BACKGROUND Lisfranc injuries have not received much attention by orthopedic doctors in the past,and there is little related research on the diagnosis and treatment of these injuries.In recent years with the rise in foot and ankle surgery,doctors are now paying more attention to this type of injury.However,there is still a high rate of missed diagnosis due to insufficient attention causing treatment delays or inadequate treatments,which eventually result in greater sequelae;including long-term pain,arthritis,foot deformity etc.In particular,for cases with a mild Lisfranc joint complex injury,the incidence of sequelae is higher.AIM To select an active surgical treatment for an atypical Lisfranc joint complex injury and to evaluate the clinical efficacy of the surgical treatment.METHODS The clinical data of 18 patients,including 10 males and 8 females aged 20-64 years with Lisfranc injuries treated in our department from January 2017 to September 2019 were retrospectively analyzed.All patients were treated with an open reduction and internal fixation method using locking titanium mini-plates and hollow screws or Kirschner wires.X-ray images were taken and follow-up was performed monthly after the operation;the internal fixation was then removed 4-5 mo after the operation;and the American Orthopedic Foot and Ankle Society(AOFAS)score was used for evaluation on the last follow-up.RESULTS All patients were followed up for 6-12 mo.A good/excellent AOFAS score was observed in 88.9%of patients.CONCLUSION For atypical Lisfranc joint complex injuries,active open reduction and internal fixation can be performed to enable patients to obtain a good prognosis and satisfactory functional recovery. 展开更多

In millimeter wave(mmWave) multiple-input multiple-output(MIMO) systems, hybrid precoding has been widely used to overcome the severe propagation loss. In order to improve the spectrum efficiency with low complexity, we propose a joint hybrid precoding algorithm for single-user mmWave MIMO systems in this paper. By using the concept of equivalent channel, the proposed algorithm skillfully utilizes the idea of alternating optimization to complete the design of RF precoder and combiner. Then, the baseband precoder and combiner are computed by calculating the singular value decomposition of the equivalent channel. Simulation results demonstrate that the proposed algorithm can achieve satisfactory performance with quite low complexity. Moreover, we investigate the effects of quantization on the analog components and find that the proposed scheme is effective even with coarse quantization. 展开更多

Background: Acute dislocation of the thumb carpometacarpal (CMC) joint is a rare and challenging injury, and its treatment remains controversial. The purpose of the study was to evaluate the clinical outcome after surgical repair of the dorsal capsulo-ligamentous complex. Materials and methods: Eight patients with a mean age of 39.8 years (range, 23 - 56 years) treated for the thumb CMC joint were treated in our hospital between 2008 and 2018. The ruptured dorsal capsulo-ligamentous complex was repaired using suture anchors in all patients, and the joint was immobilized with a Kirschner wire pinning and splint for 3 weeks. The clinical outcome was assessed by measuring the range of motion of the thumb CMC joint and the Japanese Society for Surgery of the Hand version of the Quick Disability of the Arm, Shoulder, and Hand (Q-DASH-JSSH) score. Results: The mean radial abduction of the thumb CMC joint was 66°, and the volar abduction was 63°. The mean Q-DASH-JSSH score was 4.8 (range, 0 - 15.6). There were no cases of infection, nerve disturbance, or osteoarthritis. Anatomical reduction was observed in seven patients at the final follow-up and in a rugby football player at 6 months of follow-up;however, subluxation of the thumb CMC joint at 12 months of follow-up due to reinjury was observed. Conclusion: Surgical repair of the dorsal capsulo-ligamentous complex with suture anchors is a reliable and simple treatment for acute unstable dislocation of the thumb CMC joint. 展开更多

Analysis of the stress intensity factor of welded joints under prior corrosion and complex stress fields

Taking cruciform welded joints under different corrosion degrees, together with 45° inclined angle and full penetration load-carrying fillet, as the research object, the stress intensity factor of a quarter-circular comer crack of welded joints is calculated based on FRANC3D with ABAQUS. Effects of different corrosion pit sizes, crack aspect ratios, and crack depths on stress intensity factor are analyzed. The results show that pit depth plays a major role in stress intensity factor, while the effect of pit radius is relatively small. The cracking modes of the surface and the deepest point are mode I, and mixed modes I and II, respectively. Effects of pit depths, crack aspect ratios, and crack depths on the stress intensity factor at the surface point are greater than at the deepest point. 展开更多

