BACKGROUND Ascites is the most common complication of cirrhosis.Current pharmacological interventions,such as diuretics,often become ineffective in advanced stages due to diuretic resistance.Sodium-glucose co-transporter 2(SGLT2)inhibitors have demonstrated potential in enhancing urinary sodium excretion and mitigating sodium-fluid retention.This study aims to evaluate the effects of SGLT2 inhibitors on the fractional excretion of sodium(FENa)in patients with cirrhotic ascites.AIM To determine whether adjunctive therapy with the SGLT2 inhibitor empagliflozin increases FENa compared with standard care alone in patients with cirrhosis and refractory ascites,and to evaluate its short-term safety profile.METHODS The effect of SGLT2 inhibitor empagliflozin on FENa in patients with cirrhosis and refractory ascites is a multicenter,open-label,randomized controlled trial.A total of 70 patients with refractory ascites secondary to cirrhosis will be enrolled and randomly assigned to receive either empagliflozin 10 mg daily plus standard care or standard care alone for 14 consecutive days.The primary outcome is the change in FENa from baseline to day 14.Secondary outcomes include 24-hour urinary sodium excretion,urine volume,ascites volume(assessed by ultrasound),body weight,and safety indicators.Exploratory outcomes include changes in components of the reninangiotensin-aldosterone system.RESULTS This article reports the study protocol only.No participant data have been collected or analyzed for this manuscript.CONCLUSION This protocol evaluates whether empagliflozin,added to standard therapy,increases sodium excretion and reduces fluid overload in refractory ascites. 展开更多

Heart failure(HF)with preserved ejection fraction(HFpEF)has exceeded HF with reduced ejection fraction(HFrEF),becoming the most common type of HF.Unlike HFrEF,HFpEF is primarily a chronic low-grade inflammatory process closely associated with metabolic disorders.The coexistence of HFpEF and metabolic dysfunction-associated steatotic liver disease(MASLD)presents significant clinical challenges due to shared metabolic pathophysiology and complex inter-play.Management strategies for HFpEF and MASLD remain challenging.Sodium-glucose cotransporter 2 inhibitors have shown benefits in managing both conditions.Additionally,glucagon-like peptide-1 receptor agonists are being actively investigated for their potential benefits,particularly in MASLD.A comprehensive,patient-centered approach that combines metabolic and cardiova-scular care is essential for improving outcomes in patients with HFpEF and MASLD,addressing the global metabolic health challenges. 展开更多

纳米Ti对等离子堆焊Fe基Cr_3C_2合金涂层组织与性能的影响

Effects of free iron oxyhydrates (Fed) and soil organic matter (SOM) on copper (Cu^2+) sorption-desorption behavior by size fractions of aggregates from two typical paddy soils (Ferric-Accumulic Stagnic Anthrosol (Soil H) and Gleyic Stagnic Anthrosol (Soil W)) were investigated with and without treatments of dithionite-citrate-bicarbonate and of H2O2. The size fractions of aggregates were obtained from the undisturbed bulk topsoil using a low energy ultrasonic dispersion procedure. Experiments of equilibrium sorption and subsequent desorption were conducted at soil water ratio of 1:20, 25℃. For Soil H, Cu^2+ sorption capacity of the DCB-treated size fractions was decreased by 5.9% for fine sand fraction, by 40.4% for coarse sand fraction, in comparison to 2.9% for the bnlk sample. However, Cu^2+ sorption capacities of the H2O2-treated fractions were decreased by over 80% for the coarse sand fraction and by 15% for the clay-sized fraction in comparison to 88% for bulk soil. For Soil W, Cu^2+ sorption capacity of the DCB-treated size fraction was decreased by 30% for the coarse sand fraction and by over 75% for silt sand fraction in comparison to 44.5% for the bulk sample. Cu^2+ sorption capacities of the H2O2-treated fractions were decreased by only 2.0% for the coarse sand fraction and by 15% for the fine sand fraction in comparison to by 3.4% for bulk soil. However, Cu^2+ desorption rates were increased much in H2O2-treated samples by over 80% except the clay-sized fraction (only 9.5%) for Soil H. While removal of SOM with H2O2 tendend to increase the desorption rate, DCB- and H2O2-treatments caused decrease in Cu^2+ retention capacity of size fractions, Particularly, there hardly remained Cu^2+ retention capacity by size fractions from Soil H after H2O2 treatment except for clay-sized fraction. These findings supported again the dominance of the coarse sand fraction in sorption of metals and the preference of absorbed metals bound to SOM in differently stabilized status among the size fractions. Thus, enrichment and turnover of SOM in paddy soils may have great effects on metal retention and chemical mobility in paddy soils. 展开更多

