Objective To explore the effect of acupuncture at "Jing Jiaji" (颈夹背) on sudden hearing loss by observing the changes of the threshold of auditory brainstem response (ABR) of deaf guinea pigs. Methods Sixty healthy guinea pigs were selected, with computer randomization, 20 were randomly divided into a normal control group (group A), while the rest were randomly and equally divided into a model group (group B) and an acupuncture group (group C) when turned deaf after gentamycin sulfate and furosemide were administered. No treatment was administered for either the group A or the group B, while acupuncture at "Jing Jiaji" was given for the group C for 30 days. The ABR thresholds of each group were observed and recorded separately after the treatment for 15 days and 30 days. Results The ABR threshold in the group C was lower than that in the group B after the treatment of 15 days (P〈O.05). After the treatment of 30 days, the ABR threshold in the group C was obviously lower than that in the group B (P〈0.01), and was obviously lower than that before acupuncture (P〈0.01). Conclusion Acupuncture at "Jing Jiaji" has definite effect on improving the hearing of deaf guinea pigs. 展开更多

To characterize the patterns of ABR waves in tree shrews, we must understand the hearing sensitivity and auditory function of healthy adult tree shrews. Fifteen tree shrews(30 ears) were stimulated with clicks and tone-pips at 11 different frequencies from 1 to 60 kHz. The ABR waves were recorded and analyzed. The ABR consisted of five to seven positive waves in the first 10 ms after a click stimulus, and the average hearing threshold of component III was 27.86 ± 3.78 dB SPL. Wave III was the largest and most clear. The ABR threshold was related to the tone-pip sitmulus by a "U" shaped curve. The sensitive frequency was approximately 8 kHz in tree shrews. The latencies systematically decreased with increasing stimulus frequencies. The ABR amplitudes of wave III increased as the sound pressure level increased. All of these results provide an empirical basis for future studies of hearing diseases in tree shrews. 展开更多

The auditory brainstern responses(ABR)and middle latency responses(MLR)were recorded in 24 normal mice with a 7S11A processor,and the normalparameters of ABR and MLR obtained.The results showed that the ABR of mice werecomposed of 6 waves,waves Ⅰ to Ⅵ.The mean threshold was 35.12±8.03 dB ,whichwas significantly different from the 22.63 dB of ABR found in guinea pigs.Thelatency of ABR waves Ⅰ to Ⅵizn the mice was found to be 1.30±8.03 ms ;2.02±0.24ms ;2.90±0.27 ms ;3.80±0.24 ms ;4.87±0.42 ms and 5.83±0.40 ms respectively.Nosex or strain differences were found.The MLR waveform of the mice were similar tothose of human and the latency was found to be P0 3.94±1.76 ms;P8.38±1.57ms;Na 14.98±2.54 ms;Pa 24.56±3.15 ms rcspectively.It is suggested that mice can beused for hearing research.Since the Na and Pa waves of MLR were unstable under anes-thesia,testing conditions for determining MLR in mice need further study. 展开更多

Methods Fourteen presbycusis patients elder than 90 years (presbycusis group, 91.1.4 ± 1.3 years, 26 ears) and 9 normal-hearing young adults (control group, 22.7 ± 1.2 years, 18 ears) participated in the study. Alternative click-evoked ABRs were recorded in both groups. The peak latency (PL) of peak I,Ⅲ, and V, and the inter-peak latency (IPI) of I-Ⅲ,Ⅲ-V, and I-V were compared between groups. Results In elder presbycusis patients, the occurrence rate of peak I andⅢwere both 76.9%, and that of peak V was 84.6%. In presbycusis group, the peak latencies of I, Ⅲ, V were significantly longer than that of control group (P<0.001). There was no significant difference between groups in the IPI of peak I-IⅢ (P=0.298, peakⅢ-V (P=0.254) and peak I-V (P=0.364). Conclusions Auditory brainstem responses in presbycusis pa-tients elder than 90 years showed worse wave differentiation. 展开更多

