Communication Barriers,Emotional Distress and Mental Health Promotion in Cancer Care:Insights from Rural China

Background:Communication barriers are a critical yet underexplored determinant of psychological well-being in oncology care,particularly among rural Chinese patients treated in urban hospitals.This study examined how Mandarin proficiency influences emotional distress and identified implications for mental health promotion.Methods:A sequential mixed-methods design was adopted.Quantitative data were collected from 180 rural cancer patients using the Putonghua Proficiency Test(PSC),the Hospital Anxiety and Depression Scale(HADS),and the Distress Thermometer(DT).Multiple linear regressions estimated covariate-adjusted associations between Mandarin proficiency and distress outcomes,controlling for demographic and clinical factors.Semi-structured interviews with 20 patients explored communication experiences and psychosocial impacts.Quantitative and qualitative findings were integrated for triangulated interpretation.Results:Lower Mandarin proficiency was significantly associated with higher anxiety(β=−0.30,p=0.001),depression(β=−0.25,p=0.01),and distress(β=−0.28,p=0.003),with an adjusted R^(2)=0.18(F=7.4,p<0.001).Each 10-point increase in PSC corresponded to a 0.5-point reduction in DT score(B=−0.05,p=0.003).Qualitative themes revealed confusion,stigma,isolation,and reliance on family interpreters,confirming that linguistic inequity functions as an independent psychosocial stressor in oncology care.Conclusion:Language barriers significantly heighten distress among rural Chinese cancer patients.Addressing these barriers through interpreter services,culturally responsive staff training,and inclusive educational materials should be recognized as a core strategy for mental health promotion and equitable cancer care. 展开更多

Objectives:Effective and efficient communication is a core element in healthcare systems,especially between healthcare providers and patients.This study aimed to identify communication barriers be-tween nurses and patients in primary healthcare centers in Bahrain.Methods:This is a cross-sectional study conducted across primary healthcare centers in Bahrain.Four hundred and two patients were recruited using convenience sampling.A self-administered question-naire comprising 29 items on communication barriers was used.Results:A total of 402 patients consented to participate.The majority of participants reported the following statements had large effects on communication:“shortage in the number of nurses compared to the large number of patients”(254/401,63.3%),“lack of desire of nurse to communicate with patients”(246/402,61.2%),and“negative attitude of the nurse toward the patient”(238/401,59.4%).Further,“difference in language between nurses and patients,”“lack of self-confidence by nurses,”and“nurses overwhelmed by work”were ranked as top three statements with a significant influence on commu-nication between nurses and patients.Conclusions:Communication between healthcare providers and patients is pivotal for an optimal healthcare service.Based on the findings of this study and the literature,we recommend formal training of health care workers in improving communication skills and including this not only in medical cur-riculum but also in the form of continuing medical education(CMEs)1. 展开更多

Objectives:The motives that lead to nurses’workplace silence behaviour are essential to understand,as the failure to voice their concerns and maintain silence may cause various adverse outcomes,including burnout,turnover,job dissatisfaction,low productivity,and mental and moral distress.This study aimed to identify the level and influencing motives of the workplace silence behaviour among nurses at the National Guard Hospital in Madinah.Methods:A descriptive cross-sectional survey was conducted in January and February 2023.The participants completed a questionnaire with two parts:the sociodemographic data form and the Workplace Silence Behavior Nursing Motives Scale.Data analysis was performed by descriptive statistics and the chi-square test.Results:The overall level for all workplace silence behavior motives is moderate(3.29±0.69),with Avoidance Motives dimension receiving the highest scores(3.62±0.75)and Organization-Oriented Motives dimension receiving the lowest(3.15±0.86).Chi-square test showed that Saudi nationality(X^(2)=0.015,P=0.019)and monthly income(X^(2)=0.045,P?0.013)are factors in high-moderate levels of silence behaviour.Conclusions:The overall level of workplace silence behaviour motives was found to be moderate,nurses have higher monthly salaries and non-Saudi nationals tend to have higher levels of workplace silence behaviour motives.The study results provide a reference for nursing managers to pay attention to nurses’workplace silence behavior and take corresponding intervention measures. 展开更多
