BACKGROUND Adult distal humeral fractures(DHF)comprise 2%-5%of all fractures and 30%of all elbow fractures.Treatment of DHF may be technically demanding due to fracture complexity and proximity of neurovascular struct...BACKGROUND Adult distal humeral fractures(DHF)comprise 2%-5%of all fractures and 30%of all elbow fractures.Treatment of DHF may be technically demanding due to fracture complexity and proximity of neurovascular structures.Open reduction and internal fixation(ORIF)are often the treatment of choice,but arthroplasty is considered in case of severe comminution or in elderly patients with poor bone quality.Ulnar nerve affection following surgical treatment of distal humerus fractures is a well-recognized complication.AIM To report the risk of ulnar nerve affection after surgery for acute DHFs.METHODS We retrospectively identified 239 consecutive adult patients with acute DHFs who underwent surgery with ORIF,elbow hemiarthroplasty(EHA)or total elbow arthroplasty(TEA)between January 2011 and December 2019.In all cases,the ulnar nerve was released in situ without anterior transposition.We used our institutional database to review patients’medical records for demographics,fracture morphology,type of surgery and ulnar nerve affection immediately;records were reviewed after surgery and at 2 wk and 12 wk of routine clinical outpatient follow-up.Twenty-nine percent patients were excluded due to pre-or postoperative conditions.Final follow-up examination was a telephone interview in which ulnar nerve affection was reported according to the McGowen Classification Score.A total of 210 patients were eligible for interview,but 13 patients declined participation and 17 patients failed to respond.Thus,180 patients were included.RESULTS Mean age at surgery was 64 years(range 18-88 years);121(67.3%)patients were women;59(32.7%)were men.According to the AO/OTA classification system,we recorded 47 patients with type A3,55 patients with type B and 78 patients with type C fractures.According to the McGowen Classification Score,mild ulnar nerve affection was reported in nine patients;severe affection,in two.A total of 69 patients were treated with ORIF of whom three had mild temporary ulnar nerve affection and one had severe ulnar nerve affection.In all,111 patients were treated with arthroplasty(67 EHA,44 TEA)of whom seven had mild ulnar nerve affection and one had severe persistent ulnar nerve affection.No further treatment was provided.CONCLUSION The risk of ulnar nerve affection after surgical treatment for acute DHF is low when the ulnar nerve is released in situ without nerve transposition,independently of the treatment provided.展开更多

Pain upon ejaculation, testicular touch sensitivity and dysuria are apparent. Regarding the large amount of patients undergoing laparoscopic hernia repair, the problem seems quite evident. In this prospective, clinical, randomized, double-blind study we intended to investigate the biocompatibility of three different meshes and their influence on urological affections after operative procedure. Methods: 180 male patients with primary inguinal hernia undergoing TAPP were randomized for using a heavyweight (108 g/m2), double-filament PP mesh (Prolene, 10 9 15 cm, group A, n = 60), a multifilament, heavyweight variant (116 g/m2) of PP mesh (Serapren, 10 9 15 cm, group B, n = 60), or a composite mesh (polyglactin and PP) (Vypro II, 10 9 15 cm, group C, n = 60). We compared in terms of complications (seromas, recurrence rate), urological affections and life quality (SF-36 Health Survey). The follow-up period was 60 months. Results: Convalescence in group A was slower than in groups B and C: mean-term values of the visual scales for pain development were significantly (p th week postoperatively. Shenshu (BL 23), Qihaishu (BL 24), Dachangshu (BL 25), Weizhong (BL 40), Jiaji (EX B 2), etc. are used. In treatment group, acupuncture, TDP irradiation and cupping are performed, and in control group, only acupuncture is given. After 30 sessions (3 therapeutic courses) of treatment, in treatment and control groups, the total effective rates are 91.43% and 71.43% respectively. The loudspeakers played a crucial role in rural community during the COVID-19 pandemic,and had returned to the public with the help of short video platforms.However,with the continued popularity of digital media,is th...The loudspeakers played a crucial role in rural community during the COVID-19 pandemic,and had returned to the public with the help of short video platforms.However,with the continued popularity of digital media,is the revival of the loudspeakers which belong to traditional media just a flash in the pan?What the historic process of return has the