文摘Gastric ulcer(GU)is a common digestive system disease.Acupuncture,as one of the external treatments of traditional Chinese medicine(TCM),has the characteristics of multi-target,multi-pathway and multi-level action in the treatment of GU.The relationship between meridian points and Zang-fu is an important part of the theory of TCM,which is crucial for the diagnosis and treatment of diseases. There is an external and internal link between acupoints and Zang-fu.The pathological reaction of Zang-fu can manifest as acupoint sensitization, while stimulation of acupoints can play a therapeutic role in the internal Zang-fu.Therefore,the acupoint has the functions of reflecting and treating diseases. This review explores the tender points on the body surface of patients with GU and the rules of acupoint selection. In addition,Zusanli(ST36),as one of the most used acupoints of the stomach meridian, was selected to show the mechanisms behind acupoint stimulation in the treatment of GU in greater detail, specifically in the well-studied model of the stress GU(SGU).Hence, the mechanisms of acupuncture at ST36 and points commonly used in combination with ST36 to treat SGU are discussed further.Treatment effects can be achieved through anti-inflammatory and antioxidant activities, gastric mucosal injury repair, and interaction with the brain-gut axis.In summary, this review provides evidence for a comprehensive understanding of the phenomena and mechanism of acupoint functions for GU.

文摘Transcutaneous electrical acupoint stimulation(TEAS)is a kind of physical therapy that use electric cur-rent through the electrodes placed on the surface of acupoints to produce clinical effects in the human body, which is characterized by less adverse reaction and convenient operation. It has been widely used in the treatment of various diseases. This review introduces six major clinical applications of TEAS,named analgesia, regulation of gastrointestinal function, improvement of reproductive function, enhancement of cognitive function, promotion of limb function recovery and relief of fatigue.Besides,TEAS has been ap-plied to the treatment of other chronic diseases such as hypertension and diabetes, achieving satisfactory clinical effects. However, two crucial challenges are encountered in the development of TEAS.One is the lack of standardization in the selection of parameters such as waveform, frequency, intensity and stimula-tion duration. The other is the limitation on the flexibility in the acupoint selection. This review analyzes key issues that need to be addressed in the current clinical application of TEAS,such as the selection of parameters and acupoints, and this review provides a certain reference value for optimizing regimens of TEAS and promoting its development and application.

文摘Objective:Varied acupoint selections represent a potential cause of the uncertainty surrounding the efficacy of acupuncture for knee osteoarthritis(OA).Skin temperature,a guiding factor for acupoint selection, may help to address this issue. This study explored thermal sensitization of acupoints used for the treatment of knee OA.Methods:This cross-sectional case-control study enrolled cases aged 45–75 years with symptomatic knee OA and age-and gender-matched non-knee OA controls in a 1:1 ratio. All participants underwent infrared thermographic imaging. The primary outcome was the relative skin temperature of acupoint(STA),and the secondary outcome was the absolute STA of 11 acupoints. The Z test was used to compare the relative and absolute STAs between the groups.Principal component analysis was used to extract the common factors(CFs,acupoint cluster)in the STAs.A general linear model was used to identify factors affecting the STA in the knee OA cases. For the group comparisons of relative STA,P<0.0045(adjusted for 11 acupoints through Bonferroni correction)was considered to indicate statistical significance. For other analyses,P<0.05 was used as the threshold for statistical significance. Results:The analysis included 308 participants, consisting of 151 cases(mean age:[64.58±6.67]years;male:25.83%;mean body mass index:[25.70±3.16]kg/m~2)and 157 controls(mean age:[63.37±5.96]years;male:26.11%;mean body mass index:[24.47±2.84]kg/m~2).The relative STAs of ST34(P=0.0001),EX-LE2(P<0.0001),EX-LE5(P=0.0006),SP10(P<0.0001),BL40(P=0.0012)and GB39(P=0.0037)were higher in the knee OA group. No difference was found in the STAs of ST35,ST36,SP9,GB33 and GB34.Four CFs were identified for relative STA in both groups. The acupoints within each CF were consistent between the groups. The mean values of the relative STAs across each CF were higher in the knee OA group. In the knee OA cases,no factors were observed to affect the relative STA,while age and gender were found to affect the absolute STA.Conclusion:Among patients with knee OA,thermal sensitization occurs in the acupoints of the lower extremity, exhibiting localized and regional thermal consistencies. The thermally sensitized acupoints that we identified in this study,ST34,SP10,EX-LE2,EX-LE5,GB39 and BL40,may be good choices for the acupuncture treatment of knee OA.

