Level of Stress at Nurses Working in Emergency Clinic and Central Intensive Care: University Clinical Centre of Kosovo

<strong>Background:</strong> Emergency Clinic (EC) and Central Intensive Care (CIC) are high-stress environments that directly affect the health status of nurses. <strong>Methods:</strong> The ... <strong>Background:</strong> Emergency Clinic (EC) and Central Intensive Care (CIC) are high-stress environments that directly affect the health status of nurses. <strong>Methods:</strong> The aim of this study was to assess the level of stress at nurses working in the Emergency Clinic and nurses working in the Central Intensive Care. The study included 90 nurses, 45 nurses working in Emergency Clinic and 45 nurses working in Central Intensive Care. The study applied the Emergency Nurse Stress Questionnaire as an instrument to gather the data. This questionnaire was adopted from the Operational Police Stress Questionnaire in order to serve for the function and aim of the present study. <strong>Results:</strong> Nurses’ cohort-age ranged from 20 to 62 years. The largest proportion of respondents 40 (44.4%) was in the 20 - 30 age group, 58 (64.4%) were married, 60 (66.6%) hold bachelor degree and (33.3%) were with secondary school educational level. In terms of work-related fatigue, significant difference was found in working hours, participants reported that there was a significant difference in fatigue between 12 hours shift nurses (61 ± 10.5) compared to 8 hours nurses (41 ± 23.6) with P < 0.001. Results indicated that a vast majority of participants reported moderate to high levels of stress (81% of participants). There was no significant difference in the level of stress between the two groups of participants. There were also no significant differences compared to their demographic characteristics. <strong>Conclusion:</strong> These findings emphasize the role of using and assuring adequate strategies for ensuring quality management and finding ways of facilitating the increase in the number of nursing staff in these two departments because workplace overload and fatigue are potential factors that increase nurses’ stress levels. 展开更多

The Role of Pain Comprehensive Intervention on Patient Satisfaction in the Centralized Care Mode After Knee Replacement

Methods: a total of 70 knee replacement cases participated in this study. The first case was admitted in May 2019, and the last case was admitted in July 2019. All the subjects were divided into two groups, including 34 in group A and 36 in group B. In the centralized care mode, one group administered routine form care (Group B) and the other group performed a comprehensive pain intervention (Group A). The treatment efficacy and satisfaction, bad mood, pain, comfort, and quality of survival scores were compared between pre-and post-treatment 70 patients. Results: in the comparison of the treatment effect, the results in group A is significantly better than those in group B, with large differences (P <0.05). The comparison of nursing satisfaction data showed that group A was at a high level with a statistic of P <0.05. In Group A, the bad mood score and pain score were significantly lower than in group B, and the comfort score and quality of survival score were significantly higher than in group B, with P <0.05, showing significant differences between the data. Conclusion: in the centralized care mode, the comprehensive pain intervention for patients with knee replacement has a good treatment effect and high satisfaction. Management of Congenital Disorders in a Resource-Limited Country: Organizational Model of a Specialized Working Group within a Perinatal Network

Objective: To evaluate the organizational model of a perinatal network and its relevance in a resource-limited country. Methodology: This was a mixed prospective qualitative and quantitative study conducted over a 2-year period, from January 1, 2022, to December 31, 2023. This study took place in Senegal, a country with limited resources and a weakness of hyperspecialized medical technical resources. There was no policy for the management of fetal malformations. The qualitative part was carried out through overt participant observation. The human resources and the organization of the perinatal network were described. For the quantitative part, all fetuses managed during the study period were included. The studied parameters related to neonatal care and outcomes. Qualitative variables were described using dispersion parameters, and quantitative variables were described using proportions. Results: The perinatal network includes several specialists across six hospitals. Of these hospitals, only one provided emergency pediatric surgery. The network included highly specialized human resources in prenatal diagnosis, congenital heart defects, pediatric surgery, anesthesia, and other medical specialties in perinatology. Advanced ultrasound was centralized by an obstetrician. The team decided on the follow-up methods, timing, and mode of delivery. The newborn was immediately transferred to the appropriate specialty. Over the 2-year period, 201 fetuses were managed. The rate of cesarean delivery was 76.3%. Neonatal mortality was 51.4%. Discussion: Centralizing care improves the quality of prenatal diagnosis and management of congenital defects. Mortality remains high when emergency surgery is not well available. This mortality is also due to the lack of a single center offering all perinatal care and so, the transfer of newborns. The cesarean rate increases due to underlying conditions and organizational factors. Conclusion: Public policies should prioritize the centralization of care for congenital disorders to reduce the costs of disability and mortality. 展开更多
