Objective:To compare the therapeutic effects of Shengjing Zhongzi Tang(生精种子汤Decoction for Generating Sperms) and Wuzi Yanzong Wan(五子衍宗丸Pills for Reproduction) for asthenospermia and oligozoospermia.Methods:Seventy patients with asthenospermia and oligozoospermia were divided into a treatment group of 35 cases treated with Shengjing Zhongzi Tang and a control group of 35 cases treated with Wuzi Yanzong Wan for 3 months respectively.The therapeutic effects were evaluated,the semen parameters were analyzed and the spermatic morphology was judged before and after treatment.Results:In the treatment group,the total effective rate was 91.4% with a pregnant rate of 8.6% spouses,and the markedly effective rate was 65.7%.In the control group,the total effective rate was 85.7% with a pregnant rate of 5.7% spouses,and the markedly effective rate was 54.3%.Both the total effective rate and the markedly effective rate were higher in the treatment group than those in the control group,but with no significant statistical difference(P>0.05).The spermatic density,spermatic motility and the percentage of normal spermatic morphology were obviously enhanced in the two groups with the indexes in the treatment group superior to those in the control group(P<0.05 or P<0.01).Conclusion:Shengjing Zhongzi Tang with obvious therapeutic effects for asthenospermia and oligozoospermia can significantly enhance the spermatic density and motility,and raise the pregnant rate in spouses. 展开更多

Objective:The recent advent of flow cytometry(FCM),coupled with fluorescent dyes,has been successfully applied to assess mitochondrial function.The aim of this study was to investigate the feasibility and clinical significance of detecting sperm mitochondrial function and to evaluate sperm mitochondrial function by using Rhodamine 123/propidium(Rh123/PI)dual fluorescent staining and FCM in asthenospermia and oligoasthenozoospermia.Methods:Twenty-five fertile men(with normal sperm parameters)and 230 infertile patients were examined.Fifty-five patients of the above 230 patients were selected for idiopathic infertility samples and were divided into two groups:asthenospermia(n=30)and oligoasthenozoospermia(n=25).Rh123/PI dual fluorescent staining and FCM were carried out to examine sperm mitochondrial function.Results:Significant differences were found between the normal and abnormal semen samples(P0.05)when Rh123+/PI-,Rh123-/PI+and Rh123-/PI-sperm were examined by FCM,but there was no significant difference between the asthenospermia(P=0.469) and oligoasthenozoospermia group(P=0.950)when Rh123+/PI-and Rh123-/PI+sperm were then examined;however,a significant difference was found between the 2 groups(P=0.003)when Rh123-/PI-sperm were examined.There was no correlation between Rh123-/PI-sperm and semen parameters in the normal group,but there was a significant negative correlation between the sperm concentration and Rh123-/PI-sperm in asthenospermia and oligoasthenozoospermia patients(r=-0.509,-0.660;P=0.018,0.038).Conclusion:Rh123/PI dual fluorescent staining and FCM can provide reliable information to assess the quality of sperm and reveal differences in mitochondrial membrane potential in asthenospermia and oligoasthenozoospermia. 展开更多

Differential analysis of two-dimensional gel electrophoresis profiles of spermatozoa protein in human normal motility sperm and idiopathic asthenospermia

Methods: Two-dimensional gel electrophoresis was performed on 4 normal sperm samples from healthy men and 4 sperm samples from 4 asthenospermia patients. After silver staining, the differential expression proteins were analyzed by PDQuest 2D analysis software. Results: Six differential protein spots were identified. Four spots showed increased expression in the control gels compared with the patient gels. Conclusion: The protein profiles of differential expression between the normal spermatozoa and idiopathic asthenospermia were established and some differential proteins were found. The data of this study would establish the better fundament for further isolation and identification of differentially expressed proteins in human asthenospermia sperm. 展开更多

Effectiveness of Testosterone Undecanoate Treatment in Men with Asthenospermia

Fifty men with asthenospermia were includ... Objective To assess the effectiveness of testosterone undecanoate on sperm motility and pregnancy incidence in men with asthenospermia. Methods A clinical trial was performed. Fifty men with asthenospermia were included to receive placebo (control group, n=9) or T undecanoate 80 mg/d (study group, n=41). Pregnancy incidence and sperm characteristics after 1, 2 and 3 months of medication and 3 months after the end of the trial were measured. Results Compared with the placebo, T undecanoate treatment produced a satisfactory improvement of seminal motility (F=55.904, P=0.000). In study group, the incidence of pregnancy was 28.2% while the incidence of pregnancy in control group was 11.1%. In study group, 26patients took T undecanoate for more than 3 months. In the 3 months, semen volume showed no statistical difference (F=1.206, P=0.312) before and after treatment, sperm concentration (F=0.023, P=0.000) and motility a, b, a +b (P=0.000) showed statistical differences. Motility grade a showed significantly higher increment in 2 and 3 months after treatment than 1 month and there was no statistical difference between 2 and 3 months. So did grade b. Conclusion The results indicate that T undecanoate increases semjnal motility, leading to a higher incidence of pregnancy in couples with infertility related to asthenoaspermia. 展开更多

