Background:Patient participation is crucial for medication safety among older adults with chronic diseases transitioning from hospital to home,yet evidence on its levels and influencing factors remains limited.This st...Background:Patient participation is crucial for medication safety among older adults with chronic diseases transitioning from hospital to home,yet evidence on its levels and influencing factors remains limited.This study investigated the current state and predictors of such participation during this critical period.Methods:A cross-sectional study was conducted among older adults discharged from two tertiary hospitals in Fujian,China(October-December 2023).Data were collected using a general information questionnaire,a self-developed behavior scale,the Health Literacy Management Scale,General Self-Efficacy Scale,Facilitation of Patient Involvement Scale,and Family Support Scale.Analyses included ANOVA,Pearson correlation,and multiple stepwise regression.Results:Among 302 valid responses,the mean score for participation in medication safety behaviors was 68.59±11.62,indicating a moderate level.Health literacy,self-efficacy,healthcare worker support,and family support were positively correlated with participation(all P<0.01).Multiple regression identified daily medication frequency,health literacy,self-efficacy,and healthcare worker support as significant predictors,explaining 18.3%of the variance.Conclusion:Participation in medication safety among older chronic disease patients during care transitions requires improvement.Enhancing health literacy,self-efficacy,and healthcare worker support,while considering medication frequency,can promote patient involvement and medication safety.展开更多
BACKGROUND The prevalence and severity of noncommunicable chronic diseases(NCDs)among Chinese residents have been increasing with mental health emerging as a critical challenge in disease management.AIM To examine the...BACKGROUND The prevalence and severity of noncommunicable chronic diseases(NCDs)among Chinese residents have been increasing with mental health emerging as a critical challenge in disease management.AIM To examine the interactions between depression,anxiety symptoms,and related factors,and to identify key factors in the Chinese population with NCDs.METHODS Data from the Psychology and Behavior Investigation of Chinese Residents were used in a cross-sectional survey of 6182 individuals with NCDs.This study measured depression and anxiety symptoms as well as their influencing factorsincluding social environments,individual behaviors and lifestyles,and subjective indicators.A network analysis approach was used for data assessment.RESULTS Network analysis demonstrated that several central factors(media exposure,family health,problematic internet use,suboptimal health status,intimate relationship violence,tired or little energy,and nervousness/anxious/on edge)and bridge factors(media exposure,problematic internet use,intimate partner violence,health literacy,and suboptimal health status)that significantly influenced the co-occurrence and interconnectedness of depression and anxiety symptoms.Additionally,gender,ethnicity,residency,and living status did not significantly influence the overall network strength.CONCLUSION Depression and anxiety are prevalent among the Chinese population with NCDs.Effective interventions should focus on managing key symptoms,promoting correct media use for health information,and fostering healthier family relationships.展开更多

Chronic diseases are major causes of global death and disability,significantly impacting individual health and imposing economic burdens.This study aims to investigate the causal relationship between physical activity...Chronic diseases are major causes of global death and disability,significantly impacting individual health and imposing economic burdens.This study aims to investigate the causal relationship between physical activity(PA)and the development of chronic diseases.Using a Mendelian randomization(MR)approach,we incorporated average PA and its subtypes(more than 450000 participants)as exposure measures and eight chronic diseases as outcome measures.Data were obtained from the European Genome-Wide Association Study(GWAS).The primary causal analysis technique employed was the inverse variance weighting(IVW)method,with MR-Egger,weighted median,weighted mode,and simple mode used to validate the results.Sensitivity analyses were performed to assess heterogeneity and pleiotropy.The IVW approach results show that vigorous physical activity(VPA)is associated with a modest reduction in the risk of major coronary heart disease(OR=0.95,95%CI:0.91–0.99,p=0.01).The causal directions of the other four MR methods are consistent with this result and validated by sensitivity analysis.No substantial associations were found between other levels of PA and chronic disease.Our findings underscore the importance of VPA in preventive cardiology and suggest its potential role in public health initiatives.Further research should explore the impact of PA on different demographic groups and the doseresponse relationship of VPA on heart health.展开更多
Objective:Investigating the Mediating Role of Pain in the Relationship Between Sleep Quality and Health-Related Quality of Life Among Elderly Patients with Chronic Diseases.Methods:Using data from the 2018 China Healt...Objective:Investigating the Mediating Role of Pain in the Relationship Between Sleep Quality and Health-Related Quality of Life Among Elderly Patients with Chronic Diseases.Methods:Using data from the 2018 China Health and Retirement Longitudinal Study(CHARLS),this study included 3,284 participants aged 60 years or older with chronic diseases.The sleep quality,pain status,health-related quality of life,and demographic-related data of these elderly patients were obtained.Pearson correlation analysis was conducted to assess the bivariate relationships among the variables.The mediating role of pain in the sleep quality-HRQoL relationship was tested using linear regression models,complemented by bootstrap sampling to verify the indirect effect.Results:Sleep quality was positively associated with health-related quality of life(r=0.218,p<0.001).Conversely,pain demonstrated significant negative correlations with both sleep quality(r=-0.496,p<0.001)and health-related quality of life(r=-0.067,p<0.001).The mediating effect results showed that pain played a partial mediating role between sleep quality and health-related quality of life in elderly patients with chronic diseases(effect value=0.049),and the mediating effect accounted for 23.33%.Conclusion:Pain is a mediating variable between sleep quality and health-related quality of life in elderly patients with chronic diseases.This suggests that clinical practice should incorporate pain assessment into standard clinical care of elderly patients with sleep disorders and chronic diseases.By alleviating pain,sleep quality can be improved,and subsequently,their health-related quality of life can be enhanced.展开更多