Objectives To investigate changes in evoked potentials and structure of the guinea pig cochleae during whole cochlear perfusion with glutamate.Methods CM,CAP,DPOAE,and ABR were recorded as indicators of cochlear functions during whole cochlear perfusion.The morphology of the cochlea was studied via transmission electron microscopy.Results There were no significant changes in DPOAE amplitude before and after glutamate perfusion.CM I/O function remained nonlinear during perfusion.ABR latencies were delayed following glutamate perfusion.The average CAP threshold was elevated 35 dB SPL following glutamate perfusion..The OHCs appeared normal,but the IHCs and afferent dendrites showed cytoplasmic blebs after glutamate perfusion.Conclusions While being a primary amino acid neurotransmitter at the synapses between hair cells and spiral ganglion neurons,excessive glutamate is neurotoxic and can destroy IHCs and spiral ganglion neurons.The technique used in this study can also be used to build an animal model of auditory neuropathy. 展开更多

The analysis of the Auditory Brainstem Response (ABR) is of fundamental importance to the investigation of the auditory system behavior, though its interpretation has a subjective nature because of the manual process employed in its study and the clinical experience required for its analysis. When analyzing the ABR, clinicians are often interested in the identification of ABR signal components referred to as Jewett waves. In particular, the detection and study of the time when these waves occur (i.e., the wave latency) is a practical tool for the diagnosis of disorders affecting the auditory system. In this context, the aim of this research is to compare ABR manual/visual analysis provided by different examiners. Methods: The ABR data were collected from 10 normal-hearing subjects (5 men and 5 women, from 20 to 52 years). A total of 160 data samples were analyzed and a pair- wise comparison between four distinct examiners was executed. We carried out a statistical study aiming to identify significant differences between assessments provided by the examiners. For this, we used Linear Regression in conjunction with Bootstrap, as a method for evaluating the relation between the responses given by the examiners. Results: The analysis suggests agreement among examiners however reveals differences between assessments of the variability of the waves. We quantified the magnitude of the obtained wave latency differences and 18% of the investigated waves presented substantial differences (large and moderate) and of these 3.79% were considered not acceptable for the clinical practice. Conclusions: Our results characterize the variability of the manual analysis of ABR data and the necessity of establishing unified standards and protocols for the analysis of these data. These results may also contribute to the validation and development of automatic systems that are employed in the early diagnosis of hearing loss. 展开更多

Tinnitus has often been studied using salicylate in animal models as they are capable of inducing tempo-rary hearing loss and tinnitus. Studies have recently observed enhancement of auditory evoked responses of the auditory cortex (AC) post salicylate treatment which is also shown to be related to tinnitus like behavior in rats. The aim of this study was to observe if enhancements of the AC post salicylate treatment are also present at structures in the brainstem. Four male Sprague Dawley rats with AC implanted electrodes were tested for both AC and auditory brainstem response (ABR) recordings pre and post 250 mg/kg intraperitone-al injections of salicylate. The responses were recorded as the peak to trough amplitudes of P1-N1 (AC), ABR wave V, and ABR waveⅡ. AC responses resulted in statistically significant enhancement of ampli-tude at 2 hours post salicylate with 90 dB stimuli tone bursts of 4, 8, 12, and 20 kHz. Wave V of ABR re-sponses at 90 dB resulted in a statistically significant reduction of amplitude 2 hours post salicylate and a mean decrease of amplitude of 31%for 16 kHz. WaveⅡamplitudes at 2 hours post treatment were signifi-cantly reduced for 4, 12, and 20 kHz stimuli at 90 dB SPL. Our results suggest that the enhancement chang-es of the AC related to salicylate induced tinnitus are generated superior to the level of the inferior colliculus and may originate in the AC. 展开更多

