Evaluation of Partial Pressure of Arterial Oxygen in Obese Patients in Supine Position during General Anesthesia

Background: Anesthetists should measure the concentration of supplemental oxygen to determine whether patients’ partial pressure of arterial oxygen (PaO2) is correct during general anesthesia. However, the standard PaO2 value in obese patients in the supine position is unknown. We evaluated the PaO2 with respect to the Broca-Katsura obesity index. Materials and Methods: From January 2001 to December 2013, we evaluated 472 patients aged ≥16 years old that underwent general anesthesia in the supine position. The anesthetic charts of 472 patients with an American Society of Anesthesiologists physical status of I or II were retrospectively reviewed to investigate the PaO2. Results: In patients aged 16 to 2 was 165.7 ± 25.6 mmHg at a Broca-Katsura index of 2 was 152.1 ± 23.8 mmHg at a Broca-Katsura index of 2 tended to decrease with age from 16 to 2 tended to decrease with age from 16 to <65 years at a Broca-Katsura index of 20% to <40%. 展开更多

A high temperature equilibration experiment was carried out to investigate the effect of oxygen partial pres- sure on the phase equilibria and liquidus in CaO-Al2O3- FeOx system with the intermediate oxygen partial pressures of 10.13 Pa and 1.01 × 10^-3 Pa. The equilibrated phases and their compositions of the quenched samples were analyzed by using SEM/EPMA (Scanning Electron Microscope/Electron Probe Micro Analysis) and XRD (X Ray Diffraction). The phase equilibrium results include two cases, the two phase coexistence and the three-phase coexistence in the high Al2O3 region with oxygen partial pressure of either 10.13 Pa or 1.01 × 10^-3 Pa. Effects of oxygen partial pressure and temperature on the liquidus along the primary phase fields of CaO · Al2O3 and CaO · 2Al2O3 were nota hle. With the decrease of oxygen partial pressure, the liquid area expands and the liquidus of CaO · Al2O3 and CaO · 2Al2O3 primary fields moves to the Al2O3-FeOx region. On the other hand, the liquid area of CaO Al2O3-FeOx sys tem extends extremely to the high Al2O3 region with the temperature increasing from 1 400 to 1 500℃, especially at lower oxygen partial pressure. 目的分析脓毒症肺损伤患者中医证型分布状况与肺损伤相关指标及序贯性器官衰竭评分(sequential organ failure assessment,SOFA)、急性生理和慢性健康状况Ⅱ(Acute Physiology and Chronic Health EvaluationⅡ,APACHEⅡ)评分的相关性... 目的分析脓毒症肺损伤患者中医证型分布状况与肺损伤相关指标及序贯性器官衰竭评分(sequential organ failure assessment,SOFA)、急性生理和慢性健康状况Ⅱ(Acute Physiology and Chronic Health EvaluationⅡ,APACHEⅡ)评分的相关性。方法选取2022年1月—2023年1月医院收治的脓毒症肺损伤患者80例,收集患者的一般资料、肺损伤相关指标[动脉血氧合指数(ratio of partial pressure of O_(2) in arterial blood to fraction of inspired oxygen,PaO_(2)/FiO_(2))、肺泡动脉氧分压差(alveolar-arterial oxygen gradient,P(A-a)O_(2))]及SOFA、APACHEⅡ评分情况,根据中医分型标准对脓毒症肺损伤患者进行分型,比较不同分型的脓毒症肺损伤患者以上各观察指标情况并分析相关性。结果脓毒症肺损伤患者以肺热壅痹证型占比最高、35.00%(28/80),其次为瘀血阻肺型27.50%(22/80),肺塞腑实型23.75%(19/80),水饮郁肺型13.75%(11/80)。不同证型的患者在性别、年龄方面差异均无统计学意义(P>0.05);不同证型的患者肺损伤指标PaO_(2)/FiO_(2)、P(A-a)O_(2)差异均有统计学意义(P<0.05);不同证型的患者SOFA、APACHEⅡ评分差异均有统计学意义(P<0.05)。肺热壅痹、瘀血阻肺、肺塞腑实、水饮郁肺证型与PaO_(2)/FiO_(2)呈显著负相关(P<0.05),与P(A-a)O_(2)、SOFA评分、APACHEⅡ评分呈显著正相关(P<0.05)。结论脓毒症肺损伤患者中医证型主要为肺热壅痹、瘀血阻肺、肺塞腑实、水饮郁肺,与肺损伤相关指标及SOFA、APACHEⅡ评分密切相关,掌握脓毒症肺损伤辨证分型特点并观察患者肺损伤相关指标及SOFA、APACHEⅡ评分变化,对疾病诊治及预后评估具有较高价值。 展开更多

