Methods One hundred and seventy-four cases were divided into a filiform needle group (56 cases), an electroacupuncture group (57 cases) and a warming needle group (61 cases) according to the randomized, controlled and single-blind study principles. Jiānqián (肩前 Extra), Jiānyú (肩髃 LI 15), Jiānliáo (肩髎 TE 14), Nàoshū (臑俞 SI 10), Wàiguān (外关 TE 5), Hégǔ (合谷 LI 4) were used in all three groups but treated with filiform needle, electroacupuncture and warming needle technique respectively. The needles were retained for 30 min. It was given once every other day and 5 times constituted as one course. Pain indices and activity degree of shoulders were measured and recorded before and after treatment every time. Results The total effective rate was 93.0% (53/57) in the electroacupuncture group and 95.1% (58/61) in the warming needle group, both superior to that of 78.6% (44/56) in the filiform needle group (both P〈0.01), but there was no significant difference between electroacupuncture group and warming needle group (P〉0.05). After one course of treatment, the decline indices of shoulder pain of electroacupuncture group (4.28±0.22) and warming needle group (3.74±0.17) were both significantly greater than that of filiform needle group (2.78±0.18)(both P〈0.01). And the decline indices of electroacupuncture group was also greater than that of warming needle group (P〈0.05). The improvements of shoulder activity degree of warming needle group (76.92±5.53) and electroacupuncture group (60.37±3.80) were both greater than that of filiform needle group (42.50±3.67) (both P〈0.01). And shoulder activity degree of warming needle group was also greater than that of electroacupuncture group (P〈0.01). After one course of treatment, improvement of shoulder activity degree and decline indices of shoulder pain in these three groups were better than that after the first time treatment (all P〈0.01). Conclusion All these three acupuncture therapies can achieve good therapeutic effects for frozen shoulder. The therapeutic effects of electroacupuncture and warming needle groups are superior to that of filiform needle group. All these three therapies could significantly reduce patients’ pain and improve their shoulder activity degree. The analgesic effect of electroacupuncture is the best, and the shoulder activity degree improved by warming needle is the best. The improvement of clinical therapeutic effect mainly depends on the therapy and the treatment times when the same acupoints are selected and the condition of illness are similar.展开更多

More recently, an increase in dry needling(DN) courses for physiotherapists has potentiated the rapid growth in DN practice in New Zealand(NZ). Many dry needlers do not consider DN to be a form of acupuncture; it is unknown if they have similar safety concerns. Methods: NZ registered physiotherapists practicing acupuncture and/or DN were invited to participate in an electronic survey to examine their practice and level of understanding in regard to safe needling during pregnancy. Results: Of 124 respondents, only 60(48%) would needle pregnant women, with a further 66% of those still expressing safety concerns. NZ physiotherapists practicing DN only, were more likely to needle areas related to ‘‘forbidden" points in all trimesters. However, overall, NZ physiotherapists were less likely to needle ‘‘forbidden" points than their UK peers. Conclusion: Conflicting literature and a ‘‘fear of blame" influences NZ physiotherapists' decisions to offer needling(both acupuncture and DN) during pregnancy. Further training in this field is recommended to ensure safe practice and adequate provision of acupuncture treatment options for pregnant women suffering musculoskeletal pain, such as LBP and PGP. Further research, particularly into DN, for women during pregnancy, is warranted.展开更多