Background:Depression represents a significant global mental health burden,particularly among middle-aged and older Chinese with chronic diseases in high-altitude regions,where harsh environmental conditions and limit...Background:Depression represents a significant global mental health burden,particularly among middle-aged and older Chinese with chronic diseases in high-altitude regions,where harsh environmental conditions and limited social support exacerbate mental health disparities.This paper aims to develop an interpretable machine learning prediction framework to identify the key factors of depression in this vulnerable population,thereby proposing targeted intervention measures.Methods:Utilizing data from the China Health and Retirement Longitudinal Study in 2020,this paper screened out and analyzed 2431 samples.Subsequently,Recursive Feature Elimination and Least Absolute Shrinkage and Selection Operator were applied to screen predictors from 32 alternative variables.Furthermore,through hyperparameter tuning and 5-fold cross-validation,8 machine learning modelswere constructed,namely,Random Forest,Extreme Gradient Boosting,Light Gradient Boosting Machine,Gradient Boosting Machine,K-Nearest Neighbor,Naive Bayes Classifier,Support Vector Machine,and Logistic Regression.Finally,the SHAP algorithm was applied to analyze the interpretability of the best-performing model,quantifying nonlinear relationships and threshold effects.Results:Among the respondents,the prevalence of depression was approximately 46.89%.After feature engineering screening,8 variables were retained for inclusion in the prediction model.Furthermore,the Gradient Boosting Machine performed optimally in terms of comprehensive performance,with an Area Under Receiver Operating Characteristic Curve(AUC)of 0.845,an Accuracy of 0.714,a Sensitivity of 0.655,a Precision of 0.711,a Specificity of 0.766,and an F1 of 0.682.In addition,Life satisfaction,PM2.5,Self-rated health,and Education were identified as the top 4 key factors.Meanwhile,the influence of these variables on depression showed nonlinear and threshold effects.Conclusion:This research highlights the value of machine learning in mental health.Based on the identified key factors,this paper proposed a series of policy measures to improve the health pattern of the middle-aged and elderly populations facing the dual challenges of chronic disease and environmental adversity.展开更多
To describe and analyze the current status of benefit-finding research in patients with chronic diseases and to provide a reference for clinical practice and future research in this field.Using Arksey and O’Malley’s...To describe and analyze the current status of benefit-finding research in patients with chronic diseases and to provide a reference for clinical practice and future research in this field.Using Arksey and O’Malley’s scoping review methodology as a framework,we searched PubMed,Web of Science,Science Direct,and CINAHL(4 English databases);and China National Knowledge Infrastructure,the VIP Database for Chinese Technical Periodicals,the Wan Fang Database and China Biomedical Database(4 Chinese databases)for literature on benefit finding in patients with chronic diseases,with a search time frame of construction to April 20,2024,and the literature was screened,extracted,and summarized.A total of 44 papers were included,comprising 18 in Chinese and 26 in English.Among them,a total of 19 assessment tools were used,and the influential factors investigated were categorized into socio-demographic characteristics,disease-related characteristics,psychological and behavioral characteristics,coping styles,and social support.Benefit finding is found to be a positive psychological experience and important for patients with chronic diseases,and the influencing factors are complex and diverse,with a wide range of assessment tools.However,domestic tools lack autonomy and specificity.Clinical caregivers and staff should develop relevant assessment tools and formulate a personalized research plan based on the characteristics of patients with chronic diseases in China to promote the adoption of healthy lifestyles by patients with chronic diseases,improve their self-management ability,and provide a practical basis for improving the quality of life of patients with chronic diseases.展开更多
Background:Patients are particularly vulnerable to adverse drug events during the transition from hospital to home.This risk is significantly amplified for older adults with chronic diseases,who often manage complex m...Background:Patients are particularly vulnerable to adverse drug events during the transition from hospital to home.This risk is significantly amplified for older adults with chronic diseases,who often manage complex medication regimens involving multiple drugs.Although active patient participation is recognized as a crucial component in safeguarding medication safety,there is currently a lack of a standardized,validated tool specifically designed to assess the level of such participation among this vulnerable population during this critical transition.Therefore,this study aimed to develop and validate a questionnaire for assessing the level of participation in medication safety behaviors among older adults with chronic diseases during the transition from hospital to home.Methods:Guided by the Interactive Patient Participation in Patient Safety theory,the initial questionnaire was developed through literature analysis,qualitative interviews,expert consultation,and pre-testing from April to December 2023.A total of 582 older adults with chronic diseases discharged from two tertiary hospitals in Fuzhou,China were recruited using convenience sampling.Construct validity,content validity,internal consistency,and test-retest reliability were assessed.Results:The final questionnaire comprised 20 items across three dimensions:Participation in Decision-Making,Participation in Care,Participation in Appeals.Exploratory factor analysis showed a cumulative variance contribution of 51.478%.The overall Cronbach’s a coefficient was 0.875,the scale-level content validity index was 0.930,and the test-retest reliability was 0.715.Confirmatory factor analysis indicated an acceptable model fit.Conclusion:The developed questionnaire exhibits good reliability and validity,supporting its use to evaluate participation in medication safety behaviors among Chinese older adults with chronic diseases during the hospital-to-home transition period.展开更多
