The role of ethical leadership in promoting internal whistleblowing among nurses: A dual-mediation model analysis of psychological safety and reporting attitudes

Objectives:This study aimed to investigate the dual mediating roles of psychological safety and reporting attitudes in the association between ethics-centered leadership and internal whistleblowing among nurses.Methods:A multicenter,cross-sectional study design was employed,involving a convenience sample of 273 nurses among five university hospitals from August to October 2024.Data were collected using a demographic questionnaire,Ethical Leadership Scale,Psychological Safety Scale,Positive Attitudes Toward Whistleblowing Scale,and Internal Whistleblowing Scale.Pearson correlation and structural equation model were conducted to test the hypothesized dual mediation model.Results:The average score of nurses for ethical leadership was 4.14±0.57,psychological safety was 3.80±0.64,reporting attitudes was 4.12±0.67,internal whistleblowing was 3.93±0.52.Ethical leadership had a significant effect on psychological safety(β=0.38,P<0.001),reporting attitudes(β=0.44,P<0.001),and internal whistleblowing(β=0.21,P=0.009).Furthermore,psychological safety(β=0.26,P<0.001)and reporting attitudes(β=0.27,P<0.001)significantly influenced internal whistleblowing among the nurses.The indirect effect of ethics-centered leadership on internal whistleblowing intermediated by psychological safety was significant(β=0.10,P<0.001).Similarly,reporting attitudes demonstrated a significant mediating effect(β=0.12,P=0.003).Conclusions:Ethical leadership encourages internal whistleblowing among nurses by fostering psychological safety and positive reporting attitudes.Therefore,healthcare managers should prioritize ethical leadership development,enhance psychological safety,and foster constructive reporting attitudes that enable nurses to report ethical violations confidently,without apprehension. 展开更多

BACKGROUND Breast cancer is one of the most prevalent causes of morbidity and mortality worldwide,presenting an increasing public health challenge,particularly in lowincome and middle-income countries.However,data on the knowledge,attitudes,and preventive practices regarding breast cancer and the associated factors among females in Wollo,Ethiopia,remain limited.AIM To assess the impact of family history(FH)of breast disease on knowledge,attitudes,and breast cancer preventive practices among reproductive-age females.METHODS A community-based cross-sectional study was conducted in May and June 2022 in Northeast Ethiopia and involved 143 reproductive-age females with FH of breast diseases and 209 without such a history.We selected participants using the systematic random sampling technique.We analyzed the data using Statistical Package for Social Science version 25 software,and logistic regression analysis was employed to determine odds ratios for variable associations,with statistical significance set at P<0.05.RESULTS Among participants with FH of breast diseases,the levels of knowledge,attitudes,and preventive practices were found to be 83.9%[95%confidence interval(CI):77.9-89.9],49.0%(95%CI:40.8-57.1),and 74.1%(95%CI:66.9-81.3),respectively.In contrast,among those without FH of breast diseases,these levels were significantly decreased to 10.5%(95%CI:6.4-14.7),32.1%(95%CI:25.7-38.4),and 16.7%(95%CI:11.7-21.8),respectively.This study also indicated that knowledge,attitudes,and preventive practices related to breast cancer are significantly higher among participants with FH of breast diseases compared to those without HF breast diseases.CONCLUSION Educational status,monthly income,and community health insurance were identified as significant factors associated with the levels of knowledge,attitudes,and preventive practices regarding breast cancer among reproductive-age females. 展开更多

Objective:This study aimed to explore undergraduates’knowledge,attitude,and practice/behavior of human papillomavirus(HPV)vaccination,as well as the essential influencing factors for vaccination decision-making.Methods:Through cluster and convenience sampling,2000 undergraduates from the Nursing and Language department of a university in Shanghai were sent a self-designed questionnaire.Chi-square tests,independent sample t-test/ANOVE,and multiple linear regression were used to investigate participants’knowledge and attitude on HPV vaccination,as well as the factors that predicted potential action to receive and promote HPV vaccination in the future.Results:The mean HPV knowledge score was 5.027 out of 10.Health science students showed a significantly higher knowledge mean score than the non-health science students(P<0.000).There was a statistically difference in HPV vaccination attitude among undergraduates in different grades(P<0.05).Awareness of cervical cancer and worries about the risk of cervical cancer were the significant predictors of willingness to receive and promote HPV vaccination in the future.Conclusions:It would take time for a new health product to be aware,understood,accepted,and received.Education providing and information sharing are expected to break the dawn and make the procedure processed. 展开更多