Objective:Exploring the material basis and mechanism of Kidney-invigorating and Spleenstrengthening method for treating oligospermia based on network pharmacology.Method:Select the effective elements and targets of Yishen Jianpi core drug pairs by searching the Chinese Medicine System Pharmacology Platform(TCMSP);select the disease targets of oligozoospermia by searching the human genetic information database;use R language software to select drugs and Disease common target,use Cytoscape software to build a drugdisease-target interaction network;build a common target protein interaction network(PPI)through the STRING platform;apply R language to the core drugs of Yishen Jianpi and oligospermia Enrichment analysis of GO and KEGG was performed on the common target.Result:A total of 97 effective active ingredients were obtained from Yishen Jianpi traditional Chinese medicine,and 46 possible targets for Yishen Jianpi traditional Chinese medicine to treat oligospermia were obtained.Combined with the results of KEGG enrichment analysis,it can be speculated that Yishen Jianpi traditional Chinese medicine improved male oligospermia The whole mechanism of action may be related to the regulation of human endogenous hormones.Conclusion:Through the research method of network pharmacology,the possible effect targets and pathways of Yishen Jianpi traditional Chinese medicine in the treatment of male oligospermia are predicted. 展开更多

Objective To investigate the underlying mechanism of“Zhibian”(BL54)-toward-"Shuidao"(ST28)acupuncturettechniqueforimproving reproductive function in mice with asthenospermia by regulating ferroptosis pathway.Methods Sixty male C57BL/6 mice were randomly divided into a blank group,a model group,an acupuncture group and a Fer-1 group,15 mice in each one.Except the blank group,the intraperitoneal injection with cyclophosphamide(50·kg-1.d-')was administered to establish the asthenospermia model.in the mice of the rest 3 groups for 5 consecutive days.In the acupuncture group,"Zhibian"(BL54)-toward-"Shuidao"(ST28)acupuncture technique was operated in the mice,for 20 min each time;and in the Fer-l group,Fer-1 solution(1 mg/kg)was injected intraperitoneally.The interventions of these two groups were delivered once daily and for 2 consecutive weeks.The testicular wet weight was measured and the testicular coefficient was calculated.Using sperm quality detection system,the sperm quality was detected.With ELISA used,the contents of testosterone(T),follicle-stimulating hormone(FSH)and luteinizing hormone(LH)in the serum were detected.With HE staining,testicular and epididymal morphology was observed.Immunofluorescence was used to detect the expression of reactive oxygen species(ROS)in the testes.Biochemical assay was conducted to determine the contents of malondialdehyde(MDA),reduced glutathione(GSH),and total iron ion(TFe)in the testicular tissue.Transmission electron microscopy was used to examine mitochondrial structure of the testis,while JC-1 staining was used to assess mitochondrial membrane potential in the testicular tissue.Fluorescence quantitative PCR and Western blot analyseswere employed to measure the mRNA and protein expression of solute carrier family 7 member 11(SLC7A11),glutathione peroxidase 4(GPX4),ferritin heavy chain 1(FTH1),and acyl-CoA synthetase long-chain family member 4(ACSL4)in the testicular tissue.Results Compared with the blank group,in the model group,the testicular wet weight and testicular coefficient decreased(P<0.01);the sperm concentration and sperm motility reduced(P<0.01),and the contents of T,FSH,and LH decreased in the serum(P<0.01);and the seminiferous tubules in the testis showed loose structure and deformed lumen,sperm cells were disorganized and the sperm numbers reduced;the tubular walls became thinner,and sperm numbers in the lumen less;the expression of ROS in testicular tissue,as well as the contents of MDA and TFe increased(P<0.01),and the content of GSH decreased(P<0.01);and the numbers of mitochondria reduced,the structure of cristae was serious damaged,and mitochondrial membrane potential level declined(P<0.O1);the mRNA and protein expression of SLC7A11,GPX4,and FTH1 decreased(P<0.01),while the mRNA and protein expression of ACSL4 increased(P<0.01).In comparison with the model group,the acupuncture and Fer-1 groups showed the increase of testicular wet weight and coefficient(P<0.01),sperm concentration and motility(P<0.01),and the serum contents of T,FSH,and LH(P<0.01);and the improvements in testicular and epididymal histopathology;ROS expression and the contents of MDA and TFe decreased(P<0.01),and the content of GSH elevated(P<0.05);the mitochondrial structure and numbers were ameliorated and mitochondrial membrane potential rose(P<0.01).Besides,in comparison with the model group,the mRNA expression of SLC7A1l was higher(P<0.05,P<0.01),the mRNA and protein expression of GPX4 and FTH1 increased(P<0.01,P<0.05),and the mRNA and protein expression of ACSL4 decreased(P<0.01)in the acupuncture and the Fer-1 groups;and the protein expression of SLC7A11 was higher in the Fer-1 group(P<0.05).Conclusion"Zhibian"(BL54)-toward-"Shuidao"(ST28)acupuncture technique may improve the reproductive capacity in the mice with asthenospermia by alleviating ferroptosis-induced cellular damage and ameliorating testicular function. 展开更多

