Therefore, greater therapeutic efficacy for cerebral palsy could be obtained by selecting the five Shu acupoints, which are located at the qi-intersecting point of the twelve meridians below the elbow and knee. OBJECTIVE: To treat children with spastic cerebral palsy using acupuncture and massage at the Shu and He points through a combination of rehabilitative training, and to compare the effects with routine acupuncture and massage. DESIGN: A randomized controlled clinical trial. SETTING: Rehabilitation Center for Cerebral Palsy, Department of Pediatrics, Affiliated Hospital of Liaoning University of Traditional Chinese Medicine. PARTICIPANTS: Sixty children with spastic cerebral palsy were selected from the Rehabilitation Center for Cerebral Palsy, Department of Pediatrics, Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from May 2003 to February 2006. There were 38 boys and 22 girls, aged 10 months to 4 years old, muscle strength ranging from grade I to grade IV. The children were randomly divided into a treatment group (n =30) and a control group (n =30). Informed consent for the therapeutic program was obtained from the relatives of all enrolled children. The study was approved by the Hospital's Ethical Committee. METHODS: All children were given exercise and occupational therapy based on standard treatments. (1) In the control group, the children were additionally treated with routine acupuncture and massage during the one-month basic rehabilitative training. The acupuncture needles were manipulated alternately at the acupoints of Jianyu, Shousanli, Binao, Hegu, Huantiao, Xuanzhong, and Biguan. They were treated once per day, 6 days a week, followed by one day of rest. The children's limbs were first massaged by pressing, stretching, rolling, and kneading. Then, the same acupuncture points were pressed with the thumb for 30 minutes each time, once a day, 6 days a week, followed by one day of rest. The treatment continued for 1 month. (2) In the treatment group, the children were administered acupuncture and massage at the Shu and He points during the one-month basic rehabilitative treatment. For acupuncture of the upper limbs, the following acupoints were used: Sanjian and Quchi of the Large Intestine Meridian of Hand-Yangming, and Houxi and Xiaohai of the Small Intestine Meridian of Hand-Taiyang. The acupoints were acupunctured alternately every other day. For acupuncture of the lower limbs, the following acupoints were used; Zulinqi and Yanglingquan of the Gall bladder Meridian of Foot-Shaoyang, Taibai and Yinlingquan of the Spleen Meridian of Foot-Taiyang, or Xiangu and Zusanli of the Stomach Meridian of Foot-Yangming. The acupoints were acupunctured alternately every other day. Three needles were for the medial malleolus and three needles for the lateral malleolus. They were treated once a day, 6 days a week, followed by one day of rest. The children's limbs were first massaged by means of pressing, stretching, rolling, and kneading. The same acupuncture points were then pressed with the thumb for 0.5-1 minute at each point, 30 minutes each time, once a day, 6 days a week, followed by one day of rest. MAIN OUTCOME MEASURES: Therapeutic efficacy was evaluated using gross motor function measurements, as well as activity of daily living scale before and after treatment. RESULTS: All 60 enrolled children with spastic cerebral palsy were included in the final analysis of results. (1) Comparison of gross motor function scores: The gross motor function score after treatment was significantly higher in both the treatment group and control group than before treatment (t = -3.86 and -8.882, respectively, P 〈 0.05). The scores in the treatment group were significantly higher than the control group scores (t = -7.166, P 〈 0.01). (2) Changes in activity of daily living scores: The scores for activity of daily living in the treatment group was significantly higher after treatment than before treatment (t = -3.933, P 〈 0.01), and was also significantly higher than the control group (t = -0.671, P 〈 0.05). CONCLUSION: Acupuncture and massage at the Shu and He acupoints can significantly ameliorate motor function deficits and movement disabilities in children with spastic cerebral palsy. The therapeutic efficacy at these acupoints is better than routine acupuncture and massage.展开更多

METHODS: MaSPs were identified and marked on cadavers before acrylic color was injected. The underlying structures marked with acrylic colors were observed and the anatomical structures were determined. Then, pressure was applied to each MaSP in human volunteers (lateral side of leg and medial side of leg) and blood flow on right dorsalis pedis artery was measured using duplex ultrasound while skin temperature changes were monitored using an infrared themographic camera. RESULTS: Short-term changes in the blood flow parameters, volume flow and average velocity, compared to baseline (P 〈 0.05), were observed on MaSP of the lower extremity, ML4. Changes in the peak systolic velocity of the area ML5 were also observed relative to baseline. The skin temperature of two different MaSPs on the lateral side of leg (LL4 and LL5) and four on the medial side of leg (ML2, ML3, ML4 and ML5) was significantly increased (P 〈 0.05) at 1 rain after pressure application. CONCLUSION: This study established the clear correlation between the location of MaSP, as defined in CTTM, and the underlying anatomical structures. The effect of massage can stimulate skin blood flow because results showed increased skin temperature and blood flow characteristics. While these results were statistically significant, they may not be clinically relevant, as the present study focused on the immediate physiological effect of manipulation, rather than treatment effects. Thus, this study will serve as baseline data for further clinical studies in CTTM展开更多