The aldehyde dehydrogenase 2(ALDH2) rs671 polymorphism has a prevalence of 30%–50% in Asian populations. In this study, we aimed to analyze risk factors contributing to the development of heart failure with preserved ejection fraction(HFpEF) along with the genetic exposure in Chinese patients hospitalized with cardiovascular diseases(CVD). Methods From July 2017 to October 2018, a total of 770 consecutive Chinese patients with normal left ventricular ejection fractions(LVEF) and established CVD(hypertension, coronary heart diseases, or diabetes) were enrolled in this prospective cross-sectional study. HFpEF was defined by the presence of at least one of symptom(dyspnoea and fatigue) or sign(rales and ankle swelling) related to heart failure;N-terminal pro-B-Type natriuretic peptide(NT pro-BNP ≥ 280 pg/mL);LVEF ≥ 50%;and at least one criterion related to elevated ventricular filling pressure or diastolic dysfunction(left atrial diameter > 40 mm, E/E’ ≥ 13, E’/A’ < 1 or concurrent atrial fibrillation). Logistic regression was performed to yield adjusted odds ratios(ORs) for HFp EF incidence associated with traditional and/or genetic exposures. Results Finally, among 770 patients with CVD, 92(11.9%) patients were classified into the HFpEF group according to the diagnostic criteria. The mean age of the participants was 67 ± 12 years, and 278(36.1%) patients were females. A total of 303(39.4%) patients were ALDH2*2 variant carriers. In the univariate analysis, eight exposures were found to be associated with HFpEF: atrial fibrillation, ALDH2*2 variants, hypertension, age, anaemia, smoking, alcohol consumption and sex. Multivariable logistic regression showed that 4 ‘A’ variables(atrial fibrillation, ALDH2*2 variants, age and anaemia) were significantly associated with an increased risk of HFpEF. Atrial fibrillation was associated with a 3.8-fold increased HFpEF risk(95% CI: 2.21–6.61, P < 0.001), and the other three exposures associated with increased HFpEF risk were the ALDH2*2 variant(OR = 2.41, 95% CI: 1.49–3.87, P < 0.001), age(OR = 2.14, 95% CI: 1.27–3.60, P = 0.004), and anaemia(OR = 1.79, 95% CI: 1.05–3.03, P = 0.032). These four variables predicted HFpEF incidence in Chinese CVD patients(C-statistic = 0.745, 95% CI: 0.691–0.800, P < 0.001). Conclusions 4 A traits(atrial fibrillation, ALDH2*2 variants, age and anaemia) were associated with an increased risk of HFpEF in Chinese CVD patients. Our results provide potential clues to the aetiology, pathophysiology and therapeutic targets of HFpEF. 展开更多

