Although the vast majority of meta-analyses have confirmed that exercise interventions can increase brain-derived neurotrophic factor levels in children and adolescents, the effects of specific types of exercise on brain-derived neurotrophic factor levels are still controversial. To address this issue, we used meta-analytic methods to quantitatively evaluate, analyze, and integrate relevant studies. Our goals were to formulate general conclusions regarding the use of exercise interventions, explore the physiological mechanisms by which exercise improves brain health and cognitive ability in children and adolescents, and provide a reliable foundation for follow-up research. We used the Pub Med, Web of Science, Science Direct, Springer, Wiley Online Library, Weipu, Wanfang, and China National Knowledge Infrastructure databases to search for randomized controlled trials examining the influences of exercise interventions on brain-derived neurotrophic factor levels in children and adolescents. The extracted data were analyzed using Review Manager 5.3. According to the inclusion criteria, we assessed randomized controlled trials in which the samples were mainly children and adolescents, and the outcome indicators were measured before and after the intervention. We excluded animal experiments, studies that lacked a control group, and those that did not report quantitative results. The mean difference(MD;before versus after intervention) was used to evaluate the effect of exercise on brain-derived neurotrophic factor levels in children and adolescents. Overall, 531 participants(60 children and 471 adolescents, 10.9–16.1 years) were included from 13 randomized controlled trials. Heterogeneity was evaluated using the Q statistic and I^(2) test provided by Review Manager software. The meta-analysis showed that there was no heterogeneity among the studies(P = 0.67, I^(2) = 0.00%). The combined effect of the interventions was significant(MD = 2.88, 95% CI: 1.53–4.22, P < 0.0001), indicating that the brain-derived neurotrophic factor levels of the children and adolescents in the exercise group were significantly higher than those in the control group. In conclusion, different types of exercise interventions significantly increased brain-derived neurotrophic factor levels in children and adolescents. However, because of the small sample size of this meta-analysis, more high-quality research is needed to verify our conclusions. This metaanalysis was registered at PROSPERO(registration ID: CRD42023439408).展开更多

It is associated with both maternal and perinatal morbidity and mortality. The general objective of our study is to determine the prevalence and profile of pregnant women, and to assess the maternal and perinatal prognosis of adolescent childbirth in Kisangani. Methods: This was a prospective, multicenter, case-control observational study conducted over a seven-month period, from February 1 to August 31, 2024, in primiparous adolescent gestational carriers (cases) and primiparous gestational carriers aged 20 to 34 years (controls) who delivered in five health facilities in the city of Kisangani, Democratic Republic of Congo. Results: The prevalence of teenage childbirth was 13.8%. Adolescents were more likely than controls to be in secondary education and to be unemployed. Compared with controls, pregnant adolescents were more likely to have poor ANC attendance. There was a statistical difference between the two groups in relation to pelvic anomaly, rupture of membranes on admission, hypertensive disorders, vicious presentation, caesarean section, episiotomy, postpartum anaemia and puerperal psychosis. In fact, these morbidities were more common in adolescent girls than in controls. Compared with controls, neonatal depression, prematurity, low birth weight and perinatal death were more prevalent in the newborns of teenage mothers. Conclusion: The prevalence of teenage childbirth is high in Kisangani;there is an association between unmarried status, lack of employment, low socio-economic status, poor ANC follow-up and teenage childbirth in Kisangani. The latter is also associated with high maternal and perinatal morbidity and mortality.展开更多

