BACKGROUND As lifestyles continue to change worldwide,the incidence of digestive tract carcinoma has gradually increased.Digestive endoscopy is an important tool that can assist in the diagnosis,treatment,and surgical...BACKGROUND As lifestyles continue to change worldwide,the incidence of digestive tract carcinoma has gradually increased.Digestive endoscopy is an important tool that can assist in the diagnosis,treatment,and surgical intervention for this disease.However,the examination process is affected by many factors,and patient cooperation is often poor,which can increase the risk of complications.AIM To explore the effects of integrated management and cognitive intervention on cooperation and complications in patients undergoing endoscopy for early gastrointestinal neoplasms.METHODS A total of 354 patients with early stage gastrointestinal cancer who underwent digestive endoscopy procedures between January and December 2023 at our hospital were divided into observation and control groups(177 patients in each group)in a randomized controlled blind trial.The control group received routine interventions,while the observation group received comprehensive integrated management combined with cognitive interventions.We compared the changes in adverse mood,discomfort,examination time,cooperation with the examination,and complications before and after the intervention between the two groups.RESULTS The self-rated anxiety and depression scale scores were lower in the observation group than in the control group(P<0.05).The visual analog scale scores for discomfort during intubation and examination were also lower in the observation group than in the control group(P<0.05).Furthermore,the examination time was shorter in the observation group than in the control group(P<0.05),and the degree of cooperation(94.35%)was higher in the observation group than in the control group(84.75%;P<0.05).Lastly,the incidence rates of gastrointestinal adverse reactions(10.17%vs 20.34%),choking agitation(14.69%vs 24.86%),abdominal pain(8.47%vs 18.08%),and muscle tension(5.08%vs 14.12%)were all lower in the observation group than in the control group(P<0.05).CONCLUSION Integrated management and cognitive intervention in early gastrointestinal neoplasm endoscopy alleviate mood,reduce discomfort,shorten examinations,improve cooperation,and reduce complications.展开更多
Alzheimer's disease(AD)is the most common cognitive disorder in the elderly.Its main clinical manifestations are cognitive decline(C),behavioral and psychological symptoms(B),and a decline in the activities of dai...Alzheimer's disease(AD)is the most common cognitive disorder in the elderly.Its main clinical manifestations are cognitive decline(C),behavioral and psychological symptoms(B),and a decline in the activities of daily living(A),also known as ABC symptoms.Early identification and evaluation of ABC symptoms are helpful for establishing the accurate diagnosis,comprehensive treatment,and prognosis of AD.To guide Chinese clinical practice for optimization of the comprehensive management of AD,in 2018,The Academy of Cognitive Disorder of China gathered 22 neurologists and gerontologists in China to build a consensus on the comprehensive management of AD.Based on a review of the evidence,the consensus summarizes the pathogenesis,pathological changes,clinical manifestations,evaluation,diagnosis,drug and non-drug treatment,and patient care for AD.Focus group discussion was used to establish a flowchart of comprehensive ABC management for AD patients.The new consensus provides a feasible AD management process for clinicians.展开更多