We recruited 20 women with knee OA,who then underwent Chinese massage therapy three times per week for 2 weeks. The patients underwent gait evaluation using a six-camera infrared motion analysis system. They completed Western Ontario and Mc Master Universities Osteoarthritis Index questionnaires before and after treatment. We calculated the forward speed, step width, step length, total support time percentage, initial double support time percentage, and single support time percentage. We also measured the angles at the knee, hip, and ankle during the stance phase of walking. The results showed statistically significant mean differences in knee pain relief, alleviation of stiffness, and physical function enhancement after therapy(P < 0.05). The patients gained significantly faster gait speed, greater step width, and increased total support time percentage after the Chinese massage therapy(P < 0.05). There were no significant differences in the range of motion or initial contact angles of the knee, hip,or ankle during the stance phase of walking. We concluded that Chinese massage is a beneficial complementary treatment and an alternative therapy choice for patients with knee OA for short-term pain relief. Chinese massage may improve walking ability for these patients.展开更多

The central nervous system (CNS) alterations mainly interfere with the anatomical organization of areas undergoing a phase of intense postnatal cell proliferation, disrupting plastic processes like learning, memory, and phonation. In the rat pup, prenatal malnutrition interferes with the elaboration of ultrasonic vocalizations (USVs) by poorly understood mechanisms. The neuronal caudal portion of the nucleus ambiguus (Ambc) innervates the laryngeal intrinsic muscles to produce phonation, a basic USV communication system. During postnatal development, enhanced plasticity phenomena play a fundamental role in improving brain function. Thus, the massage stimulation (MS) may accelerate growth and induce neurogenesis in different areas of the brain. The current study analyzed the effects of a daily 10-min MS on the dendritic tree and perikarya measurements of Ambc multipolar motoneurons (Golgi-Cox) of perinatally underfed (U), control (C), control massage-stimulated (CMS), and underfed massage-stimulated (UMS) groups at postnatal days (PDs) 8, 12, and 15. The data indicated that the dendritic scores were reduced (p < 0.05) in both number and density at PD8 and PD15 in the U subjects and that MS increased the values of these parameters (p < 0.05). In addition, MS induced body weight gain in both U and CMS groups, and it enhanced the dendritic density in CMS subjects. These results show that MS during the pre-weaning period restores the plastic properties of the Ambc over the hypoplastic multipolar motoneuron after the alterations caused by perinatal undernutrition.展开更多

文摘BACKGROUND Comminuted calcaneal fractures present significant treatment challenges.Open reduction and internal fixation carries risks such as infection and skin necrosis, while minimally invasive techniques may compromise reduction stability.Conservative management is generally limited to minimally displaced fractures.Traditional Chinese manual bone-setting has a long history in fracture treatment and is renowned globally for achieving functional reduction. It offers distinct advantages including lower cost, minimal soft tissue trauma, and the avoidance of expensive reduction equipment or internal fixation materials.CASE SUMMARY A 60-year-old female presented with left foot pain and limited mobility following a fall.Computed tomography scan revealed a Sanders type IV calcaneal fracture with a Böhler angle of 0°.A standardized, stepwise Traditional Chinese manual bone-setting was initially performed, followed by percutaneous screw fixation through several mini-incisions after satisfactory alignment was confirmed under fluoroscopy. The Visual Analog Scale score decreased from 5 on postoperative day 1 to 3 by day 3.The American Orthopaedic Foot and Ankle Society score improved from 73 at 6 weeks to 90 at 3 months, indicating rapid functional recovery and high patient satisfaction. At 7 months postoperatively, the American Orthopaedic Foot and Ankle Society score reached 95,prompting removal of internal fixation. At the 6-year follow-up,reduction remained well maintained, with the Böhler angle preserved at 22°.CONCLUSION The combination of Traditional Chinese manual bone-setting and percutaneous screw fixation achieved satisfactory functional reduction for comminuted calcaneal fractures.