目的探讨精浆游离miR-122—3p和miR-141—5p的稳定性及其对特发性弱精症的诊断价值。方法分析不同的室温孵育时间、反复冻融次数、4℃放置时间下精浆游离miR-122-3p和miR-141—5p的稳定性。应用实时荧光定量聚合酶链反应(real-time flu... 目的探讨精浆游离miR-122—3p和miR-141—5p的稳定性及其对特发性弱精症的诊断价值。方法分析不同的室温孵育时间、反复冻融次数、4℃放置时间下精浆游离miR-122-3p和miR-141—5p的稳定性。应用实时荧光定量聚合酶链反应(real-time fluorescence guantitative-polymerase chain reaction,RT—PCR)检测实验组和对照组精浆miR-122—3p和miR-141—5p的含量,以U6snRNA作为内参。相对定量采用2^-ΔΔCt帕法。将特发性弱精症组按精子前向运动百分比分为a组(0~20%)和b组(21%~32%),比较两组精浆miR-122—3p和miR-141—5p的含量差异。结果在不同的室温孵育时间、4℃放置时间下,精浆miR-122—3p和miR-141-5p含量无明显变化。在反复冻融的条件下,精浆miR-122-3p和miR-141—5p随着冻融次数的增加而减少。RT-PCR结果表明,特发性弱精症组的精浆游离miR-122-3p含量低于对照组,差异有统计学意义(P〈0.05),miR-141-5p含量高于对照组,差异有统计学意义(P〈0.05)。精浆游离miR-122—3p和miR-141—5p含量在特发性弱精症a组和b组之间均有显著性差异(P〈O.05)。ROC曲线结果显示,miRq22—3p曲线下面积为0.88,当以1.02为最佳截断值时,敏感度为83%,特异度为84%。miR-141—5p的曲线下面积为0.88,当以2.95为最佳截断值时,敏感度为84%,特异度为70%。结论精浆中游离miR-122-3p和miR-141—5p可稳定存在,其在特发性弱精症中的差异表达对特发性弱精症的诊断有一定的应用价值。 展开更多

Objective: To investigate the specific mechanism of hypoxia-inducible factor 1 alpha (HIF-1α) in the regulation of human sperm apoptosis, and to provide a new theoretical reference and scientific basis for the diagnosis and treatment of asthenospermia and other related conditions. Methods: Semen samples were categorized into the normal group and asthenospermia group based on sperm motility criteria. HIF-1α interfering agent cobalt chloride (CoCl2) and guanylate cyclase activator (Lificiguat, YC-1) were added respectively, with a control group established accordingly. Sperm motility (using anterior viability rate as an index), apoptosis level, ATP level, mitochondrial membrane potential, and reactive oxygen species (ROS) level were measured. The expression levels of HIF-1α, p-PI3K, and Bcl-2 in the samples were analyzed using Western blotting. Results: Following CoCl2 treatment, there was a significant increase in sperm apoptosis compared to the normal control group (12.51% ± 2.50% VS 11.15% ± 2.42%);additionally, sperm motility (45.34% ± 3.37% VS 51.36% ± 11.68%), ATP production (11.51 ± 2.87 nM/µL VS 14.99 ± 2.83 nM/µL), ROS levels, and mitochondrial membrane potential all decreased significantly (all P α and p-PI3K increased significantly while Bcl-2 expression decreased (all P α in the YC-1 treatment group were decreased, and the expression level of Bcl-2 was increased (all P α can influence human sperm apoptosis and motility through the PI3K signaling pathway. 展开更多