Objective To explore the effects of sliding-cup with different parameters on energy metabolism in the body by observing the effect of sliding-cup along the governor vessel on transcutaneous partial pressures of oxygen and carbon dioxide in Mìngmén(命门 GV 4) in sub-healthy people. Methods A total of 10 sub-healthy adults at rest were observed and sliding-cup along the governor vessel were conducted with three different parameters, jar-pressure of –0.01~ –0.02 MPa with sliding-cup for 5 min, jar-pressure of –0.02~ –0.03 MPa with sliding-cup for 5 min and jar-pressure of –0.02~ –0.03 MPa with slidingcup for 3 min. Peri Flux System5000 PF5040 module was employed to monitor the changes of transcutaneous partial pressures of oxygen(tcp O2) and carbon dioxide(tcp CO2) in GV 4 at 0 min, 5 min, 10 min, 15 min, 20 min, 25 min and 30 min after sliding-cup with above different interventions in sub-healthy subjects. Results After sliding-cup, tcp O2 increased at first and then decreased over time; there was no difference in tcp O2 after sliding-cupping with different parameters(all P0.05). After sliding-cup, tcp CO2 decreased; there was no difference in tcp CO2 after sliding-cup with different parameters(all P0.05). Conclusion Sliding-cup has a sustained effect on the body and can speed up the energy metabolism in the body. 展开更多

Objective:To explore the easily applicable indicators of practical value to evaluate the prognosis of acute respiratory distress syndrome(ARDS).Methods:Blood and biochemical tests and bloodgas analyses were performed upon entry into the ICUs,12 h,24 h,48 h and 72 h after that in 72 ARDS patients(who were admitted to the ICUs of our hospital from January 2000 to December 2009).Then APACHEⅡscores were achieved by combining relevant physiological parameters and laboratory results.Results:There was a statistical difference between the death group and survival group at different time points upon entering the ICUs in terms of APACHEⅡscore, alveolar-arterial oxygen difference and arterial blood lactate clearance rate.PaO<sub>2</sub>/FiO<sub>2</sub> values were recorded to be statistically different between the death group and survival group 24 h,48 h and 72 h,respectively after entry into the ICUs.In addition,registered linear regression existed between APACHEⅡscore,alveolar-arterial oxygen difference or PaO<sub>2</sub>/FiO<sub>2</sub> value and time. APACHEⅡscore 24 h and 72 h after entering ICUs predicted mortality with an area under the ROC curve(AUC) standing respectively at 0.919 and 0.9SS.Arterial blood lactate clearance rate 12 h, 24 h,48 h and 72 h after entering ICUs predicted mortality with an area under the ROC curve (AUC) at 0.918,0.918,0.909 and 0.991,respectively.Conclusions:APACHEⅡscore applied in combination with arterial blood lactate clearance rate is of clinical significance in assessing the prognosis of ARDS patients. 展开更多

High-temperature experiments were carried out for the slag systems of“FeO”−SiO_(2)−CaO−Al_(2)O_(3)and“FeO”−SiO_(2)−CaO−MgO at 1200℃and p(O_(2))of 10^(−7)kPa.The equilibrated samples were quenched,and the phase compositions were measured by electron probe microanalysis(EPMA).A series of pseudo-ternary and pseudo-binary phase diagrams are constructed to demonstrate their applications in copper smelting process and evaluation of the thermodynamic database.Spinel and tridymite are identified to be the major primary phases in the composition range related to the copper smelting slags.It is found that the operating window of the smelting slag is primarily determined by w_(Fe)/w_(SiO_(2))ratio in the slag.Both MgO and Al_(2)O_(3)in the slag reduce the operating window which requires extra fluxing agent to keep the slag to be fully liquid.Complex spinel solid solutions cause inaccurate predictions of the current thermodynamic database. 展开更多

