Patterns of utilization of antipsychotic drugs and direct medical costs among patients with schizophrenia in a tertiary care hospital

BACKGROUND Drug utilization research has an important role in assisting the healthcare administration to know,compute,and refine the prescription whose principal objective is to enable the rational use of drugs.Research in developing nations relating to the cost of treatment is scarce when compared with developed countries.Thus,the drug utilization research studies from developing nations are most needed,and their number has been growing.AIM To evaluate patterns of utilization of antipsychotic drugs and direct medical cost analysis in patients newly diagnosed with schizophrenia.METHODS The present study was observational in type and based on a retrospective cohort to evaluate patterns of utilization of antipsychotic drugs using World Health Organization(WHO)core prescribing indicators and anatomical therapeutic chemical/defined daily dose indicators.We also calculated direct medical costs for a period of 6 months.RESULTS This study has found that atypical antipsychotics are the mainstay of treatment for schizophrenia in every age group and subcategories of schizophrenia.The evaluation based on WHO prescribing indicators showed a low average number of drugs per prescription and low prescribing frequency of antipsychotics from the National List of Essential Medicines 2015 and the WHO Essential Medicines List 2019.The total mean drug cost of our study was 1396 Indian rupees.The total mean cost due to the investigation in our study was 1017.34 Indian rupees.Therefore,the total mean direct medical cost incurred on patients in our study was 4337.28 Indian rupees.CONCLUSION The information from the present study can be used for reviewing and updating treatment policy at the institutional level. 展开更多

Although several previous studies in Inner Mongolia examined the effects of ecological conservation on the delivery of ecosystem services, they were often limited in scope(few ecosystem services were assessed) and often suffered from confounding by spatial variation. In this study, we examined the impact of conservation measures(changes in grassland utilization patterns) on the provision of selected ecosystem services in three types of grasslands(meadow steppe in Hulun Buir, typical steppe in Xilin Gol, and semi-desert steppe in Ordos) in Inner Mongolia. We examined five utilization patterns: no use(natural grasslands), light use, moderate use, intensive use, and recovery sites(degraded sites protected from further use). Through household surveys and vegetation and soil surveys, we measured the differences in ecosystem services among the different grassland utilization patterns. We also identified spatial factors that confounded the quantification of ecosystem services in different types of grasslands. We found that light use generally provided high levels of ecosystem services in meadow steppe and typical steppe, with the main differences in the supporting ecosystem services. Surprisingly, we found no consistently positive impacts of strict conservation activities across the sites, since the results varied spatially and with respect to differences in the land-use patterns. Our study suggests that appropriate grassland utilization patterns can enhance the supply of ecosystem services and reduce negative effects on both household livelihoods and the environment. 展开更多

Exploring the utilization effect of water-land resources under the evolution of dietary patterns is of great significance in achieving sustainable global food consumption and the effective allocation of national resources.Our selected study area was China,a country with rapidly changing dietary consumption patterns,and the research period was between 1987 and 2020.Based on the material called Chinese Dietary Guidelines 2021,this study introduced the“virtual water”and the“virtual land”to quantify the utilization effect of water-land resources under the evolution of Chinese dietary patterns.Results showed that the dietary patterns gradually changed from“cereal-vegetable-based consumption”to“diversified consumption”.Food consumption’s total water footprint(WF)increased from 471.1 Gm3in 1987 to 848.8 Gm3in 2020,with a growth rate of 80.2%.Moreover,the total land requirement for food(LRF)increased from 88.8 Mha in 1987 to 129.9 Mha in 2020,with a growth rate of 46.3%.Furthermore,the meat consumption was the major contributor to the increase in total WF(104.0%)and LRF(102.1%).In contrast to the balanced diet pattern,there was no waste of water-land resources consumption for the food consumption of urban-rural residents in China between 1987 and 2020.However,the consumption of water resources would gradually approach the resource cost under the balanced diet patterns.It would eventually break through the critical value and reach the state of resource waste.In addition,the findings showed that urban residents’waste rate of water-land resources for meat consumption increased by 142.3%compared with that in 1987.The research results can provide scientific guidance for resolving the food crisis under the supply of water-land resources in China and have an essential reference for national food security and sustainable development of resources and environment. 展开更多