METHODS: Randomized controlled trials (RCTs) concerned with acupuncture treatment of angina pectoris were identified by searching Academic Source Premier, MEDLINE, Science Citation Index Expanded, and three Chinese databases (China biol- ogy medicine database, China national knowledge infrastructure, and VlP database for Chinese techni- cal periodicals). The valid data were extracted in ac- cordance with our inclusion and exclusion criteria. The main outcomes of the included studies were synthesized using Revman 5.1. RESULTS: Twenty-one articles on 16 individual studies were included and evaluated as having high or moderate risk of bias according to the stan- dards of the Cochrane Collaboration. Meta-analysis indicated that acupuncture combined with conven- tional drugs (ACCD) was superior to conventional drugs alone in reducing the incidence of acute myocardial infarction (AMI) [0R=0.18, 95%CI (0.04,0.84), P=0.03]. Moreover, ACCD was superior to con- ventional drugs in the relief of angina symptoms [0R=4.23, 95%C/(2.73, 6..56), P〈0.00001], and im- provement of electrocardiography (ECG) [0R=2.61, 95%C/(1.83, 3.73), P〈0.00001]. Acupuncture by it- self was also superior to conventional drugs for angi- na symptoms [0R=3.59, 95%C/(1.76, 7.92), P=0.0004] and ECG improvement [0R=3.07, 95%C/(1.54, 6.10), P=0.001]. ACCD was superior to conventional drugs in shortening the time to onset of angina relief [WMD=-1.40, 95% CI (-1.65, -1.15), P〈 0.00001]. However, the time to onset was signifi- cantly longer for acupuncture treatment than for conventional treatment alone [WMD=2A3, 95%O (1.63, 3.23), P〈0.000 01]. CONCLUSION: ACCD reduced the occurrence of AMI, and both acupuncture and ACCD relieved an- gina symptoms and improved ECG. However, com- pared with conventional treatment, acupuncture showed a longer delay before its onset of action. This indicates that acupuncture is not suitable for emergency treatment of heart attack. Owing to the poor quality of the current evidence, the findings of this systematic review need to be verified by more RCTs to enhance statistical power.展开更多

The conventional treatment and deglutition rehabilitation training in neurology department were used in the two groups. In group B, according to patient's condition, the rehabilitation trainings, such as respiration and neck movement range were used selectively. In group A, on the base of deglutition training, acupuncture therapy for relieving throat and opening orifice was supplemented. The acupoints included Sìshéncōng(四神聪EX HN 1),Bāihuì(百会GV 20), Tàiyang(太阳EX-HN5) bilateral, Fēngchi(风池GB 20) bilateral and Shésānzhēn(舌三针).Electrostimulator was attached on EX-HN 1, bilateral GB 20 and Shesanzhen(舌三针). Needles were retained for 30 min in each treatment. The treatment was given once a day, 5 treatments a week and the treatment for 3 weeks as 1 course. After 6 weeks of treatment, the deglutition ability scale developed by Fujishima Ichiro was adopted to determine the therapeutic effects and observe the score increase for dysphagia related to brain infarct sites before and after treatment. Results: In assessment after 6-week treatment, the improvements of deglutition ability were different corresponding to different brain infarct sites in group A. Specially, the improvements in the patients with dysphagia related to cerebral hemisphere infarction in group A were better than group B(8.68 ±1.12 vs7.32 ±0.91, P<0.05), followed by the improvements in the patients with dysphagia related to internal capsule/basal ganglia/diencephalon infarction(6.53 ±0.65 vs 6.36 ±0.84, P>0.05). Regarding the therapeutic effects in comparison of the two groups, the total effective rates in dysphagia related to cerebral hemisphere infarction were different between the two groups significantly(96.67% vs 82.75%, P< 0.05), as well as in the patients related to internal capsule/basal ganglia/diencephalon infarction(88.89% vs 66.67%,P<0.05). The therapeutic effects were not different in the patients related to cerebral stem and cerebella infarction between the two groups(P>0.05).Conclusion: The acupuncture therapy for relieving throat and opening orifice combined with deglutition training achieves the satisfactory therapeutic effects on dysphagia induced by cortical infarction in stroke. However, a large sample and multicentral clinical trial with this therapy is needed for a further argument so that this therapy is likely promoted in clinical practice in future.展开更多