A SiC/ZrSiO4?SiO2 (SZS) coating was successfully fabricated on the carbon/carbon (C/C) composites by pack cementation, slurry painting and sintering to improve the anti-oxidation property and thermal shock resistance. The anti-oxidation properties under different oxygen partial pressures (OPP) and thermal shock resistance of the SZS coating were investigated. The results show that the SZS coated sample under low OPP, corresponding to the ambient air, during isothermal oxidation was 0.54% in mass gain after 111 h oxidation at 1500 ° C and less than 0.03% in mass loss after 50 h oxidation in high OPP, corresponding to the air flow rate of 36 L/h. Additionally, the residual compressive strengths (RCS) of the SZS coated samples after oxidation for 50 h in high OPP and 80 h in low OPP remain about 70% and 72.5% of those of original C/C samples, respectively. Moreover, the mass loss of SZS coated samples subjected to the thermal cycle from 1500 ° C in high OPP to boiling water for 30 times was merely 1.61%. 展开更多

小型猪肺脏同种异体移植及术后监测

目的探讨小型猪同种异体肺移植及术后监测在肺移植研究中的可行性与参考价值。方法2例小型猪(R1和R2)行左肺同种异体移植,术前进行补体依赖细胞毒性试验及血液交叉配型,术中记录主要操作时间和开放肺动脉后动脉血氧分压(PaO_(2)),术后... 目的探讨小型猪同种异体肺移植及术后监测在肺移植研究中的可行性与参考价值。方法2例小型猪(R1和R2)行左肺同种异体移植,术前进行补体依赖细胞毒性试验及血液交叉配型,术中记录主要操作时间和开放肺动脉后动脉血氧分压(PaO_(2)),术后检测血常规、血液生化指标及炎症因子,并行多器官病理学检查。结果补体依赖细胞毒性试验结果显示供受体淋巴细胞存活率42.5%~47.3%,交叉配血均无发生凝集反应。D1、D2热缺血时间分别为17 min、10 min,冷缺血时间分别为246 min、216 min,最终R1、R2分别存活1.5 h和104 h。R1术后白蛋白(ALB)、球蛋白(GLB)均下降,丙氨酸转氨酶升高;R2术后ALB、GLB、天冬氨酸转氨酶均升高;2例受体尿素氮、血清肌酐均升高。病理结果显示R1移植肺部分实变伴炎症细胞浸润,多器官充血及损伤;R2移植肺严重坏死伴纤维增生,多器官轻中度损伤。移植肺白细胞介素-1β、白细胞介素-6表达水平均有所升高。结论小型猪同种异体肺移植模型可系统评估免疫学相容与否、术中功能及术后器官损伤,所得数据可为后续肺移植研究提供技术参考。 展开更多

目的探讨血氧、营养状况、炎性因子联合对慢性阻塞性肺疾病(COPD)合并呼吸衰竭患者无创呼吸机治疗失败的风险预测价值。方法回顾性分析2022年4月—2024年4月河南省三门峡市中心医院收治的130例COPD合并呼吸衰竭患者的临床资料。根据治... 目的探讨血氧、营养状况、炎性因子联合对慢性阻塞性肺疾病(COPD)合并呼吸衰竭患者无创呼吸机治疗失败的风险预测价值。方法回顾性分析2022年4月—2024年4月河南省三门峡市中心医院收治的130例COPD合并呼吸衰竭患者的临床资料。根据治疗结局,将其分为无创呼吸机治疗失败组、无创呼吸机治疗成功组。统计两组占比,对比两组基线资料,并将基线资料差异项代入单因素分析,单因素分析差异项以Logistic回归方程统计,据此绘制列线图建立预测模型,并以校准曲线进行内部验证,分析血氧、营养状况、炎性因子指标联合对其治疗失败的预测效能。结果本次纳入的130例COPD合并呼吸衰竭患者中,无创呼吸机治疗失败者46例(35.38%,无创呼吸机治疗失败组),无创呼吸机治疗成功者84例(64.62%,无创呼吸机治疗成功组)。无创呼吸机治疗失败组动脉血氧分压(PaO_(2))水平低于无创呼吸机治疗成功组,白介素-6(IL-6)、红细胞分布宽度/白蛋白比值(RAR)、嗜酸性粒细胞/淋巴细胞比值(ELR)水平均高于无创呼吸机治疗成功组(P<0.05),将上述指标代入单因素分析、多因素Logistic回归模型中,PaO_(2)为COPD合并呼吸衰竭患者无创呼吸机治疗失败的保护因素(OR=0.787,P<0.05),IL-6、RAR、ELR均为治疗失败的危险因素(OR=9.677、1.031、9.293,P<0.05),据此绘制列线图模型,显示上述因素联合检测对治疗失败预测效能较高,且对列线图预测模型进行拟合效度检验,校准曲线略呈线性,拟合效度良好(χ^(2)=14.461,P=0.071)。结论COPD合并呼吸衰竭患者无创呼吸机治疗失败的因素包括PaO_(2)、IL-6、RAR、ELR,且上述指标联合检测对治疗失败的预测价值较高。 展开更多

