Breast cancer is a malignant tumor originating from breast epithelial tissue.In essence,breast epithelial cells undergo gene mutation under the influence of carcinogenic factors,leading to abnormal cell proliferation ...Breast cancer is a malignant tumor originating from breast epithelial tissue.In essence,breast epithelial cells undergo gene mutation under the influence of carcinogenic factors,leading to abnormal cell proliferation and loss of organism regulation,ultimately leading to the formation of tumors with invasive and metastatic capabilities.Carcinogenic factors of breast cancer involve multiple cellular and molecular mechanisms.Among them,disseminated tumor cells(DTCs)are considered important for treating breast cancer.However,traditional bulk sequencing techniques have limitations,such as the inability to distinguish individual cell differences and dilution of information from key cell subpopulations(such as cancer stem cells and rare immune cells).Single-cell sequencing(scRNA-seq)overcomes the heterogeneity of tumors that traditional sequencing cannot capture by analysing the molecular characteristics of single cells,providing a highresolution perspective for precise typing of breast cancer,exploration of the mechanism of the microenvironment,and personalized treatment.Through this technology,researchers can identify specific gene expression profiles of different cell subpopulations,thus providing a new basis for the molecular typing and personalized treatment of breast cancer.This article explains how single-cell sequencing is used to describe the origin of disseminated tumor cells(DTCs),analyse tumor heterogeneity,metastasis,etc.,and review the current literature on the use of scRNA-seq in breast cancer treatment.In the future,cell separation and processing steps in single-cell sequencing will be further improved to ensure the accuracy of the results and broader application in clinical diagnosis and treatment.展开更多

BACKGROUND Acute disseminated encephalomyelitis(ADEM),which is rare,primarily affects children.It usually manifests as acute encephalopathy and multifocal neurological impairments after infection or vaccination.Diagno...BACKGROUND Acute disseminated encephalomyelitis(ADEM),which is rare,primarily affects children.It usually manifests as acute encephalopathy and multifocal neurological impairments after infection or vaccination.Diagnosis is still difficult due to the clinical and radiological similarity to other central nervous system disorders.Adult-onset ADEM calls for thorough reporting in order to improve diagnosis and treatment.CASE SUMMARY A 55-year-old man with hypertension had a high fever,intense headache and a steady decline in his neurological function after two weeks.Left facial paralysis was the initial symptom,which progressed to left hemiparesis,reduced consciousness level,photophobia,phonophobia,vomiting,and a focal seizure in the right leg.He had no history of autoimmune disease,vaccinations,or infections.Investigations showed negative infectious/autoimmune serology,mild cerebrospinal fluid lymphocytic pleocytosis(protein 76 mg/dL),and lymphopenia.Brain magnetic resonance imaging without contrast revealed bilateral,symmetrical T2/fluid-attenuated inversion recovery hyperintensities,primarily in the middle cerebellar peduncles,with minor involvement in the pontine and periventricular regions.Neoplastic,metabolic,vascular,and infectious conditions were not included.The patient showed spontaneous neurological improvement by Week 3 with near-complete motor recovery(limb strength 4/5)after methylprednisolone and rehabilitation,despite logistical delays in starting immunotherapy.The monophasic course and radiological/clinical remission were supported by idiopathic ADEM.CONCLUSION This case shows an uncommon,idiopathic,cerebellar-predominant ADEM variation in an adult without conventional triggers.It emphasizes the diagnostic difficulty in distinguishing ADEM from mimics(such as stroke or infection)in adults.Spontaneous improvement before treatment,although early detection is still crucial,should be highlighted,although early detection is still crucial.Increased clinician awareness,fair access to neuroimaging,and focused research on adult ADEM are crucial to fill these gaps and improve outcomes in places with limited resources.展开更多

