Artificial Intelligence in CT Imaging:A Systematic Review of Diagnostic Accuracy,Clinical Decision-Support Impact,and Integration Pathways

Artificial intelligence(AI)is rapidly transforming radiology and computed tomography(CT)imaging by enabling automated image analysis,improved diagnostic accuracy,and clinical decision-support.We performed a systematic review of peerreviewed studies published between January 1,2010 and March 31,2025 to quantify reported gains in diagnostic performance and workflow efficiency,to evaluate clinical decision-support benefits and risks,and to identify integration priorities.We searched PubMed,IEEE Xplore,Scopus,ScienceDirect,and Google Scholar and screened 128 records;26 studies met the inclusion criteria.Extracted data included study design,AI architecture,sample size,and quantitative performance metrics;study quality was assessed using Newcastle-Ottawa Scales(NOS),Cochrane RoB 2,or AMSTAR 2 as appropriate.Across included studies,AI applications in CT showed consistent improvements in sensitivity,specificity,and time-to-diagnosis in specific tasks(notably lung-nodule detection and intracranial hemorrhage triage),with reported detection-rate increases up to~20%and reduced turnaround times in several real-world implementations.Barriers include dataset bias,limited external validation,interpretability(“black-box”)concerns,workflow integration challenges,and evolving regulatory issues.Economic analyses suggest potentially favorable return on investment(ROI)in high-volume settings but are sensitive to licensing and infrastructure costs.To realize AI's benefits in CT imaging,rigorous multi-center validation,transparent reporting,humancentered workflow design,and post-deployment surveillance are essential. 展开更多

Erbium chelate as a high-performance spectral CT contrast agent for gastrointestinal tract imaging

Spectral computed tomography(CT)imaging as an advanced and non-invasive technique is of importance in the diagnosis of disease.Therefore,it is significant to develop safe and high-performance contrast agents for spectral CT imaging.Herein,we synthesized a small-molecule erbium chelate(ErDOTA dimeglumine),with the advantages of favorable colloidal and structure stability,good biocompatibility and biosafety,and sensitive spectral CT imaging ability in vitro and in vivo.Erbium chelate exhibits strong X-ray attenuation capability and the energy-dependent attenuation in the range of 40-160 keV due to the high K-edge value of Er(57.5 keV).Especially,the slope of the Hounsfield unit(HU)curve for erbium chelate is 1.5 times that of iohexol at 50 keV,and 5.6 times that of iohexol at130 keV.We then applied erbium chelate in the in vivo spectral CT imaging of healthy mice and DSSinduced colitis mice.It is found that erbium chelate is rapidly metabolized from the intestinal tract in healthy mice within 12 h due to favorable biocompatibility,while it is enriched and displays brighter signals in the inflammatory site of colon in colitis mice.Specifically,the CT value in the large intestines of colitis mice 12 h after erbium chelate administration is 53.0,which is much higher than that of healthy mice(8.3),showing great potential for sensitive and accurate diagnosis of inflammatory bowel disease.Moreover,compared with the clinically used contrast agent iohexol,erbium chelate shows better spectral CT imaging performance in both healthy and colitis mice at low to high energy settings.Superior CT imaging is also observed in CT26 tumor-bearing mice after administration with erbium chelate in comparison to iohexol.In summary,the small-molecule erbium chelate is expected to be a safe and highperfo rmance co ntrast agent for spectral CT imaging to promote the diagnosis of gastrointestinal diseases. 展开更多

