A large proportion of patients suffer from the constipation subtype of IBS(IBS-C), most commonly afflicting older individuals and those with a lower socioeconomic status. Conventional pharmacologic and nonpharmacologic treatment options have limited efficacies and/or significant adverse events, which lead to increased long-term health care expenditures. Failure to effectively treat IBS-C patients over the past decades has largely been due to a poor understanding of disease pathophysiology, lack of a global view of the patient, and an inappropriate selection of patients and treatment endpoints in clinical trials. In recent years, however, more effective and safer drugs have been developed for the treatment of IBS-C. The advancement in the area of pharmacologic treatment is based on new knowledge of the pathophysiologic basis of IBS-C and the development of drugs with increased selectivity within pharmacologic classes with recognized efficacies. This narrative review covers the spectrum of available drugs and their mechanisms of action, as well as the efficacy and safety profiles of each as determined in relevant clinical trials that have investigated treatment options for IBS-C and chronic constipation. A brief summary of laxative-based treatment options is presented, followed by up-to-date assessments for three classes of drugs: prokinetics, prosecretory agents, and bile acid modulators.展开更多

Methods: The following databases were searched from their inception until August 2017: Ovid Medline, Embase, the Cochrane Central Register of Controlled Trials (CENTRAL), and Allied and Alternative Medieine (AMED). We also searched four Chinese databases: Chinese BioMedical Database (CBM), China National Knowledge Infrastructure (CNKI), WANFANG Data, and Chinese VIP Database. Only the RCTs related to the effects of AA therapy on preventing constipation in breast cancer patients undergoing chemotherapy were included in this study. Quantitative syntheses of data from RCTs were conducted using RevMan 5.3 software. Study selection, data extraction, and validation were performed independently by two authors. Cochrane criteria for risk of bias were used to assess the methodological quality of the trials. Results: Four RCTs met the inclusion criteria, and most were of low methodological quality. Study participants in the AA plus routine care group showed significantly greater improvements in the response rate (risk ratio [RR] = 1.27, 95% confidence interval [CI] [1.14, 1.42], P 〈 0.01) with low heterogeneity (x2=2.31, P =0.31, F = 14%). In addition, when compared with routine care alone, one RCT suggested favorable statistically significant effects of AA plus routine care on Constipation Assessment Scale (CAS; mean difference [MD] =-5.07, 95% CI [-6.86, -3.28], P 〈 0.01). Furthermore, when compared with routine care alone, one RCT suggested positive statistically significant effects of AA plus routine care on Patient Assessment of Constipation-Quality of Life (PAC-QOL; MD = -1.26, 95% CI [-1.59, -0.93], P 〈 0.01). Conclusions: Overall, as a potential safety therapy, only weak evidence can support the hypothesis that AA can effectively prevent constipation in breast cancer patients undergoing chemotherapy.展开更多

METHODS: A total of 3872 patients with functional dyspepsia and 3609 patients with irritable bowel syndrome were enrolled in the study by 18 Italina thermal centres. Patients underwent a first cycle of thermal therapy for 21 d. A year later patients were re-evaluated at the same centre and received another cycle of thermal therapy. A questionnare to collect personal data on social and occupational status, family and pathological case history, life style, clinical records, utilisation of welfare and health structure and devices was administered to each patient at basal time and one year after each thermal treatment. Sixty patients with functional dyspepsia and 20 with IBS and 80 healthy controls received an evaluation of gastric output and oro-cecal transit time by breath test analysis. Breath test was performed at basal time and after water supplementaton therapies. Gastrointestinal symptoms were evaluated at the same time points. Breath samples were analyzed with a mass spectometer and a gascromatograph. Results were expressed as T1/2 and T-lag for octanoic add breath test and as oro-cecal transit time for lactulose breath test. RESULTS: A significant reduction of prevalence of symptoms was observed at the end of the first and second cycles of thermal therapy in dyspeptic and IBS patients, The analysis of variance showed a real and persistant improvement of symptoms in all patients. After water supplementation for 3 wk a reduction of gastric output was observed in 49 (87.5%) of 56 dyspepUc patients. Both T1/2 and T-lag were significantly reduced after the therapy compared to basal values [91 ± 12 (T1/2) and 53± 11 (T-lag), Tables 1 and 2] with results of octanoic acid breath test similar to healthy subjects. After water supplementation for 3 wk oro-cecal transit time was shorter than that at the beginning of the study. CONCLUSION: Mineral water supplementation treatment for functional dyspepsia or conspipation accompanying IBS can improve gastric add output and intestinal transit time.展开更多