This method reduces th...One of the most promising vector control strategies for controlling dengue fever is the stable introduction of the obligate intracellular Wolbachia bacteria strain into Aedes aegypti mosquitoes. This method reduces the mosquito’s ability to transmit dengue through reproductive strategies associated with Wolbachia infection, such as parthenogenesis, male elimination or feminization, sex ratio distortions and cytoplasmic incompatibility. Expert knowledge and a risk assessment framework were used to identify the risks associated with the release of Wolbachia-bearing mosquitoes. Then, consultations with individual participants were organized and a Bayesian network (BN) was developed to capture the relationship between the hazards as well as the likelihood of these adverse events occurring. Finally, risk was calculated from the probability and consequence estimates obtained from our Burkina Faso participants, using a questionnaire based on the identified risks. Our “Cause More Harm” study yielded 46.15% negligible risk, 44.23% very low risk and 9.62% low risk. The “socio-cultural change” parameter had the greatest influence, with the perception that the dissemination project would be poorly received by the local population. This parameter alone accounted for 80% of the most significant risks. This explains the fact that hazard RA 49 “negative messages in social media” is ranked as the highest individual risk (although the risk is low) with a calculated risk of 0.261. The risk assessment was designed to integrate the interdependent complexity of hazards likely to affect the dissemination of technology in the environment. It represents an important implementation phase in the success of this innovative research, introducing a new technology to combat the transmission of dengue fever.展开更多
BACKGROUND Erythrodermic psoriasis(EP)is a rare and life-threatening form of psoriasis associated with significant morbidity and mortality.Systemic immunosuppre-ssive therapies are often required but may predispose to...BACKGROUND Erythrodermic psoriasis(EP)is a rare and life-threatening form of psoriasis associated with significant morbidity and mortality.Systemic immunosuppre-ssive therapies are often required but may predispose to opportunistic infections.Disseminated herpes simplex virus type-1(HSV-1)is an unusual complication in otherwise immunocompetent patients and has not been reported in association with ixekizumab therapy for EP.CASE SUMMARY We describe a 49-year-old man with longstanding severe plaque psoriasis,liver cirrhosis,and bipolar disorder who developed EP involving>90%of body surface area[Psoriasis Area and Severity Index(PASI)45].Following initial stabil-ization,he was admitted to the intensive care unit(ICU)with hemodynamic instability,leukocytosis with eosinophilia,and diffuse desquamation.Ixekizumab was initiated with high-dose topical clobetasol.During his ICU stay,he developed recurrent bacteremias and neurologic decline(Glasgow Coma Scale 7/15),fo-llowed by the appearance of widespread vesicles and hemorrhagic crusts.HSV-1 infection was confirmed by polymerase chain reaction(PCR).Immunosuppressive therapy was withheld,and intravenous acyclovir was started,leading to progre-ssive improvement.After ten days,ixekizumab was reintroduced with careful monitoring,resulting in marked clinical improvement(PASI 9.7 at six weeks).The patient remained stable on long-term follow-up with oral acyclovir prophylaxis.CONCLUSION This case highlights the diagnostic and therapeutic challenges of managing EP in the setting of biologic therapy.Disseminated cutaneous HSV-1 should be considered in immunosuppressed patients presenting with new vesicular eruptions,and prompt PCR testing with early antiviral therapy is essential.A multidisciplinary approach is critical to balance immunosuppression for disease control with infection risk.展开更多

BACKGROUND Histoplasmosis is a rare infectious condition with mainly pulmonary involvement which is generally self-limiting in immunocompetent individuals.Its manifestation varies and lacks specificity.This study repo...BACKGROUND Histoplasmosis is a rare infectious condition with mainly pulmonary involvement which is generally self-limiting in immunocompetent individuals.Its manifestation varies and lacks specificity.This study reports a case of primary disseminated liver histoplasmosis in a normal host presenting as liver failure cured by liver transplantation and voriconazole.CASE SUMMARY A 43-year-old Chinese man with intermittent fever,malaise,jaundice and extreme hepatomegaly for more than 40 days was admitted to the Second Xiang-ya Hospital.The patient was immunocompetent and lacked a definitive history of exposure.His condition deteriorated to liver failure,and he promptly underwent liver transplantation to ensure survival.One year later,the patient presented with severe gastrointestinal symptoms,including fever,abdominal pain,and diarrhea.Subsequently,tissue samples acquired via gastrointestinal endoscopy were subjected to pathological examination and next-generation sequencing analysis.Through a comprehensive amalgamation of clinical presentation,biopsy pathology,and next-generation sequencing analysis,the patient was ultimately diagnosed with disseminated hepatic histoplasmosis.The patient achieved complete recovery after 6 months of voriconazole treatment.CONCLUSION In patients with chronic-hepatitis-B having atypical symptoms,histoplasmosis can be a differential diagnosis.Voriconazole is effective in treating histoplasmosis.展开更多

