its association with poor outcomes after lung transplantation is incompletely understood.The physiologic impact of lung transplantation on pulm...BACKGROUND The pathophysiology behind gastroesophageal reflux disease and its association with poor outcomes after lung transplantation is incompletely understood.The physiologic impact of lung transplantation on pulmonary function,intrathoracic pressures,and vagal innervation may affect esophageal motility,bolus clearance and reflux risk.However,the effect of changes in esophageal function after lung transplantation on the risk of poor post-transplant outcomes remains unclear.AIM To evaluate the association between change in esophageal motility pre-/post-lung transplantation and rejection outcome.METHODS This was a retrospective cohort study of lung transplant recipients who underwent both pre-and post-transplant esophageal testing including high resolution manometry(HRM)at a tertiary center.Acute cellular rejection(ACR)was defined histologically per International Society for Heart and Lung Transplantation criteria.Univariate analyses were performed using student’s t-test,χ2 test,and Spearman’s correlation where appropriate.Multivariable time-to-event analysis using Cox proportional hazards model was applied.Subjects not meeting ACR outcome were censored at death or date of last clinic visit.RESULTS 55 subjects(65%men,mean age:61,median follow-up:840 days)were included,with 17 (31%) experiencing ACR. Increase in failed swallows correlated with lower baseline total lung capacity(TLC) (R = -0.32, P = 0.05) and decreased post-transplant esophageal bolus clearance (R = -0.45, P = 0.004). Onmultivariable analysis, post-transplant hypomotility independently predicted increased ACR (HR: 3.62, 95%CI:1.11-11.8;P = 0.03). Kaplan-Meier analysis demonstrated increased ACR for subjects with increased vs unchangedfailed swallows post-transplant (P = 0.048). On Cox regression, a 20% elevated risk of ACR was found for every10% increase in failed swallows, after controlling for confounders including reflux severityCONCLUSIONEsophageal hypomotility, specifically an increase in failed swallows on HRM, from pre- to post-lungtransplantation was independently associated with ACR. Additionally, lower baseline TLC correlated with increasein failed swallows, suggesting restrictive lung disease may be associated with post-transplant esophagealhypomotility. Lung transplantation may affect esophageal function and contribute to rejection outcomes. BACKGROUND When patients with a failed hip arthroplasty are unsuitable for reimplantation,Girdlestone resection arthroplasty(GRA)is a viable treatment option.We report on a patient who was treated with a GRA due to a ...BACKGROUND When patients with a failed hip arthroplasty are unsuitable for reimplantation,Girdlestone resection arthroplasty(GRA)is a viable treatment option.We report on a patient who was treated with a GRA due to a periprosthetic infection.We discovered partial paralysis of the quadriceps muscle in this patient.We investigated the femoral nerve anatomy,particularly the nerve entry points,to better understand this phenomenon.AIM To reveal the femoral nerve anatomy with respect to severe proximal migration after GRA.METHODS Eight cadaveric hemipelves were investigated.The branches of the femoral nerve were dissected and traced distally.The GRA was performed by the direct anterior approach.Axial stress to the lower extremity was applied,and the relative movement of the femur was recorded.The femoral nerve and its entry points were assessed.RESULTS GRA led to a 3.8 cm shift of the femur in vertical direction,a 1.8 cm shift in the dorsal direction,and a 2.3 cm shift in the lateral direction.A 36.5°external shift was observed.This caused stress to the lateral division of the femoral nerve.We observed migration of the femoral nerve entry point at the following locations:(1)Vastus medialis(5.3 mm);(2)The medial part of the vastus intermedius(5.4 mm);(3)The lateral part of the vastus intermedius(16.3 mm);(4)Rectus femoris(23.1 mm);(5)Tensor vastus intermedius(30.8 mm);and(6)Vastus lateralis(28.8 mm).CONCLUSION Migration of the femur after GRA altered the anatomy of the femoral nerve.Stress occurred at the lateral nerve division leading to poor functional results.展开更多