Mesenchymal stem cells(MSCs)have emerged as promising candidates for idiopathic pulmonary fibrosis(IPF)therapy.Increasing the MSC survival rate and deepening the understanding of the behavior of transplanted MSCs are of great significance for improving the efficacy of MSC-based IPF treatment.Therefore,dual-functional Au-based nanoparticles(Au@PEG@PEI@TAT NPs,AuPPT)were fabricated by sequential modification of cationic polymer polyetherimide(PEI),polyethylene glycol(PEG),and transactivator of transcription(TAT)penetration peptide on AuNPs,to co-deliver retinoic acid(RA)and microRNA(miRNA)for simultaneously enhancing MSC survive and real-time imaging tracking of MSCs during IPF treatment.AuPPT NPs,with good drug loading and cellular uptake abilities,could efficiently deliver miRNA and RA to protect MSCs from reactive oxygen species and reduce their expression of apoptosis executive protein Caspase 3,thus prolonging the survival time of MSC after transplantation.In themeantime,the intracellular accumulation of AuPPT NPs enhanced the computed tomography imaging contrast of transplantedMSCs,allowing them to be visually tracked in vivo.This study establishes an Au-based dual-functional platform for drug delivery and cell imaging tracking,which provides a new strategy for MSC-related IPF therapy. 展开更多

The segmentation of head and neck(H&N)tumors in dual Positron Emission Tomography/Computed Tomogra-phy(PET/CT)imaging is a critical task in medical imaging,providing essential information for diagnosis,treatment planning,and outcome prediction.Motivated by the need for more accurate and robust segmentation methods,this study addresses key research gaps in the application of deep learning techniques to multimodal medical images.Specifically,it investigates the limitations of existing 2D and 3D models in capturing complex tumor structures and proposes an innovative 2.5D UNet Transformer model as a solution.The primary research questions guiding this study are:(1)How can the integration of convolutional neural networks(CNNs)and transformer networks enhance segmentation accuracy in dual PET/CT imaging?(2)What are the comparative advantages of 2D,2.5D,and 3D model configurations in this context?To answer these questions,we aimed to develop and evaluate advanced deep-learning models that leverage the strengths of both CNNs and transformers.Our proposed methodology involved a comprehensive preprocessing pipeline,including normalization,contrast enhancement,and resampling,followed by segmentation using 2D,2.5D,and 3D UNet Transformer models.The models were trained and tested on three diverse datasets:HeckTor2022,AutoPET2023,and SegRap2023.Performance was assessed using metrics such as Dice Similarity Coefficient,Jaccard Index,Average Surface Distance(ASD),and Relative Absolute Volume Difference(RAVD).The findings demonstrate that the 2.5D UNet Transformer model consistently outperformed the 2D and 3D models across most metrics,achieving the highest Dice and Jaccard values,indicating superior segmentation accuracy.For instance,on the HeckTor2022 dataset,the 2.5D model achieved a Dice score of 81.777 and a Jaccard index of 0.705,surpassing other model configurations.The 3D model showed strong boundary delineation performance but exhibited variability across datasets,while the 2D model,although effective,generally underperformed compared to its 2.5D and 3D counterparts.Compared to related literature,our study confirms the advantages of incorporating additional spatial context,as seen in the improved performance of the 2.5D model.This research fills a significant gap by providing a detailed comparative analysis of different model dimensions and their impact on H&N segmentation accuracy in dual PET/CT imaging. 展开更多

Application of ^(18) F-FDG PET/CT Imaging in Diagnosing Bladder Tumor Metastasis Lesions

Bladder tumor is the most common malignant tumor in urinary system and always com- panied with lymph node metastasis. The accurate staging plays a significant role in treatment for bladder tumor and prognostic evaluation, and the distant metastasis predicts worse prognosis. The objective of this study was to assess the clinical significance of 18F-FDG PET/CT imaging in diagnosing bladder tumor metastasis lesions. A retrospective analysis of 60 patients with bladder tumor from October 2008 to May 2010 was done. The patients were stratified based on the imaging technique. Among all 60 cases, besides the primary lesion, 81 suspected lesions were spotted and 73 confirmed as metastasis, including 50 lymph node metastases, 22 distant metastases, and 1 bone metastasis. For PET/CT imaging, its sensitivity was 94.5%, specificity 87.5%, positive predictive value 98.6%, negative predictive value 63.6% and accuracy 93.8% respectively. For CT, its sensitivity was 82.2%, specificity 50%, positive predictive value 93.8%, negative predictive value 23.5% and accuracy 79% respectively. PET/CT im- aging was superior to CT in sensitivity, specificity and accuracy. In conclusion, 18F-FDG PET/CT imaging is more significant in diagnosing bladder tumor metastasis lesions. 展开更多