Moreover, music therapy can be used to modify treatment of cerebral palsy. OBJECTIVE: To study the effects of combined treatment using acupuncture and music therapy on gross motor function measure (GMFM) of children with cerebral palsy, compared with acupuncture treatment alone. DESIGN, TIME AND SETTING: Randomized, controlled, clinical study. The experiment was conducted in Shenzhen Hospital of Traditional Chinese Medicine between January 2007 and September 2007. PARTICIPANTS: All children with cerebral palsy in the trial were from the outpatient department of Shenzhen Hospital of Traditional Chinese Medicine. The children were randomly divided into two groups: 30 children in Group B received acupuncture and music therapy, while 30 children in Group A received only acupuncture therapy. METHODS: Subjects in Groups A and B received acupuncture based on syndrome differentiation. The main acupoints were necessary for all participants. At first, flash needling was applied to the acupoints. For the remaining acupoints, the technique of transverse needling was applied to the head acupoints, and perpendicular needling was used for the other points. The inserted needles were twirled and then maintained for 30 minutes. The needle was twirled for one second every other 10 minutes, without reinforcing-reducing techniques. The therapy was performed every other day. The trial consisted of three periods each, and lasted for 84 days. Subjects in Group B received music therapy. They listened to music that they preferred while acupuncture was being performed. Following acupuncture, they were allowed to perform musical activities, such as percussion, singing, and dancing. The music therapy was scheduled for one hour, including listening to music for 30 minutes and music activities for 30 minutes. MAIN OUTCOME MEASURES: The comprehensive functional evaluation scale of cerebral palsy and the GMFM were applied to test symptoms before and after respective treatment. RESULTS: All 60 children were included in final analysis. The general function of all children with cerebral palsy improved, but the general efficiency in Group B was obviously higher than in Group A (93% and 73%, Х^2 = 4.32, P 〈 0.05). GMFM evaluation showed that the function of crawling and kneeling, standing, and walking improved more significantly in Group B than in Group A (P 〈 0.05), while the function of lying down, rolling, and sitting remained similar between the two groups (P 〉 0.05). CONCLUSION: The combination of acupuncture and music therapy on the treatment of cerebral palsy is more effective than acupuncture alone. The various activities in music therapy may affect treatment of cerebral palsy.展开更多

文摘Objective To explore better therapies for the treatment of frozen shoulder. The improvement of clinical therapeutic effect mainly depends on the therapy and the treatment times when the same acupoints are selected and the condition of illness are similar.

文摘OBJECTIVE:Acupuncture has often been used for aphasia rehabilitation in China. The purpose of this paper was to:1) provide a historic overview of acupuncture for aphasia due to stroke;2) summarize the commonly used acupuncture approaches;and 3) objectively comment on the effectiveness of acupuncture for the rehabilitation of this type of disorder.METHODS:The Elsevier database and a Chinese database(CNKI) were searched through December,2010 with the key words "aphasia, acupuncture" in English and Chinese, respectively.Case reports, uncontrolled clinical observations and controlled clinical trials were all included if acupuncture was the sole treatment or the main component of complex intervention for the rehabilitation of aphasia caused by cerebrovascular disease.RESULTS:More than 100 relevant articles were found. After analyzing these articles,we found that acupuncture for apoplectic aphasia most often included tongue, scalp, body and combination acupuncture.Tongue bleeding, deep insertion and strong stimulation were adopted by many practitioners. The ten most frequently used acupoints(or areas) were Lianquan(RN 23),Jinjin(EX-HN 12),Yuye(EX-HN 13),Tongli(HT 5),Fengchi(GB 20),Neiguan(PC 6),Baihui(DU 20),No.1,2 and 3 language sections,Sanyinjiao(SP 6) and Yamen(DU 15).CONCLUSIONS:Controlled clinical studies and a systematic literature review demonstrate that acupuncture has therapeutic effects on aphasia after stroke.
文摘Objective: Acupuncture guidelines have advised caution when treating women during pregnancy, because historical ‘‘forbidden" acupuncture points are believed to stimulate miscarriage or early labor.Despite recent research demonstrating that acupuncture is a useful and safe treatment tool for pregnancy-related low-back pain(LBP) and pelvic girdle pain(PGP), it is postulated that fear of miscarriage and subsequent blame by association, restricts its provision. Further research, particularly into DN, for women during pregnancy, is warranted.

