BACKGROUND At present,there are few studies on the risk factors for bile leakage after laparoscopic common bile duct exploration(LCBDE)for older patients with choledocholithiasis.AIM To identify the potential risk fac...BACKGROUND At present,there are few studies on the risk factors for bile leakage after laparoscopic common bile duct exploration(LCBDE)for older patients with choledocholithiasis.AIM To identify the potential risk factors for bile leakage after LCBDE in older patients.METHODS A retrospective,single-center observational analysis was performed on patients aged≥70 years with choledocholithiasis treated by LCBDE who were admitted to our center between January 2011 and August 2022.The included patients were divided into non-bile leakage and bile leakage groups.Risk factors were determined by analyzing the observation indicators.RESULTS Seventy older patients with choledocholithiasis who underwent LCBDE were included.Univariate analysis showed that positive culture of bile bacteria was a risk factor for bile leakage after LCBDE(P<0.05).We further analyzed the bile bacteria,and univariate analysis showed that Enterococcus faecalis(E.faecalis)(P<0.05)and Pseudomonas aeruginosa(P<0.05)were associated with an increased risk of postoperative bile leakage in older patients(P<0.05).Multivariate analysis showed that E. faecalis was an independent risk factor for postoperative bile leakage in older patients (P < 0.05). Theresults of antibiotic sensitivity analysis showed that E. faecalis had 100% susceptibility to penicillin, ampicillin,linezolid, vancomycin, and furantoin.CONCLUSIONE. faecalis-associated biliary tract infection is an independent risk factor for bile leakage after LCBDE in olderpatients with choledocholithiasis. We suggest coverage with antibiotics to which E. faecalis is sensitive.展开更多

BACKGROUND Pancreaticobiliary maljunction(PBM)is a rare congenital abnormality in pancreaticobiliary duct development.PBM is commonly found in children,and it often leads to acute pancreatitis and other diseases as a ...BACKGROUND Pancreaticobiliary maljunction(PBM)is a rare congenital abnormality in pancreaticobiliary duct development.PBM is commonly found in children,and it often leads to acute pancreatitis and other diseases as a result of pancreaticobiliary reflux.Roux-en-Y choledochojejunostomy is a common surgical method for the treatment of PBM,but there are several associated complications that may occur after this operation.CASE SUMMARY The patient,a 12-year-old female,was hospitalized nearly 20 times in 2021 for recurrent acute pancreatitis.In 2022,she was diagnosed with PBM and underwent laparoscopic common bile duct resection and Roux-en-Y choledochojejunostomy in a tertiary hospital.In the first year after surgery,the patient had more than 10 recurrent acute pancreatitis episodes.After undergoing abdominal computed tomography and other examinations,she was diagnosed with“residual bile duct stones and recurrent acute pancreatitis”.On January 30,2024,the patient was admitted to our hospital due to recurrent upper abdominal pain and was cured through endoscopic retrograde cholangiopancreatography.CONCLUSION This article reports a case of a child with distal residual common bile duct stones and recurrent acute pancreatitis after Roux-en-Y choledochojejunostomy for PBM.The patient was cured through endoscopic retrograde cholangiopancreatography.展开更多

However, their industrial production and use in agricult...Synthetic fertilizers are widely used to address the urgent challenge of ensuring food supplies for a growing world population in the context of climate change. However, their industrial production and use in agriculture have a negative impact on the environment and consequently on human health. While chemical fertilizers may not have to be abandoned in agricultural production systems, limiting their use could help to make agriculture sustainable and resilient to climate change. In Senegal, the level of mineral fertilizers used in market gardening has become alarming in the Niayes area. As a result, microbial biotechnologies have been promoted for biofertilizer production of common bean (Phaseolus vulgaris L.) cultivation. Rhizobial inoculums have thus been used to reduce the rate of chemical nitrogen fertilizers being applied in cropping systems. Several investigations in the laboratory, on experimental stations and in the field have shown a possibility of a significant reduction in the use of nitrogen fertilizers in common bean production. Conventional mineral fertilization use can be reduced from over 120 kg N/ha to 20 kg N/ha. This contributes both to a very significant reduction in the application rate with the same level of yield and to an improvement in the standard of living. In addition, the environmental impact of using chemical fertilizers can be mitigated. This study is a contribution to the promotion of biofertilizers adoption in agricultural systems.展开更多
[Objectives]To investigate the antibacterial mechanism of Common Cnidium Fruit using network pharmacology.[Methods]Active components and targets of Common Cnidium Fruit were screened and obtained using the TCMSP datab...[Objectives]To investigate the antibacterial mechanism of Common Cnidium Fruit using network pharmacology.[Methods]Active components and targets of Common Cnidium Fruit were screened and obtained using the TCMSP database and HIT2.0 database.The collected targets were intersected with antibacterial/bacteriostatic targets obtained from the GeneCards database and OMIM database to identify the antibacterial/bacteriostatic targets of Common Cnidium Fruit.The active component-target network diagram of Common Cnidium Fruit was constructed using Cytoscape software and topological analysis was performed.GO enrichment analysis was performed on the target genes using the DAVID database.[Results]Screening yielded 25 active components of Common Cnidium Fruit,corresponding to 77 targets.Analysis identified 25 core antibacterial/bacteriostatic targets for Common Cnidium Fruit.Network analysis indicated that Common Cnidium Fruit may exert antibacterial/bacteriostatic effects through active components such asβ-sitosterol,stigmasterol,and xanthoxylin,while activating the body's immune regulatory functions by acting on targets including CASP3,PTGS2,BCL2,JUN,and ESR1.[Conclusions]Common Cnidium Fruit may exert antibacterial/bacteriostatic effects through multiple pathways via a mechanism involving multiple components,multiple targets,and multiple pathways.展开更多