The aim of this study was to determine the factors influencing the maintenance of children and adolescents on antiretroviral therapy in the continuum of care. Methodology: This was a descriptive and analytical cross-sectional study conducted from August 1 to August 31, 2023. It included all children living with HIV, under 15 years of age, with at least two viral load results and whose parents consented to participate in the study. Participants were recruited during their child’s treatment renewal consultations. Results: The study included 143 children, mostly boys (55.2%), with a mean age of 11.54 years (±2.8). More than half (55.2) were unaware of their HIV-positive status, and most treatments were administered by parents (60.8%). Of the 99 children with an undetectable viral load at the first test, 23 (23.2%) showed a viral rebound at the last test, mainly due to poor treatment compliance (p Conclusion: Virological rebound after suppression in children is worrying. It is crucial that the national AIDS program improves Therapeutic Education, trains health workers to communicate results and encourages ongoing dialogue with young people to reinforce adherence and maintain viral suppression.展开更多

The current study aimed to analyze the effect of independent and cumulative effects of ACEs exposure on the likelihood of Posttraumatic Stress Disorder (PTSD) and a Complex PTSD (CPSTD) diagnosis in Indian adolescents. Methods: A sample of 411 schoolchildren participated in the study. It was collected data on exposure to ACEs, PTSD, CPTSD, and attachment styles. Results: It was found that exposure to bullying and threats of violence was significantly associated with PTSD and CPTSD diagnosis, and exposure to physical violence and serious illness was associated with CPTSD diagnosis. Exposure to one ACE and exposure to 2 - 3 ACEs were associated with PTSD and CPTSD diagnosis, while exposure to 4 - 5 ACEs was associated with CPTSD diagnosis. Preoccupied and fearful attachment styles were significantly associated with PTSD and CPTSD diagnosis. Conclusion: The results of the study suggest that biological, psychological, and social factors interact and contribute to the differential prevalence of ACE, attachment styles, and PTSD/CPTSD. This study emphasizes the need of addressing childhood trauma as a public health priority in India.展开更多

文摘BACKGROUND Epidemiological surveys indicate an increasing incidence of type 2 diabetes mellitus(T2DM)among children and adolescents worldwide.Due to rapid disease progression, severe long-term cardiorenal complications,a lack of effective treatment strategies, and substantial socioeconomic burdens,it has become an urgent public health issue that requires management and resolution.Adolescent T2DM differs from adult T2DM.Despite a significant increase in our understanding of youth-onset T2DM over the past two decades, the related review and evidence-based content remain limited.AIM To visualize the hotspots and trends in pediatric and adolescent T2DM research and to forecast their future research themes.METHODS This study utilized the terms“children”,“adolescents”,and“type 2 diabetes”,retrieving relevant articles published between 1983 and 2023 from three citation databases within the Web of Science Core Collection(SCI,SSCI,ESCI).Utilizing CiteSpace and VoSviewer software,we analyze and visually represent the annual output of literature, countries involved, and participating institutions. This allows us to predict trends in this research field. Our analysis encompasses co-cited authors, journal overlays, citation overlays,time-zone views, keyword analysis, and reference analysis, etc.RESULTS A total of 9210 articles were included, and the annual publication volume in this field showed a steady growth trend. The United States had the highest number of publications and the highest H-index.The United States also had the most research institutions and the strongest research capacity. The global hot journals were primarily diabetes professional journals but also included journals related to nutrition, endocrinology, and metabolism.Keyword analysis showed that research related to endothelial dysfunction, exposure risk, cardiac metabolic risk, changes in gut microbiota, the impact on comorbidities and outcomes, etc.,were emerging keywords. They have maintained their popularity in this field, suggesting that these areas have garnered significant research interest in recent years.CONCLUSION Pediatric and adolescent T2DM is increasingly drawing global attention, with genes, behaviors, environmental factors, and multisystemic interventions potentially emerging as future research hot spots.