Ti_(2)AlC,a MAX phase ceramic,has an attractive self-healing ability to restore performance via the oxidation-induced crack healing mechanism upon healing at high temperatures in air(high oxygen partial pressures).However,such healing ability to repair damages in vacuum or low oxygen partial pressure conditions remains unknown.Here,we report on the self-healing behavior of Ti_(2)AlC at a low oxygen partial pressure of about 1 Pa.The experimental results showed that the strength recovery depends on both healing temperature and time.After healing at 1400℃for 1–4 h,the healed samples exhibited the recovered strengths even exceeding the original strength of 375 MPa.The maximum recovered strength of~422 MPa was achieved in the healed Ti_(2)AlC sample after healing at 1400 for 4 h,about 13%higher than the original strength.Damages were healed by the formed℃TiCx from the decomposition of Ti_(2)AlC.The decomposition-induced crack healing as a new mechanism in the low oxygen partial pressure condition was disclosed for the MAX ceramics.The present study illustrates that key components made of Ti_(2)AlC can prolong their service life and keep their reliability during use at high temperatures in low oxygen partial pressures. 展开更多

无创高频振荡通气治疗新生儿呼吸窘迫综合征的效果及对PaO2、PaCO2的影响 被引量:12

目的:探究无创高频振荡通气治疗新生儿呼吸窘迫综合征(NRDS)的临床效果及对PaO2、PaCO2的影响。方法:选取笔者所在医院收治的48例NRDS患儿,随机分为常规组(n=24)和试验组(n=24)。常规组给予无创持续气道正压通气治疗,试验组给予无创高... 目的:探究无创高频振荡通气治疗新生儿呼吸窘迫综合征(NRDS)的临床效果及对PaO2、PaCO2的影响。方法:选取笔者所在医院收治的48例NRDS患儿,随机分为常规组(n=24)和试验组(n=24)。常规组给予无创持续气道正压通气治疗,试验组给予无创高频振荡通气治疗。对比两组PaO2、PaCO2、并发症发生率与撤机成功率;对比两组总机械通气时间、无创呼吸支持时间及住院时间。结果:治疗后6~72 h两组PaO2随时间延长均呈上升趋势,PaCO2呈下降趋势,且试验组PaO2高于常规组,PaCO2低于常规组(P<0.05);治疗后试验组与常规组并发症发生率分别为12.50%、20.83%,差异无统计学意义(P>0.05),但试验组撤机成功率为91.67%,明显高于常规组的62.50%(P<0.05);试验组总机械通气时间、无创呼吸支持时间及住院时间均短于常规组(P<0.05)。结论:无创高频振荡通气可有效缓解NRDS患儿临床症状,改善其PaO2、PaCO2值,提高撤机成功率,减轻患儿痛苦,促进患儿康复,提高患儿生存质量,值得临床推广应用。 展开更多