Objective:To explore the connotation of the integrated medical-care-rehabilitation nursing model and its application effect in elderly patients with chronic diseases.Methods:A total of 122 elderly patients with chroni...Objective:To explore the connotation of the integrated medical-care-rehabilitation nursing model and its application effect in elderly patients with chronic diseases.Methods:A total of 122 elderly patients with chronic diseases admitted to our hospital from January 2023 to June 2023 were selected and randomly divided into an observation group(62 cases)and a control group(60 cases).Both groups received routine nursing during hospitalization.After discharge,the control group received conventional continuous nursing,while the observation group was given the integrated medical-care-rehabilitation nursing model.The psychological status of the elderly patients in the two groups was compared before nursing and 6 months after nursing.Assessments were made on their clinical symptoms of mental health,self-care ability,health behaviors,and mastery of knowledge about elderly chronic diseases.Results:Six months after nursing,the scores of self-rated clinical symptoms of mental health and negative coping in both groups were lower than those before nursing(P<0.05).Meanwhile,the scores of negative coping,self-care ability,and health behaviors in both groups were higher than those before nursing(P<0.05).Conclusion:The integrated medical-care-rehabilitation nursing model can not only improve the nursing quality for elderly patients with chronic diseases but also foster their positive mentality,help them understand knowledge about diet and health care related to chronic diseases,enhance their self-care ability and health awareness,and assist them in achieving better recovery[1].展开更多
Background:Understanding the relationship between garlic and chronic diseases could help to improve prevention and reduce the burden of diseases.This study aimed to examine the association between garlic consumption a...Background:Understanding the relationship between garlic and chronic diseases could help to improve prevention and reduce the burden of diseases.This study aimed to examine the association between garlic consumption and the risk of chronic diseases.Methods:We included 26,524 participants from the Chinese Longitudinal Healthy Longevity Survey(CLHLS)with data of the frequency of garlic consumption,7658 participants from the Xinjiang multiethnic cohort(XMC)study with data of garlic intake,and 141,684 participants from the UK Biobank(UKBB)with data of the preference for garlic.The dietary pattern of garlic,including the frequency of consuming garlic,garlic intake,and garlic preference information,was collected using a food questionnaire for each cohort.Logistic regression and structural equation modeling were used to assess the effect of garlic consumption on five common chronic diseases,which comprised cancer,diabetes,hypertension,respiratory diseases,and cardiovascular disease(CVD).Results:In the CLHLS cohort,individuals who consumed garlic almost every day had a significantly lower risk of five of the most common chronic diseases(cancer:odds ratio[OR]=0.51,95%confidence interval[CI]=0.30-0.81,p=0.006;diabetes:OR=0.58,95%CI=0.43-0.76,p<0.001;hypertension:OR=0.68,95%CI=0.61-0.77,p<0.001;respiratory diseases:OR=0.77,95%CI=0.67-0.87,p<0.001;and CVD:OR=0.69,95%CI=0.59-0.80,p<0.001).Similarly,in the XMC,there was a consistent protective effect of high garlic intake on hypertension,respiratory diseases and CVD.Additionally,in the UKBB cohort,individuals who liked garlic had a decreased risk of diabetes and CVD.Notably,in three cohorts,structural equation modeling results showed that there was a significant protective total effect of garlic consumption on the five common chronic diseases.展开更多

Given the rapid increase in the prevalence of chronic diseases in aging populations,this prospective study including 17707 adults aged≥45 years from China Health and Retirement Longitudinal Study was used to estimate...Given the rapid increase in the prevalence of chronic diseases in aging populations,this prospective study including 17707 adults aged≥45 years from China Health and Retirement Longitudinal Study was used to estimate the associations between chronic discase,multimorbidity,and depression among middle-aged and elderly adults in China,and explore the mediating factors.Depressive symptoms were assessed using the 10-item Centre for Epidemiological Studies Depression Scale(CES-D-10)questionnaire.Twelve chronic physical conditions,including hypertension,diabetes,dyslipidemia,cancer,chronic lung disease,liver disease,heart failure,stroke,kidney disease,arthritis or rheumatism,asthma,digestive disease were assessed.The prevalence rates for physical multimorbidity and depression(CES-D-10≥10)were 43.23%and 36.62%,respectively.Through multivariable logistic models and generalized estimating equation(GEE)models,we found all 12 chronic physical conditions,and multimorbidity were significantly associated with depression.Both mobility problems and chronic pain explained more than 30%of the association for all chronic conditions,with particularly high percentages for stroke(51.56%)and cancer(51.06%)in mobility problems and cancer(53.35%)in chronic pain.Limited activities of daily living(ADL)explained 34.60%of the stroke-cancer relationship,while sleep problems explained between 10.15%(stroke)and 14.89%(chronic lung disease)of the association.Individuals with chronic diseases or multimorbidity are significantly more likely to be depressed.Functional symptoms involving limitations of ADL and mobility difficulties mediated much of the association between chronic diseases and incident depression.These symptoms could be targeted for interventions to ameliorate the incidence of depression among individuals with chronic conditions.展开更多