Illegal hunting and trafficking of wildlife and their derivatives extort unprecedented population decline of relatively many species pushing them towards extinction.Notwithstanding contemporary counteracting intervent...Illegal hunting and trafficking of wildlife and their derivatives extort unprecedented population decline of relatively many species pushing them towards extinction.Notwithstanding contemporary counteracting interventions at international,regional,national and local levels,wildlife farming is advocated as an alternative approach to minimize pressure on wild populations.For wildlife farming to be an effective conservation tool,the integration of wildlife forensics is inevitable to allow distinction between captive-bred and wild-caught species.To this end,we analyzed methylation rates of skeletal muscle samples(pectoralis major,triceps brachii,gastrocnemius,biceps femoris,and neck muscles)from 60 captive-bred and 30 wild-caught Common Pheasant.A total of 13,507 differentially methylated regions were identified between five wild-caught and five captive-bred individuals through whole-genome methylation sequencing(WGBS).Based on the selected five methylation sites,LOC116231076,LOC116242223,ATAD2B,EGFL6,and HS2ST,quantitative detection technique was developed using methylation-sensitive high-resolution melting curve(MS-HRM)to measure methylation rates.The results showed significant differences in methylation rates at all differential sites between wild-caught and captive-bred individuals(|t|=0.67–33.10,P=0.000–0.042).The discrimination accuracy rate of each locus was highest in the gastrocnemius muscle and lowest in the neck muscle.The discrimination accuracy rate on LOC116231076,LOC116242223,ATAD2B,EGFL6,and HS2ST methylation sites for gastrocnemius muscle was 64.98%,100.00%,68.54%,63.79%,and 63.70%,respectively;and for neck muscle it was 67.42%,68.06%,83.61%,65.04%,and68.85%,respectively.The united discrimination accuracy rate of the five loci were 100.00%for gastrocnemius muscle,99.78%for biceps femoris muscle,97.52%for pectoralis major muscle,93.96%for triceps brachii muscle,and 91.63%for neck muscle,respectively.The panel also revealed excellent repeatability,reproducibility,sensitivity and universality to mammals and avian species.This study establishes an effective,accurate and low-cost identification technology for the identification of wild and farmed Common Pheasant,and also provides a reference for the development of identification methods for other species.展开更多

Management innovation is a key driver of total factor productivity and plays a crucial role in developing new quality productive forces.As China’s capital market continues to develop,the prevalence of common ownershi...Management innovation is a key driver of total factor productivity and plays a crucial role in developing new quality productive forces.As China’s capital market continues to develop,the prevalence of common ownership—investors who hold significant stakes in multiple firms—has grown,raising the question of how it influences corporate management innovation.This study empirically investigates this issue using panel data from A-share listed companies in China between 2014 and 2022.The findings reveal an inverted U-shaped relationship between common ownership and management innovation.Specifically,institutional common ownership exerts no significant effect,while non-institutional common ownership exhibits a significant inverted U-shaped influence.Mechanism analysis suggests that common ownership often appoints shared directors,supervisors,and senior executives across firms,which in turn significantly impacts corporate management innovation.Further analysis indicates that common ownership in the primary and tertiary industries significantly influences management innovation,whereas that in the secondary industry does not show such an effect.This study offers valuable insights for corporate governance,business management,and investor decision-making and provides important references for policymakers in formulating governance-related regulations.展开更多
To illustrate the mechanisms of exosomal microRNAs(miRNAs)in common mental disorders,and explore their potential as diagnostic biomarkers and therapeutic targets,a systematic literature review of relevant studies on e...To illustrate the mechanisms of exosomal microRNAs(miRNAs)in common mental disorders,and explore their potential as diagnostic biomarkers and therapeutic targets,a systematic literature review of relevant studies on exosomal miRNAs in mental disorders was conducted.Data from cell experiments,animal models,and clinical studies were analyzed and combined to study the mechanisms and roles of exosomal miRNAs in various mental disorders.Research has shown that exosomal miRNAs,such as miR-146a,miR-223,miR-125b,and miR-451a,affect Alzheimer’s disease(AD)formation by regulating key pathways such as toll-like receptor 4(TLR4)and phosphatidylinositol 3-kinase(PI3K)/protein kinase B(Akt),respectively.MiR-146a-5p regulates the occurrence of schizophrenia through the Notch pathway.TLR4 regulates miR-146a and miR-155 in major depressive disorder(MDD),and miR-144-5p regulates the disease through PI3K/Akt.Exosomal miR-484,miR-652-3p,miR-142-3p,miR-21a-3p,and miR-21-5p regulate key pathways in bipolar disorder,autism spectrum disorder,and Rett syndrome(e.g.,TLR4,PI3K/Akt,and Epha4/TEK)and have an influence on mental disorders.Exosomal miRNAs are involved in the occurrence of mental disorders through TLR4,PI3K/Akt,and Epha4/TEK pathways,which provides a clearer understanding of disease cognition.Of these pathways,the TLR4 and PI3K/Akt pathways play a role in AD,MDD,and neurodevelopmental disorders,which can be used as an effective breakthrough in the study of mental disorders.Exosomal miRNAs could also serve as diagnostic biomarkers and therapeutic targets,providing new insights into precise interventions for mental disorders.展开更多