AIM: To assess advanced neuroendocrine tumor (NET) treatment patterns and resource utilization by tumor progression stage and tumor site in the United States. METHODS: United States Physicians meeting eligibility criteria were provided with online data extraction forms to collect patient chart data on recent NET patients. Resource utilization and treatment pattern data were collected over a baseline period (after diagnosis and before tumor progression), as well as initial and secondary progression periods, with progression defined according to measureable radiographic evidence of tumor progression. Resource categories used in the analysis include: Treatments (e.g. , surgery, chemotherapy, radiotherapy, targeted therapies), hospitalizations and physician visits, diagnostic tests (biomarkers, imaging, laboratory tests). Comparisons between categories of resource utilization and tumor progression status were examined using univariate (by tumor site) and multivariate analyses (across all tumor sites). RESULTS: Fifty-five physicians were included in the study and completed online data extraction forms using the charts of 110 patients. The physician sample showed a relatively even distribution for those affiliated with academic versus community hospitals (46% vs 55%). Forty (36.3%) patients were reported to have pancreatic NET (pNET), while 70 (63.6%) patients had gastrointestinal tract (GI)/Lung as the primary NET site. Univariate analysis showed the proportion of patients hospitalized increased from 32.7% during baseline to 42.1% in the progression stages. While surgeries were performed at similar proportions overall at baseline and progression, pNET patients, were more likely than GI/Lung NET patients to have undergone surgery during the baseline (33.3% vs 25.0%) and any progression periods (26.7% vs 23.4%). While peptide-receptor radionuclide and targeted therapy utilization was low across NET types and tumor stages, GI/Lung types exhibited greater utilization of these technologies compared to pNET. Chemotherapy utilization was also greater among GI/Lung types. Multivariate analysis results demonstrated that patients in first progression period were over 3 times more likely to receive chemotherapy when compared to baseline (odds ratio: 3.31; 95%CI: 1.46-7.48, P=0.0041). Further, progression was associated with a greater likelihood of having a study physician visit [relative risk (RR): 1.54; 95%CI: 1.10-2.17, P=0.0117], and an increased frequency of other physician visits (RR: 1.84; 95%CI: 1.10-3.10, P=0.0211). CONCLUSION: Resource utilization in advanced NET in the United States is significant overall and data suggests progression has an impact on resource utilization regardless of NET tumor site. 展开更多

Four rainwater diversion patterns including undisturbed soil ditch (T1), slope compaction (T2), asbestos sheet covering after compac- tion (T3) and plastic film covering after compaction (T4) were set in cornfield in arid areas of Yunnan Province, and effects of various patterns on rainwater collection efficiency, water utilization efficiency and corn yield were studied. The results shows that rainwater collection efficiency, water utilization efficiency and corn yield of the four diversion patterns presented the same trend, namely T4 〉T3 〉T2 〉T1. Overall, rainwater collection efficiencies of the four diversion patterns ranged from 41.7% to 58.7% ; compared with T1, water utilization efficiencies of T2 ,T3 and T4 increased by 34.9%, 53.5% and 92.5%, and maize yields of them improved by 1 767.0, 1 981.5 and 2 385.0 kg/hm2. 展开更多