But for the majority of patients, psychosocial and spiritual needs are neither addressed nor even considered a relevant factor by health care professionals. To measure such specific needs, the Spiritual Needs Questionnaire (SpNQ) was developed. The aim of this study was to validate the Chinese version of the SpNQ (SpNQ-Ch) and thus to measure psychosocial and spiritual needs of Chinese patients. METHODS: This was a cross-sectional study among 168 patients with chronic diseases who were recruited in the Changhai Hospital of Traditional Chinese Medicine, Shanghai, China using standardized questionnaires. We performed reliability and factor analyses, as well as analyses of variance, first order correlations and regression analyses. RESULTS: The 17-item SpNQ-Ch had a similar factorial structure as the original version with two main and three minor factors which accounted for 64% of variance, and internal consistency estimates (Cronbach's a) ranging from 0.51 to 0.81. Included were the 4-item scale Inner Peace Needs, the 5-item scale Giving/Generativity (with 2 sub-constructs, Praying and Sources), and a Needs, the 5-item scale Religious Needs 3-item scale Reflection/Release Needs. In Chinese patients with cancer (63%), pain affections (10%), or other chronic conditions (23%), the needs for Giving/Generativity (which refer to categories of Connectedness and Meaning) and Inner Peace Needs scored highest, while Religious Needs and the Reflection/Release Needs scored lower. CONCLUSION: The SpNQ-Ch is congruent with its primary version, and can be used in future studies with the mostly nonreligious patients from China. First findings indicate specific psychosocial and spiritual needs which should be addressed by health care professionals to support patients in their struggle with chronic illness in terms of psycho-emotional stabilization, finding hope and meaning, and thus achieving peaceful states of mind despite chronic illness.展开更多
文摘This review aims to comprehensively discuss the application status,theoretical basis,practical methods,empirical research,challenges,and future development direction of patient journey map in chronic disease management.By integrating current advances in academia and practice,this paper suggests that the patient journey map is an effective tool that can significantly improve the efficiency and quality of care delivery,increase patient engagement,and optimize the allocation of health care resources in chronic disease management.Nevertheless,the popularization and deepening application of patient journey map also faces a series of problems,such as difficulties in data collection,challenges in technology integration,and resistance to organizational change.In the future,with the progress of technology and policy support,the patient journey map is expected to become a key force to promote the innovation of chronic disease management model.Through in-depth analysis of these problems and opportunities,this paper provides valuable reference and enlightenment for healthcare providers,policy makers and relevant stakeholders.
文摘This editorial highlights a recently published study examining the effectiveness of music therapy combined with motivational interviewing(MI)in addressing an-xiety and depression among young and middle-aged patients following percuta-neous coronary intervention.It further explores existing evidence and potential future research directions for MI in postoperative rehabilitation and chronic disease management.MI aims to facilitate behavioral change and promote healthier lifestyles by fostering a trusting relationship with patients and enhan-cing intrinsic motivation.Research has demonstrated its effectiveness in posto-perative recovery for oncological surgery,stroke,organ transplants,and gastroin-testinal procedures,as well as in managing chronic conditions such as diabetes,obesity,and periodontal disease.The approach is patient-centered,adaptable,cost-effective,and easily replicable,though its limitations include reliance on the therapist’s expertise,variability in individual responses,and insufficient long-term follow-up studies.Future research could focus on developing individualized and precise intervention models,exploring applications in digital health management,and confirming long-term outcomes to provide more compre-hensive support for patient rehabilitation.

文摘Background:Patient participation is crucial for medication safety among older adults with chronic diseases transitioning from hospital to home,yet evidence on its levels and influencing factors remains limited.This study investigated the current state and predictors of such participation during this critical period.Methods:A cross-sectional study was conducted among older adults discharged from two tertiary hospitals in Fujian,China(October-December 2023).Data were collected using a general information questionnaire,a self-developed behavior scale,the Health Literacy Management Scale,General Self-Efficacy Scale,Facilitation of Patient Involvement Scale,and Family Support Scale.Analyses included ANOVA,Pearson correlation,and multiple stepwise regression.Results:Among 302 valid responses,the mean score for participation in medication safety behaviors was 68.59±11.62,indicating a moderate level.Health literacy,self-efficacy,healthcare worker support,and family support were positively correlated with participation(all P<0.01).Multiple regression identified daily medication frequency,health literacy,self-efficacy,and healthcare worker support as significant predictors,explaining 18.3%of the variance.Conclusion:Participation in medication safety among older chronic disease patients during care transitions requires improvement.Enhancing health literacy,self-efficacy,and healthcare worker support,while considering medication frequency,can promote patient involvement and medication safety.
文摘BACKGROUND The prevalence and severity of noncommunicable chronic diseases(NCDs)among Chinese residents have been increasing with mental health emerging as a critical challenge in disease management.AIM To examine the interactions between depression,anxiety symptoms,and related factors,and to identify key factors in the Chinese population with NCDs.METHODS Data from the Psychology and Behavior Investigation of Chinese Residents were used in a cross-sectional survey of 6182 individuals with NCDs.This study measured depression and anxiety symptoms as well as their influencing factorsincluding social environments,individual behaviors and lifestyles,and subjective indicators.A network analysis approach was used for data assessment.RESULTS Network analysis demonstrated that several central factors(media exposure,family health,problematic internet use,suboptimal health status,intimate relationship violence,tired or little energy,and nervousness/anxious/on edge)and bridge factors(media exposure,problematic internet use,intimate partner violence,health literacy,and suboptimal health status)that significantly influenced the co-occurrence and interconnectedness of depression and anxiety symptoms.Additionally,gender,ethnicity,residency,and living status did not significantly influence the overall network strength.CONCLUSION Depression and anxiety are prevalent among the Chinese population with NCDs.Effective interventions should focus on managing key symptoms,promoting correct media use for health information,and fostering healthier family relationships.
