Purpose:This study aims to examine the level of behavioral health integration in primary care clinics among patients with hypertension and type 2 diabetes in Chiang Mai,Thailand. It also aims to explore the provider perceived prevalence of behavioral problems and need for an expert behavioral health consultant on the primary care team and the risk and protective factors of the patients’psychological and cardio-metabolic health outcomes.Methodology:Using survey data between 2017 and 2018 regarding the level of integration as measured by a Practice Integration Profile, provider ratings of prevalence and need based on a Provider Survey, and patient lifestyle and behavioral problems with a Health Risk Assessment in six primary health care settings in Chiang Mai,Thailand(n=335).We conducted ordinary least squares regressions to explore the relationship between each of the patient psychological and cardio-metabolic health outcomes and physical, mental health and substance abuse variables while controlling for the demographics. Results:The results showed overall high levels of integration, but significant variability across clinics.Providers rated prevalence of health risk and need for a Behavioral Health Consultant as moderate. Results of the patient health risk assessment were variable, with some problems high severity, others low. Conclusion:The results reflect a need for integrated behavioral health into the health care system in Thailand especially for those who are in the chronic illness condition.Recommendations for future study include an evaluation of the translation quality and validity and reliability of the study measures under the Thai context. This study fills the research gap of lacking research on the level of integrated care in clinics in Thailand. 展开更多

Objective:Despite the known benefits of integrated primary care and behavioral health services, integrated behavioral health services have not been readily used in medical clinics in interior Alaska. With minimal resources,we recently developed an integrated primary care-behavioral health program in a medical clinic in interior Alaska to meet clinic and community needs. The objective of this study was to explore initial program outcomes and determine the feasibility of program development and implementation. Methods:We initially conducted a needs assessment for integrating behavioral health services into primary care.Program development was informed by specific clinic needs.Following program implementation, initial program outcomes were tracked with use of data from the electronic health record and patient and provider use and satisfaction surveys. The level of integration of primary care and behavioral health services was evaluated with the Practice Integration Profile. Results:A total of 188 patients were seen by behavioral health consultants during the initial pilot phase, including 44.0%referred for mental health symptoms,33.1%referred for physical health issues, and 22.3%referred for both mental and physical health issues. The initial program outcomes indicate modest clinical improvement(measured by the nine-item Patient Health Questionnaire)as well as patient and provider satisfaction with the model, and a moderate level of practice integration. Conclusion:On the basis of the initial findings,it appears that our integrated primary care-behavioral health program has the potential to serve an important role in addressing the behavioral health needs of the local population. Our implementation procedure and initial program outcomes suggest that such models are feasible in rural and small-scale settings with minimal overhead costs. 展开更多

Background: We examined family health history (FHH) as a public health intervention tool in undergraduate students. We hypothesized that the FHH assignment would positively relate to students’ FHH knowledge and health and healthcare-seeking behavioral change. Methods: Health professional students’ (n = 103) pre/post-test surveys and research papers were collected in 2011-2012, from a mid-western and southern university in the United States of America, using mixed methods research. Results: The majority of students were aged 18 - 30, women, White, had healthcare access and health insurance, and awareness of the term FHH. Significant logistic regression relationships existed between: 1) helping students understand important strengths and weaknesses in their health and quality of life and outcomes of talking with family and doctors about FHH;and 2) improving students’ understanding of what they needed to do to maintain their health and the outcome statement “FHH tells you about inherited genes.” Key themes from the research papers included actions and FHH and proposed behavioral changes. Conclusions: Quantitative findings supported the relationship between students’ assignment evaluation and knowledge change, while qualitative findings supported relationships between assignment evaluation and knowledge and behavioral change. This study highlights regional differences in students’ FHH and the need to address family support barriers to behavioral change. 展开更多