Rationale:Melioidosis is a serious opportunistic infection caused by Burkholderia(B.)pseudomallei,primarily affecting immunocompromised individuals,particularly in endemic regions.Timely diagnosis and appropriate trea...Rationale:Melioidosis is a serious opportunistic infection caused by Burkholderia(B.)pseudomallei,primarily affecting immunocompromised individuals,particularly in endemic regions.Timely diagnosis and appropriate treatment are crucial to prevent fatal outcomes.Patient concerns:Case 1 was a 34-year-old male kidney transplant recipient who presented with a 15-day history of intermittent fever,accompanied by liver and spleen abscesses.Case 2 was a 37-year-old female kidney transplant recipient who presented with acute febrile illness and developed leucopenia.Blood cultures for both patients grew B.pseudomallei.Diagnosis:Both patients were diagnosed with melioidosis caused by B.pseudomallei,with the diagnosis confirmed through pus culture from the liver abscess in Case 1 and blood culture in Case 2.Interventions:Both patients were treated with an intensive regimen of meropenem(renal-adjusted doses),followed by a 3-month course of oral cotrimoxazole for eradication therapy.Outcomes:Case 1 experienced resolution of liver and spleen abscesses after 3 months of treatment and continued to recover well.In Case 2,blood cultures became sterile after 4 weeks,with no further complications observed.Lessons:Melioidosis should be suspected in immunocompromised patients,especially kidney transplant recipients,who present with unexplained fever and sepsis-like symptoms.Early diagnosis through aspiration of abscesses and prompt treatment are critical for preventing relapses and improving patient outcomes.展开更多
BACKGROUND Atopic dermatitis(AD),or eczema,is a chronic,pruritic inflammatory skin disease affecting children and adults.Socioeconomic status(SES)plays a significant role in developing AD.However,mixed evidence from a...BACKGROUND Atopic dermatitis(AD),or eczema,is a chronic,pruritic inflammatory skin disease affecting children and adults.Socioeconomic status(SES)plays a significant role in developing AD.However,mixed evidence from a previous study by Bajwa et al makes it difficult to determine the directionality of the association.There is a lite-rature gap in understanding the causal association between AD and socioeco-nomic factors.AIM To evaluate the impact of disparities in SES on pediatric AD populations.METHODS Based on the eligibility criteria,the literature review identified eight articles since July 2021,and a descriptive analysis was conducted using an Excel spreadsheet on key components collected from the identified studies.RESULTS Eight observational studies assessed SES in pediatric AD.Five observational studies showed mixed associations between AD and SES.Sub-analysis revealed that urban areas had a higher prevalence of AD,and four studies identified a positive association between parental education and AD in the pediatric popu-lation.Socioeconomic variables,such as residential areas and household income,significantly influence disease outcomes.CONCLUSION There is mixed association between pediatric AD and SES,with AD positively associated with parental education.There is critical need to evaluate global impact of SES variables on pediatric AD.展开更多

This study investigates the potential of Metarhizium pingshaense Met_S26, a fungus from Burkina Faso, for controlling both endophilic and exophilic Aedes mosquitoes. Traditional interventions are less effective against mosquitoes resting outdoors, and the ability of the fungus to be sexually transmitted could enhance its efficacy. Aedes mosquitoes were reared and exposed to various concentrations of Metarhizium pingshaense to assess its virulence. The fungus showed significant efficacy, with lethal times (LT80) ranging from 8.67 to 11.83 days depending on the concentration. Furthermore, bioassays demonstrated effective sexual auto-dissemination, with substantial transmission of fungal spores between infected and uninfected mosquitoes, leading to lower survival rates in uninfected mates. This study highlights Metarhizium pingshaense Met_S26 as a promising tool for controlling both indoor and outdoor Aedes populations, complementing existing vector control strategies such as the Sterile Insect Technique (SIT) and Wolbachia-based approaches. Future research should explore integrating this fungus with other control methods for a comprehensive vector management strategy.展开更多