目的分析老年慢性阻塞性肺疾病(COPD)并呼吸衰竭患者血清胆碱酯酶(CHE)表达检测及其与动脉血动脉血氧分压(PaO2)的相关性。方法选取2021年6月~2023年6月收治的100例COPD老年患者为COPD组,以是否合并呼吸衰竭为依据,将COPD组分为合并组(... 目的分析老年慢性阻塞性肺疾病(COPD)并呼吸衰竭患者血清胆碱酯酶(CHE)表达检测及其与动脉血动脉血氧分压(PaO2)的相关性。方法选取2021年6月~2023年6月收治的100例COPD老年患者为COPD组,以是否合并呼吸衰竭为依据,将COPD组分为合并组(合并呼吸衰竭)与未合并组(未合并呼吸衰竭),分别为61例、39例。随访时间截止为患者出院时间,以随访结局是否为死亡为依据,将合并组分为存活组(44例)与死亡组(17例);以病情程度为依据,将合并组分为急性加重组(11例)与稳定组(50例),同期选取50例健康体检者为对照组。回顾性分析入组者临床资料,记录年龄、性别、病程、治疗情况、血气分析、病情程度、CHE水平及随访结局,对比对照组、合并组与未合并组、稳定组与急性加重组、死亡组与存活组PaO2、CHE水平,以Pearson法分析合并组患者CHE与PaO2的相关性。结果合并组与未合并组PaO2、CHE水平较对照组更低(P<0.05);合并组PaO2、CHE水平较未合并组更低(P<0.05);急性加重组PaO2、CHE水平较稳定组更低(P<0.05);死亡组PaO2、CHE水平较存活组更低(P<0.05);Pearson分析显示,合并组血清CHE水平与PaO2呈正相关(r=0.533,P<0.05)。结论CHE在老年COPD并呼吸衰竭患者血清中低表达,与PaO2呈正相关,检测CHE可准确反映患者病情程度,并可用于预后评估。 展开更多

Based on the recent estimations on the YO1.5-CeO2, ZrO2-CeO2 and ZrO2 -YO1.6 systems, isothermal sections at 1273 and 1 973 K of the ternary CeO2- ZrO2-YO1.5system are calculated. In the system of ZrO2-CeO2-Ce2O3, the complex relation between the nonstoichiometry (y) in CeO2-x, the composition of the ZrO2 -CeO2 solid solution and the oxygen partial pressure (Po2 ) for different ZrO2 containing solid solutions CexZr1-xO2-x. are evaluated from 1 473 to 1 773 K. For an analysis of the oxidation behavior of UO2 nuclear fuel pellet under a loss of water coolant accident in a spent nuclear fuel pool of an LWR, thermodynamic assessments of UO2 oxidation were carried out under various atmospheric conditions. In a steam atmosphere, it was assessed that UO2 would not be fully oxidized into U3O8 due to the relatively lower oxygen partial pressure, while UO2 will be fully oxidized into U3O8 in an air atmosphere. In an air and steam mixture atmosphere, the UO2 oxidation was dominantly affected by the air volumetric fraction, because of the relatively higher oxygen partial pressure of air. In addition, the effect of H2 volumetric fraction on the oxygen partial pressure under a mixture atmosphere was calculated, and it was revealed that UO2 pellet oxidation could be reduced above the critical value of H2 volumetric fraction. 展开更多

Discussion on Influence Study on Optimizing Emergency Nursing Process on the Rescue Effect of Emergency Chest Pain Patients

Objective: to analyze the effect of optimizing emergency nursing process on the rescue of patients with chest pain. Methods: a total of 62 patients with emergency chest pain admitted to our hospital from July 2020 to June 2021 were randomly selected. They were divided into the control group (even number of beds) with emergency nursing and the observation group (odd number of beds) with optimized emergency nursing process. There were 31 patients in each group, and the effects after the intervention were compared. Results: after the intervention, the time spent in the rescue process of the patients in the observation group was lower than that of the control group (P < 0.05), and the hospitalization time was lower than that of the control group (P < 0.05). Meanwhile, the incidence of adverse events was also lower than that of the control group (P < 0.05), and the PaCO2 level of the patients in the observation group was significantly lower than that of the control group (P < 0.05);The PaO2 level in the observation group was higher than that in the control group (P < 0.05);The quality of life score was higher than that of the control group (P < 0.05);The satisfaction was higher than that of the control group (P < 0.05);The rescue success was also higher than that of the control group (P < 0.05). Conclusion: for emergency chest pain patients, the application of optimized emergency nursing process has a significant effect in improving the rescue success rate, reducing the incidence of adverse events, reducing the length of hospital stay, and improving the quality of life. 展开更多

