Cardiac papillary fibroelastoma is a benign heart tumor that,despite its histologically benign nature,can have devastating clinical consequences.Advances in cardiac imaging have made papillary fibroelastoma the most common primary benign tumor,surpassing cardiac myxoma.These tumors predominantly occur on the surface of heart valves,with the aortic valve being the most frequently affected(44%),followed by the mitral valve(35%)and other locations.Aortic fibroelastomas are the second most common primary cardiac tumor,known for their risk of embolic complications.Surgical excision is recommended for symptomatic lesions,but the management of small and asymptomatic fibroelastomas remains uncertain,particularly in elderly patients with comorbidities.[1-3]. 展开更多

BACKGROUND Papillary fibroelastomas are rare,benign cardiac tumors typically found on the heart valves.This case series presents three patients with aortic valve fibroe-lastoma incidentally detected during endoscopic ultrasound(EUS)for pancreatic and biliary pathologies.These cases highly get the potential of EUS in identifying subtle cardiac lesions during gastrointestinal evaluation,raise important que-stions about the true prevalence of these lesions,and highlight the need for es-tablished guidelines for their management and surveillance.The detection of fib-roelastomas during routine EUS procedures emphasizes the importance of tho-rough assessment and multidisciplinary approaches in managing unexpected dis-coveries.This also highlights the potential thromboembolic risks associated with fibroelastomas and challenges in determining appropriate management strategies for patients who are asymptomatic.CASE SUMMARY The patients aged 72 years,51 years,and 42 years underwent EUS for various indications when aortic valve lesions consistent with fibroelastomas were discovered.These findings were subsequently confirmed by transesophageal echocardiography in two cases,with the third patient awaiting assessment.The lesions ranged in size from 0.61 cm to 1.6 cm in diameter and exhibited characteristic sonographic features of fibroelastomas,including hyperechoic appearance and attachment to the aortic valve leaflets.These cases highlight the potential of EUS in identifying subtle cardiac lesions during gastrointestinal evaluation,raising questions about the true prevalence of fib-roelastomas and the need for establishing management guidelines.CONCLUSION This case series raises important questions regarding the prevalence of aortic valve fibroelastoma lesions in the general population.This highlights the urgent need for comprehensive evidence-based guidelines to standardize the management and long-term surveillance of affected patients. 展开更多

Although they are histologically benign and usually asymptomatic, CPFs can lead to serious and life-threatening complications like myocardial infarction, stroke, pulmonary embolus, cardiac arrest etc. CPFs represent a rare entity in clinical medicine and literature regarding their management is limited. We report two cases which illustrate such complications arising from undiagnosed CPFs on the aortic valve. We further stress on the importance of identifying CPFs early so that they can be managed appropriately based on recommendations from the available literature. 展开更多

Papillary fibroelastomas (PFEs) are benign tumors of the endocardium that most frequently affect cardiac valves and typically present with embolic symptoms such as stroke or transient ischemic attack (TIA). Surgical excision is usually recommended for left-sided tumors and is associated with excellent long-term outcomes. The use of a robot-assisted, minimally invasive surgical approach for management of mitral valve disease is growing, and has been associated with shorter hospital stays and improved early quality of life. Three-dimensional (3D) transesophageal echocardiography (TEE) offers several advantages in the assessment of mitral valve disease and cardiac tumors, including the ability to precisely locate the site of attachment of the mass and the spatial relationships to surrounding structures. These factors are particularly important when planning a surgical approach. We report two cases of mitral valve PFEs which were successfully removed using a robot-assisted, minimally invasive surgical approach with 3D TEE imaging. This approach to treatment of PFEs is an attractive alternative to the traditional approach involving median sternotomy. 展开更多

Papillary fibroelastoma, a rare primary benign tumor, can be found anywhere in the heart, but most commonly involves the cardiac valves. Most papillary fibroelastomas do not cause symptoms and are usually incidental found by routine echocardiography or at autopsy. However, with the advent of echocardiography, more and more patients are diagnosed in life. Early diagnosis of this condition becomes very important since it represents a surgically correctable cause for systemic emboli, stroke, myocardial infarction, and sudden cardiac death. The echocardiographic findings should be confirmed by histology because the clinical differential diagnoses includes myxoma, vegetation, thrombi, lipoma, and pseudopapillary fibroelastoma. We reported a case of papillary fibroelastoma of the aortic valve and presented a comprehensive review of the literature. 展开更多

Papillary fibroelastoma of the aortic valve is an uncommon benign tumor of the heart that can present with embolic events.We report a case of 54-year-old lady with exertional chest pain and prior history of ST segment elevation myocardial infarction who was subsequently found to have a fibroelastoma of the aortic valve.The absence of angiographically significant coronary artery disease and resolution of anginal symptoms post-surgery in our patient points to the possibility of fibroelastoma causing these anginal symptoms.Although uncommon,fibroelastoma are being recognized more frequently with the help of transesophageal echocardiography.Hence,in the absence of significant coronary artery disease,we emphasize the importance of consideration of papillary fibroelastoma of the aortic valve as a cause of angina.We also discuss the key aspects of the fibroelastoma including presentation,diagnostic modalities and treatment options. 展开更多

