They might be related to the experience in the use of elderly care services, which remain...Background and Objectives: Views on living arrangement from elderly and informal caregivers are crucial to “ageing in place”. They might be related to the experience in the use of elderly care services, which remains inconclusive in previous literature. This study aimed to explore the association of previous experience in formal and informal long-term care services with views of both elderly and their informal caregivers on living arrangement. Research Design and Methods: This study adopted a cross-sectional design. Assessment records of Minimum Data Set-Home Care for community-dwelling elderly who were eligible for subsidized long-term care services in Hong Kong from 2004 to 2014 were made available. Multivariate logistic regression was applied to examine associations between both views on elder’s living arrangement from the elderly, caregivers and their previous informal caregiver support, and experience in formal care services. Results: 82,306 dyads of elderly and informal caregivers were included in the analysis. The elderly with previous use of home and community-based services (OR = 0.84, 95% CI 0.80 - 0.88) and informal caregivers (OR = 0.78, 95% CI 0.76 - 0.81) believed that the elderly should live away from home. Temporal trends of fewer elderly and caregivers supporting the idea of living away from home were also observed. Discussion and Implications: The results highlighted the importance of informal caregiver’s support and previous utilization of formal home and community-based services. It was concluded that resources and information of community-based care have a significant association with views on living at home also proper support services and training of care for the elderly should be made available to informal caregivers to reduce their burden.展开更多
BACKGROUND China has recently encountered severe challenges associated with population aging.Parents of first-generation only children face significant challenges regarding elderly care needs and the associated negati...BACKGROUND China has recently encountered severe challenges associated with population aging.Parents of first-generation only children face significant challenges regarding elderly care needs and the associated negative emotions.AIM To analyze the elderly care needs of first-generation only child parents in China and identify factors that influence negative emotions.METHODS This study used a cross-sectional design.Convenience sampling was used to select 1580 elderly individuals who met the inclusion criteria in a Chinese city between June and September 2022.A questionnaire was administered to collect general information about participants.Depression and anxiety were assessed using the patient health questionnaire-9 and generalized anxiety disorder-7 scale,respectively.A logistic regression analysis was performed to evaluate the relevant correlations.RESULTS Among 1580 first-generation only child parents,1120(70.89%)preferred family based care,324(20.51%)opted for community care,and 136(8.61%)chose institutional care,with 460(29.11%)reporting negative emotions.Significant differences in the distribution of negative emotions among only child parents were observed based on age,marital status,living conditions,disability,type of chronic disease,frailty status,and family support(P<0.05).The regression analysis indicated that disability,type of chronic disease,living environment,frailty status,and level of family support were independent risk factors for negative emotions among parents with only children(P<0.05).CONCLUSION Elderly care for parents of only children is primarily family-based.Independent risk factors for negative emotions in this group include disability,chronic disease type,and living environment.展开更多

Methods: From August 2022 to April 2023, an investigation was conducted among urban and rural elderly individuals aged over 75 years in Tangshan City using the Activities of Daily Living Scale, the Revised Community Elderly Fall Risk Assessment Tool, and the Loneliness Scale. Results: The study included 750 urban and 740 rural elderly individuals aged over 75 years. Matrix analysis revealed a significant proportion of fall safety care needs across various daily life and spiritual care requirements. Multiple factor analysis indicated that advanced age, lower education levels, a greater number of chronic diseases, and lower levels of family and social support were associated with higher care demands among the elderly in both urban and rural areas. These differences were statistically significant (P < 0.05). Conclusion: The elderly in urban and rural areas demonstrate a high demand for fall safety care. Particular attention should be given to individuals with lower education levels, those who are widowed, those with multiple chronic diseases, and those with low levels of family and social support to better meet the diverse care needs of this population.展开更多