Background:Onchocerciasis-endem ic regions are known to have a high epilepsy prevalence.Limited resources in these areas and poor access to healthcare by persons with epilepsy(PWE)result in a wide anti-epileptic treat...Background:Onchocerciasis-endem ic regions are known to have a high epilepsy prevalence.Limited resources in these areas and poor access to healthcare by persons with epilepsy(PWE)result in a wide anti-epileptic treatment gap,poor seizure control and a high burden of seizure-related complications.Recent comm unity-based surveys highlight the need for epilepsy managem ent strategies suitable for remote onchocerciasis-endemic villages to ensure better health outcom es for PWE.In this paper,we propose a feasible approach to manage PWE in such settings.Main text:Improved management of PWE in onchocerciasis-endemic areas may be achieved by decentralizing epilepsy care.Simplified approaches for the diagnosis and treatment of epilepsy may be used by non-physicians,under the supervision of physicians or specialists.To reduce the treatment gap,a regular supply of subsidized anti-epileptic drugs(AED)appropriate for different types of onchocerciasis-associated epilepsy should be instituted.Setting up a communitybased epilepsy surveillance system will enable early diagnosis and treatment of PWE thereby preventing complications.Community awareness programs on epilepsy must be implemented to reduce stigma and facilitate the social rehabilitation of PWE.Finally,strengthening onchocerciasis elimination programs by optimizing community-directed treatment with iverm eain(CDTI)and considering alternative treatment strategies might reduce the incidence of epilepsy.Conclusions:A community-based approach with task-shifting of epilepsy care from specialists to non-physician health workers will reduce epilepsy-associated morbidity.Increased advocacy and collaboration with various stakeholders is needed to establish a sustainable,cost-effective chronic care model for epilepsy that will significantly improve the quality of life of PWE in onchocerciasis-endemic regions.展开更多

Methods: 60 patients with gesta...Objective: to study and analyze the effects of perinatal comprehensive management on glucose and lipid metabolism and maternal and infant outcomes in patients with gestational diabetes. Methods: 60 patients with gestational diabetes admitted to our hospital from February 2020 to may 2022 were included. They were divided into the observation group and the control group (n 30) by double-blind random method. The control group carried out routine treatment of gestational diabetes, and the observation group carried out comprehensive perinatal management on the basis of routine treatment. Statistics and comparison were made on insulin related indicators, glucose and lipid metabolism levels, and maternal and infant outcomes between the two groups. Results: around 24 hours after delivery, the insulin resistance index of the observation group was lower than that of the control group. After delivery, the fasting blood glucose, glycosylated hemoglobin, total cholesterol and other glycolipid metabolism indexes of the observation group were lower than that of the control group. The incidence of adverse maternal and infant outcomes was lower than that of the control group, and the difference was statistically significant (P 0.05). Conclusion: the application of perinatal comprehensive management in patients with gestational diabetes is helpful to improve insulin resistance index, glucose and lipid metabolism and maternal and infant outcomes.展开更多

Methods: 120 breast cancer patients treated by dual-targeted therapy with macromolecular monoclonal antibodies were managed by a cloud platform from March to November 2019. Comprehensive online management included consultation about drugs and side effects and frequently asked questions in the dual-targeted therapy with macromolecular monoclonal antibodies. Results: In the consultation about drugs and side effects, there were five patients with fever, neutrophil, cough, and fatigue;24 with diarrhea;25 with nausea;11 with oral mucosal inflammation;10 with rashes and dry skin;8 with insomnia;and 1 with palpitation. Moreover, 110 patients with anxiety about the missed or delayed treatment were properly handled. Conclusion: The comprehensive online management of dual-targeted therapy with macromolecular monoclonal antibodies based on the cloud platform is helpful to satisfy the at-home breast cancer patients’ needs, ensure the continuity of dual-targeted therapy with macromolecular monoclonal antibodies for breast cancer patients, prevent misinformation, alleviate patients’ negative psychological emotions, and reduce patients’ economic losses. Methods: a total of 40 experimental subjects were included in the study. All subjects were patients with severe tumor infection in the intensive care unit of our hospital. The experimental period is from February 2019 to February 2021. The test subjects were randomly divided into experimental group (n = 20) and reference group (n = 20). The reference group received routine nursing while the experimental group received comprehensive nursing. Evaluate the nursing effect of the two groups and compare the related satisfaction index. Results: after nursing, the related indexes of the experimental group were significantly better than those of the reference group, and the satisfaction degree of the experimental group was higher than that of the reference group (60.00)(95.00), the difference was statistically significant (P < 0.05). Conclusion: the application of comprehensive nursing management in the clinical nursing of patients with severe tumor infection in the intensive care unit is helpful to improve the nursing satisfaction and the related indicators of patient nursing effect evaluation.展开更多