Background:We aimed to study physician attitudes toward ultrasound-guided procedures and possible improvements.We hypothesized that the usage of ultrasound in procedures may be limited by a high barrier of entry and that most physicians would choose to adopt software that provides real-time image guidance if accessible.Methods:A voluntary,cross-sectional survey of physicians at a single site was conducted using a five-point Likert scale.Data analysis included both descriptive and inferential statistical analyses and stratified by categorical descriptors,including variables of formal training,years of experience,and specialty of practice.Results:One hundred sixteen physicians responded to the survey.The majority disagreed that there was a steep learning curve(57.5%)and that they need more time to identify structures under ultrasound(85.0%).Overall attitudes were mixed about the use of additional software to improve ease of use,but most(55.4%)had positive opinions toward the addition of real-time 3D reconstruction.Respondents without formal training were significantly more likely to agree that additional software would improve ease of ultrasound-guided procedures(p=0.0389).Radiologists were significantly more likely to perceive a steeper learning curve and less likely to advocate for supplemental software compared to emergency medicine physicians,surgeons,or anesthesiologists.Conclusions:Surveyed physicians demonstrated comfort with ultrasound-guided procedures and a mixed stance toward the use of additional software to assist with procedures.Those without formal training had significantly more positive attitudes toward the use of additional technology to augment ultrasound-guided procedures,suggesting a knowledge gap that may benefit from such technology. 展开更多

Objective:Cardiopulmonary resuscitation(CPR)is one of the most important life-saving procedures in the hospital.Contrary to medical guidelines,family presence during CPR is still not accepted in some countries.Family presence during CPR depends on the nurses’attitude,which is influenced by various factors.Emotional intelligence(EI)helps nurses make wise decisions and display responsible behavior,which is necessary for proper and good performance in nursing.The study’s purpose was to determine the attitude of acute care nurses toward family presence during CPR and its relationship with EI.Methods:The descriptive-analytical study was conducted among the nurses of acute care units(intensive care unit[ICU],critical care unit,and emergency department)in teaching hospitals of Qazvin University of Medical Sciences,Iran,in 2022-2023.A total of 186 nurses were included in the study using convenience sampling.The data collection tools were a checklist of demographic characteristics,nurses’attitude toward the presence of family scale,and Siberia Schering’s EI questionnaire.The collected data were analyzed using descriptive and analytical statistics and SPSS software.Results:The mean age of the participants was(32.05±6.93)years.Of 186 nurses participating in the study,127(68.3%)were women and the rest were men.The mean score of attitude was 47.41±9.41(the minimum score of the nurses was 19 and the maximum score was 95).The mean score of EI was 99.27±8.86(the minimum score was 69 and the maximum score was 128).The results of Pearson’s correlation coefficient showed no significant relationship between the total score of attitude and EI of the participants(P=0.588).Data analysis showed that only the self-arousal dimension of EI has a significant relationship with the range of family self-control behaviors in attitude(P=0.037).Conclusions:The results showed that the nurses of acute care units have a positive attitude toward the presence of the family during CPR and have high EI.Although no significant relationship was observed between the 2 variables,paying attention to the influencing factors on the attitude of the nurse toward family presence during CPR in different societies needs more research and investigation. 展开更多

BACKGROUNDLimited research exists on attitudes and barriers to organ donation in the UnitedArab Emirates, highlighting the need for a deeper understanding of public perceptionsand challenges.AIMTo assess the attitudes and barriers toward organ donation.METHODSA cross-sectional study was adopted and included 607 samples consisting ofstudents, faculty, and staff who were selected from three universities in Ajmanand who had signed consent forms. A validated self-administered questionnairethat included 13 attitudes and 14 barrier items was used as a tool. The reliabilityof the tool was 0.89 (Cronbach's alpha). In the analysis of attitude scores, responseswere rated on a scale from 0 to 4, with 0 representing 'strongly disagree'and 4 representing 'strongly agree' for supportive attitudes towards organdonation. Participants with a total attitude score of 39 or higher indicated agreementor strong agreement with all items, reflecting a generally supportiveattitude toward organ donation. Lower scores suggested that the respondent wasneutral or disagreed with one or more items, indicating a less supportive attitudetoward organ donation. Knowledge about organ donation was assessed by selfadministeredquestionnaire that included 13 items. Analysis was done using SPSSversion 29. χ2 was used to assess associations between variables.RESULTSMost participants were young (≤ 30 years old, 83.7%), female (79.2%), from World Health Organization Eastern Mediterranean Region countries (69.5%), Muslim (82.4%), students (80.6%), single(83.9%), and from a nursing college (33.1%). The majority had no personal or family history of organ donation(93.2% and 93.9%, respectively). Supportive attitudes toward organ donation were significantly associated withreligion (P = 0.003), working status (P = 0.009), university (P = 0.019), and knowledge (P < 0.001). Additionally,those with a personal or family history of organ donation were significantly more supportive (56.8% vs 33.3%, P =0.004). Lack of awareness was the most reported barrier for organ donation (64.1%) followed by being afraid oforgan donation due to medical procedures required (51.9%).CONCLUSIONThe findings suggest that supportive attitudes toward organ donation are influenced by demographic factors,personal experiences, and knowledge levels. Lack of awareness and fear of medical procedures were the mostreported barriers to organ donation. These results highlight the need for targeted educational programs to increaseawareness and promote positive attitudes toward organ donation. 展开更多