文摘BACKGROUND: Based on clinical findings and syndromes, it has been previously shown that therapeutic methods for cerebral palsy can be greatly improved by selecting fewer, but more accurate acupoints. The therapeutic efficacy at these acupoints is better than routine acupuncture and massage.

文摘OBJECTIVE: This study aims to investigate the relationship between major signal points (MaSPs) of the lower extremities used in court-type Thai traditional massage (CTTM) and the corresponding underlying anatomical structures, as well as to determine the short-term changes in blood flow and skin temperature of volunteers experiencing CTTM. Thus, this study will serve as baseline data for further clinical studies in CTTM

文摘Massage therapy is an alternative treatment for chronic pain that is potentially related to brain plasticity. However, the underlying mechanism remains unclear. We established a peripheral nerve injury model in rats by unilateral sciatic nerve transection and direct anastomosis. The experimental rats were treated over the gastrocnemius muscle of the affected hindlimb with a customized massage instrument(0.45 N,120 times/min,10 minutes daily, for 4 successive weeks).Resting-state functional magnetic resonance imaging revealed that compared with control rats, the amplitude of low-frequency fluctuations in the sensorimotor cortex contralateral to the affected limb was significantly lower after sciatic nerve transection. However, amplitudes were significantly higher in the massage group than in a sham-massage group. These findings suggest that massage therapy facilitated adaptive change in the somatosensory cortex that led to the recovery of peripheral nerve injury and repair. This study was approved by the Animal Ethics Committee of Shanghai University of Traditional Chinese Medicine of China(approval No.201701001)on January 12,2017.

文摘The objective of this study was to evaluate the effectiveness of Chinese massage therapy in patients with knee osteoarthritis(OA) by measuring lower-limb gait parameters. Chinese massage may improve walking ability for these patients.

文摘OBJECTIVE:To evaluate the efficacy of meridian massage on motor function after a stroke.METHODS:This systematic review and Meta-analysis of randomised(RCTs)and quasi-randomised controlled trials(q RCTs)was performed to determine the effects of meridian massage on motor dysfunction following a stroke.Articles published until December 2020 related to stroke and meridian massage from National Library of Medicine,Cochrane Central Register of Controlled Trials,Embase,Korea Med,Japan science and Technology Agency,China National Knowledge Infrastructure Database,Wanfang Database,Chinese Biomedical Literature Database and China Science and Technology Journal Database were searched. The primary outcomes included Fugl-Mayer Assessment(FMA),modified Barthel Index(m BI)and clinical efficiency, while the secondary outcomes included modified Ashworth(m AS)and Berg Balance Scale(BBS).Two authors independently selected the literature, extracted the data and assessed the risk of bias, after which Meta-analysis with subgroups and sensitivity analysis was conducted.RESULTS:A total of 16 RCTs and 3 q-RCTs involved 1556 patients satisfied the inclusion.Meta-analysis showed that compared to conventional rehabilitation, meridian massage had significantly better FMA[mean difference(MD)=1.16,95%confidence interval(CI):0.43,1.89,P=0.002],FMA-L(MD=3.21,95%CI:1.97,4.45,P=0.00001),m BI(MD=8.87,95%CI:4.24,13.49,P=0.00002),clinical efficiency[relative risk(RR)=1.37,95%CI:1.24,1.51,P<0.00001],m AS(MD=-0.69,95%CI:-1.26,-0.11,P=0.02)and BBS(MD=5.07,95%CI:2.22,7.93,P=0.0005).Subgroup analysis indicated no significant difference between meridian massage and control group when the intervention duration was>8 weeks(MD=0.89,95%CI:-0.64,2.42,P=0.25)but improving motor function in the meridian massage when the intervention duration was≤8 weeks(<4 weeks intervention duration MD=0.84,95%CI:0.59,1.10,P<0.00001;4–8 weeks MD=2.41,95%CI:0.80,4.01,P=0.003).CONCLUSIONS:Meridian massage as an alternative and complementary therapy seems to have short-term benefits in improving post-stroke motor function(particularly in the lower limbs),quality of life, clinical efficiency, and balance stability and muscle spasms. The high heterogeneity of the included studies and the low methodological quality certainly limits the confidence placed in this systematic review and Meta-analysis,warranting future well-designed studies.
文摘Undernutrition produces morphological brain alterations and cognitive deficiencies in children of underdeveloped countries. These results show that MS during the pre-weaning period restores the plastic properties of the Ambc over the hypoplastic multipolar motoneuron after the alterations caused by perinatal undernutrition.
