Objective To investigate the associations between eight serum per-and polyfluoroalkyl substances(PFASs)and regional fat depots,we analyzed the data from the National Health and Nutrition Examination Survey(NHANES)2011...Objective To investigate the associations between eight serum per-and polyfluoroalkyl substances(PFASs)and regional fat depots,we analyzed the data from the National Health and Nutrition Examination Survey(NHANES)2011-2018 cycles.Methods Multiple linear regression models were developed to explore the associations between serum PFAS concentrations and six fat compositions along with a fat distribution score created by summing the concentrations of the six fat compositions.The associations between structurally grouped PFASs and fat distribution were assessed,and a prediction model was developed to estimate the ability of PFAS exposure to predict obesity risk.Results Among females aged 39-59 years,trunk fat mass was positively associated with perfluorooctane sulfonate(PFOS).Higher concentrations of PFOS,perfluorohexane sulfonate(PFHxS),perfluorodecanoate(PFDeA),perfluorononanoate(PFNA),and n-perfluorooctanoate(n-PFOA)were linked to greater visceral adipose tissue in this group.In men,exposure to total perfluoroalkane sulfonates(PFSAs)and long-chain PFSAs was associated with reductions in abdominal fat,while higher abdominal fat in women aged 39-59 years was associated with short-chain PFSAs.The prediction model demonstrated high accuracy,with an area under the curve(AUC)of 0.9925 for predicting obesity risk.Conclusion PFAS exposure is associated with regional fat distribution,with varying effects based on age,sex,and PFAS structure.The findings highlight the potential role of PFAS exposure in influencing fat depots and obesity risk,with significant implications for public health.The prediction model provides a highly accurate tool for assessing obesity risk related to PFAS exposure.展开更多

The hybrid atomistic structure-based model has been validated to be effective in investigation of G-quadruplex folding.In this study,we performed large-scale conventional all-atom simulations to complement the folding...The hybrid atomistic structure-based model has been validated to be effective in investigation of G-quadruplex folding.In this study,we performed large-scale conventional all-atom simulations to complement the folding mechanism of human telomeric sequence Htel24 revealed by a multi-basin hybrid atomistic structure-based model.Firstly,the real time-scale of folding rate,which cannot be obtained from the structure-based simulations,was estimated directly by constructing a Markov state model.The results show that Htel24 may fold as fast as on the order of milliseconds when only considering the competition between the hybrid-1 and hybrid-2 G-quadruplex conformations.Secondly,in comparison with the results of structure-based simulations,more metastable states were identified to participate in the formation of hybrid-1 and hybrid-2 conformations.These findings suggest that coupling the hybrid atomistic structure-based model and the conventional all-atom model can provide more insights into the folding dynamics of DNA G-quadruplex.As a result,the multiscale computational framework adopted in this study may be useful to study complex processes of biomolecules involving large conformational changes.展开更多

It is highly activated in cervical cancer, which continues to pose an important clinical challenge with an urgent need...The mammalian target of rapamycin(mTOR) is a critical component of the PI3K-AKT signaling pathway. It is highly activated in cervical cancer, which continues to pose an important clinical challenge with an urgent need for new and improved therapeutic approaches. Herein, we describe the structure-based drug discovery and biological evaluation of a series of m TOR kinase inhibitors as potential anti-cervical cancer agents. The results of enzymatic activity assays supported C3 as a potential m TOR inhibitor, which exhibited high inhibitory activity with an IC50 of 1.57 μM. Molecular docking and dynamics simulation were conducted to predict the binding patterns, suggesting relationships between structure and activity. The anti-proliferative assay against diverse cancer cell lines was displayed subsequently, revealing that C3 exhibited significant proliferation inhibition against cervical cancer cell He La(IC50=0.38μM) compared with other cell lines. Moreover, C3 could effectively reduce the expression of phospho-ribosomal S6 protein(p-S6) in He La cells in a dose-dependent manner. Noteworthily, m TOR signaling and other cellular pathways might contribute to the significant effect of C3 against cervical cancer simultaneously. These data indicated that C3 represented a good lead molecule for further development as a therapeutic agent for cervical cancer treatment.展开更多