文摘OBJECTIVE: To assess the effectiveness and safety of acupuncture therapy for angina pectoris. Owing to the poor quality of the current evidence, the findings of this systematic review need to be verified by more RCTs to enhance statistical power.

文摘Objectives: To observe the effectiveness of acupuncture therapy for relieving throat and opening orifice on dysphagia due to different brain infarct sites. Methods: According to the random number table, 128 patients with post-stroke dysphagia were divided into group A(63 cases, intervention with deglutition training and acupuncture) and group B(65 cases, intervention with simple deglutition training). However, a large sample and multicentral clinical trial with this therapy is needed for a further argument so that this therapy is likely promoted in clinical practice in future.
文摘BACKGROUND: The use of acupuncture has received recognition to effectively treat cerebral palsy. The various activities in music therapy may affect treatment of cerebral palsy.
文摘Objective:Emerging evidence shows the effectiveness of speech and language therapy(SLT);however, precise therapeutic parameters remain unclear.Evidence for the use of adjunctive transcranial direct current stimulation(t DCS)to treat post-stroke aphasia(PSA)is promising;however, the utility of combining t DCS and electroacupuncture(EA)has not yet been analyzed. This study assessed the therapeutic consequences of EA and t DCS coupled with SLT in subacute PSA patients who were also undergoing hyperbaric oxygen therapy(HBOT).Methods:A retrospective analysis was conducted on subacute(<6 months)PSA patients who were divided into three groups:patients who received EA plus t DCS(acupuncture group),patients who underwent t DCS(t DCS group),and patients who experienced conventional therapy(HBOT+SLT).All subjects underwent 21 days of treatment and also received conventional treatment. The aphasia battery of Chinese(ABC)was used to score pre-and post-intervention status. Results:The analysis comprised 238 patients.Cerebral infarction was the most frequent stroke type(137[57.6%]),while motor(66[27.7%])and global aphasia(60[25.2%])were the most common types of aphasia. After 21 days of intervention, the ABC scores of all patients were improved. The acupuncture group had the highest ABC scores, but only repetition, naming, and spontaneous speech were statistically improved(P<0.01).Post-hoc tests revealed significant improvement in word retrieval in the acupuncture and t DCS groups(P<0.01,P=0.037),while the acupuncture group had additional significant improvement in spontaneous conversation(P<0.01).Conclusion:Combining acupuncture and t DCS as an adjuvant therapy for subacute PSA led to significant spontaneous speech and word retrieval improvements.Future prospective,multi-ethnic,multi-center trials are warranted.
文摘To observe clinical therapeutic effects of acupuncture combined with music therapy for treatment of cerebral palsy. Methods: Sixty children with cerebral palsy were randomly divided into an acupuncture group (Group Acup.) and an acupuncture plus music group (Group Acup.+ M). Simple acupuncture was applied in Group Acup., and acupuncture at 5 groups of points plus music were applied in Group Acup. +M., The treatment was given once every two days with 3 treatments weekly, and 36 treatments constituted a therapeutic course. Therapeutic effects including movement improvement were observed for comparison after 3 courses of treatments. Results: The comprehensive functions were elevated in both groups, and the total effective rate in Group Acup. + M was obviously better than that in Group Acup (P〈0.05). Movement functions were also improved in both groups, but the differences in improvement of creeping and kneeling, standing, and walking were significant between the two groups (P〈0.01), showing the effect in Group Acup. + M was better than that in Group Acup.. Conclusion: The therapy of acupuncture plus music gained better therapeutic effect on cerebral palsy than simple acupuncture, which provided new thoughts for treating the disease by comprehensive therapies.
