The three main reasons for the low cancer curative rates in China include differences in the spectrum of cancer types, in early detection rates, and in the percentage of cancer patients receiving standardized treatment between China and developed countries.The most important mechanism for improving the curative rate is to improve early detection rates of major cancers in China using novel and affordable technologies that can be operated at home by the patients themselves.This attempt could be helpful in setting up a practical example for other developing countries with limited medical resources and a limited number of healthcare practitioners.展开更多
Purpose–Pediatric disorders,such as cerebral palsy and stroke,can result in thumb-in-palm deformity greatly limiting hand function.This not only limits children’s ability to perform activities of daily living but al...Purpose–Pediatric disorders,such as cerebral palsy and stroke,can result in thumb-in-palm deformity greatly limiting hand function.This not only limits children’s ability to perform activities of daily living but also limits important motor skill development.Specifically,the isolated orthosis for thumb actuation(IOTA)is 2 degrees of freedom(DOF)thumb exoskeleton that can actuate the carpometacarpal(CMC)and metacarpophalangeal(MCP)joints through ranges of motion required for activities of daily living.The paper aims to discuss these issues.Design/methodology/approach–IOTA consists of a lightweight hand-mounted mechanism that can be secured and aligned to individual wearers.The mechanism is actuated via flexible cables that connect to a portable control box.Embedded encoders and bend sensors monitor the 2 DOF of the thumb and flexion/extension of the wrist.A linear force characterization was performed to test the mechanical efficiency of the cable-drive transmission and the output torque at the exoskeletal CMC and MCP joints was measured.Findings–Using this platform,a number of control modes can be implemented that will enable the device to be controlled by a patient to assist with opposition grasp and fine motor control.Linear force and torque studies showed a maximum efficiency of 44 percent,resulting in a torque of 2.3971.06 in.-lbf and 0.6970.31 in.-lbf at the CMC and MCP joints,respectively.Practical implications–The authors envision this at-home device augmenting the current in-clinic and at-home therapy,enabling telerehabilitation protocols.Originality/value–This paper presents the design and characterization of a novel device specifically designed for pediatric grasp telerehabilitation to facilitate improved functionality and somatosensory learning。展开更多
Background: As the prognosis of lung cancer (LC) patients improves, subcutaneously implanted central venous access device ports (CV-ports) have frequently been used for continuing chemotherapy (CC) or palliative care ...Background: As the prognosis of lung cancer (LC) patients improves, subcutaneously implanted central venous access device ports (CV-ports) have frequently been used for continuing chemotherapy (CC) or palliative care (PC). In this study, we examined the clinical course of LC patients with subcutaneously implanted CV-ports from the time of receiving chemotherapy to the endpoint of cancer. Materials and Methods: We retrospectively reviewed the clinical data and treatment history of LC patients with subcutaneously implanted CV-ports between June 2008 and November 2013 using clinical records and a pharmacy database. Results: Of the 132 LC patients with subcutaneously implanted CV-ports, 79 (59.8%) had CV-ports for CC (the CC group) and 53 (40.2%) had CV-ports for PC (the PC group). After CV-port implantation, LC patients in the CC group received a median of two regimens with a median of 6 cycles. The median survival time of patients in the CC and PC groups was 457 and 44 days, respectively. In the CC group, the median survival time of small cell and non-small cell LC patients was 342 (95% confidence interval, 235 - 627) and 563 (95% confidence interval, 368 - 728) days, respectively. Nine patients (6.8%) had their CV-ports removed due to complications. Forty (30.3%) of the 132 enrolled patients were referred for at-home PC. The at-home death rate observed among these 40 patients was 30.0% (N = 12). Conclusion: CV-ports may contribute to seamless oncological care.展开更多
The Chinese government has issued a series of policies to promote the development of community medical and nursing integrated services.Since the development of these services involves the transition of medical and hea...The Chinese government has issued a series of policies to promote the development of community medical and nursing integrated services.Since the development of these services involves the transition of medical and health services from public spaces to private spaces,the home has become a critical factor in service provision.It is therefore essential to consider the concept of home and the sense of at-homeness in the research and practice of community medical and nursing integrated services in China to avoid prioritizing"others"over"self-being."Through an analysis of three cases from Britain,the United States,and the Netherlands,this article demonstrates that a sense of control and social connection are key components of the elderly’s sense of at-homeness.Community medical and nursing integrated services should not only enable the elderly to maintain their sense of self-control but also assist in building a supportive social network.展开更多