The purpose of this study was to examine the knowledge,attitude,motivation and behavior of the community before and after the experiment,and also to determine the effect of the experiment on increasing knowledge,attitude,motivation,and behavior related to the construction of family toilets in coastal areas.The study was conducted in Pangkep and Maros Regencies.Atotal of 50 heads of families were selected as participants using the purposive sampling method.25 participants became the experimental group and 25 people became the control group.The research variables included knowledge,attitudes,motivation,and behavior of the community in building family toilets before and after the experiment.Data collection through tests,questionnaires,and observations to each participant.The research instruments were knowledge tests,questionnaires,and observations.Data analysis used descriptive and inferential statistical analysis,with the t-test.The results of the study showed that based on the experiment,knowledge had a significant effect with a correlation coefficient of 0.94,attitudes had an effect of 0.91,motivation was 0.756,and behavior was 0.865.It can be concluded that the construction of family toilets in the coastal areas of Pangkep and Maros Regencies,before the experiment,the knowledge,attitudes,motivation,and behavior of the community were in the low category,and after the experiment increased significantly to the high category. In addition, the results of the analysis showed that the experiment had a significant effect on increasing theknowledge, attitudes, motivation, and behavior of the community towards the construction of family toilets in coastal areas. 展开更多

Assessment of Knowledge, Attitude and Vaccination Status of Hepatitis B Infection among Medical University Students in Mogadishu-Somalia

Background: Hepatitis B virus (HBV) is a primary reason for liver cancer and continues to be a worldwide public health issue. The likelihood of contracting HBV is greater in healthcare workers (HCWs) compared to individuals who are not in healthcare professions. Medical students are classified as a high-risk demographic since, like HCWs, they often come into contact with bodily fluids and blood during their clinical training. By 2030, a greater proportion of people will have received HBV vaccinations, thereby halting the spread of new infections—The Somali Ministry of Health with the help of various agencies announced to eradicate hepatitis from Somalia. The priority actions are national hepatitis strategy, hepatitis survey, public awareness, training, and capacity building. Objectives: This study aims to assess the knowledge, attitude, and vaccination status of Hepatitis B infection among medical university students in Mogadishu, Somalia, 2024. Methods: Cross-sectional study design was used in this study and the survey was carried out among medical students enrolled in Universities from April 1, 2023 to June 30, 2023. The data was analyzed using SPSS version 26.0 software, Chi-square analysis and Logistic regression analysis to identify associations between demographic factors and HBV knowledge, attitudes, and vaccination status, as well as perspectives and immunization status concerning viral hepatitis. Results: The study achieved a response rate of (96%), with 230 participants. Most students (76.5%) were aged 26 - 30 years, and (60.8%) were male. Nearly half (48.7%) were in their third year of study, and the majority (36.1%) were from the Medicine and Surgery department. While 92.2% had heard of HBV, gaps in understanding were evident. About 37.8% erroneously believed HBV could spread via handshakes, and only 33.9% were aware HBV is treatable. Awareness of HBV’s severe complications, such as liver cirrhosis and liver cancer, was reported by 61.3%, and 83% understood that vaccination could prevent infection. Positive attitudes towards HBV vaccination were prevalent. Most participants (81.3%) supported vaccination before sexual activity, and 78.3% endorsed mandatory HBV vaccination policies for healthcare workers. However, 87.4% expressed concerns about the vaccine promoting unsafe sexual behavior, and 96.1% cited cultural resistance as a barrier to vaccination. A significant proportion (80.86%) of students had not been vaccinated against HBV. Among vaccinated students, 17.4%, 15.7%, and 47.82% had received one, two, and three doses, respectively. Barriers to vaccination included safety concerns (77.4%), lack of time (86.52%), and doubts about efficacy (42.61%). Conclusion: This study highlights gaps in knowledge and vaccination coverage among medical students, which are critical for their health and future clinical practice. Enhancing awareness and vaccination rates can empower students to advocate for preventative measures in their professional environments. Despite high awareness of HBV, knowledge gaps and cultural barriers persist, affecting attitudes and vaccination uptake among medical students. Educational interventions addressing misconceptions, cultural resistance, and vaccine safety are critical. Increased advocacy for mandatory vaccination policies in healthcare settings is also essential to improve HBV prevention methods. 展开更多