The evidence ...A 1.5-hour recording of six (Gulf of Mexico) captive common bottlenose dolphins (Tursiops truncatus) at Sea Life Park, Hawaii shows that: 1) they have a song of a sequence of tonal (often slurred) notes. The evidence for their song comes from: a) their emitting songs that were complex (both tonally and rhythmically), b) their likely improvising (as inferred from their most complex songs not being repeated), c) songs based on a theme (used repeatedly), d) a bout of singing (at times) including social creativity, e) songs with a distinct beginning, middle and end section, and f) one song consisting of in part, a theme, 2) their songs are not sung in key (as determined from analysing the beginning tonal value of a note with spectral-frequency analysis), and 3) are sung in bouts, and 4) mostly with tonal-striated vocalizations (in the literature termed squawks and bray calls). Their apparent creativity (likely improvisations) and social creativity are of importance to how they evolved cognitively, to the study of song culture (between populations and delphinid species), understanding their (intra and inter-species) associations, and kinds of relationships, determining individual personalities, and perhaps will provide supporting evidence for their ability to reason. As inferred, they sing without being intentionally manipulative from: 1) their use of Gquic psychology [1] (as explained in this article), and 2) as inferred from their likely having an ability to reason [1] [2] (as deduced from their behavioural ecology (their unique exceptional evolutionary freedom), including their comparatively peaceful composure between conspecifics, in line with the proof of The Peaceful Composure Theorem), suggesting (per The Peaceful Composure Theorem) they have an egalitarian-like society. Their interspecies (displaced) aggression towards smaller odontocete species is shown to be a function of jealousies that emerge from their stronger social bonds than other species, from their compassionate nature, and females and males not pairing up in long-term associations. Species with greater cultural freedom are shown to have a more complex song providing further evidence for species culture not crucial to survival. The conclusive evidence for specie culture from my recent publications is of relevance to the scientific community’s acceptance of two new theories of evolution, and Gquic psychology. The loud burst-pulse sounds, chasing, charging and raking behaviours described in the literature could be a play behaviour, ought not be labelled as aggressive behaviours, as inferred from dog mock-fight play behaviour. In regard to future comparative behavioural ecology studies, aggressive animal behaviour should only be defined as an act of displacement (that is shown to commonly occur in the Indo-Pacific bottlenose dolphin species (Tursiops aduncus)), or as acts of biting, and hitting.展开更多
Background:The endoscopic appearance of the major duodenal papilla influences biliary cannulation and complications.This study aimed to investigate the role of major duodenal papillae in the endoscopic treatment of co...Background:The endoscopic appearance of the major duodenal papilla influences biliary cannulation and complications.This study aimed to investigate the role of major duodenal papillae in the endoscopic treatment of common bile duct(CBD)stones.Methods:This retrospective study was conducted at Bishan Hospital of Chongqing Medical University between January 2018 and August 2022.Patients with native papillae who underwent endoscopic treatment for CBD stones were recruited and divided into four groups according to Haraldsson's classification of papillae(typesⅠ-Ⅳ).Univariate and multivariate logistic regression analyses were used to identify risk factors for difficult cannulation and post-endoscopic retrograde cholangiopancreatography(ERCP)pancreatitis(PEP).Results:A total of 596 patients with CBD stones were enrolled.The proportion of patients with typeⅠpapilla was the highest(n=231,38.8%),followed by typeⅢpapilla(n=175,29.4%),typeⅣpapilla(n=101,16.9%)and typeⅡpapilla(n=89,14.9%).Difficult cannulation occurred in 188 of 596 patients(31.5%),with most cases occurring in those with typeⅢpapilla(71/175,40.6%,P=0.020).Multivariate logistic analysis revealed that age[odds ratio(OR)=1.034,95%confidence interval(CI):1.021–1.047,P<0.001],typeⅢpapilla(OR=2.255,95%CI:1.439–3.535,P<0.001),gallbladder in situ(OR=2.486,95%CI:1.346–4.590,P=0.004),and CBD diameter<10 mm(OR=1.600,95%CI:1.049–2.441,P=0.029)were risk factors for difficult cannulation.The total incidence of PEP was 10.9%.Compared with the other types of papillae,the rate of PEP was the highest in those with typeⅠpapilla(15.2%,P=0.030).Multivariate analysis demonstrated that PEP was associated with difficult cannulation(OR=1.811,95%CI:1.044–3.143,P=0.035)and white blood cells(WBCs)<10×10~9/L(OR=2.199,95%CI:1.051–4.600,P=0.036).Conclusions:The endoscopic appearance of the major papilla is an important factor that influences both biliary cannulation and outcomes.TypeⅢpapilla is more frequently difficult to cannulate in the endoscopic treatment of CBD stones.展开更多
Based on micro-survey data from 894 farmers in Sichuan Province,this study employs the Ordinary Least Squares(OLS),Two-Stage Least Squares(2SLS),mediating effect,and moderating effect models to comprehensively analyze...Based on micro-survey data from 894 farmers in Sichuan Province,this study employs the Ordinary Least Squares(OLS),Two-Stage Least Squares(2SLS),mediating effect,and moderating effect models to comprehensively analyze the mechanisms and effects of digital literacy in empowering farmers’common prosperity.Endogeneity and robustness are addressed using the Sobel test,the Bootstrap,and moderating effect plots.The findings indicate that first,after controlling for variables such as age,gender,and planting years,digital literacy significantly and positively influences the common prosperity of farmers at the 5%level;second,enhanced digital literacy promotes higher levels of technology adoption and information accumulation among farmers,thereby increasing the probability of achieving common prosperity.The mediating effects of technology adoption and information sharing account for 21.10%and 33.94%of the total effect,respectively;and third,the information environment and financial empowerment play positive moderation roles in the effect of digital literacy on farmers’common prosperity,with the moderation effects significant at the 1%and 10%levels,respectively.Therefore,we recommend policies that enhance the access of farmers to digital literacy training,strengthen rural digital infrastructure,establish integrated rural information-sharing platforms,and create an enabling environment for the adoption of digital technologies.展开更多