文摘Objective This study aimed to analyse the trend of the mental disorder spectrum in children and adolescents from 2014 to 2022 in one city in Central China and to provide actionable recommendations for the prevention and management of mental disorders. Methods In this hospital-based retrospective study,we utilized child and adolescent medical records data from the Wuhan Mental Health Center from January 2014 to December 2022 and examined the top 5 mental disorders(schizophrenia, depressive episode, bipolar disorder, pervasive developmental disorder, and unspecified mood disorder)that accounted for the overall proportion of patients admitted. The rank and proportion of these mental disorders, demographic characteristics and disease indicators were analysed. Results There was a significant upwards trend in the number of children and adolescents diagnosed with mental disorders over the past 9 years, with a sharp decline in 2020 due to the COVID-19 pandemic, followed by a rebound in 2021 and a sustained level above prepandemic figures by 2022.The average age of hospitalization decreased significantly from 20.7 to 16.2 years, with a marked increase in the 12-17-year-old age group. The proportion of female hospitalizations increased from 39.2%to 55.2%,with a corresponding decrease in male hospitalizations. There was a notable decrease in the proportion of schizophrenia cases and an ascent of depressive episode to the most prevalent position. Conclusion This study emphasizes the critical need for targeted interventions and resources for severe mental disorders in children and adolescents and the importance of early detection and management of mental disorders to mitigate long-term effects on well-being and development.
文摘Objectives:Childhood and adolescent obesity are an increasing global health concern. This study aimed to evaluate the effectiveness of digital components and interaction types in counseling interventions for prevention and treatment. Methods:All studies were searched in online databases and grey literature, including PubMed(Medline),Web of Science,CINAHL,Scopus,IEEE Xplore Digital Library,Journal of Medical Internet Research(JMIR),MedNar,EBSco Open Dissertations. The search period is from inception to June 2023,and the languages are Finnish,English and Swedish. The research quality was evaluated using the web-based data management system Covidence for prevalence studies. The study protocol was registered with PROSPERO(registration number:CRD42021247595).Results:In this review,4,407 studies were screened, and 22 were included. These involved 3,433 participants and 264 child-parent pairs. The digital approaches included multicomponent elements like internet platforms, text messaging, video conferencing, online communities, wearable technology, and mobile apps, allowing one-way,two-way,and face-to-face interactions. Two studies showed statistically significant effects of treatment on BMI and waist-to-hip ratio. Most interventions reported positive outcomes, with no significant differences between groups, and none showed null effects during followup. Conclusions:Digital multicomponents like mobile apps and wearables can help obese children and adolescents adopt healthier lifestyles. While these interventions show promise for obesity management, further research is needed to assess their effectiveness, particularly regarding nurses'perspectives.
文摘BACKGROUND The incidence of type 2 diabetes mellitus(T2DM)in children and adolescents is increasing, yet there is limited information on the available pharmacological interventions to combat T2DM and prevent associated comorbidities.AIM To assess the effectiveness of current pharmacological treatments in managing T2DM in children and adolescents. The protocol of the study was registered in PROSPERO(CRD42022382165).METHODS Searches were performed in PubMed,EMBASE,Scopus, and ClinicalTrials.gov for publications between 1990 to September 2024 without language restrictions.Randomized control trials(RCTs)of pharmacotherapy in children and adolescents with T2DM(aged<19 years)were included. The primary outcome was a change in glycated hemoglobin(HbA1c)from baseline to follow-up.Secondary outcomes were changes in body weight, body mass index(BMI),total cholesterol, triglycerides, high density lipoprotein, and low-density lipoprotein from baseline, and incidence of adverse events during study periods.Screening,full-text review, data extraction, and assessments of risk of bias were done by two reviewers.Conflicts on each step were resolved by a third reviewer.Data analysis was performed using Review Manager Version 6.5(RevMan 6.5)and‘R’software via RStudio,‘meta’and‘netmeta’.RESULTS A total of 12 studies having low to moderate risk of bias with 1658 participants, and follow-up duration 12-52 weeks were included. In our network meta-analysis,compared to control(s),the reduction of HbA1c was sig-nificantly larger for dulaglutide[mean difference(MD),95%confidence interval:-1.20,-2.12 to-0.28],followed by dapagliflozin(-0.94,-1.44 to-0.44),liraglutide(-0.91,-1.37 to-0.45),empagliflozin(-0.87,-1.40 to-0.34),exenatide(-0.59,-1.07 to-0.11)and linagliptin(-0.45,-0.87 to-0.02)while other drugs had little or no effect. While liraglutide was associated with a change in body weight[MD-2.41(-4.68,-0.14)kg],no other drug treatment was associated with significant changes in body weight,BMI,and lipids.Apart from level 1 hypoglycemia with liraglutide[risk difference(RD):0.20,0.04-0.37]and minor adverse events with dulaglutide(RD:0.24,0.08-0.40),no other treatment was associated with excess risk of hypoglycemia or minor or major adverse events.CONCLUSION Pharmacotherapy of T2DM with dulaglutide, dapagliflozin, liraglutide, empagliflozin, exenatide, and linagliptin in children is associated with modest reduction of HbA1c.Larger RCTs with longer follow-up durations are needed to guide better therapeutic decision making.