目的探讨动脉血二氧化碳分压及血乳酸与体外膜肺氧合(ECMO)患者消化道出血的关联性。方法回顾性收集2020年1月至2022年12月在广西壮族自治区人民医院急诊监护病房住院并行静脉-静脉体外膜肺氧合(VV-ECMO)或静脉-动脉体外膜肺氧合(VA-EC... 目的探讨动脉血二氧化碳分压及血乳酸与体外膜肺氧合(ECMO)患者消化道出血的关联性。方法回顾性收集2020年1月至2022年12月在广西壮族自治区人民医院急诊监护病房住院并行静脉-静脉体外膜肺氧合(VV-ECMO)或静脉-动脉体外膜肺氧合(VA-ECMO)治疗的94例患者的临床资料。根据患者ECMO治疗期间是否发生消化道出血分为出血组(21例)和非出血组(73例)。比较两组动脉血二氧化碳分压、血乳酸等临床资料。采用二元logistic回归分析ECMO患者消化道出血的危险因素。通过受试者工作特征(ROC)曲线分析相关指标对消化道出血的预测效能。结果ECMO治疗前,两组血管活性-正性肌力药物评分(VIS)、平均动脉压(MAP)、血乳酸、动脉血二氧化碳分压比较差异无统计学意义(P>0.05)。ECMO治疗后24 h,两组VIS、血乳酸、动脉血二氧化碳分压低于治疗前,MAP高于治疗前,且非出血组血乳酸、动脉血二氧化碳分压低于出血组,差异有统计学意义(P<0.05)。二元logistic回归分析结果显示,ECMO治疗后24 h动脉血二氧化碳分压和血乳酸是ECMO患者发生消化道出血的独立危险因素(P<0.05)。ROC曲线分析结果显示,ECMO治疗后24 h动脉血二氧化碳分压和血乳酸均能有效预测消化道出血的发生(P<0.05),两项指标联合可进一步提高预测效能[AUC(95%CI)=0.752(0.614~0.890),P<0.05],灵敏度、特异度分别为71.43%、79.45%。结论ECMO治疗后24 h动脉血二氧化碳分压和血乳酸可作为ECMO治疗期间患者发生消化道出血的早期预测指标,对于降低ECMO治疗期间患者消化道出血发生率,提高生存率具有重要临床意义。 展开更多

目的探讨呼吸衰竭有创机械通气患者动态监测呼吸指数(RI)、氧合指数(OI)、动脉血二氧化碳分压(PaCO_(2))对脱机后拔管的评估价值。方法选择150例行有创机械通气治疗的呼吸衰竭患者,根据拔管评估情况分为观察组和对照组,其中成功拔管的7... 目的探讨呼吸衰竭有创机械通气患者动态监测呼吸指数(RI)、氧合指数(OI)、动脉血二氧化碳分压(PaCO_(2))对脱机后拔管的评估价值。方法选择150例行有创机械通气治疗的呼吸衰竭患者,根据拔管评估情况分为观察组和对照组,其中成功拔管的75例患者为观察组,未成功拔管的75例患者为对照组。检测并比较两组患者脱机前、脱机后30 min~1 h、脱机后1~2 h的RI、OI、PaCO_(2),脱机后30 min~1 h、1~2 h的RI增加率、OI降低率及RI增加≥50%、RI增加≥100%、OI降低≥50%以及PaCO_(2)降低≥50%情况。结果两组患者脱机前的RI、OI、PaCO_(2)比较,差异均无统计学意义(P>0.05)。两组患者脱机后RI均升高;观察组脱机后30 min~1 h、1~2 h的RI(1.66±0.08)、(1.65±0.09)均明显低于对照组的(2.42±0.35)、(3.04±0.26),差异有统计学意义(P<0.05)。两组患者脱机后OI均降低;观察组脱机后30 min~1 h、1~2 h的OI(242.06±9.33)、(247.23±10.81)mm Hg(1 mm Hg=0.133 kPa)均明显高于对照组的(188.66±9.30)、(146.87±6.65)mm Hg,差异有统计学意义(P<0.05)。观察组脱机后PaCO_(2)降低,对照组脱机后PaCO_(2)先升高而后下降;观察组脱机后30 min~1 h、1~2 h的PaCO_(2)(37.29±0.88)、(37.17±1.03)mm Hg均低于对照组的(41.18±1.49)、(39.17±1.55)mm Hg,差异有统计学意义(P<0.05)。与观察组脱机后30 min~1 h、1~2 h相比,对照组RI增加率、OI降低率更高,差异有统计学意义(P<0.05),观察组指标的变化相对更低更平稳。两组患者脱机后30 min~1 h、1~2 h RI增加≥50%的比例比较,差异无统计学意义(P>0.05);观察组脱机后1~2 h的RI增加≥100%的比例29.33%明显低于对照组的54.67%,差异有统计学意义(P<0.05)。观察组脱机后30 min~1 h、1~2 h OI降低≥50%的比例4.00%、9.33%均明显低于对照组的21.33%、44.00%,差异有统计学意义(P<0.05)。两组患者脱机后30 min~1 h、1~2 h PaCO_(2)降低≥50%的比例比较,差异无统计学意义(P>0.05)。结论动态监测RI、OI、PaCO_(2)对评估呼吸衰竭有创机械通气患者脱机后能否成功拔管具有较高的临床价值,拔管成功者的RI、OI变化相对更加平稳,脱机后30 min~1 h的OI降低1/2及以上(≥50%)或(和)脱机后1~2 h的RI增加1倍及以上(≥100%)均不适宜拔管。 展开更多