loudspeakers undergone?Based on these questions,the paper applies Bourdieu’s sense of practice theory,focuses on“habit”throughout,adopts the research methods of online ethnography and fieldwork,and conducts participant observation and targeted in-depth interviews in a village in North China for about nine months.The study found that:(1)There is the lag of habit in the state’s handling of social relations in villages.Based on rational habit,village cadres used the loudspeakers to manifest their physical presence and restructure their authority;the villagers,operating under perceptual habit,rationalized the presence of the loudspeakers through using the key words such as elders,nostalgia,and homesickness;(2)the loudspeakers was maintained under the habit of village cadres and ordinary villagers,and the outbreak of the epidemic made the loudspeakers as an habit truly“appearing”;(3)the young village cadres,on the other hand,acquired this habit through imitation,transposed the loudspeakers into the short video platform,and carried out different forms of experimentation and innovation,thus surpassing the previous forms of loudspeakers practice.The return of the loudspeakers is the result of the game of different habits.As a long-lasting affection medium,the loudspeakers,on the one hand,has become an important media scene and memory field indispensable to rural community,and on the other hand,the people living on this land have a deep attachment to the loudspeakers,which is indirectly transformed into the bearer of homesickness.This brings new inspiration for us to rethink the relationship between the traditional media and the new media,the iteration of cadres and the change of rural community.展开更多

Ocean energy has progressively gained considerable interest due to its sufficient potential to meet the world’s energy demand,and the blade is the core component in electricity generation from the ocean current.Howev...Ocean energy has progressively gained considerable interest due to its sufficient potential to meet the world’s energy demand,and the blade is the core component in electricity generation from the ocean current.However,the widened hydraulic excitation frequency may satisfy the blade resonance due to the time variation in the velocity and angle of attack of the ocean current,even resulting in blade fatigue and destructively interfering with grid stability.A key parameter that determines the resonance amplitude of the blade is the hydrodynamic damping ratio(HDR).However,HDR is difficult to obtain due to the complex fluid-structure interaction(FSI).Therefore,a literature review was conducted on the hydrodynamic damping characteristics of blade-like structures.The experimental and simulation methods used to identify and obtain the HDR quantitatively were described,placing emphasis on the experimental processes and simulation setups.Moreover,the accuracy and efficiency of different simulation methods were compared,and the modal work approach was recommended.The effects of key typical parameters,including flow velocity,angle of attack,gap,rotational speed,and cavitation,on the HDR were then summarized,and the suggestions on operating conditions were presented from the perspective of increasing the HDR.Subsequently,considering multiple flow parameters,several theoretical derivations and semi-empirical prediction formulas for HDR were introduced,and the accuracy and application were discussed.Based on the shortcomings of the existing research,the direction of future research was finally determined.The current work offers a clear understanding of the HDR of blade-like structures,which could improve the evaluation accuracy of flow-induced vibration in the design stage.展开更多

It has been validate...The Brief Interactive Optimism Scale (BIOS-G), recently published, was developed to measure optimism based on the interactive personality styles theory in the general population of Mexican adults. It has been validated in both the general population in Mexico and other Latin American countries. This study aims to further consolidate the validity of the BIOS-G in a specific population. The objectives are to assess the psychometric properties of its items, validate its unidimensional structure, describe its distribution, verify its internal consistency, and examine its direct relationship with affective balance and positive affect, as well as its inverse relationship with emotional exhaustion and negative affect among Mexican university teachers. The BIOS-G, the CESQT emotional exhaustion subscale, and Scale of Positive and Negative Experience (SPANE) were administered to a non-probability sample of 213 Mexican university teachers. All items demonstrated discriminative