Methods CPFE is a rare benign tumor occuring mainly in valves and often revealed by serious cerebral, coronary and peripheral embolism or even sudden death. We report a case of mitral PFE diagnosed by transesophageal echocardiography (TEE), and surgical resection was performed. Results Surgical intervention is successful, and the follow-up courses are uneventful. Conclusion TEE is helpful to diagnosis and guiding the operation. Operation is recommended to prevent recurrent embolization complications. 展开更多

目的探究发生于心脏瓣膜的乳头状弹力纤维瘤的临床及病理特征,为临床治疗提供参考。方法对1例风湿性心脏病合并二尖瓣腱索乳头状弹力纤维瘤病人,进行手术治疗,对二尖瓣肿物的标本行病理检查进行形态学研究,并复习文献。结果心脏乳头状... 目的探究发生于心脏瓣膜的乳头状弹力纤维瘤的临床及病理特征,为临床治疗提供参考。方法对1例风湿性心脏病合并二尖瓣腱索乳头状弹力纤维瘤病人,进行手术治疗,对二尖瓣肿物的标本行病理检查进行形态学研究,并复习文献。结果心脏乳头状弹力纤维瘤病人多无特殊症状。心脏超声表现为二尖瓣腱索见一条索状稍强回声。组织学表现为:肿瘤呈乳头状突起,表面被覆单层立方上皮细胞,中心为无血管的轴心区,主要部分是特征性的淡红色的弹力纤维组织。结论风湿性心脏病是一种常见的心脏疾病,而乳头状弹力纤维瘤是一种少见且发病隐匿的心脏肿瘤,病人常无明显症状,可引起栓塞风险。该例肿瘤位于二尖瓣乳头肌腱索处很罕见。术前行超声心动图及手术完整切除肿瘤是提高心脏乳头状弹力纤维瘤病人诊断率及治疗的关键。 展开更多

目的探讨心脏乳头状弹力纤维瘤(CPF)的临床和超声心动图表现。方法回顾性分析经手术病理证实的13例CPF患者的临床表现、超声心动图特征、手术方式及预后。结果 13例患者临床表现不具特异性,为不同程度胸闷、气短等症状。所有肿瘤均为单... 目的探讨心脏乳头状弹力纤维瘤(CPF)的临床和超声心动图表现。方法回顾性分析经手术病理证实的13例CPF患者的临床表现、超声心动图特征、手术方式及预后。结果 13例患者临床表现不具特异性,为不同程度胸闷、气短等症状。所有肿瘤均为单发,位于瓣膜9例(9/13,69.23%),其中主动脉瓣4例、二尖瓣3例、三尖瓣2例;位于心腔4例(4/13,30.77%),其中右心房2例、右心室1例、左心室1例。肿瘤最大径>2mm者8例,最大径≤2mm者5例。超声检出8例,未检出5例。8例接受肿瘤切除,5例接受瓣膜置换术。结论 CPF临床表现不特异,当肿瘤体积较小时,超声心动图常无法发现;心脏瓣膜为其常见部位,尤其是主动脉瓣。CPF预后良好。 展开更多

我院1965年至1993年间,共检出心脏原发肿瘤236例,其中16例为非粘液性肿瘤,占同期心脏原发肿瘤的6.8%。包括脉管瘤7例,间叶细胞瘤5例,间皮细胞瘤、脂肪瘤、横纹肌肉瘤及纤维弹力瘤各1例。良性和恶性肿瘤各8例。

目的 探讨发生于心脏瓣膜的乳头状弹力纤维瘤的临床和病理学特征 ,为临床治疗提供依据。方法 应用光镜、组织化学、免疫组化方法对 1例发生于主动脉瓣的乳头状弹力纤维瘤进行形态学研究并复习文献。结果 心脏的乳头状弹力纤维瘤临床... 目的 探讨发生于心脏瓣膜的乳头状弹力纤维瘤的临床和病理学特征 ,为临床治疗提供依据。方法 应用光镜、组织化学、免疫组化方法对 1例发生于主动脉瓣的乳头状弹力纤维瘤进行形态学研究并复习文献。结果 心脏的乳头状弹力纤维瘤临床上缺乏特征性的症状和体征。心动超声提示主动脉瓣异常回声。组织学 :瘤组织呈乳头状生长 ,乳头中央为红染均质物 ,表面衬覆单层立方或扁平上皮。免疫组化 :乳头状表面衬覆的上皮CD31和CD34(+) ,乳头轴心则CD117和S 10 0蛋白 (+) ,弹力纤维染色 (+)。结论 发生于心脏的乳头状弹力纤维瘤十分罕见 ,主要发生于瓣膜表面。尽管在病理学方面根据此肿瘤的发生部位及组织学形态不难做出诊断 ,但因其临床症状不典型 。 展开更多