特发性弱精子症患者精浆25(OH)D水平和精液参数在IVF结局中的相互作用

目的探究特发性弱精子症(IAS)患者精浆25-羟维生素D[25(OH)D]水平和精液参数在体外受精(IVF)结局中的相互作用。方法选取2022年5月至2024年3月解放军总医院第六医学中心收治的200对行IVF助孕的夫妇作为研究对象,进行回顾性分析,男方均为... 目的探究特发性弱精子症(IAS)患者精浆25-羟维生素D[25(OH)D]水平和精液参数在体外受精(IVF)结局中的相互作用。方法选取2022年5月至2024年3月解放军总医院第六医学中心收治的200对行IVF助孕的夫妇作为研究对象,进行回顾性分析,男方均为IAS患者,根据IVF结局分为妊娠成功组(n=65)和妊娠失败组(n=135)。比较两组临床资料、精液参数、25(OH)D水平,比较不同血清25(OH)D水平患者精液质量和精浆25(OH)D水平,比较不同血清、精浆25(OH)D水平在不同精液参数下妊娠失败风险差异,采用多因素Logistic回归分析妊娠失败的影响因素,采用多元线性回归模型和变化趋势图分析血清、精浆25(OH)D水平与精液参数的关系,采用限制性立方样条模型分析血清、精浆25(OH)D水平和精液参数与妊娠失败风险的剂量反应关系,采用非条件Logistic回归模型和计算表分析血清、精浆25(OH)D水平与精液参数对妊娠失败的交互作用。结果妊娠失败组男方年龄、体重指数(BMI)、不良生活习惯比例、卵泡刺激素(FSH)水平高于妊娠成功组(P<0.05)。妊娠失败组精子浓度、精子活动率、前向运动精子百分率(PR)、血清25(OH)D、精浆25(OH)D水平低于妊娠成功组(P<0.05)。男方年龄、BMI、不良生活习惯、FSH、精子浓度、精子活动率、PR、血清25(OH)D、精浆25(OH)D是妊娠失败的独立影响因素(P<0.05)。不同血清25(OH)D水平患者PR和正常形态精子百分率(MNS)比较,差异具有统计学意义(P<0.05)。随着PR、MNS降低,血清25(OH)D水平降低(P<0.05);随着精子浓度、精子活动率、PR降低,精浆25(OH)D水平降低(P<0.05)。血清、精浆25(OH)D水平、精子浓度、精子活动率、PR与妊娠失败风险存在非线性关系(非线性检验均P<0.001)。随着精子浓度、精子活动率、PR降低,妊娠失败风险增加(P<0.05);随着血清、精浆25(OH)D水平降低,妊娠失败风险增加(P<0.05)。血清(<35.8 mmol/L)、精浆25(OH)D水平(<6.0 mmol/L)与精子浓度(<25×10^(6)/mL)、精子活动率(<40%)、PR(<32%)对妊娠失败均存在相加交互作用(P<0.05)。结论IAS患者血清、精浆25(OH)D水平与精液参数(精子浓度、活动率、PR)均存在相加交互作用,且均对IVF结局有影响,故优化血清、精浆25(OH)D水平并改善精液参数有助于提高IVF成功率。 展开更多

Insulin-like growth factor 2(IGF2)is a critical endocrine mediator implicated in male reproductive physiology.To investigate the correlation between IGF2 protein levels and various aspects of male infertility,specifically focusing on sperm quality,inflammation,and DNA damage,a cohort of 320 male participants was recruited from the Women’s Hospital,Zhejiang University School of Medicine(Hangzhou,China)between 1^(st) January 2024 and 1^(st) March 2024.The relationship between IGF2 protein concentrations and sperm parameters was assessed,and Spearman correlation and linear regression analysis were employed to evaluate the independent associations between IGF2 protein levels and risk factors for infertility.Enzyme-linked immunosorbent assay(ELISA)was used to measure IGF2 protein levels in seminal plasma,alongside markers of inflammation(tumor necrosis factor-alpha[TNF-α]and interleukin-1β[IL-1β]).The relationship between seminal plasma IGF2 protein levels and DNA damage marker phosphorylated histone H2AX(γ-H2AX)was also explored.Our findings reveal that IGF2 protein expression decreased notably in patients with asthenospermia and teratospermia.Correlation analysis revealed nuanced associations between IGF2 protein levels and specific sperm parameters,and low IGF2 protein concentrations correlated with increased inflammation and DNA damage in sperm.The observed correlations between IGF2 protein levels and specific sperm parameters,along with its connection to inflammation and DNA damage,underscore the importance of IGF2 in the broader context of male reproductive health.These findings lay the groundwork for future research and potential therapeutic interventions targeting IGF2-related pathways to enhance male fertility. 展开更多