power and internal consistency. The scale showed internal consistency (Green-Yang ordinal ω = 0.74 and McDonald ordinal ω = 0.88). The one-factor model presented good to acceptable fit to the data, with an average variance extracted of 0.65. Its distribution was negatively skewed and mesokurtic. The BIOS-G correlated directly with SPANE’s affective balance and positive emotional experiences and inversely with negative emotional experiences and emotional exhaustion, with moderate strengths of association. Its mean score was higher in men than in women and correlated positively with age. It is concluded that the BIOS-G demonstrates internal consistency and validity evidence among Mexican university teachers, and its use in this specific population is recommended.展开更多

Considering the comprehensive morphology and genesis of Podzols of the Stolowe Mountains,and the still-possible impact of frost actions and other processes related to cold climate on these soils,the main aims of this ...Considering the comprehensive morphology and genesis of Podzols of the Stolowe Mountains,and the still-possible impact of frost actions and other processes related to cold climate on these soils,the main aims of this study were to determine whether(i)the heterogeneous Podzols in the Stolowe Mountains underwent a phase of development in a cold climate,resulting in frost action features visible on the micromorphological level,and whether(ii)contemporary cryopedogenic traces are masked by the translocation of organic matter due to the podzolisation process.Four soil profiles were investigated,revealing distinct layers corresponding to different periods of soil formation.Under field observation,no explicit frost-related characteristics were observed.Nevertheless,micromorphological analysis revealed cappings of fine materials on grains or peds,as well as development of granostriated(or any striated)b-fabric that resulted from the alternating effects of thawing and freezing processes.Moreover,micromorphological analysis revealed the presence of microstructures that could be the result of cryogenic processes,such as platy,angular blocky and lenticular features,as well as plane,vugh and star-shaped void types.The translocation of organic matter during podzolisation modified or concealed the frost-related features that developed during the late Pleistocene and early Holocene.This is evident,for instance,in the accumulation of organic matter on cappings and within soil voids,which further hinders the identification of frost-related characteristics and the interpretation of the soil's evolution.Macromorphological observations enhanced with micromorphological analyses revealed three distinct layers:(i)a young upper layer composed of loose,sandy material;(ii)a deeper layer containing a spodic horizon with frost actions,involving pedofeatures associated with the Pleistocene cold climate and(iii)a deeper subsoil basal layer.The abovementioned microstructures,combined with lithological discontinuity,support the hypothesised polygenetic origin of Podzols in the studied region.展开更多

Methods: 180 male patients with primary inguinal hernia undergoing TAPP were randomized for using a heavyweight (108 g/m2), double-filament PP mesh (Prolene, 10 9 15 cm, group A, n = 60), a multifilament, heavyweight variant (116 g/m2) of PP mesh (Serapren, 10 9 15 cm, group B, n = 60), or a composite mesh (polyglactin and PP) (Vypro II, 10 9 15 cm, group C, n = 60). We compared in terms of complications (seromas, recurrence rate), urological affections and life quality (SF-36 Health Survey). The follow-up period was 60 months. Results: Convalescence in group A was slower than in groups B and C: mean-term values of the visual scales for pain development were significantly (p th week postoperatively. There were no significant differences between groups B and C. Beyond the 12th post-interventional week the differences diminished. Conclusions: Independent which kind of mesh was implanted still 5% of patients suffered from urological affections 60 month later.
文摘In the present paper, 70 cases of retrograde affection of the lumbar vertebrae are randomly and evenly divided into treatment group and control group. Shenshu (BL 23), Qihaishu (BL 24), Dachangshu (BL 25), Weizhong (BL 40), Jiaji (EX B 2), etc. are used. In treatment group, acupuncture, TDP irradiation and cupping are performed, and in control group, only acupuncture is given. After 30 sessions (3 therapeutic courses) of treatment, in treatment and control groups, the total effective rates are 91.43% and 71.43% respectively. The therapeutic effect of comprehensive treatment is significantly superior to that of simple acupuncture therapy (P<0.05).