Diabetes mellitus is the chronic disorder emerging as major world health problem which increases the rate of morbidity and mortality. The aim of the present study was to ascertain patterns of prescription of oral hypoglycemic agents to type 2 diabetic patients attending a university hospital, and to assess patient compliance. A prospective, observational and non comparative study was conducted in 200 established diabetes mellitus type 2 patients attending outpatient department at Majeedia Hospital, New Delhi, India. Prescriptions from registered patients were included in the study. Once the consultation by the physician was over, the prescriptions were reviewed and the patients were interviewed. The information was collected in an inhouse designed documentation proforma. In a pool of 200 type 2 diabetics, more than half were female (n=106, 53%). The mean age of the patients were found to be 50.4 ± 11.7 years and mean body mass index, 25.8 ± 4.4 kg/m2. A total of 432 oral hypoglycemic agents were prescribed to the patients. Seed dispersal is a fundamental process affecting destinies of seeds and seedlings, as well as regeneration dynamics and distribution patterns of communities. Recently however, declining Populus euphratica forests have failed to regenerate by seeding, while the spatial pattern and utility of their seed rains are unclear. In this study, we designed a model to simulate seed rains based on field investigations by fitting our observations to a normal distribution and testing the model with data from three years, with ac- ceptable results. We examined the simulated results of dispersed seeds patterns on the classification of three factors, i.e., density of Sophora alopecuroides, surface soil moisture and surface soil salinity. The results of seed rain simulation show that over 70% of seeds were dispersed and confined to each of the three plots where their mother trees located. The proportion of 3:7 seeds dispersed inside and outside each plots remained largely unaltered. The differences in the amounts of dispersed seed among the different levels of each of the factors were not significant, although the distributing pattern of P. euphratica in each plot varied. Therefore, in P. eu- phratica communities, the amount of seed does not become a constraint in subsequent germination, although the surface environment does. We conclude that successful P. euphratica seed regeneration relies on less than 30% of seeds dispersed over longer distance to colonize favorable growth habitats. 展开更多

The part of China,east of the Hu Huanyong Line,is commonly referred to as eastern China.It is characterized by a high population density and a well-developed economy;it also has huge energy demands.This study assesses and promotes the large-scale development of geothermal resources in eastern China by analyzing deep geological structures,geothermal regimes,and typical geothermal systems.These analyses are based on data collected from geotectology,deep geophysics,geothermics,structural geology,and petrology.Determining the distribution patterns of intermediate-to-deep geothermal resources in the region helps develop prospects for their exploitation and utilization.Eastern China hosts superimposed layers of rocks from three major,global tectonic domainsd namely Paleo-Asian,Circum-Pacific,and Tethyan rocks.The structure of its crust and mantle exhibits a special flyover pattern,with basins and mountains as well as well-spaced uplifts and depressions alternatively on top.The lithosphere in Northeast China and North China is characterized by a thin,low density crust and mantle,whereas the lithosphere in South China has a thin,low density crust and a thick,high density mantle.The middle and upper crust contain geobodies with high conductivity and low velocity,with varying degrees of development that create favorable conditions for the formation and enrichment of geothermal resources.Moderate-to-high temperature geothermal resources are distributed in the MesozoiceCenozoic basins in eastern China,although moderate temperature geothermal resources with low abundance dominate.Porous sandstone reservoirs,karstified fractured-vuggy carbonate reservoirs,and fissured granite reservoirs are the main types of geothermal reservoirs in this region.Under the currently available technical conditions,the exploitation and utilization of geothermal resources in eastern China favor direct utilization over large-scale geothermal power generation.In Northeast China and North China,geothermal resources could be applied for large-scale geothermal heating purposes;geothermal heating could be applied during winter along parts of the Yangtze River while geothermal cooling would be more suitable for summer there;geothermal cooling could also be applied to much of South China.Geothermal resources can also be applied to high value-added industries,to aid agricultural practices,and for tourism. 展开更多