Introduction:Having a primary care usual source of care(USC)is associated with better population health outcomes. However, the percent of adults in the United States(US)with a usual primary care provider is declining. We sought to identify factors associated with establishing a USC at an urgent care clinic or emergency department as opposed to primary care. Methods:We analyzed data from 57,152 participants in the All of Us study who reported having a USC.We used the Andersen Behavioral Model of Health Services Use framework and multivariable logistic regression to examine associations among predisposing, enabling, and need factors, according to the source of usual care. Results:An urgent care clinic, minute clinic,or emergency department was the source of usual care for 6.3%of our sample. The odds of seeking care at this type of facility increased with younger age, lower educational attainment, and better health status.Black and Hispanic individuals,as well as those who reported experiencing discrimination in medical settings or that their provider was of a different race and ethnicity, were also less likely to have a primary care USC.Financial concerns, being anxious about seeing a provider, and the inability to take time off from work also increased the likelihood of having a non‐primary care USC.Conclusions:Improving the rates of having a primary care USC among younger and healthy adults may be achievable through policies that can improve access to convenient, affordable primary care.Efforts to improve diversity among primary care providers and reduce discrimination experienced by patients may also improve the USC rates for racial and ethnic minority groups. 展开更多

Background:Adverse childhood experiences(ACEs)are a significant issue in adolescent health due to their robust correlation with deficits in executive functions(EF)and health risk behaviors(HRBs).This study aimed to examine the association between ACEs and a range of HRBs,including substance use, sexual risk behavior, suicidal ideation, physical inactivity, and violence. Methods:This cross-sectional study used self-administered questionnaire and cluster sampling in seven junior high schools in Samarinda,Indonesia, with a sample size of 534 students.Data analysis using descriptive statistics, the Chi-square test, the independent t-test,ANOVA,binary logistic regression, and mediation analysis with macro-PROCESS.Results:The most common ACEs were community violence(68.0%),physical neglect(52.8%),psychological/emotional abuse(52.6%),physical abuse(50.4%),and peer bullying(45.9%).Adolescents with more than five ACEs showed significantly higher involvement in smoking/vaping(67.9%),suicidal ideation(75.2%),sexual risk behavior(57.7%),bullying(64.3%),and physical fighting(59.7%)(p<0.001).ACEs were significantly correlated with EF deficits(r=0.471,p<0.01)and HRB(r=0.578,p<0.01).Regression analysis confirmed that ACEs predicted EF deficits(β=0.466,p<0.001)and HRB(β=0.469,p<0.001),with EF deficits partially mediating this relationship(β=0.107,95%CI[0.045,0.094]).In addition, two subdomains of EF deficits,self-motivation(β=0.042)and self-regulation of emotion(β=0.032),significantlymediated the relationship between ACEs and HRBs.Conclusion:These findings suggest an important role for EF deficits in linking childhood adversity to engagement in risky behaviors.Addressing ACEs and EF deficits(self-motivation and self-regulation of emotion)through early intervention may be important in reducing long-term health risks among Indonesian adolescents. 展开更多

Objective This study examined the status of health-related behaviors among rural residents and the factors influencing the practice of such behaviors. Methods One thousand and ninety subjects aged 15 years or over in a rural community, Anhui Province, China were surveyed. A questionnaire was used to collect information on the health knowledge, attitude and behavior of the subjects. Information on health behavior included smoking, drinking, dietary habits, regular exercises, sleeping pattern and oral health behavior. Results The prevalence of smoking and drinking in the male subjects was 46.5% and 46.9%, respectively. There was a positive significant association between smoking and drinking. Only 8.3% of all subjects ate three regular meals a day regularly. Among subjects who ate two meals a day, 89.7% did not have breakfast. Only 1.7% of subjects took part in regular exercise. About 85% of subjects slept 6 to 8 h per day. Only 38.4 % of the respondents had the habit of hand washing before eating and after using the lavatory. 79.3% of the subjects brushed their teeth every day, and among them, only 10.6 percent brushed their teeth twice a day Further analyses showed that 64.8% of subjects had 3-5 items of positive health behaviors out of 8 items and only 16.9% had six or more items. Logistical regression analyses suggested that better health behavior was affected by sex, age, years of education, income and health knowledge. Conclusion The status of health behaviors among rural residents was generally poor. It is thus urgent to reinforce health education in rural communities in China. 展开更多

