BACKGROUND Bariatric surgery is an effective treatment for severe obesity but is associated with an increased risk for development of eating disorders.Indeed, numerous maladaptive eating behaviors and eating disorders have been described following bariatric surgery. However, the differentiation of pathologic eating patterns from expected dietary changes following bariatric surgery can sometimes be difficult to discern.CASE SUMMARY A female in her early 40s presented for medical stabilization of severe protein calorie malnutrition after losing 52.3 kg over the last six months after Roux-en-Y gastric bypass, with subsequent development of cyclic nausea and vomiting.Fear of these aversive physical symptoms led to further restriction of nutritional intake and weight loss. The patient was diagnosed with avoidant/restrictive food intake disorder, which has not been previously reported after bariatric surgery.CONCLUSION Improvement in the diagnostic nomenclature for feeding and eating disorders is warranted for patients who have undergone bariatric surgery. 展开更多

Background We reported a case of cervical invasive vagus nerve stimulation(iVNS)treatment for avoidant/restrictive food intake disorder(ARFID)in a patient with severe anxiety and depression. This patient was even given a critical illness notice during his hospitalization and all treatment efforts were failed. Objective We aimed to verfiy the effectiveness of iVNS in a patient with ARFID.Methods We first attempted to perform cervical iVNS in this case and then observed the changes in clinical scores. We also analyzed the alterations in brain magnetic resonance imaging characteristics before and after iVNS using multi-modal neuroimagings. Results After 18 days of iVNS(from 1 to 19 July 2023),the patient's clinical symptoms improved significantly and he rapidly gained 5 kg in weight. The brain functional characteristics of this patient tended toward those of the normal group.Functional connectivities of the medial of orbitalis prefrontal cortex returned to the normal range after iVNS.Conclusion This is a precedent for performing cervical iVNS in an ARFID patient.Brain neural activity can be modulated through iVNS.The observed improvements in clinical scores and positive changes in brain function validated the effectiveness of iVNS.This case study provides evidence that this intervention technique could be used to reduce the burden on more similar ARFID patients. 展开更多

This manuscript critically evaluates the randomized controlled trial(RCT)conducted by Phiri et al,which assessed the effectiveness of virtual reality(VR)training for psychiatric staff in reducing restrictive practices(RPs).Specifically, this RCT investigated the impact of VR on the handling of aggressive patients by psychiatric staff compared to traditional training methods.Despite significant reductions in perceived discrimination in the VR group, there were no major improvements in self-efficacy or anxiety levels. The system usability scale rated the VR platform highly, but it did not consistently outperform traditional training methods.Indeed, the study shows the potential for VR to reduce RPs,although fluctuations in RP rates suggest that external factors, such as staff turnover, influenced the outcomes. This manuscript evaluates the study’s methodology, results, and broader implications for mental health training.Additionally,it highlights the need for more comprehensive research to establish VR as a standard tool for psychiatric staff education, focusing on patient care outcomes and real-world applicability. Finally, this study explores future research di-rections,technological improvements, and the potential impact of policies that could enhance the integration of VR in clinical training. 展开更多

BACKGROUND Restrictive practices(RPs)are defined by measures linked to physical and chemical restraints to reduce the movement or control behaviours during any emergency.Seclusion is an equal part of RPs intended to isolate and reduce the sensory stimulation to safeguard the patient and those within the vicinity.Using interventions by way of virtual reality(VR)could assist with reducing the need for RPs as it could help reduce anxiety or agitation by way of placing users into realistic and immersive environments. This could also aid staff to and change current RPs.AIM To assess the feasibility and effectiveness of using a VR platform to provide reduction in RP training.METHODS A randomised controlled feasibility study, accompanied by evaluations at 1 month and 6 months, was conducted within inpatient psychiatric wards at Southern Health National Health Service Foundation Trust,United Kingdom.Virti VR scenarios were used on VR headsets to provide training on reducing RPs in 3 inpatient psychiatric wards.Outcome measures included general self-efficacy scale, generalised anxiety disorder assessment 7(GAD-7),Burnout Assessment Tool 12,the Everyday Discrimination Scale, and the Compassionate Engagement and Action Scale.RESULTS Findings revealed statistically significant differences between the VR and treatment as usual groups,in the Everyday Discrimination Scale items Q8 and Q9:P=0.023 and P=0.040 respectively, indicating higher levels of perceived discrimination in the VR group. There were no significant differences between groups in terms of general self-efficacy,generalised anxiety disorder assessment 9,and Burnout Assessment Tool 12 scores.A significant difference was observed within the VR group for compassionate engagement from others(P=0.005)over time. Most respondents recorded System Usability Scale scores above 70,with an average score of 71.79.There was a significant reduction in rates of RPs in the VR group vs treatment as usual group with a fluctuating variability observed in the VR group likely due to external factors not captured in the study.CONCLUSION Ongoing advancement of VR technology enables the possibility of creating scenarios and simulations tailored to healthcare environments that empower staff by providing more comprehensive and effective training for handling situations. 展开更多