文摘Objective: to study and analyze the effects of perinatal comprehensive management on glucose and lipid metabolism and maternal and infant outcomes in patients with gestational diabetes. Methods: 60 patients with gestational diabetes admitted to our hospital from February 2020 to may 2022 were included. They were divided into the observation group and the control group (n 30) by double-blind random method. The control group carried out routine treatment of gestational diabetes, and the observation group carried out comprehensive perinatal management on the basis of routine treatment. Statistics and comparison were made on insulin related indicators, glucose and lipid metabolism levels, and maternal and infant outcomes between the two groups. Results: around 24 hours after delivery, the insulin resistance index of the observation group was lower than that of the control group. After delivery, the fasting blood glucose, glycosylated hemoglobin, total cholesterol and other glycolipid metabolism indexes of the observation group were lower than that of the control group. The incidence of adverse maternal and infant outcomes was lower than that of the control group, and the difference was statistically significant (P 0.05). Conclusion: the application of perinatal comprehensive management in patients with gestational diabetes is helpful to improve insulin resistance index, glucose and lipid metabolism and maternal and infant outcomes.
文摘Objective: The purpose of the study was to explore the application effects of the cloud platform-based comprehensive online management for breast cancer patients using dual-targeted therapy with macromolecular monoclonal antibodies. Methods: 120 breast cancer patients treated by dual-targeted therapy with macromolecular monoclonal antibodies were managed by a cloud platform from March to November 2019. Comprehensive online management included consultation about drugs and side effects and frequently asked questions in the dual-targeted therapy with macromolecular monoclonal antibodies. Results: In the consultation about drugs and side effects, there were five patients with fever, neutrophil, cough, and fatigue;24 with diarrhea;25 with nausea;11 with oral mucosal inflammation;10 with rashes and dry skin;8 with insomnia;and 1 with palpitation. Moreover, 110 patients with anxiety about the missed or delayed treatment were properly handled. Conclusion: The comprehensive online management of dual-targeted therapy with macromolecular monoclonal antibodies based on the cloud platform is helpful to satisfy the at-home breast cancer patients’ needs, ensure the continuity of dual-targeted therapy with macromolecular monoclonal antibodies for breast cancer patients, prevent misinformation, alleviate patients’ negative psychological emotions, and reduce patients’ economic losses. The online cloud platform integrated management model is crucial for managing patients with breast cancer treated by dual-targeted therapy.

文摘Objective: to explore the effectiveness of comprehensive nursing for patients with tumor infection in intensive care unit. Methods: a total of 40 experimental subjects were included in the study. All subjects were patients with severe tumor infection in the intensive care unit of our hospital. The experimental period is from February 2019 to February 2021. The test subjects were randomly divided into experimental group (n = 20) and reference group (n = 20). The reference group received routine nursing while the experimental group received comprehensive nursing. Evaluate the nursing effect of the two groups and compare the related satisfaction index. Results: after nursing, the related indexes of the experimental group were significantly better than those of the reference group, and the satisfaction degree of the experimental group was higher than that of the reference group (60.00)(95.00), the difference was statistically significant (P < 0.05). Conclusion: the application of comprehensive nursing management in the clinical nursing of patients with severe tumor infection in the intensive care unit is helpful to improve the nursing satisfaction and the related indicators of patient nursing effect evaluation.

Methods: a total of 40 neonates with digestive system diseases were studied. The patients were divided into two groups. Conventional nursing and comprehensive nursing management were applied to compare the nursing effects. Results: the patients in the research group were better than those in the reference group in terms of hemodynamics index, complication rate, cure rate, operation index and nursing satisfaction, P<0.05. Conclusion: the application of comprehensive nursing management in the surgical treatment of neonates with digestive system diseases can stabilize the hemodynamic indexes, prevent complications, shorten the treatment time, and promote recovery, which has been evaluated satisfactorily by the families of the infants.