文摘The loudspeakers played a crucial role in rural community during the COVID-19 pandemic,and had returned to the public with the help of short video platforms.However,with the continued popularity of digital media,is the revival of the loudspeakers which belong to traditional media just a flash in the pan?What the historic process of return has the loudspeakers undergone?Based on these questions,the paper applies Bourdieu’s sense of practice theory,focuses on“habit”throughout,adopts the research methods of online ethnography and fieldwork,and conducts participant observation and targeted in-depth interviews in a village in North China for about nine months.The study found that:(1)There is the lag of habit in the state’s handling of social relations in villages.Based on rational habit,village cadres used the loudspeakers to manifest their physical presence and restructure their authority;the villagers,operating under perceptual habit,rationalized the presence of the loudspeakers through using the key words such as elders,nostalgia,and homesickness;(2)the loudspeakers was maintained under the habit of village cadres and ordinary villagers,and the outbreak of the epidemic made the loudspeakers as an habit truly“appearing”;(3)the young village cadres,on the other hand,acquired this habit through imitation,transposed the loudspeakers into the short video platform,and carried out different forms of experimentation and innovation,thus surpassing the previous forms of loudspeakers practice.The return of the loudspeakers is the result of the game of different habits.As a long-lasting affection medium,the loudspeakers,on the one hand,has become an important media scene and memory field indispensable to rural community,and on the other hand,the people living on this land have a deep attachment to the loudspeakers,which is indirectly transformed into the bearer of homesickness.This brings new inspiration for us to rethink the relationship between the traditional media and the new media,the iteration of cadres and the change of rural community.

文摘We investigated the psychological effects of nail treatments on children living in child welfare facilities.We performed a single nail treatment on children living in child welfare facilities.As a result,we found that feelings of“energy”,“relaxation”,and“comfort”tended to increase more strongly after the treatment than before.Furthermore,these feelings increased even more after the nail treatment.The effects of nail treatments were unrelated to gender or previous nail experience.Furthermore,nail treatments increased communication and interaction with facility staff and other children residing in the facility,promoting self-expression,and,as a result,increasing a sense of accomplishment,acceptance from others,and self-esteem.This suggests that positive emotions persisted even after nail treatments,leading to positive changes in daily life.
文摘目的探讨人文关怀联合亲情式护理对气管肺炎患儿焦虑、恐惧情绪的影响。方法选择2023年1月~2024年4月90例支气管肺炎患儿,数字表法随机分为两组。对照组采用常规护理,观察组采用人文关怀联合亲情式护理。评估护理前后儿童焦虑性情绪障碍筛查表(The screen for child anxiety related emotional disorders,SCARED)、治疗依从性、临床症状缓解时间。结果护理后两组患儿广泛焦虑以及躯体、社交恐惧评分均降低,但观察组护理后均显著低于对照组(P<0.05);观察组治疗依从性显著高于对照组(P<0.05);观察组气促、咳嗽、发热消失时间显著短于对照组(P<0.05)。结论人文关怀联合亲情式护理能够减轻支气管肺炎患儿焦虑、恐惧情绪,提高治疗依从性,促进临床症状缓解。

文摘Ocean energy has progressively gained considerable interest due to its sufficient potential to meet the world’s energy demand,and the blade is the core component in electricity generation from the ocean current.However,the widened hydraulic excitation frequency may satisfy the blade resonance due to the time variation in the velocity and angle of attack of the ocean current,even resulting in blade fatigue and destructively interfering with grid stability.A key parameter that determines the resonance amplitude of the blade is the hydrodynamic damping ratio(HDR).However,HDR is difficult to obtain due to the complex fluid-structure interaction(FSI).Therefore,a literature review was conducted on the hydrodynamic damping characteristics of blade-like structures.The experimental and simulation methods used to identify and obtain the HDR quantitatively were described,placing emphasis on the experimental processes and simulation setups.Moreover,the accuracy and efficiency of different simulation methods were compared,and the modal work approach was recommended.The effects of key typical parameters,including flow velocity,angle of attack,gap,rotational speed,and cavitation,on the HDR were then summarized,and the suggestions on operating conditions were presented from the perspective of increasing the HDR.Subsequently,considering multiple flow parameters,several theoretical derivations and semi-empirical prediction formulas for HDR were introduced,and the accuracy and application were discussed.Based on the shortcomings of the existing research,the direction of future research was finally determined.The current work offers a clear understanding of the HDR of blade-like structures,which could improve the evaluation accuracy of flow-induced vibration in the design stage.