Methods: a total of 100 patients with acute pancreatitis in our hospital were selected from July 2020 to July 2021. Results: CT images showed hemorrhage and necrosis type of 30%, edema type of 67%, detection rate of 97%, missed diagnosis rate of 3%, CT images showed high detection rate, and there was no significant difference with pathological examination results (p > 0.05). In 30 cases of hemorrhagic necrotic type, grade A was 8%, grade B was 10%, grade C was 6%, grade D was 4%, grade E was 2%, in 67 cases of simple edematous type, grade A was 21%, grade B was 17%, grade C was 13%, grade D was 10%, grade E was 6%. It can be seen that the proportion of B grade is relatively high in hemorrhagic necrosis type, and the proportion of A grade is relatively high in simple edema type. CT imaging can clearly show the grading of pancreas. Conclusion: CT examination of acute pancreatitis is effective and of good value. 展开更多

Methods: 100 subjects were selected from March 2018 to May 2020. According to the disease types, they were divided into 60 cases of pancreatitis group and 40 cases of pancreatic cancer group. All patients underwent CT imaging examination, and the difference of CT signs between the two groups was analyzed and compared. Results: Compared with the group of pancreatitis, the group of pancreatic cancer has the features of enlarged lesion area, uniform expansion of distal pancreatic duct, clear lesion area and uneven density (P<0.05). Conclusion: the application of CT imaging examination can effectively differentiate the diagnosis of chronic pancreatitis and pancreatic cancer, which is beneficial to the later treatment of patients and has positive significance for improving the prognosis of patients. The Value Analysis of CT Imaging Technology in the Diagnosis of Femoral Head Necrosis

All patients underwent C-T examination, X-ray examination and magnetic resonance diagnosis. Statistical analysis was made on the bone state of femoral head defect diagnosed by C-T scan, X-ray and magnetic resonance, as well as the accuracy of C-T scan, X-ray and magnetic resonance diagnosis, and the sensitivity and specificity of femoral defect. Results: according to the final clinical diagnosis, out of 72 patients suspected of osteonecrosis, a total of 1 patient was diagnosed as osteoporosis. A total of 65 patients with femoral diseases, the positive rate of femoral necrosis was higher than that of CT scan and X-ray, while the positive rate of femoral necrosis was higher than that of X-ray, with statistical significance (P < 0.05). The accuracy, sensitivity and specificity of magnetic resonance diagnosis of femoral defect were higher than those of t-scan and x-ray scan, and the accuracy and sensitivity of CT scan were higher than those of x-ray, the difference was statistically significant (P < 0.05). Compared with X-ray films (P > 0.05), the features found by computed tomography of femoral necrosis have no statistical significance Conclusion: magnetic resonance diagnosis has higher value in clinical diagnosis of patients with femoral head necrosis than CT scan and X-ray films. 展开更多

Methods: a total of 60 patients who underwent intestinal three-dimensional CT examination in our hospital from January 2019 to January 2021 were selected and divided into two groups according to random number table method, with 30 patients in each group. The control group was given routine nursing intervention, and the observation group was given intestinal preparation continuous improvement intervention. The success rate of examination, awareness rate before examination, satisfaction rate of image quality and complications were compared between the two groups. Results: the success rate and awareness rate before examination in the observation group were higher than those in the control group, and the difference was statistically significant (P < 0.05). The satisfaction rate of image quality in the observation group was higher than that in the control group, and the incidence of complications in the observation group was lower than that in the control group, with statistical significance (P < 0.05). Conclusion: intestinal preparation CQI intervention can improve the success rate of intestinal three-dimensional CT examination, improve the knowledge of patients, enhance the quality of CT imaging, and reduce the incidence of complications. 展开更多

