BACKGROUND Gastric cancer presenting with peritoneal metastasis is notably associated with diminished survival prospects.The use of cytoreductive surgery in conjunction with hyperthermic intraperitoneal chemotherapy(H...BACKGROUND Gastric cancer presenting with peritoneal metastasis is notably associated with diminished survival prospects.The use of cytoreductive surgery in conjunction with hyperthermic intraperitoneal chemotherapy(HIPEC)has been shown to increase survival rates in these patients.Despite these advancements,debates persist regarding the magnitude of survival improvement attributed to this treatment modality.The present investigation examined survival outcomes following HIPEC in individuals diagnosed with gastric cancer and peritoneal metastasis,and it took a comparative analysis of patients exhibiting positive and negative cytological findings.Between April 2013 and March 2020,84 patients with advanced gastric cancer treated at our institution were categorized into three cohorts:HIPEC(20 patients with peritoneal metastasis),cytology-positive(23 patients without peritoneal nodules but with positive wash cytology),and cytology-negative(41 patients with advanced gastric cancer,no peritoneal nodules,and negative wash cytology).The HIPEC cohort underwent gastrectomy with HIPEC,while the cytology-positive and cytology-negative groups received gastrectomy alone.The demographic,pat-hological,and survival data of the groups were compared.RESULTS The HIPEC cohort-predominantly younger females-exhibited relatively extended surgical durations and high blood loss.Nevertheless,the complication rates were consistent across all three groups.Median survival in the HIPEC group was 20.00±4.89 months,with 1-year,2-year,and 3-year overall survival rates of 73.90%,28.70%,and 9.60%,respectively.These figures paralleled the survival rates of the cytology-positive group(52.20%at 1 year,28.50%at 2 years,and 19.00%at 3 years).Notably,47%of patients experienced peritoneal recurrence.CONCLUSION HIPEC may offer a modest improvement in short-term survival for patients with gastric cancer and peritoneal metastasis,mirroring the outcomes in cytology-positive patients.However,peritoneal recurrence remained high.展开更多

However, there is a lack of evidence on pathogenic mechanism and signature ...Objective: Positive peritoneal lavege cytology(CY1) gastric cancer is featured by dismal prognosis, with high risks of peritoneal metastasis. However, there is a lack of evidence on pathogenic mechanism and signature of CY1and there is a continuous debate on CY1 therapy. Therefore, exploring the mechanism of CY1 is crucial for treatment strategies and targets for CY1 gastric cancer.Methods: In order to figure out specific driver genes and marker genes of CY1 gastric cancer, and ultimately offer clues for potential marker and risk assessment of CY1, 17 cytology-positive gastric cancer patients and 31matched cytology-negative gastric cancer patients were enrolled in this study. The enrollment criteria were based on the results of diagnostic laparoscopy staging and cytology inspection of exfoliated cells. Whole exome sequencing was then performed on tumor samples to evaluate genomic characterization of cytology-positive gastric cancer.Results: Least absolute shrinkage and selection operator(LASSO) algorithm identified 43 cytology-positive marker genes, while Mut Sig CV identified 42 cytology-positive specific driver genes. CD3G and CDKL2 were both driver and marker genes of CY1. Regarding mutational signatures, driver gene mutation and tumor subclone architecture, no significant differences were observed between CY1 and negative peritoneal lavege cytology(CY0).Conclusions: There might not be distinct differences between CY1 and CY0, and CY1 might represent the progression of CY0 gastric cancer rather than constituting an independent subtype. This genomic analysis will thus provide key molecular insights into CY1, which may have a direct effect on treatment recommendations for CY1and CY0 patients, and provides opportunities for genome-guided clinical trials and drug development.展开更多

The aim of...Cervical cancer is the fourth most common cancer worldwide, accounting for 6.8% of new cancer cases and 8.1% of cancer-related deaths. About 85% of these deaths occurred in low- and middle-income countries. The aim of this study was to assess the frequency and distribution of the human papillomavirus (HPV) genotypes in women showing cytological abnormalities of the cervix at the Sourô SANOU University Hospital (CHUSS) in Bobo-Dioulasso, Burkina Faso. This is a descriptive study of women recruited at the CHUSS. The cervico-uterine smear examination was carried out at the CHUSS Anatomy and Pathology Department for cervical cancer screening. The data were collected from women with atypical cells on their cervico-uterine smear. Cervicovaginal samples were taken from consenting women and HPV genotyping was performed using the HPV Direct FLOW CHIP kit at CERBA. We obtained approval from the ethics committee. The data were analyzed using the SPSS 26 software. The results of the study showed that 67.79% of the participants were aged between 50 and 65, a group that is particularly vulnerable to persistent infection with high-risk oncogenic HPV genotypes. Of the women screened, 40.7% were HPV positive and 29.2% had multiple infections. The most common genotypes were HPV 35, followed by HPV 18, 52, 58 and 66. These data highlight the need for increased surveillance and targeted prevention strategies among this female population.展开更多
