A child should be cared for by biological parents or extended family members when possible. In the absence of these two possibilities, other options are in place such as Residential Child Care Facilities (RCCFs). However, RCCFs are generally regarded as last resorts, as it is noted that they have a negative impact on the well-being of the children. Caregivers in RCCFs should ensure that these children are being adequately cared for as they substitute their biological or extended families. It is also a child’s rights to be provided love, care and support coupled with a sense of belonging, recognition and independence. However, children without or inadequate parental care are likely to be vulnerable from all forms of violence. The aim of this qualitative, exploratory, descriptive, contextual and phenomenological study was to explore and describe the children’s experiences regarding the care received at the Residential Child Care Facilities and to provide recommendations regarding research findings. Data were collected through one on one in-depth interviews and Tech method was used to analyze data. The results of the study show that children in RCCFs have experienced both positive and negative care. The care is experienced through relationships with staff in the facilities, provision of their basic needs, knowledge of their cultural identity and their adult future preparation once they leave the facilities. Thus, it is recommended that a conducive environment that contributes to the quality of care for children in RCCFs be established, taking into consideration human resources capacity, policy guidelines and standards in place while providing care to those children.展开更多

文摘In recent years,train-tail swaying of 160 km/h electric multiple units(EMUs)inside single-line tunnels has been heavily researched, because the issue needs to be solved urgently. In this paper,a co-simulation model of vortex-induced vibration(VIV)of the tail car body is established, and the aerodynamics of train-tail swaying is studied. The simulation results were confirmed through a field test of operating EMUs.Furthermore, the influence mechanism of train-tail swaying on the wake flow field is studied in detail through a wind-tunnel experiment and a simulation of a reduced-scaled train model. The results demonstrate that the aerodynamic force frequency(i.e.,vortex-induced frequency)of the train tail increases linearly with train speed. When the train runs at 130 km/h,with a small amplitude of train-tail swaying(within 10 mm),the vortex-induced frequency is 1.7 Hz,which primarily depends on the nose shape of the train tail. After the tail car body nose is extended, the vortex-induced frequency is decreased. As the swaying amplitude of the train tail increases(exceeding 25 mm),the separation point of the high-intensity vortex in the train wake shifts downstream to the nose tip,and the vortex-induced frequency shifts from 1.7 Hz to the nearby car body hunting(i.e.,the primary hunting)frequency of 1.3 Hz,which leads to the frequency-locking phenomenon of VIV,and the resonance intensifies train-tail swaying. For the motor vehicle of the train tail, optimization of the yaw damper to improve its primary hunting stability can effectively alleviate train-tail swaying inside single-line tunnels.Optimization of the tail car body nose shape reduces the amplitude of the vortex-induced force, thereby weakening the aerodynamic effect and solving the problem of train-tail swaying inside the single-line tunnels.

文摘BACKGROUND Older patients often experience thirst after general anesthesia for gastrointestinal surgery, particularly during the post-anesthesia care unit(PACU)phase.Thirst not only compromises patient comfort but also induces anxiety, which may hinder recovery.Although previous studies have addressed postoperative thirst, research specifically examining thirst experiences and nursing needs in older patients remains limited.AIM To explore thirst experiences and nursing needs of older PACU patients following gastrointestinal surgery, aiming to inform targeted interventions.METHODS This study employed a phenomenological approach within a qualitative research framework.A purposeful sampling method was used to select 12 older patients who underwent gastrointestinal surgery under general anesthesia at a tertiary hospital in Shanghai between November and December 2024.Data were collected through semi-structured interviews and analyzed using Colaizzi’s seven-step method.Themes were extracted from the interview data.RESULTS Analysis of the interview data identified four main themes and eight subthemes:(1)Intense sensations of thirst post-surgery(subjective experience of thirst and duration of thirst);(2)Emotional experiences of thirst-related discomfort(anxiety, irritability, and helplessness);(3)Practical challenges in relieving thirst(limitations of current interventions and nursing response time);and(4)Patient expectations of nursing care(desire for more timely interventions and expectation for more proactive attention from nursing staff).CONCLUSION Older patients frequently experience substantial thirst discomfort after gastrointestinal surgery under general anesthesia.A multidisciplinary perioperative intervention plan is essential to alleviate these symptoms and enhance postoperative comfort.

文摘Objective:To explore symptom experiences and self-coping patterns during the early and late stages of chemotherapy in these patients to provide a basis for developing targeted symptom management strategies. Methods:A total of 27 patients with pancreatic cancer undergoing chemotherapy at two medical institutions were recruited between November 2023 and August 2024.Semi-structured interviews were conducted in person or over the phone.Data were analyzed using traditional content and thematic analyses. Results:Three themes were identified:symptom experience,self-coping patterns, and existing obstacles.During the early stages of chemotherapy, patients reported a higher frequency of unpleasant symptoms and recognized these symptoms earlier in the treatment course.Patients in the early stages primarily relied on external support to cope with symptoms, while those in the later stages adopted self-care strategies.Several challenges related to unpleasant symptoms were observed, which appeared to correlate with the self-coping patterns employed. Conclusion:Patients with pancreatic cancer undergoing chemotherapy experience a complex and diverse range of symptoms, with varying coping patterns at different stages of treatment.Symptom management during chemotherapy presents significant challenges.Healthcare providers should improve the ongoing monitoring of symptoms post-chemotherapy.By linking patients’symptom experiences and self-coping patterns at different stages of chemotherapy to their specific challenges, personalized symptom management strategies can be developed to enhance care quality.