Methods One hundred cases of constipation were randomly divided into an acupuncture-moxibustion group and a simple acupuncture group with 50 cases for each group. Two groups of acupoints were adopted every other day alternatively for both groups. Point group 1: Tiansha (天枢ST 25), Daheng (大横 SP 15), Qihai (气海 CV 6), Guanyuan (关元 CV 4), Ztisanli (足三里 ST 36), Shangjhxu (上巨虚 ST 37) and Sanyinjiao (三阴交 SP 6). Point group 2: Zhsngliao (中髎 BL 33), Xiatliao (下髎 BL 34), Dachangsha (大肠俞 BL 25), Shenshu (肾俞 BL 23) and Pisha (脾俞 BL 20). Acupuncture was the only therapy adopted for acupuncture group on all the above mentioned points. While, for acupuncture-moxibustion group, moxibustion with grain-shaped moxa cones was applied on Qihai (所海 CV 6), Zusatnli (足三里 ST 36), Datchangshu (大肠俞 BL 25) and Pishu (脾俞 BL 20), and acupuncture was applied on the rest points. Clinical therapeutic effect, clinical score of constipation, quality of life and score of psychological symptoms were observed before and after the treatment. Results The total effective rate in acupuncture-moxihustion group was 74.0%(37/50), which was superior to that (52.0%, 26/50) in acupuncture group (P〈0.05). The clinical scores of constipation, quality of life and scores of psychological symptoms of both groups improved after the treatment. For clinical scores of constipation, degree of difficulty in bowel movement, duration of defecation, abdominal pain, difficulty in exsufflation, incomplete sensation after defecation, obstruction sense of anus, purgative prescription dependence, score of life quality and psychological symptoms of acupuneture-moxibustion group were all better than those of simple acupuncture group (P〈0.05, P〈0.01). Conclusion The combined therapy of both moxibustion with grain-shaped moxa cones and acupuncture is safe and effective in treatment of chronic functional constipation, and the effect is superior to simple therapy of acupuncture.展开更多

文摘Objective: To evaluate the available evidence from randomized controlled trials (RCTs) of auricular acupressure (AA) therapy for preventing constipation in breast cancer patients undergoing chemotherapy. 文摘Background:In a study conducted from March to September 2021,124 cancer patients undergoing chemotherapy at our hospital were divided into two groups. The control group received routine inpatient nursing care, while the observation group received Traditional Chinese Medicine(TCM)nursing interventions in addition to routine care.Data analysis was conducted to compare the incidence of clinical adverse reactions, constipation scores, and changes in anxiety levels between the two groups. The results showed that the observation group, receiving TCM nursing interventions, had lower incidence of clinical adverse reactions and lower constipation scores compared to the control group.Additionally, anxiety levels were found to decrease significantly in the observation group post-intervention.These findings suggest that incorporating TCM nursing interventions in the care of cancer patients undergoing chemotherapy may help in reducing the occurrence of adverse reactions, alleviating constipation, and managing anxiety levels.Further research is needed to explore the full potential of integrating TCM into conventional nursing care for cancer patients. Methods:Following interventions, both groups experienced varying degrees of clinical adverse reactions, with the observation group demonstrating a significantly lower total incidence(29.03%)compared to the control group. This disparity was statistically significant(P<0.05).Furthermore,improvements were observed in defecation time(0.53±0.18)points and defecation frequency(1.17±0.25)points post-intervention.These findings suggest that the intervention had a positive impact on reducing adverse reactions and improving defecation patterns. Results:In a recent study, researchers found that individuals in the observation group experienced lower levels of difficulty with defecation and had a more regular defecation form compared to those in the control group. The results showed a significant difference in defecation difficulty and form, with the observation group scoring lower in both aspects.Interestingly, there was no significant difference in anxiety levels between the two groups prior to the intervention. However, after the intervention, both groups experienced a decrease in anxiety levels, with the observation group showing a greater reduction compared to the control group. This suggests that the intervention had a positive impact on reducing anxiety levels, particularly in the observation group, where anxiety scores were significantly lower. These findings highlight the possible benefits of certain interventions in improving both physical and psychological well-being.Conclusion:TCM nursing interventions have shown to be beneficial in reducing anxiety and improving constipation symptoms in cancer patients. These methods not only enhance the quality of life for patients but also offer a promising approach in clinical cancer treatment. The efficacy of TCM nursing highlights its value and encourages further promotion and application in future cancer care strategies.TCM nursing helps cancer patients undergoing chemotherapy with constipation and anxiety.