In view of the low performance of adaptive asymmetric joint diagonalization(AAJD), especially its failure in tracking high maneuvering targets, an adaptive asymmetric joint diagonalization with deflation(AAJDd) algorithm is proposed. The AAJDd algorithm improves performance by estimating the direction of departure(DOD) and direction of arrival(DOA) directly, avoiding the reuse of the previous moment information in the AAJD algorithm.On this basis, the idea of sequential estimation of the principal component is introduced to turn the matrix operation into a constant operation, reducing the amount of computation and speeding up the convergence. Meanwhile, the eigenvalue is obtained, which can be used to estimate the number of targets. Then, the estimation of signal parameters via rotational invariance technique(ESPRIT) algorithm is improved to realize the automatic matching and association of DOD and DOA. The simulation results show that the AAJDd algorithm has higher tracking performance than the AAJD algorithm, especially when the high maneuvering target is tracked. A group tracking algorithm for split maneuvering based on complex domain topological descriptions is proposed for the tracking of members in a maneuvering group. According to the split characteristics of a group target, split models of group targets are established based on a sliding window feedback mechanism to determine the occurrence and classification of split maneuvering, which makes the tracked objects focus by group members effectively. The track of an outlier single target is reconstructed by the sequential least square method. At the same time, the relationship between the group members is expressed by the complex domain topological description method, which solves the problem of point-track association between the members. The Singer method is then used to update the tracks. Compared with classical multi-target tracking algorithms based on Multiple Hypothesis Tracking (MHT) and the Different Structure Joint Probabilistic Data Association (DS-JPDA) algorithm, the proposed algorithm has better tracking accuracy and stability, is robust against environmental clutter and has stable time-consumption under both classical radar conditions and partly resolvable conditions. 展开更多

目的探讨腕关节镜Wafer术联合三角纤维软骨复合体(triangular fibrocartilage complex,TFCC)止点重建治疗Palmer ⅡC合并ⅠB型尺骨撞击综合征的疗效。方法回顾分析2021年7月—2024年4月收治且符合选择标准的14例Palmer ⅡC合并ⅠB型尺... 目的探讨腕关节镜Wafer术联合三角纤维软骨复合体(triangular fibrocartilage complex,TFCC)止点重建治疗Palmer ⅡC合并ⅠB型尺骨撞击综合征的疗效。方法回顾分析2021年7月—2024年4月收治且符合选择标准的14例Palmer ⅡC合并ⅠB型尺骨撞击综合征患者临床资料。其中男7例,女7例;年龄16~59岁,平均43岁。致伤原因:摔伤8例,扭伤6例。受伤至手术时间1~6个月,平均2.3个月。均存在远侧桡尺关节不稳。采用腕关节镜Wafer术联合TFCC止点重建治疗。通过比较手术前后腕掌屈-背伸活动度、腕尺偏-桡偏活动度、前臂旋前-旋后活动度、疼痛视觉模拟评分(VAS)及改良Mayo腕关节评分等指标评价手术疗效。结果14例患者术后均获随访,随访时间6~12个月,平均9.1个月。患者术前尺骨正变异(3.2±0.7)mm,术后尺骨负变异(2.2±0.6)mm,手术前后尺骨变异比较差异有统计学意义(t=23.851,P<0.001)。患者术后疼痛症状及前臂旋转功能较术前明显好转,末次随访时腕掌屈-背伸活动度、腕尺偏-桡偏活动度、前臂旋前-旋后活动度及VAS评分、改良Mayo腕关节评分均较术前显著改善,差异有统计学意义(P<0.05)。结论腕关节镜Wafer术联合TFCC止点重建治疗Palmer ⅡC合并ⅠB型尺骨撞击综合征,可有效缓解患者腕部疼痛、增强下尺桡关节稳定性和恢复腕关节功能。 展开更多

针对工业机器人的柔性关节表现的复杂迟滞特性问题,提出了一种基于改进的iTransformer(inverted transformer)模型的非线性迟滞建模方法,用于描述柔性关节的输出力矩与扭转角之间的复杂迟滞特性。采用具有输入反转嵌入层的iTransformer... 针对工业机器人的柔性关节表现的复杂迟滞特性问题,提出了一种基于改进的iTransformer(inverted transformer)模型的非线性迟滞建模方法,用于描述柔性关节的输出力矩与扭转角之间的复杂迟滞特性。采用具有输入反转嵌入层的iTransformer模型,体现时间前后信号关联性,其自注意力机制,描述了迟滞的记忆特性。为了提高迟滞模型的精度,从结构及输入前端信息两个方面对iTransformer模型进行改进:在结构上,将LSTM网络与iTransformer模型并联,以增强迟滞模型的动态特性;在输入端,设计了一个自相关模块。将输入扭转角向量进行哈达玛积运算,构成新输入向量,并与所设计类迟滞算子结合,设计一个iTransformer模型前端的自相关模块,充分体现了输入信号的关联性。与iTransformer、LSTM及PI迟滞模型相比较,实验结果表明,所提出迟滞模型,对工业机器人的复杂迟滞特性的描述,具有更高的精度和泛化能力。 展开更多