In this paper, we propose a three-layer U_(1)A_(1)U_(1)-U_(2)A_(2)U_(2)-SIS coupled model to analyze the co-evolution process of official information dissemination, civil information dissemination and epidemic transmission,considering the interdependencies between the information dissemination channels. The first layer describes the official information dissemination process. The second layer models the civil information dissemination process, considering the effects of perceived risk costs and the role of the correlation between official and civil information. The third layer represents the epidemic transmission process, highlighting the impact of the correlation between official and civil information on epidemic transmission. Then, using the microscopic Markov chain approach, we describe the information-epidemic coupled dynamics and derive the epidemic outbreak threshold. Our research demonstrates that a stronger positive correlation between official and civil information raises the epidemic threshold and suppresses the scale of epidemic transmission. Furthermore, individuals' adoption of civil information should involve a more thorough assessment of the infection risk based on their personal circumstances, which can contribute to more effective epidemic control. Moreover, enhancing infected individuals' accurate comprehension of official information can effectively curb the transmission of the epidemic. Our study highlights the importance of both official and civil information dissemination in epidemic management and provides insights for policymakers in developing effective public health and communication strategies.展开更多

文摘BACKGROUND Acute disseminated encephalomyelitis(ADEM),which is rare,primarily affects children.It usually manifests as acute encephalopathy and multifocal neurological impairments after infection or vaccination.Diagnosis is still difficult due to the clinical and radiological similarity to other central nervous system disorders.Adult-onset ADEM calls for thorough reporting in order to improve diagnosis and treatment.CASE SUMMARY A 55-year-old man with hypertension had a high fever,intense headache and a steady decline in his neurological function after two weeks.Left facial paralysis was the initial symptom,which progressed to left hemiparesis,reduced consciousness level,photophobia,phonophobia,vomiting,and a focal seizure in the right leg.He had no history of autoimmune disease,vaccinations,or infections.Investigations showed negative infectious/autoimmune serology,mild cerebrospinal fluid lymphocytic pleocytosis(protein 76 mg/dL),and lymphopenia.Brain magnetic resonance imaging without contrast revealed bilateral,symmetrical T2/fluid-attenuated inversion recovery hyperintensities,primarily in the middle cerebellar peduncles,with minor involvement in the pontine and periventricular regions.Neoplastic,metabolic,vascular,and infectious conditions were not included.The patient showed spontaneous neurological improvement by Week 3 with near-complete motor recovery(limb strength 4/5)after methylprednisolone and rehabilitation,despite logistical delays in starting immunotherapy.The monophasic course and radiological/clinical remission were supported by idiopathic ADEM.CONCLUSION This case shows an uncommon,idiopathic,cerebellar-predominant ADEM variation in an adult without conventional triggers.It emphasizes the diagnostic difficulty in distinguishing ADEM from mimics(such as stroke or infection)in adults.Spontaneous improvement before treatment,although early detection is still crucial,should be highlighted,although early detection is still crucial.Increased clinician awareness,fair access to neuroimaging,and focused research on adult ADEM are crucial to fill these gaps and improve outcomes in places with limited resources.

