Heart failure (HF), a complex clinical syndrome due to structural or functional disorder of the heart, is a major global health issue, with a prevalence of over 5.8 million in the USA alone, and over 23 million worl...Heart failure (HF), a complex clinical syndrome due to structural or functional disorder of the heart, is a major global health issue, with a prevalence of over 5.8 million in the USA alone, and over 23 million worldwide. As a leading cause of hospitalizations among patients aged 65 years or older, HF is a major consumer of healthcare resources, creating a substantial strain on the healthcare system. This paper discusses the epidemiology of HF, financial impact, and multifaceted predicaments in end-stage HF care. A search was conducted on the U.S National Library of Medicine website (www.pubmed.gov) using keywords such as end-stage heart failure, palliative care, ethical dilemmas. Despite the poor prognosis of HF (worse than that for many cancers), many HF patients, caregivers, and clinicians are unaware of the poor prognosis. In addition, the unpredictable clinical trajectory of HF complicates the planning of end-of-life care, such as palliative care and hospice, leading to underutilization of such resources. In conclusion, ethical dilemmas in end-stage HF are numerous, embroiling not only the patient, but also the caregiver, healthcare team, and society.展开更多

Methods: We approached nursing staff supervisors at 17 general hospital wards and explained the objectives of the study. Supervisors at 14 hospitals agreed to participate, giving us a sampling pool of 1929 nurses. We used a chi square test to compare nurses who had spent time with elderly relatives and those who had not on several variables related to using restraints on elderly patients. Results and Conclusion: This study found that nurses who live with elderly family members were significantly more likely to believe that restraints cause diseases (including chronic diseases) based on a chi square test (p < 0.05). The study used the questionnaire method with an in...This study aimed to clarify the dilemma of nurses working in general wards who face the ethical dilemma of restraining older people with dementia in Midwestern Japan. The study used the questionnaire method with an independently prepared questionnaire. The questionnaire was designed to ensure privacy and anonymity. The privacy and anonymity of study participants was assured. Of the 340 survey questionnaires distributed, 291 (85.6%) completed surveys were eligible number (12 additional surveys were returned without answers). An explanatory factor analysis revealed four dilemma factors among 15 items investigated (KMO value 0.84). These were: factor 1: “Execution of treatment and security,” α = 0.91;factor 2: “approach in the nursing of older people with dementia,” α = 0.93;factor 3: “A cooperative relationship in nursing of older people with dementia,” α = 0.87;and factor 4: “Priorities in nursing,” α = 0.81. The cumulative contribution ratio was 79.1%. We suggest that the program would enable nurses to cope with these dilemmas.展开更多
Revolutions,typical cases of crucial social transformations,cannot be realized successfully without a large number of activists.Therefore,creating conditions favorable for acquiring enough participants should be an im...Revolutions,typical cases of crucial social transformations,cannot be realized successfully without a large number of activists.Therefore,creating conditions favorable for acquiring enough participants should be an important topic of Marxist social science.In particular,this problem includes the “free-ride,” because the benefits of revolutionaries’ activities are gained not only by the activists but also by all other members.The paper analyzes problems such as this one,applying non-cooperative game theory to social dilemma problems.This leads to some interesting results.In this research,the problem of the workers’ choice between unity or freeride is first defined using numerical examples of the gain structure.It is defined again in a more generalized form using other parameters.In so doing,we express both the cost of participating in the movement and the gains from the concession of the ruling class.Because this analysis focuses on the importance of the number of participants,the concession of the ruling class is framed as a function of the number of participants.The results of this analysis revealed that the economic base and superstructure accurately correspond in some game structures but not in others.In other words,the social dilemma presents either as a case of prisoners’ dilemma or as a chicken game.Furthermore,this paper analyzes the influence of group size,and it was revealed that groups with a large number of members,such as a ruled class,find it particularly difficult to unite.This phenomenon is called the “large group dilemma.” In these ways,this research shows that the aforementioned type of game theory can be used to analyze the difficulties and possibilities of social movements.展开更多

Large-scale simulations indicate that, compared to the model with only one update rule, the the role of update dynamics should be paid more attention in cooperation behavior exhibits the richer phenomena, and the evolutionary game theory. Meanwhile, we also observe that the introduction of Moran rule, which needs to consider all neighbor's information, can markedly promote the aggregate cooperation level, that is, randomly selecting the neighbor proportional to its payoff to imitate will facilitate the cooperation among agents. Current results will contribute to further understand the cooperation dynamics and evolutionary behaviors within many biological, economic and social systems.展开更多

文摘This study aimed to clarify the dilemma of nurses working in general wards who face the ethical dilemma of restraining older people with dementia in Midwestern Japan. The study used the questionnaire method with an independently prepared questionnaire. The questionnaire was designed to ensure privacy and anonymity. The privacy and anonymity of study participants was assured. Of the 340 survey questionnaires distributed, 291 (85.6%) completed surveys were eligible number (12 additional surveys were returned without answers). An explanatory factor analysis revealed four dilemma factors among 15 items investigated (KMO value 0.84). These were: factor 1: “Execution of treatment and security,” α = 0.91;factor 2: “approach in the nursing of older people with dementia,” α = 0.93;factor 3: “A cooperative relationship in nursing of older people with dementia,” α = 0.87;and factor 4: “Priorities in nursing,” α = 0.81. The cumulative contribution ratio was 79.1%. We suggest that the program would enable nurses to cope with these dilemmas.
文摘Revolutions,typical cases of crucial social transformations,cannot be realized successfully without a large number of activists.Therefore,creating conditions favorable for acquiring enough participants should be an important topic of Marxist social science.In particular,this problem includes the “free-ride,” because the benefits of revolutionaries’ activities are gained not only by the activists but also by all other members.The paper analyzes problems such as this one,applying non-cooperative game theory to social dilemma problems.This leads to some interesting results.In this research,the problem of the workers’ choice between unity or freeride is first defined using numerical examples of the gain structure.It is defined again in a more generalized form using other parameters.In so doing,we express both the cost of participating in the movement and the gains from the concession of the ruling class.Because this analysis focuses on the importance of the number of participants,the concession of the ruling class is framed as a function of the number of participants.The results of this analysis revealed that the economic base and superstructure accurately correspond in some game structures but not in others.In other words,the social dilemma presents either as a case of prisoners’ dilemma or as a chicken game.Furthermore,this paper analyzes the influence of group size,and it was revealed that groups with a large number of members,such as a ruled class,find it particularly difficult to unite.This phenomenon is called the “large group dilemma.” In these ways,this research shows that the aforementioned type of game theory can be used to analyze the difficulties and possibilities of social movements.

文摘We combine the Fermi and Moran update rules in the spatial prisoner's dilemma and snowdrift games to investigate the behavior of collective cooperation among agents on the regular lattice. Large-scale simulations indicate that, compared to the model with only one update rule, the the role of update dynamics should be paid more attention in cooperation behavior exhibits the richer phenomena, and the evolutionary game theory. Meanwhile, we also observe that the introduction of Moran rule, which needs to consider all neighbor's information, can markedly promote the aggregate cooperation level, that is, randomly selecting the neighbor proportional to its payoff to imitate will facilitate the cooperation among agents. Current results will contribute to further understand the cooperation dynamics and evolutionary behaviors within many biological, economic and social systems.

Therefore, the identification of good ethical order, to call college cultural consciousness, lifting universities utilitarian development difficulties, reasonably determine the role of university social services, the ability to face the limits of educational services, social services, colleges and universities to guide ethical behavior.