文摘Immunization has long played essential roles in preventing diseases. However, the desire for precision delivery of vaccines to boost a robust immune response remains largely unmet. Here,we describe the use of acupoint delivery of nanovaccines(ADN)to elicit dual-niche immunological priming.ADN can simultaneously stimulate mast cell-assisted maturation of dendritic cells at the acupoint and enable direct delivery of nanovaccines into the draining lymph nodes. We demonstrate that ADN not only provokes antigen presentation by lymph node-resident CD8α^(+)dendritic cells, but also induces the accumulation of nanovaccines in B-cell zones, amplifying antigen-specific cytotoxic T lymphocyte responses and immunoglobulin G antibody expression in draining lymph nodes.ADN also generates systemic immune responses by causing immune memory and preventing T-cell anergy in the spleen.Further supported by evoking effective antitumor responses and high-level antiviral antibodies in mice,ADN provides a simple yet versatile platform for advanced nanovaccination.
文摘Background:Acupuncture is widely used in modulating brain excitability and motor function,as a form of complementary and alternative medicine. However, there is no existing meta-analysis evaluating the effectiveness and safety of acupuncture on corticospinal excitability(CSE),and the credibility of the evidence has yet to be quantified. Objective:This study was designed to assess the efficacy and safety of electroacupuncture(EA)and manual acupuncture(MA)in enhancing brain excitability, specifically focusing on CSE as measured by transcranial magnetic stimulation(TMS).Search strategy:This study followed a systematic approach, searching 9 databases up to August 2024 and examining grey literature,in compliance with Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines.Inclusion criteria:Studies were included if they compared the clinical efficacy of EA or MA with sham acupuncture,no treatment or usual training.Data extraction and analysis:Three investigators independently conducted literature screening, data extraction, and risk of bias assessment. The primary outcome focused on motor-evoked potentials as measured by TMS,with treatment effects quantified using mean differences or standardized mean differences between pre-and post-treatment.Subgroup analyses were conducted using mixed-effects models, while random-effects or fixed-effects models were used to estimate average treatment differences across studies. Results:Based on 34 studies involving 1031 adults, acupuncture techniques significantly enhanced CSE.EA had a greater impact than MA,with effect sizes of 0.53 mV vs 0.43 mV(95%confidence interval[CI]:[0.30,0.76],P<0.00001 vs 95%CI:[0.28,0.59],P<0.00001).The 5 most frequently used acupoints were LI4(Hegu,32 times),ST36(Zusanli,10 times),LI11(Quchi,7 times),TE5(Waiguan,6 times),and GB34(Yanglingquan,5 times). Conclusion:This systematic review indicates that both EA and MA could effectively and safely enhance CSE,bringing the corticospinal pathway closer to the threshold for firing, which may ultimately improve motor function.LI4,ST36,LI11,TE5 and GB34 are the most commonly used acupoints.
文摘The objective of this study is to analyze the relevant literature on acupoint massage applied to patients with poststroke constipation,to explore the research status, research hotspots, and research frontiers of poststroke constipation, and to provide reference for clinical practice and scientific research. With the help of CiteSpace software, the related literatures published in the China National Knowledge Infrastructure database on the application of acupoint massage to constipation after stroke were analyzed, and scientific atlas for visual analysis was drawn. The hot keywords in the field of acupoint massage for post-stroke constipation are constipation, nursing intervention, and traditional Chinese medicine(TCM)nursing, indicating the great importance of TCM nursing for such patients. In mutation word detection, words such as constipation, nursing, stroke, and defecation reflex have earlier mutation times.Massage is the keyword with the longest mutation time, suggesting it is an effective intervention for post-stroke patients and is widely recognized. The commonly used acupoints for poststroke constipation are Tianshu(ST 25),Zhongwan(CV 12),and Qihai(CV 6).These acupoints can regulate qi and blood and enhance gastrointestinal peristalsis, providing guiding significance for formulating relevant clinical practice guidelines. The research of acupoint massage in the nursing of poststroke constipation is still in the stage of practice and exploration, and the specific definition of operation process, technique, acupoint, time, and intensity is still not clear.Multicenter,large-sample,in-depth intervention research, and cohort studies hold immense significance for the future development of this discipline.

文摘Objective:To investigate whether acupoint penetration acupuncture(APA)could regulate chondrocyte autophagy and apoptosis via the PI3K/Akt-mTOR signaling pathway to reduce cartilage degeneration in knee osteoarthritis(KOA)rats. Methods: KOA was induced in rats via intra-articular injection of sodium iodoacetate resolution.Forty male Sprague-Dawley rats were randomly assigned to blank control,model,APA,electro-acupuncture(EA),and sham model groups(n=8)and those in the APA and EA groups received their respective therapies.Following completion of the treatment course, histological examinations of cartilage and muscle were conducted.Levels of apoptosis-and autophagy-related factors, including Bax,Bcl-2,mTOR,ULK-1,and Beclin-1 protein, and mRNAs were assessed.Additionally,β-endorphin(β-EP)concentrations in the brain and serum were measured. Results: Histological analysis revealed that APA alleviated cartilage and muscle damage compared with the model group.APA inhibited cartilage degeneration by modulating the expression of apoptosis-and autophagy-related proteins and mRNA,thus preventing chondrocyte apoptosis. In the APA group,Bax and mTOR protein levels were significantly lower than those in the model group(both P=.024).Conversely,the Bcl-2 expression level was significantly higher than that in the EA group(P=.035).Additionally,ULK-1 expression was significantly lower than that in the EA group(P=.045).The mRNA level of Bax was significantly higher than that in the blank control group(P<.001).However,Beclin-1 levels were significantly higher than those in both the model and EA groups(both P<.001).ELISA results showed a significant decrease in the concentration ofβ-EP in the brains of the rats in the APA group compared with those in the model group(P=.032).Conclusions: APA reduced osteoarthritis-related pain and alleviated cartilage damage by upregulating chondrocyte autophagy and down-regulating apoptosis via signaling pathways involving PI3K/Akt-mTOR in KOA rats.