麒麟丸调控CYP11A1/testosterone/CYP19A1/E2/ESR1通路治疗雷公藤多苷诱导的大鼠少弱精子症 被引量:3

目的探究麒麟丸治疗少弱精子症(AS)的作用机制。方法SD大鼠随机分为对照组、模型组、左卡尼汀(阳性药,100 mg·kg^(-1))和麒麟丸低、高剂量(0.5、1.5 mg·kg^(-1))组,除对照组外,其余4组ig给予雷公藤多苷35 mg·kg^(-1)7 d... 目的探究麒麟丸治疗少弱精子症(AS)的作用机制。方法SD大鼠随机分为对照组、模型组、左卡尼汀(阳性药,100 mg·kg^(-1))和麒麟丸低、高剂量(0.5、1.5 mg·kg^(-1))组,除对照组外,其余4组ig给予雷公藤多苷35 mg·kg^(-1)7 d诱导AS模型,从造模开始即给药,每天1次,共ig给药21 d。称量睾丸和附睾组织质量,计算大鼠生殖器官指数;通过精子参数检测系统检测各组动物的精子质量和精子形态;ELISA测定大鼠血清中性激素含量;HE染色观察给药后各组大鼠睾丸组织病理学改变,并统计生精细胞和间质细胞数量;通过免疫荧光染色检测大鼠睾丸组织中支持细胞数量;Western blotting和实时荧光定量PCR(qRT-PCR)法检测细胞色素P450家族19亚家族A成员1(CYP19A1)、细胞色素P450家族11亚家族A成员1(CYP11A1)和雌激素受体α(ESR1)的蛋白和基因表达水平。结果与对照组相比,模型组精子质量和血清睾酮(T)、雌二醇(E2)含量显著降低(P<0.01),血清黄体生成素(LH)含量显著升高(P<0.01),生殖器官指数和睾丸生精细胞、间质细胞和支持细胞数量显著降低(P<0.01),睾丸组织性激素合成限速酶CYP11A1和CYP19A1、ESR1的基因和蛋白显著下调(P<0.01);与模型组相比,麒麟丸高剂量组精子质量和血清T、E2含量显著增加(P<0.01),血清LH含量显著降低(P<0.01),生殖器官指数、睾丸生精细胞、间质细胞和支持细胞数量显著升高(P<0.01),CYP11A1和CYP19A1、ESR1基因和蛋白显著上调(P<0.01)。结论麒麟丸可通过上调性激素合成限速酶CYP11A1和CYP19A1,上调体内性激素表达治疗AS。 展开更多

目的 研究精索静脉曲张(VC)伴弱精子症(AST)不育患者应用益肾通络方联合显微手术治疗对其中医证候积分、精子活性、精浆细胞因子水平、睾丸微循环的影响。方法 经纳入、排除标准排除后纳入河南省中医院(河南中医药大学第二附属医院)2022... 目的 研究精索静脉曲张(VC)伴弱精子症(AST)不育患者应用益肾通络方联合显微手术治疗对其中医证候积分、精子活性、精浆细胞因子水平、睾丸微循环的影响。方法 经纳入、排除标准排除后纳入河南省中医院(河南中医药大学第二附属医院)2022年8月—2023年8月收治的120例VC伴AST不育患者,并将其分为进行采用益肾通络方治疗的A组、采用显微手术治疗的B组及两者联合治疗的A+B组,分组方法为随机抽签法,每组40例,由于存在期间更换治疗方案、中途退出该研究等情况最终纳入例数分别为38例、39例、39例。所有患者均治疗12周。统计3组治疗12周后的临床疗效,比较3组治疗前及治疗12周后的中医证候积分、精子活性[前向运动精子百分率、正常形态精子率、精子直线运动速度、精子脱氧核糖核酸(DNA)完整率]、精浆细胞因子水平[精浆一氧化氮(NO)、活性氧(ROS)、超氧化物歧化酶(SOD)水平]、睾丸微循环[左侧睾丸包膜动脉收缩期峰值流速(PSV)、搏动指数(PI)、阻力指数(RI)]。结果 治疗12周后,A+B组总有效率高于A组和B组(P<0.05)。3组治疗12周后中医证候各项评分及精浆NO、ROS水平、左侧睾丸包膜动脉PSV、PI、RI与治疗前比较,降低,A+B组低于A组、B组(P<0.05);精子DNA完整性、前向运动精子百分率、精子直线运动速度、正常形态精子率及与精浆SOD水平治疗前比较,升高,A+B组高于A组、B组(P<0.05)。结论 益肾通络方联合显微手术治疗VC伴AST不育患者,可有效改善其中医证候,提高精子活性,改善精浆细胞因子水平及睾丸微循环,具有较好的临床疗效。 展开更多