Thirty-four patients with cerebral infarction and 18 patients with transient ischemic attack were examined by multi-slice spiral CT scan, CT perfusion imaging, and CT angiography within 6 hours after onset. By CT perfusion imaging, 29 cases in the cerebral infarction group and 10 cases in the transient ischemic attack group presented with abnormal blood flow perfusion, which corresponded to the clinical symptoms. By CT angiography, various degrees of vascular stenosis could be detected in 41 patients, including 33 in the cerebral infarction group and eight in the transient ischemic attack group. The incidence of intracranial artery stenosis was higher than that of extracranial artery stenosis. The intracranial artery stenosis was located predominantly in the middle cerebral artery and carotid artery siphon, while the extracranial artery stenosis occurred mainly in the bifurcation of the common carotid artery and the opening of the vertebral artery. There were 34 cases (83%) with convict vascular stenosis and perfusion abnormalities, and five cases (45%) with perfusion abnormalities but without convict vascular stenosis. The incidence of cerebral infarction in patients with National Institutes of Health Stroke Scale scores 〉 5 points during onset was significantly higher than that in patients with National Institutes of Health Stroke Scale scores 〈 5 points. These experimental findings indicate that the combined application of various CT imaging methods allows early diagnosis of acute ischemic cerebrovascular disease, which can comprehensively analyze the pathogenesis and severity of acute ischemic cerebrovascular disease at the morphological and functional levels. 展开更多

Recently, three dimensions (3D) spiral CT imaging has been used in surgical diseases gradually. This study was designed to evaluate the diagnostic value of 3D spiral CT imaging of cholangiopancreatic ducts on obstructive jaundice. Methods: Thirty patients with obstructive jaundice had received B-mode ultrasonography, CT, percutaneous transhepatic cholangiography (PTC) or endoscopic retrograde cholangiopancreatography (ERCP), and 3D spiral CT imaging of cholangiopancreatic ducts preoperatively. Then the diagnose accordance rate of these examinational methods were compared after operations. Results: The diagnose accordance rate of 3D spiral CT imaging of cholangiopancreatic ducts was higher than those of B-mode ultrasonography, CT, or single PTC or ERCP, which showed clear images of bile duct tree and pathological changes. As to malignant obstructive jaundice, this examinational technique could clearly display the adjacent relationship between tumor and liver tissue, biliary ducts, blood vessels, and intrahepatic metastases. Conclusion: 3D spiral CT imaging of cholangiopancreatic ducts has significant value for obstructive diseases of biliary ducts, which provides effective evidence for the feasibility of tumor-resection and surgical options. 展开更多

In the current landscape of the COVID-19 pandemic,the utilization of deep learning in medical imaging,especially in chest computed tomography(CT)scan analysis for virus detection,has become increasingly significant.Despite its potential,deep learning’s“black box”nature has been a major impediment to its broader acceptance in clinical environments,where transparency in decision-making is imperative.To bridge this gap,our research integrates Explainable AI(XAI)techniques,specifically the Local Interpretable Model-Agnostic Explanations(LIME)method,with advanced deep learning models.This integration forms a sophisticated and transparent framework for COVID-19 identification,enhancing the capability of standard Convolutional Neural Network(CNN)models through transfer learning and data augmentation.Our approach leverages the refined DenseNet201 architecture for superior feature extraction and employs data augmentation strategies to foster robust model generalization.The pivotal element of our methodology is the use of LIME,which demystifies the AI decision-making process,providing clinicians with clear,interpretable insights into the AI’s reasoning.This unique combination of an optimized Deep Neural Network(DNN)with LIME not only elevates the precision in detecting COVID-19 cases but also equips healthcare professionals with a deeper understanding of the diagnostic process.Our method,validated on the SARS-COV-2 CT-Scan dataset,demonstrates exceptional diagnostic accuracy,with performance metrics that reinforce its potential for seamless integration into modern healthcare systems.This innovative approach marks a significant advancement in creating explainable and trustworthy AI tools for medical decisionmaking in the ongoing battle against COVID-19. 展开更多