This editorial examines the application of virtual reality(VR)training to mitigate restrictive practices(RPs)within psychiatric facilities.RPs include physical restraints, seclusion, and chemical restraints, used to ensure patient safety but with varying usage rates across regions. In recent years, there has been a growing focus on the adverse effects of RPs on both healthcare workers and patients, leading to calls for its reduction.Previous research has shown the efficiency of VR training in RP reduction. This editorial will analyze the limitations of VR training in prior research aimed at reducing RP,emphasizing that the essence of RPs is a medical safety issue, calling for careful differentiation of the causes of RPs,and avoiding the use of AR technology as a"new bottle"for"old wine"to improve the quality and reproducibility of future research in this field. 展开更多

The problem of collision avoidance for non-cooperative targets has received significant attention from researchers in recent years.Non-cooperative targets exhibit uncertain states and unpredictable behaviors, making collision avoidance significantly more challenging than that for space debris.Much existing research focuses on the continuous thrust model, whereas the impulsive maneuver model is more appropriate for long-duration and long-distance avoidance missions.Additionally,it is important to minimize the impact on the original mission while avoiding noncooperative targets. On the other hand, the existing avoidance algorithms are computationally complex and time-consuming especially with the limited computing capability of the on-board computer, posing challenges for practical engineering applications. To conquer these difficulties, this paper makes the following key contributions:(A)a turn-based(sequential decision-making)limited-area impulsive collision avoidance model considering the time delay of precision orbit determination is established for the first time;(B)a novel Selection Probability Learning Adaptive Search-depth Search Tree(SPL-ASST)algorithm is proposed for non-cooperative target avoidance, which improves the decision-making efficiency by introducing an adaptive-search-depth mechanism and a neural network into the traditional Monte Carlo Tree Search(MCTS).Numerical simulations confirm the effectiveness and efficiency of the proposed method. 展开更多

Aiming to address the Unmanned Aerial Vehicle(UAV) formation collision avoidance problem in Three-Dimensional(3-D) low-altitude environments where dense various obstacles exist, a fluid-based path planning framework named the Formation Interfered Fluid Dynamical System(FIFDS) with Moderate Evasive Maneuver Strategy(MEMS) is proposed in this study.First, the UAV formation collision avoidance problem including quantifiable performance indexes is formulated. Second, inspired by the phenomenon of fluids continuously flowing while bypassing objects, the FIFDS for multiple UAVs is presented, which contains a Parallel Streamline Tracking(PST) method for formation keeping and the traditional IFDS for collision avoidance. Third, to rationally balance flight safety and collision avoidance cost, MEMS is proposed to generate moderate evasive maneuvers that match up with collision risks. Comprehensively containing the time and distance safety information, the 3-D dynamic collision regions are modeled for collision prediction. Then, the moderate evasive maneuver principle is refined, which provides criterions of the maneuver amplitude and direction. On this basis, an analytical parameter mapping mechanism is designed to online optimize IFDS parameters. Finally, the performance of the proposed method is validated by comparative simulation results and real flight experiments using fixed-wing UAVs. 展开更多