BACKGROUND Owing to age-related physiological declines,elderly patients undergoing radical gastrectomy(RG)often experience physical and psychological challenges.Hence,appropriate nursing strategies need to be identifi...BACKGROUND Owing to age-related physiological declines,elderly patients undergoing radical gastrectomy(RG)often experience physical and psychological challenges.Hence,appropriate nursing strategies need to be identified for optimizing patient outcomes.AIM To evaluate the effects of the Neuman Systems Model-guided stepped care intervention(NGSC)model on postoperative physical and psychological recovery in elderly patients with gastric cancer who underwent RG.METHODS In total,80 elderly patients(≥65 years old)who underwent RG at the Shanxi Fenyang Hospital between January 2023 and June 2024 were included and randomly divided as follows:Control group(n=40,those receiving conventional stepped care intervention);observation group(n=40,those receiving NGSC).Postoperative recovery indices,anxiety/depression scores(Self-Rating Anxiety Scale/Self-Rating Depression Scale),quality of life(36-item short form health survey),and adverse event rates were compared between the groups.RESULTS Compared with the control group,the observation group presented significantly shorter recovery times for first bowel sounds,ambulation,flatus,oral intake,catheter removal,and hospitalization(all P<0.05).Notably,in the observation group,Self-Rating Anxiety Scale/Self-Rating Depression Scale scores and the adverse event rate were significantly lower(all P<0.05),whereas 36-item short form health survey scores were higher(P<0.05),compared with those in the control group.CONCLUSION Overall,the findings of this study show that the proposed NGSC model accelerates postoperative recovery,alleviates psychological distress,improves quality of life,and reduces complications in elderly patients with gastric cancer who underwent RG,presenting potential for clinical application.展开更多

However, there is insuffici...The progressive aging of society has become a global concern, and is expected to lead to the development of effective, sustainable, person-centered, integrated community-based care systems. However, there is insufficient evidence regarding effective integrated community-based care. In particular, few studies have focused on social aspects of the community environment related to elderly health. This study aimed to consider social aspects as evaluation items, focusing particularly on social determinants from the perspective of community-dwelling people, to explore truly effective integrated community-based care to improve elderly health. The definition of social determinants means social cohesion in social and community contexts. A literature review of English articles published in peer-reviewed journals up to October 2019 was conducted using PubMed, MEDLINE, and CINAHL with the following search terms: “social cohesion,” “elderly health,” “mental health” and “community.” Identified articles were screened based on title and abstract, and selected articles were subjected to full-text assessment and critical review. All references cited in the selected articles were also reviewed. The following inclusion criteria were used: 1) studies targeting community-dwelling elderly people or community-dwelling people including elderly people as participants;2) studies with clear descriptions of social factors in the Methods section;and 3) studies with clear descriptions of health-related items in the Methods section. From the 21 articles analyzed, of which 9 articles defined social determinants as social cohesion in social and community context, 37 items were extracted as social aspects at the community level that reflect the perspective of residents. These items can be developed as evaluation items for community-based health care outcomes through consensus among community health care providers and further investigation.展开更多
Objective:To investigate the impact of targeted nursing interventions based on frailty prediction models on peri-hospitalization clinical outcomes in middle-aged and elderly patients with upper gastrointestinal bleedi...Objective:To investigate the impact of targeted nursing interventions based on frailty prediction models on peri-hospitalization clinical outcomes in middle-aged and elderly patients with upper gastrointestinal bleeding(UGIB).Methods:A prospective cohort study was conducted,and 126 middle-aged and elderly patients with UGIB admitted from August 2024 to August 2025 were selected as the study subjects.The patients were divided into the intervention group(63 cases)and the control group(63 cases)based on whether they received nursing intervention based on frailty prediction models.The control group received routine care,while the intervention group,on the basis of routine care,used the FRAIL scale combined with laboratory indicators(albumin,hemoglobin,etc.)to establish a predictive model to evaluate patients within 24 hours of admission,and implemented multi-dimensional targeted nursing intervention for pre-frailty or frailty patients screened out.The incidence of frailty,rebleeding rate,average length of stay,hospitalization cost,and nursing satisfaction during hospitalization were compared between the two groups.Results:The incidence of frailty during hospitalization in the intervention group was 11.1%(7 cases/63 cases),significantly lower than 31.7%(20 cases/63 cases)in the control group,and the difference was statistically significant(p<0.05).The rebleeding rate of 4.8%vs 12.7%,the average length of stay of(7.2±1.5)days vs(9.1±2.2)days,and the average hospitalization cost of(23,000±6,000)yuan vs(28,000±7,000)yuan in the intervention group were all lower than those in the control group(all p<0.05).The nursing satisfaction score of the intervention group(93.5±4.2)points was higher than that of the control group(86.3±5.8)points(p<0.05).Conclusion:The frailty prediction model applied to the peri-hospitalization care of middle-aged and elderly patients with UGIB can effectively identify frailty risk.Through early targeted intervention,the incidence of frailty and rebleeding rate can be reduced,the length of hospital stay can be shortened,medical expenses can be reduced,and nursing satisfaction can be improved,which has clinical promotion value.展开更多