Clinicians use these tools to rapidly consult the evidence at the point of ca...Background: Clinical decision support tools provide suggestions to support healthcare providers and clinicians, as they attend to patients. Clinicians use these tools to rapidly consult the evidence at the point of care, a practice which has been found to reduce the time patients spend in hospitals, promote the quality of care and improve healthcare outcomes. Such tools include Medscape, VisualDx, Clinical Key, DynaMed, BMJ Best Practice and UpToDate. However, use of such tools has not yet been fully embraced in low-resource settings such as Uganda. Objective: This paper intends to collate data on the use and uptake of one such tool, UpToDate, which was provided at no cost to five medical schools in Uganda. Methods: Free access to UpToDate was granted through the IP addresses of five medical schools in Uganda in collaboration with Better Evidence at The Global Health Delivery Project at Harvard and Brigham and Women’s Hospital and Wolters Kluwer Health. Following the donation, medical librarians in the respective institutions conducted training sessions and created awareness of the tool. Usage data was aggregated, based on logins and content views, presented and analyzed using Excel tables and graphs. Results: The data shows similar trends in increased usage over the period of August 2022 to August 2023 across the five medical schools. The most common topics viewed, mode of access (using either the computer or the mobile app), total usage by institution, ratio of uses to eligible users by institution and ratio of uses to students by institution are shared. Conclusion: The study revealed that the tool was used by various user categories across the institutions with similar steady improved usage over the year. These results can inform the librarians as they encourage their respective institutions to continue using the tool to support uptake of point-of-care tools in clinical practice.展开更多

Objective:This study aimed to evaluate the effectiveness of an integrative educational intervention-combining hospital-based schooling programs,child-centered communication,and therapeutic play-on clinical,psychosocia...Objective:This study aimed to evaluate the effectiveness of an integrative educational intervention-combining hospital-based schooling programs,child-centered communication,and therapeutic play-on clinical,psychosocial,and educational outcomes among school-aged children with cancer.Materials and Methods:A quasi-experimental pre-and post-test control group design was conducted at the pediatric oncology unit of Rumah Sakit Islam Jemursari,Surabaya,Indonesia.Fifty-two hospitalized children aged 6-18 years were randomly assigned to an intervention group(n=26)or a control group(n=26).The intervention lasted 1 month and integrated hospital-based lessons aligned with the national curriculum,therapeutic play,and developmentally appropriate communication by trained staff.Outcomes included pain intensity(Wong-Baker FACES Scale),treatment adherence,quality of life(Pediatric Cancer Quality of Life-32[PCQL-32]),anxiety(Revised Children’s Manifest Anxiety Scale),and literacy-numeracy performance.Data were analyzed using paired t-tests,ANCOVA,and Cohen’s d,with significance set at P<0.05.Results:Compared with controls,the intervention group showed significant improvements in pain reduction(P<0.001),treatment adherence(P<0.001),and quality of life across all PCQL-32 domains(P<0.001).Anxiety levels decreased significantly(P<0.001),and academic performance in literacy and numeracy improved markedly(P<0.001).Conclusion:The integrative educational intervention effectively enhanced clinical recovery,psychosocial well-being,and academic continuity among hospitalized children with cancer,emphasizing the need to integrate hospital-based education and play-based communication into pediatric oncology care.展开更多

The development of interest can improve the efficiency and quality of students’learning activities.Previous research mainly explored professional interest,but less on subject interest.This study designed and validate...The development of interest can improve the efficiency and quality of students’learning activities.Previous research mainly explored professional interest,but less on subject interest.This study designed and validated a subject interest questionnaire for primary school students to assess their potential to predict academic performance and offer reference for educational assessment and counseling.A sample of 545 students from two primary schools in Beijing was selected.The questionnaire consisted of 138 questions involving eight major courses,such as Chinese,mathematics and English.Through analysis,questions with good discrimination and social desirability were screened out,and nine questions of each sub-scale were finally retained.According to the result,the reliability and validity of the questionnaire reached an acceptable level.Further analysis of exploratory and confirmatory factors revealed that the subject interest can be summarized into two dimensions:“humanities-related factor”and“mathematics-related factor”.The two dimensions are consistent with the“human and material dimensions”proposed by ACT.As a preliminary exploration,this study lays a foundation for further research on the relationship between subject interest and basic interest and provides a practical evaluation tool for primary education.展开更多