These behaviors are greatly affected by social environment and individual factors. However, few studies explored the social determinants of schoolchildren's health behaviors. In this cross-sectional study, the social determinants of health behaviors were examined among both migrant and resident students in Beijing, China. Data of 967 children from six urban and suburban primary schools were analyzed using multiple linear regression analysis. The results showed that less than 60% of children performed some healthy behaviors, although their health knowledge was generally good. Children's health behavior scores were greatly determined by school characteristics, health knowledge, demographic characteristics, and family context. Therefore, improving health education and conditions in primary schools is the priority to promote children's healthy behaviors, especially for disadvantaged children. 展开更多

Purpose: We aim to create a model of consumer health information seeking behavior via social media, then to have a better understand of it. After that further efforts should be made to provide targeted recommendations for media managers to promote health communication via social media. Methods: Our custom model was derived from literature review, empirical research was tested by the use of questionnaire investigation, and then the collected data were analyzed by structural equation model tool SmartPLS. Finally, the custom model was modified according to the experimental results of SmartPLS. Results: A total of 239 (66.39%) of the respondents were female and 121 (33.61%) were male. The maximum of two stages of age were 18 - 29 (70.56%), 30 - 39 (13.89%). Wechat (60.28%), QQ Zone (55.22%), Micro-blog (48.89%) were the most commonly used social media to obtain health information. About 44.72% of respondents used social media to obtain health information multiple times a day. The whole numerical values of AVE, cronbach’s alpha, CR and square roots of AVE were above the benchmarks of themselves and showed great reliability and validity. All the 11 hypotheses had obvious statistical significance, the P-value of eight path coefficients exhibited <0.001, one path coefficient exhibited <0.01 and two path coefficients exhibited <0.05. Conclusions: A suitable model of consumer health information seeking behavior via social media was created and some inner relationships were found. Namely, gratification of health information and its platform had a positive effect on attitudes toward health information seeking behavior. Health information literacy and health status were proved to have a significant influence on attitudes toward health information seeking behavior, subject norms and perceived behavioral control respectively. In addition, attitudes toward the health information seeking behavior, subject norms and perceived behavioral control were proved to positively associate with health information seeking behavior intention. 展开更多

Objective:This study is to address the health beliefs and health behavior of Chinese immigrants residing in the Greater Vancouver area of British Columbia(BC)Province in Canada. This article discussed Chinese immigrants’traditional Chinese medicine(TCM)use, health beliefs, and health behaviors. Methods:Information used in this study is based on data collected in the Chinese-speaking community in the Greater Vancouver area of BC in 2020–2022.Quantitative and qualitative methods were applied to this study. The first stage recruited 314 participants for the quantitative study to cross-validate an instrument tool, followed by the 2nd stage of 20 stratified random sampling out of the 314 participants for TCM-related in-depth qualitative interviews. This study focuses on the second stage of TCM qualitative interviews. Results:Results indicated that TCM health beliefs have cultural and spiritual meanings tied to the Chinese-speaking participants. There are barriers for the Chinese-speaking population to access the existing healthcare services due to their TCM health beliefs and other health needs, for example, family doctors, integrated medicine for better health outcomes, and mental health services especially during the coronavirus disease 2019(COVID-19)pandemic.Conclusion:Integrating TCM health beliefs and behaviors of Chinese-speaking immigrants into existing Canadian mainstream health services are strongly recommended. 展开更多