目的探讨血清通道蛋白4(AQP4)、高迁移率族蛋白B1(HMGB1)、纤维蛋白原样蛋白2(FGL2)水平联合颅内压和脑组织氧分压(PbtO_(2))监测在创伤性脑损伤(TBI)患者预后中的价值。方法选取2022年5月—2024年5月上海交通大学附属第六人民医院南院... 目的探讨血清通道蛋白4(AQP4)、高迁移率族蛋白B1(HMGB1)、纤维蛋白原样蛋白2(FGL2)水平联合颅内压和脑组织氧分压(PbtO_(2))监测在创伤性脑损伤(TBI)患者预后中的价值。方法选取2022年5月—2024年5月上海交通大学附属第六人民医院南院/上海市奉贤区中心医院重症医学科诊治的TBI患者128例为研究对象,根据患者治疗后随访3个月预后情况,将其分为预后不良组(n=38)、预后良好组(n=90)。采用ELISA法检测血清AQP4、HMGB1、FGL2水平;Spearman法分析TBI不同预后患者颅内压、PbtO_(2)、血清AQP4、HMGB1、FGL2与格拉斯哥昏迷量表(GCS)评分的相关性;运用ROC曲线分析颅内压、PbtO_(2)联合血清AQP4、HMGB1、FGL2对TBI患者预后的预测价值。结果预后不良组患者颅内压高于预后良好组,GCS评分、PbtO_(2)值显著低于预后良好组(t/P=7.491/<0.001、9.882/<0.001、7.215/<0.001)。预后不良组血清AQP4、HMGB1、FGL2水平明显高于预后良好组(t/P=7.106/<0.001、7.642/<0.001、7.383/<0.001);患者PbtO_(2)与GCS评分呈显著正相关(r/P=0.523/<0.001),而颅内压、血清AQP4、HMGB1、FGL2与GCS评分呈显著负相关(r/P=-0.515/<0.001、-0.492/<0.001、-0.617/<0.001、-0.569/<0.001);血清AQP4、HMGB1、FGL2、颅内压、PbtO_(2)及五者联合预测TBI患者预后的曲线下面积(AUC)分别为0.882、0.876、0.817、0.825、0.756、0.969,五者联合优于各自单独预测TBI患者预后的价值(Z/P=2.803/0.005、2.769/0.006、3.543/<0.001、3.269/0.001、3.956/<0.001)。结论TBI患者颅内压、血清AQP4、HMGB1、FGL2水平显著升高,PbtO_(2)显著降低,与患者预后有着紧密联系,联合检测对TBI患者预后有更高的预测价值。 展开更多