Clinicians use these tools to rapidly consult the evidence at the point of care, a practice which has been found to reduce the time patients spend in hospitals, promote the quality of care and improve healthcare outcomes. Such tools include Medscape, VisualDx, Clinical Key, DynaMed, BMJ Best Practice and UpToDate. However, use of such tools has not yet been fully embraced in low-resource settings such as Uganda. Objective: This paper intends to collate data on the use and uptake of one such tool, UpToDate, which was provided at no cost to five medical schools in Uganda. Methods: Free access to UpToDate was granted through the IP addresses of five medical schools in Uganda in collaboration with Better Evidence at The Global Health Delivery Project at Harvard and Brigham and Women’s Hospital and Wolters Kluwer Health. Following the donation, medical librarians in the respective institutions conducted training sessions and created awareness of the tool. Usage data was aggregated, based on logins and content views, presented and analyzed using Excel tables and graphs. Results: The data shows similar trends in increased usage over the period of August 2022 to August 2023 across the five medical schools. The most common topics viewed, mode of access (using either the computer or the mobile app), total usage by institution, ratio of uses to eligible users by institution and ratio of uses to students by institution are shared. Conclusion: The study revealed that the tool was used by various user categories across the institutions with similar steady improved usage over the year. These results can inform the librarians as they encourage their respective institutions to continue using the tool to support uptake of point-of-care tools in clinical practice.

文摘Objective:This study aimed to evaluate the effectiveness of an integrative educational intervention-combining hospital-based schooling programs,child-centered communication,and therapeutic play-on clinical,psychosocial,and educational outcomes among school-aged children with cancer.Materials and Methods:A quasi-experimental pre-and post-test control group design was conducted at the pediatric oncology unit of Rumah Sakit Islam Jemursari,Surabaya,Indonesia.Fifty-two hospitalized children aged 6-18 years were randomly assigned to an intervention group(n=26)or a control group(n=26).The intervention lasted 1 month and integrated hospital-based lessons aligned with the national curriculum,therapeutic play,and developmentally appropriate communication by trained staff.Outcomes included pain intensity(Wong-Baker FACES Scale),treatment adherence,quality of life(Pediatric Cancer Quality of Life-32[PCQL-32]),anxiety(Revised Children’s Manifest Anxiety Scale),and literacy-numeracy performance.Data were analyzed using paired t-tests,ANCOVA,and Cohen’s d,with significance set at P<0.05.Results:Compared with controls,the intervention group showed significant improvements in pain reduction(P<0.001),treatment adherence(P<0.001),and quality of life across all PCQL-32 domains(P<0.001).Anxiety levels decreased significantly(P<0.001),and academic performance in literacy and numeracy improved markedly(P<0.001).Conclusion:The integrative educational intervention effectively enhanced clinical recovery,psychosocial well-being,and academic continuity among hospitalized children with cancer,emphasizing the need to integrate hospital-based education and play-based communication into pediatric oncology care.

文摘The development of interest can improve the efficiency and quality of students’learning activities.Previous research mainly explored professional interest,but less on subject interest.This study designed and validated a subject interest questionnaire for primary school students to assess their potential to predict academic performance and offer reference for educational assessment and counseling.A sample of 545 students from two primary schools in Beijing was selected.The questionnaire consisted of 138 questions involving eight major courses,such as Chinese,mathematics and English.Through analysis,questions with good discrimination and social desirability were screened out,and nine questions of each sub-scale were finally retained.According to the result,the reliability and validity of the questionnaire reached an acceptable level.Further analysis of exploratory and confirmatory factors revealed that the subject interest can be summarized into two dimensions:“humanities-related factor”and“mathematics-related factor”.The two dimensions are consistent with the“human and material dimensions”proposed by ACT.As a preliminary exploration,this study lays a foundation for further research on the relationship between subject interest and basic interest and provides a practical evaluation tool for primary education.