BACKGROUND The American Heart Association defines cardiovascular health in terms of four behaviors(smoking, diet, physical activity, and body weight)and three factors(plasma glucose, cholesterol, and blood pressure).By this definition, the preva-lence of ideal cardiovascular health behaviors and factors(ICHBF)is negatively correlated with all-cause mortality and risks of cardiovascular and cerebrovas-cular diseases and malignancy.AIM To investigate the changing trends of cardiovascular and cerebrovascular health scores in the Kailuan study population from 2006 to 2011.METHODS The Kailuan population data from three health checkups held in 2006-2007,2008-2009,and 2010-2011 were analyzed, and the constituent ratios of cardiovascular and cerebrovascular health behaviors and factors at ideal, intermediate, and poor levels were calculated by using Huffman and Capewell method.Simultaneously, the cardiovascular and cerebrovascular health behavior and factor scores were calculated.RESULTS From 2006 to 2007,the proportion of people with ideal physical exercise, low salt diet, ideal body mass index, ideal total cholesterol level,no smoking, ideal blood sugar, and ideal blood pressure was 13.12%,9.34%,49.17%,64.20%,49.27%,69.99%,and 20.55%,respectively,in men with a health score of 8.46,and 12.00%,9.13%,61.60%,64.28%,98.19%,78.90%and 36.92%in women, with a score of 10.02.From 2008 to 2009,the proportion was 16.09%,14.04%,51.94%,65.02%,40.18%,66.44%,and 17.04%in men, with a score of 8.18,and 16.860%,17.360%,64.010%,67.433%,98.220%,76.370%,and 42.340%in women, with a score of 10.12.From 2010 to 2011,the proportion was 12.22%,17.65%,49.40%,68.33%,48.17%,64.67%,and 14.68%in males, having a score of 8.21,while in females, the proportion was 11.83%,18.09%,49.40%,67.85%,98.82%,74.52%,and 37.78%,with a score of 9.90.Core Tip:The American Heart Association defines ideal cardiovascular health as the concurrent presence of ideal health parameters(blood glucose, total cholesterol, and blood pressure levels)and ideal health behaviors(weight status, diet, physical activity, and smoking). Our study design was retrospective and based on the Kailuan study. This prospective study was initiated in July 2006 to evaluate the risk factors and interventions for cardiovascular diseases and cerebrovascular diseases in the Kailuan community population, with several articles having been published on such chronic non-communicable diseases.A fixed population was constituted from these 57659 participants, and their cerebrovascular and cardiovascular health parameters and behaviors were surveyed from 2006 to 2011.Furthermore,the distributions of cardiovascular health parameters and behaviors were portrayed, and their health scores were estimated.cholesterol level,no smoking, ideal blood sugar, and ideal blood pressure was 16.09%,14.04%,51.94%,65.02%,40.18%,66.44%,and 17.04%,and it was 16.86%,17.36%,64.01%,67.433%,98.22%,76.37%,and 42.34%for women. From 2010-2011,the proportion of men with ideal physical exercise, low salt diet, ideal body mass index, and ideal total cholesterol level,no smoking, ideal blood sugar, and ideal blood pressure was 12.22%,17.65%,49.40%,68.33%,48.17%,64.67%,and 14.68%,and it was 11.83%,18.09%,49.40%,67.85%,98.82%,74.52%,and 37.78%in women. The prevalence of ideal cardiovascular and cerebrovascular health behaviors and factors of different genders from 2006 to 2011 is shown in Table 1 and Table 2.The prevalence of poor and intermediate cardiovascular and cerebrovascular health behaviors and factors of different genders from 2006 to 2011 was described in our previous research[11].The health scores of men and women in 2006,2008,and 2010 were 8.46 and 10.02,8.18 and 10.12,as well as 8.21 and 9.90,respectively;the distribution of cardiovascular and cerebrovascular health behaviors and factor scores of different genders from 2006 to 2011 are shown in Table 3 and Table 4.The mean±SD of body mass index, blood pressure, fasting blood glucose, and total cholesterol levels of different genders from 2006 to 2011 are shown in Table 5 and Table 6. 展开更多