[Objectives]To systematically evaluate the impact of interventions based on the Common Sense Model of Self-Regulation(CSM)on the psychosocial adaptation of young and middle-aged patients with acute coronary syndrome(A...[Objectives]To systematically evaluate the impact of interventions based on the Common Sense Model of Self-Regulation(CSM)on the psychosocial adaptation of young and middle-aged patients with acute coronary syndrome(ACS),providing evidence-based support for clinical practice.[Methods]A systematic review was conducted using a literature search method,systematically searching through Chinese and English databases such as PubMed,Embase,Cochrane Library,CINAHL,China National Knowledge Infrastructure(CNKI),Wanfang Database,and VIP Database,from the database inception to December 31,2024.The search focused on studies related to the impact of the CSM on the psychosocial adaptation of young and middle-aged ACS patients.Two researchers independently performed literature screening,quality assessment,and data extraction.[Results]A total of 18 studies were included,comprising 12 randomized controlled trials,4 quasi-experimental studies,and 2 cohort studies,involving 2847 young and middle-aged ACS patients.Interventions based on the CSM significantly improved patients disease perception,emotional regulation,self-efficacy,and quality of life.Patients in the intervention group showed significant reductions in anxiety and depression levels,cardiac-related fear,and improvements in disease perception accuracy,treatment adherence,and social function recovery.[Conclusions]Interventions based on the CSM can effectively promote the psychosocial adaptation of young and middle-aged ACS patients,improve their disease perception and emotional state,and enhance their quality of life.It is recommended that this model be widely applied in the clinical care of young and middle-aged ACS patients.展开更多
To enhance energy interaction among low-voltage stations(LVSs)and reduce the line loss of the distribution network,a novel operation mode of the micro-pumped storage system(mPSS)has been proposed based on the common r...To enhance energy interaction among low-voltage stations(LVSs)and reduce the line loss of the distribution network,a novel operation mode of the micro-pumped storage system(mPSS)has been proposed based on the common reservoir.First,some operation modes of mPSS are analyzed,which include the separated reservoir mode(SRM)and common reservoir mode(CRM).Then,based on the SRM,and CRM,an energy mutual assistance control model between LVSs has been built to optimize energy loss.Finally,in the simulation,compared to the model without pumped storage in the LVS,the SRMand CLRMcan decrease the total energy loss by 294.377 and 432.578 kWh,respectively.The configuration of mPSS can improve the utilization rate of the new energy source generation system,and relieve the pressure of transformer capacity in the LVS.Compared with the SRM,the proposed CRM has reduced the total energy loss by 138.201 kWh,increased the new energy consumption by 161.642 kWh,and decreased the line loss by 7.271 kWh.With the efficiency of the mPSS improving,the total energy loss reduction of CRM will be 3.5 times that of SRM.Further,the CRMcan significantly reduce the reservoir capacity construction of mPSS and ismore suitable for scenarios where the capacity configuration of mPSS is limited.展开更多
With the increasing importance of multimodal data in emotional expression on social media,mainstream methods for sentiment analysis have shifted from unimodal to multimodal approaches.However,the challenges of extract...With the increasing importance of multimodal data in emotional expression on social media,mainstream methods for sentiment analysis have shifted from unimodal to multimodal approaches.However,the challenges of extracting high-quality emotional features and achieving effective interaction between different modalities remain two major obstacles in multimodal sentiment analysis.To address these challenges,this paper proposes a Text-Gated Interaction Network with Inter-Sample Commonality Perception(TGICP).Specifically,we utilize a Inter-sample Commonality Perception(ICP)module to extract common features from similar samples within the same modality,and use these common features to enhance the original features of each modality,thereby obtaining a richer and more complete multimodal sentiment representation.Subsequently,in the cross-modal interaction stage,we design a Text-Gated Interaction(TGI)module,which is text-driven.By calculating the mutual information difference between the text modality and nonverbal modalities,the TGI module dynamically adjusts the influence of emotional information from the text modality on nonverbal modalities.This helps to reduce modality information asymmetry while enabling full cross-modal interaction.Experimental results show that the proposed model achieves outstanding performance on both the CMU-MOSI and CMU-MOSEI baseline multimodal sentiment analysis datasets,validating its effectiveness in emotion recognition tasks.展开更多
BACKGROUND Textbook outcome(TO),an emerging composite metric for surgical quality assessment,has recently gained recognition for evaluating perioperative results.Laparoscopic transcystic common bile duct exploration(L...BACKGROUND Textbook outcome(TO),an emerging composite metric for surgical quality assessment,has recently gained recognition for evaluating perioperative results.Laparoscopic transcystic common bile duct exploration(LTCBDE)has become a widely adopted minimally invasive technique for treating cholecystolithiasis with choledocholithiasis.Despite its growing clinical application,TO has not yet been formally defined for LTCBDE,nor have its failure-associated risk factors been systematically examined.AIM To define TO for LTCBDE,establish standardized criteria,and identify risk factors for TO failure via logistic regression.METHODS A retrospective cohort of 388 patients who underwent LTCBDE in combination with laparoscopic cholecystectomy at the Department of Biliopancreatic Surgery,Tongji Hospital,from January 2018 to October 2024,was analyzed.The study delineated TO criteria for LTCBDE,calculated the rate of TO achievement,and employed logistic regression to determine independent predictors of TO failure.RESULTS TO was defined as the absence of the following seven criteria:Conversion to open surgery,postoperative complications(Clavien-Dindo grade≥2),biliary leakage(International Study Group of Pancreatic Surgery/International Study Group of Liver Surgery grade B/C),delayed removal of drainage tube(>4 days),postoperative interventions,prolonged length of stay(>7 days),and 30-day readmission or mortality.Among 388 patients,276(71.1%)achieved TO.The primary causes of TO failure included delayed removal of drainage tube(94 cases,83.9%),prolonged length of stay(50 cases,44.6%).Multivariate analysis revealed four independent risk factors for TO failure:Preoperative endoscopic retrograde cholangiopancreatography(P=0.022),advanced age(P=0.010),prolonged anesthesia time(P<0.001),and elevated preoperative alkaline phosphatase levels(P=0.048).These findings suggest that applying the concept of TO to LTCBDE enhances surgical quality evaluation and supports early identification of high-risk patients,facilitating personalized clinical decisions and optimizing individual management.展开更多

文摘BACKGROUND At present,there are few studies on the risk factors for bile leakage after laparoscopic common bile duct exploration(LCBDE)for older patients with choledocholithiasis.AIM To identify the potential risk factors for bile leakage after LCBDE in older patients.METHODS A retrospective,single-center observational analysis was performed on patients aged≥70 years with choledocholithiasis treated by LCBDE who were admitted to our center between January 2011 and August 2022.The included patients were divided into non-bile leakage and bile leakage groups.Risk factors were determined by analyzing the observation indicators.RESULTS Seventy older patients with choledocholithiasis who underwent LCBDE were included.Univariate analysis showed that positive culture of bile bacteria was a risk factor for bile leakage after LCBDE(P<0.05).We further analyzed the bile bacteria,and univariate analysis showed that Enterococcus faecalis(E.faecalis)(P<0.05)and Pseudomonas aeruginosa(P<0.05)were associated with an increased risk of postoperative bile leakage in older patients(P<0.05).Multivariate analysis showed that E. faecalis was an independent risk factor for postoperative bile leakage in older patients (P < 0.05). Theresults of antibiotic sensitivity analysis showed that E. faecalis had 100% susceptibility to penicillin, ampicillin,linezolid, vancomycin, and furantoin.CONCLUSIONE. faecalis-associated biliary tract infection is an independent risk factor for bile leakage after LCBDE in olderpatients with choledocholithiasis. We suggest coverage with antibiotics to which E. faecalis is sensitive.

