Background:The yin deficiency type of perimenopausal syndrome(PMS)as a common category of PMS based on the theory of traditional Chinese medicine(TCM)has a high prevalence with severe symptoms and long course of disease. Therefore,it is necessary to construct a prediction model to assist in diagnosis. Objective:This study aimed to investigate the independent predictors of the yin deficiency type of PMS and to develop a clinical prediction model of this disease. Methods:PMS patients who attended the Third Affiliated Hospital of Zhejiang Chinese Medical University between February 2020 and August 2023 were selected and divided chronologically into training and validation groups.Logistic regression analysis was applied in the training group to clarify the independent predictors of the yin deficiency type of PMS,and a nomogram was plotted.Internal and external validations were performed in the training and validation groups to evaluate the model’s accuracy, goodness of fit,and clinical adaptability. Results:Hot flashes and sweating(≥10 episodes/day),palpitations,emotional fluctuations, and abnormal sexual activity were independent predictors of the yin deficiency type of PMS(P>0.05).Based on the clinical prediction model constructed, the area under the receiver operating characteristic curve(AUR OC)in the training group was 0.989(95%CI 0.980–0.998),and the AUR OC in the validation group was 0.971(95%CI 0.940–0.999).This demonstrates that the model has superior prediction performance. The Hosmer-Lemeshow test was used to evaluate the model’s goodness of fit with P=0.596 for the training group and P=0.883 for the validation group, indicating a good fit.The decision curve analysis(DCA)curve and clinical impact curve(CIC)indicated good clinical adaptability. Conclusion:The model can accurately predict the occurrence of the yin deficiency type of PMS,which may help clinicians identify such patients at an early stage. 展开更多

BACKGROUND Gastroparesis is a common digestive disorder characterized by delayed gastric emptying, which can lead to symptoms such as nausea, vomiting, abdominal pain, and poor appetite.Traditional Chinese medicine(TCM)has been used for centuries to treat gastrointestinal disorders, including gastroparesis.TCM theory suggests that spleen and stomach qi deficiency syndrome is one of the main pathogenic factors in gastroparesis.Nursing care plays an important role in the treatment of gastroparesis, and TCM nursing interventions have shown promising results in improving patient outcomes. However, there is limited research on the clinical effectiveness of TCM nursing interventions for gastroparesis with spleen stomach deficiency syndrome. This study aimed to evaluate the clinical effect of TCM nursing intervention in the treatment of gastroparesis with spleen stomach deficiency syndrome and to compare it with routine nursing interventions.AIM To analyze the clinical effect of traditional Chinese medicine nursing intervention in the treatment of gastric paraplegia with spleen stomach deficiency syndrome.METHODS From January 2020 to July 2021,80 patients with gastroparesis of spleen stomach qi deficiency type diagnosed in our hospital were selected for the study. The 80 patients were randomly divided into a control group and an experimental group, with 40 cases in each group.During the treatment period, the control group received routine nursing interventions, while the experimental group received traditional Chinese medicine nursing procedures.Compare the nursing effects of the two groups and observe the changes in traditional Chinese medicine symptom scores, pain levels, and sleep quality before and after treatment.RESULTS After treatment, comparing the treatment effects of the two groups, the total effective rate of the experimental group was significantly higher than that of the control group, with statistical significance(P<0.05).There was no statistically significant difference in the TCM symptom score, visual analogue scale(VAS)score, and Pittsburgh sleep quality index(PSQI)score between the two groups before treatment(P>0.05).However,after treatment, the TCM syndrome scores,VAS scores, and PSQI scores of the experimental group were significantly lower than those of the control group, with a statistically significant difference(P<0.05).CONCLUSION In the clinical nursing intervention of patients with mild gastroparesis due to spleen and stomach qi deficiency, the traditional Chinese medicine nursing plan has good clinical application value and nursing effect, and has a good effect on improving patients’pain and sleep quality. 展开更多

BACKGROUND Polycystic ovary syndrome(PCOS) is characterized by hyperandrogenism, hyperinsulinemia, ovarian polycystic changes, and irregular ovulation, often occurring in women of childbearing age for whom it can be a cause of infertility. Hypothalamus-pituitary-ovarian axis dysregulation is important in the pathogenesis of PCOS and the associated chronic excess of sex hormones can lead to cardiovascular and cerebrovascular diseases, diabetes, and malignancies such as endometrial cancer, and breast cancer. At present, most scholars agree that lifestyle interventions in conjunction with drug treatment can help PCOS patients achieve their goals of successful pregnancy and childbirth.AIM To investigate the clinical effect of an online and offline(O2O) preventive health management model on PCOS with kidney deficiency and phlegm dampness.METHODS A total of 82 patients with PCOS of the kidney deficiency and phlegm dampness type who were admitted to Beijing Luhe Hospital Affiliated to Capital Medical University from April 2019 to June 2020 were randomly divided into two groups. The treatment group was treated with oral Diane-35 for 3 mo and received preventive O2O medical health management for 6 mo(including eating and living, exercise, drug management). The control group was treated with oral Diane-35 for 3 mo and completed outpatient health education. The traditional Chinese medicine(TCM) syndrome score, acne score, hair score, sex hormone level and clinical effects were compared between the two groups before and after the intervention. RESULTS After treatment, the TCM syndrome score, acne score, and serum luteinizing hormone/follicle stimulating hormone level were significantly lower in the treatment group than in the control group(P < 0.05). After 3 mo of treatment, the TCM syndrome curative effect index in the treatment group was 97.30% compared to 54.05% in the control group(P < 0.05), whereas the total treatment effect in the treatment group was 91.89%, compared to 54.05% in the control group(P < 0.05).CONCLUSION An integrated therapeutic approach incorporating medication, TCM methods and social media is more effective than standard treatment for PCOS. 展开更多

With the popularization of clinical ultrasound technology, small follicular ovulation infertility is gradually being recognized in clinical practice.Low follicle ovulation refers to the phenomenon that the average value of the three diameter lines of the dominant follicle is less than 14mm-18mm under continuous ultrasound monitoring. The dominant follicle ovulates before it develops to maturity, which is a kind of ovulation disorder and one of the leading causes of unexplained infertility[1],accounting for 25%-30%of the total number of patients with infertility[2].At present,Western medicine mostly uses ovulation, stimulating drugs to treat this disease.Still, due to the clinical result of“high ovulation but low pregnancy”,there are certain obstacles in treatment. The purpose of this article is to discuss Zhang Xiaofeng’s understanding of TCM of this disease. The tutor believes that the location of the disease is in the kidney, and the mechanism of the disease is kidney yin deficiency and blood heat. The treatment principle is sequential therapy, nourishing yin and clearing heat, taking into account the protection of the spleen and stomach, and the therapeutic effect is perfect. 展开更多