文摘In this article,we comment on the recent network analysis by Li et al,which explores depression,anxiety,and their associated factors in individuals with noncommunicable chronic diseases(NCDs).We highlight three often-overlooked domains in this population:Mental vulnerability(depression,anxiety,fatigue,nervousness),dysfunction in family functioning(family health,intimate partner violence),and digital exposure(media exposure,problematic internet use),and examine their dynamic interrelations within a psychosocial network framework.The findings reveal a complex system in which emotional dysregulation,family relational adversity,and digital behaviors mutually reinforce one another to heigh-ten psychiatric risk.Notably,traditional demographic factors such as gender,ethnicity,and residence had minimal impact on overall network strength.We argue that these interconnected domains represent modifiable yet underrecognized targets for mental health intervention among medically vulnerable populations.Strategies that promote emotional resilience,strengthen family systems,and enhance digital literacy may play a pivotal role in disrupting maladaptive network pathways and improving quality of life for individuals with NCDs.Future research should prioritize longitudinal and multimodal designs to clarify causal mechanisms and support tailored,systems-level psychosocial interventions.
文摘Chronic diseases,or NCDs(noncommunicable diseases),constitute a major global health challenge,causing millions of deaths and imposing substantial economic burdens annually.This paper introduces the Health Score,a comprehensive framework for assessing chronic disease risk by integrating diverse determinants of health,including social,economic,environmental,behavioral,treatment,culture,and nature factors.The Health Score,ranging from 0 to 850,quantifies indivi dual and population-level health risks while identifying protective factors through a structured methodology that supports targeted interventions at individual,corporate,and community scales.The paper highlights the rising prevalence of chronic diseases in the United States,projecting that nearly half of the population will be affected by 2030,alongside a global economic burden expected to reach trillions of dollars.Existing surveillance tools,such as the CDS(Chronic Disease Score)and CDIs(Chronic Disease Indicators),are examined for their roles in monitoring health disparities.The Health Score advances a holistic,proactive approach,emphasizing lifestyle modifications,equitable healthcare access,economic opportunities,social support,nature exposure,cu ltural awareness,and community engagement.By elucidating the complex interplay of health determinants,this framework equips stakeholders with actionable insights to implement effective prevention strategies,ultimately fostering healthier,more resi lient populations.

文摘Nuclear receptors(NRs)function as crucial transcription factors in orchestrating essential functions within the realms of development,host defense,and homeostasis of body.NRs have garnered increased attention due to their potential as therapeutic targets,with drugs directed at NRs demonstrating significant efficacy in impeding chronic disease progression.Consequently,these pharmacological agents hold promise for the treatment and management of various diseases.Accumulating evidence emphasizes the regulatory role of exosome-derived microRNAs(miRNAs)in chronic inflammation,disease progression,and therapy resistance,primarily by modulating transcription factors,particularly NRs.By exploiting inflammatory pathways such as protein kinase B(Akt)/mammalian target of rapamycin(mTOR),nuclear factor kappa-B(NF-κB),signal transducer and activator of transcription 3(STAT3),and Wnt/β-catenin signaling,exosomes and NRs play a pivotal role in the panorama of development,physiology,and pathology.The internalization of exosomes modulates NRs and initiates diverse autocrine or paracrine signaling cascades,influencing various processes in recipient cells such as survival,proliferation,differentiation,metabolism,and cellular defense mechanisms.This comprehensive review meticulously examines the involvement of exosome-mediated NRs regulation in the pathogenesis of chronic ailments,including atherosclerosis,cancer,diabetes,liver diseases,and respiratory conditions.Additionally,it elucidates the molecular intricacies of exosome-mediated communication between host and recipient cells via NRs,leading to immunomodulation.Furthermore,it outlines the implications of exosome-modulated NR pathways in the prophylaxis of chronic inflammation,delineates current limitations,and provides insights into future perspectives.This review also presents existing evidence on the role of exosomes and their components in the emergence of therapeutic resistance.
文摘Chronic diseases are major causes of global death and disability,significantly impacting individual health and imposing economic burdens.This study aims to investigate the causal relationship between physical activity(PA)and the development of chronic diseases.Using a Mendelian randomization(MR)approach,we incorporated average PA and its subtypes(more than 450000 participants)as exposure measures and eight chronic diseases as outcome measures.Data were obtained from the European Genome-Wide Association Study(GWAS).The primary causal analysis technique employed was the inverse variance weighting(IVW)method,with MR-Egger,weighted median,weighted mode,and simple mode used to validate the results.Sensitivity analyses were performed to assess heterogeneity and pleiotropy.The IVW approach results show that vigorous physical activity(VPA)is associated with a modest reduction in the risk of major coronary heart disease(OR=0.95,95%CI:0.91–0.99,p=0.01).The causal directions of the other four MR methods are consistent with this result and validated by sensitivity analysis.No substantial associations were found between other levels of PA and chronic disease.Our findings underscore the importance of VPA in preventive cardiology and suggest its potential role in public health initiatives.Further research should explore the impact of PA on different demographic groups and the doseresponse relationship of VPA on heart health.