听力学联合检测技术在突发性耳聋诊断中的应用

目的探究听力学联合检测技术在突发性耳聋(sudden deafness,SD)诊断中的临床应用价值。方法选取2022年1月至2024年3月在连云港市立东方医院初步诊断为SD的患者298例。其中,女性135例,男性163例,平均年龄(53.3±3.5)岁。分别采用声... 目的探究听力学联合检测技术在突发性耳聋(sudden deafness,SD)诊断中的临床应用价值。方法选取2022年1月至2024年3月在连云港市立东方医院初步诊断为SD的患者298例。其中,女性135例,男性163例,平均年龄(53.3±3.5)岁。分别采用声导抗、纯音测听、耳声发射及听性脑干反应听力学技术联合检测,评估中耳、耳蜗、听神经及听觉中枢功能。对于有耳蜗后听力学检查异常者,行影像学检查。结果298例患者声导抗图均为A型;纯音测听检测结果为感音神经性听力丧失;耳声发射检查异常283例,耳声发射正常15例;听性脑干反应I、III、V波潜伏期延长16例,其中I~V峰间期延长7例,III~V峰间期延长9例。经脑核磁共振成像检查提示听神经瘤9例,脑内巨大血管瘤2例,脑内转移癌2例,大脑局灶梗死3例。SD低频下降型治愈率72.9%,平坦下降型治愈率57.9%,高频下降型好转率40.8%,全聋型效果最差,治愈率0,无效率42.3%。9例听神经瘤患者术后术耳极重度听力丧失。脑内血管瘤2例,经手术治疗,1例听力好转。2例脑内转移癌患者因听力丧失治疗无效。结论研究显示多种听力学联合检测技术在SD诊疗中具有较高应用价值,对耳聋的定性、定位、定量以及鉴别诊断方面具有显著的协同作用,提高了诊断的准确性和可靠性,减少了误诊和漏诊,对疾病的转归和个体化治疗提供了帮助。 展开更多

目的探究国产人工耳蜗植入者使用电诱发听性脑干反应(electrically evoked stem responses,EABR)和行为测听测定人工耳蜗电流级阈值(T值)的相关性,通过EABR测试客观设定T值进行人工耳蜗编程的可行性。方法选取2023年宁波大学附属第一医... 目的探究国产人工耳蜗植入者使用电诱发听性脑干反应(electrically evoked stem responses,EABR)和行为测听测定人工耳蜗电流级阈值(T值)的相关性,通过EABR测试客观设定T值进行人工耳蜗编程的可行性。方法选取2023年宁波大学附属第一医院植入国产人工耳蜗的患者22例(25耳),其中,男性9例,女性13例,平均年龄(21.92±11.30)岁,19例为单侧耳蜗植入,3例为双侧耳蜗植入。分别选取1、7、13、19、24号电极,测定能引出EABR和行为测试的最小电流刺激量;将行为测试测定的电刺激阈值设定为T值,并设置为人工耳蜗植入的程序①;根据EABR测试的电刺激阈值作为T值,设置为程序②,患者分别使用程序①、②进行助听听阈检测,分析两个助听听阈和两个T值之间的相关性。结果第1、7、13、19、24号电极的V波潜伏期分别为(3.30±0.33)ms、(3.41±0.30)ms、(3.52±0.33)ms、(3.59±0.27)ms、(3.54±0.46)ms;EABR测试的T值分别为(83.20±10.40)CL、(89.20±13.52)CL、(101.00±15.48)CL、(102.20±19.26)CL、(110.80±18.97)CL;行为测听测定的T值分别为(79.00±18.91)CL、(80.04±17.04)CL、(86.64±19.38)CL、(90.68±17.37)CL、(104.60±17.91)CL;二者之间存在显著相关性(P<0.05)。两次助听听阈极显著相关(P<0.01)。结论国产人工耳蜗EABR测得的电刺激阈值和行为测听测得的电流级阈值之间存在显著相关性,可使用EABR测试来辅助国产人工耳蜗的编程。 展开更多

目的探讨将短声听性脑干反应(c-ABR)与多频稳态反应(ASSR)联合应用于评估非综合征型腭裂(NSCP)并发分泌性中耳炎患儿听力状况的临床价值。方法回顾性分析2017年1月—2023年12月确诊的36月龄以下无法配合行为测听NSCP传导性听力损失严重... 目的探讨将短声听性脑干反应(c-ABR)与多频稳态反应(ASSR)联合应用于评估非综合征型腭裂(NSCP)并发分泌性中耳炎患儿听力状况的临床价值。方法回顾性分析2017年1月—2023年12月确诊的36月龄以下无法配合行为测听NSCP传导性听力损失严重程度,根据c-ABR检测中V波阈值判断严重程度,同时结合ASSR判断此类患儿听力损失频率分布,采用Spearman系数评估c-ABR与ASSR的相关性,并依据年龄、性别、腭裂程度进行分组比较。结果本研究筛选了588例NSCP,最终纳入330例(660耳),其中男103例(206耳),女227例(454耳)。根据c-ABR将听力损失严重程度分为正常(118耳)、轻(442耳)、中(100耳)。以ASSR的结果补充低频听力,其中0.5 kHz的未引出率较高。整体上c-ABR与ASSR阈值相关性为2 kHz相关性最高(r=0.359),按性别、年龄、腭裂程度分层后,其中婴儿期r=0.350,幼儿期r=0.433;男r=0.255,女r=0.386;腭裂Ⅱ度r=0.331,Ⅲ度r=0.747以上相关性均具有统计学意义(P<0.01)。结论c-ABR与ASSR是NSCP听力学评估的重要工具,有助于NSCP传导性听力损失的早期诊断与干预,当腭裂程度为Ⅲ度c-ABR和ASSR结果高度相关,且需重视0.5 kHz的听力补充检查;在临床检测中需互相补充,二者联合应用更有利于准确评估NSCP听力损失程度。 展开更多