In this paper, according to the characteristics of failed sucker rod-pumped well randomness and strong outburst, with the gray GM (1,1) forecast model and the Markov forecast model combined, gray GM (1,1) forecast model is utilized to handle the primary data of an oilfield, and Markov forecast model is utilized to calculate the state transfer probability of forecast value. Then, the gray Markov forecast model considering the influence of randomness factors is formed. Then we made the filling material gunited and solidified to maintain the filling goal of the gob-side entry retaining beside the lane. Considering the law of energy conservation and law of pressure distribution for retaining the active and Static soil of the wall, we analyzed the reliability of a gangue blocking facilities and the stability of the filling mate- rial in the lane. 文摘Combined acupuncture and Chinese herb decoction were applied for 5 cases with graphospasm.Acupuncture included scalp acupuncture and body acupuncture.Using transcranial repeated needling,Baihui(百会GV20),Chorea-trembling controlled area,Emotional area,as well as Dazhui(大椎GV14),Fengchi(风池GB20),and Yingxiang(迎香EX-HN8)etc.were stimulated by proposed by Sun Shentian.Meanwhile,the patients took orally Chinese herb decoction,on the basis of the Siwu decoction(四物汤four substances decoction)addition and subtraction,relieve symptoms under syndrome differentiation.The treatment was carried out for 4 courses in total.One course consisted of 5 days,with an interval of 2 days between two courses.After treatment of 5 patients,2 patients were cured,2 patients achieved markedly effective response,and 1 patient achieved effective response.

文摘In the construction of the filling gob-side entry retaining in a lane, we utilize the self-slide natural phenomenon of a falling gangue in an inclined coal seam goaf. First, we put the falling gangue of goal above the laneway and made it the main filling material by adopting the measurement of flexible supporting system combined with those of rigid supporting system. Then we made the filling material gunited and solidified to maintain the filling goal of the gob-side entry retaining beside the lane. Considering the law of energy conservation and law of pressure distribution for retaining the active and Static soil of the wall, we analyzed the reliability of a gangue blocking facilities and the stability of the filling mate- rial in the lane. We analyze the figures to see the stability. The result shows that the gangue block sup- porting system is reliable, and has been successfully practically applied.
文摘Several methods have been proposed in recent years to counteract climate change and ocean acidification by removing CO2 from the atmosphere (Carbon Dioxide Removal). The most versatile and widely applicable of these methods is enhanced weathering of olivine, which is capable of removing billions of tons of CO2 from the atmosphere at moderate cost compared to the high cost of Carbon Capture and Storage (CCS). Despite its disadvantages, CCS is still the favored solution of many governments. Many misunderstandings of the potential of enhanced weathering persist. This paper addresses some of these misunderstandings, mainly caused by models which ignore established facts.

文摘A learning with error problem based encryption scheme that allows secure searching over the cipher text is proposed. Both the generation of cipher text and the trapdoor of the query are based on the problem of learning with errors. By performing an operation over the trapdoor and the cipher text, it is able to tell if the cipher text is the encryption of a plaintext. The secure searchable encryption scheme is both cipher text and trapdoor indistinguishable. The probabilities of missing and failing match occurrence in searching are both exponentially small.

文摘Appropriate heart rate in a failing pulmonary ventricle remains unknown, particularly in congenital heart diseasewith unique hemodynamics. A 71-year-old male with repaired tetralogy of Fallot and a pacemaker for a sinusnode dysfunction suffered from heart failure symptoms with preserved left ventricular function. Simply changingthe pacemaker’s lower rate from 60 to 75 bpm, New York Heart Association classification improved from III to II,and hemodynamic parameters drastically improved. We regarded this case as informative. Appropriate heart ratecould be higher in congenital patients with failing right and non-failing left ventricles than in adults with malfunctioningLV.