文摘Brain-derived neurotrophic factor is a crucial neurotrophic factor that plays a significant role in brain health. This metaanalysis was registered at PROSPERO(registration ID: CRD42023439408).
文摘Introduction: Adolescent childbirth is a public health and social problem worldwide. The latter is also associated with high maternal and perinatal morbidity and mortality.
文摘Introduction: Reducing and maintaining viral load is crucial to reducing morbidity and mortality associated with HIV infection in children. It is crucial that the national AIDS program improves Therapeutic Education, trains health workers to communicate results and encourages ongoing dialogue with young people to reinforce adherence and maintain viral suppression.

文摘Objective:Strabismus is associated with a higher prevalence of idiopathic thoracic scoliosis(ITS)in children. This study aims to investigate whether superior oblique palsy(SOP)poses a higher risk for developing ITS among children and adolescents. Methods:A cross-sectional study. The study group comprised 347 consecutive candidates for SOP surgery aged 4 to 18 years;Patients within the same age range with ocular trauma were enrolled as the control group.Preoperative chest plain radiographs were used to measure the Cobb angle.Demographic information and clinical data, including diopter, best corrected visual acuity, deviation degree, and binocular function, were analyzed. Results:A significantly higher prevalence of ITS was found in study group compared with control group(12.68%vs 4.18%,P<0.001).Additionally,the mean Cobb angle was lagger in SOP group than that in control group(5.02°±3.87°vs 3.84°±3.09°,P<0.001).Males in SOP group showed a higher prevalence of ITS(12.9%vs 2.87%,P=0.007),but there was no significant difference in females between two groups(12.3%vs 7.69%,P=0.295).Good near stereopsis acuity was significantly associated with high prevalence of thoracic scoliosis(P<0.001).Multivariate logistic regression analysis revealed that a large distant magnitude of deviation(>20 PD)and near stereoacuity were significantly associated with ITS.Conclusions:Patients with SOP have a significantly higher risk of developing idiopathic thoracic scoliosis, especially those with good near stereoscopic and large distant magnitude of deviation.
文摘Sleep disturbances and disorders are commonly associated with attention-deficit/hyperactivity disorder(ADHD),with affected children frequently experiencing delayed sleep onset, frequent nighttime awakenings, morning fatigue, and excessive daytime sleepiness.A bidirectional relationship exists between ADHD and sleep disturbances:Sleep deprivation is linked to increased inattention and emotional dysregulation, while evening hyperactivity and impulsivity may contribute to difficulty falling asleep.Central nervous system stimulants remain the first-line treatment for managing ADHD symptoms;however, insomnia can be a common side effect.Notably, newer and long-acting stimulants formulations have demonstrated extended benefit into evening with limited impact on sleep in recent studies. This narrative review summarizes the various sleep disorders associated with ADHD,outlines clinical approaches for sleep assessment, and evaluates the impact of ADHD treatments on sleep. It also highlights both nonpharmacological and pharmacological interventions for managing sleep problems in children with ADHD.Finally, the review offers practical recommendations to help clinicians optimize both sleep and daytime functioning in patients with ADHD.ADHD should be conceptualized as a 24-hour disorder, requiring integrated treatment strategies that address both daytime symptoms and nighttime sleep challenges.