Examining thyroid fine-needle aspiration(FNA)can grade cancer risks,derive prognostic information,and guide follow-up care or surgery.The digitization of biopsy and deep learning techniques has recently enabled comput...Examining thyroid fine-needle aspiration(FNA)can grade cancer risks,derive prognostic information,and guide follow-up care or surgery.The digitization of biopsy and deep learning techniques has recently enabled computational pathology.However,there is still lack of systematic diagnostic system for the complicated gigapixel cytopathology images,which can match physician-level basic perception.In this study,we design a deep learning framework,thyroid segmentation and hierarchy fine-needle aspiration(TshFNA)-Examiner to quantitatively profile the cancer risk of a thyroid FNA image.In the TshFNA-Examiner,cellular-intensive areas strongly correlated with diagnostic medical information are detected by a nuclei segmentation neural network;cell-level image patches are catalogued following The Bethesda System for Reporting Thyroid Cytopathology(TBSRTC)system,by a classification neural network which is further enhanced by leveraging unlabeled data.A cohort of 333 thyroid FNA cases collected from 2019 to 2022 from I to VI is studied,with pixel-wise and image-wise image patches annotated.Empirically,TshFNA-Examiner is evaluated with comprehensive metrics and multiple tasks to demonstrate its superiority to state-of-the-art deep learning approaches.The average performance of cellular area segmentation achieves a Dice of 0.931 and Jaccard index of 0.871.The cancer risk classifier achieves a macro-F1-score of 0.959,macro-AUC of 0.998,and accuracy of 0.959 following TBSRTC.The corresponding metrics can be enhanced to a macro-F1-score of 0.970,macro-AUC of 0.999,and accuracy of 0.970 by leveraging informative unlabeled data.In clinical practice,TshFNA-Examiner can help cytologists to visualize the output of deep learning networks in a convenient way to facilitate making the final decision.展开更多
Although metabolic homeostasis disruption,cellular damage,and premature senescence caused by salinity stress are well-documented in the literature,there are few studies investigating cytological changes induced by sal...Although metabolic homeostasis disruption,cellular damage,and premature senescence caused by salinity stress are well-documented in the literature,there are few studies investigating cytological changes induced by salinity stress within the altered metabolic landscape of rice,and this study aims to fill that gap.The cytological characterization of root tips(in terms of mitotic index and chromosomal abnormalities such as stickiness,laggards,fragments,bridges,micronuclei,ring chromosomes,and total mitotic abnormalities)was conducted on 10 experimental rice landraces from coastal Bangladesh,grown under post-imbibitional salinity stress(PISS),while correlating these changes with their metabolic status.The results revealed a strong correlation between salinity-induced cytological changes in root cells(mitotic index and chromosomal abnormalities)and the redox interactome status of all experimental rice landraces.The landraces Kutepatnai,Talmugur,Nonakochi,and Benapol,which exhibited a higher ability to mitigate PISS-induced chromosomal abnormalities and improve mitotic index,also showed lower accumulation of oxidative stress markers(protein carbonylation,lipid peroxidation,prooxidant accumulation,oxidative stress index,reactive oxygen species(ROS)-antioxidative stress index,and efficiency of ROS processing via the Halliwell-Asada pathway)compared with more susceptible landraces(Charobalam,Jotaibalam,Kachra,and Lalmota).These findings underscore the role of redox biology in preventing chromotoxic effects under salinity stress.Hierarchical cluster analysis and principal component analysis,used to determine variations and similarities among the experimental rice landraces based on cytological attributes,redox interactome,and physiological phenotypes,classified the landraces according to their salinity tolerance and sensitivity.This study proposes a novel approach for exploring redox-regulated cytological fingerprints as a tool for identifying salinity-tolerant rice landraces.展开更多