目的探讨噪声性隐匿性听力损失(NIHHL)发生后是否会导致小鼠记忆功能障碍,并分析记忆相关蛋白N-甲基-D-天冬氨酸受体2A(NR2A)和N-甲基-D-天冬氨酸受体2B(NR2B)在NIHHL小鼠海马区的表达变化。方法采用12只听力正常的雄性C57小鼠,随机分为... 目的探讨噪声性隐匿性听力损失(NIHHL)发生后是否会导致小鼠记忆功能障碍,并分析记忆相关蛋白N-甲基-D-天冬氨酸受体2A(NR2A)和N-甲基-D-天冬氨酸受体2B(NR2B)在NIHHL小鼠海马区的表达变化。方法采用12只听力正常的雄性C57小鼠,随机分为2组,每组6只。NIHHL组接受110 dB噪声暴露2 h,CON组不予噪声暴露。在噪声暴露前、暴露后1 d和1个月分别进行听性脑干反应(ABR)测试及行为学实验;噪声暴露后1个月,取两组小鼠海马组织和双侧耳蜗组织,采用Western blotting检测海马区NR2A和NR2B蛋白表达,利用免疫荧光染色检测耳蜗带状突触数量。结果与CON组相比,NIHHL组在噪声暴露后1 d听阈升高(P<0.05),ABRⅠ波幅值降低(P<0.01),Ⅰ波潜伏期延长(P<0.05);噪声暴露后1个月,NIHHL组听阈和ABRⅠ波潜伏期恢复至正常水平(P>0.05),但ABRⅠ波幅值仍较低(P<0.05),耳蜗内毛细胞带状突触数量减少(P<0.05)。行为学测试显示:水迷宫中首次抵达原平台位置的潜伏期延长(P<0.01),在目标象限的游泳距离缩短(P<0.05);Y迷宫中自主交替率降低(P<0.01);新物体识别中认知指数降低(P<0.01)。此外,海马区NR2B蛋白表达下调(P<0.05)。结论NIHHL小鼠海马区NR2B蛋白表达下调,可能与其记忆功能障碍的发生发展有关。 展开更多

Vocalizations play a critical role in mate recognition and mate choice in a number of taxa, especially, but not limited to, orthopterans, frogs, and birds. But receivers can only recognize and prefer sounds that they can hear. Thus a fundamental question linking neurobiology and sexual selection asks-what is the threshold for detecting acoustic sexual displays? In this study, we use 3 methods to assess such thresholds in tdngara frogs: behavioral responses, auditory brainstem responsesz and multi unit electrophysiological recordi ngs from the midbrain.We show that thresholds are lowest for multiunit recordings (ca. 45 dB SPL), and then for behavioral responses (ca. 61 dB SPL), with auditory brainstem responses exhibiting the highest thresholds (ca. 71 dB SPL). We discuss why these estimates differ and why, as with other studies, it is unlikely that they should be the same. Although all of these studies estimate thresholds they are not measuring the same thresholds;behavioral thresholds are based on signal salienee whereas the 2 neural assays estimate physiological thresholds. All 3 estimates, however, make it clear that to have an appreciation for detection and salienee of acoustic signals we must listen to those signals through the ears of the receivers. 展开更多