文摘Phallogocentrism as cultural abuse of sex is a difficult issue that has been addressed by many modem Western feminist philosophers. By comparing their insights with those deriving from Chinese Confucianism and Daoism, I propose the concept of ＂affectionate respect＂ as an intellectual counterbalance to phallogocentrism. In this essay, I have discussed certain arbitrary fallacies based on masculine predominance and spotlighted the merits of being female in balancing emotion and reason, justice and fairness, and institutionally-biased powers and the human rights of innate dignity. To achieve gender justice and equality before God and under Heaven must be logically and morally extended to law and politics.
文摘The Brief Interactive Optimism Scale (BIOS-G), recently published, was developed to measure optimism based on the interactive personality styles theory in the general population of Mexican adults. It has been validated in both the general population in Mexico and other Latin American countries. This study aims to further consolidate the validity of the BIOS-G in a specific population. The objectives are to assess the psychometric properties of its items, validate its unidimensional structure, describe its distribution, verify its internal consistency, and examine its direct relationship with affective balance and positive affect, as well as its inverse relationship with emotional exhaustion and negative affect among Mexican university teachers. The BIOS-G, the CESQT emotional exhaustion subscale, and Scale of Positive and Negative Experience (SPANE) were administered to a non-probability sample of 213 Mexican university teachers. All items demonstrated discriminative power and internal consistency. The scale showed internal consistency (Green-Yang ordinal ω = 0.74 and McDonald ordinal ω = 0.88). The one-factor model presented good to acceptable fit to the data, with an average variance extracted of 0.65. Its distribution was negatively skewed and mesokurtic. The BIOS-G correlated directly with SPANE’s affective balance and positive emotional experiences and inversely with negative emotional experiences and emotional exhaustion, with moderate strengths of association. Its mean score was higher in men than in women and correlated positively with age. It is concluded that the BIOS-G demonstrates internal consistency and validity evidence among Mexican university teachers, and its use in this specific population is recommended.
文摘The dynamics of student engagement and emotional states significantly influence learning outcomes.Positive emotions resulting from successful task completion stand in contrast to negative affective states that arise from learning struggles or failures.Effective transitions to engagement occur upon problem resolution,while unresolved issues lead to frustration and subsequent boredom.This study proposes a Convolutional Neural Networks(CNN)based approach utilizing the Multi⁃source Academic Affective Engagement Dataset(MAAED)to categorize facial expressions into boredom,confusion,frustration,and yawning.This method provides an efficient and objective way to assess student engagement by extracting features from facial images.Recognizing and addressing negative affective states,such as confusion and boredom,is fundamental in creating supportive learning environments.Through automated frame extraction and model comparison,this study demonstrates reduced loss values with improving accuracy,showcasing the effectiveness of this method in objectively evaluating student engagement.Monitoring facial engagement with CNN using the MAAED dataset is essential for gaining insights into human behaviour and improving educational experiences.
文摘For the following decades after the American Civil War,most American literature works presented the war in a romantic or sentimental manner.However,Stephen Crane’s The Red Badge of Courage gave it a naturalistic touch to tell the story of Henry Fleming,the main character,overcoming cowardice and becoming a hero.Through its revolutionary criticism of psychoanalysis and capitalism,Deleuze and Guattari contributed a legacy to postmodern philosophy with their affect philosophy,with virtuality at its pivot.Using the Deleuzo-Guattarian affective philosophy as a scope,it is clear that the maturing of Henry is in fact a sequence of two attempts of deterritorialization in the war,one failed and the other successful.The heroism Henry acquires is demonstrated in the form of virtual death and disconnection with meaning;nonetheless,his return to the society makes it an incomplete destratification,which,at the same time,indicates the opportunity to be a true hero is forever lost to Henry,lost on the battlefield.