文摘Common bean(Phaseolus vulgaris L.)is a vital source of protein and essential nutrients for human consumption and plays a key role in sustainable agriculture due to its nitrogen-fixing ability(Nadeem et al.,2021).Kidney beans,a subcategory of dry common beans,are highly valued for their rich protein,dietary fiber,low fat content,and various trace elements(Garcia-Cordero et al.,2021).Despite the release of several de novo genome assemblies(Goodstein et al.,2012;Schmutz et al.,2014;Vlasova et al.,2016;Cortinovis et al.,2024),existing common bean genomes remain incomplete,particularly in complex regions such as centromeres and telomeres,limiting a comprehensive understanding of the genomic landscape.
文摘While endoscopic retrograde cholangiopancreatography(ERCP)remains the primary treatment modality for common bile duct stones(CBDS)or choledocho-lithiasis due to advancements in instruments,surgical intervention,known as common bile duct exploration(CBDE),is still necessary in cases of difficult CBDS,failed endoscopic treatment,or altered anatomy.Recent evidence also supports CBDE in patients requesting single-step cholecystectomy and bile duct stone removal with comparable outcomes.This review elucidates relevant clinical anatomy,selection indications,and outcomes to enhance surgical understanding.The selection between trans-cystic(TC)vs trans-choledochal(TD)approaches is described,along with stone removal techniques and ductal closure.Detailed surgical techniques and strategies for both the TC and TD approaches,including instrument selection,is also provided.Additionally,this review comprehensively addresses operation-specific complications such as bile leakage,stricture,and entrapment,and focuses on preventive measures and treatment strategies.This review aims to optimize the management of CBDS through laparoscopic CBDE,with the goal of improving patient outcomes and minimizing risks.
文摘BACKGROUND Pancreaticobiliary maljunction(PBM)is a rare congenital abnormality in pancreaticobiliary duct development.PBM is commonly found in children,and it often leads to acute pancreatitis and other diseases as a result of pancreaticobiliary reflux.Roux-en-Y choledochojejunostomy is a common surgical method for the treatment of PBM,but there are several associated complications that may occur after this operation.CASE SUMMARY The patient,a 12-year-old female,was hospitalized nearly 20 times in 2021 for recurrent acute pancreatitis.In 2022,she was diagnosed with PBM and underwent laparoscopic common bile duct resection and Roux-en-Y choledochojejunostomy in a tertiary hospital.In the first year after surgery,the patient had more than 10 recurrent acute pancreatitis episodes.After undergoing abdominal computed tomography and other examinations,she was diagnosed with“residual bile duct stones and recurrent acute pancreatitis”.On January 30,2024,the patient was admitted to our hospital due to recurrent upper abdominal pain and was cured through endoscopic retrograde cholangiopancreatography.CONCLUSION This article reports a case of a child with distal residual common bile duct stones and recurrent acute pancreatitis after Roux-en-Y choledochojejunostomy for PBM.The patient was cured through endoscopic retrograde cholangiopancreatography.

文摘Synthetic fertilizers are widely used to address the urgent challenge of ensuring food supplies for a growing world population in the context of climate change. However, their industrial production and use in agriculture have a negative impact on the environment and consequently on human health. While chemical fertilizers may not have to be abandoned in agricultural production systems, limiting their use could help to make agriculture sustainable and resilient to climate change. In Senegal, the level of mineral fertilizers used in market gardening has become alarming in the Niayes area. As a result, microbial biotechnologies have been promoted for biofertilizer production of common bean (Phaseolus vulgaris L.) cultivation. Rhizobial inoculums have thus been used to reduce the rate of chemical nitrogen fertilizers being applied in cropping systems. Several investigations in the laboratory, on experimental stations and in the field have shown a possibility of a significant reduction in the use of nitrogen fertilizers in common bean production. Conventional mineral fertilization use can be reduced from over 120 kg N/ha to 20 kg N/ha. This contributes both to a very significant reduction in the application rate with the same level of yield and to an improvement in the standard of living. In addition, the environmental impact of using chemical fertilizers can be mitigated. This study is a contribution to the promotion of biofertilizers adoption in agricultural systems.

文摘[Objectives]To investigate the antibacterial mechanism of Common Cnidium Fruit using network pharmacology.[Methods]Active components and targets of Common Cnidium Fruit were screened and obtained using the TCMSP database and HIT2.0 database.The collected targets were intersected with antibacterial/bacteriostatic targets obtained from the GeneCards database and OMIM database to identify the antibacterial/bacteriostatic targets of Common Cnidium Fruit.The active component-target network diagram of Common Cnidium Fruit was constructed using Cytoscape software and topological analysis was performed.GO enrichment analysis was performed on the target genes using the DAVID database.[Results]Screening yielded 25 active components of Common Cnidium Fruit,corresponding to 77 targets.Analysis identified 25 core antibacterial/bacteriostatic targets for Common Cnidium Fruit.Network analysis indicated that Common Cnidium Fruit may exert antibacterial/bacteriostatic effects through active components such asβ-sitosterol,stigmasterol,and xanthoxylin,while activating the body's immune regulatory functions by acting on targets including CASP3,PTGS2,BCL2,JUN,and ESR1.[Conclusions]Common Cnidium Fruit may exert antibacterial/bacteriostatic effects through multiple pathways via a mechanism involving multiple components,multiple targets,and multiple pathways.
文摘Based on Marxist theory of social reproduction and the dual dimensions of“wealth”and“common prosperity,”this study examines the impact of the digital economy in promoting common prosperity across the four key stages:production,distribution,exchange,and consumption.The findings indicate that the digital economy fosters productivity growth and plays significant regulatory roles in primary distribution,redistribution,and third distribution.Moreover,the digital economy overcomes the spatiotemporal constraints of exchange and enhances consumption equality,thereby generating both“wealth”and“common prosperity”effects.However,there are still persistent challenges,including the displacement of traditional outputs,monopolization by digital platforms,the impact on employment,and income disparities arising from the digital divide.To address these challenges,we recommend optimizing the development policies of the digital economy,accelerating the growth of digital productivity,strengthening regulation of the digital platforms,building new talent supply systems,actively addressing the digital divide,and advancing market-oriented reform of data elements to better promote common prosperity.