Côte d’Ivoire is one of the most vulnerable countries to schistosomiasis, and its prevalence varies by region. This cross-sectional survey was conducted in schools in the Upper Sassandra region to determine the knowledge, attitude, and practices of school children regarding schistosomiasis. In total, 354 students participated in the survey. Our results showed that, compared to the girls, the boys had a higher level of knowledge about schistosomiasis OR = 1.605 and p = 0.05. Regarding the signs, symptoms, and modes of transmission and prevention of schistosomiasis, the participants reported a low level of knowledge p > 0.05 and OR 1. In conclusion, it is important to target health messages in schools to reach the most vulnerable students effectively. This initiative aims to provide children with basic knowledge and skills in the transmission of schistosomiasis. 展开更多

Instantaneous Global Navigation Satellite System(GNSS)attitude determination method which achieves real-time attitude determination using GNSS signal has been extensively studied,particularly the one using a priori attitude information replacing the code measurements to enhance the successful rate for ambiguity resolution.However,there exists a key limitation that this method relies on considerable Monte Carlo sampling particles to construct the pdf of ambiguities,resulting in significant computational burdens.To address this limitation,this paper provides a rapid single-epoch GNSS attitude determination method based on a priori attitude information.It utilizes a second-order Taylor expansion to analytically estimate the covariance of the baseline conditioned on a priori attitude information.This is followed by deriving the float solution and covariance of ambiguities,which are then processed using the standard LAMBDA method for integer ambiguity resolution.Experimental results demonstrate that our method is both efficient and accurate,significantly reducing computational costs compared to previous methods,thereby enhancing its applicability for GNSS-based attitude determination. 展开更多

The accuracy of the full-scale aircraft static tests is greatly influenced by the aircraft attitude.This paper proposes an aircraft attitude optimization method based on the characteristics of the test.The aim is to address three typical problems of ttitude control in the full-scale aircraft static tests:(1)The coupling of rigid-body displacement and elastic deformation after large deformation,(2)the difficulty of characterizing the aircraft attitude by measurable structure,and(3)the insufficient adaptability of the center of gravity reference to complex loading conditions.The methodology involves the establishment of two observation coordinate systems,a ground coordinate system and an airframe coordinate system,and two deformation states,before and after airframe deformation.A subsequent analysis of the parameter changes of these two states under different coordinate systems is then undertaken,with the objective being to identify the key parameters affecting the attitude control accuracy of large deformation aircraft.Three optimization objective functions are established according to the test loading characteristics and the purpose of the test:(1)To minimize the full-scale aircraft loading angle error,(2)to minimize the full-scale aircraft loading additional load,and(3)to minimize the full-scale aircraft loading wing root additional bending moment.The optimization calculation results are obtained by using the particle swarm optimization algorithm,and the typical full-scale aircraft static test load condition of large passenger aircraft is taken as an example.The analysis of the results demonstrates that by customizing the measurable structure of the aircraft as the observation point for the aircraft attitude,and by obtaining the translational and rotational control parameters of the observation point during the test based on the optimization objective function,the results are reasonable,and the project can be implemented and used to control the aircraft's attitude more accurately in complex force test conditions. 展开更多

Shield attitudes,essentially governed by intricate mechanisms,impact the segment assembly quality and tunnel axis deviation.In data-driven prediction,however,existing methods using the original driving parameters fail to present convincing performance due to insufficient consideration of complicated interactions among the parameters.Therefore,a multi-dimensional feature synthesizing and screening method is proposed to explore the optimal features that can better reflect the physical mechanism in predicting shield tunneling attitudes.Features embedded with physical knowledge were synthesized from seven dimensions,which were validated by the clustering quality of Shapley Additive Explanations(SHAP)values.Subsequently,a novel index,Expected Impact Index(EII),has been proposed for screening the optimal features reliably.Finally,a Bayesian-optimized deep learning model was established to validate the proposed method in a case study.Results show that the proposed method effectively identifies the optimal parameters for shield attitude prediction,with an average Mean Squared Error(MSE)deduction of 27.3%.The proposed method realized effective assimilation of shield driving data with physical mechanism,providing a valuable reference for shield deviation control. 展开更多