Temporal trends of fewer elderly and caregivers supporting the idea of living away from home were also observed. Discussion and Implications: The results highlighted the importance of informal caregiver’s support and previous utilization of formal home and community-based services. It was concluded that resources and information of community-based care have a significant association with views on living at home also proper support services and training of care for the elderly should be made available to informal caregivers to reduce their burden.

文摘BACKGROUND China has recently encountered severe challenges associated with population aging.Parents of first-generation only children face significant challenges regarding elderly care needs and the associated negative emotions.AIM To analyze the elderly care needs of first-generation only child parents in China and identify factors that influence negative emotions.METHODS This study used a cross-sectional design.Convenience sampling was used to select 1580 elderly individuals who met the inclusion criteria in a Chinese city between June and September 2022.A questionnaire was administered to collect general information about participants.Depression and anxiety were assessed using the patient health questionnaire-9 and generalized anxiety disorder-7 scale,respectively.A logistic regression analysis was performed to evaluate the relevant correlations.RESULTS Among 1580 first-generation only child parents,1120(70.89%)preferred family based care,324(20.51%)opted for community care,and 136(8.61%)chose institutional care,with 460(29.11%)reporting negative emotions.Significant differences in the distribution of negative emotions among only child parents were observed based on age,marital status,living conditions,disability,type of chronic disease,frailty status,and family support(P<0.05).The regression analysis indicated that disability,type of chronic disease,living environment,frailty status,and level of family support were independent risk factors for negative emotions among parents with only children(P<0.05).CONCLUSION Elderly care for parents of only children is primarily family-based.Independent risk factors for negative emotions in this group include disability,chronic disease type,and living environment.
文摘Against the backdrop of the accelerated aging of the population,the elderly care model integrating medical and elderly care services has become a crucial approach to addressing the challenges of elderly care.As direct providers of integrated medical and elderly care services,the professional competence of elderly caregivers directly affects the quality of services and the quality of life of the elderly.This paper aims to conduct an in-depth study on the professional competence of elderly caregivers under the integrated medical and elderly care model.Firstly,it clarifies the components of elderly caregivers’professional competence,analyzes the existing problems in their current professional competence,and proposes targeted improvement strategies.The purpose is to build a high-quality and professional team of elderly caregivers,provide strong support for the development of the integrated medical and elderly care cause,and promote the professionalization of integrated medical and elderly care services.