文摘Under the background of Artificial Intelligence(AI)and medical data informationization,intelligent Traditional Chinese Medicine(TCM)emerges as a new medical model.In this study,we introduce Affective Computing into the diagnosis and treatment process of TCM FiveTone Therapy.And proposes an intelligent diagnosis and treatment system including Five-tone diagnosis,monitoring and treatment,to achieve intelligent music matching.This system based on sound-image database for standardized collection and diagnosis of the Five-tone.The assessment and monitoring process are then visually presented,and an emotion analysis model is established by using affective computing.On the other hand,the system employs virtual reality(VR)visual media music and somatosensory music to play and intelligently match the Five-tone,solving issues in the clinical treatment of Five-tone therapy,such as the difficulties of quantifying emotional data,limited ability for generalization feature extraction,and relatively insufficient research on the intelligence of Five-tone therapy.This study can promote the integration of technology in the fields of healthcare,information,and music,thereby enhancing the accuracy of diagnosis and treatment.It holds significant importance in advancing the intelligent development of Traditional Chinese Medicine diagnosis and treatment.

文摘Considering the comprehensive morphology and genesis of Podzols of the Stolowe Mountains,and the still-possible impact of frost actions and other processes related to cold climate on these soils,the main aims of this study were to determine whether(i)the heterogeneous Podzols in the Stolowe Mountains underwent a phase of development in a cold climate,resulting in frost action features visible on the micromorphological level,and whether(ii)contemporary cryopedogenic traces are masked by the translocation of organic matter due to the podzolisation process.Four soil profiles were investigated,revealing distinct layers corresponding to different periods of soil formation.Under field observation,no explicit frost-related characteristics were observed.Nevertheless,micromorphological analysis revealed cappings of fine materials on grains or peds,as well as development of granostriated(or any striated)b-fabric that resulted from the alternating effects of thawing and freezing processes.Moreover,micromorphological analysis revealed the presence of microstructures that could be the result of cryogenic processes,such as platy,angular blocky and lenticular features,as well as plane,vugh and star-shaped void types.The translocation of organic matter during podzolisation modified or concealed the frost-related features that developed during the late Pleistocene and early Holocene.This is evident,for instance,in the accumulation of organic matter on cappings and within soil voids,which further hinders the identification of frost-related characteristics and the interpretation of the soil's evolution.Macromorphological observations enhanced with micromorphological analyses revealed three distinct layers:(i)a young upper layer composed of loose,sandy material;(ii)a deeper layer containing a spodic horizon with frost actions,involving pedofeatures associated with the Pleistocene cold climate and(iii)a deeper subsoil basal layer.The abovementioned microstructures,combined with lithological discontinuity,support the hypothesised polygenetic origin of Podzols in the studied region.
文摘This paper is dedicated to constructing a theoretical framework for the identification and treatment of affective disorders in traditional Chinese medicine based on the“five-organ view”.Through in-depth analysis of the theoretical connotation of the“five-organ concept”,we discussed the characteristics of the five-organ mechanism of affective-philosophical disorders in detail,systematically constructed a system of identification based on the association of the five organs,and proposed a comprehensive and holistic treatment strategy.The results of the study clearly show that the theoretical framework can provide systematic theoretical guidance for the clinical diagnosis and treatment of affective-philosophical disorders in Chinese medicine,help to improve the diagnostic and therapeutic effects of affective-philosophical disorders,and provide new ideas and methods for the theoretical development and clinical practice of affective-philosophical disorders in Chinese medicine,which is of important theoretical and practical significance,and can further promote the modernization of the development of affective-philosophical disorders in Chinese medicine.
