目的分析急性一氧化氮中毒(Acute nitric oxide,NO)患者心肌肌钙蛋白1(Cardiac troponin I,cTnI)、淀粉酶(Amylase,AMS)、中性粒细胞/淋巴细胞比值(Neutrophil lymphocyte ratio,NLR)、涎液化糖链抗原(Krebs von den lungen-6,KL-6)水...目的分析急性一氧化氮中毒(Acute nitric oxide,NO)患者心肌肌钙蛋白1(Cardiac troponin I,cTnI)、淀粉酶(Amylase,AMS)、中性粒细胞/淋巴细胞比值(Neutrophil lymphocyte ratio,NLR)、涎液化糖链抗原(Krebs von den lungen-6,KL-6)水平变化及预后评估价值。方法选取大兴区人民医院急诊科2020年3月-2022年7月收治的97例急性NO中毒患者,治疗前,使用全自动免疫透射比浊法检测cTnⅠ,酶联免疫吸附法检测AMS,并用全自动血液细胞分析仪检测NLR。给予患者特效解毒剂或拮抗剂,结合患者中毒情况给予氧疗、支气管扩张剂、高压氧、糖皮质激素等治疗。采用电话随访30 d内预后情况,将其分为预后良好组和预后不良组,对比两组患者基础资料和血清学指标,分析cTnI、AMS、NLR影响急性NO中毒患者预后的因素及预测价值。结果97例急性NO中毒患者,预后不良患者有22例(22.68%)。预后不良组中毒程度为重度、涎液化糖链抗原(Krebs von den lungen-6,KL-6)、AMS、cTnⅠ、NLR水平高于预后良好组(P<0.05)。Logistic回归分析显示,中毒程度为重度[OR=3.401(95%CI:1.162~9.950)]、AMS[OR=3.904(95%CI:1.334~11.422)]、cTnⅠ[OR=4.035(95%CI:1.379~11.805)]、NLR[OR=4.559(95%CI:1.558~13.337)]、KL-6[OR=3.104(95%CI:2.602~22.277)]是急性NO中毒患者预后不良的影响因素(P<0.05)。ROC曲线分析发现,AMS、cTnⅠ、NLR、KL-6联合预测急性NO中毒患者预后不良的灵敏度、特异度、曲线下面积(Area under curve,AUC)均高于各指标单一预测(P均<0.05)。结论AMS、cTnⅠ、NLR、KL-6水平变化与急性NO中毒患者预后不良相关,联合检测能够早期预测和改善急性NO中毒患者预后不良。展开更多