文摘Objective: To examine the daily life care, spiritual comfort, and fall safety care needs of the elderly in urban and rural areas of Tangshan, along with their influencing factors. Methods: From August 2022 to April 2023, an investigation was conducted among urban and rural elderly individuals aged over 75 years in Tangshan City using the Activities of Daily Living Scale, the Revised Community Elderly Fall Risk Assessment Tool, and the Loneliness Scale. Results: The study included 750 urban and 740 rural elderly individuals aged over 75 years. Matrix analysis revealed a significant proportion of fall safety care needs across various daily life and spiritual care requirements. Multiple factor analysis indicated that advanced age, lower education levels, a greater number of chronic diseases, and lower levels of family and social support were associated with higher care demands among the elderly in both urban and rural areas. These differences were statistically significant (P < 0.05). Conclusion: The elderly in urban and rural areas demonstrate a high demand for fall safety care. Particular attention should be given to individuals with lower education levels, those who are widowed, those with multiple chronic diseases, and those with low levels of family and social support to better meet the diverse care needs of this population.
文摘The increasingly severe aging problem in economically developed regions has created a pressing need for effective smart elderly care policies.This study takes Fujian Province as a case to systematically explore the characteristics and development trends of regional smart elderly care policies based on data-driven analysis,aiming to provide a rational and replicable framework for policy optimization in other regions.Using 59 policy documents from Fujian Province and its prefecture-level cities as the data source,this research employs quantitative methods combined with policy text mining techniques to examine the temporal distribution,content features,and keyword themes of these policies.The analysis reveals three core focus areas:the development and application of intelligent elderly care technologies,the construction and improvement of integrated healthcare and elderly care systems,and the enhancement of public elderly care services.This data-driven approach not only identifies key policy priorities and practical directions in Fujian Province but also offers a scientific tool for policy formulation and evaluation in similar contexts.While the study provides a systematic analysis framework based on empirical data,its findings are limited to Fujian Province and require further validation for broader applicability.Future research could extend to other regions and incorporate evaluations of policy implementation outcomes to refine the data-driven policy analysis system.
文摘With the intensifying aging population,rural elderly care services are facing challenges such as uneven medical resources and inadequate facilities.Taking Qinhuangdao City as an example,this paper explores ways to improve rural elderly care services through the construction of a remote medical service network.This paper analyzes the current status of rural elderly care services in Qinhuangdao City,pointing out that issues such as the uneven distribution of medical resources between urban and rural areas,poor accessibility,and low service quality urgently need to be addressed.The necessity of accelerating the construction of a remote medical network is proposed,including reducing medical costs,optimizing resource allocation,and disease prevention.Specific measures cover aspects such as policy support,integration of medical and elderly care services,talent cultivation,and technology promotion.At the same time,the potential challenges and risks faced by the remote medical service network in improving rural elderly care services are evaluated,and corresponding countermeasures and suggestions are proposed.Research shows that remote medical care can effectively improve the quality of rural elderly care services and help achieve proper medical care for the elderly.

文摘This empirical study examines the integration of medical and elderly care services in Guangyuan City through stratified random sampling,targeting 283 eligible residents aged 60+from three representative integrated institutions.The research employed a self-designed questionnaire titled“Humanistic Care Experience and Demand Influencing Factors”to collect subjective perceptions regarding basic care,psychological support,health education,and social interaction using a Likert five-point scale.Descriptive statistics,one-way ANOVA,and multiple regression modeling were conducted with SPSS 25.0 software.Results indicated that overall humanistic care experience scores were moderately high,with the“caregiving”dimension scoring highest and“psychological/social support”the lowest.Analysis revealed significant correlations between educational attainment,income,self-reported health status,and family support frequency(P<0.001),particularly among seniors with higher education,better health,and stronger family support.While integrated care facilities provide relatively comprehensive basic care,they should focus on addressing gaps in humanistic services,especially providing targeted support for groups with“low education,low income,poor health,and lack of family support.”