The temperature equation is described by a fractional step method. Thick and thin grids are made use of to form a complete set. Piecewise threefold quadratic interpolation, symmetrical extension, calculus of variations, commutativity of operator product, decomposition of high order difference operators and prior estimates are also made use of. Then the extracts were analyzed with GC/MS. Comparison of experimental data between FTTB coal and clarain from Tongting bituminous (CTTB) coal was carried out. The results show that the kinds of small molecule components detected by GC/MS of FTTB are less than those of CTTB. Long-chain alkanes exist mostly in the extracts of fusain. Macromolecular networks are predominant in the FTTB coal mainly composed of inertinite in the coal petrography. The size of micropores in the FTTB coal is relatively small, and the development of micropores is relatively low. Thus the content of aromatic compounds with affinity for micropores is relative low in FTTB, while the content of long-chain alkanes with affinity for macromolecule networks is relatively high. Sub-components in exinite determine the distribution of long-chain alkanes extracted in the last stage. Odd-numbered carbon distribution appears when resin is most in exinite, while high carbon alkane distribution appears when exinite is dominant in cutinite. Small aromatic molecules are firstly packed in micropores, and exist in a free state after micropores are saturated. 展开更多

The estimation of fractional vegetation cover(FVC) is important for identifying and monitoring desertification, especially in arid and semiarid regions. By using regression and pixel dichotomy models, we present the comparison of Sentinel-2A(S2) multispectral instrument(MSI) and Landsat 8(L8) operational land imager(OLI) data regarding the retrieval of FVC in a semi-arid sandy area(Mu Us Sandland, China, in August 2016). A combination of unmanned aerial vehicle(UAV) high-spatial-resolution images and field plots were used to produce verified data. Based on a normalized difference vegetation index(NDVI) regression model, the results showed that, compared with that of L8, the coefficient of determination(R2) of S2 increased by 26.0%, and the root mean square error(RMSE) and the sum of absolute error(SAE) decreased by 3.0% and 11.4%, respectively. For the ratio vegetation index(RVI) regression model, compared with that of L8, the R2 of S2 increased by 26.0%, and the RMSE and SAE decreased by 8.0% and 20.0%, respectively. When the pixel dichotomy model was used, compared with that of L8, the RMSE of S2 decreased by 21.3%, and the SAE decreased by 26.9%. Overall, S2 performed better than L8 in terms of FVC inversion. Additionally, in this paper, we develop a verified scheme based on UAV data in combination with the object-based classification method. This scheme is feasible and sufficiently robust for building relationships between field data and inversion results from satellite data. Further, the synergy of multi-source sensors(especially UAVs and satellites) is a potential effective way to estimate and evaluate regional ecological environmental parameters(FVC). 展开更多

Based on historical runs,one of the core experiments of the fifth phase of the Coupled Model Intercomparison Project (CMIP5),the snow depth (SD) and snow cover fraction (SCF) simulated by two versions of the Flexible Global OceanAtmosphere-Land System (FGOALS) model,Grid-point Version 2 (g2) and Spectral Version 2 (s2),were validated against observational data.The results revealed that the spatial pattern of SD and SCF over the Northern Hemisphere (NH) are simulated well by both models,except over the Tibetan Plateau,with the average spatial correlation coefficient over all months being around 0.7 and 0.8 for SD and SCF,respectively.Although the onset of snow accumulation is captured wellby the two models in terms of the annual cycle of SD and SCF,g2 overestimates SD/SCF over most mid-and high-latitude areas of the NH.Analysis showed that g2 produces lower temperatures than s2 because it considers the indirect effects of aerosols in its atmospheric component,which is the primary driver for the SD/SCF difference between the two models.In addition,both models simulate the significant decreasing trend of SCF well over (30°-70°N) in winter during the period 1971-94.However,as g2 has a weak response to an increase in the concentration of CO2 and lower climate sensitivity,it presents weaker interannual variation compared to s2. 展开更多

Fe-Al/Cr3C2 coatings were sprayed on low steel by high velocity arc spraying(HVAS) technology. The influences of oxides on erosion, corrosion and wear behavior for high velocity arc sprayed Fe-Al/Cr3C2 coatings were studied. The results show that HVAS-sprayed Fe-Al/Cr3C2 coatings have good erosion, heat corrosion and wear resistance. The erosion resistance improves with the increase of the temperature. On one hand, the ferrous oxides are incompact, so they peel off the surface of the coatings easily during the high temperature erosion. On the other hand, compact Al2O3 films on the surface can protect the coatings. 展开更多