On November 13, 2016, an MW7.8 earthquake struck Kaikoura in South Island of New Zealand. By means of back-projection of array recordings, ASTFs-analysis of global seismic recordings, and joint inversion of global seismic data and co-seismic In SAR data, we investigated complexity of the earthquake source. The result shows that the 2016 MW7.8 Kaikoura earthquake ruptured about 100 s unilaterally from south to northeast(~N28°–33°E), producing a rupture area about 160 km long and about 50 km wide and releasing scalar moment 1.01×1021 Nm. In particular, the rupture area consisted of two slip asperities, with one close to the initial rupture point having a maximal slip value ~6.9 m while the other far away in the northeast having a maximal slip value ~9.3 m. The first asperity slipped for about 65 s and the second one started 40 s after the first one had initiated. The two slipped simultaneously for about 25 s.Furthermore, the first had a nearly thrust slip while the second had both thrust and strike slip. It is interesting that the rupture velocity was not constant, and the whole process may be divided into 5 stages in which the velocities were estimated to be 1.4 km/s, 0 km/s, 2.1 km/s, 0 km/s and 1.1 km/s, respectively. The high-frequency sources distributed nearly along the lower edge of the rupture area, the highfrequency radiating mainly occurred at launching of the asperities, and it seemed that no high-frequency energy was radiated when the rupturing was going to stop. 展开更多

MRI评估下尺桡关节稳定性和尺骨茎突骨髓水肿对TFCC损伤的诊断价值

目的:基于MRI探讨下尺桡关节(DRUJ)稳定性特征及尺骨茎突骨髓水肿(BME)对腕关节三角纤维软骨复合体(TFCC)周围型与中央型损伤的诊断价值,构建MRI联合诊断模型以优化术前对TFCC损伤分型的准确性。方法:回顾性分析2020年1月-2024年1月在... 目的:基于MRI探讨下尺桡关节(DRUJ)稳定性特征及尺骨茎突骨髓水肿(BME)对腕关节三角纤维软骨复合体(TFCC)周围型与中央型损伤的诊断价值,构建MRI联合诊断模型以优化术前对TFCC损伤分型的准确性。方法:回顾性分析2020年1月-2024年1月在本院经关节镜证实的132例TFCC损伤(周围型94例,中央型38例)患者的临床和MRI资料。所有患者术前行常规腕关节MRI检查,并对DRUJ稳定性和尺骨茎突BME进行评估。采用卡方检验分析MRI分型诊断、DRUJ稳定性和尺骨茎突BME的组间差异,将有统计学差异的变量纳入二元Logistic回归分析筛选周围型损伤的预测因子并构建联合模型,评估MRI分型、DRUJ稳定性特征和联合模型对TFCC损伤分型的诊断效能。结果:周围型组中BME阳性率及DRUJ不稳定者占比显著均高于中央型组(P<0.001)。Logistic回归分析显示BME阳性(OR=0.335,95%CI:0.132~0.854)及DRUJ不稳定(OR=0.103,95%CI:0.013~0.428)是TFCC周围型损伤的独立预测因子(P均=0.001)。联合模型(MRI分型+DRUJ不稳定+尺骨茎突BME)区分TFCC损伤分型的AUC为0.814(95%CI:0.727~0.902),敏感度为65.8%,特异度为86.2%,联合模型的AUC明显高于MRI直接诊断(AUC=0.728)及DRUJ不稳定+BME模型(AUC=0.746)。结论:BME与DRUJ稳定性评估可作为TFCC损伤分型的辅助影像标志物,联合模型通过整合解剖稳定性及尺骨茎突骨髓水肿MRI征象可提升诊断特异性,为术前精准分型、手术决策和个体化诊疗提供重要依据。 展开更多