In anurans, calling behaviour is strongly seasonal and circadian. Previous studies have revealed that a uditory sensitivity in frogs exhibits seasonal plasticity, and electroencephalographic signals exhibit highly correlated circadian patterns;of which, the circadian rhythm remains unknown. In this study,the circadian rhythm and intersexual differences of auditory sensitivity were tested in the Emei music frog(Nidirana daunchina). This was achieved by comparing thresholds and latencies of auditory brainstem responses(ABRs) evoked by tones and clicks stimuli between male and female frogs during the day and at night,respectively. Our results revealed that both auditory thresholds and latencies had no differences between day and night except the la tencies in 3.5–4.0 kHz frequencies. However, the thresholds of tone pip evoked ABRs differed significantly between male and female frogs from 2.5 to 5.0 kHz. This demonstrated that the auditory sensitivity of Emei music frogs exhibits sexual dimorphism at high frequencies, with female frogs exhibiting greater auditory sensitivity than that of male frogs. Simultaneously, the power spectra of male advertisement calls are matched well with the frequency range of auditory sensitivity in male and female frogs,which supports the matched filter hypothesis. Our study enhances the understanding of circadian plasticity and sexual dimorphism of auditory sensitivity in frogs. 展开更多

Objective:To analyze neurophysiological characteristics in infants and young children with auditory neuropathy(AN) and explore their clinical significance.Methods: Audiological measurements(acoustic immittance, EOAEs, ABR, CM, MLR and ERPs) and peripheral neurological tests were conducted and evaluated in 13 infants and young children with AN. Six of them received high-resolution temporal bone CT scans and/or cerebral MRI examinations.Results: All of the children showed type “A” tympanograms with abseatation of stapedial reflexes. EOAEs were normal in 12 of 13 subjects. In one child who had a history of anoxia during the birth,the EOAEs were not elicited. Click-evoked ABRs were absent in 12 of 13 subjects when maximum output of the instrument was reached. The CM potentials were presented bilaterally in all individuals,which were independent of the EOAEs and ABR. Of eight cases tested,all had clear MLR and six showed normal ERPs(P300 and MMN). Peripheral neurological tests and radiological findings were within the normal ranges. Conclusion: The diagnosis of AN in infants and young children should focus on analyzing their neurophysiological characteristics,especially on CM,MLR and ERPs. Combined use of EOAEs, ABR and CM was recommended for hearing screening on newborns with high risk factors. 展开更多

目的研究感音神经性听力损失婴幼儿及儿童听性脑干反应(ABR)阈值与行为测听阈值的相关性,并获得短纯音ABR(tb-ABR)与相应频率行为测听的校准因子,为预估行为测听阈值提供临床参考。方法选取2014年2月至2022年9月耳鼻咽喉头颈外科确诊为... 目的研究感音神经性听力损失婴幼儿及儿童听性脑干反应(ABR)阈值与行为测听阈值的相关性,并获得短纯音ABR(tb-ABR)与相应频率行为测听的校准因子,为预估行为测听阈值提供临床参考。方法选取2014年2月至2022年9月耳鼻咽喉头颈外科确诊为感音神经性听力损失的婴幼儿及儿童作为研究对象,对患儿行短声ABR(click-ABR)及500、1000、2000和4000 Hz频率的Tb-ABR测试,并在随访中获取其行为测听阈值。分析不同频率声刺激下ABR阈值与行为测听阈值的相关性,比较Tb-ABR阈值与行为测听阈值差异,并建立回归方程及获得校准因子。结果纳入感音神经性听力损失患儿61例(118耳),其中男40例(77耳)、女21例(41耳)。完成ABR中位年龄1.9(0.7~3.4)岁,完成行为测听中位年龄3.4(2.0~4.4)岁。click-ABR阈值与2000 Hz、4000 Hz行为测听阈值的相关系数分别为0.73和0.74。Tb-ABR阈值与500 Hz、1000 Hz、2000 Hz、4000 Hz行为测听阈值间呈强相关性(r=0.82~0.87,P<0.01)。基于Tb-ABR阈值预估行为测听阈值的线性回归方程如下,500Hz:y=0.87x+2.55,1000 Hz:y=0.89x+4.29,2000 Hz:y=0.79x+14.70,4000 Hz:y=0.99x+0.84。当500、1000、2000和4000 Hz频率下Tb-ABR阈值在50、70和90 dB nHL时,校准因子分别为5、10、10 dB,5、5、10 dB,0、5、5 dB以及0、0、0 dB。结论Tb-ABR阈值与行为测听阈值具有高度相关性,获得的校准因子及回归方程可有效预估行为测听阈值,为感音神经性听力损失婴幼儿及儿童后期助听干预精准调试提供了重要参考。 展开更多

