Acupuncture-like therapies, including electroacupuncture (EA) and transcutaneous electrical acupoint stimulation (TEAS), become more popular world-wide. Increasing evidence has demonstrated that EA and TEAS are effective in treating gynecological disorders, especially infertility. This present paper describes how to select acupoints for the treatment of infertility from the view of theories of traditional Chinese medicine and how to determine critical parameters of electric pulses of ENTEAS based on results from animal and clinical studies. It summarizes the principles of clinical application of EA/rEAS in treating various kinds of reproductive disorders, such as polycystic ovary syndrome (PCOS), pain induced by oocyte retrieval, diminished ovarian reserve, embryo transfer, and oligosperrnia/ asthenospermia. The possible underlying mechanisms mediating the therapeutic effects of EA/TEAS in reproductive medicine are also examined.展开更多

Methods Computer retrieval was carried out in such databases as Pub Med, Embase, Cochrane Library, web of science, Chinese biomedicine database(CBM), China National Knowledge Infrastructure(CNKI), Wan Fang and VIP in order to collect the randomized controlled trials(RCT) concerning acupoint stimulation therapy for treatment of POI. Software R 3.40 and stata 14.0 were used for Meta-analysis and network Meta-analysis, and Rev Man 5.3 was used for plotting the risk bias diagrams. Results Fortythree RCTs were included in total, involving 3046 POI patients and 18 acupoint stimulation therapies and comprehensive therapies. Metaanalysis showed:(1) The curative effects of acupoint stimulation therapy(RR=1.25, 95%CI [1.07,1.45]), acupoint stimulation therapy+Chinese herbal medicine(RR=1.25, 95%CI [1.18,1.32]) and acupoint stimulation therapy+ hormone replacement therapy(HRT)(RR=1.20, 95%CI [1.12,1.29]) were all superior to that of HRT, indicating that the differences were statistically significant(Z=2.90, P=0.04; Z=7.56, P〈0.000 01; Z=4.06, P〈0.000 01).(2) Compared with HRT, the occurrence rate of adverse effect of acupoint stimulation therapy was lower, and the safety was superior to that of HRT(RR=0.18, 95%CI [0.08,0.41]), indicating that the differences were statistically significant(Z=4.08, P〈0.000 1). Forty-two direct comparisons and 110 indirect comparisons were generated according to network Meta-analysis, among which, 38 comparisons were statistically significant. Network Metaanalysis results with HRT as control showed: the therapeutic measures ranking top 3 according to the curative effect sequence were catgut embedment in acupoint, moxibustion and warming-needle moxibustion, successively, and all the 3 measures were monotherapies without reflecting the advantages of comprehensive therapy. HRT ranked 17 th among the 18 included therapeutic measures. Conclusion On the basis of current evidences, acupoint stimulation therapy has a better clinical efficacy and safety for treatment of POI when compared with HRT. The acupoint stimulation therapies ranking the top 3 have more significant curative effects, but the long-term efficacy and the effect on the ovarian function still need to be further explored. In addition, the conclusion of this study still needs to be verified through a large number of RCTs with reasonable designs and appropriate methods.展开更多

文摘Transcutaneous electrical acupoint stimulation(TEAS)is a kind of physical therapy that use electric cur-rent through the electrodes placed on the surface of acupoints to produce clinical effects in the human body, which is characterized by less adverse reaction and convenient operation. It has been widely used in the treatment of various diseases. This review introduces six major clinical applications of TEAS,named analgesia, regulation of gastrointestinal function, improvement of reproductive function, enhancement of cognitive function, promotion of limb function recovery and relief of fatigue.Besides,TEAS has been ap-plied to the treatment of other chronic diseases such as hypertension and diabetes, achieving satisfactory clinical effects. However, two crucial challenges are encountered in the development of TEAS.One is the lack of standardization in the selection of parameters such as waveform, frequency, intensity and stimula-tion duration. The other is the limitation on the flexibility in the acupoint selection. This review analyzes key issues that need to be addressed in the current clinical application of TEAS,such as the selection of parameters and acupoints, and this review provides a certain reference value for optimizing regimens of TEAS and promoting its development and application.

文摘Abstract: With the rapid development of assisted reproductive technology, various reproductive disorders have been effectively addressed. The possible underlying mechanisms mediating the therapeutic effects of EA/TEAS in reproductive medicine are also examined.

文摘Objective To systematically evaluate the effectiveness and safety of acupoint stimulation therapy for treatment of premature ovarian insufficiency(POI). In addition, the conclusion of this study still needs to be verified through a large number of RCTs with reasonable designs and appropriate methods.

文摘BACKGROUND Acupuncture promotes the recovery of gastrointestinal function and provides analgesia after major abdominal surgery. The effects of transcutaneous electrical acupoint stimulation(TEAS)remain unclear.AIM To explore the potential effects of TEAS on the recovery of gastrointestinal function after gastrectomy and colorectal resection.METHODS Patients scheduled for gastrectomy or colorectal resection were randomized at a 2:3:3:2 ratio to receive:(1)TEAS at maximum tolerable current for 30 min immediately prior to anesthesia induction and for the entire duration of surgery, plus two 30-min daily sessions for 3 consecutive days after surgery(perioperative TEAS group);(2)Preoperative and intraoperative TEAS only;(3)Preoperative and postoperative TEAS only;or(4)Sham stimulation. The primary outcome was the time from the end of surgery to the first bowel sound.RESULTS In total,441 patients were randomized;405 patients(58.4±10.2 years of age;247 males)received the planned surgery. The time to the first bowel sounds did not differ among the four groups(P=0.90;log-rank test). On postoperative day 1,the rest pain scores differed significantly among the four groups(P=0.04;Kruskal–Wallis test).Post hoc comparison using the Bonferroni test showed lower pain scores in the perioperative TEAS group(1.4±1.2)than in the sham sti-mulation group(1.7±1.1;P=0.04).Surgical complications did not differ among the four groups.CONCLUSION TEAS provided analgesic effects in adult patients undergoing major abdominal surgery, and it can be added to clinical practice as a means of accelerating postoperative rehabilitation of these patients.
文摘 217 cases of chronic bronchitis and asthma were clinically treated and analyzed for the effects of combining electric stimulation with topical application of drug on acupoints. The results suggested that the combined therapy was superior to unitary therapy (P<0.05). It is indicated that the combined therapy has a good curative effect in both short- and long-terms.
文摘Longterm potentiation (LTP ) is usually induced by direct brain stimulation. An attempt has been made to evoke LTP in dentate granule cells of hippocampus by acupoint stimulation in anesthetized rats. Assuming a gradual increasing course, LTP rose to 146% at the end of one hour. After applying such stimulation to the awake rats for six days (once everyday), their discriminative learning capacity in Y maze test markedly improved as compared with that of the control.