Photodynamic therapy(PDT),as a non-invasive treatment,is expected to be widely used in clinical cancer treatment.In this paper,a near-infrared(NIR)light responsive Lu_(2)O_(3):Yb/Er/Li-Ce6@MIL-101/GOx-PDA(LCMGP)nanodiagnostic platform was prepared for CT/PA imaging-guided multimodality synergistic therapy.Under 808 nm laser irradiation,Lu_(2)O_(3):Yb/Er/Li-Ce6(LC)NPs in the nano-diagnostic platform are activated and sensitized by NIR light,resulting in the generation of more single-linear oxygen(^(1)O_(2))for efficient PDT.Glucose oxidase(GOx)and MIL-101(Fe)nanozymes can utilize their excellent enzyme cascade catalytic properties and peroxidase-like catalytic properties to undergo glycolysis reactions and catalyze the decomposition of endogenous hydrogen peroxide(H_(2)O_(2)),producing a large amount of hydroxyl radicals(·OH),achieving efficient chemodynamic therapy(CDT).The functionalized modification of polydopamine(PDA)endows the LCMGP nanoparticles with higher photothermal conversion efficiency,which can prompt high local temperature at the tumor site under the irradiation of NIR light to achieve efficient photothermal therapy(PTT).In addition,hyperthermia accelerates the catalytic efficiency of the nanozymatic cascade of LCMGP nanoparticles to enhance ROS production while increasing cellular uptake,resulting in PTT-induced PDT/CDT synergistic effect(96%).Based on the excellent photothermal conversion effect of PDA and the high X-ray atte nuation effect of Lu/Fe,the LCMGP nanotherapeutic system demonstrates good CT/PA imaging function,providing new insights and ideas for imaging-mediated multimodal synergistic therapy. 展开更多

Nanotheranostics,which combine the therapeutic and diagnostic functions in one integrated system,have received extensive attentions in cancer treatments because they enable non-invasive diagnosis,tumor-targeted drug delivery,and real-time monitoring of therapeutic response.However,due to the high systemic toxicity of commonly used chemotherapeutics,current treatment still has limitations.Herein,to simultaneously achieve safe cancer therapy and therapeutic response monitoring,an iodinated prodrug strategy was proposed.2,3,5-Triiodobenzoic acid(TIBA)was used to modify both paclitaxel(PTX)and the polymeric vehicle,so that the encapsulation efficiency of PTX could be increased and the systemic toxicity could be reduced.As-prepared prodrug nanoparticles could accumulate passively in the tumor site and promptly release loaded drugs in response to the overexpressed GSH in cancer cells,which then caused efficient cell cycle arrest and apoptosis like that of the parent PTX.With this rational design,safe and efficient antitumor therapy and real-time computer tomography(CT)imaging could be simultaneously realized,facilitating potential CT imaging-guided therapy of metastatic breast cancer. 展开更多

Photoacoustic(PA) tomography(PAT) breaks the barrier for high-resolution optical imaging in a strong lightscattering medium, having a great potential for both clinical implementation and small animal studies. However,many organs and tissues lack enough PA contrast or even hinder the propagation of PA waves. Therefore, it is challenging to interpret pure PAT images, especially three-dimensional(3 D) PA images for deep tissues, without enough structural information. To overcome this limitation, in this study, we integrated PAT with X-ray computed tomography(CT) in a standalone system. PAT provides optical contrast and CT gives anatomical information. We performed agar, tissue phantom, and animal studies, and the results demonstrated that PAT/CT imaging systems can provide accurate spatial registration of important complementary contrasts. 展开更多

Enhancing the stability of ^(68)Ga-labeled RNA aptamers for pancreatic β-cell and insulinoma imaging through nucleoside modifications