文摘Objective:Investigating the Mediating Role of Pain in the Relationship Between Sleep Quality and Health-Related Quality of Life Among Elderly Patients with Chronic Diseases.Methods:Using data from the 2018 China Health and Retirement Longitudinal Study(CHARLS),this study included 3,284 participants aged 60 years or older with chronic diseases.The sleep quality,pain status,health-related quality of life,and demographic-related data of these elderly patients were obtained.Pearson correlation analysis was conducted to assess the bivariate relationships among the variables.The mediating role of pain in the sleep quality-HRQoL relationship was tested using linear regression models,complemented by bootstrap sampling to verify the indirect effect.Results:Sleep quality was positively associated with health-related quality of life(r=0.218,p<0.001).Conversely,pain demonstrated significant negative correlations with both sleep quality(r=-0.496,p<0.001)and health-related quality of life(r=-0.067,p<0.001).The mediating effect results showed that pain played a partial mediating role between sleep quality and health-related quality of life in elderly patients with chronic diseases(effect value=0.049),and the mediating effect accounted for 23.33%.Conclusion:Pain is a mediating variable between sleep quality and health-related quality of life in elderly patients with chronic diseases.This suggests that clinical practice should incorporate pain assessment into standard clinical care of elderly patients with sleep disorders and chronic diseases.By alleviating pain,sleep quality can be improved,and subsequently,their health-related quality of life can be enhanced.

文摘Objective:To study the application effect of comprehensive geriatric assessment tool-oriented chronic disease trajectory nursing in community elderly hypertension.Methods:Sixty nine elderly hypertensive patients admitted to the hospital from April 2022 to April 2024 were randomly assigned to the intervention group(n=35)and the control group(n=34).The control group received routine community hypertension health management,while the intervention group underwent comprehensive geriatric assessment for comprehensive screening of elderly health issues based on routine management,and implemented targeted nursing interventions based on the characteristics of the chronic disease trajectory.After three months of intervention,the blood pressure,medication compliance,self-efficacy,and quality of life of the two groups were evaluated and analyzed.Results:After the intervention,the intervention group showed significant improvement in blood pressure control,medication compliance,self-management efficacy scores,and quality of life scores compared to the control group(P<0.05).Conclusion:Combining comprehensive geriatric assessment with the concept of chronic disease trajectory nursing in the management of community elderly hypertension can effectively optimize the effect of chronic disease management,which has important practical value.

文摘Thousands of years ago,the concept of Jiu Zuo Shang Rou(久坐伤肉long-time sitting damages muscles)was introduced in traditional Chinese medicine(TCM).It was clearly recorded in ancient books,that prolonged sitting disrupts the circulation of qi and blood,damages muscles,impairs spleen function,and ultimately leads to diseases.Modern biomedical evidence shows that sedentary behavior,including prolonged sitting,affects endocrine,metabolic,and physiological functions,increasing the risk of chronic diseases.This article systematically reviews TCM records of the health impacts of long-time sitting and biomedical findings,to explore the pathophysiological mechanisms underlying the health risks of prolonged sitting.By integrating TCM's preventative philosophy,namely Zhi Wei Bing(治未病preventing a disease before it arises),with modern preventive medicine,this study offers insights into strategies for mitigating the health risks associated with sedentary behavior.

文摘Background:Depression represents a significant global mental health burden,particularly among middle-aged and older Chinese with chronic diseases in high-altitude regions,where harsh environmental conditions and limited social support exacerbate mental health disparities.This paper aims to develop an interpretable machine learning prediction framework to identify the key factors of depression in this vulnerable population,thereby proposing targeted intervention measures.Methods:Utilizing data from the China Health and Retirement Longitudinal Study in 2020,this paper screened out and analyzed 2431 samples.Subsequently,Recursive Feature Elimination and Least Absolute Shrinkage and Selection Operator were applied to screen predictors from 32 alternative variables.Furthermore,through hyperparameter tuning and 5-fold cross-validation,8 machine learning modelswere constructed,namely,Random Forest,Extreme Gradient Boosting,Light Gradient Boosting Machine,Gradient Boosting Machine,K-Nearest Neighbor,Naive Bayes Classifier,Support Vector Machine,and Logistic Regression.Finally,the SHAP algorithm was applied to analyze the interpretability of the best-performing model,quantifying nonlinear relationships and threshold effects.Results:Among the respondents,the prevalence of depression was approximately 46.89%.After feature engineering screening,8 variables were retained for inclusion in the prediction model.Furthermore,the Gradient Boosting Machine performed optimally in terms of comprehensive performance,with an Area Under Receiver Operating Characteristic Curve(AUC)of 0.845,an Accuracy of 0.714,a Sensitivity of 0.655,a Precision of 0.711,a Specificity of 0.766,and an F1 of 0.682.In addition,Life satisfaction,PM2.5,Self-rated health,and Education were identified as the top 4 key factors.Meanwhile,the influence of these variables on depression showed nonlinear and threshold effects.Conclusion:This research highlights the value of machine learning in mental health.Based on the identified key factors,this paper proposed a series of policy measures to improve the health pattern of the middle-aged and elderly populations facing the dual challenges of chronic disease and environmental adversity.