文摘Illegal hunting and trafficking of wildlife and their derivatives extort unprecedented population decline of relatively many species pushing them towards extinction.Notwithstanding contemporary counteracting interventions at international,regional,national and local levels,wildlife farming is advocated as an alternative approach to minimize pressure on wild populations.For wildlife farming to be an effective conservation tool,the integration of wildlife forensics is inevitable to allow distinction between captive-bred and wild-caught species.To this end,we analyzed methylation rates of skeletal muscle samples(pectoralis major,triceps brachii,gastrocnemius,biceps femoris,and neck muscles)from 60 captive-bred and 30 wild-caught Common Pheasant.A total of 13,507 differentially methylated regions were identified between five wild-caught and five captive-bred individuals through whole-genome methylation sequencing(WGBS).Based on the selected five methylation sites,LOC116231076,LOC116242223,ATAD2B,EGFL6,and HS2ST,quantitative detection technique was developed using methylation-sensitive high-resolution melting curve(MS-HRM)to measure methylation rates.The results showed significant differences in methylation rates at all differential sites between wild-caught and captive-bred individuals(|t|=0.67–33.10,P=0.000–0.042).The discrimination accuracy rate of each locus was highest in the gastrocnemius muscle and lowest in the neck muscle.The discrimination accuracy rate on LOC116231076,LOC116242223,ATAD2B,EGFL6,and HS2ST methylation sites for gastrocnemius muscle was 64.98%,100.00%,68.54%,63.79%,and 63.70%,respectively;and for neck muscle it was 67.42%,68.06%,83.61%,65.04%,and68.85%,respectively.The united discrimination accuracy rate of the five loci were 100.00%for gastrocnemius muscle,99.78%for biceps femoris muscle,97.52%for pectoralis major muscle,93.96%for triceps brachii muscle,and 91.63%for neck muscle,respectively.The panel also revealed excellent repeatability,reproducibility,sensitivity and universality to mammals and avian species.This study establishes an effective,accurate and low-cost identification technology for the identification of wild and farmed Common Pheasant,and also provides a reference for the development of identification methods for other species.

文摘BACKGROUND Recurrence of common bile duct(CBD)calculi within 30 days following T-tube cholangiography is exceedingly rare.CASE SUMMARY This article details an instance of choledocholithiasis involving a 1.2 cm×0.9 cm stone located in the lower and middle segments of the CBD,identified 30 days after T-tube cholangiography,accompanied by multiple microstones.Magnetic resonance cholangiopancreatography revealed dilation of both intrahepatic and extrahepatic bile ducts,with the widest segment of the CBD measuring approximately 2 cm.The patient underwent laparoscopic choledochal exploration followed by choledochojejunostomy using the Roux-en-Y technique.CONCLUSION Although recurrence of choledocholithiasis within such a short postoperative period is exceedingly uncommon,this case underscores the necessity for clinicians to remain vigilant regarding the potential for early postoperative recurrence.