Diabetes and insulinoma represent opposing alterations in pancreatic β-cell mass,with diabetes resulting from irreversible β-cells damage and insulinoma arising from abnormal proliferation.Early diagnosis of both conditions necessitates effectiveβ-cell mass detection.Current detection methods are limited in diagnosing each condition individually or lacking timely and accurate detection.Diabetes is typically identified only after significantβ-cell loss,while insulinoma can evade conventional imaging due to their small size.Positron emission tomography/computed tomography(PET/CT)imaging,combining anatomical and functional data,enhances diagnostic accuracy but faces challenges in specificity.This study employed two RNA aptamers(m12–3773 and 1–717)modified to enhance RNase resistance and conjugated with68Ga to create ^(68)Ga-NOTA-Ap.^(68)Ga-NOTA-Ap was administered to rats with pancreaticβ-cell damage and mice with insulinoma to evaluate its ability to image islets,detect changes in pancreatic β-cell mass(BCM),and identify insulinoma.Modified with methoxy and fluoro,RNA aptamers exhibited enhanced stability and RNases resistance while retaining their dissociation constants(K_(d)).Furthermore,^(68)Ga-NOTA-Ap effectively detected changes of BCM in rats with pancreatic β-cell damage and imaged insulinoma in mice through recognition of abnormalβ-cell proliferation by recognizing clusterin and transmembrane p24 trafficking protein 6(TMED6)on pancreaticβ-cell.The developed ^(68)Ga-NOTA-Ap shows promise for early screening of diabetes and insulinoma due to its high sensitivity,specificity,and non-invasive nature.It has potential clinical applications for monitoring pancreatic β-cell function and diagnosing insulinoma. 展开更多

Objective:To analyze the clinical characteristics and chest CT imaging characteristics of patients with confirmed COVID-19(COVID-19)and patients with suspected COVID-19.Methods:The study time span was from February 2020 to May 2020.The case samples were selected from 72 patients with confirmed covid-19 and suspected covid-19 diagnosed and treated by The First People’s Hospital of Yinchuan and Yinchuan Temporary Emergency Hospital,including 38 patients with confirmed covid-19 and 34 patients with suspected covid-19.All patients underwent laboratory examination and chest CT examination,and the specific examination results were compared and analyzed.Results:There were significant differences in number of white blood cell,percentage of lymphocytes,creatine kinase and erythrocyte sedimentation rate between confirmed and suspected COVID-19 patients(P<0.05).The CT imaging characteristics of COVID-19 patients were compared with those of suspected COVID-19 patients.The lesions of COVID-19 patients were mostly characterized by mixed ground glass density and pure ground glass density.There were vascular thickening and interstitial thickness increase,and accompanied by bronchiectasis or air bronchogram.The distribution of lesions was mostly subpleural without pleural effusion.The lesion area of suspected COVID-19 patients mostly showed solid density and mixed ground glass density.The lesion was distributed along bronchovascular and pleural effusion was observed.Conclusion:There are some differences in biochemical indexes and chest CT images between confirmed and suspected covid-19 patients,which can be used for differential diagnosis. 展开更多

Value of SPECT/CT Fusion Imaging in Differential Diagnosis of Benign and Malignant Spinal Lesions

Methods: all the subjects were diagnosed with SPECT/CT fusion imaging technology, and then all the subjects were diagnosed with CT-guided percutaneous spinal biopsy. 42 cases of benign lesions were confirmed and 18 cases of malignant lesions, accounting for 70.00% and 30.00% respectively. Taking CT-guided biopsy as the gold standard, the detection rates of benign and malignant spinal lesions by SPECT/CT fusion imaging were statistically compared. Results: there was no significant difference between differential diagnosis accuracy and gold standard in SPECT/CT fusion imaging technique (P > 0.05). Conclusion: it has high differential diagnosis accuracy, can effectively identify patients' diseases, and is worthy of clinical application. 展开更多