文摘To the Editor:As a student actively involved in advocacy for heart disease prevention,I was particularly intrigued by the policymaking implications of digital interventions in the article "Digital components and interaction types in counseling interventions for childhood and adolescent obesity:A systematic review" [1].Through my work as the Founder and President of the nonprofit Guardians of Heart,I have developed and promoted early educational programs, digital tools, and community outreach initiatives designed to instill heart-healthy habits in youth, intending to mitigate risk factors such as obesity that significantly contribute to cardiovascular disease later in life.
文摘Background: Exposure to adverse childhood experiences (ACEs) is associated with a higher likelihood of developing psychological disorders among adolescents. This study emphasizes the need of addressing childhood trauma as a public health priority in India.
文摘BACKGROUND The impact of mental health, including anxiety,on academic performance has been studied in several countries, but few data exist for Sudan.AIM To investigate the correlation between anxiety scores and academic performance among adolescent schoolchildren in Northern Sudan.METHODS This cross-sectional study was carried out during the 2021–2022 academic year among schoolchildren from randomly selected schools in Almatamah locality,River Nile State,Northern Sudan.A questionnaire was used to collect the parti-cipants’sociodemographic data.Anxiety levels were assessed using the ques-tionnaire tools of the General Anxiety Disorder 7-item scale.Students’academic performance scores were obtained from school records.Multivariate linear regression was performed.RESULTS A total of 328 students participated in this study, comprising 156 boys(47.6%)and 172 girls(52.4%).The median[interquartile range(IQR)]age of the students was 14.9 years(13.9–15.6 years). The median(IQR)of the students’academic scores was 27.6%(22.2%–48.1%).The median(IQR)of the students’anxiety scores was 0(0–3).In univariate linear analysis, anxiety scores were positively associated with academic performance scores(coefficient=1.19,P value<0.001).In multivariate linear analysis, after controlling for age, sex, parental education, occupation, and body mass index-Z-score,anxiety scores were positively associated with academic performance scores(coefficient=0.60,P value<0.001).CONCLUSION The current study revealed a positive correlation between anxiety scores and students’academic performance scores. However, the association between mental health disorders, including anxiety, and academic performance is a complex issue. Therefore, further studies are recommended.

文摘Objectives:Attachment is a profound and enduring connection to the emotion children progressively form with their parents as they mature. It significantly impacts the social and psychological development of kids and teenagers. This study aimed to explore the latent profiles and longitudinal transition patterns of parent-child and peer attachments among adolescents. Methods:A cohort of 914 participants from China completed the measures with a twelve-month interval. There were 46.8%boys and 53.2%girls in this survey.Latent profile analysis(LPA)was adopted to explore the distinct profiles reflecting different parent-child and peer attachment response patterns at each time point.Latent transition analysis(LTA)was used to examine the membership of distinct latent profiles and how individuals move between profiles over time. Results:Three latent profiles were found:the poor parent-child communication profile, the moderate attachment profile, and the good attachment profile. It was shown that the transition probability from the poor parent-child communication and good attachment profiles to the moderate attachment profile was higher than the transition probability between the poor parent-child communication and good attachment profiles.Patterns of parent-child and peer attachments were associated with depression and anxiety. Conclusion:This study demonstrates differences in adolescents’attachment to fathers, mothers, and peers and the need for targeted interventions for groups of adolescents with moderate levels of parent-child and peer attachment.
文摘This study was carried out in order to investigate the professionals’ feelings concerning the care in the Center for Children and Youth Psychosocial Care, as well as to evidence the advantages and difficulties of this work. This was a qualitative research guided by the social phenomenology of Al-fred Schütz. 13 health professionals from CAPSi between November 2008 and January 2009 were interviewed. We identified three categories: First feelings, difficulties of care and motivation for the care. The first category, awakened feelings, relates to the beginning of work with children and adolescents in CAPSi. The second motivation for the care, relates to the points highlighted as conducive to work. The third, difficulties in Caring for expressing the difficulties encountered in mental health of children and adolescents in CAPSi. We concluded that the care depends of knowledge, experience and service conditions in order to attend the needs of children and adolescents in CAPSi.