文摘To describe and analyze the current status of benefit-finding research in patients with chronic diseases and to provide a reference for clinical practice and future research in this field.Using Arksey and O’Malley’s scoping review methodology as a framework,we searched PubMed,Web of Science,Science Direct,and CINAHL(4 English databases);and China National Knowledge Infrastructure,the VIP Database for Chinese Technical Periodicals,the Wan Fang Database and China Biomedical Database(4 Chinese databases)for literature on benefit finding in patients with chronic diseases,with a search time frame of construction to April 20,2024,and the literature was screened,extracted,and summarized.A total of 44 papers were included,comprising 18 in Chinese and 26 in English.Among them,a total of 19 assessment tools were used,and the influential factors investigated were categorized into socio-demographic characteristics,disease-related characteristics,psychological and behavioral characteristics,coping styles,and social support.Benefit finding is found to be a positive psychological experience and important for patients with chronic diseases,and the influencing factors are complex and diverse,with a wide range of assessment tools.However,domestic tools lack autonomy and specificity.Clinical caregivers and staff should develop relevant assessment tools and formulate a personalized research plan based on the characteristics of patients with chronic diseases in China to promote the adoption of healthy lifestyles by patients with chronic diseases,improve their self-management ability,and provide a practical basis for improving the quality of life of patients with chronic diseases.

文摘Background:Patients are particularly vulnerable to adverse drug events during the transition from hospital to home.This risk is significantly amplified for older adults with chronic diseases,who often manage complex medication regimens involving multiple drugs.Although active patient participation is recognized as a crucial component in safeguarding medication safety,there is currently a lack of a standardized,validated tool specifically designed to assess the level of such participation among this vulnerable population during this critical transition.Therefore,this study aimed to develop and validate a questionnaire for assessing the level of participation in medication safety behaviors among older adults with chronic diseases during the transition from hospital to home.Methods:Guided by the Interactive Patient Participation in Patient Safety theory,the initial questionnaire was developed through literature analysis,qualitative interviews,expert consultation,and pre-testing from April to December 2023.A total of 582 older adults with chronic diseases discharged from two tertiary hospitals in Fuzhou,China were recruited using convenience sampling.Construct validity,content validity,internal consistency,and test-retest reliability were assessed.Results:The final questionnaire comprised 20 items across three dimensions:Participation in Decision-Making,Participation in Care,Participation in Appeals.Exploratory factor analysis showed a cumulative variance contribution of 51.478%.The overall Cronbach’s a coefficient was 0.875,the scale-level content validity index was 0.930,and the test-retest reliability was 0.715.Confirmatory factor analysis indicated an acceptable model fit.Conclusion:The developed questionnaire exhibits good reliability and validity,supporting its use to evaluate participation in medication safety behaviors among Chinese older adults with chronic diseases during the hospital-to-home transition period.
文摘Objective:To explore the connotation of the integrated medical-care-rehabilitation nursing model and its application effect in elderly patients with chronic diseases.Methods:A total of 122 elderly patients with chronic diseases admitted to our hospital from January 2023 to June 2023 were selected and randomly divided into an observation group(62 cases)and a control group(60 cases).Both groups received routine nursing during hospitalization.After discharge,the control group received conventional continuous nursing,while the observation group was given the integrated medical-care-rehabilitation nursing model.The psychological status of the elderly patients in the two groups was compared before nursing and 6 months after nursing.Assessments were made on their clinical symptoms of mental health,self-care ability,health behaviors,and mastery of knowledge about elderly chronic diseases.Results:Six months after nursing,the scores of self-rated clinical symptoms of mental health and negative coping in both groups were lower than those before nursing(P<0.05).Meanwhile,the scores of negative coping,self-care ability,and health behaviors in both groups were higher than those before nursing(P<0.05).Conclusion:The integrated medical-care-rehabilitation nursing model can not only improve the nursing quality for elderly patients with chronic diseases but also foster their positive mentality,help them understand knowledge about diet and health care related to chronic diseases,enhance their self-care ability and health awareness,and assist them in achieving better recovery[1].

文摘Background:Understanding the relationship between garlic and chronic diseases could help to improve prevention and reduce the burden of diseases.This study aimed to examine the association between garlic consumption and the risk of chronic diseases.Methods:We included 26,524 participants from the Chinese Longitudinal Healthy Longevity Survey(CLHLS)with data of the frequency of garlic consumption,7658 participants from the Xinjiang multiethnic cohort(XMC)study with data of garlic intake,and 141,684 participants from the UK Biobank(UKBB)with data of the preference for garlic.The dietary pattern of garlic,including the frequency of consuming garlic,garlic intake,and garlic preference information,was collected using a food questionnaire for each cohort.Logistic regression and structural equation modeling were used to assess the effect of garlic consumption on five common chronic diseases,which comprised cancer,diabetes,hypertension,respiratory diseases,and cardiovascular disease(CVD).Results:In the CLHLS cohort,individuals who consumed garlic almost every day had a significantly lower risk of five of the most common chronic diseases(cancer:odds ratio[OR]=0.51,95%confidence interval[CI]=0.30-0.81,p=0.006;diabetes:OR=0.58,95%CI=0.43-0.76,p<0.001;hypertension:OR=0.68,95%CI=0.61-0.77,p<0.001;respiratory diseases:OR=0.77,95%CI=0.67-0.87,p<0.001;and CVD:OR=0.69,95%CI=0.59-0.80,p<0.001).Similarly,in the XMC,there was a consistent protective effect of high garlic intake on hypertension,respiratory diseases and CVD.Additionally,in the UKBB cohort,individuals who liked garlic had a decreased risk of diabetes and CVD.Notably,in three cohorts,structural equation modeling results showed that there was a significant protective total effect of garlic consumption on the five common chronic diseases.