目的:探讨“秩边透水道”针法调控铁死亡途径改善弱精子症小鼠生殖功能的可能机制。方法:将60只雄性C57BL/6小鼠随机分成空白组、模型组、针刺组和Fer-1组,每组15只。除空白组外,其余3组小鼠给予环磷酰胺(50 mg·kg^(-1)·d^(-... 目的:探讨“秩边透水道”针法调控铁死亡途径改善弱精子症小鼠生殖功能的可能机制。方法:将60只雄性C57BL/6小鼠随机分成空白组、模型组、针刺组和Fer-1组,每组15只。除空白组外,其余3组小鼠给予环磷酰胺(50 mg·kg^(-1)·d^(-1))腹腔注射诱导弱精子症模型,连续5 d。针刺组小鼠给予“秩边透水道”针法干预,每次20 min;Fer-1组给予铁死亡抑制剂Fer-1溶液(1 mg/kg)腹腔注射。两组均每日1次,连续干预2周。称量各组小鼠睾丸湿重并计算睾丸系数,采用精子质量检测系统检测小鼠精子质量;ELISA法检测小鼠血清睾酮(T)、卵泡刺激素(FSH)、黄体生成激素(LH)含量;HE染色法观察小鼠睾丸和附睾形态;免疫荧光法检测小鼠睾丸活性氧(ROS)表达水平;生化法检测小鼠睾丸组织丙二醛(MDA)、还原型谷胱甘肽(GSH)和总铁离子(TFe)含量;透射电镜观察小鼠睾丸组织线粒体结构;JC-1染色检测小鼠睾丸组织线粒体膜电位水平;实时荧光定量PCR法和Western blot法检测小鼠睾丸组织溶质转运蛋白家族7成员11(SLC7A11)、谷胱甘肽过氧化物酶4(GPX4)、铁蛋白重链1(FTH1)、长链脂酰-Co A合成酶4(ACSL4)mRNA和蛋白表达。结果:与空白组比较,模型组小鼠睾丸湿重和睾丸系数降低(P<0.01);精子浓度和精子活力降低(P<0.01);血清T、FSH和LH含量降低(P<0.01);睾丸内生精小管结构疏松、管腔变形,精子细胞排列紊乱、精子数量减少;附睾小管管壁变薄,管腔内精子数量较少;睾丸组织ROS表达水平及MDA、TFe含量升高(P<0.01),GSH含量降低(P<0.01);线粒体数量减少,嵴结构严重破坏;线粒体膜电位水平降低(P<0.01);睾丸组织SLC7A11、GPX4、FTH1mRNA和蛋白表达降低(P<0.01),ACSL4 mRNA和蛋白表达升高(P<0.01)。与模型组比较,针刺组和Fer-1组小鼠睾丸湿重和睾丸系数升高(P<0.01);精子浓度和精子活力升高(P<0.01);血清T、FSH和LH含量升高(P<0.01);睾丸和附睾组织病理结构改善;睾丸组织ROS表达水平及MDA、TFe含量降低(P<0.01),GSH含量升高(P<0.05);线粒体形态结构和数量改善;线粒体膜电位水平升高(P<0.01)。与模型组比较,针刺组和Fer-1组小鼠睾丸组织SLC7A11 mRNA表达升高(P<0.05,P<0.01),GPX4、FTH1 mRNA和蛋白表达升高(P<0.01,P<0.05),ACSL4mRNA和蛋白表达水平降低(P<0.01);Fer-1组小鼠睾丸组织SLC7A11蛋白表达升高(P<0.05)。结论:“秩边透水道”针法可能通过减轻铁死亡引发的细胞损伤,改善睾丸功能,从而提升弱精子症小鼠的生殖能力。 展开更多