Periodic patternmining has become a popular research subject in recent years;this approach involves the discoveryof frequently recurring patterns in a transaction sequence. However, previous algorithms for periodic patternmining have ignored the utility (profit, value) of patterns. Additionally, these algorithms only identify periodicpatterns in a single sequence. However, identifying patterns of high utility that are common to a set of sequencesis more valuable. In several fields, identifying high-utility periodic frequent patterns in multiple sequences isimportant. In this study, an efficient algorithm called MHUPFPS was proposed to identify such patterns. To addressexisting problems, three new measures are defined: the utility, high support, and high-utility period sequenceratios. Further, a new upper bound, upSeqRa, and two new pruning properties were proposed. MHUPFPS usesa newly defined HUPFPS-list structure to significantly accelerate the reduction of the search space and improvethe overall performance of the algorithm. Furthermore, the proposed algorithmis evaluated using several datasets.The experimental results indicate that the algorithm is accurate and effective in filtering several non-high-utilityperiodic frequent patterns. 展开更多

新街台格庙矿区位于毛乌素沙地东北缘,受气候变化与人类活动双重影响,区域水资源短缺、生态承载力已接近上限,而大量煤矿排水却难以有效利用。针对上述问题,分析了水资源均衡配置在半干旱区蓝绿空间融合的重要性,结合区域地质历史环境... 新街台格庙矿区位于毛乌素沙地东北缘,受气候变化与人类活动双重影响,区域水资源短缺、生态承载力已接近上限,而大量煤矿排水却难以有效利用。针对上述问题,分析了水资源均衡配置在半干旱区蓝绿空间融合的重要性,结合区域地质历史环境变迁、自然地理现状和矿区开发沉陷预期,提出了区域蓝绿空间构建的基本框架。同时,通过遥感影像分析、年鉴统计数据等综合研究,系统分析了区域沙地-湖泊-草原景观格局下的蓝绿空间要素及属性特征,并重点围绕矿井水量质特征及其综合利用潜力,提出了区域蓝绿空间供需平衡和生态系统稳定提升的蓝绿协同融合策略及空间格局分区优化建议,以期逐步实现自然资源保护利用、城乡人居环境质量提升和区域生态格局安全稳定,从而探索形成矿地融合发展的半干旱煤矿区生态综合治理和资源综合利用新模式。 展开更多

训诂学视角下正治反治辨证价值新解

正治反治作为中医学重要理论,其在临床实践中的指导价值尚存局限,注家对相关经文的注解亦存有争议。为此,本文基于训诂学方法,结合《黄帝内经》文本及相关文献,通过对经文结构框架及关键词的解析,重新诠释正治反治的核心概念,挖掘其辨... 正治反治作为中医学重要理论,其在临床实践中的指导价值尚存局限,注家对相关经文的注解亦存有争议。为此,本文基于训诂学方法,结合《黄帝内经》文本及相关文献,通过对经文结构框架及关键词的解析,重新诠释正治反治的核心概念,挖掘其辨证价值。以伤寒热病发热与痈肿二证为例,文章示范了如何在“因用”术语所蕴含的道家因循为用方法论指导下,依据主病在传变规律中“始同终异”的特征,具体应用正治反治“逆从病势盛衰变化”治则。研究深入指出,正治反治的核心在于通过对病传规律的掌握,提炼辨证要点并把握生机路径,进而辨势别异、循生机施治。以伤寒示例为起点,基于该辨证论治认知思维的逐步拓展,可能是《伤寒论》构建疾病辨治体系的切入点。这可为正治反治现代化发展提供新思路。 展开更多