Ship motion attitude is influenced by dynamic marine conditions,presenting significant challenges in developing effective prediction networks.Contemporary prediction networks demonstrate limitations in hidden feature extraction,long-term dependency maintenance,and frequency characteristic incorporation.This paper presents an enhanced model integrating the informer network with a Time Convolutional Network(TCN)and a Frequency-Enhanced Channel Attention Mechanism(FECAM).The model employs a TCN for multi-feature extraction and applies Dimension-Segment-Wise(DSW)embedding for comprehensive multi-dimensional sequence analysis.Furthermore,it incorporates discrete cosine transform within the FECAM module for thorough data frequency analysis.The model integrates these components with the informer model for multivariate prediction.This approach maintains the informer model's capabilities in long-term multivariate prediction while enhancing feature extraction and local frequency information capture from ship motion attitude data,thus improving long-term multivariate prediction accuracy.Experimental results indicate that the proposed model outperforms traditional ship motion attitude prediction methods in forecasting future motion,reducing attitude prediction errors,and improving prediction accuracy. 展开更多

BACKGROUND Ischemic bowel disease(IBD)is a critical condition caused by reduced blood flow to the intestines,leading to tissue damage and potentially severe complications.Early recognition and timely management are essential for improving patient outcomes and reducing morbidity and mortality associated with IBD.AIM To evaluate the knowledge,attitude and practice(KAP)of healthcare professionals regarding IBD.METHODS This cross-sectional study was conducted among healthcare professionals in China from November 2023 to December 2023 using a self-designed questionnaire.RESULTS A total of 315 valid questionnaires were analyzed,with 215 participants(68.25%)being female.The mean KAP scores were 17.55±5.35(range:0-24),27.65±2.77(range:8-40),and 18.88±4.23(range:6-30),respectively.Multivariate linear regression analysis revealed the following factors to be independently associated with knowledge:Age 26-35 years(β=2.80,95%CI:0.31-5.30,P=0.028),professional title(β=2.66,95%CI:0.91-4.41,P=0.003),position(β=-3.78,95%CI:-5.45 to-2.11,P<0.001),participation in IBD-related training(β=3.45,95%CI:2.39-4.51,P<0.001),and admission of more than five IBD cases in the past month(β=3.25,95%CI:1.58-4.92,P<0.001).Attitude was independently associated with knowledge(β=0.20,95%CI:0.15-0.26,P<0.001)and being a nurse or nursing supervisor(β=-1.30,95%CI:-2.16 to-0.40,P=0.003).Practice was independently associated with knowledge(β=0.20,95%CI:0.10-0.30,P<0.001)and attitude(β=0.24,95%CI:0.06-0.42,P=0.007).Structural equation modeling demonstrated direct effects of knowledge on attitude(β=0.24,P<0.001)and practice(β=0.26,P<0.001),as well as of attitude on practice(β=0.22,P=0.012).CONCLUSION Healthcare professionals demonstrated adequate knowledge but moderate attitude and inactive practice regarding IBD.Addressing the gaps in attitude and practice through targeted training programs and interventions is essential for improving patient care and outcomes. 展开更多

Good mental well-being is necessary for achievement in science at any level.There are extrinsic factors that have the potential to determine a learner’s mental well-being,and the role of the parent(s)cannot be undermined.Therefore,this study examined the relationships between parental engagement(PE),parents’attitude to science(PAS),and mental well-being(MWb)among junior science students.This descriptive survey involved a population of Junior Secondary Three(3)students studying Basic Science in public co-educational schools.A sample of three hundred(300)respondents was randomly drawn from 10 schools within the metropolis.Quantitative data were sourced using three validated questionnaires and analyzed using descriptive and inferential statistics.The research showed that PE(2.19)and PAS(2.13)are relatively high,while MWb(2.51)is moderately normal.The influence of PE and PAS on MWb is significant.Both PE and PAS have a significant positive relationship with MWb.Also,PE and PAS were found to jointly contribute to students’MWb.In conclusion,PE and PAS have implications for the child’s mental well-being and ultimate success in science.We,therefore,recommend that more parental engagement be encouraged by improving sensitization,and orientation programs could be organized for parents of science students.This may provide an avenue for parents to be better equipped to support their young scientists. 展开更多