文摘One of the most promising vector control strategies for controlling dengue fever is the stable introduction of the obligate intracellular Wolbachia bacteria strain into Aedes aegypti mosquitoes. This method reduces the mosquito’s ability to transmit dengue through reproductive strategies associated with Wolbachia infection, such as parthenogenesis, male elimination or feminization, sex ratio distortions and cytoplasmic incompatibility. Expert knowledge and a risk assessment framework were used to identify the risks associated with the release of Wolbachia-bearing mosquitoes. Then, consultations with individual participants were organized and a Bayesian network (BN) was developed to capture the relationship between the hazards as well as the likelihood of these adverse events occurring. Finally, risk was calculated from the probability and consequence estimates obtained from our Burkina Faso participants, using a questionnaire based on the identified risks. Our “Cause More Harm” study yielded 46.15% negligible risk, 44.23% very low risk and 9.62% low risk. The “socio-cultural change” parameter had the greatest influence, with the perception that the dissemination project would be poorly received by the local population. This parameter alone accounted for 80% of the most significant risks. This explains the fact that hazard RA 49 “negative messages in social media” is ranked as the highest individual risk (although the risk is low) with a calculated risk of 0.261. The risk assessment was designed to integrate the interdependent complexity of hazards likely to affect the dissemination of technology in the environment. It represents an important implementation phase in the success of this innovative research, introducing a new technology to combat the transmission of dengue fever.
文摘BACKGROUND Erythrodermic psoriasis(EP)is a rare and life-threatening form of psoriasis associated with significant morbidity and mortality.Systemic immunosuppre-ssive therapies are often required but may predispose to opportunistic infections.Disseminated herpes simplex virus type-1(HSV-1)is an unusual complication in otherwise immunocompetent patients and has not been reported in association with ixekizumab therapy for EP.CASE SUMMARY We describe a 49-year-old man with longstanding severe plaque psoriasis,liver cirrhosis,and bipolar disorder who developed EP involving>90%of body surface area[Psoriasis Area and Severity Index(PASI)45].Following initial stabil-ization,he was admitted to the intensive care unit(ICU)with hemodynamic instability,leukocytosis with eosinophilia,and diffuse desquamation.Ixekizumab was initiated with high-dose topical clobetasol.During his ICU stay,he developed recurrent bacteremias and neurologic decline(Glasgow Coma Scale 7/15),fo-llowed by the appearance of widespread vesicles and hemorrhagic crusts.HSV-1 infection was confirmed by polymerase chain reaction(PCR).Immunosuppressive therapy was withheld,and intravenous acyclovir was started,leading to progre-ssive improvement.After ten days,ixekizumab was reintroduced with careful monitoring,resulting in marked clinical improvement(PASI 9.7 at six weeks).The patient remained stable on long-term follow-up with oral acyclovir prophylaxis.CONCLUSION This case highlights the diagnostic and therapeutic challenges of managing EP in the setting of biologic therapy.Disseminated cutaneous HSV-1 should be considered in immunosuppressed patients presenting with new vesicular eruptions,and prompt PCR testing with early antiviral therapy is essential.A multidisciplinary approach is critical to balance immunosuppression for disease control with infection risk.

文摘BACKGROUND Histoplasmosis is a rare infectious condition with mainly pulmonary involvement which is generally self-limiting in immunocompetent individuals.Its manifestation varies and lacks specificity.This study reports a case of primary disseminated liver histoplasmosis in a normal host presenting as liver failure cured by liver transplantation and voriconazole.CASE SUMMARY A 43-year-old Chinese man with intermittent fever,malaise,jaundice and extreme hepatomegaly for more than 40 days was admitted to the Second Xiang-ya Hospital.The patient was immunocompetent and lacked a definitive history of exposure.His condition deteriorated to liver failure,and he promptly underwent liver transplantation to ensure survival.One year later,the patient presented with severe gastrointestinal symptoms,including fever,abdominal pain,and diarrhea.Subsequently,tissue samples acquired via gastrointestinal endoscopy were subjected to pathological examination and next-generation sequencing analysis.Through a comprehensive amalgamation of clinical presentation,biopsy pathology,and next-generation sequencing analysis,the patient was ultimately diagnosed with disseminated hepatic histoplasmosis.The patient achieved complete recovery after 6 months of voriconazole treatment.CONCLUSION In patients with chronic-hepatitis-B having atypical symptoms,histoplasmosis can be a differential diagnosis.Voriconazole is effective in treating histoplasmosis.