This study aims to systematically review the various factors influencing corporate tax avoidance.Tax avoidance refers to legal strategies used to minimize tax liabilities and has become a critical issue in accounting and corporate governance. The study examines key determinants of tax avoidance, including firm characteristics(such as size, leverage, and multinational scale),managerial attributes, executive compensation, ownership structure, corporate social responsibility(CSR)performance,as well as the impact of regulations and legal reforms. The review findings highlight that the motivations behind tax avoidance are multifaceted, driven by the interaction of economic incentives, organizational ethics, external pressures, and public policies. Moreover, strict regulatory environments and strong CSR practices can mitigate tax avoidance behaviors, although their effectiveness is often contingent upon a firm’s cultural and political context. This study offers a comprehensive mapping of the current literature and recommends future research that integrates additional variables and broader time spans to enhance the understanding of tax avoidance behavior across different national contexts. 展开更多

Multiple quadrotors target encirclement is widely used in the intelligent field,as it can effectively monitor and control target behavior. However,it faces the danger of collision,as well as difficulties in localization and tracking. Therefore,we propose a complete target encirclement method.Firstly, based on Hooke's law,a collision avoidance controller is designed to maintain a safe flying distance among quadrotors. Then, based on the consensus theory,a formation tracking controller is designed to meet the requirements of formation transformation and encirclement tasks, and a stability proof based on Lyapunov was provided.Besides, the target detection is designed based on YOLOv5s,and the target location model is constructed based on the principle of pinhole projection and triangle similarity. Finally,we conducted experiments on the built platform, with 3 reconnaissance quadrotors detecting and localization 3 target vehicles and 7 hunter quadrotors tracking them. The results show that the minimum average error for localization targets with reconnaissance quadrotors can reach 0.1354 m,while the minimum average error for tracking with hunter quadrotors is only 0.2960 m.No quadrotors collision occurred in the whole formation transformation and tracking experiment. In addition, compared with the advanced methods, the proposed method has better performance. 展开更多

Purpose We aimed to determine:(a)the chronic effects of interval training(IT)combined with blood flow restriction(BFR)on physiological adaptations(aerobic/anaerobic capacity and muscle responses)and performance enhancement(endurance and sprints),and(b)the influence of participant characteristics and intervention protocols on these effects. Methods Searches were conducted in PubMed,Web of Science(Core Collection),Cochrane Library(Embase,ClinicalTrials.gov,and International Clinical Trials Registry Platform),and Chinese National Knowledge Infrastructure on April 2,with updates on October 17,2024.Pooled effects for each outcome were summarized using Hedge's g(g)through meta-analysis-based random effects models, and subgroup and regression analyses were used to explore moderators. Results A total of 24 studies with 621 participants were included.IT combined with BFR(IT+BFR)significantly improved maximal oxygen uptake(VO2_(max))(g=0.63,I^(2)=63%),mean power during the Wingate 30-s test(g=0.70,I^(2)=47%),muscle strength(g=0.88,I^(2)=64%),muscle endurance(g=0.43,I^(2)=0%),time to fatigue(g=1.26,I^(2)=86%),and maximal aerobic speed(g=0.74,I^(2)=0%)compared to IT alone.Subgroup analysis indicated that participant characteristics including training status,IT intensity, and IT modes significantly moderated VO2_(max)(subgroup differences:p<0.05).Specifically,IT+BFR showed significantly superior improvements in VO2_(max)compared to IT alone in trained individuals(g=0.76)at supra-maximal intensity(g=1.29)and moderate intensity(g=1.08)as well as in walking(g=1.64)and running(g=0.63)modes.Meta-regression analysis showed cuff width(β=0.14)was significantly associated with VO2_(max)change,identifying 8.23 cm as the minimum threshold required for significant improvement.Subgroup analyses regarding muscle strength did not reveal any significant moderators. Conclusion IT+BFR enhances physiological adaptations and optimizes aspects of endurance performance, with moderators including training status,IT protocol(intensity, mode, and type),and cuff width. This intervention addresses various IT-related challenges and provides tailored protocols and benefits for diverse populations. 展开更多