文摘Objective:To study the application effect of early postoperative enteral nutrition in the nursing care of elderly patients with gastric cancer.Methods:Sixty elderly patients with gastric cancer admitted to the hospital from January 2022 to January 2024 were selected and divided into groups according to the random number table method.The control group(n=30)received routine nursing after surgery,while the observation group(n=30)received early enteral nutrition nursing after surgery.The perioperative indexes,nutritional indexes before and after nursing,humoral and cellular immune indexes before and after nursing,gastrointestinal hormone levels before and after nursing,and the incidence of postoperative complications were compared between the two groups.Results:The perioperative indexes of the observation group were better than those of the control group(P<0.05).Before nursing,there was no significant difference in the nutritional indexes between the two groups(P>0.05).After nursing,the nutritional indexes of the observation group were better than those of the control group(P<0.05).Before nursing,there was no significant difference in the humoral and cellular immune indexes between the two groups(P>0.05).After nursing,the humoral and cellular immune indexes of the observation group were better than those of the control group(P<0.05).Before nursing,there was no significant difference in the gastrointestinal hormone levels between the two groups(P>0.05).After nursing,the gastrointestinal hormone levels of the observation group were better than those of the control group(P<0.05).The incidence of postoperative complications in the observation group was lower than that in the control group(P<0.05).Conclusion:For elderly patients with gastric cancer,early enteral nutrition nursing after surgery can improve their nutritional indexes and gastrointestinal hormone levels,enhance their immune ability,and prevent the incidence of postoperative complications,with good effects.
文摘Objective:To explore the application effect of continuous nursing in elderly patients with chronic diseases.Methods:A total of 100 elderly patients with chronic diseases were selected and randomly divided into experimental group and control group,50 cases in each group.Patients in the experimental group received continuous nursing,while patients in the control group received routine nursing.Comparison of two groups of patients’quality of life,health and nursing satisfaction.Results:the patient’s quality of life,health and nursing satisfaction were significantly higher than control group(p<0.05).Conclusion:Continuous nursing has a significant application effect in elderly patients with chronic diseases,which is worthy of further promotion and application.

文摘Objective:To investigate the effectiveness of a competency-oriented TCM workshop teaching model for integrated medical and elderly care in geriatric nursing positions.Methods:A stratified sampling method was employed to select 100 nurses from three integrated medical-care institutions in Guangyuan City.Using digital randomization,they were divided into a control group(conventional teaching model)and an educational reform group(competency-oriented TCM workshop model).The two groups were compared in training performance,evaluation of teaching models,job competency,and TCM nursing service quality.Results:The educational reform group demonstrated statistically significant higher scores(P<0.05),better evaluations of teaching models,enhanced job competencies,and improved TCM nursing service quality compared to the control group.Conclusion:This competency-oriented TCM workshop teaching model effectively enhances nursing competencies in geriatric care,fully leverages TCM’s unique advantages,better meets elderly patients’medical and care needs,and improves the elderly experience in integrated medical-care institutions.

文摘The expansion of the scale of the elderly care industry,the acquisition of market share,and the seizure of high profits depend on the consistency between the ecological niche of the elderly care industry and the actual resource and environmental conditions.Based on the situation theory of ecological niche,this paper expands the factor of“energy”and represents the three dimensions of“state,”“potential,”and“energy”from three aspects:market niche,technology niche,and resource niche.Taking 220 listed companies as samples,this paper improves the traditional catastrophe progression evaluation model and uses structural equation modeling to test the validity of the indicator system,thereby conducting evaluation research on the ecological niche of the elderly care industry.From the results of niche potential energy measurement,the three dimensions of market niche,resource niche,and technology niche are unevenly developed,reflecting the lack of competitiveness of the elderly care industry.