文摘Rationale:Melioidosis is a serious opportunistic infection caused by Burkholderia(B.)pseudomallei,primarily affecting immunocompromised individuals,particularly in endemic regions.Timely diagnosis and appropriate treatment are crucial to prevent fatal outcomes.Patient concerns:Case 1 was a 34-year-old male kidney transplant recipient who presented with a 15-day history of intermittent fever,accompanied by liver and spleen abscesses.Case 2 was a 37-year-old female kidney transplant recipient who presented with acute febrile illness and developed leucopenia.Blood cultures for both patients grew B.pseudomallei.Diagnosis:Both patients were diagnosed with melioidosis caused by B.pseudomallei,with the diagnosis confirmed through pus culture from the liver abscess in Case 1 and blood culture in Case 2.Interventions:Both patients were treated with an intensive regimen of meropenem(renal-adjusted doses),followed by a 3-month course of oral cotrimoxazole for eradication therapy.Outcomes:Case 1 experienced resolution of liver and spleen abscesses after 3 months of treatment and continued to recover well.In Case 2,blood cultures became sterile after 4 weeks,with no further complications observed.Lessons:Melioidosis should be suspected in immunocompromised patients,especially kidney transplant recipients,who present with unexplained fever and sepsis-like symptoms.Early diagnosis through aspiration of abscesses and prompt treatment are critical for preventing relapses and improving patient outcomes.
文摘BACKGROUND Atopic dermatitis(AD),or eczema,is a chronic,pruritic inflammatory skin disease affecting children and adults.Socioeconomic status(SES)plays a significant role in developing AD.However,mixed evidence from a previous study by Bajwa et al makes it difficult to determine the directionality of the association.There is a lite-rature gap in understanding the causal association between AD and socioeco-nomic factors.AIM To evaluate the impact of disparities in SES on pediatric AD populations.METHODS Based on the eligibility criteria,the literature review identified eight articles since July 2021,and a descriptive analysis was conducted using an Excel spreadsheet on key components collected from the identified studies.RESULTS Eight observational studies assessed SES in pediatric AD.Five observational studies showed mixed associations between AD and SES.Sub-analysis revealed that urban areas had a higher prevalence of AD,and four studies identified a positive association between parental education and AD in the pediatric popu-lation.Socioeconomic variables,such as residential areas and household income,significantly influence disease outcomes.CONCLUSION There is mixed association between pediatric AD and SES,with AD positively associated with parental education.There is critical need to evaluate global impact of SES variables on pediatric AD.
文摘In Burkina Faso, recent dengue outbreaks were exacerbated by Aedes aegypti mosquitoes, which have developed resistance to conventional insecticides. This study investigates the potential of Metarhizium pingshaense Met_S26, a fungus from Burkina Faso, for controlling both endophilic and exophilic Aedes mosquitoes. Traditional interventions are less effective against mosquitoes resting outdoors, and the ability of the fungus to be sexually transmitted could enhance its efficacy. Aedes mosquitoes were reared and exposed to various concentrations of Metarhizium pingshaense to assess its virulence. The fungus showed significant efficacy, with lethal times (LT80) ranging from 8.67 to 11.83 days depending on the concentration. Furthermore, bioassays demonstrated effective sexual auto-dissemination, with substantial transmission of fungal spores between infected and uninfected mosquitoes, leading to lower survival rates in uninfected mates. This study highlights Metarhizium pingshaense Met_S26 as a promising tool for controlling both indoor and outdoor Aedes populations, complementing existing vector control strategies such as the Sterile Insect Technique (SIT) and Wolbachia-based approaches. Future research should explore integrating this fungus with other control methods for a comprehensive vector management strategy.

文摘Official and civil information, as distinct information sources, significantly influence public behavior and the dynamics of epidemic transmission. In this paper, we propose a three-layer U_(1)A_(1)U_(1)-U_(2)A_(2)U_(2)-SIS coupled model to analyze the co-evolution process of official information dissemination, civil information dissemination and epidemic transmission,considering the interdependencies between the information dissemination channels. The first layer describes the official information dissemination process. The second layer models the civil information dissemination process, considering the effects of perceived risk costs and the role of the correlation between official and civil information. The third layer represents the epidemic transmission process, highlighting the impact of the correlation between official and civil information on epidemic transmission. Then, using the microscopic Markov chain approach, we describe the information-epidemic coupled dynamics and derive the epidemic outbreak threshold. Our research demonstrates that a stronger positive correlation between official and civil information raises the epidemic threshold and suppresses the scale of epidemic transmission. Furthermore, individuals' adoption of civil information should involve a more thorough assessment of the infection risk based on their personal circumstances, which can contribute to more effective epidemic control. Moreover, enhancing infected individuals' accurate comprehension of official information can effectively curb the transmission of the epidemic. Our study highlights the importance of both official and civil information dissemination in epidemic management and provides insights for policymakers in developing effective public health and communication strategies.