The nutritional status of an individual can significantly influence its life history traits, including development, growth, reproduction, and survival. In the predatory mite Phytoseiulus persimilis Athias-Henriot(Acari:Phytoseidae),the plasticity of life history traits, such as developmental time and size at maturity,is influenced by the quality and quantity of food. This study aimed to investigate the effects of dietary restriction at different life stages(i.e.,juvenile,early adulthood, and later adulthood)on the lifespan and fecundity of P persimilis. We found that reduced dietary intake during early adulthood resulted in a shorter lifespan for both male and female P persimilis.Furthermore, this study demonstrated a sex-specific response to dietary restriction:it extended the lifespan of males but reduced that of females during later adulthood.Diet restriction during the postovipositional period of females showed the most variable life history trait response. Our results showed that the impact of diet restriction at different life stages can have combined influences on the postovipositional duration of P persimilis, where the individuals receiving diet restriction during immature development and early adulthood had a greater reduction in ovipositional duration as those experiencing diet restriction during late adulthood. In addition,we observed a positive correlation between the lifespan and fecundity of females, with higher prey availability increasing both. The insights obtained from our research contribute to a better understanding of the aging process and dietary requirements of P persimilis, which can facilitate the development of more effective biological control strategies using this predator for spider mites in agriculture. 展开更多

In the realm of missile defense systems, the self-sufficient maneuver capacity of missile swarms is pivotal for their survival.Through the analysis of the missile dynamics model,a time-efficient cooperative attack strategy for missile swarm is proposed.Based on the distribution of the attackers and defenders, the collision avoidance against the defenders is considered during the attack process.By analyzing the geometric relationship between the relative velocity vector and relative position vector of the attackers and defenders, the collision avoidance constrains of attacking swarm are redefined. The key point is on adjusting the relative velocity vectors to fall outside the collision cone. This work facilitates high-precision attack toward the target while keeping safe missing distance between other attackers during collision avoidance process.By leveraging an innovative repulsion artificial function,a time-efficient cooperative attack strategy for missile swarm is obtained.Through rigorous simulation, the effectiveness of this cooperative attack strategy is substantiated.Furthermore,by employing Monte Carlo simulation, the success rate of the cooperative attack strategy is assessesed and the optimal configuration for the missile swarm is deduced. 展开更多

AIM:To evaluate the prevalence and the causes of blindness, severe visual impairment(SVI),and visual impairment(VI)and to investigate the frequency of cataract surgery in people aged≥50y in Yueqing,Zhejiang Province,China.METHODS:A population-based,cross-sectional study was performed using the Rapid Assessment of Avoidable Blindness technique.Eight-seven clusters, each consisting of 50 people aged≥50y,were selected by probabilityproportionate-to-size sampling.Three outreach teams conducted door-to-door visits.Visual acuity(VA)was measured using a tumbling E chart.Lens status and causes of VI were assessed by ophthalmologists for individuals with a VA of<6/12 in either eye.A standardized questionnaire was used to collect information about cataract surgeries.RESULTS:Of 4350 eligible individuals,4120 were examined with a 94.7%response rate.Age-and genderadjusted prevalence of blindness,SVI,and VI were 0.5%(95%CI,0.3%–0.7%),0.7%(95%CI,0.4%–1.0%),and 4.8%(95%CI,4.2%–5.5%),respectively.Age was associated with an increased prevalence of VI,and the most common cause of VI was untreated cataracts, with the main barriers to cataract surgery being a lack of knowledge or awareness about cataracts.Of the 415 eyes operated on for cataracts,68(16.4%)eyes had a poor outcomes(VA<6/60)and 303(73.0%)had a good outcomes(VA>6/18).CONCLUSION:Prevalence rates of blindness,SVI,and VI in Yueqing are lower than other reported Chinese population-based studies.Cataracts remain the most common cause of blindness and VI. 展开更多

Objectives:To investigate the impact of a supervised bloodflow restriction(BFR)training program performed at different times of the morning on body composition and muscle performance in older, inactive adults. Methods:A single-arm intervention of supervised BFR resistance training was performed three times per week for six weeks.Participants(n=24;aged 65+years)were categorized into early morning(n=13;05:00–08:59)or late morning(n=11;09:00–12:00)groups.Primary outcomes were changes in body composition, total work, average peak power, average peak torque, muscle strength, and physical function. Results:Mixed model analysis of variance revealed a significant within-subject effect of time for all strength measures(p ranging from 0.017 to<0.001)and some physical function measures including the 30 s chair stand test,30 s bicep curl test, and grip strength(p ranging from 0.015 to<0.001).No between-group or time by group interaction effect was observed for all outcomes. Conclusions:This study showed that only six weeks of BFR training, performed at different time of the morning, did not enhance muscle mass and performance, but provided similar changes in muscle strength and some physical function tests in older adults. 展开更多