文摘Management innovation is a key driver of total factor productivity and plays a crucial role in developing new quality productive forces.As China’s capital market continues to develop,the prevalence of common ownership—investors who hold significant stakes in multiple firms—has grown,raising the question of how it influences corporate management innovation.This study empirically investigates this issue using panel data from A-share listed companies in China between 2014 and 2022.The findings reveal an inverted U-shaped relationship between common ownership and management innovation.Specifically,institutional common ownership exerts no significant effect,while non-institutional common ownership exhibits a significant inverted U-shaped influence.Mechanism analysis suggests that common ownership often appoints shared directors,supervisors,and senior executives across firms,which in turn significantly impacts corporate management innovation.Further analysis indicates that common ownership in the primary and tertiary industries significantly influences management innovation,whereas that in the secondary industry does not show such an effect.This study offers valuable insights for corporate governance,business management,and investor decision-making and provides important references for policymakers in formulating governance-related regulations.
文摘To illustrate the mechanisms of exosomal microRNAs(miRNAs)in common mental disorders,and explore their potential as diagnostic biomarkers and therapeutic targets,a systematic literature review of relevant studies on exosomal miRNAs in mental disorders was conducted.Data from cell experiments,animal models,and clinical studies were analyzed and combined to study the mechanisms and roles of exosomal miRNAs in various mental disorders.Research has shown that exosomal miRNAs,such as miR-146a,miR-223,miR-125b,and miR-451a,affect Alzheimer’s disease(AD)formation by regulating key pathways such as toll-like receptor 4(TLR4)and phosphatidylinositol 3-kinase(PI3K)/protein kinase B(Akt),respectively.MiR-146a-5p regulates the occurrence of schizophrenia through the Notch pathway.TLR4 regulates miR-146a and miR-155 in major depressive disorder(MDD),and miR-144-5p regulates the disease through PI3K/Akt.Exosomal miR-484,miR-652-3p,miR-142-3p,miR-21a-3p,and miR-21-5p regulate key pathways in bipolar disorder,autism spectrum disorder,and Rett syndrome(e.g.,TLR4,PI3K/Akt,and Epha4/TEK)and have an influence on mental disorders.Exosomal miRNAs are involved in the occurrence of mental disorders through TLR4,PI3K/Akt,and Epha4/TEK pathways,which provides a clearer understanding of disease cognition.Of these pathways,the TLR4 and PI3K/Akt pathways play a role in AD,MDD,and neurodevelopmental disorders,which can be used as an effective breakthrough in the study of mental disorders.Exosomal miRNAs could also serve as diagnostic biomarkers and therapeutic targets,providing new insights into precise interventions for mental disorders.
文摘A 1.5-hour recording of six (Gulf of Mexico) captive common bottlenose dolphins (Tursiops truncatus) at Sea Life Park, Hawaii shows that: 1) they have a song of a sequence of tonal (often slurred) notes. The evidence for their song comes from: a) their emitting songs that were complex (both tonally and rhythmically), b) their likely improvising (as inferred from their most complex songs not being repeated), c) songs based on a theme (used repeatedly), d) a bout of singing (at times) including social creativity, e) songs with a distinct beginning, middle and end section, and f) one song consisting of in part, a theme, 2) their songs are not sung in key (as determined from analysing the beginning tonal value of a note with spectral-frequency analysis), and 3) are sung in bouts, and 4) mostly with tonal-striated vocalizations (in the literature termed squawks and bray calls). Their apparent creativity (likely improvisations) and social creativity are of importance to how they evolved cognitively, to the study of song culture (between populations and delphinid species), understanding their (intra and inter-species) associations, and kinds of relationships, determining individual personalities, and perhaps will provide supporting evidence for their ability to reason. As inferred, they sing without being intentionally manipulative from: 1) their use of Gquic psychology [1] (as explained in this article), and 2) as inferred from their likely having an ability to reason [1] [2] (as deduced from their behavioural ecology (their unique exceptional evolutionary freedom), including their comparatively peaceful composure between conspecifics, in line with the proof of The Peaceful Composure Theorem), suggesting (per The Peaceful Composure Theorem) they have an egalitarian-like society. Their interspecies (displaced) aggression towards smaller odontocete species is shown to be a function of jealousies that emerge from their stronger social bonds than other species, from their compassionate nature, and females and males not pairing up in long-term associations. Species with greater cultural freedom are shown to have a more complex song providing further evidence for species culture not crucial to survival. The conclusive evidence for specie culture from my recent publications is of relevance to the scientific community’s acceptance of two new theories of evolution, and Gquic psychology. The loud burst-pulse sounds, chasing, charging and raking behaviours described in the literature could be a play behaviour, ought not be labelled as aggressive behaviours, as inferred from dog mock-fight play behaviour. In regard to future comparative behavioural ecology studies, aggressive animal behaviour should only be defined as an act of displacement (that is shown to commonly occur in the Indo-Pacific bottlenose dolphin species (Tursiops aduncus)), or as acts of biting, and hitting.
文摘Background:The endoscopic appearance of the major duodenal papilla influences biliary cannulation and complications.This study aimed to investigate the role of major duodenal papillae in the endoscopic treatment of common bile duct(CBD)stones.Methods:This retrospective study was conducted at Bishan Hospital of Chongqing Medical University between January 2018 and August 2022.Patients with native papillae who underwent endoscopic treatment for CBD stones were recruited and divided into four groups according to Haraldsson's classification of papillae(typesⅠ-Ⅳ).Univariate and multivariate logistic regression analyses were used to identify risk factors for difficult cannulation and post-endoscopic retrograde cholangiopancreatography(ERCP)pancreatitis(PEP).Results:A total of 596 patients with CBD stones were enrolled.The proportion of patients with typeⅠpapilla was the highest(n=231,38.8%),followed by typeⅢpapilla(n=175,29.4%),typeⅣpapilla(n=101,16.9%)and typeⅡpapilla(n=89,14.9%).Difficult cannulation occurred in 188 of 596 patients(31.5%),with most cases occurring in those with typeⅢpapilla(71/175,40.6%,P=0.020).Multivariate logistic analysis revealed that age[odds ratio(OR)=1.034,95%confidence interval(CI):1.021–1.047,P<0.001],typeⅢpapilla(OR=2.255,95%CI:1.439–3.535,P<0.001),gallbladder in situ(OR=2.486,95%CI:1.346–4.590,P=0.004),and CBD diameter<10 mm(OR=1.600,95%CI:1.049–2.441,P=0.029)were risk factors for difficult cannulation.The total incidence of PEP was 10.9%.Compared with the other types of papillae,the rate of PEP was the highest in those with typeⅠpapilla(15.2%,P=0.030).Multivariate analysis demonstrated that PEP was associated with difficult cannulation(OR=1.811,95%CI:1.044–3.143,P=0.035)and white blood cells(WBCs)<10×10~9/L(OR=2.199,95%CI:1.051–4.600,P=0.036).Conclusions:The endoscopic appearance of the major papilla is an important factor that influences both biliary cannulation and outcomes.TypeⅢpapilla is more frequently difficult to cannulate in the endoscopic treatment of CBD stones.

文摘Based on micro-survey data from 894 farmers in Sichuan Province,this study employs the Ordinary Least Squares(OLS),Two-Stage Least Squares(2SLS),mediating effect,and moderating effect models to comprehensively analyze the mechanisms and effects of digital literacy in empowering farmers’common prosperity.Endogeneity and robustness are addressed using the Sobel test,the Bootstrap,and moderating effect plots.The findings indicate that first,after controlling for variables such as age,gender,and planting years,digital literacy significantly and positively influences the common prosperity of farmers at the 5%level;second,enhanced digital literacy promotes higher levels of technology adoption and information accumulation among farmers,thereby increasing the probability of achieving common prosperity.The mediating effects of technology adoption and information sharing account for 21.10%and 33.94%of the total effect,respectively;and third,the information environment and financial empowerment play positive moderation roles in the effect of digital literacy on farmers’common prosperity,with the moderation effects significant at the 1%and 10%levels,respectively.Therefore,we recommend policies that enhance the access of farmers to digital literacy training,strengthen rural digital infrastructure,establish integrated rural information-sharing platforms,and create an enabling environment for the adoption of digital technologies.