塔里木盆地勘探以典型喀斯特岩溶缝洞型和主干走滑断裂断控缝洞型储层为特色,形成了以塔河-轮南为代表的岩溶型、以顺北-富满为代表的构造型2个典型优势成储条件下油气勘探经典案例,但随着油气勘探向超深层甚至万米特深层进军,非典型优... 塔里木盆地勘探以典型喀斯特岩溶缝洞型和主干走滑断裂断控缝洞型储层为特色,形成了以塔河-轮南为代表的岩溶型、以顺北-富满为代表的构造型2个典型优势成储条件下油气勘探经典案例,但随着油气勘探向超深层甚至万米特深层进军,非典型优势成储条件下是否能发育规模储集体,是制约后续勘探决策与实现突破的关键问题。为此,通过系统分析盆地近10多年来在岩溶型、断控型、相控型勘探新进展与典型案例,探讨了非典型优势成储条件下不同成储要素的表现形式与成储机制,丰富和完善了超深层-万米特深层海相碳酸盐岩储层成因机理认识。研究结果表明:①完善了盆内中小尺度走滑断裂与逆冲断裂控储作用认识,建立了多序级走滑断裂带储集体栅状结构模型,指出了逆冲断裂带控储作用受断裂平面分带、垂向分层与岩溶作用差异共同控制;②深化了盆地岩溶复杂区储层形成、改造与保存动态过程研究,剖析了以于奇西为代表的复杂岩溶区4种古地貌组合单元(岩溶平原、浅峰丛洼地、深峰丛洼地和溶丘洼地)缝洞体发育特征及差异,指出了浅峰丛洼地区为规模缝洞储层保存最有利区;③与内幕不整合相关的岩溶储层正在成为盆地重要的勘探类型,揭示了上震旦统—中下奥陶统碳酸盐岩内幕多期次、不同级别不整合面及暴露溶蚀作用,在中下寒武统台缘带及台内不整合面附近发育较好储集体深度已突破万米。结论认为:①首次在盆地发现石灰岩孔隙型储层,突破了以往石灰岩基质物性较差的传统认识,进一步开拓了新的油气储层勘探新类型;②指出了盆地碳酸盐岩储层形成受走滑-逆冲及复合断裂活动、有利沉积环境与相带、多类型岩溶作用的协同控制,提出了断-相-溶“多元复合”联控规模成储新模式并在顺北隆2井油气勘探实践中得以证实,上述模式及成功实践为中国超深-万米特深层海相碳酸盐岩油气勘探提供新的借鉴。 展开更多

以“澳门历史城区虚拟现实体验艺术创作人才培训”项目为背景,应用联合分析法探讨标志设计决策过程,旨在提升品牌认知与信息传达。通过群体讨论确定标志的基本设计要素及其水平,结合整体轮廓法和正交试验设计生成16种标志组合方案,并进... 以“澳门历史城区虚拟现实体验艺术创作人才培训”项目为背景,应用联合分析法探讨标志设计决策过程,旨在提升品牌认知与信息传达。通过群体讨论确定标志的基本设计要素及其水平,结合整体轮廓法和正交试验设计生成16种标志组合方案,并进行视觉复杂性和偏好调查。基于调查结果,以方差分析检验标志设计要素之间的交互作用,确定各方案的总体效用函数和应用模式,获取各设计准则的成分效用值及要素的相对重要性。研究结果发现标志图形(56.60%)、标志文字(24.61%)和标志色彩(18.79%)在设计决策中具有不同的重要性。标志的视觉复杂性对用户偏好有显著影响,中等视觉复杂性的设计最受欢迎,过于简单或复杂的设计则较少受到青睐。采用联合分析法构建的标志设计决策优化模式,可协助设计师筛选出最优方案,提高标志设计决策的客观性,使设计决策过程更加系统性,可作为设计方案评估与选择的参照工具。 展开更多

[目的]介绍关节镜下自体掌长肌腱重建治疗下尺桡关节不稳合并三角纤维软骨复合体损伤的手术技术和初步临床结果。[方法]取腕关节掌侧切口,取腱器切取掌长肌腱。暴露桡骨远端骨质,用4.5 mm空心钻钻取隧道。关节镜监视下前叉定位器钻取尺骨隧道,将编织的掌长肌腱穿过桡骨远端隧道后穿过尺骨隧道,尾端用带线锚钉固定。[结果]所有患者均顺利完成手术,术中均未出现严重并发症。患者均获得随访,随访时间平均(10.0±1.4)个月,与术前相比,末次随访时,患者改良Mayo腕关节功能评分[(61.0±3.1)分,(90.5±8.2)分, P<0.001]显著改善,无患者需要翻修或者二次手术。[结论]关节镜下自体掌长肌腱重建TFCC治疗三角纤维软骨复合体损伤合并下尺桡关节不稳,可以恢复TFCC形态,同时解剖重建下尺桡关节稳定性,初步临床效果满意。 展开更多