目的分析分泌性中耳炎(OME)婴幼儿气导短声(c-ABR)和短纯音听性脑干反应(Tb-ABR)阈结果,探讨气导Tb-ABR反应阈对OME婴幼儿分频听力评估的临床意义。方法回顾性分析2018年1月至2022年12月完成ABR反应阈测试的0~24月龄OME婴幼儿以及正常... 目的分析分泌性中耳炎(OME)婴幼儿气导短声(c-ABR)和短纯音听性脑干反应(Tb-ABR)阈结果,探讨气导Tb-ABR反应阈对OME婴幼儿分频听力评估的临床意义。方法回顾性分析2018年1月至2022年12月完成ABR反应阈测试的0~24月龄OME婴幼儿以及正常对照婴幼儿的测试结果。比较两组间气导c-ABR和Tb-ABR反应阈,分析OME组内气导c-ABR和Tb-ABR反应阈相关性。结果OME组43例,男28例、女15例,中位年龄为8(3~14)月龄;气导c-ABR完成检测43例(60耳)、Tb-500 Hz ABR完成检测32例(45耳)、Tb-1000 Hz ABR完成检测27例(39耳)、Tb-2000 Hz ABR完成检测11例(17耳)、Tb-4000 Hz ABR完成检测11例(17耳)。对照组21例,男10例、女11例,中位年龄为6(4~8.25)月龄;气导c-ABR和4个频率Tb-ARB完成检测21例(42耳)。OME组气导c-ABR和Tb-500 Hz、Tb-1000 Hz、Tb-2000 Hz、Tb-4000 Hz ABR反应阈均高于正常对照组(P<0.01)。OME组内气导Tb-ABR 4个频率反应阈均高于气导c-ABR反应阈(P<0.05)。各频率反应阈间呈正相关性(P<0.05),Tb-2000 Hz与Tb-4000 Hz ABR反应阈间相关系数为0.925。结论OME婴幼儿在低、中、高频均可能发生气导听力下降;与气导c-ABR相比,气导Tb-ABR反应阈可提供更多分频听力信息;测试时间有限时,Tb-2000 Hz和Tb-4000 Hz ABR或可选其一完成。 展开更多

目的分析2286例新生儿听力筛查与相关随访结果。方法选取2019年1月1日至2023年12月31日在解放军总医院第四医学中心产科出生的活产儿2286例,初筛采用畸变耳声发射(distortion product otoacoustic emission,DPOAE),未通过新生儿42 d时... 目的分析2286例新生儿听力筛查与相关随访结果。方法选取2019年1月1日至2023年12月31日在解放军总医院第四医学中心产科出生的活产儿2286例,初筛采用畸变耳声发射(distortion product otoacoustic emission,DPOAE),未通过新生儿42 d时复筛采用DPOAE联合自动听性脑干反应(automatic auditory brainstem response,AABR);对出生后未初筛新生儿77例和初筛未通过且未按时复筛新生儿30例进行电话随访,对筛查、随访结果进行统计和分析。结果初筛率97.5%,初筛通过率94.8%,复筛率89.2%,复筛通过率70.1%。右耳初筛通过率96.7%高于左耳初筛通过率95.2%,女婴初筛通过率96.1%高于男婴初筛通过率93.5%。复筛107例(214耳)中,DPOAE联合AABR筛查未通过率19.6%高于单独DPOAE或AABR未通过率7.5%。2019年至2023年出生后未初筛新生儿电话随访率87.0%,同期初筛未通过且未按时复筛新生儿电话随访率96.7%。结论本中心新生儿听力初筛率、初筛通过率和复筛率均达到指南要求;不同耳别和性别影响新生儿初筛通过率;DPOAE联合AABR复筛未通过率高于单一方法,支持联合应用以减少漏诊;相关随访率未达到100%,需加强随访管理及针对性宣教,可进一步提升新生儿听力保健水平,以减少漏诊与延迟干预的风险。 展开更多