文摘Objective:To explore the application effect of continuous nursing in elderly patients with chronic diseases.Methods:A total of 100 elderly patients with chronic diseases were selected and randomly divided into experimental group and control group,50 cases in each group.Patients in the experimental group received continuous nursing,while patients in the control group received routine nursing.Comparison of two groups of patients’quality of life,health and nursing satisfaction.Results:the patient’s quality of life,health and nursing satisfaction were significantly higher than control group(p<0.05).Conclusion:Continuous nursing has a significant application effect in elderly patients with chronic diseases,which is worthy of further promotion and application.

文摘Chronic diseases are the leading global causes of death in China and the world, especially hypertension 'and diabetes, and the main reasons are heredity, abnormal metabolism and an unhealthy lifestyle for dietary. Barley is taditional medicine in China. There is an very important effect for functional foods for preventing chronic diseases of barley grains and barley grass powder as well as its products because of its high contents of minerals, vitamins, amino acids, antioxidant enzymes, chlorophyll and bioactive compounds such as β-glucans, phenolic compounds, GABA, alkaloid, tocopherols and tocotrienols, dietary fiber and so on. There are a lot of challenges to promote health for functional food with barley grains and its grass in China. Barley grain will produce pearled barley, grits, flakes and flour, and it adds texture, flavor, aroma, nutritional and medicinal value to product. Barley grass powder can be produced in barley green, barley grass rice noodle, barley grass ersi, barley grass noodles and barley green beer and other new functional foods. The future for barley use in food products is improving and very promising.

文摘Given the rapid increase in the prevalence of chronic diseases in aging populations,this prospective study including 17707 adults aged≥45 years from China Health and Retirement Longitudinal Study was used to estimate the associations between chronic discase,multimorbidity,and depression among middle-aged and elderly adults in China,and explore the mediating factors.Depressive symptoms were assessed using the 10-item Centre for Epidemiological Studies Depression Scale(CES-D-10)questionnaire.Twelve chronic physical conditions,including hypertension,diabetes,dyslipidemia,cancer,chronic lung disease,liver disease,heart failure,stroke,kidney disease,arthritis or rheumatism,asthma,digestive disease were assessed.The prevalence rates for physical multimorbidity and depression(CES-D-10≥10)were 43.23%and 36.62%,respectively.Through multivariable logistic models and generalized estimating equation(GEE)models,we found all 12 chronic physical conditions,and multimorbidity were significantly associated with depression.Both mobility problems and chronic pain explained more than 30%of the association for all chronic conditions,with particularly high percentages for stroke(51.56%)and cancer(51.06%)in mobility problems and cancer(53.35%)in chronic pain.Limited activities of daily living(ADL)explained 34.60%of the stroke-cancer relationship,while sleep problems explained between 10.15%(stroke)and 14.89%(chronic lung disease)of the association.Individuals with chronic diseases or multimorbidity are significantly more likely to be depressed.Functional symptoms involving limitations of ADL and mobility difficulties mediated much of the association between chronic diseases and incident depression.These symptoms could be targeted for interventions to ameliorate the incidence of depression among individuals with chronic conditions.
文摘OBJECTIVE: Even in secular societies, a small portion of patients find their spirituality to help cope with illness. But for the majority of patients, psychosocial and spiritual needs are neither addressed nor even considered a relevant factor by health care professionals. To measure such specific needs, the Spiritual Needs Questionnaire (SpNQ) was developed. The aim of this study was to validate the Chinese version of the SpNQ (SpNQ-Ch) and thus to measure psychosocial and spiritual needs of Chinese patients. METHODS: This was a cross-sectional study among 168 patients with chronic diseases who were recruited in the Changhai Hospital of Traditional Chinese Medicine, Shanghai, China using standardized questionnaires. We performed reliability and factor analyses, as well as analyses of variance, first order correlations and regression analyses. RESULTS: The 17-item SpNQ-Ch had a similar factorial structure as the original version with two main and three minor factors which accounted for 64% of variance, and internal consistency estimates (Cronbach's a) ranging from 0.51 to 0.81. Included were the 4-item scale Inner Peace Needs, the 5-item scale Giving/Generativity (with 2 sub-constructs, Praying and Sources), and a Needs, the 5-item scale Religious Needs 3-item scale Reflection/Release Needs. In Chinese patients with cancer (63%), pain affections (10%), or other chronic conditions (23%), the needs for Giving/Generativity (which refer to categories of Connectedness and Meaning) and Inner Peace Needs scored highest, while Religious Needs and the Reflection/Release Needs scored lower. CONCLUSION: The SpNQ-Ch is congruent with its primary version, and can be used in future studies with the mostly nonreligious patients from China. First findings indicate specific psychosocial and spiritual needs which should be addressed by health care professionals to support patients in their struggle with chronic illness in terms of psycho-emotional stabilization, finding hope and meaning, and thus achieving peaceful states of mind despite chronic illness.