文摘BACKGROUND Owing to age-related physiological declines,elderly patients undergoing radical gastrectomy(RG)often experience physical and psychological challenges.Hence,appropriate nursing strategies need to be identified for optimizing patient outcomes.AIM To evaluate the effects of the Neuman Systems Model-guided stepped care intervention(NGSC)model on postoperative physical and psychological recovery in elderly patients with gastric cancer who underwent RG.METHODS In total,80 elderly patients(≥65 years old)who underwent RG at the Shanxi Fenyang Hospital between January 2023 and June 2024 were included and randomly divided as follows:Control group(n=40,those receiving conventional stepped care intervention);observation group(n=40,those receiving NGSC).Postoperative recovery indices,anxiety/depression scores(Self-Rating Anxiety Scale/Self-Rating Depression Scale),quality of life(36-item short form health survey),and adverse event rates were compared between the groups.RESULTS Compared with the control group,the observation group presented significantly shorter recovery times for first bowel sounds,ambulation,flatus,oral intake,catheter removal,and hospitalization(all P<0.05).Notably,in the observation group,Self-Rating Anxiety Scale/Self-Rating Depression Scale scores and the adverse event rate were significantly lower(all P<0.05),whereas 36-item short form health survey scores were higher(P<0.05),compared with those in the control group.CONCLUSION Overall,the findings of this study show that the proposed NGSC model accelerates postoperative recovery,alleviates psychological distress,improves quality of life,and reduces complications in elderly patients with gastric cancer who underwent RG,presenting potential for clinical application.
文摘The progressive aging of society has become a global concern, and is expected to lead to the development of effective, sustainable, person-centered, integrated community-based care systems. However, there is insufficient evidence regarding effective integrated community-based care. In particular, few studies have focused on social aspects of the community environment related to elderly health. This study aimed to consider social aspects as evaluation items, focusing particularly on social determinants from the perspective of community-dwelling people, to explore truly effective integrated community-based care to improve elderly health. The definition of social determinants means social cohesion in social and community contexts. A literature review of English articles published in peer-reviewed journals up to October 2019 was conducted using PubMed, MEDLINE, and CINAHL with the following search terms: “social cohesion,” “elderly health,” “mental health” and “community.” Identified articles were screened based on title and abstract, and selected articles were subjected to full-text assessment and critical review. All references cited in the selected articles were also reviewed. The following inclusion criteria were used: 1) studies targeting community-dwelling elderly people or community-dwelling people including elderly people as participants;2) studies with clear descriptions of social factors in the Methods section;and 3) studies with clear descriptions of health-related items in the Methods section. From the 21 articles analyzed, of which 9 articles defined social determinants as social cohesion in social and community context, 37 items were extracted as social aspects at the community level that reflect the perspective of residents. These items can be developed as evaluation items for community-based health care outcomes through consensus among community health care providers and further investigation.

文摘Objective:To investigate the impact of targeted nursing interventions based on frailty prediction models on peri-hospitalization clinical outcomes in middle-aged and elderly patients with upper gastrointestinal bleeding(UGIB).Methods:A prospective cohort study was conducted,and 126 middle-aged and elderly patients with UGIB admitted from August 2024 to August 2025 were selected as the study subjects.The patients were divided into the intervention group(63 cases)and the control group(63 cases)based on whether they received nursing intervention based on frailty prediction models.The control group received routine care,while the intervention group,on the basis of routine care,used the FRAIL scale combined with laboratory indicators(albumin,hemoglobin,etc.)to establish a predictive model to evaluate patients within 24 hours of admission,and implemented multi-dimensional targeted nursing intervention for pre-frailty or frailty patients screened out.The incidence of frailty,rebleeding rate,average length of stay,hospitalization cost,and nursing satisfaction during hospitalization were compared between the two groups.Results:The incidence of frailty during hospitalization in the intervention group was 11.1%(7 cases/63 cases),significantly lower than 31.7%(20 cases/63 cases)in the control group,and the difference was statistically significant(p<0.05).The rebleeding rate of 4.8%vs 12.7%,the average length of stay of(7.2±1.5)days vs(9.1±2.2)days,and the average hospitalization cost of(23,000±6,000)yuan vs(28,000±7,000)yuan in the intervention group were all lower than those in the control group(all p<0.05).The nursing satisfaction score of the intervention group(93.5±4.2)points was higher than that of the control group(86.3±5.8)points(p<0.05).Conclusion:The frailty prediction model applied to the peri-hospitalization care of middle-aged and elderly patients with UGIB can effectively identify frailty risk.Through early targeted intervention,the incidence of frailty and rebleeding rate can be reduced,the length of hospital stay can be shortened,medical expenses can be reduced,and nursing satisfaction can be improved,which has clinical promotion value.