文摘AIM: To evaluate the efficacy of water supplementation treatment in patients with functional dyspepsia or irritable bowe syndrome (IBS) accompanying predominant constipation. CONCLUSION: Mineral water supplementation treatment for functional dyspepsia or conspipation accompanying IBS can improve gastric add output and intestinal transit time.

文摘Objective To compare the differences of therapeutic effects of chronic functional constipation treated with the combined therapy of moxibustion and acupuncture and simple acupuncture. Conclusion The combined therapy of both moxibustion with grain-shaped moxa cones and acupuncture is safe and effective in treatment of chronic functional constipation, and the effect is superior to simple therapy of acupuncture.
文摘Objective: To evaluate the short-term and middle-long-term therapeutic effects of deep puncture of Tianshu (天枢 ST 25) for slow transit constipation (STC). Methods: Sixty cases of STC patients were evenly randomized into Methods: group and medication group. In acupuncture group, electroaoupuncture (EA, 20 Hz, continuous waves and tolerable strength) was applied to bilateral Tienshu (ST 25) for 30 min after deep puncture. The treatment was given once daily, 5 sessions every week, two weeks altogether. Patients of control group were ordered to take Lactulose ( 10mL/time, b. i. d), two weeks altogether. Cleveland Constipation Score (CCS) and colonic transit time (COT) were used to evaluate the therapeutic effect. Results: After treatment, both COS and COT showed that the therapeutic effect of acupuncture group was significantly superior to that of medication group (P〈0.05). Six-months' follow up showed that the therapeutic effect of acupuncture still maintained in 13 of the 22 cases visited, while that of control group only maintained in 1 of the 19 cases. Conclusion: The therapeutic effect of deep puncture of Tianshu (ST 25) for STC is definite and has a middle-long-term effect. The patients have no any unfavorable reactions.
文摘This paper reports that 65 cases with functional constipation were treated by putting one Wang Bu Liu Xing (semen vaccariae) seed on each acupoint, Intestinum crassum, Intestinum tenue,Sympathicum, Lung, Spleen, Subcortex, of auricula and pressing, associated with acupuncture at Zusanli point on both sides. The patients were instructed to press the stuck seed twice a day for 3 to 5 minutes each time. Auricular acupoints of both sides were used alternately every 2 days. The electric acupuncture was applied at Zusanli points about 30 minutes each time every other day. Ten times (i. e. 20 days) make one course of treatment and the routine treatment consists of 1 to 2 courses. The results showed that all the patients were cured, patients with Shi Mi (the constipation of excess type) were cured after one course of treatrnent while Xu Mi (the constipation of deficiency type) were cured after two courses. The study suggests that this kind of treatment is an ideal therapy for functional constipation. The pathogenesis and mechanism of treatment of auricular acupuncture pressing associated with acupuncture at Zusanli point for constipation are discussed.