九子回春方联合健脾灸治疗脾肾两虚型弱精子不育症的临床疗效

目的:观察九子回春方联合健脾灸对脾肾两虚型弱精子不育症患者的有效性和安全性。方法:纳入上海市第七人民医院脾肾两虚型弱精子症患者82例,随机分为观察组和对照组,每组各41例。对照组口服五子衍宗丸联合健脾灸,五子衍宗丸每日1剂,2次... 目的:观察九子回春方联合健脾灸对脾肾两虚型弱精子不育症患者的有效性和安全性。方法:纳入上海市第七人民医院脾肾两虚型弱精子症患者82例,随机分为观察组和对照组,每组各41例。对照组口服五子衍宗丸联合健脾灸,五子衍宗丸每日1剂,2次/d,每次1袋,健脾灸每周1次;观察组口服九子回春方联合健脾灸,九子回春方每日2次,早、晚各服用1次,每次200 mL,健脾灸每周1次。两组患者疗程均为12周,并随访12周。治疗期间脱失及剔除12例,最终可供评估病例70例,对照组35例,观察组35例。记录6个月内患者配偶受孕情况;进行治疗前后脾肾两虚型中医证候评分;检测两组患者治疗前与治疗后精液量、精液常规参数、精子正常形态、精子DNA碎片指数、精浆果糖、精浆酸性磷酸酶与精浆α-葡糖苷酶水平;检测两组患者治疗前后肝、肾功能相关安全性指标。结果:在6个月的观察期间,与本组治疗前比较,观察组与对照组患者治疗后精液量有所升高,但差异无统计学意义,精子浓度、精子活率、a+b级精子比例、精子正常形态、精浆果糖、精浆酸性磷酸酶、精浆α-葡糖苷酶、a级精子比例、直线精子活率、直线精子浓度、直线精子数均明显升高(P<0.05),精子DNA碎片指数、脾肾两虚型中医证候评分均明显降低(P<0.05)。与对照组治疗后比较,观察组临床疗效优于对照组(Z=-2.276,P<0.05),观察组配偶受孕率为14.3%,高于对照组的2.9%,但差异无统计学意义,观察组精子活率、a+b级精子比例、精浆果糖、a级精子比例、正常精子形态、α-葡糖苷酶、直线精子活率均高于对照组(P<0.05),观察组精子DNA碎片指数、脾肾两虚型中医证候评分均低于对照组(P<0.01)。二组治疗中未见严重不良反应,治疗后安全性指标未见异常。结论:九子回春方联合健脾灸用于脾肾两虚型男性弱精不育症患者可提升精子活率与精子浓度、改善脾肾两虚型弱精子症中医相关证候及精子形态、降低精子DNA碎片指数、并可调节精浆生物酶水平,值得在临床实践中进一步推广及应用。 展开更多

目的:回顾分析1例精子鞭毛多发形态异常(MMAF)患者的临床特点及遗传学病因。方法:选取在宁波大学附属第一医院生殖中心就诊的1例严重弱精子症患者作为研究对象,收集其临床资料与检查结果。采用高通量测序以及生物信息学等方法分析其遗... 目的:回顾分析1例精子鞭毛多发形态异常(MMAF)患者的临床特点及遗传学病因。方法:选取在宁波大学附属第一医院生殖中心就诊的1例严重弱精子症患者作为研究对象,收集其临床资料与检查结果。采用高通量测序以及生物信息学等方法分析其遗传学病因,使用Sanger测序对其家系进行验证。同时使用透射电镜观察精子超微结构并使用免疫荧光分析FSIP2蛋白在精子中的定位。结果:患者精子形态表现为MMAF,透射电镜显示患者精子存在纤维鞘及9+2微管结构紊乱;测序发现其FSIP2复合杂合变异(c.17798C>T,c.5927T>G),分别遗传自其父亲、母亲。根据美国医学遗传学与基因组学学会相关指南,患者的变异为致病性变异。患者配偶通过卵细胞胞质内单精子注射助孕获得1个胚胎,移植后未临床妊娠。结论:本研究报道了1例精子鞭毛多发形态异常患者FSIP2基因c.17798C>T,c.5927T>G变异。上述发现拓宽了FSIP2基因的变异谱,为精子鞭毛多发形态异常患者的遗传及辅助生殖咨询提供了思路。 展开更多