1990—2018年四川盆地耕地利用稳定性时空分异及驱动机制分析

稳定利用耕地对于保持耕地的稳定生产、保障区域粮食安全具有重大意义。然而,当前鲜见针对粮食主产区的耕地利用稳定性研究。本研究利用高精度土地利用数据集评价了1990—2018年四川盆地耕地的利用稳定性时空分异特征;并基于人工神经网... 稳定利用耕地对于保持耕地的稳定生产、保障区域粮食安全具有重大意义。然而,当前鲜见针对粮食主产区的耕地利用稳定性研究。本研究利用高精度土地利用数据集评价了1990—2018年四川盆地耕地的利用稳定性时空分异特征;并基于人工神经网络模型,结合自然和人类活动因子,分析了四川盆地耕地利用稳定性的驱动机制。结果表明,四川盆地1990—2018年不稳定耕地面积为6.67×10^(4) km^(2),占总耕地面积的33.9%。不稳定耕地主要分布于川中丘陵区、山岭地带、城市边缘以及盆周山区。1990—2018年,四川盆地不稳定耕地总面积经历了两轮先降后升的波动变化,总体增长率为8.5%。其中,建设用地邻近度对耕地利用稳定性的影响最为显著(相对重要性达到了0.28),其次是水体邻近度和气候生产潜力(相对重要性分别为0.25和0.17)。这一现象可能由近30年四川盆地城市化进程加快激化的人地矛盾,以及农村人口迁移导致的青壮年劳动力流失所导致。地方政府应有效阻止城市房地产开发商做出“占优补劣”“占平补坡”等不当行为,并在农村通过开展土地整治工程等措施盘活耕地资源,同时结合气候敏感区资源禀赋合理规划耕地利用布局并制定耕作制度。 展开更多

背景目前,我国农村基层医疗机构住院患者多重慢病流行现状、住院服务利用及影响因素仍不明晰,不利于农村居民多重慢病的有效防治。目的本研究旨在调查广西壮族自治区乡镇卫生院住院患者多重慢病的共患模式、住院服务利用情况及影响因素... 背景目前,我国农村基层医疗机构住院患者多重慢病流行现状、住院服务利用及影响因素仍不明晰,不利于农村居民多重慢病的有效防治。目的本研究旨在调查广西壮族自治区乡镇卫生院住院患者多重慢病的共患模式、住院服务利用情况及影响因素,为深入了解农村基层医疗卫生机构住院诊疗所面临的多重慢病现况及改善相应的医疗卫生服务质量提供流行病学依据。方法采用多阶段分层抽样法与整群抽样法,随机抽取广西壮族自治区南宁市、梧州市、柳州市、玉林市和贵港市5市中10家乡镇卫生院,通过住院系统和慢性病管理系统收集以上10家乡镇卫生院2021年1月—2023年6月去识别化的所有多重慢病住院患者病历资料。采用Kruskal-Wallis H检验、Mann-Whitney U检验和多分类Logistic回归分析等统计学方法,探查广西壮族自治区乡镇卫生院多重慢病住院患者人口学特征、多重慢病的患病率、共患慢病数量、共患模式、住院次数以及影响共患慢病病种数和住院次数的因素。结果最终纳入9330例多重慢病住院患者,平均年龄为(68.1±11.8)岁,多重慢病患病率为31.24%(9330/29865)。共患慢病病种数以2种居多(53.22%),其次为3种(28.76%)、4种(12.92%)及≥5种(5.10%)。高血压是最常见的共患慢病,高血压、慢性颈腰椎病、慢性胃肠道疾病、脑卒中、糖尿病、高脂血症及慢性肺病的组合是患2种、3种、4种及≥5种慢病中最高发的共患模式;患2种、3种、4种及≥5种慢病的住院次数中位数波动于1~3次,患2种、3种及4种慢病中患病率前10位共患模式的住院次数比较,差异有统计学意义(P<0.05)。增龄、高BMI、女性、正在吸烟或戒烟、饮酒、医保类型、少数民族、婚姻状况与共患慢病数量增加相关(P<0.05);增龄、女性、少数民族、婚姻状况、低文化程度、正在吸烟或戒烟、饮酒、共患慢病数量、医保类型与住院次数增多相关(P<0.05)。结论广西壮族自治区乡镇卫生院住院患者多重慢病患病率较高;高血压是最常见的共患慢病,其与慢性颈腰椎病、慢性胃肠道疾病、脑卒中、糖尿病、高脂血症及慢性肺病的多元组合构成患病率最高的共患模式。共患慢病数量和住院服务利用的影响因素复杂多样,应采取多层面的防治策略和措施应对农村基层医疗卫生机构所面临的多重慢病的挑战。 展开更多