文摘Herbicide safeners alleviate herbicide toxicity while preserving weed-control efficacy in common buckwheat.A three-year field experiment was performed to measure the effects of quizalofop-P-ethyl(QPE)alone or in combination with gibberellin or brassinolide on soil enzyme activity and yield components in common buckwheat.The herbicide-brassinolide application yielded the greatest increases in soil enzyme activity and yield components without sacrificing weed control efficacy.It is recommended for use in common buckwheat cultivation in the Loess Plateau.

文摘[Objectives]To systematically evaluate the impact of interventions based on the Common Sense Model of Self-Regulation(CSM)on the psychosocial adaptation of young and middle-aged patients with acute coronary syndrome(ACS),providing evidence-based support for clinical practice.[Methods]A systematic review was conducted using a literature search method,systematically searching through Chinese and English databases such as PubMed,Embase,Cochrane Library,CINAHL,China National Knowledge Infrastructure(CNKI),Wanfang Database,and VIP Database,from the database inception to December 31,2024.The search focused on studies related to the impact of the CSM on the psychosocial adaptation of young and middle-aged ACS patients.Two researchers independently performed literature screening,quality assessment,and data extraction.[Results]A total of 18 studies were included,comprising 12 randomized controlled trials,4 quasi-experimental studies,and 2 cohort studies,involving 2847 young and middle-aged ACS patients.Interventions based on the CSM significantly improved patients disease perception,emotional regulation,self-efficacy,and quality of life.Patients in the intervention group showed significant reductions in anxiety and depression levels,cardiac-related fear,and improvements in disease perception accuracy,treatment adherence,and social function recovery.[Conclusions]Interventions based on the CSM can effectively promote the psychosocial adaptation of young and middle-aged ACS patients,improve their disease perception and emotional state,and enhance their quality of life.It is recommended that this model be widely applied in the clinical care of young and middle-aged ACS patients.

文摘To enhance energy interaction among low-voltage stations(LVSs)and reduce the line loss of the distribution network,a novel operation mode of the micro-pumped storage system(mPSS)has been proposed based on the common reservoir.First,some operation modes of mPSS are analyzed,which include the separated reservoir mode(SRM)and common reservoir mode(CRM).Then,based on the SRM,and CRM,an energy mutual assistance control model between LVSs has been built to optimize energy loss.Finally,in the simulation,compared to the model without pumped storage in the LVS,the SRMand CLRMcan decrease the total energy loss by 294.377 and 432.578 kWh,respectively.The configuration of mPSS can improve the utilization rate of the new energy source generation system,and relieve the pressure of transformer capacity in the LVS.Compared with the SRM,the proposed CRM has reduced the total energy loss by 138.201 kWh,increased the new energy consumption by 161.642 kWh,and decreased the line loss by 7.271 kWh.With the efficiency of the mPSS improving,the total energy loss reduction of CRM will be 3.5 times that of SRM.Further,the CRMcan significantly reduce the reservoir capacity construction of mPSS and ismore suitable for scenarios where the capacity configuration of mPSS is limited.

文摘With the increasing importance of multimodal data in emotional expression on social media,mainstream methods for sentiment analysis have shifted from unimodal to multimodal approaches.However,the challenges of extracting high-quality emotional features and achieving effective interaction between different modalities remain two major obstacles in multimodal sentiment analysis.To address these challenges,this paper proposes a Text-Gated Interaction Network with Inter-Sample Commonality Perception(TGICP).Specifically,we utilize a Inter-sample Commonality Perception(ICP)module to extract common features from similar samples within the same modality,and use these common features to enhance the original features of each modality,thereby obtaining a richer and more complete multimodal sentiment representation.Subsequently,in the cross-modal interaction stage,we design a Text-Gated Interaction(TGI)module,which is text-driven.By calculating the mutual information difference between the text modality and nonverbal modalities,the TGI module dynamically adjusts the influence of emotional information from the text modality on nonverbal modalities.This helps to reduce modality information asymmetry while enabling full cross-modal interaction.Experimental results show that the proposed model achieves outstanding performance on both the CMU-MOSI and CMU-MOSEI baseline multimodal sentiment analysis datasets,validating its effectiveness in emotion recognition tasks.
文摘BACKGROUND Textbook outcome(TO),an emerging composite metric for surgical quality assessment,has recently gained recognition for evaluating perioperative results.Laparoscopic transcystic common bile duct exploration(LTCBDE)has become a widely adopted minimally invasive technique for treating cholecystolithiasis with choledocholithiasis.Despite its growing clinical application,TO has not yet been formally defined for LTCBDE,nor have its failure-associated risk factors been systematically examined.AIM To define TO for LTCBDE,establish standardized criteria,and identify risk factors for TO failure via logistic regression.METHODS A retrospective cohort of 388 patients who underwent LTCBDE in combination with laparoscopic cholecystectomy at the Department of Biliopancreatic Surgery,Tongji Hospital,from January 2018 to October 2024,was analyzed.The study delineated TO criteria for LTCBDE,calculated the rate of TO achievement,and employed logistic regression to determine independent predictors of TO failure.RESULTS TO was defined as the absence of the following seven criteria:Conversion to open surgery,postoperative complications(Clavien-Dindo grade≥2),biliary leakage(International Study Group of Pancreatic Surgery/International Study Group of Liver Surgery grade B/C),delayed removal of drainage tube(>4 days),postoperative interventions,prolonged length of stay(>7 days),and 30-day readmission or mortality.Among 388 patients,276(71.1%)achieved TO.The primary causes of TO failure included delayed removal of drainage tube(94 cases,83.9%),prolonged length of stay(50 cases,44.6%).Multivariate analysis revealed four independent risk factors for TO failure:Preoperative endoscopic retrograde cholangiopancreatography(P=0.022),advanced age(P=0.010),prolonged anesthesia time(P<0.001),and elevated preoperative alkaline phosphatase levels(P=0.048).These findings suggest that applying the concept of TO to LTCBDE enhances surgical quality evaluation and supports early identification of high-risk patients,facilitating personalized clinical decisions and optimizing individual management.