目的探究六味地黄丸对弱精子症(Asthenospermia,AZS)大鼠睾丸组织磷酸肌醇3激酶(Phosphoinositol 3 kinase,PI3K)-蛋白激酶B(Protein kinase B,AKT)-哺乳动物雷帕霉素靶蛋白(Mammalian target of rapamycin,mTOR)通路的影响。方法将30... 目的探究六味地黄丸对弱精子症(Asthenospermia,AZS)大鼠睾丸组织磷酸肌醇3激酶(Phosphoinositol 3 kinase,PI3K)-蛋白激酶B(Protein kinase B,AKT)-哺乳动物雷帕霉素靶蛋白(Mammalian target of rapamycin,mTOR)通路的影响。方法将30只SD大鼠连续28 d灌胃20 mg/(kg·d)雷公藤多苷(Tripterygium glycosides,TGS)悬浮液,构建AZS大鼠模型,按随机数字表法分为弱精子症组、六味地黄丸组[157.5 mg/(kg·d)连续灌胃4周]及六味地黄丸+PI3K抑制剂组(六味地黄丸给药第四周开始连续一周腹腔注射40μl/d LY294002),每组各10只。另选10只SD大鼠作为对照组。检测精子参数;苏木精及伊红(Hematoxylin and eosin,H&E)染色观察AZS大鼠睾丸的病理状况;使用比色法和酶联免疫吸附测定法(Enzyme linked immunosorbent assay,ELISA)测定睾丸组织中的丙二醛(Malondialdehyde,MDA)、8-羟基脱氧鸟苷(8-hydroxydeoxyguanosine,8-OHdG)、总抗氧化能力(Total antioxidant capacity,T-AOC)和总超氧化物歧化酶(Total superoxide dismutase,T-SOD);ELISA检测血清促卵泡激素(Follicle-stimulating hormone,FSH)、促黄体生成素(Luteinizing hormone,LH)、睾酮(Testosterone,T)、雌二醇(Estradiol,E_(2));Western blot评估PI3K-AKT-mTOR通路蛋白水平。结果与对照组比较,弱精子症组大鼠精子浓度、精子活力及精子活动性均降低(P<0.05),血清FSH、LH水平均升高(P<0.05),T、E_(2)水平明显降低(P<0.05),睾丸组织MDA、8-OHdG水平均升高(P<0.05),T-SOD、T-AOC、PI3K、p-AKT/AKT及p-mTOR/mTOR均降低(P<0.05);与弱精子症组比较,六味地黄丸组大鼠精子浓度、精子活力及精子活动性明显升高(P<0.05),血清FSH、LH水平均降低(P<0.05),T、E_(2)水平升高(P<0.05),睾丸组织病理损伤减轻,MDA、8-OHdG水平均降低(P<0.05),T-SOD、T-AOC、PI3K、p-AKT/AKT及p-mTOR/mTOR水平升高(P<0.05);与六味地黄丸组比较,六味地黄丸+PI3K抑制剂组大鼠精子浓度、精子活力及精子活动性明显降低(P<0.05),血清FSH、LH水平均升高(P<0.05),T、E_(2)水平明显降低(P<0.05),睾丸组织MDA、8-OHdG水平均升高(P<0.05),T-SOD、T-AOC、PI3K、p-AKT/AKT及p-mTOR/mTOR水平降低(P<0.05)。结论六味地黄丸通过激活PI3K-AKT-mTOR信号通路,上调AZS大鼠雄性激素,抑制氧化应激,改善AZS大鼠精子质量和生殖功能。 展开更多

火土既济方对脾肾阳虚型弱精子症患者的临床疗效观察

目的:观察火土既济方对脾肾阳虚型弱精子症患者的有效性及安全性。方法:采用随机对照临床试验研究方法将脾肾阳虚型弱精子症患者分为试验组和对照组。试验组给予火土既济方,对照组给予左卡尼汀口服溶液,比较两组治疗前后精液量、精子浓... 目的:观察火土既济方对脾肾阳虚型弱精子症患者的有效性及安全性。方法:采用随机对照临床试验研究方法将脾肾阳虚型弱精子症患者分为试验组和对照组。试验组给予火土既济方,对照组给予左卡尼汀口服溶液,比较两组治疗前后精液量、精子浓度、前向运动精子(PR)百分率、中医症状评分变化及配偶妊娠情况。结果:共纳入115例患者,试验组55例,对照组60例。试验组在治疗前和治疗后3个月精子浓度分别为(160.71±53.7)×10^(6)/mL、(187.19±41.89)×10^(6)/mL,PR百分率分别为(20.37±9.42)%、(26.7±6.92)%,PR+非前向运动精子(NP)百分率分别为(32.06±8.49)%、(34.89±7.25)%。治疗后两组精子活力较治疗前有显著改善(P<0.05),试验组精子浓度较治疗前也有显著增加(P<0.05)。与对照组比较,试验组患者精子浓度及精子活力的改善情况更为显著(P<0.05)。对照组患者配偶妊娠率为11.67%,试验组患者配偶妊娠率为29.09%,差异有统计学意义(P<0.05)。对照组治疗12周后与治疗前比较中医症状评分下降不显著(P>0.05),试验组治疗12周后与治疗前比较中医症状评分下降显著(P<0.05)。与对照组相比,试验组治疗后中医症状评分显著降低(P<0.05)。结论:火土既济方可安全有效地提高脾肾阳虚型弱精子症患者的精子浓度和活力,改善患者症状,提高其配偶妊娠率,值得临床推广。 展开更多
